1. Extent of optico-carotid recess is significantly associated with presence of bony dehiscences and bone thickness in the optico-carotid area

Abstract

Background: The objectives of this study were to evaluate the frequency of bony dehiscences in the optico-carotid recess (OCR) area and to measure the thickness of the bony lamellas bordering the OCR, according to our previously proposed OCR classification taking into account the extent of the recess. Methodology: A total of 100 human cadaver heads (n= 200 sphenoid sinuses) were investigated. Samples were divided into groups according to the presence and extent of OCR (no OCR, sub-optical OCR, latero-optical OCR). Bony dehiscences were visually identified and bone thickness was measured by using a high-resolution micrometer. Results: A bony dehiscence in the OCR area was observed in 20%. A significant difference in bony dehiscence occurrence rate between OCR types was found. The wall thickness of the bony carotid artery- and optic nerve canals bordering the OCR were 0.25 ± 0.16 mm and 0.27 ± 0.15 mm, respectively. Significant differences between OCR groups in bony wall thickness of the carotid artery canal and optic nerve canal were found. Samples with a latero-optical OCR had a significant thinner wall of the carotid artery and optic nerve canal than samples with a sub-optical OCR and no OCR. Conclusions: The current results indicate that the presence of an extended OCR, e.g. latero-optical, is highly associated with a greater risk of bony dehiscences and thinner bony lamellas in the OCR region.

Abstract

Abstract The feasibility and surgical effort of a pre-lacrimal window approach (PLWA) depends on the width of the bony window anterior to the nasolacrimal duct. This study aimed to investigate gender-specific differences in feasibility of PLWA. A consecutive series of paranasal computed tomography scans from 50 females (n = 100) and 50 males (n = 100) were retrospectively analyzed. The primary outcome measure was the antero-posterior length of the bony pre-lacrimal window (BPLWA). The secondary outcome measure was the distribution of Simmen’s PLWA feasibility types (major, moderate and minor surgical effort). Abstract

Pathologies located at the posterior surface of the anterior wall and the floor (alveolar recess) of the maxillary sinus are challen- ging to visualize and access. Open approaches including lateral rhinotomy, Caldwell-Luc procedure or midfacial degloving allow a broad visualization but with a significant likelihood of post-operative morbidity (1). Access to the anterior wall and floor of the maxillary sinus by traditional endoscopic approaches is very difficult . The pre-lacrimal window approach (PLWA), as a modified endoscopic medial maxillectomy, provides a wide access to these challenging areas with a very low morbidity rate in total.

