6. Short stature in Scottish schoolchildren : a community study, with special emphasis on the prevalence of severe growth hormone deficiency

Abstract

The primary objective of this study has been to determine timr prevalence of growth hormone deficiency among Scottish school children. To achieve this it m first essential to identify a large number of very small children in a defined population and it was proposed to study thereafter all those comprising the smallest 1% in the population. Clearly, sany children had factors other than growth horsona deficiency contributing to their short stature, and therefore the literature relating both to this and the influences of genetic, environmental and other biological factors upon growth has been reviewed. The method originally proposed for identifying these children was based upon a central computer-baaed file of the heights of children at the school entrance medical inspection. Intractable difficulties inherent in this approach soon emerged and, as these would have prevented the identification of all short children within a defined population and made it impossible to accurately estimate the true prevalence of growth hormone deficiency, this method had to be discarded. The revised method entailed personally screening the heights of 48,221 children attending all education authority schools and & selection of independent schools in Edinburgh, Glasgow and Aberdeen, and identifying from them all children who were -2.5 standard deviations or more below the mm height for their chronological age (N = 449). Permission was sought from the parents of those found to be of small stature to undertake studies of the medical and social background and where appropriate to gather auxological data. Where no definite cause for short stature was apparent, these children were screened for growth horaone deficiency wherever possible. Children who failed to produce adequate growth hormone levels on the screening test and/or those whose twelve aonth height velocities were below average (less than the 25th centile for chronological age) were then further investigated for growth hormone deficiency with an insulin tolerance test. A group of control children from a similar social background but of average height for age was also selected in .Edinburgh and Glasgow. c.oae of the social and medical data from this group has been compared to the group of children with short stature in an attempt to identify any significant differences, the study has confirmed the strong association of previously recognised environmental and genetic factors with short stature. The results of the study also suggest that severe growth horaone deficiency is a acre comma cause of short stature than previously thought and that it frequently remains undiagnosed for longer than necessary.

Abstract

Research has determined that breast feeding is not a major contributor to cognitive development. In Australian tests between 1979 and 1982, assessments were made for bottle-fed and breast-fed children at ages two, four, seven and 11 to 13. After weighting for environmental, biomedical and sociodemographic factors, a statistically insignificant beneficial effect was observed for breast feeding.

Abstract

Abstract Background Home visiting programs comprising intensive and sustained visits by professionals (usually nurses) over the first two years of life show promise in promoting child health and family functioning, and ameliorating disadvantage. Australian evidence of the effectiveness of sustained nurse home visiting in early childhood is limited. This paper describes the method and cohort characteristics of the first Australian study of sustained home visiting commencing antenatally and continuing to child-age two years for at-risk mothers in a disadvantaged community (the Miller Early Childhood Sustained Home-visiting trial). Methods and design Mothers reporting risks for poorer parenting outcomes residing in an area of socioeconomic disadvantage were recruited between February 2003 and March 2005. Mothers randomised to the intervention group received a standardised program of nurse home visiting. Interviews and observations covering child, maternal, family and environmental issues were undertaken with mothers antenatally and at 1, 12 and 24 months postpartum. Standardised tests of child development and maternal-child interaction were undertaken at 18 and 30 months postpartum. Information from hospital and community heath records was also obtained. Discussion A total of 338 women were identified and invited to participate, and 208 were recruited to the study. Rates of active follow-up were 86% at 12 months, 74% at 24 months and 63% at 30 months postpartum. Participation in particular data points ranged from 66% at 1 month to 51% at 24 months postpartum. Rates of active follow-up and data point participation were not significantly different for the intervention or comparison group at any data point. Mothers who presented for antenatal care prior to 20 weeks pregnant, those with household income from full-time employment and those who reported being abused themselves as a child were more likely to be retained in the study. The Miller Early Childhood Sustained Home-visiting trial will provide Australian evidence of the effectiveness of sustained nurse home visiting for children at risk of poorer health and developmental outcomes. Abstract

This report presents an analysis of the data from the first wave of the Longitudinal Study of Australian Children (LSAC) to explore the wellbeing of 5,107 children in the infant cohort of the study and the 4,983 children, aged 4 to 5 years, in the child cohort. Wave 1 of LSAC includes measures of multiple aspects of children’s early development. These developmental measures are summarised in the LSAC Outcome Index, a composite measure which includes an overall index as well as three separate domain scores, tapping physical development, social and emotional functioning, and learning and cognitive development.

