Abstract

RIVM rapport:Bedrijven krijgen van de overheid een vergunning voor de hoeveelheid chemische stoffen, waaronder Zeer Zorgwekkende Stoffen (ZZS(Zeer Zorgwekkende Stoffen)), die ze mogen uitstoten. De industrie stoot vaak mengsels van verschillende stoffen tegelijk uit naar water en lucht, maar de vergunningverlener beoordeelt meestal de risico’s per stof en niet van het hele mengsel. Mensen en het milieu kunnen aan zulke mengsels worden blootgesteld. De schadelijke effecten van een mengsel kunnen groter zijn dan de effecten van één stof (cumulatie). Hoe groot die kans is, hangt af van de samenstelling, de concentraties en de schadelijkheid van de stoffen. In een eerder onderzoek heeft het RIVM verkend welke mogelijkheden er zijn om bij de vergunningverlening meer rekening te houden met het effect van een mengsel. Die mogelijkheden zijn nu verder uitgewerkt. Het RIVM heeft onder andere methoden ontwikkeld om de effecten van stoffenmengsels voor mens en milieu te kunnen inschatten bij de vergunningverlening. Uitvoeringsinstanties, zoals omgevingsdiensten, kunnen deze methoden gebruiken. Het RIVM beveelt aan de voorstellen met uitvoeringsinstanties verder uit te werken, zodat ze in de praktijk goed zijn uit te voeren. Ook heeft het RIVM een methode ontwikkeld om in kaart te brengen waar in Nederland ZZS het meest voorkomen en/of effecten hebben op mens en milieu. Deze informatie kan bijvoorbeeld helpen bij het al dan niet toestaan van uitstoot in een gebied waar al veel schadelijke stoffen in het milieu zitten. Het RIVM benadrukt dat er nog onzekerheden zijn in deze methode en kaarten. Dat komt voor een groot deel door een gebrek aan gegevens over ZZS. Het RIVM werkt aan een database die meer gegevens over de uitstoot van ZZS door bedrijven op gaat leveren. Zo ontstaat naar verwachting een beter beeld van ZZS-mengsels in het milieu. In algemene zin benadrukt het RIVM om zo min mogelijk ZZS naar de leefomgeving uit te stoten. Dat verkleint de mogelijke cumulatieve effecten van ZZS voo, In the Netherlands, companies are required to obtain government permits that dictate the amount of chemicals, including Substances of Very High Concern (SVHCs; Dutch ZZS), they can emit. Typically, industrial companies emit mixtures of various substances simultaneously to water and air. However, permit issuers often assess risks on a persubstance basis rather than considering the entire mixture. People and the environment may be exposed to these mixtures. The risk that a mixture will have harmful effects (cumulation) could be higher than the probability of the constituent substances having harmful effects individually. The level of this risk depends on the mixture’s composition, the concentrations of the individual substances and the hazard posed by them. In a previous study, RIVM explored possibilities to better account for the effects of mixtures in the permitting process. These possibilities have now been developed further. Among other things, RIVM has devised methods to actually gauge the effects of substance mixtures on humans and the environment during the permit issuance process. Authorities, such as environmental services, can use these methods in their work. RIVM recommends working together with these authorities to refine the proposed methods for practical applicability. Furthermore, RIVM has created a method to map where SVHCs are most prevalent and/or where their effects on humans and the environment are most significant in the Netherlands. This information can aid decisions on permitting emissions in areas already burdened with significant environmental pollutants. RIVM emphasises that uncertainties still exist in these methods and maps, mainly due to a lack of data on SVHCs. RIVM is developing a database that is expected to provide more information on the emission of SVHCs by companies, which will lead to a better understanding of SVHC mixtures in the environment. Abstract

