Olgu, Cystic fibrosis and alpha 1 antitrypsin deficiency are two important autosomal recessive multi organ diseases They are complex and severe diseases that involve various organs including pancreas Pancreatic disease in cystic fibrosis varies from complete loss of exocrine and endocrine functions to nearly normal pancreatic function A strong correlation between genotype and phenotype is recognized Pancreatic involvement is characterized by malabsorption of fat and protein and growth failure Fat malabsorption leads to special problems with the fat soluble vitamins Enzyme and fat soluble vitamin replacements are the milestones of the malabsorption treatment Gene therapy offers the best hope for a cure of cystic fibrosis Although an association between alpha1 antitrypsin deficiency and chronic pancreatitis has been reported in several case reports in prospective studies it has been shown that pancreatitis prevalence in patients with alpha1 antitrypsin deficiency was not different from that of healthy controls Thus it was concluded that alpha 1 antitrypsin deficiency was not related to the pathogenesis of idiopathic or hereditary chronic pancreatitis However protective role of alpha 1 antitrypsin against pancreatic auto digestion suggests that the deficiency of that protein may worsen the pancreatic disease rather than causing it Early detection and management of pancreatic insufficiency is essential to optimize health and outcomes in cystic fibrosis and alpha 1 antitrypsin deficiency patients Turk Arch Ped 2009; 44: 7 11 Key words: Alfa 1 antitripsin deficinecy cystic fibrosis pancreas

Giriş ve amaç: Triküspit anüler düzlem sistolik hareketi (TAPSE) sağ ventrikül sistolik fonksiyonlarını değerlendirmede kullanılan ekokardiyografik bir ölçümdür. Erişkinlerde normal TAPSE değerleri belirlenmişken çocuklarda referans aralıklarla ilgili yapılan çalışma sayısı yeterli değildir. Bu çalışmada ülkemizdeki sağlıklı çocukların referans TAPSE değerlerinin belirlenmesi amaçlanmıştırYöntem ve gereç: Çalışmada polikliniğimize üfürüm nedeni ile yönlendirilen, yaşları 0-18 arasında değişen 670 sağlıklı çocuk değerlendirildi. Kardiyak problemi olanlar veya başka bir hastalığı olanlar çalışmaya dahil edilmedi. TAPSE ölçümü M-mod ekokardiyografi tekniği kullanılarak ölçüldü. Çocuklarda yaş ve yüzey alanının TAPSE ile ilişkisi araştırıldı.Bulgular: Ortalama TAPSE değerleri 19,96±5,49 bulundu. Kız ve erkek çocuklar arasında TAPSE değerleri açısından anlamlı bir fark saptanmadı. Artan yaş ve yüzey alanı ile beraber TAPSE değerlerinin arttığı tespit edildi. Ortalama TAPSE değeri yenidoğan yaş grubunda 9±1,35 iken 13-18 yaş grubunda 26,27±3,37 bulundu. Çalışmada kalp hızı ile TAPSE arasında ise negatif korelasyon olduğu tespit edildi.Sonuç: Bu çalışmada ülkemizde sağlıklı çocuklarda TAPSE değerlerinin normal aralıkları persantil tablolar oluşturularak sunuldu. Bu referans değerler çocuklarda sağ ventrikül sistolik fonksiyonlarını değerlendirmek için günlük pratikte yol gösterici olabilir.Anahtar kelimeler: TAPSE, referans değerler, sağlıklı çocuklar, sağ ventrikül sistolik fonksiyonu Introduction and objectives: Tricuspid annular plane systolic excursion (TAPSE) is an echocardiographic measure that allows us to assess right ventricular systolic function. Although normal values are available in adults, there are not enough studies for references of TAPSE in children. Our objective was to establish reference values for TAPSE in our country.Methods and materials: This study included 670 healthy children (age range, 0 to 18 years) referred to our out-patient department for assessment of cardiac murmur. Patients with confirmed cardiac or any other disease were excluded. TAPSE was measured using M-mode echocardiography. The correlation between ages and body surface areas in children and TAPSE values were investigated. Results: Mean TAPSE values were 19,96±5,49. There was no significant differences in TAPSE values between female or male children. TAPSE values increased by the increasing age and body surface area. TAPSE ranged from a mean of 9±1,35 in neonates to 26,27±3,37 in 