In recent years, Romania's stomatology private practice sector has seen substantial growth, with many dentists fully committing to building and expanding their own practices, often funded by their personal income. This study aimed to explore how various postures affect the muscle groups of dentists (380), particularly focusing on identifying positions that may jeopardize their musculoskeletal health. A group of dentists effectively participated in this study (10), adhering to their regular work routines while wearing wearable sensors on their backs to monitor posture and activity. The data gathered from these sensors were analyzed using the RULA (rapid upper-limb assessment) and REBA (rapid entire-body assessment) tools. The findings indicated that the head and shoulder movements during dental procedures involved considerable and repetitive angular shifts, which could strain the neck and back muscles and heighten the risk of musculoskeletal problems. Additionally, the standing postures adopted by the dentists were associated with an increased risk of postural issues and greater overall fatigue. After achieving some milestones in TB control, the Government of India launched the National Tuberculosis Elimination Program (NTEP) to end TB by 2025. The role of private practitioners in the success of national health programs in India cannot be overemphasised, considering they cater to three times more population than the public sector. Aim: To understand the knowledge and perceived bottlenecks of private practitioners in achieving the goals of NTEP. Materials and Methods: A web-based cross-sectional survey was conducted by Community Medicine Department of Pramukhswami Medical College, Bhaikaka University, Karamsad, Anand, Gujarat, India, using a semi-structured questionnaire among randomly selected 110 doctors affiliated with the Indian Medical Association (IMA) from October 2020 to December 2020. The survey questionnaire was prepared by an experienced TB Chest expert and consensually validated. It covered all aspects of the Revised National TB Control Program (RNTCP) training modules. Descriptive statistics (Mean, SD, Frequency (%), etc.,) along with independent t-test and Analysis of Variance (ANOVA) were used to present the profile of the participants as well as associated factors with the knowledge score. A p-value of less than 0.05 was considered statistically significant. Results: Out of 110 invitations, 97 (88%) doctors responded. Most of the respondents were males 71 (73%), specialists (81, 83.5%), and had been practicing for more than 20 years (66, 68%). The mean (SD) score was 3.31±1.27 out of six for Multiple Choice Questions and 8.05±3.77 out of 15 for True/False questions. Only 37 (38%) participants scored above the passing benchmark of 60%, indicating a substantial knowledge gap. A higher Outpatient Department (OPD) strength (>50 patients per day) and attending a TB training program by the Government were associated with a higher total score (p=0.018). Conclusion: The knowledge of private practitioners about the diagnosis and management of Tuberculosis was found to be suboptimal. Participants also expressed a few systemic challenges like poor communication and the complexity of the system in the notification of Tuberculosis cases. Innovative training programs, proper communication, and supportive supervision will help in engaging the private practitioners to achieve TB elimination goals.

Introduction: While the provision of unapproved regenerative medicine has been problematic worldwide, few studies have examined the implementation status of regenerative medicine (RM) in the specific field. This study aimed to determine the current status of therapy and clinical research in the obstetrics and gynecology (OBGYN) in Japan under the Act on the Safety of Regenerative Medicine (RM Act). Methods: Detailed data were extracted from publicly available websites provided by the Ministry of Health, Labour, and Welfare. We extracted descriptive details, including risk classification of the RM Act, modality, target disease, locality, institution, and administration route. For therapy, the price for each modality was evaluated. Results: The total number of therapeutic provision plans in OBGYN (1.9% of RM in Japan) are classified as Class II (moderate) risk. Most were administered in clinics in urban areas for treating endometrial or ovarian infertility by locally administering platelet-rich plasma (PRP) or autologous mesenchymal stem cells (MSCs). OBJECTIVE To compare results from 2 previously published surveys aimed at determining how pyometra is treated in high-quality, high-volume spay-neuter clinics (HQHVSNCs) and private practices (PPs), with a specific focus on identifying opportunities for partnership between the clinic types to improve access to care in pyometra treatment. SAMPLE Responses from 123 PPs and 87 HQHVSNCs. METHODS A previously published survey of PPs conducted from June to July 2021 and a similar previously published survey of HQHVSNCs conducted from June to July 2020 were compared regarding demographics, pyometra treatment availability, estimated outcome, alternative outcome, disruption, cost, and protocol between HQHVSNCs and PPs. A comparison was also run of the surveys' responses regarding referral of a pyometra to an HQHVSNC by a PP. RESULTS Although PPs estimated that economic euthanasia is the most likely alternative outcome for 13% of patients unable to access care at their practice, 80% would be reluctant to refer to an HQHVSNC. The 3 primary reasons indicated by PPs with a reluctance to refer were a perceived lack of prompt appointment availability (20%), lack of aftercare (18%), and lack of adequate equipment (14%). In contrast, most HQHVSNCs estimated that they generally accept referrals the same day (50%) or the next day (30%) and 95% of HQHVSNCs reported having the resources needed to treat most pyometras. CLINICAL RELEVANCE There are several opportunities for partnership between PPs and HQHVSNCs in the treatment of pyometra. The most frequent concerns expressed by PPs regarding referral to HQHVSNCs were not shared by HQHVSNCs or could be ameliorated by referral to an HQHVSNC for surgery and transfer back to the PP for aftercare. Introduction. Community-based primary care physiotherapy has developed through private practice, fee-for-service model in Aotearoa New Zealand where independent businesses operate in competition. Aim. We aimed to explore how the private practice model of physiotherapy impacts patient care, physiotherapists, and professional behaviour. Methods. Six physiotherapists managing musculoskeletal conditions in a primary care private practice in Aotearoa New Zealand were recruited using maximum variation purposive sampling. In-depth individual face-toface semi-structured interviews were audio-recorded, transcribed verbatim, and analysed using Interpretive Description. Inductive data analysis synthesised and contextualised data, creating a thematic framework that developed across interviews. Results. All physiotherapy participants discussed concerns about culture and professionalism in private practice physiotherapy despite not being asked about these. Three themes were identified. 'Competitive business model and lack of collaboration' - participants thought that competition between practices resulted in a lack of trust, collegiality, and collaboration, and pressure on clinicians to maintain income. '(Un)professional behaviour' - participants thought that physiotherapists were defensive and averse to scrutiny, resulting in reluctance to admit when they needed help, or to undertake peer review or seek second opinions. 'Lack of support and mentoring' - the professional culture in private practice was perceived to reduce support and mentoring, with negative impacts that affected physiotherapists at all stages of career. Conclusion. This exploratory qualitative study suggests that competition dominates communication and collaboration in private practice physiotherapy and may have wider implications for professionalism and the quality of patient care. BACKGROUND AND OBJECTIVES: Extensive literature has shown that poor sleep quality, anxiety symptoms and catastrophizing contribute to worsening of temporomandibular disorder (TMD) conditions. However, most studies are conducted in public institutions. Therefore, the aim of this study was to evaluate the influence of sleep quality on pain characteristics and psychological factors in patients with symptomatic TMD treated in a private orofacial pain clinic. METHODS: This is a cross-sectional study. Forty-four adult patients were selected according to the Axis I of the diagnostic criteria for TMD (DC/TMD) and completed the questionnaires Pittsburgh Sleep Quality Index(PSQI); Pain Catastrophizing Scale (PSC) and Generalized Anxiety Disorder Scale (GAD-7). The sample was divided into poor sleep quality and good sleep quality based on PSQI scores. The Visual Analogue Scale (VAS) quantified pain intensity and the pain duration was recorded in months. RESULTS: 63.64% of the patients had muscle TMD (myofascial pain), 18.18% had joint TMD (arthralgia) and 18% had both disorders. 72.73% of all patients had pain for more than 6 months. Patients reporting poor sleep quality had more anxiety symptoms (p=0.009) and catastrophic thoughts related to pain (p=0.006); of these, 93.7% had pain for more than 6 months. Negative correlations were observed between age and pain intensity, and between age and anxiety symptoms. CONCLUSION: Poor sleep quality was significantly associated with anxiety, catastrophizing and pain duration in TMD patients treated in orofacial pain private practice.

