Background: Bruxism is a recurrent parafunctional habit characterized by clenching or grinding teeth and/or jaw contractions. It is quite common among dental students and is associated with psychosocial factors such as stress and anxiety. This study aimed to compare bruxism awareness and self-assessment among clinical students with and without bruxism. Methods: This study included systemically healthy individuals aged 18 to 27 years. Intraoral and extraoral examinations were performed on 128 4th- and 5th-grade dental students with (64) and without bruxism (64), 4th and 5th-grade dental students at Bolu Abant İzzet Baysal University who met the inclusion criteria. The diagnosis of bruxism was based on the 2018 International Consensus and data collected through a validated 33-item questionnaire covering general information and bruxism awareness. The questionnaire included multiple-choice questions on bruxism types, risk factors, symptoms, and impact on periodontal tissues. Statistical analyses were performed via IBM SPSS Statistics, and p < 0.05 was considered statistically significant. Results: Clenching and teeth grinding during sleep, tooth wear, muscle fatigue, pain in the temples, jaw pain, joint trismus, neck pain, and jaw joint pain and clenching while awake were significantly different in bruxism patients (p < 0.05). There was no significant difference between the two groups in terms of bruxism type, symptoms, treatment methods, periodontal tissues, occlusal trauma, diagnostic methods, or the relationship between lifestyle and bruxism (p > 0.05). While 71.9% of those diagnosed with bruxism stated that they had bruxism, 68.8% of those who were not diagnosed stated that they did not have bruxism. Conclusion: Although dental students are aware of bruxism, their understanding of its multifactorial nature and treatment options needs improvement. Increasing this knowledge could reduce the prevalence of bruxism among dental students and improve patient care. Objectives: Bruxism is a parafunctional orofacial behavior. For diagnosis, wearable devices that use sounds as biomarkers can be applied to provide the necessary information. Human beings emit various verbal and nonverbal sounds, making it challenging to identify bruxism‐induced sounds. We wanted to investigate whether the acoustic emissions of different oral behaviors have distinctive characteristics and if the placement of the transducer has an impact on recording the sound signals. Material and Methods: Sounds from five oral behaviors were investigated: jaw clenching, teeth grinding, reading, eating, and drinking. Eight transducers were used; six were attached to the temporal, frontal, and zygomatic bones with the aid of medical tape, and two were integrated into two commercial earphones. The data from 15 participants were analyzed using time‐domain energy, spectral flux, and zero crossing rate (ZCR). Results: In summary, all oral behaviors showed distinct characteristic features except jaw clenching, though there was a peak in the recording, possibly due to tooth tapping, before its expected onset. For teeth grinding, the transducer placement did not have a significant impact (p > 0.05) based on energy, spectral flux, and ZCR. For jaw clenching, the transducer placement had an impact with regard to spectral flux (p < 0.01). For reading and eating, the transducer placement had a significant impact with regard to energy (p < 0.05 for reading, p < 0.01 for eating), spectral flux (p < 0.001 for reading, p < 0.01 for eating), and ZCR (p < 0.001 for both reading and eating). For drinking, the transducer placement only had a significant impact with regard to ZCR (p < 0.01). Conclusions: We were able to record the sounds of various oral behaviors from different locations on the head. The aim of this systematic review is to evaluate the impact of estrogen levels on the occurrence of temporomandibular disorders (TMDs) in humans. Searches were conducted in the same databases as follows: PubMed, the Cochrane Collaboration database, and the Scopus database. In accordance with the MeSH database and previous work, the following keywords were used: 'estrogens' and 'temporomandibular joint disorders'. Twelve studies were included in the review and were assessed for the quality of evidence. Estrogen levels are associated with pain modulation in the temporomandibular joint and the entire orofacial region. There is insufficient evidence to either confirm or refute the influence of estrogen on the occurrence of TMDs. The study was registered under the identifier: 10.17605/OSF.IO/BC7QF. This study aims to assess the presence and progression of TMD in adult patients undergoing orthognathic