Background/Aims: Intradialytic hypotension (IDH) is a serious and frequent complication of hemodialysis (HD). Thus far, data are scarcely available to assess the impact of first versus subsequent HD sessions of the week in IDH. Therefore, the purpose of this work was to evaluate IDH risk in patients on thrice-weekly HD. Methods: We conducted an analysis of all blood pressure (BP) measurements obtained during 492 HD treatments given to 41 prevalent adult patients over a one month period. A logistic regression model for repeated binary observations was used to determine the association between hypotension and patient and dialysis factors. Results: The incidence of IDH was 32.5%. First dialysis session of the week was associated with a 9% higher risk of hypotension relatively to the second one. The risk was even higher from the first to the third session of the week (60%) and from the second to the third (50%). A higher hypotension odds ratio was also associated with age (1.03, 90%CI: 1.01-1.06), higher predialysis BP (1.04, 90%CI: 1.03-1.05) and higher phosphorus level (1.38, 90%CI: 1.07-1.76). The risk decreased 24.4% for each additional antihypertensive drug taken by the patient. Conclusions: The odds of hypotension occurrence decrease throughout dialysis sessions of the week. Background: The arteriovenous fistula (AVF) is the preferred vascular access for hemodialysis. Four different cannulation techniques can be used: Rope Ladder (RL), Buttonhole (BH), Multiple Single cannulation Technique (MuST), or Area Puncture (AP). Objective: To build and validate a decision-making instrument for the optimal AVF cannulation technique in hemodialysis. Methodology: This two-stage methodological study using the Delphi method was conducted between October 2021 and February 2022 with 27 experts. The decision-making instrument for choosing the cannulation technique was divided into four blocks: physical assessment, ultrasound assessment, vascular access graphical representation, and observations. The content validity index = 0.90 was used in the analysis. Results: The instrument’s structure obtained the unanimous agreement of the experts and an overall content validity index of 0.94. Conclusion: The instrument under analysis proved to have the face and content validity for nursing consultations., Marco contextual: La fístula arteriovenosa se considera el acceso vascular de preferencia para la hemo diálisis y puede realizarse mediante cuatro técnicas de canulación diferentes, escalera, ojal, MuST o área. Objetivo: Construir y validar una herramienta de apoyo a la decisión para la técnica ideal de canulación de la fístula arteriovenosa en hemodiálisis. Metodología: Estudio metodológico en dos etapas mediante la técnica Delphi entre octubre de 2021 y febrero de 2022 con 27 jueces expertos. El instrumento de apoyo al modelo de decisión para la canulación se segmentó en cuatro bloques: evaluación física, evaluación ecográfica, esquema fotográfico con dermopigmentación y observaciones. Para el análisis, se utilizó el índice de validez de contenido = 0,90. Resultados: El consenso final de los jueces se obtuvo mediante un acuerdo unánime sobre la estructura del instrumento y un índice de validez de contenido global de 0,94. Conclusión: El instrumento estudiado demostró ser válido en apariencia y contenido para aplicarlo en la consulta de enfermería., Enquadramento: A fístula arteriovenosa é considerada como o acesso vascular de eleição para hemodiálise, a sua utilização pode ser realizada através de quatro técnicas de canulação distintas: Escada, Botoeira, MuST ou Área. Objetivo: Construir e validar um instrumento de apoio à decisão para a técnica de canulação ideal da fistula arteriovenosa em hemodiálise. Metodologia: Estudo metodológico em duas etapas mediante a realização da técnica de Delphi entre outubro 2021 e fevereiro de 2022 através de 27 juízes peritos. O instrumento de apoio ao modelode decisão para a canulação foi segmentado em quatro blocos: avaliação física, avaliação ecográfica esquema fotográfico com dermopigmentação e observações. Para análise, utilizou-se o índice de validade de conteúdo = 0,90. Resultados: Obteve-se consenso final de juízes através de uma concordância unânime na estrutura do instrumento e um índice de validade de conteúdo global de 0,94. Conclusão: O instrumento em estudo revelou-se válido em aparência e conteúdo para aplicação em consulta de enfermagem.

