Recently, researchers are facing great challenges to form the homogeneous solutions of various liquids at biomedical and industrial levels. Such homogeneous solutions can be controlled through stratification phenomenon directly which is ignored by the researchers in the existing literature specifically quadratic stratification and melting heat mechanism which is valid only for higher temperature processes. Thus, to form the homogeneous and stable liquid solutions for various industrial processes, we investigate the melting heat phenomenon in quadratic stratified Jeffery Nano fluid flow deformed due to linearly stretching sheet along with stagnation point. Inclined constant magnetic field is implemented on the fluid through an arbitrary angel. Characteristics of heat and transportations are disclosed through viscous dissipation, Brownian motion and thermphoresis. Temperature and concentration of the surrounding fluids are assumed higher than the stretching surface. The resultant governing equations are reduced to ordinary differential equations through proper transformations. Convergent analytical series solutions are computed via homotopic approach. Impact of various emerging parameters on temperature, velocity and concentration fields are illustrated and discussed comprehensively. Sherwood and Nusselt numbers and drag force are scrutinized mathematically, physically and graphically. It is analyzed from the study that (i) dominant melting reduces the temperature field (ii) higher thermal and solutal stratification decay the temperature and concentration fields respectively (iii) higher thermophoresis enhances the temperature field. Further, it is concluded that stable and homogeneous solutions may be made by increasing melting and reducing stratification phenomena. This study will help us to provide actual amount of heat for heating processes in industries and biomedicine which is the basic requirement for excellent quality of products.

3. Toward a globally lunar calendar: a machine learning-driven approach for crescent moon visibility prediction

Abstract This paper presents a comprehensive approach to harmonizing lunar calendars across different global regions, addressing the long-standing challenge of variations in new crescent Moon sightings that mark the beginning of lunar months. We propose a machine learning (ML)-based framework to predict the visibility of the new crescent Moon, representing a significant advancement toward a globally unified lunar calendar. Our study utilized a dataset covering various countries globally, making it the first to analyze all 12 lunar months over a span of 13 years. We applied a wide array of ML algorithms and techniques. These techniques included feature selection, hyperparameter tuning, ensemble learning, and region-based clustering, all aimed at maximizing the model’s performance. The overall results reveal that the gradient boosting (GB) model surpasses all other models, achieving the highest F1 score of 0.882469 and an area under the curve (AUC) of 0.901009. However, with selected features identified through the ANOVA F-test and optimized parameters, the Extra Trees model exhibited the best performance with an F1 score of 0.887872, and an AUC of 0.906242. We expanded our analysis to explore ensemble models, aiming to understand how a combination of models might boost predictive accuracy. The Ensemble Model exhibited a slight improvement, with an F1 score of 0.888058 and an AUC of 0.907482. Additionally, the geographical segmentation of the dataset enhanced predictive performance in certain areas, such as Africa and Asia. In conclusion, ML techniques can provide efficient and reliable tool for predicting the new crescent Moon visibility that would support the decisions of marking the beginning of new lunar months.

It has been studied that both two-dimensional (2D) MoS2 and V2O5, which are classified as transition metal dichalcogenides and transition metal oxides, are good photocatalyst materials. For this purpose, the hydrothermal method was practiced to synthesize V2O5(1−x)MoS2(x) (X = 1–5% w/w) nanocomposites with different 1–5% w/w weight percent of MoS2 as a prominent photocatalyst under laser irradiation for 2, 4, 6, 8, and 10 min to tune photocatalytic degradation of industrial wastage water. The surface of the 2D molybdenum nanolayered matrix was efficaciously decorated with V2O5 nanoparticles. The crystal phase and layered structures of the V2O5(1−x)MoS2(x) (X = 1–5% w/w) nanocomposites samples were verified by X-ray diffraction and scanning electron microscopy, atomic force microscopy, X-ray photoelectron spectroscopy respectively. In the range of the UV visible spectrum, the increment in light absorption from 3.6 to 14.5 Ω−1 cm−1 with an increase of active surface from 108 to 169 μm2{{\rm{\mu }}{\rm{m}}}^{2} with increased MoS2 doping percentage. Furthermore, dielectric findings like the complex dielectric function, tangent loss, electrical conductivity, quality factors, and impedance of V2O5(1−x)MoS2(x) (X = 1–5% w/w) nanocomposites are studied. According to photoluminescence studies, the intensity of peaks decreases when laser irradiation time and doping percentages of MoS2 are increased. As a result, a small peak indicates a decrement rate of electron–hole pair recombination, which increases the capacity for separation. Thermo-gravimetric analysis and differential thermal analysis results revealed that weight loss decreased from 0.69 to 0.35 mg and thermal stability increased with increased doping concentrations. Methylene blue was degraded in 150 min, proving that the prepared MoS2-doped V2O5 material was a stable and economically low-cost nanocomposite for photocatalytic activity.