Background The variable relationship between the orbicularis supercilii and frontalis muscles may cause side effects with botulin toxin; steadily frowning is the usual recommendation. Eliminating the need to frown until all injections are over might enhance efficacy and lessen the neurotoxicity risks related to muscle exhaustion. Objectives The goal of this study was to investigate whether the digital caliper–assessed distances between the medial and lateral corrugator injection sites and the midline differed significantly in patients persistently frowning from patients resting. Methods Seventy-six individuals with glabellar wrinkles treated with botulinum toxin in the authors' private practices for glabellar lines voluntarily contributed their retrospective data about the positions of injection sites. The investigators had freely chosen to inject the medial and lateral sections of the corrugator supercilii during persistent maximum frown or at rest. Results The mean differences in distances between the medial and lateral injection points and the estimated midline were clinically trivial (1.0 ± 1.28 and 1.5 ± 2.53 mm, respectively), although the distances between the relaxed and contracted medial and lateral injection sites exceeded 1.8 and 2.5 mm in 33.9% and 46.4% of patients, respectively. Distance differences, not significant before 40 years old, were statistically significant only for the lateral injections in older patients (on average, 1.9 ± 3.01 mm). Conclusions The average distances from the medial and lateral injection points and the midline appear minimally different in patients who frown until all injections are over and, conversely, patients who rest. Abstract Objectives To explore the perceptions that Colombians have about voluntary private health insurance plans (VPHI) in the health system to identify the tensions that exist between the public and private systems. Methods A qualitative case study approach with 46 semi structured interviews of patients, healthcare workers, healthcare administrators, decision-makers, and citizens. Interviews were recorded, transcribed, anonymized, digitally stored, and analyzed following grounded theory guidelines. Results We developed a paradigmatic matrix that explores how, in a context mediated by both the commodification of health and social stratification, perceptions about the failures in the public health system related to lack of timely care, extensive administrative procedures, and the search for privileged care led to positioning VPHI as a solution to these failures. The interviewees identified three consequences of using VPHI: first, the worsening of problems of timely access to care in the public system; second, higher costs for citizens translated into double payment for technologies and services to which they are entitled; third, the widening of inequity gaps in access to health services between people with similar needs but different payment capacities. Conclusions These findings can help decision makers to understand citizens´ perceptions about the implications that VPHI may have in worsening equity gaps in the Colombian health system. It also shows, how VPHI is perceived as a double payment for services covered within social security plans and suggests that the perceived lack of timely access to care in the public systems and the fear that citizens have for themselves or their family members when using suboptimal healthcare are important drivers to purchase these private insurances.

The career trajectory of medical professionals, particularly in specialized fields like gastroenterology, can significantly impact healthcare and research. This study aimed to analyze career choices among gastroenterology fellows in the US and investigate the factors influencing these choices. We utilized data from the American Medical Association on internal medicine subspecialty fellows. The study examined career plans of gastroenterology fellows and compared them with those of other subspecialties. A chi-square test was performed to assess differences in career choices and practice settings. Among gastroenterology fellows, 46% opted for private practice, 28% pursued further training, and 26% chose academia. Notably, gastroenterology fellows were more inclined toward private practice than their counterparts in other subspecialties (46.3% vs 38.4%) and were less likely to pursue academic careers (25.6% vs 30.7%). This study highlights a concerning trend among recent gastroenterology fellowship graduates favoring private practice over academic careers or additional training. To sustain and strengthen academic medicine in gastroenterology, interventions such as scholarships, mentorship programs, and loan repayment initiatives tailored to academic pursuits could play a crucial role. Aims: The aims of this register study were: 1. To study whether the type of ownership of the dental practice was correlated with the type of dental care provided, that is public versus private ownership and professional (dentist or dental hygienist) versus non-professional ownership. 2. To study the extent of follow-up of patients who have undergone two types of treatments. Material and method: Two types of dental care were defined in the two groups studied, periodontitis/peri-implantitis and comprehensive restorative/rehabilitation. All relevant treatment codes that fall under these definitions are noted when they are performed. Also, the follow-up of each treatment code is noted. Differences in dental and socioeconomic status over time and between regions were adjusted for. A drop-out analysis was performed. Results: Dental practices owned by dentists or dental hygienists schedule follow-up appointments for patients who have undergone comprehensive restorative or rehabilitation dentistry more often than practices with other types of ownership. Dental practices owned by dentists or dental hygienists follow up patients with periodontitis and peri-implantitis less frequently. Conclusion: Type of ownership of a dental business influences the extent to which periodontal, and comprehensive restorative or rehabilitation dentistry were followed up.