Objective: Analyze the level of stress and preliminary signs of Burnout Syndrome in nurses working in Intensive Care Units (ICUs) of COVID-19 and, in other ICUs, in the context of the pandemic. Methodology: Cross-sectional, descriptive study with a quantitative analytical approach, carried out from February to May 2021, with nurses from the ICUs of a Teaching Hospital in the interior of the state of São Paulo. We applied a structured form through Google Forms, associated with the Health Professional Stress Questionnaire from Gomes & Teixeira and the JBEILI Questionnaire, inspired by the Maslach Burnout Inventory – MBI (Chafic Jbeili). For statistical analyses, the significance level of p ≤ 0.050 was adopted, and correlation analysis, normality test, Wilcoxon statistical tests, Mann-Whitney U, and Spearman correlation tests were performed. Results: The study population consisted of 61 nurses. Of these, 38 work in the COVID-19 ICUs and 23 in the other ICUs. There were no statistically significant differences regarding the level of stress and the preliminary analysis of Burnout between the two groups. As for the level of global stress identified in the QSPS, the classification of high stress (34%) predominated, followed by high stress (24%) and, concerning the preliminary analysis of Burnout, 82% of the total population presented the Burnout Syndrome, predominating the classification "Initial Stage of Burnout." Conclusion: The results score global stress levels from “quite” to “high” and predominance of the initial and installation phase of Burnout syndrome in nurses from COVID-19 ICUs and other ICUs. The results point to global stress levels ranging from “quite” to “high” stress, with Burnout predominating in nurses in the COVID-19 ICUs and other ICUs in the initial phase and beginning of the syndrome's onset. Objetivo: Analizar el nivel de estrés y signos preliminares del Síndrome de Burnout en enfermeros que laboran en las Unidades de Cuidados Intensivos (UCI) del COVID-19 y, en otras UCI, en el contexto de la pandemia. Metodología: Estudio descriptivo transversal con abordaje analítico cuantitativo, realizado de febrero a mayo de 2021, con enfermeros de las UCI de un Hospital Docente del interior del estado de São Paulo. Se aplicó un formulario estructurado através de Google Forms, asociado al Health Professional Stress Questionnaire de Gomes & Teixeira y al JBEILI Questionnaire, inspirado en el Maslach Burnout Inventory - MBI (Chafic Jbeili). Para los análisis estadísticos se adoptó el nivel de significancia de p ≤ 0,050 y se realizaron análisis de correlación, prueba de normalidad, pruebas estadísticas de Wilcoxon, U de Mann-Whitney y pruebas de correlación de Spearman. Resultados: La población de estudio estuvo conformada por 61 enfermeras. De estos, 38 trabajan en las UCI COVID-19 y 23 en las otras UCI. No hubo diferencias estadísticamente significativas en cuanto al nivel de estrés y el análisis preliminar de Burnout entre los dos grupos. En cuanto al nivel de estrés global identificado en el QSPS, predominó la clasificación de estrés alto (34%), seguido de estrés alto (24%) y, en cuanto al análisis preliminar de Burnout, el 82% de la población total presentó el Síndrome de Burnout, predominando la clasificación "Etapa inicial de Burnout". Conclusión: Los resultados puntúan los niveles de estrés global de “bastante” a “alto” y predominio de la fase inicial y de instalación del síndrome de Burnout en enfermeros de UCI COVID-19 y otras UTI. Conclusion: Los resultados apuntan a niveles de estrés global que van desde “bastante” a “alto” estrés, con predominio del Burnout en enfermeras de las UCI COVID-19 y otras UCI en la fase inicial y comienzo del inicio del síndrome. Objetivo: Analisar o nível de estresse e sinais preliminares da Síndrome de Burnout nos enfermeiros que trabalham nas Unidades de Terapia Intensivas (UTIs) da COVID-19 e, nas demais UTIs, no contexto da pandemia. Metodologia: Estudo transversal, descritivo com abordagem quantitativo do tipo analítico, de um contexto específico, executado de fevereiro a maio de 2021, com enfermeiros das UTIs, de um Hospital de Ensino, do interior do estado de São Paulo. Foi aplicado um formulário estruturado por meio do Google Forms, associados aos Questionário de Stress nos Profissionais de Saúde de Gomes & Teixeira e Questionário JBEILI, inspirado no Maslach Burnout Inventory – MBI (Chafic Jbeili). Para as análises estatísticas, foi adotado o nível de significância p ≤ 0,050, sendo realizadas análises de correlação, teste de normalidade, testes estatísticos Wilcoxon, U de Mann-Whitney e Correlação de Spearman. Resultados: A população do estudo foi composta por 61 enfermeiros. Destes, 38 atuam nas UTIs COVID-19 e 23, nas demais UTIs. Não houve diferença estatística significativa, em relação ao nível de estresse e a análise preliminar de Burnout entre os dois grupos. Quanto ao nível de estresse global identificados no QSPS, a classificação de “bastante estresse” foi predominante (34%), seguido de elevado estresse (24%). Na análise preliminar de Burnout, 82% da população total apresentou a Síndrome de Burnout, sendo a classificação “Fase inicial da Burnout” a mais pronunciada. - Abstract