5. Distribution and functional perspective analysis of epiphytic and endophytic bacterial communities associated with marine seaweeds, Alexandria shores, Egypt

Abstract There is an enormous diversity of life forms present in the extremely intricate marine environment. The growth and development of seaweeds in this particular environment are controlled by the bacteria that settle on their surfaces and generate a diverse range of inorganic and organic chemicals. The purpose of this work was to identify epiphytic and endophytic bacterial populations associated with ten common marine macroalgae from various areas along the Mediterranean Sea coast in Alexandria. This was done to target their distribution and possible functional aspects. Examine the effects of the algal habitat on the counting and phenotypic characterization of bacteria, which involves grouping bacteria based on characteristics such as shape, colour, mucoid nature, type of Gram stain, and their ability to generate spores. Furthermore, studying the physiological traits of the isolates under exploration provides insight into the optimum environmental circumstances for bacteria associated with the formation of algae. The majority of the bacterial isolates exhibited a wide range of enzyme activities, with cellulase, alginase, and caseinase being the most prevalent, according to the data. Nevertheless, 26% of the isolates displayed amylolytic activity, while certain isolates from Miami, Eastern Harbor, and Montaza lacked catalase activity. Geographical variations with the addition of algal extract may impact on the enumeration of the bacterial population, and this might have a relationship with host phylogeny. The most significant observation was that endophytic bacteria associated with green algae increased in all sites, while those associated with red algae increased in Abu Qir and Miami sites and decreased in Eastern Harbor. At the species level, the addition of algal extract led to a ninefold increase in the estimated number of epiphytic bacteria for Cladophora pellucida in Montaza. Notably, after adding algal extract, the number of presented endophytic bacteria associated with Codium sp. increased in Abu Qir while decreasing with the same species in Montaza. In addition to having the most different varieties of algae, Abu Qir has the most different bacterial isolates.

Abstract Background Acid sphingomyelinase deficiency (ASMD) or Niemann–Pick disease types A, A/B, and B is a progressive, life-limiting, autosomal recessive disorder caused by sphingomyelin phosphodiesterase 1 (SMPD1) gene mutations. There is a need to increase the understanding of morbidity and mortality across children to adults diagnosed with ASMD. Methods This observational retrospective survey analysed medical records of patients with ASMD with retrievable data from 27 hospitals in France, diagnosed/followed up between 1st January 1990 and 31st December 2020. Eligible records were abstracted to collect demographic, medical/developmental history, and mortality data. Survival outcomes were estimated from birth until death using Kaplan–Meier survival analyses; standardised mortality ratio (SMR) was also explored. Results A total of 118 medical records of patients with ASMD (type B [n = 94], type A [n = 15], and type A/B [n = 9]) were assessed. The majority of patients were males (63.6%); the median [range] age at diagnosis was 8.0 [1.0–18.0] months (type A), 1.0 [0–3] year (type A/B), and 5.5 [0–73] years (type B). Overall, 30 patients were deceased at the study completion date; the median [range] age at death for patients with ASMD type A (n = 14) was 1 [0–3.6] year, type A/B (n = 6) was 8.5 [3.0–30.9] years, and type B (n = 10) was 57.6 [3.4–74.1] years. The median [95% confidence interval (CI)] survival age from birth in patients with ASMD type A and type A/B was 2.0 [1.8–2.7] years and 11.4 [5.5–18.5] years, respectively. Survival analysis in ASMD type B was explored using SMR [95% CI] analysis (3.5 [1.6–5.9]), which showed that age-specific deaths in the ASMD type B population were 3.5 times more frequent than those in the general French population. The causes of death were mostly severe progressive neurodegeneration (type A: 16.7%), cancer (type B: 16.7%), or unspecified (across groups: 33.3%). Conclusions This study illustrated a substantial burden of illness with high mortality rates in patients with ASMD, including adults with ASMD type B, in France.