Refractory ischemic priapism is a difficult to treat clinical entity for which there are a host of shunt procedures identified, but no singular agreed upon technique for surgical therapy. Recent literature describes success using a penoscrotal decompression technique that uses a similar dissection of a penoscrotal penile prosthesis placement. We demonstrate that this technique is easily applicable in the private practice setting as it uses a familiar setup to most general urologists in our case report.

Objective: To compare the use of sealants in permanent molars between public and private dentists in Brazil. Material and Methods: This was a cross-sectional and analytical web survey study. The convenience sample consisted of dentists in Brazil who answered a pre-tested online form released via social media between July and October 2021. Descriptive analysis was performed using absolute and relative frequencies (%) and associations using the Chi-square test (p0.05). Conclusion: The percentage of use of sealants among dentists in Brazil is high, especially among professionals in the public service, and the most used materials were resinous sealants and glass ionomer cement.

Abstract Background No research has been conducted to assess whether antibiotic prophylaxis prescribing differs by dental setting. Therefore, the goal of this study was to compare the prescribing of antibiotic prophylaxis in Veterans Affairs (VA) and non-Veterans Affairs settings. Methods This was a retrospective study of veteran and non-veteran dental patients with cardiac conditions or prosthetic joints between 2015–2017. Multivariable log binomial regression analysis was conducted to compare concordant prescribing by setting with a sub-analysis for errors of dosing based on antibiotic duration (i.e., days prescribed). Results A total of 61,124 dental visits that received a prophylactic antibiotic were included. Most were male (61.0%), and 55 years of age or older (76.2%). Nearly a third (32.7%) received guideline concordant prophylaxis. VA dental settings had a lower prevalence of guideline concordant prescribing compared to non-VA settings in unadjusted results (unadjusted prevalence ratio [uPR] = 0.92, 95% CI: 0.90–0.95). After adjustment, prevalence of guideline concordant prescribing was higher in those with prosthetic joints in the VA setting (adjusted prevalence ratio [aPR] = 1.73, 95% CI: 1.59–1.88), with no difference identified in those without a prosthetic joint (aPR = 0.99, 95% CI: 0.96–1.01). Concordance of dosing was higher in VA compared to non-VA settings (aPR = 1.11, 95% CI: 1.07–1.15). Conclusions VA has a higher prevalence of guideline concordant prescribing among those with prosthetic joints and when assessing dosing errors. Though the presence of an integrated electronic health record (EHR) may be contributing to these differences, other system or prescriber-related factors may be responsible. Future studies should focus on to what extent the integrated EHR may be responsible for increased guideline concordant prescribing in the VA setting.

After pediatric residency, the author worked in a rural community where he was able to immediately practice skills acquired during training such as intubations, bag-mask ventilation, IV placement, ear irrigation, foreign body removal from eyes and ears, abscess incision and drainage, intraosseous placement for rapid hydration of a severely dehydrated infant, EKG, X-ray readings, and ear-irrigations to cite but a few examples. Furthermore, the writer acquired other high-valued procedural skills such as neonatal male circumcision, frenotomy, ligation of supernumerary digits, and manual separation of labial adhesions. The author feels that he could only have acquired and maintained these skills by working in a busy rural pediatric practice. When the writer later became a faculty member, he was able to effectively train medical students and pediatric residents to acquire these same skills. Even though there is a paucity of research information on procedural skills acquisition among general pediatric residents, the writer proposes that the recruitment of full-time general academic pediatricians with real-world experience may be potentially beneficial for the training of medical students and pediatric residents. What do private practitioners perceive to be the benefits, barriers, costs and risks of hosting physiotherapy students on clinical placement? What models of placement are used and what support would private practitioners like to enable them to continue hosting students? A national mixed-methods study comprising a survey and four focus groups. Forty-five private practitioners from six states and territories who host on average 208 students per year (approximately one-third of all physiotherapy private practice placements in Australia) completed the survey. Fourteen practitioners participated in focus groups. Participants reported that hosting placements helped to recruit graduates and assisted private practitioners in developing clinical and educator knowledge and skills. Cost (both time and financial) and difficulties securing a sufficient caseload for students were perceived barriers to hosting placements. Hosting placements was perceived to be low risk for clients due to supervised care, but there was potential for risk to business reputation and income when hosting a poorly performing student. Participants mostly described a graded exposure placement model whereby final-year students progressed from observation to shared care to providing care under supervision. Participants perceived that they could be assisted in hosting placements if they were to receive additional financial and personalised support from universities. Private practitioners perceived hosting students to be beneficial for the practice, the profession, staff and clients; however, they did report them to be costly and time-consuming. Children with developmental language disorder (DLD) experience communication difficulties and receive speech–language therapy (SLT) services in public schools and/or private clinics in Cyprus. This study aims to analyze the context and content of communication between parents/guardians (P/Gs) and speech-language therapists (SLTs) in private practice. The conducted cross-sectional study utilized nonprobability convenience sampling to recruit P/Gs of children with DLD. Participants completed a questionnaire regarding their sociodemographic characteristics, the context of communication, topics discussed, and collaboration level with the SLTs. A total of 189 P/Gs participated, of which 84.1% were married, 79.4% were mothers, and 52.4% held at least a college/university degree. Mothers primarily communicated with SLTs in the setting, discussing their children's performance, progress, behavior, difficulties, and homework. However, 48.7% felt well-informed, 47.1% fully understood their children's goals, 55.6% received thorough briefings from SLTs, 51.3% observed at least a portion of the therapy session, and 77.8% received at least an adequate amount of homework. Additionally, 73.6% reported the use of technology during SLT, and 74.5% expressed good/great satisfaction with their children's progress. Of the 31.1% reporting difficulties, 64.3% mentioned some difficulties related to homework completion and the children's behavior. Milutin Jovanović belongs to the generation of architects who significantly contributed to the urban and architectural development of Belgrade between the two world wars. However, since there has been no adequate valorization of his creativity, a complete list of his achievements and basic biographical data have remained completely unknown. By reconstructing Jovanović's works, chronologically systematized data about his movement in the architectural profession was obtained. He began his professional engagement as an assistant at the architectural department of the Belgrade Technical Faculty, and later devoted himself to private practice. During the entire interwar period, Jovanović designed dozens of residential buildings and one hotel, and was also dedicated to other design tasks. He was hired by wealthy representatives of the middle class - merchants, clerks, doctors, entrepreneurs and many others, for whom he designed predominantly representative high-rise residential buildings throughout the central city core. In accordance with the developments on the architectural scene of that time, respecting the requests of the clients, his achievements were in two dominant stylistic directions - academicism and modernism. The discovery of buildings designed in the spirit of modernism is particularly significant, because for a long time it was considered that Jovanović's main style was academicism. Given the lack of personal legacy and complete historiographical sources, the research was based on studying of the fund of the Technical Directorate of the City of Belgrade stored in the Historical Archive of Belgrade. Therefore, this work is the first monographic review of the life and work of the architect Milutin Jovanović.