Objective: To perform a cross-cultural adaptation and content validation of the Kidney Transplant Understanding Tool for the Brazilian context. Methods: A methodological study conducted in the city of Pernambuco, Brazil, which performed the five steps of a Canadian cross-cultural adaptation protocol. The pre-final version of the instrument was evaluated by 36 participants in renal replacement therapy and seven specialists in nephrology Results: For each item, the Item Content Validity Index - I-CVI ≥ 0.85 was achieved; and 0.99 for the mean Scale Content Validity Index - S-CVI/Ave in conceptual and content equivalence. The binomial test showed a p-value ≥ 0.05 for all items; and the Kappa Coefficient of Agreement was 0.9. Conclusion: The adapted instrument was found to be clear by the participants, and the content was validated by the experts. The Kidney Transplant Understanding Tool (K-TUT-Br) was validated for use in the context of kidney transplantation in Brazil. Its application may enable nurses to implement of strategic changes in technical and care structures using evidence-based practices focusing on knowledge and health literacy. [ABSTRACT FROM AUTHOR]

27. Self-care in Preserving the Vascular Network: Old Problem, New Challenge for the Medical Staff

Teaching/educating patients with end stage renal disease (ESRD) and identifying their self-care behaviors for vascular network preservation are very important. However, the self-care behaviors regularly performed by patients are still unknown. We compared self-care behaviors for vascular network preservation performed by patients who are/are not followed-up by the nephrologist. The study design was a prospective, observational and comparative study. Inclusion criteria were as follows: ESRD patients (at stages 4 or 5); at least 18 years old; in pre-dialysis with at least a 6-month follow-up period by the nephrologist or who started dialysis in emergency and were not followed-up by the nephrologist; with no memory problems; and medically stable. Primary outcome was the frequency of self-care behaviors for vascular network preservation. Secondary outcome was the comparison between self-care behaviors by ESRD patients who were/were not followed-up by the nephrologist. The study involved 145 patients, 64.1% were female, the mean age was 69.5 years and the self-care behaviors mean score was 36.8% (with a SD of 39.8%). The number of patients followed-up and not followed-up by the nephrologist was 109 (group 1) and 36 (group 2), respectively. Social characteristics were similar in the two groups (P > 0.05). The mean self-care behaviors were 29.4% and 59.2% in groups 1 and 2, respectively (P = 0.000). Patients performed self-care behaviors for vascular network preservation with a relatively low frequency (the mean score was 36.8% only). Patients not followed by the nephrologist performed self-care behaviors more often than those who were followed (59.2% vs. 29.4% respectively, P = 0.000). info:eu-repo/semantics/publishedVersion

Vascular access dysfunction is a serious problem in dialysis units. Some patients have complex dysfunctions that are difficult to resolve. In this article, we report the case a of two patients with radiocephalic arteriovenous fistulae (RC-AVF) who had stenosis/occlusion of the forearm median vein and where we used the basilic vein of the forearm as a solution. We reviewed the use of this surgical solution in RC-AVF. Two male patients on hemodialysis exhibited stenosis/occlusion of the forearm median vein. The forearm basilic vein was isolated and rotated toward the forearm median vein in order to solve RC-AVF problems. One patient had fistula thrombosis 5 months after the procedure, while for the other patient, the fistula continues to work without problems. Literature describes only a few cases using the forearm basilic vein or the brachial vein for fistula recovery. This procedure increased the patency of fistulas. This approach has been proven to be a good solution for solving outflow problems using the superficial or deep veins, increasing fistula patency and avoiding the need to place a central venous catheter and all the related complications. info:eu-repo/semantics/publishedVersion

29. Two-Stage Basilic Vein Transposition: Second Stage Results

The increasing survival of hemodialysis patients results in the depletion of superficial venous capital justifying the use of the basilic vein. Many groups still prefer an arteriovenous graft due to transposition complexity and the time needed to achieve maturation. In this work we review the results of our series of basilic vein transpositions (BVT). BVTs were performed in two stages: first, creation of the fistula; second, transposition of the vein using three small incisions in the arm. All patients had to have direct arterialization of the basilic vein, therefore, patients with previous ipsilateral wrist fistulas also followed a two-stage protocol. Data were retrospectively revised from all transposition procedures made between September 2005 and November 2012. Patency and complication rates were the primary outcomes evaluated. A total of 276 basilic veins were transposed. Usage rate was 82.2%. 