It thus became the first-ever targeted therapy to receive accelerated FDA approval for metastatic bladder cancer. In the BLC2001 trial, erdafitinib demonstrated an overall response rate of 40% in patients with urothelial carcinoma. Common adverse events include hyperphosphatemia and retinopathy and require regular monitoring. While the increase in serum phosphate levels has been determined to be a pharmacodynamic marker of response, further interrogation of other clinical, genomic, and transcriptomic biomarkers is warranted. Results of the ongoing Phase III trial, THOR, which is comparing erdafitinib to the standard of care (chemotherapy or immunotherapy), are expected to confer full approval. Establishing guidelines for optimal erdafitinib sequencing with immunotherapy and other approved targeted therapies (enfortumab vedotin and sacituzumab govitecan) remains an unmet need.Keywords: advanced urothelial carcinoma, erdafitinib, fibroblast growth factor receptor, targeted therapy, patient selection

Abstract

Background: Health-care needs change throughout the life course. It is thus crucial to assess whether health systems provide access to quality health care for all ages. Drawing from the Global Burden of Diseases, Injuries, and Risk Factors Study 2019 (GBD 2019), we measured the Healthcare Access and Quality (HAQ) Index overall and for select age groups in 204 locations from 1990 to 2019. Methods: We distinguished the overall HAQ Index (ages 0-74 years) from scores for select age groups: the young (ages 0-14 years), working (ages 15-64 years), and post-working (ages 65-74 years) groups. For GBD 2019, HAQ Index construction methods were updated to use the arithmetic mean of scaled mortality-to-incidence ratios (MIRs) and risk-standardised death rates (RSDRs) for 32 causes of death that should not occur in the presence of timely, quality health care. Across locations and years, MIRs and RSDRs were scaled from 0 (worst) to 100 (best) separately, putting the HAQ Index on a different relative scale for each age group. We estimated absolute convergence for each group on the basis of whether the HAQ Index grew faster in absolute terms between 1990 and 2019 in countries with lower 1990 HAQ Index scores than countries with higher 1990 HAQ Index scores and by Socio-demographic Index (SDI) quintile. SDI is a summary metric of overall development. Findings: Between 1990 and 2019, the HAQ Index increased overall (by 19·6 points, 95% uncertainty interval 17·9-21·3), as well as among the young (22·5, 19·9-24·7), working (17·2, 15·2-19·1), and post-working (15·1, 13·2-17·0) age groups. Large differences in HAQ Index scores were present across SDI levels in 2019, with the overall index ranging from 30·7 (28·6-33·0) on average in low-SDI countries to 83·4 (82·4-84·3) on average in high-SDI countries. Similarly large ranges between low-SDI and high-SDI countries, respectively, were estimated in the HAQ Index for the young (40·4-89·0), working (33·8-82·8), and post-working (30·4-79·1) groups. Absolute convergence in HAQ Index was estimated in the young group only. In contrast, divergence was estimated among the working and post-working groups, driven by slow progress in low-SDI countries. Interpretation: Although major gaps remain across levels of social and economic development, convergence in the young group is an encouraging sign of reduced disparities in health-care access and quality. However, divergence in the working and post-working groups indicates that health-care access and quality is lagging at lower levels of social and economic development. To meet the needs of ageing populations, health systems need to improve health-care access and quality for working-age adults and older populations while continuing to realise gains among the young. Abstract

This paper presents independent multi-instrument observations that address the physical mechanisms of how ultralow-frequency (ULF) wave-associated electric fields initiate ionospheric density fluctuation and scintillation at the equator. Since the magnetic field at the equator is entirely embedded in a relatively high-collision and high-conductivity medium, the condition may not be possible for the geomagnetic field to fluctuate due to ULF wave activity. This implies that the fluctuating electric field at the equator may not be produced through equatorial dynamo action due to fluctuating magnetic fields. Instead, the electric field penetrates from high latitudes and produces fluctuating magnetic field as well as modulates the vertical drift and hence causes the density to fluctuate at the equatorial region. We demonstrate this by estimating the ULF associated fluctuating electric field at high latitudes and at the equatorial region by applying the appropriate attenuation factor as it penetrates to lower latitudes. The periodicity of both electric field and density fluctuations appears to be between 6 and 9 min, which is a typical period of ULF waves in the Pc5 range. Because of its large amplitude and long periods compared to other ULF wave frequency bands, the Pc5 wave-associated electric field, which can even be estimated using magnetograms with low sensitivity and low sampling rate (e.g., 1 min), can easily penetrate to the lower latitude region and produce significant ionospheric density fluctuations that can be strong enough to create scintillation at the equatorial region.