Abstract Background Celiac disease (CD) is caused by an immune response to gluten and treatment is adherence to a gluten-free diet. Guidelines from studies in large academic settings recommend registered dietitian (RD) referrals at time of diagnosis and periodic testing for micronutrient deficiencies. There is limited data to guide follow-up parameters in a large, community-based practice. The purpose of this study was to evaluate guideline adherence in this setting. Methods This retrospective study conducted in 2019 assessed CD care based on follow-up rates, micronutrient testing, symptoms, and serology results in cohorts with and without RD referrals. Patients in this study were followed at Rockford Gastroenterology Associates (RGA): a large, private GI practice. Patients were included if they had a diagnosis of CD from 1/2014 through 12/2018, based on positive serology and/or duodenal biopsy. Patient data was collected by chart review and analyzed through Microsoft Excel. Fisher’s exact and Chi-square tests were used for the statistical analysis and were calculated through the Statistical Product and Service Solutions (SPSS) software. Results 320 patients were initially reviewed and a cohort of 126 patients met inclusion criteria. 69.8% had a RD referral. 65.9% had at least one lab test order for any of the 6 micronutrients. Of 63 patients tested for iron, 11 were iron deficient (8 with RD referral). Of 64 patients tested for vitamin D, 21 were deficient (17 with referral). 80.2% attended at least one follow-up appointment, but 34.9% had only one follow-up visit over a mean follow up duration of 5.82 months. 79 patients had follow-up data for symptoms or serology and were separated into 4 categories (with vs. without RD referral): (1) asymptomatic and negative serology (32% vs. 26%), (2) symptomatic and negative serology (28% vs. 16%), (3) asymptomatic and positive serology (27% vs. 32%), (4) symptomatic and positive serology (13% vs. 26%). Category 1 yielded a fisher exact test value of 2.62 (p = 0.466). Conclusions RD referral, micronutrient testing, and close follow-up are important parameters that affect outcomes in patients with CD. Rates for dietitian referral, some micronutrient testing and follow-up visits were higher than 50%, though results from this study were not statistically significant. Further standardization of follow-up testing and monitoring for CD will help minimize discrepancies between community-based and large, academic GI practices.

