3. Possible effects of hyperparathyroidism in the loss of osseointegration of dental implants: A case report

Abstract

Background: Hyperparathyroidism (HPT) is a common endocrine disorder with potential complications on the skeletal, renal, neurocognitive and cardiovascular systems. Its association with the lack of osseointegration of dental implants has not been described in the medical literature. Case presentation: This case report aims to discuss two cases of dental implant loss in which a high level of parathormone (PTH) was found in the absence of any other systemic or local comorbidity, suggesting the possible correlation between HPT and implant. Both patients were referred to the clinic complaining about prosthetic complications, gingival inflammation and mobility of the dental prosthesis. After a Cone-Beam computed Tomography evaluation, all implants of both patients were removed for rejection arising from periimplantitis and then four implants were inserted in the patient 1 and five implants in the patient 2. For both patients short implants (Bicon LLC, Boston, Massachusetts, USA), featured by 4 mm in diameter and 5 mm in height, were used and the prosthesis substructure was made of Trinia® (Bicon LLC, Boston, Massachusetts, USA). During the fifth year, the patients reported complications and the implant treatment failure. In order to establish the causes of failure, a thorough investigation was performed. Since no causes were detected, the patients were required to perform a blood test to evaluate bone metabolism and specifically to assess parathyroid-hormone levels (PTH), calcium levels and vitamin D. Results: The results of the blood tests showed normal calcemia, vitamin D deficiency and elevated PTH levels in both patients. After an endocrinologist's consultation, secondary hyperparathyroidism was diagnosed. Conclusion: It is reasonable to assume that the loss of osseointegration of dental implants can be correlated with the effects of HPT.

7. Implant Survival Rate and Prosthetic Complications of Full-arch Implant-supported Fixed Prostheses In Edentulous Upper Jaws: A Cohort Study

Abstract

Full-arch implant-supported fixed dental prostheses, an implant-supported prosthesis, which the patient must carefully manage in home oral hygiene procedures, is the fixed alternative to a mucosal-supported or implant-anchored prosthesis. Clinically, the execution of this rehabilitation requires four implants for the lower jaw and six implants for the upper jaw. The aim of the present study is to verify the survival-rate of implants for implant-fixed prostheses in the edentulous upper jaw. This retrospective study was carried out on patients who received an upper full-arch fixed prostheses on six implants for rehabilitation. A total of 36 patients were included and evaluated from a clinical and radiographical point of view. The follow-up period in which the data were collected on the upper full-arch is between 60 and 84 months. A total of 232 implants were inserted and monitored in this period. Clinical and radiographic evaluations were carried out on all 232 implants, with constant re-evaluation. The total implant survival rate is 93.1%, a value which is similar to previous studies already published on the topic. There were few prosthetic complications, mainly the fracture of anterior prosthetic teeth. Most of these full-arch, which as antagonist had another previously made full-arch implant-supported fixed dental prostheses or overdenture on four or overdenture on two implants, achieves good results in this study at 84 months., Las prótesis dentales fijas implantosoportadas de arcada completa, una prótesis implantosoportada, que el paciente debe manejar cuidadosamente en los procedimientos de higiene bucal domiciliaria, es la alternativa fija a una prótesis soportada por mucosas o anclada a implantes. Clínicamente, la ejecución de esta rehabilitación requiere cuatro implantes para el maxilar inferior y seis implantes para el maxilar superior. El objetivo del presente estudio es verificar la tasa de supervivencia de los implantes para prótesis fijadas sobre implantes en el maxilar superior edéntulo. Este estudio retrospectivo se llevó a cabo en pacientes que recibieron prótesis fijas de arcada completa superior sobre seis implantes para rehabilitación. Se incluyeron y evaluaron desde el punto de vista clínico y radiográfico un total de 36 pacientes. El período de seguimiento en el que se recogieron los datos de la arcada completa superior es de entre 60 y 84 meses. En este período se insertaron y controlaron un total de 232 implantes. Se llevaron a cabo evaluaciones clínicas y radiográficas de los 232 implantes, con una reevaluación constante. La tasa de supervivencia total de los implantes es del 93,1%, valor similar a estudios previos ya publicados sobre el tema. Hubo pocas complicaciones protésicas, principalmente la fractura de los dientes protésicos anteriores. La mayoría de estos pacientes de arcada completa, que como antagonistas habían confeccionado previamente otras prótesis dentales fijas implantosoportadas de arcada completa o sobredentaduras de cuatro o sobredentaduras de dos implantes, logran buenos resultados en este estudio a los 84 meses.