13-18 years age group. There was found to be a negative correlation between TAPSE values and heart rate in the study.Conclusion: Normal values of healthy children for TAPSE in our country were presented as percentile charts in this study. These reference values could guide the clinicians to evaluate the systolic functions of right ventricle in children in daily practice. Giriş: Hastanemiz yenidoğan yoğun bakım ünitesinde izlenen bebekler arasında doğumsal kalp hastalığı tanısı alanları sıklık, risk faktörleri ve prognozları açısından retrospektif olarak incelemek ve bu hastaların mortalitesinin azaltılması için yapılması gerekenleri literatür ışığında değerlendirmek.Gereç ve Yöntem: Ocak 2008-Ocak 2013 tarihleri arasında Uludağ Üniversitesi, yenidoğan yoğun bakım ünitesinde izlenen 1,175 hastadan konjenital kalp hastalığı nedeniyle yatışı yapılan 99 hasta alınmıştır.Bulgular: Çalışmaya alınan 99 olgunun 54’ü erkek %54,5 , 45’i kız %45,5 idi. Olguların %82,9’u term bebeklerden oluşuyordu ve ortalama doğum kiloları 3,142±592 gr bulundu. Siyanotik konjenital kalp hastalıklarından en sık 14 olgu %14,1 ile büyük arter transpozisyonu görülürken 8 hastada Fallot tetralojisi saptandı. Asiyanotik konjenital kalp hastalıklarından en sık total atrio ventriküler AV kanal defekti saptandı. Antenatal tanı alma oranı 55 hasta ile %55,6 idi. Hastaların 35’i %35,3 kaybedildi, ortalama ölüm günü 22. gündü. Kaybedilen hastaların çoğunluğunu büyük arter transpozisyonu ve total AV kanal defekti tanılı olgular oluşturuyordu.Sonuç: Konjenital kalp hastalıklarında mortaliteyi düşürmek için yapılması gereken iki önemli unsur vardır. Birincisi gebelikte annelerin takiplerinin düzenli yapılması ve antenatal tanı alma oranlarının daha da arttırılmasıdır. İkincisi ise sadece konjenital kalp hastalıkları ile ilgilenen bir ekip tarafından kısa sürede opere edilerek yine kardiyak cerrahi post operatif bakım tecrübesi olan bir ekiple izlenmesidir. İdeal olan bölgesel kardiyak cerrahi yoğun bakım merkezleri kurarak, tüm doğan konjenital kalp hastalıklarının doğum sonrası bu merkezlere sevk edilmesi ve konusunda tecrübeli ekip tarafından erken zamanda opere edilerek, bakımının sağlanmasıdır. Ülkemizde konjenital kalp hastalıklarının morbiditesi ve mortalitesinin iyileştirilebilmesi için sağlık bakanlığı tarafından konu ivedilikle ele alınmalı ve sorunun çözümüne yönelik organizasyonlar gerçekleştirilmelidir, Introduction: To evaluate babies diagnosed as congenital heart disease in terms of frequency, risk factors and prognosis and to determine the issues to be covered to reduce the mortality in these patients in the scope of the literature.Materials and Methods: Ninety-nine patients diagnosed as congenital heart disease were enrolled among 1.175 patients who were admitted to neonatal intensive care unit in Uludağ University Hospital between January 2008 and January 2013 Results: Fifty-four patients 54.5% of 99 cases included in the study were male and 45 45.5% cases were female. Of cases, 82.9% were term and mean birth weight was 3.142±592 gr. Most common diagnosis was transposition of great arteries seen in 14 cases 14.1% , followed by complete atrioventricular septal defect and aortic hypoplasia, 11 cases for each. Mortality rate was 35.3% 35 of 99 patients and mean time of death was 22 days. Antenatal diagnosis rate was 55.6% 55 of 99 patients . Conclusions: It appears to be two major issues to address in order to reduce the mortality in congenital heart defects. Firstly, rate of proper follow-up in pregnancies and prenatal diagnosis must be increased. Secondly, these patients must be operated in the exact appropriate time by exclusive congenital heart surgeons, and subsequently managed by experienced post-operative care staff. Therefore, establishment of regional cardiac surgery/intensive care units and referral of all congenital heart defects to these centers would be ideal. In order to reduce the mortality and morbidity of congenital heart defects, these issues must be covered by the Ministry of Health and necessary initiatives must be established as urgently as possible.