INTRODUCTION:To fill the void created by insufficient dental terminologies, a multi-institutional workgroup was formed among members of the Consortium for Oral Health Research and Informatics to develop the Dental Diagnostic System (DDS) in 2009. The adoption of dental diagnosis terminologies by providers must be accompanied by rigorous usability and validity assessments to ensure their effectiveness in practice. OBJECTIVES:The primary objective of this study was to describe the utilization and correct use of the DDS over a 4-y period. METHODS:Electronic health record data were amassed from 2013 to 2016 where diagnostic terms and Current Dental Terminology procedure code pairs were adjudicated by calibrated dentists. With the resultant data, we report on the 4-y utilization and validity of the DDS at 5 dental institutions. Utilization refers to the proportion of instances that diagnoses are documented in a structured format, and validity is defined as the frequency of valid pairs divided by the number of all treatment codes entered. RESULTS:Nearly 10 million procedures ( n = 9,946,975) were documented at the 5 participating institutions between 2013 and 2016. There was a 1.5-fold increase in the number of unique diagnoses documented during the 4-y period. The utilization and validity proportions of the DDS had statistically significant increases from 2013 to 2016 ( P < 0.0001). Academic dental sites were more likely to document diagnoses associated with orthodontic and restorative procedures, while the private dental site was equally likely to document diagnoses associated with all procedures. Overall, the private dental site had significantly higher utilization and validity proportions than the academic dental sites. CONCLUSION:The results demonstrate an improvement in utilization and validity of the DDS terminology over time. These findings also yield insight into the factors that influence the usability, adoption, and validity of dental terminologies, raising the need for more focused training of dental students. KNOWLEDGE TRANSFER STATEMENT:Ensuring that providers use standardized methods for documentation of diagnoses represents a challenge within dentistry. Background: Recent graduates are well-positioned to share their preparedness for practice experiences and how their undergraduate education and training have prepared them for transitioning to the practice environment. Aim: The study aimed to investigate the perceptions and experiences of optometrists who graduated from South African institutions between 2016 and 2020 concerning practicereadiness. Setting: The study was conducted among graduates working in South Africa's private sector. Methods: This cross-sectional qualitative study used non-probability purposive sampling. Invitations to participate were emailed to all optometrists who met the inclusion criteria. Qualitative data were collected using online semi-structured focus group interviews. Data were anonymised, transcribed, and thematically analysed using interpretive content analysis. Results: Fifteen optometrists participated in three focus group sessions between 14 December 2020 and 24 February 2021. Four major themes emerged, namely university training, post-graduation learning curve, independent practice, and being an optometrist. Overall, participants perceived their clinical education and training to be strong; however, certain areas of weakness were highlighted regarding preparedness for practice. Conclusion: Key areas for strengthening optometry training in South Africa include training optometry students using a flexible approach to the clinical examination routine and increasing clinical exposure opportunities during training. Strengthening courses such as practice management and optical dispensing, both essential for private practice, are also required. Contribution: The findings of this study contribute to the strengthening of optometry education and training in South Africa. Background and Objectives: A dental environment possesses a serious ergonomic health issue on the dental team members which in turn contributes to the development of work-related musculoskeletal disorders (WMSDs). The present research is aimed at evaluating the frequency of musculoskeletal disorders and their associated sociodemographic and work environment risk factors among dentists in the United Arab Emirates. Material and Methods: In this cross-sectional study, a pretested and validated questionnaire was sent via email as well as on different social media platforms to a total of 497 dentists. A total of 179 dentists completed the study survey, with a total response rate of 36%. A binary logistic regression model was conducted to identify significant risk factors associated with WMSDs. Results: Dentists in the United Arab Emirates experience a high prevalence of work-related musculoskeletal disorders (90.4%) which were associated with high levels of stress in the work environment. Furthermore, dentists in the private sector worked more clinical hours per day (p = 0.000) and had more financial stress (p = 0.007) as compared to those in the governmental sector. Gender (p = 0.007), age (p = 0.01), monthly income (p = 0.073), work experience (p = 0.037), number of patients treated per day (p = 0.049), and the use of an ergonomic dental chair (p = 0.005) were all factors associated with a greater number of affected body regions by WMSDs. Multivariate binary logistic regression for the number of regions affected by WMSDs revealed that not using an ergonomic dental chair (OR 2.70, 95% CI, 1.14–6.36) and high stress in the work environment (OR 1.31, 95% CI 1.02 to1.67) were associated with more body regions being affected by WMSDs. Conclusions: This study highlights the high prevalence rate of WMSDs among dentists in the UAE. Future research should be directed towards reducing stress in the work environment, increasing awareness regarding the importance of an ergonomic dental chair, and reducing gaps between private and governmental practices. Introduction: Malpractice is one of the most significant hazards to patient safety in healthcare and is considered the second factor associated with the overall quality of the healthcare system. Aims and Objectives: This study aimed to assess the prevalence of malpractice in dentistry and its possible causes in Riyadh, Saudi Arabia during 2009-2015. Methodology: A cross-sectional retrospective analysis of the dental complaints filed by the patients in the general directorate of forensic medicine centers in Riyadh was reviewed, and the data was collected. Descriptive statistics of frequency distribution and percentages were calculated using the collected data. The Chi-square test for categorical variables was performed with a significance level set at P < 0.05. Results: The Riyadh region reported 168 (14.5%) dental malpractice cases during 2009- 2015. The highest number of malpractice cases was reported in 2010 (18.45%), and the least number of cases was reported in 2011 (10.1%). Prosthodontic specialty had the highest number of lawsuits (32.4%), followed by orthodontics (20.2%) and endodontics (15.8%). The Chi-square test showed no association between the different specialties and types of malpractice (P = 0.881) and between compensation and the type of malpractice (P = 0.832). Among the 114 cases, financial compensation was the most common retribution. Conclusion: Dental malpractice prevalence was high in the private sector, followed by the government and military sectors, with the highest number of cases registered against the prosthodontic specialty. Dental malpractice is a serious concern that can be prevented by fair and honest policies and thorough patient education.

The questionnaire was divided into four sections that included participants’ demographic data, preferred criteria on dentists and dental office design, and table for the importance of each of the factors studied.Results: A sample size of 5317 participants completed the questionnaire with a mean age of 30.5 ± 11.6 years old, 57% were females. Most of the participants had no preference on their dentist’s nationality (63%) or gender (44%). There was a preference for dentists who are consultants (38%), well known (83%), and has an explanatory-style consultation (78%). Participants preferred scrub and lab coat attires for their male (41%) and female (38%) dentists. Less than half of the participants (44.8%) preferred the dentist to have a social media account. Of all social media platforms, Twitter was the most preferred one (37%). White clinic color was more preferable than other colors (56%). Of all the factors studied, dentist’s reputation, clinical rank and cost of treatment were considered very important for most of the participants when selecting their dentist.Conclusion: Overall, Saudi participants have specific preferences on their dentists and dental office design. Background: Although the orofacial-function improvement exercise (OFIE; oral exercise) was first introduced in Korea 10 years prior, it is still not covered by medical insurance, and no detailed survey on the dissemination of related programs has been conducted. Therefore, this study investigated the actual status of the education and practice of OFIE among the elderly and at elderly welfare institutions in the Seoul and Gyeonggi Provinces.Methods: Senior citizens aged more than 65 years old, public health centers (total of 69) and elderly welfare institutions (including nursing homes and elderly welfare centers, total of 56) per administrative area in the Seoul and Gyeonggi Provinces were targeted. We analyzed 200 elderly people and 93 institutions who agreed to participate in the survey. For the elderly, general characteristics, experience and route, current practice, and necessity regarding OFIE were investigated. For institutions, the history and plan of education programs on OFIE were investigated. Results: Regardless of the general characteristics, both the rate of experience and practice for OFIE were low overall; moreover, although they felt it was necessary, they had insufficient motivation for its implementation. Moreover, only a few institutions which were operating the education about OFIE regardless of the COVID-19 situation.Conclusion: Although OFIE is necessary for the elderly, its distribution remains insufficient. Therefore, further efforts are needed to expand the education and raise the awareness of oral exercise among elderly individuals and senior welfare institutions.

