The article presents a discussion about the article by Doctor of Economics, professor Andrei Evgenievich Busygin on the problem of the evolution of higher education in Russia since Soviet times, organized by the editorial board of the journal. In connection with the possible turn of the Russian higher education system from the Bologna process to the more familiar Soviet system, it is important to think over and evaluate the possible risks, pros and cons with a historical digression into this problem. Now the topic of the development of the higher education system is relevant and in demand, it is especially acute for the younger generation and representatives of higher education

Objective: Long-standing gastroesophageal reflux disease is the most common cause of a cicatricial stricture of the esophagus. The treatment of this pathology involves a wide range of methods including conservative and surgical options. Surgeons can encounter technical difficulties in case of concomitant neck and chest pathology.Clinical case: We report a case of a decompensated cicatricial stricture of the esophagus with concomitant paraesophageal hiatal hernia, refractory gastroesophageal reflux disease, and nontoxic multinodular goiter (166.9 cm3). Selecting the optimal management for such patients is often a challenge. Staged treatment significantly improves postoperative quality of life, but the increased length of hospital stay can negatively impact patient compliance.

The round table is devoted to the discussion on a wide range of problems related to new challenges and prospects for the development of modern society. These are the challenges of global instability, bio- and cybersecurity, pandemic problems, digitalisation, which all lead the world to large-scale crises: economic, environmental, anthropological, etc. In the conditions of global instability, there is a rise of connections and interactions that fix new social changes, some other integrity, and the crisis of the technogenic civilisation. In this case, modernity is perceived not as a completed project, but as a process of updating social reality, a person’s ideas about society. It includes the main aspects of human relations to the world, and opens up a great civilisational diversity, which only grows with time. The catalysts of change on the way to a new modernity are values that reflect changes in life meanings under the influence of social life. Constantly emerging new risks and challenges motivate the reset of values that are tested in the depths of culture and only after that become ideals for imitation and reproduction. Currently, there is another change of milestones. Modernity imposes its own requirements on the processes taking place in society, and largely determines the course of global evolution as the path of ascent of complex systems to higher levels of organisation and complexity.

Background. Acute mesenteric thrombosis (AMT) is a condition that develops with thrombosis in the vessels of the mesenteric pool, which causes hypoperfusion of the intestinal wall and leads to secondary inflammatory changes. Mortality in OMT is 80–85 %. Early diagnosis of OMT is difficult due to the lack of specific clinical signs and laboratory markers.Objective. To identify preoperative risk factors for adverse clinical outcomes in OMT.Design and Methods. A single-center retrospective observational cohort study included 47 patients diagnosed with OMT. The following patient data were analyzed: age, gender, body mass index (BMI), assessment of the patients’ condition according to the SOFA, APACHE II, EuroSCORE scales. Various laboratory parameters in the preoperative and postoperative periods were analyzed.Results. The incidence of oncological diseases and history of acute cerebrovascular accident (ACV) were significantly higher in the group of lethal outcomes of OMT than in the group of surviving patients. The correlation of higher serum levels of lactate, D-dimer, creatinine, leukocytosis and procalcitonin in the preoperative period with the development of an unfavorable clinical outcome was revealed.Conclusion. The main risk factors for the development of an unfavorable clinical outcome of mesenteric thrombosis in patients are the presence of malignant neoplasms, a history of stroke, a high level of mixed venous blood lactate, creatinine, D-dimers, procalcitonin, leukocytosis in the preoperative period.

Amiodarone is a drug used in the treatment of life-threatening arrhythmias, which can lead to the development of amiodarone-induced thyrotoxicosis. In most cases this pathology can be treated by conservative methods; surgical treatment is resorted to in cases of thyrotoxicosis refractory to medical treatment. This case report describes surgical treatment of a patient with amiodarone-induced thyrotoxicosis, progressive heart failure, neurological pathology, bilateral pneumonia, functioning tracheostomy, systemic infectious process, multiple organ dysfunction syndrome, who was treated in the intensive care unit. Due to the lack of response to therapy with antithyroid drugs (thiamazole, lithium preparations and pulse therapy with prednisolone) and a progressive deterioration of the condition in a short period of time, according to vital indicators, the patient underwent thyroidectomy. In the postoperative period, there was a decrease in the occurrence of chronic heart failure symptoms. Medical control of cardiac arrhythmias was achieved. Surgical stage proceeded without complications in the period of 30-days. The patient was discharged for outpatient rehabilitation treatment.