BackgroundNormothermic machine perfusion (NMP) protocols using blood-based solutions are commonly used in the assessment of kidneys before transplantation. This procedure is, nevertheless, limited by blood availability and warrants the search for alternatives. We compared a blood-based solution with a serum-like preservation solution (Aqix) enriched with colloids with and without red blood cells (RBCs).MethodsPorcine kidneys retrieved from an abattoir were subjected to 30 min of warm ischemia, followed by 3 h of hypothermic oxygenated machine perfusion at 4 °C. Subsequently, kidneys (n = 6 per group) were evaluated with NMP for 4 h with 5 different solutions: diluted blood, Aqix with BSA ± RBCs, or Aqix with dextran 40 ± RBCs.ResultsThroughout NMP, markers of renal function and tubular metabolism were favorable in groups with RBCs. The addition of RBCs resulted in 4- to 6-fold higher oxygen consumption rates. Controls had significantly higher ATP levels post-NMP, exhibited decreased production of oxidative stress markers, and had the highest creatinine clearance. In conclusion, this study shows that the addition of RBCs during NMP reduced renal injury, improved function, and was associated with increased renal metabolism.ConclusionsAlthough the RBC-BSA-supplemented Aqix solution was also able to support metabolism and renal function, a blood-based perfusion solution remains superior.

Abstract

Aim of the study: The coronavirus disease 2019 (COVID-19) pandemic significantly impacted cancer care. In this study, clinical patient characteristics related to COVID-19 outcomes and advanced care planning, in terms of non-oncological treatment restrictions (e.g. do-not-resuscitate codes), were studied in patients with cancer and COVID-19. Methods: The Dutch Oncology COVID-19 Consortium registry was launched in March 2020 in 45 hospitals in the Netherlands, primarily to identify risk factors of a severe COVID-19 outcome in patients with cancer. Here, an updated analysis of the registry was performed, and treatment restrictions (e.g. do-not-intubate codes) were studied in relation to COVID-19 outcomes in patients with cancer. Oncological treatment restrictions were not taken into account. Results: Between 27th March 2020 and 4th February 2021, 1360 patients with cancer and COVID-19 were registered. Follow-up data of 830 patients could be validated for this analysis. Overall, 230 of 830 (27.7%) patients died of COVID-19, and 60% of the remaining 600 patients with resolved COVID-19 were admitted to the hospital. Patients with haematological malignancies or lung cancer had a higher risk of a fatal outcome than other solid tumours. No correlation between anticancer therapies and the risk of a fatal COVID-19 outcome was found. In terms of end-of-life communication, 50% of all patients had restrictions regarding life-prolonging treatment (e.g. do-not-intubate codes). Most identified patients with treatment restrictions had risk factors associated with fatal COVID-19 outcome. Conclusion: There was no evidence of a negative impact of anticancer therapies on COVID-19 outcomes. Timely end-of-life communication as part of advanced care planning could save patients from prolonged suffering and decrease burden in intensive care units. Early discussion of treatment restrictions should therefore be part of routine oncological care, especially during the COVID-19 pandemic.

18. Investigation of the air quality around the landfill Sint Maarten 2019 : Measurements and results of the MOD field visit in January 2019

Abstract

At the beginning of 2019, the RIVM measured the air quality around the landfill at Philipsburg, Sint Maarten for two weeks. No or hardly any harmful substances were measured. During the measurement period there were no open fires at the landfill. As a result, the RIVM is unable to assess the potential health risks of substances released in the event of an open fire at the landfill. In order to do so, it is necessary to take measurements during an open fire. The local fire department could perform this task. RIVM can support the fire brigade with specialized equipment and knowledge. The measurements were taken by the Environmental Incident Service (MOD) of the National Institute for Public Health and the Environment (RIVM) between 24th January and 6th February 2019. The measurements were taken at a distance of 500 to 2500 metres from the landfill. The RIVM did not perform any measurements at the landfill itself. The locations were chosen to provide a good insight into the possible exposure of the local population. Measurements were taken to identify the following substances: particulate matter (PM10), inorganic gases, Volatile Organic Components (VOC), aldehydes, Polycyclic Aromatic Hydrocarbons (PAHs), dioxins and Polychlorinated Biphenyls (PCB). This is a broad "package" of substances that might be relevant in case of a fire. From the 206 samples taken, a representative selection of 90 samples was analyzed in special laboratories. In some cases, the concentrations of aluminum and possibly of chromium measured were found to exceed the standards that apply if people were to breathe these substances continuously throughout their lives. However, the health effects of these exceedances are negligible. For PAHs, some samples exceeded the standards that would apply if these substances were ingested daily during a lifetime. This results in an almost negligible health risk. The odour nuisance that people experience can cause health problems such as nausea and headache.

