One of the most important objectives of teachers, parents, school administrators, and students is to improve student scores on standardized tests, such as the State of Texas Assessment for Academic Readiness (STAAR) in eighth-grade science. This quasi-experimental study examined the science achievement scores between schools that used different textbooks and labs when delivering instruction. This study utilized a quantitative approach, using archival data and survey design. Analysis of covariance (ANCOVA) and multiple regression were used to analyze the data while controlling STAAR eighth-grade reading scores to reveal significant differences between classes. The sample and population for this study were predominantly eighth-grade Hispanic students in South Texas. Analysis of covariance showed that classes that used labs with high hand-on experiences with greater direct student participation received higher science scores on state assessments. Additionally, reading scores were significantly related to science scores. Multiple regression findings indicated that textbooks and labs were significant predictors of student achievement on the STAAR eighth-grade science class result in South Texas for Spring 2015. The findings of this study may serve as a catalyst for improving student achievement in science through changes in textbook adoption and doing labs in science. The result suggests the need to research further to investigate other contributing factors of student achievement.

Patients with cirrhosis have high systemic inflammation (TNFα, CRP, and IL-6) that is associated with poor outcomes. These biomarkers need continuous non-invasive monitoring, which is difficult with blood. We studied the AWARE sweat-sensor to measure these in passively expressed sweat in healthy people (N = 12) and cirrhosis (N = 32, 10 outpatients/22 inpatients) for 3 days. Blood CRP, TNFα, IL6, levels, and liver function and quality of life were measured. We found that CRP, TNFα, and IL6 were correlated in sweat and serum among both groups and were evaluated in inpatients versus outpatients/controls. IL6 is associated with lower transplant-free survival. Sweat monitoring nocturnal CRP/IL6 elevations in cirrhosis versus controls. Outpatients with cirrhosis had inflammation levels that elevated during the evening and peaked towards the early night periods. The levels start to fall much later at night and early morning. These data suggest that further investigation of continuous measurement of sweat biomarkers in cirrhosis is warranted. [ABSTRACT FROM AUTHOR]

Health science represents the fastest-growing industry in the United States, a trend not predicted to slow for the next decade. The high demand for health professionals has led to increased demand for anatomy and physiology (A&P) courses and a transition to hybrid-style labs to meet the necessary increased enrollment without increasing the need for lab space and compromising educational quality. The transition to hybrid-style labs has only accelerated and become more important with the COVID-19 pandemic. It has been relatively unclear, however, how the hybrid style has impacted student learning, satisfaction, and performance. Furthermore, there is a general lack of research investigating the advantages and disadvantages across hybrid, online, and face-to-face styles. This lack of research is particularly obvious within biological sciences, including A&P. Attendance, practical grades, and overall grades were compared between face-to-face, hybrid, and online A&P lab styles. A survey was administered to compare student perceptions and experiences between hybrid and online lab styles. The hybrid style had many of the same advantages as online and face-to-face styles yet very few disadvantages. Hybrid lab students reported better experiences and greater satisfaction, attended more labs, and outperformed online lab students. Face-to-face lab students, however, outperformed hybrid lab students. These findings can ultimately inform how to best implement, organize, and improve hybrid-style labs to maximize student learning, achievement, and enjoyment.

This perspective provides a historical account of the isolation and nomenclature of the cyanobacterial strains currently known as Synechococcus elongatus. The story focuses on an isolate from the San Francisco Bay area of California (Pasteur Culture Collection PCC 7942) that has, for decades, been the genetic model for this species, and its close relative isolated from Waller Creek in Texas (PCC 6301, also known as the University of Texas at Austin Culture Collection of Algae UTEX 625). Until recently, these strains have been the only representatives of the species. A new wild isolate, UTEX 3055, is distinctly different from the prior reference strains. S. elongatus strains have been widely used by labs around the world to discover fundamental cellular processes and to engineer cyanobacteria to generate useful products. Background: With access to and uptake of pre-exposure prophylaxis (PrEP), the United States can prevent new HIV infections. To end the HIV epidemic, health insurance plans must facilitate access to comprehensive preventive care benefits. Since plan benefit designs vary considerably by plan, it is difficult to systematically determine plans that facilitate and restrict preventive services for PrEP. Methods: We applied an unsupervised machine learning method to cluster 17,061 Qualified Health Plans offered to individuals. We examined the clusters to draw conclusions about the types of benefits insurance companies tend to group together in plans. Then we analyzed the geographic distribution of those clusters across the United States to assess geographic inequities in access to HIV preventive care. Results: Our method uncovered three cohesive clusters of plans. Plans in Cluster 1: the least restrictive cluster, facilitate access to preventive care using copays over coinsurance on almost all benefits; Cluster 2: the moderately restrictive cluster, plans cover HIV prevention benefits with copays but restrict access to general health