Gi rifl: Costello sendromu postnatal büyüme ve gelişme geriliği, kaba yüz, gevşek deri, ilerleyici olmayan kardiyomiyopati ve sempatik kişilik ile karakterize otozomal dominant geçiş gösterdiği düşünülen bir sendromdur. Bu çalışmanın amacı Costello sendromu tanısı konulan ve konjenital kalp hastalığı olan altı olgunun klinik bulgularının değerlendirilmesidir. Gereç ve Yöntem: Çalışmada üfürüm nedeni ile çocuk kardiyoloji polikliniğine 2002-2013 yılları arasında başvuran ve Costello sendromunun fenotipik özellikleri olan 6 olgu değerlendirildi. Olguların yakınmaları, klinik bulguları, tedavi ve klinik gidişi incelendi. Bulgular: Olguların 3’ü kız, 3’ü erkekti ve ortalama yaşları 34±12 ay 7 ay-11 yaş idi. Tüm hastalarda kaba yüz görünümü, el içi ve ayak tabanı çizgilerinde belirginlik, hipertelorizm, epikantus, basık burun kökü ve kardiyak muayenelerinde üfürüm mevcut idi. Hastaların hepsinde konjenital kalp hastalığı saptandı. Hastaların 5’inde değişik derecelerde pulmoner stenoz saptandı ve bu hastaların 3’üne balon pulmoner valvüloplasti yapıldı. 3 hastada atriyal septal defekt tespit edilirken 1 hastada hipertrofik kardiyomiyopati saptandı. 2 hastada mental retardasyon mevcut idi. Sonuç: Psikomotor gelişme geriliği, kaba yüz görünümü, seyrek saçları, derin avuç içi ve ayak tabanı çizgileri olan ve depo hastalığı düşünülen hastalarda özellikle pulmoner stenoz veya hipertrofik kardiyomiyopati varlığında Costello sendromu mutlaka akla gelmelidir, In tro duc ti on: Costello syndrome is probably an autosomal dominant inherited disorder that is characterized by postnatal growth retardation, developmental delay, coarse facies, loose skin, nonprogressive cardiomyopathy and friendly behavior. The aim of this study was to evaluate a variety of clinical findings and cardiac involvement of six patients diagnosed as Costello syndrome. Materials and Methods: Six cases that had a typical features of Costello syndrome admitted to pediatric cardiology department due to cardiac murmur between 2002-2013 years were evaluated. Their complaints, clinical findings, treatments and clinical courses were examined. Results: Three of the patients were male and the mean age of patients was 34±12 months 7 months to 11 years . All of the cases had coarse faces, deep palmar and plantar creases, hypertelorism, epicanthal folds, depressed nasal bridges and cardiac murmur at auscultation. Congenital heart disease was found in all patients. Five patients had pulmonary stenosis and balloon pulmonary valvuloplasty was performed in 3 of these patients. Atrial septal defect was detected in 3 patients and 1 of the patient had hypertrophic cardiomyopathy. Mental retardation was found in 2 patients.Conclusions: Costello syndrome must be keep in mind in all patients with psychomotor retardation, coarse faces, sparse hairs, deep palmar and plantar creases, phenotypical features resembling a storage disease especially in association with pulmonary stenosis and hypertrophic cardiomyopathy

Giriş: Senkop geçiren hastalarda anamnez, fizik muayene ve elektrokardiyografi EKG ile etiyoloji açıklanamıyorsa genellikle Holter monitörizasyonu HM yapılır. Bu çalışmada senkoplu çocuklarda HM’nin tanısal değerinin belirlenmesi amaçlanmıştır.Gereç ve Yöntem: Bu çalışmada 2010-2014 yılları arasında hastalara uygulanan toplam 3,122 HM retrospektif olarak incelendi. Hastaların cinsiyet, tanı yaşı, ayrıntılı anamnez, fizik muayene, 12-derivasyon EKG ve ekokardiyografik sonuçları standart form kullanılarak kayıt edildi.Bulgular: Çalışmaya yaş ortalaması 13,21±3,67 olan 323 hasta dahil edildi. Hastaların 199’u kız, 124’ü erkek idi. Tüm hastaların 284’ünde %87,9 Holter sonuçları normal saptanırken 11 hastada %3,4 senkopu açıklayan anormal Holter sonuçları tespit edildi. Anormal Holter sonuçları olan 11 hastanın üçünde HM öncesinde uygulanan EKG ile tanı konduğu için HM’nin tanısal değeri %2,4 olarak hesaplandı. Aile öyküsü pozitif olan çocuklarda ise HM’nin tanısal değeri %16,6 olarak bulundu. Çalışmada EKG’si normal olan yedi hastada ise HM sonucu ile uzun QT sendromu düşünüldü, Introduction: Holter monitoring HM is usually used in patients with syncope when etiology cannot be explained with history, physical examination and electrocardiography ECG . In this study the objective was to evaluate the diagnostic value of HM in children with syncope.Materials and Methods: Databases were collected retrospectively by analyzing the HM results of 3.122 pediatric