7. Temporal and Spatial Analysis for Precipitation Trend and Homogeneity in Semi-Arid Area, Northwestern Iraq as a Case Study

Detecting the trends and homogeneity in precipitation records is fundamental in climate change and hydrological studies. This study investigates the homogeneity and trends of precipitation time series at four stations in north-west Iraq. The analysis encompasses the annual, seasonal, and monthly time scale at a significant level of 5%. Four tests were employed to assess the homogeneity analysis of the precipitation time series: Standard Normal Homogeneity, Buishand, Pettitt, and Von Neumann Ratio. Then two methods were applied to detect the trend from 1970–2020. Sequential Mann- Kendall and Innovative trend tests were employed to track the significant trend changes and to find the magnitude, tendency, and significance of the trend during this period for precipitation. Microsoft Excel was utilized to achieve the concepts of homogeneity and trend for the precipitation time series. The results of the homogeneity test confirmed the suitability and homogeneity of the precipitation for further analysis. The trend analysis showed that there is an annual increasing (positive) trend in precipitation at Duhok and Rabia stations by 14 mm/decade and 0.25 mm/decade, respectively. Conversely, Mosul and Tal-Afer stations exhibited a decreasing (negative) trend by 21 mm/decade and 8.9 mm/decade, respectively. Spring precipitation exhibited a statistically significant decreasing trend in all Northwestern stations. The most substantial value in the Mosul station by 11mm/decade. This study highlights the importance of investigating drought and flood adaptation strategies to address the impact of climate changes impacts at the mentioned area.

8. Origin of spring water chemistry and its appropriateness for drinking requirements in a semi-arid environment (Ain Larbi, Algeria)

A comparative assessment of water quality for drinking purposes was conducted in Ain Larbi area using the Weighted Arithmetic Water Quality Index (WA-WQI). The origin of minerals is assessed by a saturation index approach and the Gibbs diagram. Also, mineral correlation process and ion exchange mechanism are detected by the Principal Component Analysis (PCA). The PCA results indicate that the groundwater quality in the Ain Larbi study area primarily results from weathering and geochemical alteration of rock. The saturation index and Gibbs diagram support these findings. The WA-WQI was calculated for each station based on physicochemical parameters. During the wet season, three out of five classes of WQI were observed in the present study, including excellent (25%), good (70%), and poor (5%). In the dry season, the classes excellent (25%), good (30%), and poor (45%) were represented. The present situation demonstrates how hydrogeological regimes and water–rock interactions affect groundwater ion concentrations. Our results contribute to identifying drinking water requirements in the Ain Larbi region, promoting sustainable water management. 9. Ferrate as a coagulant prior to sand filters treating secondary wastewater effluent for reuse

Wastewater reuse is one of the crucial water resources in Egypt due to the ongoing need to increase water resources and close the supply-demand gap. In this study, a new coagulant has been investigated before sand filters as an advanced wastewater treatment method. The sand filter pilot was run at a hydraulic loading rate of 0.75 m/h and two different dosages of three coagulants (Alum, FeCl3, and Ferrate VI) were selected using the jar tests. The sand filter without coagulant removed 12% of BOD5 and 70% of turbidity. Applying in-line coagulation before the sand filter provided effluents with better quality, especially for turbidity, organics, and microorganisms. Ferrate provided the highest removal of turbidity (90%) and BOD5 (93%) at very low dosages and lower costs compared with other coagulants, however, it adversely impacted both conductivity and dissolved solids. A significant effect on reducing bacteria was obtained with 40.0 mg/L of alum. According to the study's findings, the ferrate coagulant enhanced the sand filter's performance producing effluents with high quality, enabling it to meet strict water reuse regulations as well as aquatic environmental and health preservations. 10. Morphological and molecular characterization of Dermanyssus gallinae (Acari: Gamasida: Dermanyssidae) isolates of Al- Baha province

