For 5 years, our family medicine clinic's physician-pharmacy team managed anticoagulation onsite. Now, against our recommendations and desires as a clinic, anticoagulation at our site is no longer managed by our local interdisciplinary team. Instead, it is being managed by our system's centralized anticoagulation team. Although some may point out that anticoagulation management is one small element of our practice, we believe eliminating this could open the door to other changes to our scope of practice. Anticoagulation belongs in primary care where comprehensive care, ongoing relationships between patients and care teams, and flexible office visit agendas optimize this service. The nonlinear transmission and the excited-state absorption spectra of three fluorene derivatives exhibiting large two-photon absorptivity were measured by the third harmonic of a picosecond Nd:YAG laser. We analyzed their capability for exhibiting stimulated emission in polar solvents and found that asymmetrical fluorene compounds with a diphenylamino substituent exhibited large Stokes shifts (~8000 [cm.sup.-1]), high quantum yields (~0.9 - 1.0), and no optical gain over their entire fluorescence spectral region. In contrast, a symmetrical fluorene derivative with vinylphenylbenzothiazole substituents in positions 2 and 7 underwent lasing under one-photon excitation by use of picosecond pulsed irradiation. High-order multiphoton excitation processes are becoming a reality for fluorescence imaging and phototherapy treatment because they afford minimization of scattered light losses and a reduction of unwanted linear absorption in the living organism transparency window, making them less susceptible to photodamage, while improving the irradiation penetration depth and spatial resolution. We report the four-photon-excited fluorescence emission of (7-benzothiazol-2-yl-9,-didecylfluoren-2-yl)diphenylamine in hexane and its four-photon absorption cross section [[sigma].sub.4]' = 8.1 x [10.sup.-109] [cm.sup.8] [s.sup.3] [photon.sup.-3] for the transition [S.sub.0] [right arrow] [S.sub.1] when excited at 1600 nm with a tunable optical parametric generator (OPG) pumped by picosecond laser pulses. Suicide is the tenth leading cause of death in America. Particularly at risk, Veterans are 1.5 times more likely to die by suicide than non-Veterans, and the suicide rate among service members has risen over the last decade. In the present study, we (1) assessed risk factors for suicidal ideation, suicide attempts, and suicide death within and between Veterans and service members, (2) identified the most commonly studied and (3) the strongest risk factors for suicide-related outcomes among Veterans and service members, and (4) compared overall and risk factor-specific meta-analytic prediction of suicide-related outcomes in Veterans and service members, as determined in the present meta-analysis, to that of the general population. Authors harvested longitudinal effects predicting suicidal ideation, suicide attempts, or suicide deaths in Veterans or service members until May 1, 2020. Traumatic Brain Injury, substance/alcohol use disorders, prior Self-Injurious Thoughts and Behavior, PTSD, and depressive symptoms were among the most commonly studied risk factors. Anger/aggression was particularly strong risk factors, providing a source for future study and intervention efforts. When combined, risk factors conferred similar risk for suicide attempts and suicide death among Veterans, service members, and the general population. However, when analyzing p-values, factors conferred significantly more risk of suicidal ideation among Veterans and service members as compared to the general population. A method using the Ullmann amination reaction that enabled access to some fluorine analogues derived from synthetic intermediates was developed. This resulted in effective synthesis of key intermediates enabling new two-photon absorbing chromophores from fluorine.

The nature of the HIV epidemic in the United States and Canada has changed with a shift toward rural areas. Socioeconomic factors, geography, cultural context, and evolving epidemics of injection drug use are coalescing to move the epidemic into locations where populations are dispersed and health care resources are limited. Rural-urban differences along the care continuum demonstrate the implications of this sociogeographic shift. Greater attention is needed to build a more comprehensive understanding of the rural HIV epidemic in the United States and Canada, including research efforts, innovative approaches to care delivery, and greater community engagement in prevention and care.