The concept of a multi-civilisational world, the philosophical foundations of which are set out in the new monograph of academician A. V. Smirnov «The logic of sense as a philosophy of mind» (2021). Historical experience and analysis of modern practice shows that a new multi-civilisational integrity will not work, as long as only the Western project is considered as the only possible perspective of modern civilisation, and its ideals and values are presented as a role model. At the same time, the implementation of the value orientations of the consumer society outside Europe generates new crises and contradictions, not fitting into the national and cultural characteristics of other countries. Therefore, the way out to the points of growth of a new civilisational integrity should be not found in the West at all, but should be explored in those cultures that have preserved the traditional soil under the influence of modernisation processes. The new civilisational world does not arise as a completed project; it grows out of the global dynamic chaos left over from the outgoing civilisation. The catalysts of civilisational changes on the way to a new integrity are values that carry changes in life meanings under the influence of social life. New risks and challenges faced by countries require timely and adequate changes. Recently, the world has faced global instability, adaptation to which has become a motive for the transition to a new model of civilisational development.

The article considers the role of culture in the process of transition to a new civilisation. The author notes that the stable foundation of the future civilisation is formed in culture, based on the expertise of historical experience, traditions, value preferences, and national characteristics of the peoples living in countries. In conditions of increasing global instability, when it is not clear how to cope with crises, and where are the roots of new challenges and risks, it is essential to identify scenarios that lead to catastrophic consequences in order not to fall into these traps, or at least to see and minimise them. The answer to the new challenges of the time will certainly be found. It has already started forming in the culture, absorbing centuries-old experience of development, traditions and dominant values. The main function of culture is precisely to preserve in memory only that confirms the ideal image of the people. History can constantly return to the same plot in search of acceptable answers for each generation. And here an important feature of cultural identity is considered language as a code and a mechanism for cognition of reality. The building of a new ideal of civilisational development is fixed through the disclosure of a complex developing value system. In such a situation, culture is increasingly associated with the environment of preservation and possible transmission of spiritual civilisational heritage. Now it is impossible to say what a new system of values will be in a new civilisation, it is just emerging.

Type 2 diabetes prevalence is increasing dramatically worldwide. Conservative therapy doesn’t bring stable effect and is often insufficient, not to mention the lack of prospects to cure the disease. Fortunately, accumulating evidence points towards the notion that a complete remission of type 2 diabetes is feasible following a choice of surgical interventions. The efficacy of bariatric surgery in particular for achieving glycemic control has highlighted surgery as a candidate curative intervention for type 2 diabetes. When compared to intensive medical therapy and lifestyle intervention, metabolic surgery has shown superiority in achieving reducing number of medications and metabolic factors improvement, which translates in long-term benefits on diabetes progression and complications. Understanding factors that predict diabetes remission can help to select patients who will benefit most from bariatric surgery and to choose the most effective type of operation. This literature review analyzes studies of the most significant clinical and biochemical predictors of remission of type 2 diabetes mellitus after bariatric interventions, as well as highlights well-known mathematical prediction models.

The article considers the age-old path of development of sociology at the Belarusian State University (BSU). It is argued that the institutionalization of sociology on the territory of Belarus began with the opening of the Belarusian State University and the creation of the Department of Sociology and Primitive Culture (S. Z. Katzenbogen), where students were given lectures on genetic sociology, issues of labor, law, cultural history, and sociology of family and marriage. After the identification of historical materialism with sociology, the teaching of sociological disciplines was excluded from the curricula of BSU. Sociology revived in the 1960s with the creation of the Problematic Research Laboratory of Sociological Studies at the BSU (PRLSS–BSU) (first head – Prof. I. N. Lushchitsky). In its depths, as well as in the sector of applied sociology at the Department of Philosophy of the Humanities Faculties (Prof. G. P. Davidyuk), personnel were trained and the necessary conditions were created for the opening of the Department of Sociology and the Department of Sociology in 1989 (the first head of the department was Prof. A. N. Elsukov). Professor G. P. Davidyuk is rightfully considered the founder of modern Belarusian sociology. At present, at BSU, centuries-old traditions in the field of sociological science and education are being developed at the Center for Sociological and Political Researches (headed by Prof. D. G. Rotman) and at the Department of Sociology of the Faculty of Philosophy and Social Sciences (headed by Prof. A. N. Danilov).

