Amaç: Arka bacak iskemisi/reperfüzyonun (İ/R) sıçan modellerinde, rapamisinin memelilerdeki hedefi (mTOR)/inhibitör-κB-α/nükleer faktör-κB p65 sinyal ileti yolu etkinliğinin, artan oksidatif/nitrozatif stres ve inflamatuvar yanıt yoluyla organ zedelenmelerine aracılık ettiğini daha önce göstermiştik. mTOR’un İ/R zedelenmesine katkıda bulunduğuna ilişkin önceki bulgularımızı referans alarak, bu çalışmada arka bacak İ/R'ye bağlı hedef ve uzak organ zedelenmelerinde mTOR ile MEK1/ERK1/2 yolu arasındaki olası etkileşime odaklanmayı amaçladık. Gereç ve Yöntem: Erkek Wistar sıçanlar dört gruba ayrıldı. Arka bacak İ/R, her iki arka ekstremitelerine turnikeler uygulanarak iskemi oluşturuldu. İskemiden 4 saat sonra turnikeler açılarak 4 saat reperfüzyon uygulandı. 4 saatlik reperfüzyondan sonra kan, böbrek ve gastroknemius kası izole edildi.Bulgular: Arka bacak İ/R uygulaması gastroknemius kasında, böbrekte ve/veya serumda rpS6, MEK1, ERK1/2, tümör nekroz faktörü-α, indüklenebilir nitrik oksit sentaz, gp91phox, p22phox ve nitrotirozinin fosforilasyonu ve/veya ekspresyonu ile birlikte nitrit düzeylerinde artışa neden oldu. Ayrıca, İ/R uygulanan sıçanların dokularında nikotinamit adenin dinükleotit fosfat oksidaz ve miyeloperoksidaz seviyeleri arttı. mTOR'un seçici inhibitörü olan rapamisin, sıçanlarda kas ve böbrek dokusunda İ/R'nin neden olduğu yukarıda bahsedilen tüm etkileri ortadan kaldırdı.Sonuç: Bu veriler, MEK1/ERK1/2 yolunun etkinliğinin, mTOR’un aracılık ettiği arka bacak İ/R’nin neden olduğu hedef ve uzak organ zedelenmesine katkıda bulunduğunu göstermektedir., Purpose: We have previously demonstrated that activation of the mammalian target of rapamycin (mTOR)/inhibitory-κB-α/nuclear factor-κB p65 signaling pathway mediates organ injuries through increased oxidative/nitrosative stress and inflammatory response in rat models of hind limb ischemia/reperfusion (HL I/R). Following up our previous findings regarding I/R injury through mammalian target of rapamycin (mTOR), we aimed to focus on the possible interaction between mammalian target of rapamycin (mTOR and mitogen-activated protein kinase kinase (MEK)1/extracellular signal-regulated kinase (ERK) 1/2 pathway in hind limb ischemia/reperfusion (HL I/R) resulting in target and remote organ injuries in the present study. Materials and Methods: Male Wistar rats were divided into four groups. HL I/R was induced by occluding with tourniquets of both hind limbs. Following 4 h, the tourniquets were removed following reperfusion for 4 h. After 4 h of reperfusion blood, kidney, and gastrocnemius muscle were collected.Results: HL I/R caused an increase in phosphorylation and/or expression of rpS6, MEK1, ERK1/2, tumor necrosis factor-α, inducible nitric oxide synthase, gp91phox, p22phox, and nitrotyrosine as well as nitrite levels in gastrocnemius muscle, kidney, and/or serum. Additionally, nicotinamide adenine dinucleotide phosphate oxidase and myeloperoxidase levels were increased in the tissues of rats subjected to HL I/R. Rapamycin, the selective inhibitor of mTOR, abolished all the effects mentioned above caused by HL I/R in the rat’s muscle and kidney. Conclusion: These data suggest that activation of the MEK1/ERK1/2 pathway contributes to mTOR-mediated HL I/R-induced target and remote organ injury.