Abstract

Aim of the study: Patients with cancer might have an increased risk for severe outcome of coronavirus disease 2019 (COVID-19). To identify risk factors associated with a worse outcome of COVID-19, a nationwide registry was developed for patients with cancer and COVID-19. Methods: This observational cohort study has been designed as a quality of care registry and is executed by the Dutch Oncology COVID-19 Consortium (DOCC), a nationwide collaboration of oncology physicians in the Netherlands. A questionnaire has been developed to collect pseudonymised patient data on patients’ characteristics, cancer diagnosis and treatment. All patients with COVID-19 and a cancer diagnosis or treatment in the past 5 years are eligible. Results: Between March 27th and May 4th, 442 patients were registered. For this first analysis, 351 patients were included of whom 114 patients died. In multivariable analyses, age 65 years (p < 0.001), male gender (p Z 0.035), prior or other malignancy (p Z 0.045) and active diagnosis of haematological malignancy (p Z 0.046) or lung cancer (p Z 0.003) were independent risk factors for a fatal outcome of COVID-19. In a subgroup analysis of patients with active malignancy, the risk for a fatal outcome was mainly determined by tumour type (haematological malignancy or lung cancer) and age (65 years). Conclusion: The findings in this registry indicate that patients with a haematological malignancy or lung cancer have an increased risk of a worse outcome of COVID-19. During the ongoing COVID-19 pandemic, these vulnerable patients should avoid exposure to severe acute respiratory syndrome coronavirus 2, whereas treatment adjustments and prioritising vaccination, when available, should also be considered

Abstract

SUMMARY: Assessment of donor kidney quality is based on clinical scores or requires biopsies for histological assessment. Noninvasive strategies to identify and predict graft outcome at an early stage are, therefore, needed. We evaluated the perfusate of donation after brain death (DBD) kidneys during nonoxygenated hypothermic machine perfusion (HMP). In particular, we compared perfusate protein profiles of good outcome (GO) and suboptimal outcome (SO) 1‐year post‐transplantation. Samples taken 15 min after the start HMP (T1) and before the termination of HMP (T2) were analysed using quantitative liquid chromatography–tandem mass spectrometry (LC‐MS/MS). Hierarchical clustering of the 100 most abundant proteins showed discrimination between grafts with a GO and SO at T1. Elevated levels of proteins involved in classical complement cascades at both T1 and T2 and a reduced abundance of lipid metabolism at T1 and of cytoskeletal proteins at T2 in GO versus SO was observed. ATP‐citrate synthase and fatty acid‐binding protein 5 (T1) and immunoglobulin heavy variable 2‐26 and desmoplakin (T2) showed 91% and 86% predictive values, respectively, for transplant outcome. Taken together, DBD kidney HMP perfusate profiles can distinguish between outcome 1‐year post‐transplantation. Furthermore, it provides insights into mechanisms that could play a role in post‐transplant outcomes. [ABSTRACT FROM AUTHOR]

27. Investigation of the air quality around the landfill Sint Maarten 2019 : Measurements and results of the MOD field visit in January 2019