It is concluded that BSU is one of the leading universities in the post-Soviet space and it possesses everything necessary for its further development. Sociological science and scientific schools are actively developing at BSU. The university staff includes well-known specialists who provide the training of highly qualified personnel with competences to work in conditions of global instability and readiness to give an adequate respond to the new challenges of the time.

An overview of the clinical observation of a patient with the consequences of severe traumatic brain injury, with a tracheoesophageal fistula is presented. The article discusses a staged multidisciplinary approach to treatment.

The OBJECTIVE of the study was to analyze the experience of performing thyroidectomy (TE) in patients with amiodarone-induced thyrotoxicosis (AmIT) at our centre.METHODS AND MATERIALS. The study included 12 patients with AmIT who underwent TE. Medical records were analyzed to assess the features of the AmIT and indications for TE. We also studied the operation protocols and postoperative follow-up data. Intraoperative, early and long-term postoperative complications were recorded. The long-term TE results were evaluated by the dynamics of the left ventricular ejection fraction (LVEF) based on the echocardiography data.RESULTS. The main indications for TE included the resistance of thyrotoxicosis to medication and worsening of the cardiac pathology. No cases of thyrotoxicosis progression or thyrotoxic crisis were registered during the operation. The vocal cord paresis developed in one case, completely restored in a year. Blood loss was minimal in all cases. Other intraoperative complications were absent. Not a single death was registered in the early postoperative period. At this period, a short paroxysm of atrial fibrillation resolved on its own was registered in patient with arrhythmogenic right ventricular dysplasia. A patient with biventricular chronic heart failure of a high functional class died 39 days after the operation due to a massive pulmonary thromboembolism. The long-term results of TE were evaluated in eight patients. In four out of five patients with initially reduced LVEF, it increased. In three patients with initially normal LVEF, it did not change.CONCLUSION. Thyroidectomy is an effective and safe treatment in patients with AmIT, including those with the persistent thyrotoxicosis and severe cardiac pathology. The success is possible when the preparation of patients for the intervention is carried out by a team of specialists experienced in treating of such patients.

The article deals with the specificity of philosophical knowledge, metaphysics as its semantic core and the problems of transforming modern humanitarian university education, which was developed in the work of Vladimir Mironov. V. V. Mironov is a well-known Russian scientist, doctor of philosophy, professor, corresponding member of the Russian Academy of Sciences, dean of the faculty of philosophy of the Lomonosov Moscow State University. The author analyses current problems related to the role of philosophy in shaping the image of the future in modern culture, reforming university education, the consequences of introducing scientometric methods into the educational process, the readiness of higher education responding to the challenges of the times, global instability, commercialisation, digitalisation, etc. The scientific position of V. V. Mironov on topical problems of transforming modern society, its assessment and reflections on the prospects for teaching philosophy and the humanities.

The characteristics and principles of surgical treatment of giant presacral neurogenic tumors are analyzed on the example of authors’ own series of cases and based on the literature review. Three clinical cases of surgical treatment of patients with giant presacral neuromas are presented. Besides routine preoperative clinical examination, 3D-printed models of pelvic bones, tumors and blood vessels were made based on CT-angiography data. In two cases, giant L5 root neuromas with presacral extension were removed through retroperitoneal approach, and in one case, two-stage removal of the giant presacral neurofibroma through the median laparotomic and dorsal approaches was performed. Giant presacral neurogenic tumors are the rare pathology in the practice of spinal surgeons. Surgical treatment should be carried out by a multidisciplinary surgical team. It should be borne in mind that life-threatening complications are common in the early postoperative period. Application of customized 3D-printed models is an additional useful tool of perioperative planning.