benefits with coinsurance; and Cluster 3: the most restrictive cluster, plans cover almost all benefits using coinsurance. Overall, increased prior authorization requirements tend to accompany reductions in out-of-pocket costs. Examining the geographic plan distribution, states with at least one rating area where at least 75% of plans offered are in the most restrictive cluster included: Georgia, Illinois, Missouri, Oklahoma, Texas, Virginia, and Wyoming. Conclusions: Insurance plan design is complex. To address the ambitious call to end the HIV epidemic in this country, plans should also take into account both public health and health equity factors to create plan designs that ensure access to critical preventive services for people who need them most. Addressing the growing disparities in PrEP access along racial and ethnic lines should be a national priority, and federal and state insurance regulators as well as insurance plans themselves should be part of the conversation about how to ensure people who would benefit from PrEP can access it. Better state/federal regulation of plan design to ensure access is consistent, equitable, and based on clinical recommendations will reduce the variability across plan designs. Aged hydrocarbon wells, if proper care is not ensured, can crack, get corroded, and leak subsurface fluids. Permian Basin in Texas, home to thousands of such wells, has seen numerous blowouts and wastewater leaks. Our study employs surface deformation derived from satellite observations, and injection well records to investigate these events. The results reveal an over‐pressurized wastewater aquifer producing a surface uplift of 20 cm/yr, likely due to wastewater being injected tens of kilometers away. Focusing on a January 2022 blowout resulting in 3 cm subsidence in 2 weeks, our geophysical model suggests aquifer over‐pressurization as the cause. With an excess pressure of over 3 MPa in the aquifer, several more such blowouts are possible in the near future. This research highlights the urgent need to better understand the impact of subsurface fluid injection and calls for prompt action to mitigate the environmental effects of oil and gas production. Plain Language Summary: Wastewater, a byproduct during oil extraction, is generally injected back into the ground. Permian Basin has witnessed several incidents of leakage of wastewater in the last 3 years. Recently in January 2022, huge amounts of wastewater were expelled at a high pressure from an old well. We used satellite data and wastewater injection data to understand the cause of these events. We found that the wastewater injection happening nearly several kilometers away is responsible for these leakages. We also discovered a highly pressurized wastewater lake below the surface in this region using geophysical modeling. Due to high pressures, the land in this region rose by 40 cm in just 2 years. Meanwhile, a part of this region sunk by 3 cm because of the leakage in January 2022. Our findings raise concerns about the potential for more leakages in the near future if action is not taken. Background: Human papillomavirus (HPV) infections can cause cancers of the cervix, vagina, vulva, penis, anus, and oropharynx. The most recently approved HPV vaccine, Gardasil-9, protects against HPV infection and can prevent HPV-associated invasive cancers. However, Gardasil-9 is one of the most underused vaccines in the US today. Young adults are at risk for HPV infection, but many are not vaccinated. This study uses a randomized controlled trial (RCT) to test an innovative multilevel intervention to increase HPV vaccination rates among young adults. In this paper, we describe the research protocol. Methods: The study uses a two by three factorial design. A total of 1200 young adults in Texas, age 18–26 years, who have not been previously fully vaccinated against HPV will be randomly assigned to one of six conditions to receive: (1) standard CDC information about HPV vaccination (control); (2) video narratives about HPV vaccination; (3) written narratives about HPV vaccination; or (4–6) enhanced access to HPV vaccine combined with (4) standard CDC information, (5) video narratives, or (6) written narratives. The two primary outcomes are the rate of HPV vaccination initiation by 3-month follow-up and rate of HPV vaccination completion by 9-month follow-ups. We will determine the impact of the individual level intervention (i.e., persuasive narratives through video or written format), the systemic level intervention (i.e., enhanced access to HPV vaccines), and the combination of both levels, on HPV vaccination initiation and completion. We will also use purposive sampling to select participants to take part in semi-structured interviews/focus groups to better understand the mechanisms of the intervention. Discussion: Recruitment and data collection began in March 2022. We expect to complete data collection by March 2026. We expect that narratives, enhanced access, and the combination of both will improve HPV vaccination initiation and completion rates among young adults. If proven successful, these individual- and system-level interventions can be easily disseminated in regions with low HPV vaccination rates to improve HPV vaccination, and ultimately decrease HPV-related cancer burden. In recent years, there has been much focus on the use of wastewater‐based epidemiology (WBE) in urban centers, particularly for SARS‐CoV‐2 monitoring. However, less is known about the application of WBE in rural settings or in areas of limited resources. Most WBE programs in low‐resource communities have occurred outside the United States. To reap the benefits, WBE would need to be tailored to better reflect the socioeconomic challenges, technical barriers, communication limitations, and variable wastewater infrastructures associated with rural communities. The objective of this review is to evaluate the potential opportunities and challenges of deploying the current SARS‐CoV‐2 monitoring methodologies in small, rural communities, with a particular focus on rural Texas. For