13. Initial minimum viable product development in software startups:a startup ecosystem perspective

Abstract

Context: Software startups are new companies that aim at developing innovative software-intensive products that stand out from those of other companies in the target market. A successful startup tries to scale its business rapidly, which in turn leads to job creation and an increase number in local products. Product development is an essential aspect of a software startup’s business, and therefore it is important to have a suitable startup ecosystem around a startup to support a new product idea from conception to development to a minimum viable product (MVP) and finally to a full-fledged product. Objective: The objective of this doctoral research is to understand the effect of the startup ecosystem elements on an initial MVP development in software startups and thereby assist startup companies in developing successful products. Method: To achieve the objective, two multivocal literature reviews and multiple empirical studies were conducted to examine: a) the elements in a startup ecosystem, b) initial MVP development in software startups, and c) the effects of startup ecosystem elements on the initial MVP development phase. Results: This doctoral research identified eight main elements in the startup ecosystem that affect the startup and its product development. Additionally, it was found that a product idea is based on a problem experienced by customers or on an unserved customer need. The requirements for developing an initial MVP from the product idea usually come from internal sources, and the elicited requirements are stored in text documents and prioritized based on their value to customers and stakeholders. These requirements are converted into product features, of which some can be used to develop a prototype that can act as an initial MVP. Furthermore, it was observed that the startup ecosystem elements can influence the initial MVP development phase. For instance, supporting organizations such as incubators and accelerators affect initial MVPs by assisting inexperienced founders with training and mentoring during MVP development. Conclusion: A prototype can be used as an initial MVP. Also, experienced founders prefer to develop an initial MVP by themselves, while, inexperienced founders need support from supporting organizations in a startup ecosystem during initial MVP development. Tiivistelmä Konteksti: Ohjelmistoalan startup-yritykset ovat uusia yrityksiä, jotka pyrkivät kehittämään innovatiivisia tuotteita monille eri kohdemarkkinoille. Uuden yrityksen menestyksekkään käynnistämisen tavoitteena on nopea kasvu, mikä puolestaan luo työpaikkoja ja nostaa kotimaista tuotetarjontaa markkinoilla. Koska tuotekehitys on tärkeä osa ohjelmiston käyttöönottoa, on tärkeää, että uuden yrityksen ympärillä on olemassa sopiva ekosysteemi, joka tukee uutta tuoteideaa konseptista pienimmän toimivan tuotteen kehittämiseen ja edelleen täyteen tuotteeseen. Tavoite: Tämän tutkimuksen tavoitteena on ymmärtää startup-ekosysteemin roolia ensimmäisen pienimmän toimivan tuotteen kehittämisessä ohjelmistoalan startup-yrityksissä ja sen kautta auttaa startup-yrityksiä menestyvien tuotteiden kehittämisessä. Menetelmät: Tutkimustavoitteen saavuttamiseksi tehtiin kaksi kirjallisuuskatsausta sekä useita tapaustutkimuksia, joissa tarkasteltiin a) startup-ekosysteemin elementtejä, b) ensimmäisen pienimmän toimivan tuotteen kehittämistä ja c) startup-ekosysteemin elementtien vaikutusta ensimmäisen pienimmän toimivan tuotteen kehittämisvaiheessa. Tulokset: Tutkimuksessa tunnistettiin kahdeksan tärkeää elementtiä startup -ekosysteemissä, jotka voivat vaikuttaa suoraan tai välillisesti yrityksen käynnistämiseen ja sen tuotekehitykseen. Lisäksi selvitettiin, että tuoteidea perustuu asiakkaiden kokemiin ongelmiin tai täyttymättömiin tarpeisiin. Pienimmän toimivan tuotteen vaatimukset ovat yleensä peräisin yrityksen sisäisistä lähteistä. Vaatimukset on tallennettu tekstidokumentteihin, ja ne priorisoidaan asiakkaille ja sidosryhmille syntyvän arvon mukaan. Vaatimukset muutetaan tuoteominaisuuksiksi, joista osaa voidaan käyttää, kun kehitetään prototyyppiä ensimmäiseksi pienimmäksi toimivaksi tuotteeksi. Incorporation of existing Advanced Application programs, such as Security Analysis, Dispatcher Power Flow, and Unit Economic Dispatch provides the basis for an automated Import Optimization that effectively helps to extend Security studies performed by Florida Power and Light System Operations.

Abstract

The concept of ‘Minimum Viable Product’ (MVP) is largely adapted in the software industry as well as in academia. Minimum viable products are used to test hypotheses regarding the target audience, save resources from unnecessary development work and guide a company towards a stable business model. As the game industry is becoming an important business domain, it is not surprise that the concept has been adopted also in the game development. This study surveys how a Minimum Viable Game (MVG) is defined, what is reported in extant literature as well as present results from a small case study survey done to nine game development companies. The study shows that despite popularity of minimum viable games in the industrial fora, the presented views on the concept are diverged and there is lack of practical guidelines and research supporting game companies. This study points out research gaps in the area as well as calls for actions to further develop the concept and to define guidelines.