surgery to correct dentoskeletal open bite and evaluate whether these changes can be attributed to the intervention. A retrospective cohort study was conducted on 44 adult patients (14 males and 30 females) aged 18 to 43 years. Articular assessments were performed to evaluate temporomandibular joint (TMJ) health and functionality before (T0) and after (T1) combined orthodontic-surgical treatment. TMJ health was assessed by maximum mouth opening, joint noises, parafunctions (bruxism or clenching), joint locking, TMJ pain, masticatory muscle pain, and headaches. Statistical analyses used McNemar's Exact Test and paired T-tests. The study shows a significant reduction (p < 0.05) in symptoms, except for locking, with the most substantial decrease in headaches (p = 0.0001). Overall, articular symptoms markedly decreased post-surgery, with sustained joint functionality. Restoring physiological occlusion in patients with anterior open bite is crucial for maintaining the stomatognathic system's balance. Background: Nowadays, stabilization splints for the management of bruxism and temporomandibular disorders (TMD) can be produced utilizing a digital workflow comprising a digital impression of the teeth, digital splint design, and computer-aided manufacturing of the splints. The latter is usually a milling process, however, more recently 3D printing gained popularity due to its better cost and time efficiency. It remains unknown whether 3D printed stabilization splints are inferior to milled splints regarding clinical outcomes. Methods: This clinical trial assesses the non-inferiority of 3D printed occlusal splints compared to milled occlusal splints in a monocentric prospective randomized single-blinded crossover trial with two cohorts. One cohort includes 20 participants with bruxism, the other 20 participants with pain-related TMD, i.e., myalgia, myofascial pain, or arthralgia of the jaw muscles/the temporomandibular joint(s) diagnosed according to the Diagnostic Criteria for Temporomandibular Disorders (DC/TMD). Michigan-type stabilization splints are fabricated in a digital workflow by milling or 3D printing using CE-marked materials within their intended purpose. The participants wear a milled and a 3D printed splint in a randomized order for 3 months each, with follow-up visits after 2 weeks and 3 months. Investigated outcome parameters are oral health-related quality of life (OHRQoL) evaluated by the Oral Health Impact Profile (OHIP-G14), participant satisfaction as rated on a visual analog scale, therapeutic efficacy, and technical result of the splints. In this context, therapeutic efficacy means antagonist wear and—in the TMD group—reduction of pain/disability assessed by the Graded Chronic Pain Scale (GCPS v2.0) and clinical assessment following the DC/TMD standard, while technical outcome measures splint fit, wear and fracture rate. Discussion: The trial will provide important information on the clinical outcome of 3D printed stabilization splints in comparison to milled splints and will, therefore, enable an evidence-based decision in favor of or against a manufacturing process. This, in turn, will guarantee for a maximum of the patient's OHRQoL during splint therapy, therapeutic efficacy, and longevity of the splints. Introduction: Headache is an illness with high prevalence and adverse effects on quality of life. As oral or dental problems such as pain can trigger or aggravate it, we aimed to investigate and compare oral and dental health status in patients affected with chronic headaches and healthy individuals. Methods: The present case-control study included 60 patients with chronic headaches (case) and 60 healthy individuals (control) in Rasht, Iran. The demographic characteristics and clinical examinations, including decayed/missing/filled teeth (DMF-T) and community periodontal index of treatment needs (CPITN) indices, as well as bruxism, frequency of tooth brushing and flossing, and maxillary and mandibular tooth wear were recorded in a checklist. Data analysis was performed using the IBM SPSS version 28 at a significance level of 0.05. Results: The case group consisted of 25 men (41.7%) and 35 women (58.3%) with an average age of 32.55 ± 6.62 years, while the control group had 27 men (45%) and 33 women (55%) with an average age of 30.95 ± 6.33 years. The study groups were not significantly different in bruxism, frequency of tooth brushing and flossing, DMFT, CPTIN, and maxillary and mandibular tooth wear. Conclusion: It seems that chronic headaches do not significantly affect the oral and dental health of the sufferers. Objectives: Temporomandibular disorders (TMDs) have a relatively high prevalence among