this, we conducted an inventory of rural communities in the state of Texas and their wastewater infrastructure. Based on specific rural examples, we evaluated the potential of current WBE methodologies used in urban settings to monitor for emerging biological agents of concern such as SARS‐CoV‐2. Our findings include an overview of rural wastewater capacity across rural Texas, a look at current WBE efforts to detect SARS‐CoV‐2, and recommendations for future implementation in two cities in rural counties, Kerrville and Valentine. WBE is a rapidly evolving public health tool with several notable advantages associated with cost, access, and adaptability. It is of particular use in resource‐limited communities that often exhibit healthcare disparities. This study presents the first overview of the feasibility of implementing WBE in the rural settings of Texas. This article will discuss the proactive approach taken by EHSO to implement a formal decommissioning process for the research labs of Emory University. The steps included developing a working relationship between research administrators, researchers and EHS personnel. Other important steps included providing lab personnel with uniform decommissioning guidelines and redistribution of useable laboratory reagents, equipment and supplies to other laboratories on campus, and increasing customer service to researchers by providing hazardous waste pickup by EHS. These steps have led to decrease in the amount of laboratory cleanouts performed by EHS and a decrease in the amount of unknown or obsolete chemicals and reagents found in vacated labs. These steps have also encouraged University sustainability through the redistribution of useable laboratory supplies and equipment. [ABSTRACT FROM AUTHOR]

Background: Schools are a key setting for supporting youth physical activity, given their broad reach and diverse student populations. Organizational readiness is a precursor to the successful implementation of school-based physical activity opportunities. The R = MC2 heuristic (Readiness = Motivation x Innovation-Specific Capacity x General Capacity) describes readiness as a function of an organization's motivation and capacity to implement an innovation and can be applied to better understand the implementation process. The purpose of this study was to explore the barriers to and facilitators of implementing school-based physical activity opportunities in the context of organizational readiness. Methods: We analyzed interview data from 15 elementary school staff (principals, assistant principals, physical education teachers, and classroom teachers) from a school district in Texas. We focused on factors related to adopting, implementing, and sustaining a variety of school-based physical activity opportunities. We used the Framework Method to guide the analysis and coded data using deductive (informed by the R = MC2 heuristic) and inductive approaches. Themes were generated using the frequency, depth, and richness of participant responses. Results: Four themes emerged from the data: (1) implementation is aided by the presence of internal and external relationships; (2) physical activity opportunities compete with other school priorities; (3) seeing the benefits of physical activity opportunities motivates school staff toward implementation; and (4) staff buy-in is critical to the implementation process. Themes 1–3 aligned with subcomponents of the R = MC2 heuristic (intra- and inter-organizational relationships, priority, and observability), whereas Theme 4 (staff buy-in) related to multiple subcomponents within the Motivation component but was ultimately viewed as a distinct construct. Conclusion: Our results highlight and explain how key readiness constructs impact the implementation of school-based physical activity opportunities. They also highlight the importance of obtaining staff buy-in when implementing in the school setting. This information is critical to developing readiness-building strategies that help schools improve their capacity to deliver physical activity opportunities effectively. Limb salvage is a difficult path for patients to travel as there is no guarantee of the outcome, often the major factor is perfusion. For patients who underwent transmetatarsal amputation (TMA), success rate is crucial as the next option is most likely a major amputation. We performed a 10 years (2010–2020) retrospective review of patients that underwent a TMA and had an angiogram or computed tomography angiography (CTA) perioperatively at the Dallas VA Medical Center. Failure after TMA was defined as a patient requiring a proximal amputation within 1 year. There were 125 TMAs performed between 2010 and 2020 at the institution. Forty‐four (35.2%) patients had an angiogram/CTA peri‐operative and met the inclusion criteria. Seventeen subjects (38.6%) had a higher level of amputation. Of the 17 failures, 2 (11.8%) patients had no patent vessel runoff to the foot, 9 (52.9%) had one vessel, 4 (23.5%) had two vessels, and 2 (11.8%) had three vessels runoff. One vessel runoff to the foot yielded a high rate of poor outcomes (56.3%) defined as a higher level of amputation. Background: Paranoia is a spectrum of fear-related experiences that spans diagnostic categories and is influenced by social and cognitive factors. The extent to which social media and other types of media use are associated with paranoia remains unclear. Objective: We aimed to examine associations between media use and paranoia at the within- and between-person levels. Methods: Participants were 409 individuals diagnosed with schizophrenia spectrum or bipolar disorder. Measures included sociodemographic and clinical characteristics at baseline, followed by ecological momentary assessments (EMAs) collected 3 times daily over 30 days. EMA evaluated paranoia and 5 types of media use: social media, television, music, reading or writing, and other internet or computer use. Generalized linear mixed models were used to examine paranoia as a function of each type of media use and vice versa at the within- and between-person