Plain English summary A question frequently raised in the eating disorders field is whether treatments that were developed and tested in research environments can achieve the same results in real-world clinical settings, where patients’ diagnoses are presumed to be more complex, clinicians less specialized, and multi-professional care teams less coordinated. The purpose of this article is to outline a model for implementing evidence-driven, outpatient treatments for eating disorders in non-academic clinical settings, specifically private practices and specialty programs. We describe the philosophy, infrastructure, training processes, personnel, and procedures utilized to optimize care delivery and to create accountability for both scientifically-adherent practice and positive patient outcomes. We also outline ways to be producers—not just consumers—of research in the private sector, and to train the next generation of scientifically-informed eating disorder specialists, all with the goal to bridge the research-practice divide.

A new patient satisfaction measure called the Radiation Oncology Patient Satisfaction (ROPS) questionnaire is used in this study to measure satisfaction data for radiation oncology in private practice. Limitations of existing literature on patient satisfaction demonstrated a need to develop a questionnaire that was more tailored to analyze patient satisfaction among those utilizing private oncology centers within the U.S. healthcare system. This need was met by this study's development of the ROPS questionnaire, which was a variation of 2 existing constructs that are presented in the text. The questionnaire was fielded among patients at a private cancer radiation treatment clinic in Florida (n = 950). Data analysis tested the psychometric properties of our revised construct and its predictive validity for 2 patient satisfaction outcomes: (1) likelihood of recommending this treatment center to family members/close friends and (2) overall satisfaction with treatment. Each of the ROPS variables were found to make a legitimate contribution to evaluating patients' overall satisfaction with radiation treatment. Findings indicated organizational setting was of vital importance when conducting patient- centered research on satisfaction. Treatment factors in ROPS can be adjusted to measure satisfaction with chemotherapy or other modalities in addition to radiation treatment. Authors recommend radiation oncology clinics regularly monitor patient satisfaction, especially if/when they experience organizational changes, such as when a new physician joins the practice or if/when the national or local landscape undergoes significant shifts in norms and expectations like we have seen happen with COVID-19. Background: Despite its potential to improve patient access, streamline administration, and enhance healthcare efficiency, systematic data on online appointment scheduling (OAS) in medical practices is lacking., Method: Prospective, single center study conducted at an ophthalmology practice. Over a period of 12 weeks, all booked appointments, both online made via the OAS system, and offline made by practice staff via phone, email, or in-person, were recorded across four doctor consultations. Patient demographics, booking type (new appointment/rescheduled/cancelled), date and time of the booking, and the next available appointment slot were documented., Results: 1080 interactions (new booking 76.8%, rescheduling 13.1%, cancellation 10.1%) were made by 880 patients via OAS and 2427 appointments offline by 1902 patients. Patients booking offline were older than those booking online (mean 50.3 ± 28.1 years vs. 41.7 ± 22.2, p < 0.0001). No difference between the groups was found regarding sex distribution (both, female around 59%, male around 41%, p = 0.81). New patients showed a preference for booking online (p < 0.0001). 63.1% of online bookings occurred on weekdays during practice hours. 36.8% of cancellations/reschedulings were made one to two days before the appointment. These vacant slots were promptly filled again, as OAS was primarily used for near-term bookings (booking time to appointment ≤7 days in 47.7%; selection of the earliest available appointment in 44.1%). Hypertension poses a critical threat to health in India, being the predominant risk factor for mortality and disability. With over 70% of outpatient care being provided by the private sector, our study investigated the practices and incentives of private health providers in screening, diagnosing, treating, monitoring, and counseling hypertension patients in rural and peri-urban India. Conducted from April 2020 to February 2021, the qualitative study involved 46 participants, including various healthcare professionals and patients in three Telangana state districts. Analysis revealed a lack of recognition of hypertension's gravity among private providers in these areas, leading to inconsistent screening practices and varied diagnostic thresholds, particularly among rural medical practitioners. Both formal and informal providers lacked standardized protocols and follow-up mechanisms, with limited technical knowledge about hypertension observed, especially among rural practitioners who were often the first point of contact. Drug effectiveness, and incentives from pharmaceutical sales representatives influenced prescribing practices. Diagnostic labs also offered financial incentives for patient referrals. Thus, aligning providers' objectives with patient preferences and public health goals is crucial. To encourage evidence-based hypertension care, the government and NGOs could implement strategies such as tailored incentives, financial rewards, tax benefits, accreditation, and recognition for private healthcare providers. Professional bodies in the private sector should establish programs emphasizing quality assurance and certifications. Future research should focus on designing and testing new models for private sector hypertension service delivery, coupled with targeted interventions to enhance care in rural and peri-urban settings., (© 2024. Cost and insurance coverage remain important barriers to mental health care, including psychotherapy and mental health counseling services ("psychotherapy"). While data are scant, psychotherapy services are often delivered in private practice settings, where providers frequently do not take insurance and instead rely on direct pay. In this cross-sectional analysis, we use a large national online directory of 175 083 psychotherapy providers to describe characteristics of private practice psychotherapy providers who accept and do not accept insurance, and assess self-reported private pay rates. Overall, about one-third of private practice psychotherapists did not accept insurance, with insurance acceptance varying substantially across states. We also found significant session rate differentials, with Medicaid rates being on average 40% lower than reported cash pay rates, which averaged $143.26 a session. Taken together, low insurance acceptance across a broad swath of mental health provider types means that access to care is disproportionately reliant on patients' ability to afford out-of-pocket payments-even when covered by insurance. Introduction: While the provision of unapproved regenerative medicine has been problematic worldwide, few studies have examined the implementation status of regenerative medicine (RM) in the specific field. This study aimed to determine the current status of therapy and clinical research in the obstetrics and gynecology (OBGYN) in Japan under the Act on the Safety of Regenerative Medicine (RM Act)., Methods: Detailed data were extracted from publicly available websites provided by the Ministry of Health, Labour, and Welfare. We extracted descriptive details, including risk classification of the RM Act, modality, target disease, locality, institution, and administration route. For therapy, the price for each modality was evaluated., Results: The total number of therapeutic provision plans in OBGYN (1.9% of RM in Japan) are classified as Class II (moderate) risk. Most were administered in clinics in urban areas for treating endometrial or ovarian infertility by locally administering platelet-rich plasma (PRP) or autologous mesenchymal stem cells (MSCs). The price using MSCs is approximately eight times more expensive that of those involving PRP (1832.1 ± 1139.8 vs 240.8 ± 106.5 thousand yen, p < 0.0001). Regarding research, four plans (2.2%) were submitted to target implantation failure and advanced gynecological cancer using autologous lymphocytes, dendritic cells, or MSCs., Conclusion: The RM Act permits knowledge of the current status of regenerative medicine even for unapproved uses in a specific clinical field. Aims and Objectives: This study aimed to assess the prevalence of malpractice in dentistry and its possible causes in Riyadh, Saudi Arabia during 2009-2015. Methodology: A cross-sectional retrospective analysis of the dental complaints filed by the patients in the general directorate of forensic medicine centers in Riyadh was reviewed, and the data was collected. Descriptive statistics of frequency distribution and percentages were calculated using the collected data. The Chi-square test for categorical variables was performed with a significance level set at P < 0.05. Results: The Riyadh region reported 168 (14.5%) dental malpractice cases during 2009- 2015. The highest number of malpractice cases was reported in 2010 (18.45%), and the least number of cases was reported in 2011 (10.1%). Prosthodontic specialty had the highest number of lawsuits (32.4%), followed by orthodontics (20.2%) and endodontics (15.8%). The Chi-square test showed no association between the different specialties and types of malpractice (P = 0.881) and between compensation and the type of malpractice (P = 0.832). Among the 114 cases, financial compensation was the most common retribution. Conclusion: Dental malpractice prevalence was high in the private sector, followed by the government and military sectors, with the highest number of cases registered against the prosthodontic specialty. Abstract Background The coronavirus outbreak (COVID-19) in China has influenced every aspect of life worldwide. Given the unique characteristics of the dental setting, the risk of cross-infection between dental practitioners and patients is high in the absence of adequate protective measures, and dentists may develop severe anxiety in relation to the current pandemic. The limited provision of services and widespread closure of dental practices have raised concerns among dental professionals about the financial impact. The present study assessed the frequency of dental practice closure during the pandemic’s first wave in several countries and whether closures and their associated factors differ between the private and non-private sectors. Methods An electronic cross-sectional survey questionnaire was sent to dentists in several countries, from April to May 2020. The survey assessed professional, practice related and country-level structural factors elucidating the reason for practice closure. Multilevel logistic regression was used to assess the association between practice closure and these factors, and differences were evaluated by sector type. Results Dentists from 29 countries (n = 3243) participated in this study. Most of the participants (75.9%) reported practice closure with significantly higher percentage in the private sector than the non-private sector. Greater pandemic-related fears were associated with a significantly higher likelihood of practice closure in the private (odds ratio [OR] = 1.54, 95% confidence interval [CI] 1.24, 1.92) and non-private (OR = 1.38, 95% CI 1.04, 1.82) sectors. Dentists in non-private rural areas (OR = 0.58, 95% CI 0.42, 0.81), and those in hospitals (overall OR = 0.60, 95% CI 0.36, 0.99) reported a low likelihood of closure. A high likelihood of closure was reported by dentists in the academia (OR = 2.13, 95% CI 1.23, 3.71). More hospital beds at the country-level were associated with a lower likelihood of closure in the non-private sector (OR = 0.65, 95% CI 0.46, 0.91). Private- sector dentists in high- income countries (HICs) reported fewer closures than those in non-HICs (OR = 0.55, 95% CI 0.15, 1.93). Conclusions Most dentists reported practice closure because of COVID-19, and greater impacts were reported in the private sector than in the non-private sector. Closure was associated with professional, practice, and country-levels factors.