ZusammenfassungDiese DVO Leitlinien, die in erster Linie für Allgemeinmediziner und Spezialisten für Knochenerkrankungen bestimmt sind, sollten von allen im klinischen und ambulanten Bereich tätigen medizinischen Fachkräften angewendet werden. Ziel der Leitlinie ist die Verbesserung der Diagnose, Prävention und Behandlung von Osteoporose und der Folgen der Erkrankung auf der Grundlage evidenzbasierter Medizin.Klare Empfehlungen, welche Patienten zu diagnostizieren und behandeln sind (basierend auf Risikofaktoren [einschließlich sekundärer Osteoporose]) sowie Primär-, Sekundär- oder Tertiärprävention werden dargestellt, mit dem Schwerpunkt auf der postmenopausalen Osteoporose und der Osteoporose bei Männern.Die Identifizierung von Patienten mit einem hohen Risiko für Frakturen wird hervorgehoben, und spezifische Schwellenwerte für die Intervention sind definiert (20 % Hüftfrakturrisiko innerhalb von 10 Jahren diagnostischer Schwellenwert, 30 % Hüftfrakturrisiko innerhalb von 10 Jahren therapeutische Schwelle). Die Diagnose von Osteoporose basiert auf der Anamnese des Patienten, der körperlichen Untersuchung, dem Funktionstest (z. B. Timed Up and Go Test), konventionellen Röntgenaufnahmen der Brust- und Lendenwirbelsäule und der Bestimmung der Knochenmineraldichte (BMD) durch das DXA Verfahren.Die Anamnese ist entscheidend für die Abschätzung des Frakturrisikos auf der Grundlage von 40 wissenschaftlich überprüften Risikofaktoren, die das Frakturrisiko mindestens verdoppeln (z. B. Begleiterkrankungen, Hüftfrakturen in der Familie, prävalente Frakturen an jedem Ort, Lebensstil, Anwendung von Medikamenten, körperliche Aktivität und Stürze). Röntgenaufnahmen der Brust- und Lendenwirbelsäule sind wichtig, um prävalente Wirbelkörperfrakturen zu erkennen. Beim Fehlen eines großen Traumas kann jede Fraktur bei Erwachsenen über dem Alter von 50 Jahren eine Diagnose von Osteoporose nahelegen, mit dem höchsten Risiko für eine nachfolgende Fraktur innerhalb einer kurzen Zeit nach der ersten Fraktur. BMD-Messungen mit DXA sind wichtig, um das individuelle Frakturrisiko besser abschätzen zu können. Eine grundlegende Laboruntersuchung ist obligatorisch, um verschiedene Formen der sekundären Osteoporose ausschließen zu können.Der DVO-Patientenfindungs-Algorithmus basiert auf dem Geschlecht, Alter, Knochenmineraldichte und vorbestehenden Frakturen als wichtigste Informationen. Die Indikation für eine aktive anti-osteoporotische Therapie kann durch multiple Risikofaktoren modifiziert und verfeinert werden. Dieser Algorithmus wurde seit dem Richtlinien-Update 2006 verwendet und wurde entsprechend der internationalen Literatur zu Risikofaktoren für Osteoporose und osteoporotische Frakturen aktualisiert und angepasst.Die Behandlung der Osteoporose enthält viele Therapiepfeiler. Zusammen mit Empfehlungen für Bewegung, Physiotherapie und Sturzprävention sowie Ernährung (z. B. Calcium, Vit. D), werden pharmakologische Behandlungen basierend auf evidenzbasierter Medizin empfohlen. Die aktiven Anti-Osteoporose-Medikamente müssen für die Indikation postmenopausale Osteoporose und männliche Osteoporose in Deutschland, Österreich und der Schweiz zugelassen sein. Das Management und die Vorbeugung von häufigen oder seltenen Nebenwirkungen aufgrund von Anti-Osteoporose-Behandlungen, die in der klinischen Praxis angewendet werden, werden ebenfalls detailliert behandelt.

Abstract

Dysregulation of the hypothalamic-pituitary-adrenocortical axis with deficient glucocorticoid feedback and alterations in the serotonergic system have been identified as biological correlates of mood disorders. Close examination of the interaction between these systems may offer insights into the pathophysiology of anxiety disorders and depression to understand how stress and these disorders are related. In this study, we investigated the relationship between plasma levels of cortisol and the dominant inhibitory serotonergic receptor, serotonin-1A (5-HT1A). Using positron emission tomography (PET) and the radioligand [carbonyl-11C]WAY-100635, we quantified the 5-HT1A receptor binding. Data from 12 male patients with social phobia and 18 matched control subjects were analysed. Seven brain regions were investigated: the anterior and posterior cingulate cortices, hippocampus, amygdala, medial orbitofrontal and retrosplenial cortices, and dorsal raphe nucleus. Partial correlation analysis, controlled for age and radiochemical variables, was performed to demonstrate the association between cortisol plasma levels and 5-HT1A receptor binding. Cortisol plasma levels were significantly lower in patients with social phobia compared to healthy controls. Moreover, we found strong negative correlations between cortisol plasma levels and 5-HT1A binding in the amygdala (r=-0.93, p=0.0004), hippocampus (r=-0.80, p=0.009), and retrosplenial cortex (r=-0.48, p=0.04) in patients with social phobia. Within the former two regions, these associations were significantly higher in patients than in healthy controls. This PET study confirms a negative association between plasma cortisol levels and the 5-HT1A receptor distribution consistent with studies in rodents and non-human primates. Dysregulation of the cortisol level might increase the vulnerability for mood disorders by altering limbic 5-HT1A receptors.