levels. Results: Of the 409 participants, the following subgroups reported at least 1 instance of media use: 261 (63.8%) for using social media, 385 (94.1%) for watching TV, 292 (71.4%) for listening to music, 191 (46.7%) for reading or writing, and 280 (68.5%) for other internet or computer use. Gender, ethnoracial groups, educational attainment, and diagnosis of schizophrenia versus bipolar disorder were differentially associated with the likelihood of media use. There was a within-person association between social media use and paranoia: using social media was associated with a subsequent decrease of 5.5% (fold-change 0.945, 95% CI 0.904-0.987) in paranoia. The reverse association, from paranoia to subsequent changes in social media use, was not statistically significant. Other types of media use were not significantly associated with paranoia. Conclusions: This study shows that social media use was associated with a modest decrease in paranoia, perhaps reflecting the clinical benefits of social connection. However, structural disadvantage and individual factors may hamper the accessibility of media activities, and the mental health correlates of media use may further vary as a function of contents and contexts of use. Sweet sorghum is an attractive feedstock for the production of renewable chemicals and fuels due to the readily available fermentable sugars that can be extracted from the juice, and the additional stream of fermentable sugars that can be obtained from the cell wall polysaccharides in the bagasse. An important selection criterion for new sweet sorghum germplasm is resistance to anthracnose, a disease caused by the fungal pathogen Colletotrichum sublineolum. The identification of novel anthracnose-resistance sources present in sweet sorghum germplasm offers a fast track towards the development of new resistant sweet sorghum germplasm. We established a sweet sorghum diversity panel (SWDP) of 272 accessions from the USDA-ARS National Plant Germplasm (NPGS) collection that includes landraces from 22 countries and advanced breeding material, and that represents ~15% of the NPGS sweet sorghum collection. Genomic characterization of the SWDP identified 171,954 single nucleotide polymorphisms (SNPs) with an average of one SNP per 4,071 kb. Population structure analysis revealed that the SWDP could be stratified into four populations and one admixed group, and that this population structure could be aligned to sorghum's racial classification. Results from a two-year replicated trial of the SWDP for anthracnose resistance response in Texas, Georgia, Florida, and Puerto Rico showed 27 accessions to be resistant across locations, while 145 accessions showed variable resistance response against local pathotypes. A genome-wide association study identified 16 novel genomic regions associated with anthracnose resistance. Four resistance loci on chromosomes 3, 6, 8 and 9 were identified against pathotypes from Puerto Rico, and two resistance loci on chromosomes 3 and 8 against pathotypes from Texas. In Georgia and Florida, three resistance loci were detected on chromosomes 4, 5, 6 and four on chromosomes 4, 5 (two loci) and 7, respectively. One resistance locus on chromosome 2 was effective against pathotypes from Texas and Puerto Rico and a genomic region of 41.6 kb at the tip of chromosome 8 was associated with resistance response observed in Georgia, Texas, and Puerto Rico. Introduction: Immune checkpoint inhibitor (ICI) pneumonitis causes substantial morbidity and mortality. Estimates of real-world incidence and reported risk factors vary substantially. Methods: We conducted a retrospective review of 419 patients with advanced non-small cell lung cancer (NSCLC) who were treated with anti-PD-(L)1 with or without anti-CTLA-4 therapy. Clinical, imaging, and microbiological data were evaluated by multidisciplinary adjudication teams. The primary outcome of interest was grade ≥2 (CTCAEv5) pneumonitis. Clinicopathologic variables, tobacco use, cancer therapies, and preexisting lung disease were assessed for univariate effects using Cox proportional hazards models. We created multivariate Cox proportional hazards models to assess risk factors for pneumonitis and mortality. Pneumonitis, pneumonia, and progression were modeled as time-dependent variables in mortality models. Results: We evaluated 419 patients between 2013 and 2021. The cumulative incidence of pneumonitis was 9.5% (40/419). In a multivariate model, pneumonitis increased the risk for mortality (HR 1.6, 95% CI, 1.0-2.5), after adjustment for disease progression (HR 1.6, 95% CI, 1.4-1.8) and baseline shortness of breath (HR 1.5, 95% CI, 1.2-2.0). Incomplete resolution was more common with more severe pneumonitis. Interstitial lung disease was associated with higher risk for pneumonitis (HR 5.4, 95% CI, 1.1-26.6), particularly in never smokers (HR 26.9, 95% CI, 2.8-259.0). Conclusion: Pneumonitis occurred at a high rate and significantly increased mortality. Interstitial lung disease, particularly in never smokers, increased the risk for pneumonitis. This article examines the predisposing risk factors and incidence of pneumonitis in patients with non-small cell lung cancer undergoing immune checkpoint inhibitor therapy and explores the potential modifying effects of smoking history and preexisting lung disease. Since the start of the COVID-19 pandemic, wastewater surveillance has emerged as a powerful tool used by public health authorities to track SARS-CoV-2 infections in communities. In May 2020, the Houston Health Department began working with a coalition of municipal and academic partners to develop a wastewater monitoring and reporting system for the city of Houston, Texas. Data collected from the system are integrated with other COVID-19 surveillance data and communicated through different channels to local authorities and the general public. This information is used to shape policies and inform actions to mitigate and prevent the spread of COVID-19 at municipal, institutional, and individual levels. Based on the success of this monitoring