The aim - to study was to assess the condition of the lungs and pleural cavities in HIV-infected patients with community-acquired pneumonia by ultrasound examination. Material and methods. During the period from May 2020 to February 2021, 7 HIV-infected patients with community-acquired pneumonia were observed, who underwent ultrasound of the lungs and pleural cavities. Results and discussion. Ultrasound of the lungs is the most affordable method of diagnosis in outpatient settings, at the pre-hospital stage to solve the issue of patient routing. Ultrasound is mandatory for quick triage of patients with suspected pneumonia in the emergency department. Given that ultrasound is not associated with radiation exposure to the patient, the examination of pregnant women, newborns and children with suspected pneumonia of any etiology should begin with ultrasound of the lungs, pleural cavities and mediastinum. Ultrasound of the lungs can be performed after pneumonia in order to monitor rehabilitation to assess the nature of changes in the chest cavity and determine the prognosis of the disease. In the article, the editorial Board of the journal refers to a significant event in the life of the region of recent times – the awarding of the honorary title of «City of Labor Valour» to Saratov Saratov. The reasons for making such a decision by the President of the Russian Federation are considered. It shows the contribution of Saratov as one of the largest military-industrial centers of the country in ensuring victory in the Great Patriotic War and the most important facts of the labor feat of its residents.

The article considers the features of the political field of strong and weak power, and social institutions and structures that influence the transformation process. The authors propose a new reading of the concept of political field, and its main characteristics are the status roles of subjects, configuration of space, nature of the relationship of opposing forces, and methods for preserving the priorities of the opposing subjects. The article identifies interaction problems between the government and the opposition, mechanisms of power retention, tendencies and prospects for the development of the political field of weak and strong power under crises, global instability and government mistakes that lead to dramatic changes in the public opinion. The authors note that clashes in the political field restructure the political space in a new way, which is due to both peculiarities of the historical development and the social-economic and political situation. Transformations of the post-Soviet world are not complete, and the transition of post-Soviet countries to the sustainable development is possible only by adaptation to new realities. The peculiarity of the political field is the constant confrontation of its subjects: the more unstable the political field, the higher the unpredictability of events and the role of the leader. In the contemporary state, there is always a danger of destabilization due to information wars and new forms of influence on the mass consciousness, values and behavior. The empirical basis of the article are monitoring sociological studies conducted from 1987 to identify the level of social tension and political stability, fullness of the political space, rankings of politicians, etc.

The article substantiates the theoretical foundations of the technology of forming a new progress ideal, reveals the role of culture and its potential in this process. The author justifies the existence and constant updating of the cultural code, its fundamental vital meanings and basic values, suggests the possibility of destructive changes when shifting the cultural status of basic values. The author considers of the article as the main conclusions, firstly, the mechanism of transition of society to a new state, where new types of sociality arise, fundamental life meanings and basic values are updated; secondly, the author shows that the formation of a new matrix of values and the renewal of life meanings is a condition for the transition to a new type of civilizational development, which is growing within modern society; thirdly, the revolution in the field of communication allows the transfer to the traditional cultural status of pseudocultural or extra-cultural values, thereby opening the way for the destruction of the foundations of culture.

The differential cross sections for the 12C(3He, t)12N reaction leading to formation of the 1+ (ground state), 2+ (0.96 MeV), 2− (1.19 MeV), and 1− (1.80 MeV) states of 12N are measured at E(3He) = 40 MeV. The analysis of the data is carried out within the modified diffraction model and distorted wave Born approximation. Increased rms radii have been obtained for the ground, 2− (1.19 MeV), and 1− (1.80 MeV) states. We revealed that 12B, 12N, and 12C in the isobaric analogue states with T = 1, and spin-parities 2− and 1− have increased radii and exhibit properties of neutron and proton halo states.

Aim of the study. To evaluate the risk factors for the occurrence of intramuscular hematomas in patients with severe coronavirus infection receiving anticoagulant therapy.Materials and methods. Intramuscular hematomas in five patients with severe COVID-19 disease are reported in the paper. The criteria for selecting patients for the study included respiratory distress requiring oxygen, radiographic signs of severe pneumonia, anticoagulant therapy using low molecular weight heparin (LMWH), and spontaneous intramuscular hematoma. Clinical manifestations, blood coagulation results, conservative and surgical management were analyzed.Results. Standard regimen anticoagulation therapy in patients with coronavirus infection requires vigilance because of a risk of development of hemorrhagic complications.Сonclusion. When assessing a patient with hematomas, an emphasis should be given to examination of patients and changes in hemoglobin and hematocrit levels. Best strategy of anticoagulant therapy for patients with coronavirus infection and high risk of VTE, as well as optimal laboratory monitoring during LMWH administration are yet to be explored.