Abstract

Objectives: Various studies indicate that serotonin regulates impulsivity and the inhibitory control of aggression. Aggression is also known to be modified by sex hormones, which exert influence on serotonergic neurotransmission. The present study aimed to elucidate potential interactions between human aggression, the inhibitory serotonergic 5‐HT(1A) receptor, and sex hormones. Experimental Design: Thirty‐three healthy volunteers (16 women, aged 26.24 ± 5.5 yr) completed a validated questionnaire incorporating five dimensions of aggression. Subsequently, all subjects underwent positron emission tomography with the radioligand [carbonyl‐(11)C]WAY‐100635 to quantify 5‐HT(1A) binding potentials (BP(ND)s) in the prefrontal cortex, limbic areas, and midbrain. Also, plasma levels of testosterone, 17ß‐estradiol and sex hormone‐binding globulin (SHBG) were measured. Relations between aggression scores, regional 5‐HT(1A) BP(ND)s, and hormone levels were analyzed using correlations, multivariate analyses of variance, and linear regressions. Principal Observations: Statistical analyses revealed higher 5‐HT(1A) receptor BP(ND)s in subjects exhibiting higher aggression scores in prefrontal (all P < 0.041) and anterior cingulate cortices (P = 0.016). More aggressive subjects were also characterized by lower SHBG levels (P = 0.015). Moreover, higher SHBG levels were associated with lower 5‐HT(1A) BP(ND)s in frontal (P = 0.048) and cingulate cortices (all P < 0.013) and in the amygdala (P = 0.03). Conclusions: The present study provides first‐time evidence for a specific interrelation between the 5‐HT(1A) receptor distribution, sex hormones, and aggression in humans. Our findings point to a reduced down‐stream control due to higher amounts or activities of frontal 5‐HT(1A) receptors in more aggressive subjects, which is presumably modulated by sex hormones. Abstract

A new method of momentum measurement of charged particles through multiple Coulomb scattering (MCS) in the OPERA lead-emulsion target is presented. It is based on precise measurements of track angular deviations carried out thanks to the very high resolution of nuclear emulsions. The algorithm has been tested with Monte Carlo pions. The results are found to describe within the expected uncertainties the data obtained from test beams. We also present a comparison of muon momenta evaluated through MCS in the OPERA lead-emulsion target with those determined by the electronic detectors for neutrino-charged current interaction events. Abstract

The OPERA experiment is based on a hybrid technology combining electronic detectors (EDs) and nuclear emulsions. OPERA collected muon-neutrino interactions during the 2008 and 2009 physics runs of the CNGS neutrino beam, produced at CERN with an energy range of about 5-35 GeV. A total of 5.3 × 1019 protons on target equivalent luminosity have been analysed with the OPERA EDs: scintillator strips target trackers and magnetic muon spectrometers equipped with resistive plate gas chambers and drift tubes, allowing a detailed reconstruction of muon-neutrino interactions. Charged current (CC) and neutral current (NC) interactions are identified, using the measurements in the EDs, and the NC/CC ratio is computed. The momentum distribution and the charge of the muon tracks produced in CC interactions are analysed. Calorimetric measurements of the visible energy are performed for both the CC and NC samples. For CC events, the Bjorken-y distribution and the hadronic shower profile are computed. Abstract

Proton therapy is nowadays becoming a wide spread clinical practice in cancer therapy and sophisticated treatment planning systems are routinely used to exploit at best the ballistic properties of charged particles. The information on the quality of the beams and the range of the protons is a key issue for the optimization of the treatment. For this purpose, proton radiography can be used in proton therapy to obtain direct information on the range of the protons, on the average density of the tissues for treatment planning optimization and to perform imaging with negligible dose to the patient. We propose an innovative method based on nuclear emulsion film detectors for proton radiography, a technique in which images are obtained by measuring the position and the residual range of protons passing through the patient's body. Nuclear emulsion films interleaved with tissue equivalent absorbers can be fruitfully used to reconstruct proton tracks with very high precision. The first prototype of a nuclear emulsion based detector has been conceived, constructed and tested with a therapeutic proton beam at PSI. The scanning of the emulsions has been performed at LHEP in Bern, where a fully automated microscopic scanning technology has been developed for the OPERA experiment on neutrino oscillations. After track reconstruction, the first promising experimental results have been obtained by imaging a simple phantom made of PMMA with a step of 1 cm. A second phantom with five 5 x 5 mm^2 section aluminum rods located at different distances and embedded in a PMMA structure has been also imaged. Abstract

