1. Dispersion of benthic flumes in deep-dea mining: What lessons can be learned from dredging?

Abstract

Over the past decade, deep-sea mining (DSM) has received renewed interest due to scarcity of raw materials. Deep-sea mining has been spurred by the need for critical resources to support growing populations, urbanization, high-tech applications and the development of a green energy economy. Nevertheless, an improved understanding of how mining activities will affect the deep-sea environment is required to obtain more accurate assessment of the potential environmental impact. In that regard, the sediment plumes that are generated by the mining activity have received the highest concern, as these plumes might travel for several kilometers distance from the mining activity. Various plume sources are identified, of which the most profound are those generated by the excavation and collection process of the seafloor mining tool and the discharge flow to be released from the surface operation vessel after initial dewatering of the ore. In this review, we explore the physical processes that govern plume dispersion phenomena (focusing in the main on benthic plumes), discuss the state of the art in plume dispersion analysis and highlight what lessons can be learned from shallow water applications, such as dredging, to better predict and reduce the spread and impact of deep-sea mining plumes.

Abstract

A reliable system for grading operative difficulty of laparoscopic cholecystectomy would standardise description of findings and reporting of outcomes. The aim of this study was to validate a difficulty grading system (Nassar scale), testing its applicability and consistency in two large prospective datasets.Patient and disease-related variables and 30-day outcomes were identified in two prospective cholecystectomy databases: the multi-centre prospective cohort of 8820 patients from the recent CholeS Study and the single-surgeon series containing 4089 patients. Operative data and patient outcomes were correlated with Nassar operative difficultly scale, using Kendall's tau for dichotomous variables, or Jonckheere-Terpstra tests for continuous variables. A ROC curve analysis was performed, to quantify the predictive accuracy of the scale for each outcome, with continuous outcomes dichotomised, prior to analysis.A higher operative difficulty grade was consistently associated with worse outcomes for the patients in both the reference and CholeS cohorts. The median length of stay increased from 0 to 4 days, and the 30-day complication rate from 7.6 to 24.4% as the difficulty grade increased from 1 to 4/5 (both p 0.001). In the CholeS cohort, a higher difficulty grade was found to be most strongly associated with conversion to open and 30-day mortality (AUROC = 0.903, 0.822, respectively). On multivariable analysis, the Nassar operative difficultly scale was found to be a significant independent predictor of operative duration, conversion to open surgery, 30-day complications and 30-day reintervention (all p 0.001).We have shown that an operative difficulty scale can standardise the description of operative findings by multiple grades of surgeons to facilitate audit, training assessment and research. It provides a tool for reporting operative findings, disease severity and technical difficulty and can be utilised in future research to reliably compare outcomes according to case mix and intra-operative difficulty.

3. Distribution of and hydrographic controls on ferromanganese crusts: Tropic Seamount, Atlantic

Abstract

Hydrogenetic ferromanganese crusts are likely to be exploited as resources for critical metals in the near future, yet the processes controlling where and how they grow are poorly understood. Using detailed mapping of seafloor outcrop and well constrained hydrographic modelling alongside scanning electron microscope imagery of samples from the Tropic Seamount, a star-shaped guyot located in the Tropical East Atlantic, we investigate the relationship between currents, ferromanganese crustal texture and the locations and intensity of crustal erosion. Here, we report the distribution of FeMn crusts and explore factors controlling their growth and erosion. We find that just over 35% of the summit plateau of the guyot exposes some form of ferromanganese crust mineralisation, with the rest variably covered by plains of mobile sediment and slim cliff exposures of carbonate. The steep flanks of the guyot largely expose ferromanganese crust both in situ and as debris flows. The strongest currents are located on the upper flanks of the guyot, the central part of its eastern limb, and across the summit plateau. Three categories of surface morphologies are identified; from pristine botryoidal surfaces to flat areas that have been completely polished by the erosive action of currents and sediment. The relationship between the outcrop of crusts, their erosional states and the hydrographic current regime to which they are exposed is complicated. There is a general correlation between the degree of erosion and location across the seamount, with the least eroded being found on the flanks below 2000 m water depth and the most heavily eroded crusts largely restricted to the summit area. Furthermore, the pristine samples all reside in areas that rarely experience current magnitudes over 0.2 m/s, suggesting that above this the currents have the ability to erode ferromanganese crust. Abstract