and reporting system to drive public health protection efforts, the wastewater surveillance program is likely to become a standard part of the public health toolkit for responding to infectious diseases and, potentially, other disease-causing outbreaks. Background: Uninsured and underinsured patients face specialty care access disparities that prevent them from obtaining the care they need and negatively impact their health and well-being. We aimed to understand how making specialty care electronic consultations (eConsults) available at a multi-site Federally Qualified Health Center (FQHC) in central Texas affected uninsured patients' care-seeking experiences and impacted their ability to receive the needed care. Methods: We used concepts from Ecological Systems Theory to examine individual, interpersonal, organization-level, social, and health policy environment factors that impacted patients' access to specialty care and the use of eConsults. We conducted thematic analysis of semi-structured, qualitative interviews with patients about seeking specialty care while uninsured and with uninsured patients and FQHC PCPs about their experience using eConsults to obtain specialists' recommendations. Results: Patients and PCPs identified out-of-pocket cost, stigma, a paucity of local specialists willing to see uninsured patients, time and difficulty associated with travel and transportation to specialty visits, and health policy limitations as barriers to obtaining specialty care. Benefits of using eConsults for uninsured patients included minimizing/avoiding financial stress, expanding access to care, expanding scope of primary care, and expediting access to specialists. Concerns about the model included patients' limited understanding of eConsults, concern about cost, and worry whether eConsults could appropriately meet their specialty needs. Conclusions: Findings suggest that eConsults delivered in a primary care FQHC addressed uninsured patients' specialty care access concerns. They helped to address financial and geographic barriers, provided time and cost savings to patients, expanded FQHC PCPs' knowledge and care provision options, and allowed patients to receive more care in primary care. Many ixodid species on the South Texas coastal plain can transmit pathogens to their hosts. Substantial areas are dominated by gulf cordgrass, Spartina spartinae (Trin.) Merr. ex Hitch. The S. spartinae habitat was examined in terms of abiotic and biotic factors that likely impinge upon ixodids using the plant for questing. Natural enemies, temperature, humidity, and plant structure were investigated as possible mortality factors and improving ixodid survival. Spartina spartinae (soil and foliage) harbored few natural enemies of ixodids, and soil salinity was nonlethal. Conditions were cooler and more humid inside S. spartinae clumps than in the canopies; hence, questing ixodids can rehydrate inside the clump when threatened by desiccation. Leaves were mostly "V" shaped in cooler months and, during warm months, the tightly folded leaf slot predominated, into which larvae crawled. Immature ixodids were more abundant in the concave side of the "V"-shaped leaves than on the exposed convex side. Larvae sought refuge from potential lethal ambient air conditions by entering tight warm season leaf folds. The leaf "V" and fold offer opportunities for rehydration on the leaf without moving to the clump's base. In addition to five other species collected on the grass, a relatively heavy larval southern cattle fever tick, Rhipicephalus (Boophilus) microplus (Canestrini), population was detected, suggesting that abundances in the South Texas cattle fever tick eradication quarantine zone are increasing and might spread further into Texas. Presently, there are an estimated 6.9 million wild pigs (Sus scrofa) in the U.S., which cause over US$1 billion in damage to agriculture, environmental impacts, and control costs. However, estimates of damage have varied widely, creating a need for standardized monitoring and a method to accurately estimate the economic costs of direct wild pig damage to agriculture. The goal of our study was to integrate remotely sensed imagery from drones and crop harvest data to quantify wild pig damage in corn fields. We used drones with natural color (red, green, blue) cameras to monitor corn fields at different growth stages in an agricultural matrix in Delta County, Texas, USA, during 2019–2020. We flew 36 drone missions and classified wild pig damage in 18 orthomosaics by a combination of manually digitizing and deep‐learning algorithms. We compared estimates of damage from drone imagery to those derived from ground‐based transect surveys, to verify pig damage. Finally, we compared damaged areas of fields to maps of collected real‐time yields at harvest to estimate yield loss. All classified drone orthomosaics of pig damage had >80% overall accuracy for all growth stages. Ground transect surveys, which subsampled 2.6–4.1% of the field, were found to miss damage compared to the complete field coverage provided by drone imagery. Most damage occurred in latter growth stages, when corn ears were maturing, seed was most nutritious, and producers had already invested in the majority of annual crop inputs. Wild pigs damaged up to 9.2% of a single monitored field, which resulted in a mean loss of 3,416 kg of corn/ha and a direct cost to producers of US$17.18–48.24 per ha of damage. Drone imagery, when combined with spatially‐ explicit, harvest yield data, provides an accurate assessment of crop damage and yield loss due to wild pigs in the currency required for the cost‐benefit evaluation of management actions. The article underscores that the Galveston National Laboratory in Texas has passed the disaster test. It states that this laboratory is one of various BSL-4 facilities that have been erected since the terrorist and anthrax attack in 2001. The U. S. Government Accountability Office said that two of the five current BSL-4 facilities had security problems, which include poor guard facilities to lax camera systems.