8% (N = 22) of the fistulas were never used due to thrombosis. Secondary patency rates at 1 and 2 years were, respectively, 84% and 66.3%. Complications occurred in 61.6% (N = 170) of fistulas and 65.9% (N = 112) of which had to undergo surgical or endovascular revision. The most frequent complication was vein stenosis (39.7% of late complications, N = 92). Albeit its greater technical complexity, the transposed basilic vein represents a hemodialysis access with good patency rates. Complication rates, although high, are less than those of CVC or prosthetic grafts. Dysfunction problems with vascular access are a concern to patients and dialysis units. The vascular surgeon should analyse such dysfunction and perform a careful assessment of the vascular network in order to find new fistula layouts. We introduce and discuss the case of creation of a radio-cephalic fistula with outflow into the forearm basilic vein through rotation of the forearm basilic vein toward the cephalic vein in the forearm of an 88-year-old hemodialysis male patient. Vascular access is essential for the implementation of hemodialysis (HD). The arteriovenous fistula (AVF) can be constructed in various locations using various veins. However, the quality of the veins will influence the construction site, as well as the functioning of the AVF. Careful analysis of the vascular network allows options for the development of new fistulas presentations. We present and discuss the case of a woman aged 69 years in HD in which a brachial-cephalic fistula with drainage to basilic vein was created, through rotation of the cephalic vein on the forearm level. This kind of access serves to prolong the time spent dialyzing through native fistulae, with their reduced complications and greater cost-effectiveness. info:eu-repo/semantics/publishedVersion

Several guidelines recommend the importance of educating the patient about the care of vascular access. Nurses have a key role in promoting the development of self-care behaviors by providing the necessary knowledge to patients, so that they develop the necessary skills to take care of the arteriovenous fistula (AVF). This article describes the process of building a scale of assessment of self-care behaviors with arteriovenous fistula in hemodialysis (ASBHD-AVF). This is a cross-sectional study in which the development, construction, and validation process followed the directions of the authors Streiner and Norman. This is a convenience sample, sequential, and nonprobabilistic constituted by 218 patients. The study was conducted in two stages during 2012-2014. The first phase corresponds to the scale construction process, 64 patients participated, while the second corresponds to the evaluation of metric properties and 154 patients participated. The principal component analysis revealed a two-factor structure, with factorial weights between 0.805 and 0.511 and between 0.700 and 0.369, respectively, explaining 39.12% of the total variance of the responses. The Cronbach's alpha of the subscale management of signs and symptoms is 0.797 and from the subscale prevention of complications is 0.722. The ASBHD-AVF revealed properties that allow its use to assess the self-care behaviors in the maintenance and conservation of the AVF. info:eu-repo/semantics/publishedVersion

Vascular access is one of the leading causes of mobilization of financial resources in health systems for people with chronic kidney disease on hemodialysis. Physical examination of the arteriovenous fistula (AVF) has demonstrated its effectiveness in identifying complications. We decided to evaluate the influence of nurses' professional experience in the detection of complications of the AVF (venous stenosis and steal syndrome). The study took place in eight hemodialysis centers between May and September of 2011 in the north of Portugal. Sample was constituted by registered nurses. The nurses involved in the experiment were divided in two groups: those who had more than 5 years of experience and those who had less than 5 years of experience. Ninety-two nurses participated in the study: 34 nurses had less than 5 years of professional experience and 58 had more than 5 years of professional experience. In the practices considered by nurses in the detection of venous stenosis, there were no differences observed between the groups (P > 0.05). In steal syndrome, there were no differences observed between the groups in the practices of the nurses in the detection of this complication of the AVF (P > 0.05). 35. Mental vulnerability of nursing degree students in higher education: exploratory study

RESUMO Objetivos: avaliar os consumos de substâncias psicoativas, a morbilidade psiquiátrica e analisar as variáveis que estão associadas a maior risco de adoecer dos estudantes de enfermagem. Métodos: estudo transversal, exploratório e descritivo de cariz quantitativo. Amostra constituída por 980 estudantes do Curso de Licenciatura em Enfermagem em que foi aplicada o Mental Health Inventory (MHI). Foram respeitados os princípios éticos expressos na Declaração de Helsínquia. Resultados: os resultados sugerem níveis preocupantes de morbilidade essencialmente em termos de, solidão (48.5%), nervosismo e apreensão (48.2%), tensão e irritabilidade (68.6%). Sentem tristeza (59.3%), ansiedade, preocupação (65%) e depressão (61.4%). Conclusão: os dados apontam, para a existência de tristeza, tensão, ansiedade, nervosismo e depressão, evidenciando a necessidade de implementação de programas de promoção