Purpose . To study the frequency of general surgical diseases development snd their features in patients after heart transplantation (HTx). Methods . From January 2010 to December 2018 it was performed 112 HTx (mean age – 46.7±14.0 years old; 82 – male, 30 – female). During 30 days after HTx 9 patients died. After discharge all recipients (n=98) were included in dispensary observation list. We retrospectively analyzed patients (n=35) who underwent general surgery manipulations in more than 30 days after HTx. All surgical interventions have been done under the reduction of immunosuppression therapy. Results . During 9 years of post-heart transplant follow-up 45 surgical interventions were performed, 7% (n=3) of them due to infectious complications, 31% (n=14) – oncology and others (62%, n=28). Most of manipulations were planned (39 from 45, 87%), the following general surgery interven- tions prevailed: laparoscopic cholecystectomy (n=13) and those to remove inguinal and umbilical hernia (n=12). During the 1st year the frequency of diseases required surgical treatment was 26% (n=11), infectious causes took place in 5 patients, non-infectious – in 6. Subsequently the incidence of infectious complications decreased that could be associated with the minimization of immunosuppressive therapy. Oncology was more frequent long-term after HTx – more than 3 years: among them the development of colon polyps prevailed and all recipients underwent polypectomy. There was no impact of age, gender, causes of chronic heart failure, obesity, immunosuppressive regimen (including the induction) on the frequency of general surgery diseases development (p>0.05). Conclusion . Based on our experience, we proposed an algorithm of examination, the features of surgical tactics and preparation for it in heart transplanted recipients are described. The important role of post-heart transplant follow-up in the timely detection of diseases requiring general surgical care is given.

The article discusses versions of fighting for the political future of the world, the results of which are not yet predetermined. Development of the political process in the twenty-first century illustrates that the world is entering a stage of permanent crisis of liberal democracy. The democratic wave of the 1990s died out and led only to the growth of right-wing extremism, nihilism and nationalism all over the world. It happened not only in the governments that arose out of dictatorships at the end of the twentieth century, but also in some old democracies. Democracy has ceased to cultivate authoritative and strong leaders. Society cannot always be in transformation or trauma state. Sooner or later a new reality will be built. Social anomia is not a permanent process, as it would inevitably become part of normative system. The new authoritarianism is cultivated by the ineffectiveness of power, disbelief in its ability to curb the situation of chaos and discontent, and to bring life back to normal. Today, no country in the world can be a model for imitation. There is not any ideal that others would like to borrow. Building a new ideal of civilizational development is fixed through the disclosure of a complex ethical system in which value conflict acts as an inevitable component. Most of the post-Soviet countries failed to advance their societies to a more suitable level of economy, to reach the positions dictated by the modern information age, and to provide the population with new high standards of living. Convergent trends are becoming increasingly noticeable in the Republic of Belarus with authoritarian rule. Evolutionary nature is inherent in the Belarusian transformation. It is based on the mentality of the people and the social responsibility of government. The post-Soviet world is not in a hurry to part with acquired sovereignty and is not ready to share even a part of its rights with supranational structures.

The article presents the data of two research projects - European Values Study (EVS) and sociological monitoring of the information field of the Republic of Belarus conducted by the Centre for Sociological and Political Studies of the Belarusian State University. The authors make the following conclusions: the basic values of the Belarusians are quite similar to the values of other European countries; the values of the Belarussians have developed for centuries under the influence of the key events in the European history; the values of society change slowly and can be transformed only in the life cycle of at least two generations; changes in the basic values are impossible under any external or internal pressure. Any attempts to quickly change value preferences of the people under pressure usually lead to the accumulation of negative moods and finally to social upheavals. Contemporary technologies open wide opportunities for informational violence and manipulation of public consciousness. The informational impact of the traditional media and the Internet (including social media) became one of the main factors affecting the structure and content of individual value orientations. The media create the sustainable intellectual background that determines the individual perception and subsequent behavioral response to events in the country and the world. The changing everyday reality forms a new value matrix, and the media become the key technological player due to the new information-communication possibilities.

