6. Posttransplant adenoviral enteropathy in patients with small bowel transplantation

Abstract

Transplant patients on immunosuppression represent a risk group for opportunistic infections, including adenoviral infections. The clinical and histopathologic findings of posttransplant adenoviral enteropathy in 3 adult intestinal transplant patients are described. The histopathologic pitfalls for the differential diagnosis between intestinal adenoviral enteropathy and acute rejection are discussed. Adenoviral enteropathy is an opportunistic infection that may follow aggressive treatment for small bowel allograft rejection, but which may require no specific treatment. It is associated with mild mixed inflammation in the lamina propria and slight increase of crypt apoptosis, resembling low-grade acute rejection. The identification of characteristic viral inclusions in the surface or crypt epithelium points to the diagnosis of adenoviral enteropathy, in spite of the increased crypt apoptosis. 8. Effect of Liver Transplantation on Inflammatory Bowel Disease in Patients With Primary Sclerosing Cholangitis

Abstract

This report investigates the influence of liver transplantation and concomitant immunosuppression on the course of progression of inflammatory bowel disease (IBD) and discusses statistical methodology appropriate for such settings. The data on 303 patients who underwent liver transplantation for primary sclerosing cholangitis (PSC) were analyzed using person-time analysis and Cox regression, with the duration of IBD as the time variable and transplantation as a segmented time-dependent covariate, to take into account both posttransplant and pretransplant history of IBD. The need for colectomy and appearance of colorectal cancer were taken as outcome measures. The only significant risk factor in the multivariate model for colectomy was transplantation itself, which increased the risk of colectomy due to intractable disease (Wald statistic; P = .001). None of the variables available for analysis were found to influence the risk of colon cancer significantly. Graphs showing the dependence of the instantaneous risk of cancer on the time from onset of IBD and its independence from the latter in the case of colectomy are presented. The use of a unique statistical methodology described for the first time in this setting led us to the somewhat surprising conclusion that transplantation and concomitant use of immunosuppression accelerate the progression of IBD. At the same time, transplantation does not affect the incidence of colorectal cancer. These results confirm the findings of some recent studies and can potentially shed new light on the disease pathogenesis.

Abstract

Distal splenorenal shunts, a surgical connection between the spleen and kidney veins, was performed on 125 patients with liver disease who had hemorrhages from diseased blood vessels in the esophagus (the connection between the throat and the stomach). All 125 patients had been followed without any being lost to follow-up evaluation. Liver disease was documented in 85 patients: 45 had parasitic liver disease (schistosomiasis, usually found in Asia), 17 had nonalcoholic fibrotic liver changes (cirrhosis) and 23 had a mixed degenerative change to the liver. Specific measurements of these groups were made and the death rate was explored statistically. The cumulative survival of all patients was 74.1 percent. Less than six percent of the patients had a recurrent hemorrhage. The data show that the shunt had almost a ninety-five percent likelihood of remaining open; the patients with parasitic liver disease had a better survival rate, with a low incidence of brain damage after the procedure compared with the other groups.