Encephalopathy in lung transplant (LTx) patients has a wide differential due to the high risk of infection and malignancy in the setting of immunosuppression. A combination of history, imaging, labs, cultures, and pathology is often required to make a diagnosis. We describe a rare case of encephalopathy due to amoebic encephalitis diagnosed through brain biopsy. A 64-year-old man who was two years out from a left single LTx for pulmonary fibrosis presented with encephalopathy, seizures, and fever in the setting of long-term voriconazole treatment for known Aspergillus flavus mycetomas in the native right lung. He resided in south Texas and endorsed swimming in freshwater lakes. CSF studies showed a lymphocytic predominance and infectious studies including viral, fungal, toxoplasma, and AFB cultures as well as cytology were negative. A Brain MRI was obtained that showed enhancement within the bifrontal lobes (Figure 1A). It was initially suspected that his findings were due to disseminated aspergillus infection and he was started on amphotericin. Symptoms did not improve, and repeat imaging showed worsening of the lesions (Figure 1B and 1C). He then underwent a brain biopsy that showed amoebas with a thick GMS-positive cyst wall morphologically consistent with Acanthaoeba species. Amoebas are environmental protozoan parasites known to cause central nervous system infections in immunocompromised hosts. Often the lesions are found in the temporal and parietal lobes leading to a wide range of neurologic symptoms including encephalopathy and seizures. A brain biopsy is required to confirm the diagnosis. Due to the high mortality and low incidence of this disease there are no known effective treatments. This patient was treated with a multi-drug regimen including trimethoprim-sulfamethoxazole, miltefosine, and posaconazole. Nonetheless, he expired within a few weeks of the diagnosis being confirmed. Background: Diabetic Foot Infection (DFI) guidelines recommend empiric methicillin-resistant Staphylococcus aureus (MRSA)-targeted therapy in settings where there is high prevalence of MRSA infections or in cases of severe infection; however, they do not provide recommendations for de-escalation. This approach has the potential to increase unnecessary use of broad-spectrum antibiotics; therefore, additional strategies are needed to optimize appropriate antibiotic use. This study evaluates the effect of MRSA nasal PCR testing on MRSA-targeted antibiotic use and clinical outcomes in patients with DFI. Methods: This was a retrospective quasi-experimental study of patients admitted to South Texas Veterans Health Care System for DFI, with or without osteomyelitis (OM), who had an MRSA nasal PCR and culture data. Eligible patients were identified from the Corporate Data Warehouse and reviewed via electronic health record. Patients were allocated into two groups: PRE (5/1/2019-4/30/2020) and POST (12/1/2020-11/30/2021) protocol implementation for de-escalation or avoidance of MRSA-targeted antibiotics. The primary outcome was median (interquartile range [IQR]) hours of empiric inpatient MRSA-targeted antibiotic therapy. A Wilcoxon Rank Sum test was used to assess the difference between the groups for the primary outcome. Secondary outcomes included the proportion of patients needing MRSA coverage added back for MRSA after de-escalation, hospital readmission, length of hospital stay (LOS), patient mortality, and acute kidney injury. Results: A total of 151 patients were included (83 PRE; 68 POST). Most patients were male (98% PRE; 97% POST) with a median age of 64 (IQR, 56–72) years. Incidence of MRSA in DFI in the cohort was 14.7% overall (12% PRE and 17.6% POST). MRSA was detected via nasal PCR in 12% of patients 15.7% PRE and 7.4% POST). After protocol implementation, there was a significant decrease in empiric MRSA-targeted antibiotic therapy use, from a median of 72 (IQR, 27–120) hours in the PRE group, to 24 (IQR, 12–72) hours in the POST group (p < 0.01). No significant differences were found for other secondary outcomes. Conclusion: This study of patients presenting to a Veterans Affairs (VA) hospital with DFI identified a statistically significant decrease in median duration of MRSA-targeted antibiotic use post-protocol implementation. Aims: This study aimed to determine the association of stress and salivary cortisol levels in the adult Indian population with and without temporomandibular disorder (TMD) and to validate it with bite force. Settings and Design: The present study had an observational, case--control study design. Materials and Methods: This study sample comprised two groups of 25 cases and 25 controls between 18 and 45 years of age. Diagnostic criteria-TMD questionnaire Axis I was used to assess TMD classification, the TMD Disability Index and modified Perceived Stress Scale (PSS) questionnaires were filled, and salivary cortisol levels were measured using electrochemiluminescence immunoassay (ECLIA). Bite force analysis was performed using a portable load indicator. Statistical Analysis Used: To characterize and analyze the study variables, means, standard deviations, Mann--Whitney U-test, and logistic regression were