university students. This study aimed to identify independent risk factors for TMD in university students and develop an effective risk prediction model. Methods: This study included 1,122 university students from four universities in Changchun City, Jilin Province, as subjects. Predictive factors were screened by using the least absolute shrinkage and selection operator (LASSO) regression and the machine learning Boruta algorithm in the training cohort. A multifactorial logistic regression analysis was used to construct a TMD risk prediction model. Internal validation of the model was conducted via bootstrap resampling, and an external validation cohort comprised 205 university students undergoing oral examinations at the Stomatological Hospital of Jilin University. Results: The prevalence of TMD among university students was 44.30%. Ten predictive factors were included in the model, comprising gender, facial cold stimulation, unilateral chewing, biting hard or resilient foods, clenching teeth, grinding teeth, excessive mouth opening, malocclusion, stress, and anxiety. The model demonstrated good predictive ability with area under the receiver operating characteristic curve (AUC) values of 0.853, 0.838, and 0.821 in the training cohort, internal validation cohort, and external validation cohort, respectively. The calibration curves demonstrated that the predicted results were consistent with the actual results, and the decision curve analysis (DCA) indicated the model's high clinical utility. Conclusions: An online nomogram of TMD in university students with good predictive performance was constructed, which can effectively predict the risk of TMD in university students. The model provides a useful tool for the early identification and treatment of TMDs in university students, helping clinicians to predict the probability of TMDs in each patient, thus providing more personalized and accurate treatment decisions for patients. Objective: To determine the prevalence of self-reported bruxism, awareness of bruxism's adverse effects, and bruxism management among Thai undergraduate dental students. Methods: An online self-reported questionnaire was designed and tested for validity and reliability and then distributed to dental students at 12 universities in Thailand from September 2020 to January 2021. Results: There were 464 respondents (338 females and 126 males) with an average age of 21.42 ± 2.06 years. The prevalence of self-reported awake and sleep bruxism was 35.78% and 37.28%, respectively, and 19.40% had both awake and sleep bruxism. Awareness scores were higher for dentists than for patients (p< 0.001). Only 12.14% of sleep bruxers still wear an occlusal splint for bruxism management. Conclusion: The prevalence of self-reported awake and sleep bruxism was high among Thai dental students. Background: Temporomandibular joint has biomechanical and neurological interactions with cervical region in a complex functioning that is addressed as cranio-cervical mandibular system. Subjects and methods: In a prospective cohort study of 67 patients, their mean age 24.3 ± 5.6 years of both genders with temporomandibular disorder "bruxism," patients had fulfilled a questionnaire regarding bruxism manifestations. Pain pressure algometer was used to assess pain pressure threshold for trigger point in masseter, temporalis, sternocleidomastoid, and trapezius, and bubble inclinometer was used to assess cervical proprioception for all cervical ranges. Results: There was a statistical significant indirect weak correlation between proprioception error of cervical flexion and pain pressure threshold of masseter (r = − 0.333) (p = 0.006), between proprioception error of cervical flexion and pain pressure threshold of trapezius (r = − 0.363) (p = 0.003), and also between proprioception error of cervical left-side bending and pain pressure threshold of trapezius (r − 0.298; p 0.014), while there was significant direct weak correlation between proprioception error of cervical left rotation and cervical bending to left (r 0.315; p 0.009), where the p-value was < 0.05. Conclusion: The study proved that temporomandibular joint pain was correlated with impaired neck proprioception in whom with bruxism. Trial registration: NCT05657353. Diagnosis of bruxism is challenging because not all contractions of the masticatory muscles can be classified as bruxism. Conventional methods for sleep bruxism detection vary in effectiveness. Some provide objective data through EMG, ECG, or EEG; others, such as dental implants, are less accessible for daily practice. These methods have targeted the masseter as the key muscle for bruxism detection. However, it is important