da saúde mental nos estudantes de enfermagem. ABSTRACT Objective: to evaluate the use of psychoactive substances, psychiatric morbidity and to analyze the variables associated to higher risk of sickness of nursing students. Methods: cross-sectional, exploratory and descriptive quantitative-oriented study. The sample consists of 980 nursing degree students. It was used the Mental Health Inventory (MHI) for data collection. Declaration of Helsinki’ ethical principles were respected. Results: students refer loneliness (48.5%); nervous and apprehensive feelings (48.2%); tension and irritability (68.6%); sadness feelings (59.3%); anxiety and worry (65%), and depression (61.4%). Conclusions: the results indicate the existence of sadness, tension, nervousness, and anxiety, hopelessness and depression feelings. A doença renal crónica carateriza-se por ser uma doença que ocorre pela falência renal. No último estadio, a pessoa com doença renal crónica é sujeita à terapia de substituição renal, como a hemodiálise. Uma vez em hemodiálise torna-se imperioso a adoção de um regime terapêutico adequado nas suas várias dimensões. No regime terapêutico é de elevada importância estudar as perceções que os enfermeiros que trabalham nesta área têm acerca dos níveis de adesão medicamentosa. Os objetivos deste estudo foram: - identificar a avaliação na adesão ao regime medicamentoso, - descrever as barreiras dos enfermeiros na avaliação da adesão ao regime medicamentoso, e - identificar estratégias utilizadas pelos enfermeiros para melhorar a adesão ao regime medicamentoso, das pessoas com doença renal crónica em programa regular de hemodiálise. Trata-se de um estudo qualitativo, descritivo e exploratório, onde se avaliou o contributo dos enfermeiros para a adesão ao regime medicamentoso, sendo descritas as estratégias que os mesmos utilizam. O instrumento de colheita de dados selecionado foi a entrevista semiestruturada realizada a doze enfermeiros. Para a análise da informação procedeu-se a análise de conteúdo segundo a técnica proposta por Bardin (2001). Os resultados revelam que poderá existir um grande contributo dos enfermeiros na adesão ao regime medicamentoso nas pessoas com doença renal crónica em programa regular de hemodiálise. A adesão ao regime medicamentoso pode ser melhorada através de uma relação de proximidade com a pessoa doente ou com a família, de forma a consciencializar os mesmos. A avaliação da adesão feita pelos enfermeiros considerou categorias como avaliação de sinais e sintomas, o conhecimento da doença e as estratégias de avaliação. Os fatores para adesão ao regime medicamentoso foram identificados em vários níveis, tais como problemas: relacionados com o doente como iliteracia, idade, escolaridade, falta de consciencialização; relacionados com o ambiente social e familiar e relacionados com a organização. Por outro lado, também foram identificadas estratégias que os enfermeiros podem desenvolver, a fim de aumentar a adesão ao regime medicamentoso, como ensino educativo e atividades dentro da dinâmica de tratamento, que atendam às necessidades das pessoas.

O ensino do raciocínio diagnóstico em enfermagem voltado para a saúde da criança e do adolescente é bastante relevante para a formação de enfermeiros críticos, reflexivos e com conhecimentos específicos sobre esses períodos ímpares do ciclo de vida do indivíduo. Neste contexto, acredita-se que a simulação clínica é uma estratégia de ensino positiva e benéfica para a formação dos futuros enfermeiros. O presente estudo tem como objetivo analisar a eficácia da simulação clínica no ensino do raciocínio diagnóstico em enfermagem voltado para a saúde da criança e do adolescente. Estudo metodológico, executado em três etapas, a saber: construção dos cenários para a simulação clínica; validação do conteúdo desses cenários pelos especialistas; e aplicação da simulação como estratégia educativa no ensino do raciocínio diagnóstico em enfermagem voltado para a saúde da criança e do adolescente no período de dezembro de 2018 a setembro de 2019. Nas duas primeiras etapas, foram elaborados quatro casos clínicos destinados ao pré e pósteste e dois cenários para o processo de simulação, os quais tiveram seus conteúdos analisados por 25 especialistas que julgaram quanto à relevância, especificidade e coerência das pistas fornecidas em cada caso e cenário e o diagnóstico de enfermagem, por meio da Escala de Acurácia Diagnóstica. Na terceira etapa, a estratégia foi aplicada aos alunos do sétimo período do curso de graduação de uma universidade pública do Nordeste do Brasil. Eles foram divididos randomicamente nos grupos de intervenção e controle. O grupo de intervenção, além de participar da simulação, avaliou ainda a satisfação quanto ao processo. Os dados foram analisados por meio de estatística descritiva e inferencial. O projeto foi aprovado pelo Comitê de Ética em Pesquisa da instituição responsável, mediante Parecer número 3.175.619 e Certificado de Apresentação para Apreciação Ética 07990418.4.0000.5537. Os resultados mostraram que os quatro casos clínicos (Coeficiente S: 0,916) e os dois cenários (S=0,535) criados obtiveram coeficiente de concordância aceitável entre os especialistas. No experimento, o pré-teste indicou homogeneidade dos grupos intervenção e controle. No pós-teste, não