Purpose: Ramipril is effective in treating inflammatory myocardial injury by reducing cytokines such as TNF-?, IL-6, and IL-8. In this study, we investigated the effect of cytokine gene polymorphisms on inflammatory response which might be reduced by ramipril. Material and Methods: Of 102 patients undergoing on-pump coronary artery bypass grafting surgery, 51 were the treatment group which received ramipril and the remaining 51 were the non-treated control group. TNF-?, IL-6, and IL-8 were measured with Enzyme-Linked Immuno Sorbent Assay (ELISA), before anesthesia induction (t1), at 20 min following cross clamping (t2), at the end of the operation (t3), and 24 hours after anesthesia (t4). Genotyping was performed with PCR method. Results: While TNF-? increase began with surgery, IL-6 and IL-8 increase began with cardiac arrest during cardiopulmonary bypass and continued until the end of the operation. In contrast, only IL-8 remained high in the control group during the postoperative period, while TNF-?, IL-6, and IL-8 began to decrease. The decreases in IL-6 at t3 and in TNF-? at t4 were significant. Conclusion: Ramipril might have a role in preventing inflammatory myocardial injury by reducing cytokine and TnT levels after cardiac arrest., Amaç: Ramipril, inflamatuar miyokardial hasarın tedavisinde TNF-?, IL-6 ve IL-8 gibi sitokin seviyelerini azaltarak etkili olmaktadır. Çalışmamızda, ramipril tedavisinin, koroner arter bypass cerrahisi esnasında oluşan inflamatuar yanıt ve miyokardiyal hasara yönelik etkisi üzerinde, sitokin gen polimorfizmlerinin herhangi bir rolü olup olmadığını araştırdık. Gereç ve Yöntem: Çalışmamıza, koroner arter bypass cerrahisi uygulanan hastalardan cerrahi öncesi ramipril alan 51, kontrol grubu olarak ise ramipril almayan 51 olmak üzere toplam 102 hasta dahil edildi. Her iki grupta, anestezi başlamadan hemen önce (t1), kros klemp sonrası 20. dakikada (t2), ameliyatın sonunda (t3) ve anestezinin başlangıcından 24 saat sonra (t4) alınan kan örneklerinden TNF?, IL-6, IL-8 ve TnT serum düzeyleri ELISA yöntemi ile, genotipleme ise PCR-RFLP yöntemleri ile belirlendi. Bulgular: Her iki grupta TNF-? artışı cerrahi ile başlarken, IL-6 ve IL-8 artışı kardiyopulmoner bypass sırasında kardiyak arrestle başlayıp ameliyat sonuna kadar devam etmiştir. Postoperatif dönemde ise sadece IL-8 kontrol grubunda yüksek kalırken; çalışma grubunda TNF-?, IL-6, IL-8 düşüşe geçmektedir. t3 zaman aralığında IL-6, t4 zamanında TNF? değerlerinde anlamlı azalma gözlenmiştir. Sonuç: Ramiprilin kardiyak arrest sonrası sitokin ve TnT düzeylerini azaltarak inflamatuar miyokardiyal hasarı önlemede yeri olabilir.

Periferik arteriyel hastalık ve koroner arter hastalığı, dünyadaki mortalite ve morbiditenin en sık sebepleri arasında yer almaktadır.Kardiyovasküler hastalıkların tedavisinde, baypas ve endarterektomi gibi cerrahi girişimler, ilaçlar, perkütan vasküler girşimler ve egzersiz uygulamalarını içeren metotlar uygulanmaktadır. Yeterli derecede revaskülarizasyon sağlanamayan hastalarda ise terapotik anjiyogenezis çalışmaları önemli bir seçenek olarak devam etmektedir.Çalışmamızda, terapotik anjiyogenezis oluşturmak amacıyla, arka bacaklarında kronik arteriyel iskemi oluşturulan ratlara, intramusküler lipoteikoik asit (LTA) enjeksiyonu yapılarak anjiyogenezis sürecini değerlendirdik. Herbirinde onar rat bulunan toplam dört grup oluşturuldu. Sham grubuna sadece iskemi modeli oluşturuldu, SF (serum fizyolojik) grubuna iskemi oluşturulduktan sonra SF enjeksiyonu yapıldı, LTA grubuna iskemi oluşturulmasını takiben intramusküler LTA enjekte edildi, kontrol grubuna ise iskemi modeli oluşturulmadı ve herhangi bir işlem uygulanmadı. Ratlar otuz gün boyunca takip edildikten sonra sakrifiye edilerek gastroknemius kasından doku örnekleri alındı, ayrıca 5cc kan alınarak biyokimyasal ve histopatolojik analizler yapıldı. Toplanan kan örneklerinde proanjiyogenik faktörlerden VEGF (vasküler endotel büyüme faktörü) ve Anjiopoietin-1 (Ang-1); anti-anjiyogenik faktörlerden endostatin ve anjiostatinin serumdaki düzeyleri ölçüldü. Alınan kas dokusu örnekleri ışık ve elektron mikroskobisi ile değerlendirildi.Yapılan değerlendirmeler sonucunda, LTA grubunda diğer gruplara göre Ang-1 düzeyi, yeni oluşan kapillerite sayısı, endostatin ve anjiostatin bakımından anlamlı derecede artış tespit edildi (p0,05).Bulgularımız, kronik iskemi modelinde LTA' nın anjiyogenezisi etkin bir biçimde sağladığını, aynı zamanda proanjiyogenik ve anti-anjiyogenik sitokinlerin her ikisinde birden artışa yol açarak anjiyogenezisin kontrollü bir süreçte geliştiğini telkin etmektedir, ancak, farklı LTA dozları ile yapılacak yeni çalışmalara ihtiyaç vardır. Peripheral arterial diseases and coronary arterial diseases