patients between 2010-2014. Gender, age at diagnosis, detailed clinical history, physical examination, 12-lead electrocardiographic and echocardiographic results were noted using standardized form.Results: The study included 323 patients with syncope with a mean age of 13.21±3.67. There were 199 female and 124 male patients in this study. Among all patients 284 87.9% had normal HM results, while 11 3.4% patients had abnormal Holter studies that consider to explain as syncope. Three of 11 patients with abnormal Holter results diagnosed through ECG before HM, hence, the diagnostic value of HM was calculated as 2.4%. In contrast, diagnostic value of HM in patients with positive family history was found to be 16.6%. In this study, 7 patients were considered to have long QT syndrome according to their HM findings. Conclusions: Detailed clinical history has a great value in children with syncope. As a result, HM has low diagnostic value if the patients are not in high risk group. However, HM was considered to be important because of concealed long QT syndrome especially if the patient had positive family history and exercise related syncope

Introduction: Patent Ductus Arteriosus PDA , a cardiac pathology commonly seen inpreterm infants, has negative effects on mortality and morbidity. Persistent patency ofPDA is positively correlated with respiratory distress syndrome RDS , prolongedrespiratory support, pulmonary hemorrhage, broncopulmonary dysplasia BPD ,necrotizing enterocolitis NEC , intraventricular hemorrhage, renal failure,neurodevelopmental impairment cerebral palsy , retinopathy of prematurity and death.The standard treatment regimen is to close symptomatic PDA and cyclooxygenaseinhibitors such as indomethacin, ibuprofen are the first choises. Our aim in this study isto report PDA rate, treatment and complications in premature infants. Materials and Methods: This study retrospectively enrolled 103 infants born, Giriş: Patent duktus arteriozus PDA , preterm yenidoğanlarda sıklıkla karşılaşılan,morbidite ve mortalite üzerine olumsuz etkileri olan kardiyak bir patolojidir. PDA’nın açıkkalması ile respiratuvar distres sendromu RDS , uzamış respiratör desteği, pulmonerhemoraji, bronkopulmoner displazi BPD , nekrotizan enterokolit NEK , intraventrikülerkanama İVK , renal yetmezlik, nörogelişimsel kusur serebral palsi , prematureretinopatisi ROP ve ölüm arasında anlamlı ilişki vardır. Semptomatik PDA’larınkapatılması standart hale gelmiş ve ilk tercih indometazin ve ibuprofen İBU gibisiklooksijenaz inhibitorleri olmuştur. Bu çalışmada preterm bebeklerde PDA sıklığı,tedavisi ve komplikasyonlarının araştırılması amaçlanmıştır.Gereç ve Yöntem: Ocak 2010 ile Kasım 2011 tarihleri arasında 33 hafta ve öncesindedoğan, konjenital major anomalisi veya konjenital kalp hastalığı olmayan, ilk hafta içindeekokardiyografisi EKO yapılmış 103 bebek retrospektif olarak değerlendirildi. PDA’ylailişkili pulmoner hemoraji, NEK, BPD, ROP ve ölüm oranları incelendi.Bulgular: Çalışmada 103 bebeğin 45’i erkek ve 58’i kızdı. Yetmiş bebek sezeryan, 33 bebeknormal doğumla doğmuştu. Bebeklerin gestasyonel haftası ortalama 29,7±2,2, PDA’sıaçık grupta gebelik haftası daha düşük olup 28,8±2,3 hafta idi. Bebeklerin ortalama tartısı1323±375 g idi. Aynı şekilde APGAR skoru ortalama 7,25±1,83 olup PDA’sı açık gruptaanlamlı düşüktü 6,7±1,9 . Doğumdan ortalama 4,8±4,4 gün sonra EKO yapıldı ve 103bebeğin 48 %46 ’inde PDA saptandı. PDA’sı açık olan grupta ortalama doğum tartısı1162±351 g, PDA’sı kapalı grupta ise 1465±340 g bulundu. Purpose: We delineate the clinical spectrum and describe the histology in arterial tortuosity syndrome (ATS), a rare connective tissue disorder characterized by tortuosity of the large and medium-sized arteries, caused by mutations in SLC2A10. Methods: We retrospectively characterized 40 novel ATS families (50 patients) and reviewed the 52 previously reported patients. We performed histology and electron microscopy (EM) on skin and vascular biopsies and evaluated TGF-beta signaling with immunohistochemistry for pSMAD2 and CTGF. Results: Stenoses, tortuosity, and aneurysm formation are widespread occurrences. Severe but rare vascular complications include early and aggressive aortic root aneurysms, neonatal intracranial bleeding, ischemic stroke, and gastric perforation. Thus far, no reports unequivocally document vascular dissections or ruptures. Of note, diaphragmatic hernia and infant respiratory distress