houve diferença estatística significante de desempenho entre os grupos, contudo, os resultados mostraram discreta melhora no desempenho de raciocínio diagnóstico do grupo intervenção em comparação ao grupo controle. O grupo intervenção conseguiu inferir mais diagnósticos e seus componentes do que o grupo controle. Na avaliação da estratégia educativa, os alunos manifestaram alta satisfação e relevância para melhorar as habilidades de raciocínio diagnóstico voltado para a saúde da criança e do adolescente. Conclui-se que a estratégia sobre o ensino do raciocínio diagnóstico em enfermagem voltado para a saúde da criança e do adolescente não foi eficaz, porém promissora e atrativa para os estudantes de graduação. The teaching of diagnostic reasoning in nursing focused on the health of children and adolescents is very relevant for the training of critical, reflective nurses with specific knowledge about these unique periods in the individual's life cycle. In this context, it is believed that realistic simulation is a positive and beneficial teaching strategy for the training of future nurses. This study aims to analyze the effectiveness of realistic simulation in teaching diagnostic reasoning in nursing focused on the health of children and adolescents. Methodological study, carried out in three stages, namely: construction of scenarios for realistic simulation; validation of the content of these scenarios by specialists; and, application of simulation as an educational strategy in the teaching of diagnostic reasoning in nursing aimed at the health of children and adolescents in the period from December 2018 to September 2019. In the first two stages, four clinical cases were prepared for the pre and post-test and two scenarios for the simulation process, which had their contents analyzed by 25 experts who judged the relevance, specificity and coherence of the clues provided in each case. and nursing scenario and diagnosis, using the Diagnostic Accuracy Scale. In the third stage, the strategy was applied to students in the seventh period of the undergraduate course at a public university in northeastern Brazil. They were randomly divided into the intervention and control groups. The intervention group, in addition to participating in the simulation, also assessed satisfaction with the process. The data were analyzed using descriptive and inferential statistics. The project was approved by the Research Ethics Committee of the responsible institution, under opinion number 3,175,619 and Certificate of Presentation for Ethical Appreciation 07990418.4.0000.5537. The results showed that the four clinical cases (S coefficient: 0.916) and the two scenarios (S = 0.535) created obtained an acceptable coefficient of agreement among specialists. In the experiment, the pre-test indicated homogeneity of the intervention and control groups. In the post-test, there was no statistically significant difference in performance between the groups, however the results showed a slight improvement in the diagnostic reasoning performance of the intervention group compared to the control group. The intervention group was able to infer more diagnoses and their components than the control group. In evaluating the educational strategy, students expressed high satisfaction and relevance to improve diagnostic reasoning skills aimed at the health of children and adolescents. It is concluded that the strategy on teaching diagnostic reasoning in nursing focused on the health of children and adolescents was not effective, but promising and attractive for undergraduate students.

It is a methodological research of the type of development. The educational booklet entitled ¿Support booklet for renal patients using vascular access: mirror, mirror mine¿ was based on Callista Roy's theory and based on NANDA-I, NOC and NIC. The study was conducted in four phases, the first consisting in the elaboration and submission of the project to the ethics committee, the second phase was conducted the bibliographic research and the interview stage with 40 participants to direct the content addressed, as they supported the third phase. which was the construction of the booklet and, later, the last one consisted in the validation of the content, appearance and suitability of the material, with 26 content judges (assistants and teachers) and three design and marketing technicians. After adjustments of the booklet, it was performed with validation with the target population. The ethical-legal aspects of research involving human subjects were respected according to Resolution 466¿2012 and the project was approved by the Research Ethics Committee of the University of Fortaleza (UNIFOR), under Opinion Nº 3.341.418, under the CAAE protocol.: 13477119.7.0000.5052. Considering the results, the integrative review identified 16 articles that addressed aspects about physiopsychosocial changes, orientations, and positive and negative coping strategies. As for the interviews, with the patients it was possible to unveil the feelings, perceptions and experiences about the therapy and the confrontation with the vascular access. Thus, according to the data compiled from the integrative review and interviews, the booklet was constructed and validated. That said, we consulted with a graphic designer to build