Objective To investigate the application of mycophenolate mofetil (MMF) in oral mucosal pemphigoid and provide a clinical reference. Methods One case of glucocorticoids combined with MMF in the treatment of oral mucosal pemphigoid was reported, and the clinical application of MMF in oral mucosa-related bullous diseases was discussed. Results One patient with a clinical diagnosis of “oral mucosal pemphigoid” was treated with methylprednisolone (36 mg, qd, morning dose) or combined hydroxychloroquine sulfate (0.1 g/time, bid) and thalidomide capsules (50 mg, qd, bedtime) and other drugs. The patient’s disease was slowly controlled but prone to recurrence. The treatment regimen was immediately adjusted, i.e., methylprednisolone (36 mg, qd, morning dose) was combined with MMF (0.5 g/time, bid) for 2 weeks, which resulted in ideal lesion healing control. After 8 weeks of methylprednisolone combined with MMF, the dose of methylprednisolone was gradually reduced to 12 mg, qd, and MMF was reduced to 0.5 g, qd, the patient’s symptoms improved significantly, and no obvious lesions were found in the mouth. The dose was then reduced and maintained according to the principle of pemphigoid treatment. Methylprednisolone (8 mg, qd, morning dose) and MMF (0.5 g, qd) have been used for 6 months of maintenance treatment, and they are still being followed up. As yet, the patient’s condition is stable without obvious lesions and new blisters, and no obvious side effects have been observed. A review of the literature shows that MMF is widely used in the field of dermatology to treat a variety of immune diseases, such as connective tissue diseases and autoimmune blistering diseases. According to the reports of adverse reactions to MMF, digestive system reactions are the most common adverse reactions; therefore, patients with active gastrointestinal diseases should be treated with caution, followed by bone marrow suppression, and it is recommended to monitor liver function and blood routine in patients using MMF. The safety and efficacy of MMF for treating pemphigoid involving the skin have been reported in the literature, but oral mucosal doctors still lack experience for treating mucous membrane pemphigoid. Conclusions As a new immunosuppressant, MMF has high safety and no obvious side effects and can be considered as a combination adjuvant drug for patients with severe clinical disease and refractory oral mucosal pemphigoid.

The three-dimensional network structure, which is formed by the precipitated wax crystals, is generally considered as the direct cause for gelling of waxy crude oil. Through interaction with wax molecules, the two polar components, namely the resin and asphaltene,participate in the wax crystallization and subsequently alter the wax precipitation properties. This makes it rather complicated to explore the gelling behavior of crude oil. The micro-dynamic behaviors among resin, asphaltene and wax molecules have a direct bearing on the morphology of mesoscopic wax crystals, and then play a decisive role in the macro-rheological property of waxy crude oil multiphase system. Macroscopically, the effect of resin and asphaltene on the rheology of waxy crude oil and its characterization methods were summarized based on the rheological experiments. Mesoscopically, microscopic experiments and molecular simulation were adopted to analyze the effect of resin and asphaltene on the morphology and particle size of wax crystal, as well as the synergistic mechanism among resin, asphaltene and wax crystals. Microscopically, the molecular coalescence and interphase interaction were introduced through the molecular dynamics simulation. Meanwhile, the effect of resin and asphaltene molecules on the gelling properties of waxy crude oil was discussed. Additionally, prospect was made to the application of molecular dynamics simulation in the field of waxy crude oil gelling, which is expected to lay a theoretical foundation for the development of waxy oil pipeline transportation technology and provide reference to the research, development and application of pour point depressant.