syndrome (IRDS) are frequently observed. Skin and vascular biopsies show fragmented elastic fibers (EF) and increased collagen deposition. EM of skin EF shows a fragmented elastin core and a peripheral mantle of microfibrils of random directionality. Skin and end-stage diseased vascular tissue do not indicate increased TGF-beta signaling. Conclusion: Our findings warrant attention for IRDS and diaphragmatic hernia, close monitoring of the aortic root early in life, and extensive vascular imaging afterwards. EM on skin biopsies shows disease-specific abnormalities. Background: In this study, we present our five-year extracorporeal membrane oxygenation experiences in patients requiring mechanical support after pediatric open heart surgery. Methods: We retrospectively reviewed the medical records of 29 children (16 males, 13 females; mean age 21.6 months; range 5 days to 162 months) who underwent open heart surgery and required extracorporeal membrane oxygenation support due to postcardiotomy circulatory failure between February 2010 and March 2015. Results: The most common diagnosis was tetralogy of Fallot in eight patients (27.5%). The most common extracorporeal membrane oxygenation indication was failure to wean from cardiopulmonary bypass in 12 (41%) patients. The mean duration of extracorporeal membrane oxygenation support was 6.9 days (range 14 hours to 32 days). The most common complication related to extracorporeal membrane oxygenation support was renal insufficiency in 14 patients (48.3%). Fourteen patients (48%) were able to be successfully weaned from extracorporeal membrane oxygenation support, while six patients (20.7%) were discharged without any neurological sequelae. No significant predictor of mortality was found. Failure to wean from cardiopulmonary bypass resulted improved outcomes than other extracorporeal membrane oxygenation indications. Conclusion: Extracorporeal membrane oxygenation provides an effective cardiopulmonary support for cardiopulmonary failure after pediatric open heart surgery. Careful patient selection, and correct timing and appropriate management of extracorporeal membrane oxygenation are crucial for optimal outcomes.

The Amplatzer septal occluder (ASO) has become the device of choice for interventional closure of atrial septal defects (ASDs) in many institutions during the last decades. Although excellent results have been reported for the device, concerns have arisen about the long-term complications (1). Of these complications, thrombus formation was rarely seen after 1 year in patients (2–4). This is the first report of a pediatric patient of a huge thrombus developing on an ASO device detected by transthoracic echocardiography on a routine examination after 1 year of implantation without a risk factor.

Objective: Tricuspid annular plane systolic excursion (TAPSE) is an echocardiographic measurement used for evaluating right ventricular systolic function. While established reference values of TAPSE exist for the adult population, only a limited number of studies have attempted to evaluate reference values for the pediatric population. The aim of the present study was to determine the reference values for TAPSE in healthy children in Turkey. Methods: A total of 765 healthy children aged between 0 and 18 years, all of whom were referred to our clinic with cardiac murmurs, were evaluated prospectively. Patients with no cardiac pathologies or other disorders were excluded from the study. The measurement of TAPSE was obtained using a 2D-guided M-mode technique with echocardiography, and the relationship between age and surface area with TAPSE was investigated. The statistical analysis was carried out using the SPSS 20.0 software package (SPSS Inc., Chicago, IL, USA, 2012). Results: The mean TAPSE value was found to be 19.56 +/- 5.54 mm, and no significant difference was identified between male and female children. TAPSE values showed a positive correlation with increasing age and surface area. The mean TAPSE value was 9.09 +/- 1.36 mm in newborns and 25.91 +/- 3.60 mm in the 13-18 years age group. A negative correlation was seen between TAPSE and heart rate. Conclusion: In the present study, the reference values for TAPSE in healthy Turkish children were presented in percentile tables and the corresponding z-scores were determined. These reference values may be useful in daily practice for the evaluation of right ventricular systolic function in children.