the booklet. To validate the evaluative aspects of the content, language, illustration, layout, motivation and culture of the booklet were considered validated by the content judges, with an overall CVI of 0.94 and an S-CVI of 0.98. The mean level of agreement of the evaluated categories ranged from 0.90 to 0.98, being: content (S-CVI/Ave: 0.92); language (S-CVI/Ave: 0.90); illustration (S-CVI/Ave: 0.98); layout (S-CVI/Ave: 0.96); motivation (S-CVI/Ave: 0.94) and culture (S-CVI/Ave: 0.92). The second group of technical judges satisfactorily evaluated the educational technology with an overall CVI of 0.89 and an S-CVI of 0.89, as follows: content (S-CVI/Ave: 1.00); language (S-CVI/Ave: 0.77); illustration (S-CVI/Ave: 0.88); layout (S-CVI/Ave: 1.00) and motivation (S-CVI/Ave: 0.66). Moreover, in the opinion of the target audience, it considered the booklet as satisfactory, with a degree of agreement higher than 80%, which ranged from 82 to 100% of 10 the positive responses. Therefore, after suggestions and contributions from the content judges, technicians and the target audience, the readjustments of the booklet were made in order to make it more effective. Therefore, the educational booklet entitled ¿Support booklet for renal patients using vascular access: mirror, mirror of mine¿ proved to be a valid and reliable educational material to be used in order to promote adherence to self-care by people undergoing hemodialysis. O estudo teve como objetivo conceber uma tecnologia educativa do tipo Cartilha em apoio aos pacientes renais crônicos em uso de dispositivos vasculares. Trata-se de uma pesquisa metodológica do tipo desenvolvimento. A cartilha educativa intitulada ¿Cartilha de apoio ao paciente renal em uso de acesso vascular: espelho, espelho meu¿ teve como princípio, a teoria de Callista Roy e fundamentada no NANDA-I, NOC e NIC. O estudo foi realizado em quatro fases, sendo composta a primeira pela elaboração e submissão do projeto ao comitê de ética, a segunda fase foi realizado o levantamento bibliográfico e a etapa de entrevista com 40 participantes para direcionar o conteúdo abordados, visto que fundamentaram a terceira fase que foi a construção da cartilha e, posteriormente a última consistiu na validação de conteúdo, aparência e adequabilidade do material junto a 26 juízes de conteúdo (assistenciais e docentes) e três técnicos da área de design e marketing. Após adequações da cartilha, foi procedido com a validação junto à população-alvo. Os aspectos ético-legais da pesquisa envolvendo seres humanos foram respeitados de acordo com a Resolução 466¿2012 e o projeto foi aprovado pelo Comitê de Ética em Pesquisa da Universidade de Fortaleza (UNIFOR), sob Parecer nº 3.341.418, sob o protocolo CAAE: 13477119.7.0000.5052. Considerando os resultados, a revisão integrativa identificou 16 artigos que abordavam aspectos acerca das mudanças fisiopsicossociais, orientações e estratégias de enfrentamento positivos e negativos. Isto posto, foi procedido a consulta com um designer gráfico para a construção da cartilha. Para a validação quanto aos aspectos avaliativos do conteúdo, linguagem, ilustração, layout, motivação e cultura da cartilha foram considerados validados pelos juízes de conteúdo, com IVC global de 0,94 e um S-CVI de 0,98. O nível de média de concordância das categorias avaliadas variou de 0,90 a 0,98, sendo: conteúdo (S-CVI/Ave: 0,92); linguagem (S-CVI/Ave: 0,90); ilustração (S-CVI/Ave: 0,98); layout (S-CVI/Ave: 0,96); motivação (S-CVI/Ave: 0,94) e cultura (S-CVI/Ave: 0,92). O segundo grupo de juízes técnicos avaliaram satisfatoriamente a tecnologia educativa com IVC global de 0,89 e um S-CVI de 0,89, sendo: conteúdo (S-CVI/Ave: 1,00); linguagem (S-CVI/Ave: 0,77); ilustração (S-CVI/Ave: 0,88); layout (S-CVI/Ave: 1,00) e motivação (S-CVI/Ave: 0,66). Ademais, na apreciação do público-alvo considerou a cartilha como satisfatória, com grau de concordância superior a 80%, na qual variou de 82 a 100% das respostas positivas. Logo, após sugestões e contribuições dos juízes de conteúdo, técnicos e do público-alvo foram realizados os reajustes da cartilha a fim de torná-la mais efetiva. Portanto, a cartilha educativa intitulada ¿Cartilha de apoio ao paciente renal em uso de acesso vascular: espelho, espelho meu¿ mostrou-se um 8 material educativo válido e confiável para ser utilizada a fim de promover a adesão ao autocuidado das pessoas que fazem hemodiálise. 45. Effectiveness of a structured intervention in the development of selfcare behaviors with arteriovenous fistula in hemodialysis patients