The work is devoted to the development of technology of computer-aided design of elastic elements of wind tunnel balances, used in the testing of scale models of prospective aircrafts in wind tunnels. Modern technologies of automated design are based on the application of methods of parametric and topological optimization of structures, as well as the use of finite element analysis of stress-strain state of structures. These computational methods are implemented in software packages for engineering analysis and design (CAD/CAE systems). The problem of optimization of elastic elements of multi-component load cells is formulated. The main aspects of application of parametric and topological optimization methods in relation to elastic element design are considered. The practical application of topological optimization methods is limited by the high complexity of the resulting geometry. There is a problem of interpretation of the problem solution results. At the same time, the elastic element geometry is determined primarily by technological constrains associated with used manufacturing technologies. It is shown that the introduction of technological constraints in the formulation of the topological optimization problem allows to obtain geometry that is suitable for practical application. An algorithm for solving the problem of optimization with technological constraints has been developed, based on the correlation analysis of two geometrical models, performed using mutual correlation functions, showing the degree of difference or similarity of these models. The algorithm is based on a combination of parametric and topological optimization methods, so it can be attributed to parametric-topological optimization methods. The proposed algorithm has been tested. It is shown that the results of algorithm using are adequate, can be easily interpreted and suitable for practical use.

We studied the dynamics of mouse-like rodent communities in the area of self-growing vegetation, which had undergone deforestation. The research is based on the results of continuous monitoring conducted from 1978 to 2019. Pitfall traps was the method of catching small mammals during the monitoring period. We used Simpson’s Diversity Index to quantify species diversity. The community similarity was evaluated by the percentage of species through Czekanowski-Sørensen Index. The studies were carried out near the “Azhendarovo” Biological Station (54°45ʹ N, 87°01ʹ E). The results of the studies showed that natural primeval communities of the taiga zone before deforestation were characterized by a multidominant structure. The dominant group included the Alexandromys oeconomus Pallas, 1776, and codominant species are represented by the genus Clethrionomys. A characteristic feature of the small mammals’ population of taiga forests is the preponderance of the Apodemus peninsulae (Thomas, 1907) over the Apodemus agrarius Pallas, 1771. On meadowlands, the genus Microtus voles prevailed. These were largely the Al. oeconomus, which accounted for 43% of all mouse-like rodents. After the deforestation, the structure changed. In the early stage of deforestation, the dominant species among rodents was the Al. oeconomus. The composition of dominant species in the recovering areas of cut-down taiga began to approach to the original state 40 years after the deforestation. Meadow communities followed the path of transformation, having no analogs in the initial period and were characterized by a significant amount of ruderal vegetation.

The differential cross sections of the $^{11}\mathrm{B}(^{3}\mathrm{He},d)^{12}\mathrm{C}$ reaction leading to formation of the ${0}^{+}$ ground state and the 15.11-MeV 1 ${}^{+}$, 16.57-MeV ${2}^{\ensuremath{-}}$, and 17.23-MeV ${1}^{\ensuremath{-}}$ excited states of $^{12}\mathrm{C}$ are measured at ${E}_{\text{lab}}=25$ MeV. The analysis of the data is carried out within the coupled-reaction-channels method for the direct proton transfer to the bound and unbound states. The rms radii of the last proton in all states studied are determined. A comparison of the rms radii of the $^{12}\mathrm{B}, ^{12}\mathrm{C}$, and $^{12}\mathrm{N}$ nuclei in the isobaric analog states (IASs) with isospin $T=1$ determined by different methods allows us to arrive at a conclusion that these nuclei in the ${1}^{\ensuremath{-}}$ excited states at ${E}_{x}=2.62, 17.23,$ and $1.80$ MeV, respectively, possess one-nucleon (neutron or proton) halo structure. The enlarged radii and a large probability of the last neutron to be outside of the range of the interaction potential are also found for the ${2}^{\ensuremath{-}}$ states of $^{12}\mathrm{B}, ^{12}\mathrm{C}$, and $^{12}\mathrm{N}$ at ${E}_{x}=1.67, 16.57,$ and $1.19$ MeV, respectively. These IASs also can be regarded as candidates for states with one-nucleon (neutron or proton) halo.