Introduction: Holter monitoring (HM) is usually used in patients with syncope when etiology cannot be explained with history, physical examination and electrocardiography (ECG). In this study the objective was to evaluate the diagnostic value of HM in children with syncope. Materials and Methods: Databases were collected retrospectively by analyzing the HM results of 3.122 pediatric patients between 2010-2014. Gender, age at diagnosis, detailed clinical history, physical examination, 12-lead electrocardiographic and echocardiographic results were noted using standardized form. Results: The study included 323 patients with syncope with a mean age of 13.21±3.67. There were 199 female and 124 male patients in this study. Among all patients 284 (87.9%) had normal HM results, while 11 (3.4%) patients had abnormal Holter studies that consider to explain as syncope. Three of 11 patients with abnormal Holter results diagnosed through ECG before HM, hence, the diagnostic value of HM was calculated as 2.4%. In contrast, diagnostic value of HM in patients with positive family history was found to be 16.6%. In this study, 7 patients were considered to have long QT syndrome according to their HM findings. Conclusions: Detailed clinical history has a great value in children with syncope. As a result, HM has low diagnostic value if the patients are not in high risk group. However, HM was considered to be important because of concealed long QT syndrome especially if the patient had positive family history and exercise related syncope. Giriş: Senkop geçiren hastalarda anamnez, fizik muayene ve elektrokardiyografi (EKG) ile etiyoloji açıklanamıyorsa genellikle Holter monitörizasyonu (HM) yapılır. Bu çalışmada senkoplu çocuklarda HM’nin tanısal değerinin belirlenmesi amaçlanmıştır. Gereç ve Yöntem: Bu çalışmada 2010-2014 yılları arasında hastalara uygulanan toplam 3,122 HM retrospektif olarak incelendi. Hastaların cinsiyet, tanı yaşı, ayrıntılı anamnez, fizik muayene, 12-derivasyon EKG ve ekokardiyografik sonuçları standart form kullanılarak kayıt edildi. Bulgular: Çalışmaya yaş ortalaması 13,21±3,67 olan 323 hasta dahil edildi. Hastaların 199’u kız, 124’ü erkek idi. Tüm hastaların 284’ünde (%87,9) Holter sonuçları normal saptanırken 11 hastada (%3,4) senkopu açıklayan anormal Holter sonuçları tespit edildi. Anormal Holter sonuçları olan 11 hastanın üçünde HM öncesinde uygulanan EKG ile tanı konduğu için HM’nin tanısal değeri %2,4 olarak hesaplandı. Aile öyküsü pozitif olan çocuklarda ise HM’nin tanısal değeri %16,6 olarak bulundu. Çalışmada EKG’si normal olan yedi hastada ise HM sonucu ile uzun QT sendromu düşünüldü. Sonuç: Senkop geçiren çocuklarda ayrıntılı anamnez çok değerlidir. Sonuç olarak HM’nin, yüksek risk grubu olmayan hastalarda tanısal değeri düşüktür. Ancak özellikle pozitif aile öyküsü olanlarda ve eforla ilişkili senkopu olan çocuklarda gizli uzun QT sendromu açısından HM’nin değerli olabileceği düşünüldü.