employed (STATA 14.2 [Texas, USA]). Shapiro--Wilk test was used to test the normality of the data. P < 0.05 was considered statistically significant (95% power). Results: Female gender was proportionately higher in both the groups (P = 0.508), TMD Disability Index was significantly higher for cases (P < 0.001), TMD cases perceived higher stress levels (P = 0.011), there was no statistically significant difference in salivary cortisol level between cases and controls (P = 0.648), and the median bite force was lower for cases (P = 0.0007). Conclusions: This study concluded that the chance of developing TMD increased with age. An increase in the TMD Disability Index score and modified PSS scores; and a decrease in the bite force increased the likelihood of TMD. Modified PSS score was negatively correlated with salivary cortisol concentrations, indicating a two-way response to TMD symptoms. [ABSTRACT FROM AUTHOR]

Purpose: Carbapenem-resistant Enterobacterales (CRE) are subject to intense global monitoring in an attempt to maintain awareness of prevalent and emerging resistance mechanisms and to inform treatment and infection prevention strategies. CRE and extended-spectrum beta-lactamase (ESBL)-producing Enterobacterales are not usually examined collectively in regards to their shared pool of resistance determinants. Here, we genetically and phenotypically assess clinical isolates of CRE and extended-spectrum beta-lactamase (ESBL)-producing Enterobacterales in the growing region of Central Texas, where CRE are emergent and occurrence of non-carbapenemase-producing-CRE (non-CP-CRE) infections is increasing. Methods: CRE (n=16) and ESBL-producing Enterobacterales (n=116) isolates were acquired from a regional hospital in Central Texas between December 2018 and January 2020. Isolates were assessed genetically and phenotypically using antibiotic susceptibility testing, targeted PCR, and whole genome sequencing. Results: CRE infections are increasing in incidence in Central Texas, and Klebsiella pneumoniae is causing the majority of these infections. Moreover, K. pneumoniae sequence type (ST) 307 is commonly found among both non-CP-CRE and EBSL-producing strains. Isolates carry similar plasmids harboring the gene for the ESBL CTX-M-15 and belong to the global lineage, rather than the Texas lineage, of ST307. Antibiotic resistance profiles, sequence data, and clinical records suggest that porin mutations may promote the transition of ST307 isolates from ESBL-producing to non-CP-CRE. In addition to antibiotic resistance mechanisms, several CRE isolates harbor active colicinogenic plasmids, which might influence the competitiveness of these bacteria during patient colonization. Conclusion: K. pneumoniae of the global ST307 lineage is circulating in Central Texas and is responsible for both non-CP CRE and ESBL-producing Enterobacterales infections. The contemporary distribution of crenobiontic endemics in central Texas is enigmatic because only some springs are occupied by crenobionts despite other unoccupied springs having seemingly suitable habitats. In the absence of complete paleo-records, a cohesive and widely accepted explanation for this biogeography has eluded researchers for many years. We suggest that data on contemporary species with obligate coevolution, such as parasites with multiple obligate hosts in their life cycles, can help to fill intervening gaps in the paleo-record because the contemporary distribution of such a parasite indicates that its hosts cohabitated without interruption since the arrival of the parasite. To test this conjecture, we studied one such parasite endemic to a select few central Texas springs, Huffmanela huffmani. By studying the distribution of the intermediate host, geologic and paleo-climatic records, performing lab experiments with live animals, and examining archived museum specimens of the definitive hosts from the 1950s, we were able to test multiple predictions about how the distribution of H. huffmani became what it is today. Our results corroborate a narrative suggesting that several severe droughts since the Wisconsin glaciations are responsible for having sculpted the present-day distribution of central Texas crenobionts. (1) Background: This paper focuses on the development of a community assessment for telehealth using an interprofessional lens, which sits at the intersection of public health and urban planning using multistakeholder input. The paper analyzes the process of designing and implementing a telemedicine plan for the City of Brownsville and its surrounding metros. (2) Methods: We employed an interprofessional approach to CBPR which assumed all stakeholders as equal partners alongside the researchers to uncover the most relevant and useful knowledge to inform the development of telehealth community assessment. (3) Results: Key findings include that: physicians do not have the technology, financial means, or staff to provide a comprehensive system for telemedicine; and due to language and literacy barriers, many patients are not able to use a web-based system of telemedicine. We also found that all stakeholders believe that telehealth is a convenient tool that has the capacity