Abstract

RIVM rapport:At the beginning of 2019, the RIVM measured the air quality around the landfill at Philipsburg, Sint Maarten for two weeks. No or hardly any harmful substances were measured. During the measurement period there were no open fires at the landfill. As a result, the RIVM is unable to assess the potential health risks of substances released in the event of an open fire at the landfill. In order to do so, it is necessary to take measurements during an open fire. The local fire department could perform this task. RIVM can support the fire brigade with specialized equipment and knowledge. The measurements were taken by the Environmental Incident Service (MOD) of the National Institute for Public Health and the Environment (RIVM) between 24th January and 6th February 2019. The measurements were taken at a distance of 500 to 2500 metres from the landfill. The RIVM did not perform any measurements at the landfill itself. The locations were chosen to provide a good insight into the possible exposure of the local population. Measurements were taken to identify the following substances: particulate matter (PM10), inorganic gases, Volatile Organic Components (VOC), aldehydes, Polycyclic Aromatic Hydrocarbons (PAHs), dioxins and Polychlorinated Biphenyls (PCB). This is a broad "package" of substances that might be relevant in case of a fire. From the 206 samples taken, a representative selection of 90 samples was analyzed in special laboratories. In some cases, the concentrations of aluminum and possibly of chromium measured were found to exceed the standards that apply if people were to breathe these substances continuously throughout their lives. However, the health effects of these exceedances are negligible. For PAHs, some samples exceeded the standards that would apply if these substances were ingested daily during a lifetime. This results in an almost negligible health risk. The odour nuisance that people experience can cause health problems such as nausea and headache., Begin 2019 heeft het RIVM twee weken lang de luchtkwaliteit gemeten rond de stortplaats van Philipsburg, Sint Maarten. Er zijn niet of nauwelijks schadelijke stoffen gemeten. In de meetperiode waren er geen uitslaande branden op de vuilstort. Het RIVM kan dus niet beoordelen wat de mogelijke gezondheidsrisico's zijn van stoffen die vrijkomen bij uitslaande branden op de stortplaats. Om dat wel te kunnen doen, is het noodzakelijk om tijdens een brand te meten. Deze taak zou de lokale brandweer kunnen uitvoeren. Het RIVM kan de brandweer indien nodig ondersteunen met apparatuur en kennis. De metingen zijn tussen 24 januari en 6 februari uitgevoerd door de Milieu Ongevallen Dienst (MOD) van het RIVM. Op afstanden van 500 tot 2500 meter van de stortplaats zijn op diverse plekken metingen gedaan. Op de afvalberg zelf is niet gemeten. De meetlocaties zijn zo gekozen dat ze een goed inzicht geven in de mogelijke blootstelling voor de lokale bevolking. Er zijn metingen gedaan naar: fijn stof (PM10), anorganische gassen, Vluchtige Organische Componenten (VOC), aldehyden, Polycyclische Aromatische Koolwaterstoffen (PAK's), dioxinen en Polychloorbifenylen (PCB). Dit is een breed 'pakket' van stoffen waar bij een brand naar kan worden gekeken. Van de 206 genomen monsters is een representatieve selectie van 90 monsters geanalyseerd in speciale laboratoria. In enkele gevallen overschrijden de gemeten concentraties aluminium en chroom de normen die gelden als mensen deze stoffen continu, hun leven lang inademen. Het gezondheidseffect van deze overschrijdingen is echter verwaarloosbaar. Voor PAK's overschrijden enkele monsters de normen die gelden als deze stoffen gedurende een heel leven via de mond zouden worden ingenomen. Dit levert een vrijwel verwaarloosbaar gezondheidsrisico op. De geurhinder die mensen ervaren kan gezondheidsklachten veroorzaken, zoals misselijkheid en hoofdpijn.

