Abstract

Summary: With the development of analytical instruments, the academic system has become more complicated, producing new journals one after another. Therefore, it became much important to clarify what is original of "Medicines". As the name Medicines indicates, it includes science and practice of caring for a patient and managing the diagnosis, prognosis, prevention, treatment or palliation of their injury or disease. Therefore, Medicines differs from other journals with similar title in that it covers a wide range of fields, from traditional medicine to rapidly developing molecular-targeted drugs, focusing on their pharmacological effects, structure-activity relationships, metabolic pathways, gene expression, using cultured cells, animals, and clinical trials. Most parts of this Special Issue were written by our editorial board members, who described the research topics and how they are engaged in their field of research. All of these articles are their life-long story. We collected a total number of 22 original works by basic researchers and clinical doctors. We hope that readers can get a chance to know the current status of diverse fields of medicines, and what your most important research themes are after reading these articles.

Abstract

Summary: Scale up and implementation of new point-of-care (POC) diagnostics is a global health priority to enable the adoption of new evidence-based POC diagnostics and to replicate and extend the reach of POC diagnostics. Global private and public sector agencies have significantly increased their investment in the development of POC diagnostics to meet the unmet needs of patients in resource-limited settings, particularly disease burdened settings with limited access to laboratory infrastructure. However, previous research has demonstrated that the availability of health technologies in these settings does not always guarantee patient-centered outcomes. The applicability, effectiveness and sustainability of diagnostic technologies is affected by the involvement of all stakeholders during planning and implementation, which must be relevant to each specific context and sensitive to local culture. Factors such as infrastructure, resources, values and characteristics of participants can influence the implementation, scalability and sustainability of health interventions such as POC diagnostics. This book, "Implementation and Scale up of Point of Care (POC) Diagnostics in Resource-Limited Settings", presents literature reviews and primary research studies focusing on the implementation and scale up of POC diagnostics in resource-limited settings.

Summary: The imperative to increase healthcare access, efficiency and effectiveness is nearly impossible to meet without reliance on technology. Telemedicine tools now include video, e-mail, text messaging, apps, and other mobile health modalities, deployed synchronously, asynchronously and in hybrid combinations to offer assessment, consultation, direct treatment, and integrated care. Within telemedicine, mental health is particularly well suited to technology platforms due to several inherent factors: provider shortages that are often more acute than in other specialties; the relatively reduced need for laboratory tests, imaging studies and physical examinations; the stigma still attached to mental illness; and diagnosis-specific obstacles that can complicate in person visits (e.g., pronounced fear in social anxiety disorder). The need to increase access, efficiency and effectiveness, combined with the relative ease of translating mental health interventions to technology-mediated delivery, have led researchers to explore various platforms, including: computerized cognitive behavioral therapy; online psychotherapy and online psychopharmacology; mobile therapy; virtual/augmented reality exposure therapy; serious games; and artificial intelligence. Yet, despite often positive efficacy data and an ever more technology-reliant lifestyle, the reach of telemental health remains relatively limited in comparison with its potential. We aim to capture the current status of digital and telecommunications technologies in mental health and the field's future trends and directions. Recently published data-as well as newly introduced platforms-have put both the promise and challenges of telemental health in sharp focus, making this an opportune time to compile a collection of high-quality, evidence-based and diverse articles that touch on various aspects of this technology revolution. This Research Topic collects articles that examine digital mental health interventions across different platforms and modes of delivery. The scope is broad to reflect the richness of the field and the many questions it raises: standalone vs. supported models; comparisons with "traditional" modalities; psychotherapeutic vs. psychopharmacological interventions; efficacy; patient attrition; cost-effectiveness; ethical issues; and insurer and malpractice coverage.