ObjectiveTo examine adherence to guideline-concordant hypertension treatment practices at community health centers (CHCs) compared with private physicians' offices.Data sources/study settingNational Ambulatory Medical Care Survey from 2006 to 2010.Study designWe examined four guideline-concordant treatment practices: initiation of a new medication for uncontrolled hypertension, use of fixed-dose combination drugs for patients on multiple antihypertensive medications, use of thiazide diuretics among patients with uncontrolled hypertension on ≥3 antihypertensive medications, and use of aldosterone antagonist for resistant hypertension, comparing use at CHC with private physicians' offices overall and by payer group.Data collection/extraction methodsWe identified visits of nonpregnant adults with hypertension at CHCs and private physicians' offices.Principal findingsMedicaid patients at CHCs were as likely as privately insured individuals to receive a new medication for uncontrolled hypertension (AOR 1.0, 95 percent CI: 0.6-1.9), whereas Medicaid patients at private physicians' offices were less likely to receive a new medication (AOR 0.3, 95 percent CI: 0.1-0.6). Use of fixed-dose combination drugs was lower at CHCs (AOR 0.6, 95 percent CI: 0.4-0.9). Thiazide use for patients was similar in both settings (AOR 0.8, 95 percent CI: 0.4-1.7). Use of aldosterone antagonists was too rare (2.1 percent at CHCs and 1.5 percent at private clinics) to allow for statistically reliable comparisons.ConclusionsIncreasing physician use of fixed-dose combination drugs may be particularly helpful in improving hypertension control at CHCs where there are higher rates of uncontrolled hypertension.