8. Comparative Evaluation of Marginal Adaptation and Dimensional Stability of Three Bioceramic Root Repair Materials: A VP-SEM Analysis.

Abstract

This study investigated the marginal adaptation of three recently introduced bioceramic root repair materials, EdgeBioCeramic RetroFill, Endocem MTA, and One-Fil PT, using VP-SEM analysis. Extracted single-rooted lower incisors were used to simulate retrograde fillings. The results showed no statistically significant differences in the marginal gap between the materials and the dentin walls. All three materials exhibited good dimensional stability, with gap sizes comparable to previously published research on similar materials. The mean GAP was 3.91 ± 2.56 for EdgeBioCeramic RetroFill, 4.32 ± 2.69 for Endocem MTA, and 4.50 ± 2.54 for One-Fil PT. This study employed VP-SEM, a valuable tool for analyzing bioceramic materials without altering their properties. The findings suggest the possibility of daily clinical use of these bioceramics by endodontists and general practitioners that could find applications in retrograde fillings and perforation repairs. However, further in vivo studies are needed to confirm long-term stability and assess the influence of sample preparation methods. [ABSTRACT FROM AUTHOR]

Abstract

Background.: This study evaluated the gingival crevicular fluid (GCF) and Peri- implant crevicular fluid (PICF) concentrations of interleukin-1 beta (IL-1β), interleukin-6 (IL-6), tumor necrosis factor-alpha (TNF-α) and active metalloproteinase-8 (a-MMP-8) in sites with healthy conditions vs. sites affected by periodontitis (PER) and peri-implantitis (PIM). Methods.: Periodontally healthy (PH) sites with PER, sites with peri-implant health (PIH), and sites with PIM were investigated intra-individually, according to the inclusion criteria of each group. Probing pocket depth (PPD), plaque index, gingival index, and the presence or absence of bleeding on probing (BoP) were evaluated. In GCF and PICF samples, IL-1β, IL-6, and TNF-α were quantified by ELISA Duoset® kit in combination with Ultramark® micro-ELISA digital reader; a-MMP8 concentration was analyzed by a chairside test (Perio/ImplantSafe®) in combination with a digital reader (ORALyzer®). Results.: The concentrations of IL-6 and IL-1β, TNF-α, and a-MMP-8 were significantly higher in the PIM and PER sites compared to healthy sites (P <0.05).>P<0.05),>P>0.05). Conclusion.: aMMP-8, IL-6, IL-1β, and TNF-α presented higher GCF/PICF concentrations in diseased periodontal and peri-implant sites. Abstract

(1) The possibility of knowing information about the anatomy in advance, in particular the arrangement of the endodontic system, is crucial for successful treatment and for avoiding complications during endodontic therapy; the aim was to find a correlation between a minimally invasive and less stressful endodontic access on Ni-Ti rotary instruments, but which allows correct vision and identification of anatomical reference points, simplifying the typologies based on the shape of the pulp chamber in coronal three-dimensional exam views. (2) Based on the inclusion criteria, 104 maxillary molars (52 maxillary first molars and 52 maxillary second molars) were included in the study after 26 Cone Beam Computed Tomography (CBCT) acquisitions (from 15 males and 11 females). And linear measurements were taken with the CBCT-dedicated software for subsequent analysis. (3) The results of the present study show data similar to those already published about this topic. Pawar and Singh's simplified classification actually seems to offer a schematic way of classification that includes almost all of the cases that have been analyzed. (4) The use of a diagnostic examination with a wide Field of View (FOV) and low radiation dose represents an exam capable of obtaining a lot of clinical information for endodontic treatment.