Background The ability to accurately predict operative duration has the potential to optimise theatre efficiency and utilisation, thus reducing costs and increasing staff and patient satisfaction. With laparoscopic cholecystectomy being one of the most commonly performed procedures worldwide, a tool to predict operative duration could be extremely beneficial to healthcare organisations. Methods Data collected from the CholeS study on patients undergoing cholecystectomy in UK and Irish hospitals between 04/2014 and 05/2014 were used to study operative duration. A multivariable binary logistic regression model was produced in order to identify significant independent predictors of long (> 90 min) operations. The resulting model was converted to a risk score, which was subsequently validated on second cohort of patients using ROC curves. Results After exclusions, data were available for 7227 patients in the derivation (CholeS) cohort. The median operative duration was 60 min (interquartile range 45–85), with 17.7% of operations lasting longer than 90 min. Ten factors were found to be significant independent predictors of operative durations > 90 min, including ASA, age, previous surgical admissions, BMI, gallbladder wall thickness and CBD diameter. A risk score was then produced from these factors, and applied to a cohort of 2405 patients from a tertiary centre for external validation. This returned an area under the ROC curve of 0.708 (SE = 0.013, p 90 min increasing more than eightfold from 5.1 to 41.8% in the extremes of the score. Conclusion The scoring tool produced in this study was found to be significantly predictive of long operative durations on validation in an external cohort. As such, the tool may have the potential to enable organisations to better organise theatre lists and deliver greater efficiencies in care.

Abstract

Background The aim was to describe the management of benign gallbladder disease and identify characteristics associated with all-cause 30-day readmissions and complications in a prospective population-based cohort. Methods Data were collected on consecutive patients undergoing cholecystectomy in acute UK and Irish hospitals between 1 March and 1 May 2014. Potential explanatory variables influencing all-cause 30-day readmissions and complications were analysed by means of multilevel, multivariable logistic regression modelling using a two-level hierarchical structure with patients (level 1) nested within hospitals (level 2). Results Data were collected on 8909 patients undergoing cholecystectomy from 167 hospitals. Some 1451 cholecystectomies (16·3 per cent) were performed as an emergency, 4165 (46·8 per cent) as elective operations, and 3293 patients (37·0 per cent) had had at least one previous emergency admission, but had surgery on a delayed basis. The readmission and complication rates at 30 days were 7·1 per cent (633 of 8909) and 10·8 per cent (962 of 8909) respectively. Both readmissions and complications were independently associated with increasing ASA fitness grade, duration of surgery, and increasing numbers of emergency admissions with gallbladder disease before cholecystectomy. No identifiable hospital characteristics were linked to readmissions and complications. Conclusion Readmissions and complications following cholecystectomy are common and associated with patient and disease characteristics.

Abstract

Background The aims of this prospective population-based cohort study were to identify the patient and hospital characteristics associated with emergency cholecystectomy, and the influences of these in determining variations between hospitals. Methods Data were collected for consecutive patients undergoing cholecystectomy in acute UK and Irish hospitals between 1 March and 1 May 2014. Potential explanatory variables influencing the performance of emergency cholecystectomy were analysed by means of multilevel, multivariable logistic regression modelling using a two-level hierarchical structure with patients (level 1) nested within hospitals (level 2). Results Data were collected on 4744 cholecystectomies from 165 hospitals. Increasing age, lower ASA fitness grade, biliary colic, the need for further imaging (magnetic retrograde cholangiopancreatography), endoscopic interventions (endoscopic retrograde cholangiopancreatography) and admission to a non-biliary centre significantly reduced the likelihood of an emergency cholecystectomy being performed. The multilevel model was used to calculate the probability of receiving an emergency cholecystectomy for a woman aged 40 years or over with an ASA grade of I or II and a BMI of at least 25·0 kg/m2, who presented with acute cholecystitis with an ultrasound scan showing a thick-walled gallbladder and a normal common bile duct. The mean predicted probability of receiving an emergency cholecystectomy was 0·52 (95 per cent c.i. 0·45 to 0·57). The predicted probabilities ranged from 0·02 to 0·95 across the 165 hospitals, demonstrating significant variation between hospitals. Conclusion Patients with similar characteristics presenting to different hospitals with acute gallbladder pathology do not receive comparable care.

Abstract

The accuracy of process-based models decreases through the sequence water levels and currents to sediment transports and in turn to evolving morphology. Especially, the validity of longer-term (decadal) simulations is uncertain. The aim here is to develop, apply and compare models capable of indicating likely estuarine morphologies 50 years hence. From these should come estimates of associated changes in flood risks under various management and climate change scenarios. “Bottom-Up” process-based models, combined with Lagrangian particle-tracking, have been considered for accuracy and sensitivities to formulation and forcing conditions. A SHELL Application Framework aims to facilitate “Hybrid” coupling of 1-D hydrodynamic models and top-down (T-D) “regime” and physical constraints on estuary form. An ASMITA-type model (Stive et al. 1998) models sediment inputs and transports between aggregated estuarine elements (flats, channels, delta) to predict adaptation capacity to sea-level rise. An 'Inverse' hybrid model uses a diffusion-type T-D model equation to retrieve time-averaged “forcing” of recent morphological change as a basis for prediction. An Analytical Emulator is based on 1-D dynamical equations with simplifying assumptions. The various models have been applied to eight estuaries to give an 'ensemble' of predictions for 2050 morphologies for intercomparison.