Giriş: Hastanemiz yenidoğan yoğun bakım ünitesinde izlenen bebekler arasında doğumsal kalp hastalığı tanısı alanları sıklık, risk faktörleri ve prognozları açısından retrospektif olarak incelemek ve bu hastaların mortalitesinin azaltılması için yapılması gerekenleri literatür ışığında değerlendirmek. Gereç ve Yöntem: Ocak 2008-Ocak 2013 tarihleri arasında Uludağ Üniversitesi, yenidoğan yoğun bakım ünitesinde izlenen 1,175 hastadan konjenital kalp hastalığı nedeniyle yatışı yapılan 99 hasta alınmıştır. Bulgular: Çalışmaya alınan 99 olgunun 54’ü erkek (%54,5), 45’i kız (%45,5) idi. Olguların %82,9’u term bebeklerden oluşuyordu ve ortalama doğum kiloları 3,142±592 gr bulundu. Siyanotik konjenital kalp hastalıklarından en sık 14 olgu (%14,1) ile büyük arter transpozisyonu görülürken 8 hastada Fallot tetralojisi saptandı. Asiyanotik konjenital kalp hastalıklarından en sık total atrio ventriküler (AV) kanal defekti saptandı. Antenatal tanı alma oranı 55 hasta ile %55,6 idi. Hastaların 35’i (%35,3) kaybedildi, ortalama ölüm günü 22. gündü. Kaybedilen hastaların çoğunluğunu büyük arter transpozisyonu ve total AV kanal defekti tanılı olgular oluşturuyordu. Sonuç: Konjenital kalp hastalıklarında mortaliteyi düşürmek için yapılması gereken iki önemli unsur vardır. Birincisi gebelikte annelerin takiplerinin düzenli yapılması ve antenatal tanı alma oranlarının daha da arttırılmasıdır. İkincisi ise sadece konjenital kalp hastalıkları ile ilgilenen bir ekip tarafından kısa sürede opere edilerek yine kardiyak cerrahi post operatif bakım tecrübesi olan bir ekiple izlenmesidir. İdeal olan bölgesel kardiyak cerrahi yoğun bakım merkezleri kurarak, tüm doğan konjenital kalp hastalıklarının doğum sonrası bu merkezlere sevk edilmesi ve konusunda tecrübeli ekip tarafından erken zamanda opere edilerek, bakımının sağlanmasıdır. Ülkemizde konjenital kalp hastalıklarının morbiditesi ve mortalitesinin iyileştirilebilmesi için sağlık bakanlığı tarafından konu ivedilikle ele alınmalı ve sorunun çözümüne yönelik organizasyonlar gerçekleştirilmelidir. Introduction: To evaluate babies diagnosed as congenital heart disease in terms of frequency, risk factors and prognosis and to determine the issues to be covered to reduce the mortality in these patients in the scope of the literature. Materials and Methods: Ninety-nine patients diagnosed as congenital heart disease were enrolled among 1.175 patients who were admitted to neonatal intensive care unit in Uludağ University Hospital between January 2008 and January 2013. Results: Fifty-four patients (54.5%) of 99 cases included in the study were male and 45 (45.5%) cases were female. Of cases, 82.9% were term and mean birth weight was 3.142±592 gr. Most common diagnosis was transposition of great arteries seen in 14 cases (14.1%), followed by complete atrioventricular septal defect and aortic hypoplasia, 11 cases for each. Mortality rate was 35.3% (35 of 99 patients) and mean time of death was 22 days. Antenatal diagnosis rate was 55.6% (55 of 99 patients). Conclusions: It appears to be two major issues to address in order to reduce the mortality in congenital heart defects. Firstly, rate of proper follow-up in pregnancies and prenatal diagnosis must be increased. Secondly, these patients must be operated in the exact appropriate time by exclusive congenital heart surgeons, and subsequently managed by experienced post-operative care staff. Therefore, establishment of regional cardiac surgery/intensive care units and referral of all congenital heart defects to these centers would be ideal. In order to reduce the mortality and morbidity of congenital heart defects, these issues must be covered by the Ministry of Health and necessary initiatives must be established as urgently as possible.