to consider that the temporalis muscle is also active during bruxism among masticatory muscles. Moreover, studies have predominantly examined sleep bruxism in the supine position, but other anatomical positions are also associated with sleep. In this research, we have collected EMG data to detect the maximum voluntary contraction of the temporalis and masseter muscles in three primary anatomical positions associated with sleep, i.e., supine and left and right lateral recumbent positions. A total of 10 time domain features were extracted, and six machine learning classifiers were compared, with random forest outperforming others. The models achieved better accuracies in the detection of sleep bruxism with the temporalis muscle. An accuracy of 93.33% was specifically found for the left lateral recumbent position among the specified anatomical positions. These results indicate a promising direction of machine learning in clinical applications, facilitating enhanced diagnosis and management of sleep bruxism. [ABSTRACT FROM AUTHOR]

Background/Objectives: Temporomandibular disorders (TMD) encompass a range of musculoskeletal and neuromuscular conditions affecting the temporomandibular joint (TMJ) and associated structures. This cross-sectional study, conducted in a Portuguese TMD department, aimed to assess the relationship between malocclusion and TMD severity. Methods: Data on demographic variables, TMD clinical symptoms, and malocclusion classes were collected using the EUROTMJ database. The Chi-square test (χ2) identified associations, with their intensity measured by Cramér's V (φc). Results: The study included 1170 patients (932 females and 238 males), with a mean age of 41.73 ± 16.80 years. Most patients exhibited Angle Class I malocclusion (85.5%), followed by Angle Class II (13.5%) and Angle Class III (1.1%). Class II malocclusion was associated with increased TMD severity (p < 0.001), higher myalgia levels (p = 0.002), more frequent disc displacement without reduction (p = 0.002) and lower maximum mouth opening values (Class II: 38.13 ± 7.78 mm, Class I: 39.93 ± 8.67 mm). Significant associations were also found between malocclusion type and arthralgia (p = 0.021), mouth-opening limitation (p = 0.016), and TMJ crepitus (p = 0.017). In cases of malocclusion, the presence of oral signs of bruxism explained the degree of myalgia, disc displacement, and severity (p = 0.003; p = 0.048; p = 0.045). Conclusions: This study highlights that (1) the most common type of dental malocclusion in TMD patients was Class I; (2) Class II malocclusion was associated with increased TMD severity and oral signs of bruxism; and (3) Class III was rarely observed in TMD consultation. Aim: This study examines the prevalence and severity of orofacial disorders in patients with fibromyalgia syndrome (FMS). The research assesses the correlation with the Fibromyalgia Assessment Status (FAS) index. The goal is to improve the clinical approaches to these patients. Methods: A cross-sectional study was conducted using a structured questionnaire focused on the correlation between FMS and orofacial problems. The research involved 107 rheumatology patients diagnosed with FMS. Statistical analyses, including Spearman's correlation, were utilized to investigate the relationships between the FAS index scores and various orofacial symptoms. Results: Of the participants, 11.2% responded that they were aware of the correlation between fibromyalgia and oral health. The statistical analysis showed statistically significant correlations between the FAS index and symptoms such as gum bleeding, teeth grinding during the day, and neck pain (p < 0.05). The correlation between the FAS index and joint noise upon opening the mouth, as well as dissatisfaction with one's smile, also proved to be highly significant (p < 0.001). Conclusion: The outcomes demonstrate that, as the FAS index increases, the likelihood of developing orofacial disorders also increases among FMS patients. Smoking a cigarette before bed or first thing in the morning is a common habit. In this review, the relationship between smoking and sleep is investigated based on the existing literature. Out of 6504 unique items that were identified via a PubMed search related to smoking and sleep, 151 were included in this review. Tobacco smoking disrupts sleep architecture by reducing slow wave and rapid eye movement (REM) sleep and undermining sleep quality. Furthermore, smoking affects sleep-related co-morbidities, such as obstructive sleep apnea–hypopnea syndrome (OSAHS), insomnia, parasomnias, arousals, bruxism, and restless legs, as well as non-sleep-related conditions such as