constitute the most common reasons in terms of morbidity and mortality all over the world.Surgical methods like bypass and endartherectomies and other methods that consist of medical therapies, percutaneous vascular interventions and specific exercises take part in the treatment of cardiovascular diseases. Studies in medical angiogenesis constitute an important method of therapy in patients in whom adequate revascularization fails to be established.In our study we investigated and evaluated the angiogenesis process in rats in which ischemia was formed in the hind limbs and lipoteichoic acid (LTA) was injected intramuscularly in an effort to induce angiogenesis. Four rat groups were formed with 10 rats in each. The sham group contained rats with ischemia model only, the SF (saline) group contained rats with ischemia and injection of SF intramuscularly and the LTA group contained rats with ischemia and the injection of LTA intramuscularly. The control group rats had no ischemia formed and no injection. After following the rats for a 30 day period they were sacrificed and tissue specimens from their gastrocnemial muscles for histopathological analysis and blood samples of 5 cc. for biochemical analysis were retrieved. Vascular endothelial growth factor (VEGF), angiopoietin1 (Ang-1), endostatin and angiostatin as the proangiogenic and antiangiogenic cytokines were studied from the blood samples. The muscular specimens were evaluated under the light and with electron microscopes.Ang -1 levels, number of newly developed capillaries, endostatin and angiostatin levels were found to be significantly higher (p0,05).Our results suggest that LTA helps in the development of angigenesis effectively and also provides a controlled angiogenesis by stimulating both the proangiogenic and antiangiogenic cytokines simultaneously. New studies with different LTA doses may add more to the results of this study. 52

19. Can Pre-Operative HbA1c Values in Coronary Surgery be a Predictor of Mortality?

- Abstract
Objective: One of the most frequently studied parameters in terms of outcome estimation in cardiac surgery is HbA1c. Several studies in literature suggest that high HbA1c value increases mortality and morbidity, but there is no relation between them. The primary aim of the present study is to investigate whether HbA1c value in diabetic patients undergoing coronary bypass graft surgery is an independent predictor for post-operative mortality and morbidity, and our secondary goal was to determine independent risk factors that cause mortality and morbidity in the same patient population., Methods: 380 diabetic patients diagnosed with diabetes who underwent coronary surgery with cardiopulmonary bypass in Mersin State hospital between July 2014 to December 2016 after the approval of the Mersin University Faculty of medicine ethics committee were included in this retrospective, observational, and cross-sectional study. Patient demographic and perioperative information were obtained from the electronic information operating system and from anesthesia-intensive care follow-up forms. The HbA1c threshold was accepted as 7%, which was reported to be more appropriate for evaluating high-risk groups., Results: Three hundred and fifty-four patients with complete access to the data were included in the study. The mean age of the patients was 60.8±9.4 years. 37% of the patients (131 patients) were female. The number of patients with HbA1c≥7 was 194 (54,8%) in the entire patient population. In this study, high HbA1c (≥7) values in diabetic patients undergoing isolated coronary bypass graft surgery were not found to be independent predictors of post-operative mortality and morbidity. Mortality was seen in 28 patients (7.9%). Ejection fraction (EF) was found to be an independent predictor factor for pre-operative factors in logistic regression models constructed according to mortality predictors (OR:0.94; 95% CI: 0.90-0.99; p=0.016). Complications were seen in 50 patients (14.1%). In the models formed from the point of view of the complication predicators, only EF was found to be independent predictor (OR:0.95; 95% CI: 0.92-0.98; p=0.004). It was found that HbA1c was not predictive in all models for mortality and complication (p>0.05)., Conclusion: There are reports in the literature that mortality increases 4-fold when HbA1c value is higher than 8.6% in coronary surgery. However, there is a view that HbA1c alone cannot predict mortality in coronary surgery if diabetes associated factors are excluded. In this study, high HbA1c (≥7) values in diabetic patients undergoing isolated coronary bypass graft surgery were not found to be independent predictors of post-operative mortality and morbidity.