Giriş: Costello sendromu postnatal büyüme ve gelişme geriliği, kaba yüz, gevşek deri, ilerleyici olmayan kardiyomiyopati ve sempatik kişilik ile karakterize otozomal dominant geçiş gösterdiği düşünülen bir sendromdur. Bu çalışmanın amacı Costello sendromu tanısı konulan ve konjenital kalp hastalığı olan altı olgunun klinik bulgularının değerlendirilmesidir. Gereç ve Yöntem: Çalışmada üfürüm nedeni ile çocuk kardiyoloji polikliniğine 2002-2013 yılları arasında başvuran ve Costello sendromunun fenotipik özellikleri olan 6 olgu değerlendirildi. Olguların yakınmaları, klinik bulguları, tedavi ve klinik gidişi incelendi. Bulgular: Olguların 3’ü kız, 3’ü erkekti ve ortalama yaşları 34±12 ay (7 ay-11 yaş) idi. Tüm hastalarda kaba yüz görünümü, el içi ve ayak tabanı çizgilerinde belirginlik, hipertelorizm, epikantus, basık burun kökü ve kardiyak muayenelerinde üfürüm mevcut idi. Hastaların hepsinde konjenital kalp hastalığı saptandı. Hastaların 5’inde değişik derecelerde pulmoner stenoz saptandı ve bu hastaların 3’üne balon pulmoner valvüloplasti yapıldı. 3 hastada atriyal septal defekt tespit edilirken 1 hastada hipertrofik kardiyomiyopati saptandı. 2 hastada mental retardasyon mevcut idi. Sonuç: Psikomotor gelişme geriliği, kaba yüz görünümü, seyrek saçları, derin avuç içi ve ayak tabanı çizgileri olan ve depo hastalığı düşünülen hastalarda özellikle pulmoner stenoz veya hipertrofik kardiyomiyopati varlığında Costello sendromu mutlaka akla gelmelidir. Introduction: Costello syndrome is probably an autosomal dominant inherited disorder that is characterized by postnatal growth retardation, developmental delay, coarse facies, loose skin, nonprogressive cardiomyopathy and friendly behavior. The aim of this study was to evaluate a variety of clinical findings and cardiac involvement of six patients diagnosed as Costello syndrome. Materials and Methods: Six cases that had a typical features of Costello syndrome admitted to pediatric cardiology department due to cardiac murmur between 2002-2013 years were evaluated. Their complaints, clinical findings, treatments and clinical courses were examined. Results: Three of the patients were male and the mean age of patients was 34±12 months (7 months to 11 years). All of the cases had coarse faces, deep palmar and plantar creases, hypertelorism, epicanthal folds, depressed nasal bridges and cardiac murmur at auscultation. Congenital heart disease was found in all patients. Five patients had pulmonary stenosis and balloon pulmonary valvuloplasty was performed in 3 of these patients. Atrial septal defect was detected in 3 patients and 1 of the patient had hypertrophic cardiomyopathy. Mental retardation was found in 2 patients. Conclusions: Costello syndrome must be keep in mind in all patients with psychomotor retardation, coarse faces, sparse hairs, deep palmar and plantar creases, phenotypical features resembling a storage disease especially in association with pulmonary stenosis and hypertrophic cardiomyopathy.

Giriş: Patent duktus arteriozus (PDA), preterm yenidoğanlarda sıklıkla karşılaşılan, morbidite ve mortalite üzerine olumsuz etkileri olan kardiyak bir patolojidir. PDA’nın açık kalması ile respiratuvar distres sendromu (RDS), uzamış respiratör desteği, pulmoner hemoraji, bronkopulmoner displazi (BPD), nekrotizan enterokolit (NEK), intraventriküler kanama (İVK), renal yetmezlik, nörogelişimsel kusur (serebral palsi), premature retinopatisi (ROP) ve ölüm arasında anlamlı ilişki vardır. Semptomatik PDA’ların kapatılması standart hale gelmiş ve ilk tercih indometazin ve ibuprofen (İBU) gibi siklooksijenaz inhibitorleri olmuştur. Bu çalışmada preterm bebeklerde PDA sıklığı, tedavisi ve komplikasyonlarının araştırılması amaçlanmıştır. Gereç ve Yöntem: Ocak 2010 ile Kasım 2011 tarihleri arasında 33 hafta ve öncesinde doğan, konjenital major anomalisi veya konjenital kalp hastalığı olmayan, ilk hafta içinde ekokardiyografisi (EKO) yapılmış 103 bebek retrospektif olarak değerlendirildi. PDA’yla ilişkili pulmoner hemoraji, NEK, BPD, ROP ve ölüm oranları incelendi. Bulgular: Çalışmada 103 bebeğin 45’i erkek ve 58’i kızdı. Yetmiş bebek sezeryan, 33 bebek normal doğumla doğmuştu. Bebeklerin gestasyonel haftası ortalama 29,7±2,2, PDA’sı açık grupta gebelik haftası daha düşük olup 28,8±2,3 hafta idi. Bebeklerin ortalama tartısı 1323±375 g idi. Aynı şekilde APGAR skoru ortalama 7,25±1,83 olup PDA’sı açık grupta anlamlı düşüktü (6,7±1,9). Doğumdan ortalama 4,8±4,4 gün sonra EKO yapıldı ve 103 bebeğin 48 (%46)’inde PDA saptandı. PDA’sı açık olan grupta ortalama doğum tartısı 1162±351 g, PDA’sı kapalı grupta ise 1465±340 g bulundu. PDA’sı açık bebeklerde pulmoner hemoraji, NEK, BPD, ROP ve ölüm oranları PDA’sı kapalı olanlarla karşılaştırıldığında istatistiksel olarak anlamlı ölçüde yüksek bulundu (p