CO2 sequestration in sediments as solid hydrate is considered a potential way to capture and store anthropogenic CO2. When CO2 hydrate is formed in front of CO2 migration, the injection channel will be blocked, and the removal of hydrate blockage becomes the first problem that must be faced. This work proposed an N2 injection method to remove CO2 hydrate blockage. Based on numerical simulation, a study was conducted using TOUGH+MIXHYD v.1.0 to confirm the feasibility of N2 injection and compare it to depressurization. The spatial and temporal distribution characteristics of pressure, temperature, hydrate saturation, and gas saturation were investigated. Under the combined effects of temperature, pressure, and gas composition, secondary CO2-N2 hydrate can form far from the injection point, causing an increase in local temperature and hydrate saturation. The rate of CO2 hydrate dissociation using direct depressurization is significantly slower compared to N2 injection methods. As the pressure of N2 injection increases, the rate of CO2 hydrate dissociation notably accelerates, which does not show a significant increase with increasing injection temperature. This work introduced a novel approach to addressing the issue of CO2 hydrate blockage, which holds prominent significance for the advancement of hydrate-based CO2 geological sequestration. Food abundance plays an important role in the reproduction of fish, especially multiple spawners. Multiple spawners can exhibit various biological strategies when under starvation stress. However, the reproductive strategy used in these fish species remains unknown. To explore whether rare minnows ( Gobiocypris rarus ) prioritize survival over current reproduction under starvation conditions, paired adult rare minnows were starved for 0, 5, 10, 15 d and their spawning activities were recorded. Anatomical and histological characteristics of unpaired adult rare minnows were examined during starvation and following re-feeding. It was found that only 30-70% of paired rare minnows spawned within 5 d after deprivation of food. Though spawning activity was suppressed by starvation, rare minnows starved for 0, 5, 10, and 15 d waited 3.89 ± 0.78 d, 5.57 ± 3.36 d, 5.83 ± 5.15 d and 6.57 ± 4.50 d, respectively, after re-feeding to resume spawning. The average inter-spawning interval and length until egg production of those starved for 15 d was 4.60 ± 2.37 d and 139.1 ± 67.9 d, respectively, when they spawned with rhythm, which were significantly different from those starved for 0 d ( P < 0.05). Anatomical and histological data further revealed that late maturing oocytes degraded after 8 d of starvation, but the gonadosomatic index recovered to initial levels after 8 d of re-feeding. Thus, reproduction of rare minnows was markedly affected by starvation, but rapidly returned to normal upon re-feeding. These observations demonstrated that paired rare minnows prioritize survival by channeling energy from the liver and absorbing late maturing oocytes instead of continuing to reproduce under food deprivation condition.

The ionic composition of water is important for all fish. In the present study, the effects of total hardness and Ca(2+):Mg(2+) ratio on early life stages of rare minnows (Gobiocypris rarus), a promising laboratory fish in China, were evaluated. Paired parent fish were transferred to spawning aquaria (16 L) containing water at different total hardness and Ca:Mg ratios, and their offspring were further cultured at 25 ± 1 °C and 12:12-h light:dark photoperiod. Fertilization rates were not affected by total hardness to 480 mg L(-1) CaCO3, but egg size decreased with increasing total hardness. Ca:Mg ratios less than 1:20 or greater than 8:1 had adverse influences on hatching, feeding, development, larval growth, and survival. Embryos and larvae incubated in Mg(2+)- and Ca(2+)-deficient waters exhibited high malformation rates and high mortality. Our results demonstrate that rare minnows can adapt to a wide range of total hardness and Ca:Mg ratios, although an imbalance between Ca(2+) and Mg(2+) in water is toxic to this species. To increase the comparability and usefulness of test results, we recommend the use of reconstituted or drinking water of defined total hardness and Ca:Mg ratio for the culture and toxicity testing of rare minnows.

Background: Nomograms for prostate and colorectal cancer are included in the Surveillance, Epidemiology, and End Results (SEER) Cancer Survival Calculator, under development by the National Cancer Institute. They are based on the National Cancer Institute's SEER data, coupled with Medicare data, to estimate the probabilities of surviving or dying from cancer or from other causes based on a set of patient and tumor characteristics. The nomograms provide estimates of survival that are specific to the characteristics of the tumor, age, race, gender, and the overall health of a patient. These nomograms have been internally validated using the SEER data. In this paper, we externally validate the nomograms using data from Kaiser Permanente Colorado., Methods: The SEER Cancer Survival Calculator was externally validated using time-dependent area under the Receiver Operating Characteristic curve statistics and calibration plots for retrospective cohorts of 1102 prostate cancer and 990 colorectal cancer patients from Kaiser Permanente Colorado., Results: The time-dependent area under the Receiver Operating Characteristic curve statistics were computed for one, three, five, seven, and 10 year(s) postdiagnosis for prostate and colorectal cancer and ranged from 0.77 to 0.89 for death from cancer and from 0.72 to 0.81 for death from other causes. The calibration plots indicated a very good fit of the model for death from cancer for colorectal cancer and for the higher risk group for prostate cancer. For the lower risk groups for prostate cancer (<10% chance of dying of prostate cancer in 10 years), the model predicted slightly worse prognosis than observed. Except for the lowest risk group for colorectal cancer, the models for death from other causes for both prostate and colorectal cancer predicted slightly worse prognosis than observed., Conclusions: The results of the external validation indicated that the colorectal and prostate cancer nomograms are reliable tools for physicians and patients to use to obtain information on prognosis and assist in establishing priorities for both treatment of the cancer and other conditions, particularly when a patient is elderly and/or has significant comorbidities. The slightly better than predicted risk of death from other causes in a health maintenance organization (HMO) setting may be due to an overall healthier population and the integrated management of disease relative to the overall population (as represented by SEER)., (Published by Oxford University Press 2014.)