Abstract

OBJECTIVE: To assess whether socioeconomic position, maternal intelligence (IQ), and the home environment are inter-related to cognitive development in childhood. STUDY DESIGN: Prospective cohort study (n = 723) with cognitive tests at ages 2, 4, 7, and 11 to 13 years. RESULTS: There were statistically significant positive associations of father's occupational prestige, Home Observation for Measurement of Environment (HOME) score, and maternal IQ with cognitive performance in childhood. After adjustment for confounding factors, there was an increase in cognitive development by 0.8 to 2.0, 2.9 to 4.8, and 4.2 to 9.0 points for a 10-unit increment in father's occupational prestige, maternal IQ, and HOME score, respectively. CONCLUSIONS: These results demonstrate that socioeconomic position, maternal IQ, and the home environment are independently and positively predictive of children's cognitive development. These findings provide additional rationale for implementing social policies that reduce socioeconomic inequalities.

Abstract

Background: Vaccine-preventable diseases cause significant morbidity and mortality worldwide and in developing countries in particular. Information on coverage and reasons for non-vaccination is vital to enhance overall vaccination activities. Of the several survey techniques available for investigating vaccination coverage in a given setting, the Lot Quality Technique (LQT) remains appealing and could be used in developing countries by local health personnel of district or rural health authorities to evaluate their performance in vaccination and many other health-related programs. This study aimed to evaluate vaccination coverage using LQT in a selected semi-urban setting in Turkey. Methods: A LQT-based cross-sectional study was conducted in Kecioren District on a representative sample of residents aged 12-23 months in order to evaluate coverage for routine childhood vaccines, to identify health units with coverage below 75%, and to investigate reasons for non-vaccination. Results: Based on self-reports, coverage for BCG, diphtheria-pertussis-tetanus (DPT-3), oral polio-3, hepatitis-3, and measles vaccines ranged between 94-99%. Coverage for measles was below 75% in five lots. The relatively high educational and socioeconomic status of parents in the study group alone could not minimize the "considerable" risk of vaccine-preventable diseases in the District and dictates a continuity of efforts for improving vaccination rates, with special emphasis on measles. We believe that administrative methods should be backed up by household surveys to strengthen vaccination monitoring and that families should be trained and motivated to have their children fully vaccinated according to the recommended schedule and in a timely manner. Conclusion: This study identified vaccine coverage for seven routine vaccines completed before the age of 24 months as well as the areas requiring special attention in vaccination services. The LQT, years after its introduction to health-related research, remains an appealing technique for rapid evaluation of the extent of a variety of local health concerns in developing countries, in rural areas in particular, and is very efficient in determining performance of individual subunits in a given service area. Training of local health personnel on use of the LQT could expedite response to local health problems and could even motivate them in conducting their own surveys tailored to their professional interests. 39. Exposure to Environmental Lead and VisualMotor Integration at Age 7 Years

Abstract

Early childhood exposure to environmental lead may result in subtle deficits in neuropsychological development. Most studies, however, have reported global measures of development, and the findings have not been consistent. In this report, we examine the association between blood lead concentration and a specific aspect of neuropsychological development, visual-motor integration. Each child in a cohort of 494 children living in and around the lead smelting town of Port Pirie, South Australia, was followed for its first 7 years of life. Serial blood samples were collected at various ages to estimate the lifetime burden of each individual. At the time of each blood sampling, systematic information was obtained on a wide range of other variables relevant to child development. We evaluated visual-motor integration at age 7 with the Beery Developmental Test of Visual-Motor Integration (mean score: 13.4). There was an inverse relation between blood lead concentration and visual-motor performance. After adjustment for potential confounding factors, both prenatal and postnatal blood lead concentrations exhibited a dose-related inverse association with children's visual-motor performance. For an increase in lifetime average blood lead concentration from 10 μag per dl (0.48 μmol per liter) to 30 μg per dl (1.45 μmol per liter), the estimated deficit in children's visual-motor performance was 1.6 points (95 confidence interval 0.3–2.9). The results indicate that visual-motor integration may be a more sensitive index than global measures of development, such as intelligence quotient, for the assessment of lead effects on child development.