A realização de comportamentos de autocuidado com a FAV é importante para que a pessoa com DRC consiga identificar sinais/sintomas ou situações que comprometam o funcionamento da FAV. Para aquisição desses comportamentos é necessário que o enfermeiro implemente programas educativos que permitam a capacitação da pessoa, em programa de HD, para a realização do autocuidado com a FAV. Este trabalho de investigação foi desenhado no sentido de identificar os comportamentos de autocuidado, de comparar a frequência desses comportamentos em centros de HD no norte de Portugal e nos Açores, de identificar os fatores que interferem com a sua frequência e de avaliar a eficácia de uma intervenção estruturada na frequência dos comportamentos de autocuidado com a FAV. Foram desenvolvidos dois estudos: um prospetivo e comparativo (Estudo 1) e outro quase-experimental (Estudo 2), nas duas regiões do país. Os comportamentos de autocuidado foram identificados através da ECAHD-FAV. Não foram identificadas diferenças estatisticamente significativas entre os dois grupos nas outras variáveis sociodemográficas e clínicas, exceto na situação profissional, em que existiam mais participantes reformados no norte de Portugal que nos Açores (85.4% e 50.0%, p=0.003, respetivamente) e, quanto à transmissão da informação, os enfermeiros estiveram menos envolvidos no grupo do norte do país que no grupo dos açoreanos (48,8% e 77.1%, p=0.011). A frequência de comportamentos de autocuidado com a FAV pela pessoa com DRC em HD, na primeira fase, foi equivalente tanto no continente português como no arquipélago (80.1% e 75.8%, p=0.078, respetivamente). Os participantes continentais apresentaram maior frequência de comportamentos de autocuidado com a FAV em comparação com os insulares (89.6% e 77.5%, p=0.0002, respetivamente), na subescala de gestão de sinais e sintomas. Após a implementação da IEAC-FAV (terceira fase) verificou-se um efeito positivo nos participantes dos Açores. - Abstract
Objective: to evaluate the effect of an educational video on the knowledge, attitude, and practice of self-care with arteriovenous fistula in patients undergoing hemodialysis treatment., Method: randomized controlled clinical trial, with two arms and single-blind. The intervention used an educational video on arteriovenous fistula self-care. The Fistula Self-Care Knowledge, Attitude, and Practice Scale was applied to 27 renal patients on hemodialysis in the control group and 28 in the intervention group at baseline, after seven and fourteen days. The data was analyzed using the Statistical Package for the Social Sciences software, using the chi-square test, Student's t-test, Mann-Whitney test, and Friedman's test with post-hoc analysis for multiple comparisons., Results: there were statistically significant differences in the knowledge and practice of self-care with the fistula at 0, 7 and, 14 days in the intervention (p= 0.004 and p<0.001, respectively) and control groups (p<0.001 for knowledge and practice). Attitude showed a significant difference at follow-up (p<0.001), but the post-hoc analysis did not confirm the significance obtained., Conclusion: patients' knowledge and practice showed significant increases at follow-up in the control and intervention groups, while the increase in attitude was not significant in either group. 47. Maintaining Vascular Access Response in a Severely COVID-19 Hit Country.

- Abstract
Introduction: The COVID-19 pandemic has caused an unprecedented disruption in healthcare systems worldwide, and Portugal was no exception. We analyze the impact of the COVID-19 pandemic in activity of our Vascular Access Center (Grupo Estudos Vasculares - GEV)., Material and Methods: This is a retrospective study, during March 2019 and February 2021. An analysis of surgical and appointments records in 2,495 patients from 25 hemodialysis centers followed by GEV was performed. Patients were divided into two periods: non-pandemic (NPP) (March 2019 to February 2020) and pandemic periods (PP) (March 2020 to February 2021). The number of surgeries and appointments were analyzed per month and per week. The number of thrombosis were analyzed in both periods. Normality was tested by the Shapiro-Wilk test and by the Lilliefors (Kolmogorov-Smirnov) test. Comparisons were made by the t-test (paired samples) when normality was not rejected and by the Wilcoxon test otherwise. All the variables (normal or otherwise) were described by the usual descriptive measures such as the mean, median and quartiles. No categorical data were collected. To avoid COVID-19 infection a set of measures were created: Sars-cov-2 PCR test for every patient, individual protection equipment for staff, rotating teams and schedules, and only one patient allowed in the circuit to the intervention room. A descriptive statistical analysis was performed with SPSS version28. The statistical significance was confirmed for p-value < 0.05., Results: A total of 1756 surgeries and 800 appointments were made in both PP and NPP. Comparing the periods, we performed 914 (52%) interventions in the NPP and 842 (48%) in PP, 423 (53%) consultations in NPP and 377 (47%) at PP. Comparing the NPP and PP by months we observed more appointments in the NPP (p=0.004). However, the difference in the number of surgeries did not reach statistical significance (p=0.533). There were more thrombosis during the summer and fall in the NPP and PP. A total of 138 in NPP and 131 in PP thrombosis were observed in the 2 years period. There was no record of COVID-19 infections between all GEV staff (n=25)., Conclusion: Due to timed and tight set of measures taken in the pandemic lockdown by GEV, a similar clinical and surgical activity regarding hemodialysis patients was obtained in both periods (PP and NPP). The hypothesis that the PP could diminish vascular access assessment/intervention or more thrombosis could occur was not verified at our institution. The set of measures established to deal with the COVID-19 pandemic was also effective to prevent infection in staff members.