Summary I was totally there!: Understanding engagement in entertainment-education narratives By Lonneke van Leeuwen Introduction Narratives have the power to influence their recipients’ health behaviors. With the entertainment-education (E-E) strategy, health organizations turn this narrative power to good account by employing narratives in their health promoting campaigns. E-E programs, mostly in the form of televised narratives, have been shown to effectively encourage a variety of health-related behaviors. Because of these positive results, the E-E strategy is considered a promising communication strategy to encourage healthy behaviors. One quality of E-E narratives that has been shown to be crucial for narrative impact is the ability of E-E narratives to engage target recipients. Engaged recipients may experience four dimensions of narrative engagement (NE): narrative understanding, attentional focus, emotional engagement, and narrative presence. Although evidence is growing that NE plays a role in E-E narratives’ impact (hereafter: E-E impact), little is known about how NE leads to E-E impact and about how NE emerges in recipients of narratives. Objective The objective of this dissertation is to provide a better understanding of NE in E-E narratives, by investigating how NE is associated with E-E narratives’ impact, and by investigating the processes that contribute to experiencing NE. Context The studies described in this dissertation are conducted within the context of NE in E-E narratives aiming to discourage alcohol (binge) drinking among adolescents and young adults. In 2008, the televised E-E drama series Roes (High in English) was broadcast on national television. Roes consists of 11 case stories (25 minutes each) portraying negative experiences and outcomes of adolescent protagonist(s) drinking alcohol and/or using other drugs. Research questions Three research questions are addressed in this dissertation: RQ1: Does Roes discourage alcohol (binge) drinking in E-E narrative recipients? RQ2a: Are NE dimensions associated with E-E impact on alcohol (binge) drinking? And, if so: RQ2b: Do negative and positive thoughts about alcohol (binge) drinking mediate associations between NE dimensions and E-E impact on alcohol (binge) drinking? RQ3: Which psychological processes experienced during narrative reception contribute to experiencing NE dimensions? Main findings This dissertation has shown that Roes discouraged alcohol (binge) drinking. Viewing multiple episodes of Roes positively predicted a decrease in alcoholic drinks consumed per occasion, an increase in the intention to decrease alcohol use, and an increase in perceived normative pressure. One year after exposure, the impact on the intention to decrease alcohol use was still present. One of the Roes episodes, Verliefd (In love in English), was further examined. After the participants had viewed this episode, their beliefs relating to the negative outcomes of alcohol binge drinking (BD) were more contra-BD as compared to their beliefs prior to viewing the episode. Attitudes towards BD and willingness to engage in BD also became more contra-BD. Then, the roles of the NE dimensions attentional focus, narrative understanding, emotional engagement, and narrative presence in E-E impact were investigated. It was shown that the NE dimensions attentional focus, emotional engagement, and narrative presence were positively associated with E-E impact. Attentional focus was associated both with stronger beliefs about the severity of the negative outcomes of BD and with a lowered intention to engage in BD. Emotional engagement and narrative presence were associated with stronger beliefs that BD leads to negative outcomes (negative outcome beliefs), and with stronger beliefs about being vulnerable to these negative outcomes (vulnerability beliefs). Surprisingly, the NE dimension narrative understanding was associated with increased willingness to engage in BD. No evidence was found that negative thoughts about BD mediated associations between NE dimensions and E-E impact. Relations between NE dimensions, positive thoughts about BD, and E-E impact could not be investigated: only one participant reported a positive thought about BD in response to Verliefd. Based on these findings we conclude that attentional focus, emotional engagement, and narrative presence are important for E-E impact, and that thoughts about BD do not play a role therein. Because NE dimensions were found to be associated with E-E impact, it was investigated which psychological processes contribute to NE dimensions. Negative thoughts about the perceptual persuasiveness of Verliefd negatively associated with emotional engagement and narrative presence. Furthermore, it was shown that enjoyment of Verliefd was a strong contributor to NE, mainly through contributing to attentional focus. Another strong contributor to NE was narrative realism, mainly through contributing to narrative understanding. Finally, personal relevance, character involvement, and perceived severity contributed mainly through emotional engagement. This dissertation shows that E-E narratives can be an effective health communication strategy to discourage alcohol (binge) drinking in young people. Furthermore, this dissertation provides health communication researchers and media psychologists with insights into the role of NE in E-E impact, and offers E-E developers practical recommendations about how to create engaging and impactful E-E narratives.