Background: An ectoparasite known as Dermanyssus gallinae feeds on infected blood with a high frequency in European chicken farms resulting in significant economic losses. Aim: The objective of the current work was to characterize D. gallinae, which infests laying hens on farms in Southern Al-Baha morphologically, molecularly, and to determine evolutionary relationship between the species. Methods: All mites that were morphologically recognized as D. gallinae were submitted to molecular analysis by PCR, which focused on the mitochondrial cytochrome oxidase subunit I (cox1) and internal transcribed spacers (ITS) of ribosomal DNA. Results: Morphological identification of the parasites uncovered three distinct features: a triangular anal shield, a broader than longer sternal shield, and a rounded posterior genitoventral shield. Each D. gallinae sample was amplified using a single band, measuring 550 bp for the cox1-targeting PCR, and 530 bp for the ITS-targeting PCR. The sequences of D. gallinae were added to the GenBank. Conclusion: At molecular identification level, this research identifies D. gallinae in Al-Baha for the first time. The results collectively provide a foundation for further research to understand the epidemiology and the part of this superfamily in the epidemiology of certain zoonosis. 11. The moderating role of ethical awareness in the relationship between nurses’ artificial intelligence perceptions, attitudes, and innovative work behavior: a cross-sectional study

Abstract Background Artificial intelligence is rapidly advancing and being integrated into healthcare, potentially revolutionizing patient care and improving outcomes by leveraging large datasets and complex algorithms. Aim Investigate the moderating role of ethical awareness between nurses’ artificial intelligence perceptions, attitudes, and innovative work behaviors. Design and Methods A cross-sectional descriptive correlational design adhering to STROBE guidelines. A non-probability convenience sample of 415 Alexandria Main University Hospital nurses was analyzed. Statistical methods included one-way ANOVA, the student t-test, and the Pearson coefficient, with results evaluated for significance at the 5% level and internal consistency assessed via Cronbach’s α. Linear regression assessed the predicting and moderating effect between ethical awareness, nurses’ artificial intelligence perceptions, attitudes, and innovative work behavior. The perceptions of using the Artificial Intelligence Scale, general attitudes towards the Artificial Intelligence Scale, ethical awareness of Using Artificial Intelligence, and the Employee Innovative Behavior Scale were used to respond to the research aim. Results The study revealed that perception of AI use among nurses has a mean score of 50.25 (SD = 3.49), attitudes towards AI have a mean score of 71.40 (SD = 4.98), ethical awareness regarding AI use shows a mean score of 43.85 (SD = 3.39), and nurses innovative behavior exhibits a mean score of 83.63 (SD = 5.22). Attitude and ethical awareness were statistically significant predictors of innovation. Specifically, for every one-unit increase in attitude, innovative work behaviors increase by 1.796 units (p = 0.001), and for every one-unit increase in ethical awareness, innovative work behaviors increase by 2.567 units (p = 0.013). The interaction effects between perception, ethical awareness, attitude, and ethical awareness were also examined. Only the interaction between attitude and ethical awareness was found to be significant (p = 0.002), suggesting that the effect of attitude on innovative work behaviors depends on the level of ethical awareness. In other words, ethical awareness moderates the relationship between attitudes and innovative work behaviors rather than perception and innovation. Conclusion There is a statistically significant correlation between attitude, ethical awareness, and creativity, highlighting that ethical awareness moderates the relationship between attitudes and innovative work behaviors. These findings emphasize the importance of ethical awareness in fostering positive attitudes towards AI and enhancing innovative practices in nursing, ultimately contributing to nurses’ well-being.

12. Dietary Pediastrum boryanum microalgal extract improves growth, enhances immunity, and regulates immune-related genes in Nile tilapia

