The Egrikar Fe-Cu skarn type mineralization is located approximately 100 km NW of Gumuhane in NE Turkey. Geochemical analyses of diorite, which is thought to cause skarn, indicate medium K calc-alkaline (1.3 to similar to 2.3 wt % K2O), metaluminous to peraluminous (A/CNK = 0.76 to 1.13) and resembles a continental arc granite. The age of U-Pb zircon obtained from diorite is 42.3 +/- 1.0 Ma, which reflects the age of skarn metasomatism after intrusion into Late Cretaceous sandy carbonates. Skarn formation zones are recognized as endo- and exoskarn. The endoskarn zone mainly consists of pyroxene with hedenbergite (Di(4.31-28.14)Hd(59.29-80.82)Jo(5.0-35.55)) at prograde stage. Mn content of pyroxene increases a slight from endo to exoskam. Pyroxene is replaced by epidote and hornblende at the retrograde stage. The exoskam zone comprises garnet with andradite (Ad(91-99)Gr(01-07)) and grossular-andradite (Ad(44.95)Gr(05-54)), epidote and quartz as silicate minerals, and magnetite and hematite as ore minerals. Magnetite is the abundant ore mineral and is accompanied by chalcopyrite and minor pyrite at retrograde stage.

Giriş:Koroner arter ektazi (KAE)’den sorumlu olanetyolojik ve patojenik etmenler belirgin değildir. Bizler KAE’si olan veolmayan olguları resistin düzeyleri açısından kıyaslamayı, böylelikleresistinin KAE patogenezinde bir rolü olup olmadığını saptamayı amaçladık. Hastalarve Yöntem: Çalışma 81 olgudanoluşmaktaydı. Olguların 42’sinde (15’i kadın, ortalama yaş 60.4 ± 9.0 yıl), KAEmevcut olup, 39 olguda (22’si kadın, ortalama yaş 56.2 ± 10.7 yıl) ise normalkoroner arter anatomisi vardı. Normal komşu segmentlere kıyasla en az 1.5 katkoroner arter dilatasyonu mevcut olgular KAE olarak kabul edildiler. Koroneranjiyografinin gerçekleştirildiği gün alınan kan örneklerinde resistindüzeyleri ölçüldü. Bulgular:Her iki grubun referans özellikleri benzerdi. Serumresistin düzeyi KAE grubunda [ortalama 703.5 ± 828.2 ng/L, ortanca 379.5(40-492) ng/L] kontrol grubuna göre (ortalama 313.5 ± 252.6 ng/L, ortanca 256(30-1244) ng/L] anlamlı şekilde daha yüksek saptandı (p= 0.001).Sonuç:KAE veaterosklerozis ortak histopatolojik ve klinik özellikler arz etmektedir.Resistin, aterosklerozun gelişiminde ve klinik tablonun oluşumundaki yeri iyibilinen bir polipeptid olup, aynı zamanda KAE oluşumunda da rol alıyorolabilir. Resistinin KAE’nin oluşumunda ki yerinin daha iyi anlaşılabilmesiiçin büyük ölçekli çalışmalara ihtiyaç duyulmaktadır., Introduction:The etiological and pathogenic factors responsiblefor coronary artery ectasia (CAE) are unclear. Therefore, we aimed to comparesubjects with and without CAE with respect to resistin levels and determinewhether resistin plays a role in the aetiology or pathogenesis of CAE.Patientsand Methods: This study enrolled a total of 81 subjects,of whom 42 had CAE [15 female (F), mean age 60.4 ± 9.0 years] and 39 had anormal coronary anatomy (22 F, mean age 56.2 ± 10.7 years). Using coronaryartery diameters of the control group as reference, subjects having coronaryartery dilatation that was at least 1.5 times larger than the normal adjacentsegments were considered to have CAE. Resistin levels were measured from bloodsamples obtained on the day of the coronary angiography.Results: Boththe groups had similar baseline characteristics. Serum resistin levels weresignificantly higher in the CAE group [mean 703.5 ± 828.1 ng/L, median 379.5(40-4092) ng/L] than in the control group [mean 313.5 ± 252.6 ng/L, median 256(30-1244) ng/L] (p= 0.001).Conclusion:CAE and atherosclerosis share commonhistopathological and clinical characteristics. Resistin, a polypeptide with aknown role in the development and clinical presentation of atherosclerosis, mayalso mediate the formation of CAE. There is a need for future studies with alarger sample size to better delineate the effect of resistin on thedevelopment of CAE.&nbsp