The OPERA detector at the Gran Sasso underground laboratory (LNGS) was used to measure the atmospheric muon charge ratio in the TeV energy region. We analyzed 403069 atmospheric muons corresponding to 113.4 days of livetime during the 2008 CNGS run. We computed separately the muon charge ratio for single and for multiple muon events in order to select different energy regions of the primary cosmic ray spectrum and to test the Rμ dependence on the primary composition. The measured Rμ values were corrected taking into account the charge-misidentification errors. Data have also been grouped in five bins of the "vertical surface energy" ℰμcos θ. Abstract

The NA52 experiment searched for long-lived charged strangelets in 158 A GeV/c Pb+Pb collisions at CERN SPS. We collected 1013 Pb+Pb interactions looking for negatively charged strangelets and 3 × 1011 Pb+Pb interactions for positively charged ones. No evidence for the production of strangelets has been observed. The upper strangelet production limits are discussed. Besides the strangelet searches NA52 was able to identify particles and anti-particles over a wide range in rapidity. Results of the invariant differential particle production cross sections including fragments up to carbon and 5 are presented.

Abstract

Abstract

The OPERA neutrino oscillation experiment is based on the use of the Emulsion Cloud Chamber (ECC). In the OPERA ECC, nuclear emulsion films acting as very high precision tracking detectors are interleaved with lead plates providing a massive target for neutrino interactions. We report on studies related to the effects occurring from the contact between emulsion and lead. A low radioactivity lead is required in order to minimize the number of background tracks in emulsions and to achieve the required performance in the reconstruction of neutrino events. It was observed that adding other chemical elements to the lead, in order to improve the mechanical properties, may significantly increase the level of radioactivity on the emulsions. A detailed study was made in order to choose a lead alloy with good mechanical properties and an appropriate packing technique so as to have a low enough effective radioactivity., Comment: 19 pages, 11 figures

Abstract

The OPERA experiment, designed to conclusively prove the existence of $\rm \nu_\mu \to \nu_\tau$ oscillations in the atmospheric sector, makes use of a massive lead-nuclear emulsion target to observe the appearance of $\rm \nu_\tau$'s in the CNGS $\rm \nu_\mu$ beam. The location and analysis of the neutrino interactions in quasi real-time required the development of fast computer-controlled microscopes able to reconstruct particle tracks with sub-micron precision and high efficiency at a speed of 20 cm^2 / h. This paper describes the performance in particle track reconstruction of the European Scanning System, a novel automatic microscope for the measurement of emulsion films developed for OPERA., Comment: 13 pages, 10 figures

Abstract

We present new results of the CERN experiment NA52 on the centrality dependence of π±, K±, p, d, and production yields near zero transverse momentum and at several rapidities, from 64% to 4% of the total Pb + Pb cross section. Baryon yields increase nearly linearly and K± yields faster than linearly with the number of participating nucleons Np. The antibaryon yields increase less than linearly with Np, indicating absorption. The centrality and rapidity dependence of the π−/π+ ratio indicates Coulomb interaction of the pions with the projectile spectator protons. Within the framework of a coalescence model the radius of the particle source has been estimated from the ratios d/p2 and /2. The source radii are similar for matter and antimatter and are found to increase with ~ Np0.30−0.36.

Abstract

The OPERA neutrino detector at the underground Gran Sasso Laboratory (LNGS) was designed to perform the first detection of neutrino oscillations in appearance mode, through the study of nu_mu to nu_tau oscillations. The apparatus consists of a lead/emulsion-film target complemented by electronic detectors. It is placed in the high-energy, long-baseline CERN to LNGS beam (CNGS) 730 km away from the neutrino source. In August 2006 a first run with CNGS neutrinos was successfully conducted. A first sample of neutrino events was collected, statistically consistent with the integrated beam intensity. After a brief description of the beam and of the various sub-detectors, we report on the achievement of this milestone, presenting the first data and some analysis results., Submitted to the New Journal of Physics

Abstract

The NA52 experiment searches for long-lived massive strange quark matter particles, so-called {ital strangelets}, produced in Pb-Pb collisions at a beam momentum of {ital p}{sub lab}=158 AGeV/{ital c}. Upper limits for the production of strangelets at zero degree production angle covering a mass to charge ratio up to 120 GeV/{ital c}{sup 2} and lifetimes {ital t}{sub lab}{approx_gt}1.2 {mu}s are given. Abstract

We investigate antinuclei production in Pb + Pb interactions at 158 GeV/ c per nucleon at zero degree production angle. We quote invariant differential production cross sections for antiprotons and antideuterons. The corresponding antideuteron to antiproton ratio at midrapidity is 4.2 · 10 −4 . One antihelium-3 nucleus was observed. The results are discussed in the framework of a simple coalescence model.