Abstract Background Identifying alternative sustainable feed sources with high nutritional values is crucial for the future of environmentally and socially responsible aquaculture. In this regard, microalgae have been proven to have positive effects on fish health, which overwhelmed our interest in this study. Methods Pediastrum boryanum (P. boryanum) was incorporated into Nile tilapia feed at concentrations of 0, 0.75, and 1.5 mg/kg, as control, PbExt0.75, and PbExt1.5 groups to assess its effects on growth and biochemical indices, oxidant/antioxidant activities, immune and stress-related gene expression, and intestinal morphology. Results After 8 weeks, fish fed P. boryanum supplemented feed exhibited significant increases in final weight, length, condition factor, body weight gain, and specific growth rate, while the spleen-somatic index (SSI) and hepatosomatic index (HSI) showed no significant differences compared to the control group. Dietary P. boryanum supplementation also enhanced IgM levels and lysozyme activity, along with no marked effect on markers of liver function enzymes (alanine aminotransferase/ALT and aspartate aminotransferase/AST) or protein status (total protein and albumin). Furthermore, P. boryanum addition increased the activity of superoxide dismutase (SOD), catalase (CAT), and reduced glutathione (GSH) enzymes, highlighting its antioxidant potential, whereas malondialdehyde (MDA) concentrations showed no significant differences among the groups. Gene expression analysis revealed that tumor necrosis factor-α (TNF-α), interleukin-10 (IL-10), and transforming growth factor-β1 (TGF-β1) expression notably increased in groups fed P. boryanum containing feed, while no significant difference was observed in hepatic Heat Shock Protein 70 (HSP70) mRNA expression. Histopathological examination revealed no adverse effects of P. boryanum supplementation on the liver, spleen, or intestinal tissues. Villous height and villous surface area were notably increased in the high P. boryanum supplementation group, suggesting improved intestinal integrity and nutrient absorption. Conclusion Dietary P. boryanum supplementation can potentially improve growth performance, immune response, antioxidant status, and intestinal health of Nile tilapia, making it a promising candidate for sustainable aquaculture.

Abstract Background Kitchen workers face a disproportionately high risk of injuries and illnesses. Yet, despite the ubiquity of kitchen-related injuries, there has been a relative lack of comprehensive research on this topic, particularly in developing countries. Ain Shams University, as a prestigious educational institution with its diverse kitchen staff, provides an ideal setting for studying the challenges faced in professional kitchens. This study aims to measure the prevalence of work-related injuries and illnesses among kitchen workers of two major students’ hostels at Ain Shams University in Cairo and to explore their risk factors. Methods A cross-sectional analytical study involving kitchen staff from two hostels that house students at the university was carried out in 2021. The study involved all kitchen staff in the dorms for students (n = 128). Data was collected using a structured interview questionnaire. Results The findings showed a significant prevalence of work-related illnesses (81.3%) and injuries (77.3%) among restaurant employees. Age, education, and job type all had a statistically significant relationship with the frequency of work-related injuries over the previous year. Additionally, there was a statistically significant relationship between age, job type, and the frequency of diseases among kitchen workers (p ≤ 0.05). Conclusion Cooks and other food service employees are more susceptible to occupational diseases and injuries in the workplace. Restaurants and the university hostel authorities should implement preventative measures and policies to lower the risk of harm among these employees by offering occupational health and safety services such as training and education programs, personal protective equipment, and regular health examinations.

Abstract Bulk zinc oxide (ZnO-BPs) and its nanoparticles (ZnO-NPs) are frequently used in various products for humans. Helisoma duryi embryos can serve as effective model organisms for studying the toxicity of NPs. This study aimed to compare the teratogenic potency of ZnO-BPs and ZnO NPs in the embryonic stages of H. duryi to evaluate the utility of this snail as a bioindicator for ZnO-NPs in the aquatic environment. The mechanisms of teratogenesis were evaluated by determination of the LC50, studying the effect of sub-lethal concentrations of both ZnO forms on the embryos, and studying their enzyme activity, oxidative stress, and biochemical analysis. The SDS-PAGE electrophoresis was undertaken to assess the effect of ZnO-BPs and ZnO NPs on protein synthesis. The results revealed that the veliger stage of H. duryi is the specific stage for bulk and nano ZnO. ZnO-NPs proved to be more toxic to snails’ embryos than ZnO-BPs. Exposure to ZnO influences specific types of defects in development, which in the case of BPs are far less drastic than those caused by NPs. Thus, the toxicity of ZnO-NPs in embryonic development is due to their unique physicochemical properties. The observed malformations include mainly hydropic malformation, exogastrulation, monophthalmia, shell misshapen, and cell lyses. Almost all tested oxidative biomarkers significantly changed, revealing that ZnONPs display more oxidative stress than ZnO-BPs. Also, the low concentration of ZnO induces many disturbances in the organic substances of veliger larvae, such as a decrease in the total protein and total lipid levels and an increase in the glycogen level. The results indicated that ZnO-BPs increase the number of protein bands. Conversely, ZnO-NPs concealed one band from treated egg masses, which was found in the control group. Embryos of snail are an appropriate model to control freshwater snails. This study demonstrates that H. duryi embryos can serve as effective model organisms to study the toxicity of ZnO-NPs.