cardiovascular, metabolic, respiratory, neurologic, psychiatric, inflammatory, gynecologic and pediatric issues, while poor sleep quality also seems to worsen the chances of successful smoking cessation. This paper presents a method of calculating and correcting grinding face gears on an internal gear grinding machine. The generating principle of face gears is studied, and the feasibility of grinding motion on an internal gear grinding machine is analyzed. Then, the motions that need to be followed for grinding are analyzed based on the gear machine tool structure. Four main error sources causing tooth surface deviation in the grinding movements are proposed. The mathematical modeling of the grinding of face gears containing proposed error sources on an internal gear grinding machine is accurately established. The influence of the error sources on the topological deviations of the tooth surface is explored. A sensitivity matrix is established for the influence of various error factors on the tooth surface deviations. The correction values of each error factor are obtained in the case of existing tooth surface deviations. Finally, a virtual machining experiment is conducted, which proves the accuracy of the proposed method for characterizing grinding and realizing corrections. [ABSTRACT FROM AUTHOR]

Driving stress is a multifaceted phenomenon, and the experience of driving invokes stress. Driving causes the activation of stress-response mechanisms, leading to short-term and long-term stress responses resulting in physiological and behavioral changes. The aim of this study was to evaluate driving stress-initiated effects on orofacial functions and health behaviors in the Riyadh population. A cross-sectional survey was conducted in Riyadh using a pre-validated set of questionnaires for habitual information, a driving stress assessment using a driving-behavior inventory, and an assessment of parafunctional habits and effects on orofacial functions. The results indicate that nearly 50% of the sample spends more than two hours commuting, and more than 50% of the sample has inadequate sleep and insufficient exercise. Oral parafunctional habits like nail biting (p = 0.039) and lip or object biting (p = 0.029) had a significant correlation with aggressive driving behaviors, whereas the grinding of teeth (p = 0.011), the clenching of jaws (p = 0.048), lip or object biting (p = 0.018), and pain in mastication (p = 0.036) had a positive correlation with driving dislikes. Driving stress can be detrimental to one's health and not only impacts health behaviors but also induces oral parafunctional habits and adversely affects orofacial regions and functions. Acute driving stress responses may be transient. However, prolonged driving stress can be maladaptive and can increase the risk of chronic diseases including chronic temporomandibular joint disorders and parafunctional habit-related changes in the oral cavity. Background: As interest in health increases, the number of people engaging in weight training to enhance muscle strength and muscle mass is on the rise. High-intensity weight training has been reported to induce tooth clenching habits, leading to tooth damage and temporomandibular joint (TMJ) abnormalities. Consequently, it is essential to investigate the impact of weight training accompanied by tooth clenching on oral health and to develop guidelines based on these findings. Methods: The study included male participants aged 25 years and older, comprising 15 non-exercisers and 15 professional fitness trainers who have been engaged in weight training for over 5 years. Data were collected using a self-administered questionnaire to gather information on age, exercise experience, total weight lifted in three major weight training exercises (deadlift, bench press, and squat), and teeth clenching habits. Additionally, examinations for tooth cracks and fractures, TMJ noise and pain, maximum mouth opening, occlusal force, and occlusal contact area were conducted. Results: Compared to non-exercisers, professional fitness trainers who have been engaged in weight training for over 5 years exhibited a higher prevalence of teeth clenching habits. While there were no significant differences between the two groups in occlusal contact area and the extent of tooth fractures, there were significant differences in occlusal force and the degree of tooth cracks. Furthermore, unlike joint noise and TMJ pain, the maximum mouth opening was significantly reduced in professional fitness trainers compared to non-exercisers. Conclusion: Our study suggests that the continuation of high-intensity exercise, which induces teeth clenching habits, may cause tooth damage and TMJ disorders.