Abstract

The plasma concentration of vitamins A and E varies with the amount of concurrent lipids and thus requires lipid standardization. The present study compares a new multiple regression-based method for adjusting vitamins A and E concentrations for cholesterol and triglycerides with previous methods (adjustment for cholesterol only, and adjustment for the sum of cholesterol and triglycerides). The results show that the new method can reduce influence of the concurrent lipids.

Abstract

This work is part of an ongoing project to develop a Superheated Superconducting Granule (SSG) detector for cold dark matter and neutrinos. The response of SSG devices to nuclear recoils has been explored irradiating SSG detectors with a 70MeV neutron beam. The aim of the experiment was to test the sensitivity of Sn, Al and Zn SSG detectors to nuclear recoil energies down to a few keV. The detector consisted of a hollow teflon cylinder (0.1cm$^3$ inner volume) filled with tiny superconducting metastable granules embedded in a dielectric medium. The nuclear recoil energies deposited in the SSG were determined measuring the neutron scattering angles with a neutron hodoscope. Coincidences in time between the SSG and the hodoscope signals have been clearly established. In this paper the results of the neutron irradiation experiments at different SSG intrinsic thresholds are discussed and compared to Monte Carlo simulations. The results show that SSG are sensitive to recoil energies down to $\sim$1keV. Abstract

From a neuroimaging point of view, deep brain stimulation (DBS) in psychiatric disorders represents a unique source of information to probe results gained in functional, structural and molecular neuroimaging studies in vivo. However, the implementation has, up to now, been restricted by the heterogeneity of the data reported in DBS studies. The aim of the present study was therefore to provide a comprehensive and standardized database of currently used DBS targets in selected psychiatric disorders (obsessive–compulsive disorder (OCD), treatment-resistant depression (TRD), Gilles de la Tourette syndrome (GTS)) to enable topological comparisons between neuroimaging results and stimulation areas. A systematic literature research was performed and all peer-reviewed publications until the year 2012 were included. Literature research yielded a total of 84 peer-reviewed studies including about 296 psychiatric patients. The individual stimulation data of 37 of these studies meeting the inclusion criteria which included a total of 202 patients (63 OCD, 89 TRD, 50 GTS) was translated into MNI stereotactic space with respect to AC origin in order to identify key targets. The created database can be used to compare DBS target areas in MNI stereotactic coordinates with: 1) activation patterns in functional brain imaging (fMRI, phfMRI, PET, MET, EEG); 2) brain connectivity data (e.g., MR-based DTI/tractography, functional and effective connectivity); 3) quantitative molecular distribution data (e.g., neuroreceptor PET, post-mortem neuroreceptor mapping); 4) structural data (e.g., VBM for neuroplastic changes). Vice versa, the structural, functional and molecular data may provide a rationale to define new DBS targets and adjust/fine-tune currently used targets in DBS based on this overview in stereotactic coordinates. Furthermore, the availability of DBS data in stereotactic space may facilitate the investigation and interpretation of treatment effects and side effect of DBS by comparing these to neuroimaging results. The present study thus improves comparability between functional, structural and molecular data in standard stereotactic space gained in neuroimaging studies with surgical targets for DBS, which is among other possible implications of crucial importance for the definition of new targets for effective DBS.

Abstract

The response of Superheated Superconducting Granule (SSG) devices to nuclear recoils has been explored by irradiating SSG detectors with a 70Me$\!$V neutron beam. In the past we have tested Al SSG and more recently, measurements have been performed with Sn and Zn detectors. The aim of the experiments was to test the sensitivity of SSG detectors to recoil energies down to a few ke$\!$V. In this paper, the preliminary results of the neutron irradiation of a SSG detector made of Sn granules 15-20$\mu$m in diameter will be discussed. For the first time, recoil energy thresholds of $\sim$1ke$\!$V have been measured.