to increase patient access and care. (4) Conclusions: Ultimately, the use of an interprofessional community-based participatory research (CBPR) design allowed our team to bring together local knowledge with that of trained experts to advance the research efforts. [This paper is part of the Focused Collection on Instructional labs: Improving traditions and new directions.] This paper presents a historical analysis of the development, implementation, research, and assessment of an evidence-based introductory algebra-based physics course. The course is laboratory based and taught with an inquiry-based pedagogy. The course has remained unique and has been taught consistently for over 20 years. It is taught predominantly to health science majors and is completely laboratory based without a lecture and without a required text. Students learn by working through a guided inquiry manual in groups, doing experiments to explore the nature of the world around them. They develop both quantitative and qualitative models based on their experiments. The focus is as much on critical thinking, modeling, and thinking like a scientist, as on physics content. I discuss the course throughout the years, presented in the context of the field of physics education research historically and the state of our department locally. The results of multiple forms of assessment are presented chronologically. The assessments were designed to answer research questions about the students' conceptual understanding and thinking skills, the effects of class size expansion, and questions about learning assistants' pedagogical content knowledge. One finding was that the conceptual understanding of students in the inquiry-based course was significantly higher than students taught traditionally but not significantly different from students taught by other research-informed pedagogies. However, we found that students taught in the inquiry-based format showed more evidence of the application of thinking skills as assessed by our thinking skills rubric in free-response exam problems. We have demonstrated that it is possible to expand a laboratory-based, inquiry-based physics course from sections of 24 students to sections of 60 students with the help of learning assistants and we have developed and validated a set of questions that can be used to measure learning assistants' pedagogical content knowledge or for learning assistant training purposes.

Background: People with HIV (PWH) commonly have elevated cholesterol and triglycerides levels that have been linked to medications. However, healthy behaviors including lifestyle changes can lower high cholesterol (CHOL) or high triglycerides (TG), thereby reducing individual risk for cardiovascular diseases. This study aimed to determine the prevalence and risk factors associated with high CHOL or TG among PWH in Texas. Methods: Cross-sectional data of 981 PWH from the 2015–2017 Texas and Houston Medical Monitoring Projects were examined. High CHOL or TG was identified by medical chart diagnosis, CHOL or TG medication use, or most recent fasting lab ≥ 200 mg/dl (total CHOL) or ≥ 150 mg/dl (TG). High CHOL or TG associations with sociodemographic and clinical characteristics were assessed using Rao-Scott chi-square tests. Prevalence of high CHOL or TG development was calculated using multivariable logistic regression model. Results: High CHOL or TG prevalence was 41% with participants being mostly male (73%), ≥ 40 years (68%), with overweight (31%) or obesity (28%), and virally suppressed (62%). Compared with PWH < 40 years of age, PWH in their 40s, 50s, and ≥ 60s were 57%, 64%, and 62% more likely to have high CHOL or TG, respectively. Participants with overweight and obesity were 41% and 30% more likely to have high CHOL or TG than those with normal weight (BMI: 18.5– < 25), respectively. Conclusion: Since high CHOL and TG are modifiable CVD risk factors, increased education and lifestyle modification interventions are warranted to prevent the development of high CHOL or TG among PWH. Fireworks are typically discharged as a mark of celebration and joy in many societies spanning various cultures. In the United States of America, 4th July is celebrated as the Independence Day when the nation overthrew the British colonial yoke in 1776. While this day instills a sense of patriotism in every American's heart, it is also a major PM2.5 air pollution concern. This study is first of its type in the Lower Rio Grande Valley (RGV) Region of South Texas, USA, that characterizes fine particulate matter pollution. Using a low-cost sensor (TSI BlueSky Air Quality Monitor), real-time PM2.5 measurements were assessed at eleven different locations in four different towns and cities of Lower RGV Region: Brownsville, Edinburg, Weslaco, and Port Isabel. Hourly PM2.5 concentrations from July 03-06, 2021 are presented in this research work. Intraurban PM2.5 spatial and temporal variations provide an insight on the general population's exposure burden during the festive period. Results indicate an increase in fine particulate matter pollution across the region, but the levels do not exceed the U.S. National Ambient Air Quality Standards (NAAQS).