Context: Private practice dentistry is predominant in Nigeria. Audits of fixed-prosthodontic practice in these settings are rare and will highlight capacity and guide training. This study aimed to assess the scope and quality of fixed-prosthodontic practice in such settings in Nigeria. Objectives: The study sought to assess: • Participants’ sociodemographic and their diagnostic and preoperative practices. • The quality of impression making and operative technique, reviews and maintenance, temporization practices and communication with the laboratory. • The range of treatments given and provision of advanced treatments. Materials and Methods: An adapted questionnaire was administered electronically on Nigerian dentists practicing fixed-prosthodontics in private practice settings. Retrieved data underwent descriptive statistics and associations were tested with the Fischer’s Exact and Chi-Square tests using IBM SPSS Statistics version 21. Statistical significance was set at p≤0.05. Results: A total of 100 valid responses with a male:female ratio of 2.1:1 from 16 states were retrieved. Fifty-nine participants had only first degrees and mean experience was 14.3±9.5 years. There were 27 adequate responses with regards to range of treatments offered. Sixty-three participants practiced direct temporization. Twenty-one and 14 participants regularly practiced implantology and CAD/CAM dentistry respectively. Direct temporization was significantly associated with increasing education (X2=6.03, p=0,05) and experience (X2=13.2, p=0.03). Conclusion: Only a few Nigerian dentists in private practice gave an adequate range of treatment. Most of them prefer direct temporization. Implantology and CAD/CAM dentistry practice are improving, but are still not very common.

This article, written by private practitioners in anaesthesia who have gone through the exciting and mundane phases of private practice examines the pro and cons of private practice; this can possibly help postgraduate students contemplating to take up private practice as a career.

Abstract Background A growing demand for physicians exists worldwide. Due to political changes, economic incentives and new workplace expectations a trend from single-handed practices to group practices has been noticed in many countries over the last years. In view of this background, our study aimed to identify determinants for success or failure of inter-physician collaboration in order to positively influence future collaboration in anticipation of the important role group practices may play in future health care delivery. Methods We chose a qualitative study design, using semi-structured phone interviews to collect data from physicians and non-physician consultants with experience in inter-physician collaboration that were analysed using content analysis. Eleven physicians with experience in collaborative working and fourteen non-physician consultants specializing in advice to health care professionals participated. Results Education in entrepreneurial skills as well as implementation of good practice management in preparation for collaboration are crucial modifiable facilitators for successful inter-physician collaboration. Furthermore, open communication and realistic insight into the mode of acquaintance, moral concept and degree of specialisation of the colleagues involved play major roles for the success of inter-physician collaboration in group practices. Conclusions There are several underlying themes beyond clinical expertise concerning success or failure of group practices. To influence future collaboration positively, it is important to focus on management and communication skills as well as to address basic understanding of economics.

BackgroundAlthough the simplification of antiretroviral (AVR) treatment regimens and follow-up has led to fewer constraints for patients with HIV, their follow-up remains of paramount importance to optimize AVR therapy, to detect and prevent HIV-related morbidity, and prevent secondary infections. The problem of follow-up interruption in French Guiana has been persistent and seemingly impervious to efforts to alleviate it.ObjectiveThe objective was to follow the trend of follow-up interruptions and to test the hypothesis that an increasing number of patients was, in fact, followed by private practitioners.MethodUsing the complementary lenses of the hospital HIV cohort and the health insurance information system, we looked at the incidence of follow-up interruption and the proportion of patients followed by private practitioners.ResultsWe tallied 803 persons that were not known to have died and who were lost to follow-up. Over time, hospital outpatients were lost to follow-up significantly sooner. By contrast, there was a significant trend with more and more patients exclusively followed by private practitioners.ConclusionWhile hospital outpatient care remains by far the most common mode of patient care, there seems to be a gradual erosion of this model in favor of private practice.

Objective: To explain how Dutch novice physical therapists experience their transition from student to physical therapist in private practice., Methods: A qualitative, phenomenological study was performed in The Netherlands to collect personal experiences from novice physical therapists who graduated <1 year ago. Data were collected with semi-structured interviews and analyzed using interpretative phenomenological analysis., Results: The transition experience from student to novice physical therapist is a personal, complex, and context-dependent phenomenon. Sixteen novice physical therapists (of whom 10 were still working in private practice) said their transition was liberating, stressful, insecure, unexpected, chaotic, and challenging. The variety in experiences occurred from the fact that the impact of an experience varies from one individual to the other, depending on their previous (life) experiences, uncertainty tolerance, coping, and personal and professional environment. Four overarching themes emerged: (1) 'Suddenly, I was on my own', (2) 'I was unprepared for the hassle and demands of clinical practice', (3) 'I couldn't make the impact I expected', and (4) 'I had to find a new me'., Conclusions: A novice physical therapist's transition is a context-related phenomenon, unique for each individual and determined by individual experiences and coping strategies. Novice physical therapists described a feeling of 'faking': they had to pretend to know what they were doing [while treating a patient] while in reality, they felt like they did not. For some novice physical therapists, a primary reason to leave the profession.

This paper looks at child psychiatric services in the private sector and all that can be done with child psychiatric services in the private sector. The need for public-private partnerships, the role of child psychiatrists in private practice providing integrated services, and the need for school mental health services by private psychiatrists is emphasized. Increasing research productivity among practicing counsellors has long been an aim of the New Zealand Association of Counsellors as evidenced by several articles addressing the topic published in the New Zealand Journal of Counselling over the last 20 years. Similarly, in a profession increasingly pressured to be more accountable, it is no longer considered good enough for counsellors and counselling agencies merely to espouse the "art of counselling" perspective in justifying their practices. Instead, researching counselling, especially counselling impact, is vital to the wider acceptance of the profession. Such research depends, in large part, on encouraging more counsellors to participate in researching their own casework, either as individuals or as a member of a research team or group. This article references two examples of the second of those options (specifically, collaboration between a counsellor in private practice and outside researchers) to demonstrate that even rudimentary collaboration between counsellors and researchers can produce useful knowledge about the process and impact of counselling and can have positive effects on a counsellor's practice. Increased collaboration of this sort is one way to fulfil the need for counsellors and researchers to generate more practice-based research (PBR), especially outcome research (Sexton et al., 1997).