Abstract

A viral agent was isolated from the fetal liver of an aborted equine fetus. The isolate hemagglutinated red blood cells from guinea pig, rhesus monkey and rooster. By hemagglutination inhibition tests, the isolate was shown to be antigenically distinct from parvoviruses of bovine and canine origin. Specific hemagglutination inhibiting antibody against the viral isolate was exhibited by 26 of 136 horse sera tested. The isolated virus showed properties compatible with those of an autonomous parvovirus including size, morphology, stability to ether treatment and heating to 56 degrees C, the presence of a 5300 base DNA genome, characteristic protein composition and density (1.405 g/mL). The virus was classified as an equine parvovirus.

16. Measurement and modelling of deep sea sediment plumes and implications for deep sea mining.

Abstract

Deep sea mining concerns the extraction of poly-metallic nodules, cobalt-rich crusts and sulphide deposits from the ocean floor. The exploitation of these resources will result in adverse ecological effects arising from the direct removal of the substrate and, potentially, from the formation of sediment plumes that could result in deposition of fine sediment on sensitive species or entrainment of sediment, chemicals and nutrients into over-lying waters. Hence, identifying the behaviour of deep-sea sediment plumes is important in designing mining operations that are ecologically acceptable. Here, we present the results of novel in situ deep sea plume experiments undertaken on the Tropic seamount, 300 nautical miles SSW of the Canary Islands. These plume experiments were accompanied by hydrographic and oceanographic field surveys and supported by detailed numerical modelling and high resolution video settling velocity measurements of the in situ sediment undertaken in the laboratory. The plume experiments involved the controlled formation of benthic sediment plumes and measurement of the plume sediment concentration at a specially designed lander placed at set distances from the plume origin. The experiments were used as the basis for validation of a numerical dispersion model, which was then used to predict the dispersion of plumes generated by full-scale mining. The results highlight that the extent of dispersion of benthic sediment plumes, resulting from mining operations, is significantly reduced by the effects of flocculation, background turbidity and internal tides. These considerations must be taken into account when evaluating the impact and extent of benthic sediment plumes.

Abstract

Introduction: The hazards of surgical smoke are well documented and electrosurgical units (ESUs) are an integral part of surgical practice. The aim of this study was to gauge the opinions of general surgical consultants, specialist registrars and senior theatre nurses in the Wessex Region towards the hazards of ESU smoke., Materials and Methods: A literature search was carried out using Ovid Medline. A questionnaire was sent to 169 consultants, SpRs and nurses in the 14 hospitals across the Wessex Region, exploring current practices, perceived hazards and whether adequate precautions were currently in use., Results: Only 3 of 98 surgeons used dedicated smoke extractors, despite the fact the majority (72%) felt that, currently, inadequate precautions were taken to protect staff and patients from surgical smoke. There was also uncertainty about the hazards amongst the respondents., Conclusions: The use of smoke extraction equipment is very limited. Greater awareness of the hazards and available technology to extract fumes from the theatre environment might lead to greater uptake.

Abstract

A diagnosis of Yersinia pseudotuberculosis in a cat was established by microbiological and histological means. Clinical signs were general malaise, anorexia and vomition. Significant laboratory findings included a serum glutamicpyruvic transaminase elevation and the presence of urine bilirubin and urobilinogen. Histological examination of liver biopsies revealed numerous varying-sized foci of caseation necrosis containing large numbers of Gram-negative coccobacilli. Bacteriological techniques, treatment and control measures are discussed.

Abstract

In 1975 an outbreak of group A meningococcal disease began in Seattle, Washington, and cases subsequently were recognized throughout the Pacific Northwest. Nearly one-half of the affected persons were Native Americans; two-thirds were alcohol abusers and/or habitués of skid road communities. In Seattle, group A meningococci colonized asymptomatic persons only if these individuals had contact with skid road (P = .006). The epidemic strain may have spread from American Indians in Manitoba, Canada. Traditional migration routes connect the two populations; asymptomatic American Indians on reservations in Washington carried group A meningococci. Vaccination programs were undertaken in four cities but only after cases occurred. In Seattle, vaccination reached 80% of the target population and was associated with a significant decrease in incidence of the disease, but cases recurred after the program ended. The social habits of skid road communities, combined with the "case-triggering" approach to, and premature termination of, vaccination programs, may have resulted in 56% of regional cases occurring after the start of the vaccination program in Seattle.

Abstract

From July 1972 to December 1975, an unusual outbreak of diphtheria in Seattle, Wash., resulted in a total of 558 cases and carriers, mostly among heavy alcohol users. Skin infections were predominant. Four white men died. The highest attack rate was among native American Indians. Environmental contamination and poor personal hygience were believed to be important in continuation of the epidemic, but could not be proved. Control measures included casefinding, isolation and quarantine, sanitizing dwelling units and mass immunization with Td toxoid. The high-risk geographic area was the city's Skid Road. This area continues to be the reservoir of continuing infection, but not all population subgroups there have been at equal risk. Spread to other geographic areas of the city and county has been minimal and remains under control.