Abstract

The presented results are part of a feasibility study of a Superheated Superconducting Granule (SSG) device for weakly interacting massive particles (WIMPs) detection. The sensitivity of SSG to nuclear recoils has been explored irradiating SSG detectors with a 70MeV neutron beam proving that energy thresholds of ∼1keV can be reached in 30µm Zn and 17µm Sn granules. The successful irradiation experiments with neutrons encouraged us to plan a prototype SSG dark matter detector. The status of the project will be presented and the expected counting rate for spin-independent WIMP interactions in SSG detectors will be discussed.

Abstract

In this paper we describe the technology of building a vacuum-tight high voltage feedthrough which is able to operate at voltages up to 30 kV. The feedthrough has a coaxial structure with a grounded sheath which makes it capable to lead high voltage potentials into cryogenic liquids, without risk of surface discharges in the gas phase above the liquid level. The feedthrough is designed to be used in ionization detectors, based on liquefied noble gases, such as Argon or Xenon.

Abstract

In this paper we report on a detailed study of ionization signals produced by Compton electrons and alpha-particles in a Time Projection Chamber (TPC) flled with different mixtures of liquid Argon and Nitrogen. The measurements were carried out with Nitrogen concentrations up to 15% and a drift electric feld in the range 0-50 kV/cm. A prediction for proton ionization signals is made by means of interpolation. This study has been conducted in view of the possible use of liquid Ar-N2 TPCs for the detection of gamma-rays in the resonant band of the Nitrogen absorption spectrum, a promising technology for security and medical applications.

Abstract

Area-specific and stimulation-dependent changes of human brain activation by selective serotonin reuptake inhibitors (SSRI) are an important issue for improved understanding of treatment mechanisms, given the frequent prescription of these drugs in depression and anxiety disorders. The aim of this neuroimaging study was to investigate differences in BOLD-signal caused by administration of the SSRIs escitalopram and citalopram using pharmacological functional magnetic resonance imaging (pharmaco-fMRI). Eighteen healthy subjects participated in a placebo-controlled, randomized, double-blind study in cross-over repeated measures design. Each volunteer performed facial emotional discrimination and a sensorimotor control paradigm during three scanning sessions. Citalopram (20 mg/d), escitalopram (10 mg/d) and placebo were administered for 10 days each with a drug-free period of at least 21 days. Significant pharmacological effects on BOLD-signal were found in the amygdala, medial frontal gyrus, parahippocampal, fusiform and middle temporal gyri. Post-hoc t-tests revealed decreased BOLD-signal in the right amygdala and left parahippocampal gyrus in both pharmacological conditions, compared to placebo. Escitalopram, compared to citalopram, induced a decrease of BOLD-signal in the medial frontal gyrus and an increase in the right fusiform and left parahippocampal gyri. Drug effects were concentrated in brain regions with dense serotonergic projections. Both escitalopram and citalopram attenuated BOLD-signal in the amygdala and parahippocampal cortex to emotionally significant stimuli compared to control stimuli. We believe that reduced reactivity in the medial frontal gyrus found for escitalopram compared to citalopram administration might explain the response differences between study drugs as demonstrated in previous clinical trials.

Abstract

This paper describes the design, realization and operation of a prototype liquid Argon Time Projection Chamber (LAr TPC) detector dedicated to the development of a novel online monitoring and calibration system exploiting UV laser beams. In particular, the system is intended to measure the lifetime of the primary ionization in LAr, in turn related to the LAr purity level. This technique could be exploited by present and next generation large mass LAr TPCs for which monitoring of the performance and calibration plays an important role. Abstract

This paper reports on the charged pion production yields measured by the SPY/NA56 experiment for 450 GeV/c proton interactions on beryllium targets. The present data cover a secondary momentum range from 7 GeV/c to 135 GeV/c in the forward direction. An experimental accuracy ranging from 5 to 10\%, depending on the beam momentum, has been achieved, limited mainly by the knowledge of the beam acceptance. These results will be relevant in the calculation of neutrino fluxes in present and future neutrino beams.