With this change, one of the fundamental rights and freedoms, the European Human Rights Agreement contained in our constitution, gives anyone with a claim of being violated by government forces the right to appeal to the Constitutional Court. In this way, the Constitution has opened the door to international standards of freedom for implementing domestic law in our country.It recognizes the right for people to ensure their fundamental rights and freedoms through an individual method of application. Feodalite üzerinde çok farklı tanımlar ve tartışmalar yapılsa da feodalitenin anavatanı Ortaçağ Avrupa'sı olduğuna hiç şüphe yoktur. Ortaçağ Avrupası'nın tarih sahnesindeki yönetim birimleri içerisindeki en büyük sancıların ve olumsuzlukların yaşandığı feodalite sistemi, tüm bunlara rağmen Avrupa'nın yeniden doğuşuna ve yükselişine sebep olmuştur. Ortaçağ'dan Yeniçağ'a geçiş sürecinde feodal sistemden yenilikçi ve modern bir döneme nasıl geçildiğini ortaya koyması bakımından tezimiz dikkat çekmektedir. We report a case of hypertrophic cardiomyopathy due to systolic total narrowing of side branches of all major coronary arteries.

We report on a 29‐year‐old woman with severe hemolytic anemia following mitral valve annuloplasty. Although hemolysis due to mechanical prosthetic mitral valve is well recognized, hemolytic anemia associated with mitral valve repair is an uncommon condition. Reoperation may be considered if the patient has serious and persistent anemia. Although valve replacement is suggested to be a unique intervention, it may not be the solution every time because of mechanical effects. Various mechanisms of hemolysis related to mitral valve repair were suggested, but sufficient and precise data is not available. In this case, we tried to emphasize whether all hemolytic anemias that occur after mitral valve repair require surgical treatment. Copyright © 2010 Wiley Periodicals, Inc.

Aim: The etiology, diagnosis and treatment options of patients with moderate-large volumepericardial effusions were evaluated, retrospectively.Patients and methods: 104 patients with pericardial effusion were enrolled into the study (54male, 50 female, mean age of 53±18 years, ranged from 14 to 85 years). The clinical features,laboratory tests and trans-thoracic echocardiography findings of the patients were recorded.Both diagnostic and therapeutic aimed pericardiocentesis procedures were performed viasubxiphioid route and glucose, total protein, cholosterol, lactic dehydrogenase levels, adenosinedeaminase activity, polymerase chain reaction for tuberculosis, cytological, microbiologicalexaminations and cultures were obtained from pericardial fluid.Results: By transthoracic echocardiography, 70 patients (67.3%) had large volume and 34patients had moderate volume pericardial effusion. Pericardiocentesis was performed to a totalof 46 patients and pericardial fluid was exudate in 22 patients (47.8%) and transudate in 24patients (52.2%). The most common cause of effusion was idiopathic and the other causes wereneoplasia, congestive heart failure and tuberculosis, respectively.Conclusion: The main problem in pericardial effusion is to find the etiology for clinicians.Carefully taken medical story, detailed physical examination and pericardiocentesis procedureif needed will help us to diagnose the disease and to start the treatment for the specific etiology, Amaç: Orta-büyük miktarda perikardiyal efüzyonlu olguların; etyoloji, tanı ve tedavi yöntemlerigeriye dönük değerlendirildi.Hastalar ve Yöntem: Çalışmaya kliniğimizde perikardiyal efüzyonla takip edilen 104 hastadahil edildi (54’ü erkek, 50’si kadın; ortalama yaş 53±18 yıl; dağılım 14–85 yıl). Hastalarınklinik özellikleri, laboratuvar ve transtorasik ekokardiyografi bulguları kaydedildi.Perikardiyosentez işlemi tanısal ve/veya tedavi amaçlı olarak subksifoid yaklaşımla yapıldı veperikardiyal sıvıda glikoz, total protein, kolesterol ve laktik dehidrogenaz düzeyi, adenozindeaminaz aktivitesi ve tüberküloz (tbc) polimeraz zincir reaksiyonu (PCR) bakıldı, sitolojik vemikrobiyolojik inceleme yapılarak, kültür sonuçları değerlendirildi.Bulgular: Transtorasik ekokardiyografi ile 70 hastada (%67,3) büyük, 34 hastada (%32,7) ortaderecede perikardiyal sıvı saptandı. Perikardiyosentez uygulanan 46 hastanın 22’sinde (%47,8)perikardiyal sıvı eksuda, 24’ünde (%52,2) transuda özelliğindeydi. Etyolojik neden olarak ensık idyopatik perikardit bulunurken, diğer en sık etyolojik sebebler sırasıyla neoplazi, konjestifkalp yetmezliği ve tüberkülozdu.Sonuç: Perikardiyal efüzyonlarda etyolojinin saptanması klinisyenler için temel sorundur.Dikkatli alınan öykü, detaylı fizik muayene ve gerekli görülen hastalarda yapılanperikardiyosentez işlemi ile spesifik tanı konulmalı ve etyolojiye yönelik tedaviye hemenbaşlanmalıdır