Abstract

Aerial parts of Nepeta hindostana (Roth) Haines collected from Gorakhpur Division, U. P., India possessed an essential oil in 0.42% yield. GC and GC-MS analysis of the oil revealed recognition of thirty three compounds, representing 91.0% (area percent) of the total oil composition. Oil was rich in sesquiterpene hydrocarbons, exhibited higher percentage of (E) β - farnesene (10.4%) followed by ageratochromene (9.7%), spiro [4.5] decan-1-one, 6-hydroxy (9.5%), b -caryophyllene (8.6%) and spiro [4.5] decan-6-ol, 6-methyl (8.2%). At 60 µL, oil showed 80.7% antioxidant activity by β-carotene bioassay (IC 50 = 8 µL) and 73.4% by DPPH free radical scavenging bioassay (IC 50 = 8.5 µL). Furthermore, the oil was tested for its antimicrobial activity in opposition to five plant and human pathogenic bacteria namely Bacillus subtilis, Erwinia herbicola, Escherichia coli, Pseudomonas putida, Salmonella typhi and six post-harvest fungi such as Aspergillus flavus Link., A. niger van Tieghem, A. ochraceus Wilhelm, A. terreus Thom, Fusarium nivale (Fries) Cesati and F. oxysporum Schlecht. The oil was found to be more efficacious for E. coli, Erw. herbicola and A. ochraceus.

Abstract

Although software startups are considered important for economic development due to their ability to quickly create cutting-edge technologies and their potential to scale to a wide market, contextual knowledge about the product idea development process of startups is poorly understood in the literature. This study explores the idea validation process of software startups in an attempt to understand the practices used for idea validation, discover how the process is affected by the founder’s prior competencies, and determine the effect of those practices on requirement gathering. We conducted an exploratory multiple case study in nine software startups to determine the kind of practices they used for idea validation. We identified ten practices used as elements of the idea validation process. Our results show that idea validation is a highly non-linear process in which several validation practices are used in varying combinations and timing. The most frequently used practices included copying existing products, prototyping, utilizing expert support, and cooperating closely with customers. The founder’s prior competencies also influenced the selected practices. Copying and prototyping were common practices when the founders had prior competencies in the application area, while utilizing expert support was a widespread practice to compensate for the founder’s missing competencies. We also observed that the idea validation practices identified in the study serve requirement gathering at different levels of abstraction, varying from business-related requirements down to design-level requirements.

Abstract

Rhinophyma is painless benign swelling due to hypertrophy of the sebaceous gland of the face and nose in particular. In a neglected case, it presented as ‘Proboscis-nose’. As it was hanging in front of nose it was compromising breathing ability of the patient during sleep. It was posing difficulty in placing normal size anstomical face-mask. We placed naso-pharyngeal airway under local anesthesia to retain nasal patency and largest size facemask (size-5) in a manner that it’s air filled cushion was kept deflated and wide mandibular arch was placed nasally to encase the swelling inside mask and then cushion was inflated to make it air tight fit on face. This helped us to get airtight fitting of the anatomical face mask and through nasopharyngeal airway allowed ventilation of the lungs during induction of general anesthesia before intubation. Rest of management and orotracheal intubation was achieved uneventful. We suggest use of nasal and oropharyngeal air way use and reverse uses of large facemask to attain bag-mask ventilation during induction of anesthesia in such cases.

Abstract

Background: Japanese encephalitis (JE) is a major public health concern in Asia including India. Objectives: To evaluate an in-house developed dipstick enzyme-linked immunosorbent assay (ELISA) test vis-à-vis two commercial kits for detection of JE virus-specific IgM antibodies. Setting and Design: Comparative study carried out in Research and Development centre. Materials and Methods: A total of 136 specimens comprising 84 serum and 52 CSF samples were tested by in-house dipstick ELISA, Pan-Bio IgM capture ELISA (Pan-Bio, Australia) and JEV CheX IgM capture ELISA (XCyton, India). Results: The overall agreement among all three tests was found to be 92% with both serum and cerebrospinal fluid (CSF) samples. The sensitivity of the dipstick ELISA was found to be 91% with serum and 89% with CSF samples respectively. The specificity of the dipstick ELISA with reference to both commercial assays was found to be 100% in serum and CSF samples in this study. Conclusions: The in-house dipstick ELISA with its comparable sensitivity and specificity can be used as a promising test in field conditions since it is simple, rapid and requires no specialized equipment.