Abstract

Lateralization is a well described aspect of the human brain. A plethora of morphological, cytological and functional studies describes hemispheric asymmetry in auditory and language areas. However, no study has reported cortical lateralization in the healthy human brain in vivo on the level of neurotransmitter receptors and in relation to functional organization so far. In this study, we assessed the distribution of the main inhibitory serotonergic receptor (the 5-HT1A receptor) and analyzed its regional binding with regard to hemisphere, sex and plasma levels of sex steroid hormones (testosterone, estradiol, progesterone). We quantified the 5-HT1A receptor binding potential by positron emission tomography (PET) using the highly selective and specific radioligand [carbonyl-11C]WAY-100635 and measured hormone levels in thirty-four (16 females, 18 males) healthy right-handed subjects. The obtained data were analyzed in an automated region of interest (ROI) based approach investigating 14 auditory, language and limbic areas. We found significantly higher 5-HT1A receptor binding in the superior and middle frontal gyri of the right hemisphere, the triangular and orbital parts of the inferior frontal gyrus, the supramarginal gyrus, the superior gyrus of the temporal pole and the middle temporal gyrus. Regions of the primary and secondary auditory cortex (Heschl's gyrus and superior temporal gyrus) and the Rolandic operculum displayed significantly higher receptor binding in the left hemisphere. 5-HT1A receptor binding was 1.8-2.9% higher in right frontal ROIs and 2-3.6% higher in left primary and secondary auditory regions. There was no hemispheric difference in 5-HT(1A) receptor binding in the hippocampus, amygdala, and insula. Post-hoc testing suggested that lateralization of 5-HT1A receptor binding differed between the sexes in the triangular part of the inferior frontal gyrus. For the first time, this PET study shows lateralization of the main inhibitory receptor of the serotonergic system in functionally asymmetric organized regions of the healthy human brain in vivo.

Abstract

Material and methods The AMSP study is a drug safety program that ensures the continuous assessment of severe adverse drug reactions (ADR) in psychiatric inpatients under the natural conditions of routine clinical treatment. Furthermore on two reference days per hospital and per year, the following data are recorded for all patients on the wards under AMSP surveillance: all drugs applied on that day with the daily dosage for psychotropic drugs, ICD diagnosis, age, and sex. Data is stored at the study center in Munich. Due to the increasing number of participating clinics over the years only data of 33 hospitals (5 austrian, 22 german and 6 swiss institutions including university and non-university institutions) who participated since 2003 will be presented.

Abstract

There is an increasing interest in the underlying mechanisms of the antidepressant and anxiolytic treatment effect associated with changes in serotonergic neurotransmission after treatment with selective serotonin (5-HT) reuptake inhibitors (SSRIs) in humans. The 5-HT(1A) receptor is known to play a crucial role in the pathophysiology of affective disorders, and altered 5-HT(1A) receptor binding has been found in anxiety patients. SSRI treatment raises the 5-HT level in the synaptic cleft and might change postsynaptic receptor densities. Therefore, our study in patients suffering from anxiety disorders investigated the effects of long-term treatment with escitalopram on the 5-HT(1A) receptor. A longitudinal positrone emission tomography (PET) study in 12 patients suffering from anxiety disorders was conducted. Two dynamic PET scans were performed applying the selective 5-HT(1A) receptor antagonist [carbonyl-(11)C]WAY-100635. Eight regions of interest were defined a priori (orbitofrontal cortex, amygdala, hippocampus, subgenual cortex, anterior and posterior cingulate cortex, dorsal raphe nucleus and cerebellum as reference). After the baseline PET scan, patients were administered escitalopram (average dose of 11.2+/-6.0 mg day(-1)) for a minimum of 12 weeks. A second PET scan was conducted after 109+/-27 days. 5-HT(1A) receptor binding potentials in 12 patients were assessed by PET applying the Simplified Reference Tissue Model.There was a significant reduction in the 5-HT(1A) receptor binding potential after a minimum of 12 weeks of escitalopram treatment in the hippocampus (P=0.006), subgenual cortex (P=0.017) and posterior cingulate cortex (P=0.034). The significance of the hippocampus region survived the Bonferroni-adjusted threshold for multiple comparisons. These PET data in humans in vivo demonstrate a reduction of the 5-HT(1A) binding potential after SSRI treatment.