In the evolving economic and geopolitical landscape, regional integration serves as a strategy for countries to enhance global influence and optimise trade relations. The present study assesses the impact of trade liberalisation under the African Continental Free Trade Area (AfCFTA) on Algeria’s exports, contributing thus to the debate on the potential effects of trade agreements involving developing countries. The study relies on official data gathered from international institutions to compute essential trade indicators, particularly those related to concentration and regional orientation. Our analysis enabled us to categorise products based on the possible effects they are likely to encounter. The results show that only four natural resource-based products will continue to enjoy a trade expansion. More importantly, a large number of non-hydrocarbon products are not likely to increase despite tariff reductions. According to our findings, the effects of the AfCFTA on trade creation are limited. The analysis revealed that Algeria currently lacks the essential prerequisites for a successful integration into the African market. To facilitate its integration process, a series of complementary measures must be implemented.

Patients were categorized into: one-stent strategy (Provisional group, n = 44) and a two-stent strategy (TAP, DK crush, or Culotte technique, n = 18), based on the coronary bifurcation site, angle, side branch diameter and Medina classification. Background: Birth injuries to the brachial plexus are highly rare; however, they necessitate treatment and follow-up from infancy until skeletal maturity. Many complications may arise as a result of primary nerve surgery. Objectives: To study the long-term complications of nerve repair procedures in patients with obstetric brachial plexus palsy. Patients and methods: This was retrospective research that was performed on 8 cases having obstetric brachial plexus palsy and underwent nerve surgery procedure. The ages ranged from 3 to 20 months. The assessment of the surgical procedure outcomes involves post-operative evaluation of both short and long-term complication. Results: Our study showed that the most frequent long-term complications were decreased innervation (25%), followed by decreased strength and stamina and balanced discoordination (12.50%) while there were no complications encountered in 50% of cases. The short-term complications included wound infection, seroma and hematoma. Conclusion: The nerve surgery procedure is an efficient primary approach in the management of cases with brachial plexus injury. However, a long-term follow-up is usually associated with late squeal such as reduced innervation and decrease in the overall strength and stamina.

The study aimed to evaluate the effects of multi-strain probiotic (MSP) on broiler growth, health, and nutrient utilization under high animal protein concentrate conditions. Male Ross-308 broilers were assigned to four groups: a control (C), MSP at 50 mg/kg (MSP50), MSP at 100 mg/kg (MSP100), and Zinc bacitracin (ZB) as a positive control. MSP supplementation significantly improved feed intake, weight gain, and feed conversion efficiency, with MSP100 demonstrating the best feed conversion ratio during both starter and finisher phases. MSP also enhanced dressing percentages and positively influenced blood lipid profiles. The supplementation reduced harmful bacterial growth (E. coli, Salmonella, Clostridium perfringens) while promoting beneficial bacteria (Lactobacillus). Moreover, MSP positively impacted gut histomorphology, showing higher villus height, width, and crypt depth in the MSP100 group. Digestibility of dry matter and crude protein was enhanced in birds fed the MSP100 diet. Overall, MSP supplementation exhibited positive effects on broiler performance, health, and nutrient digestibility, indicating its potential as an antibiotic alternative for improving practices in the poultry feed industry.

Background: Type I diabetes mellitus (T1DM) is one of the most common chronic autoimmune diseases worldwide. miRNAs are a class of small non-coding RNA molecules that have been linked to immune system functions, β-cell metabolism, proliferation, and death, all of which contribute to pathogenesis of TIDM. Dysregulated miRNAs have been identified in Egyptian TIDM patients. Aim: Several miRNAs were profiled in Egyptian TIDM patients to determine whether they can be used as molecular biomarkers for T1DM. The relationship between the investigated miRNAs and pro-inflammatory cytokines (TNF-α and IL-6) has also been evaluated in the development of TIDM, in addition to the creation of a proposed model for TIDM prediction. Patients & methods: Case-control study included 177 Egyptian patients with confirmed type I diabetes mellitus and 177 healthy individuals. MiRNA-34 and miRNA-146 were detected in serum samples using real-time PCR, whereas TNF-α and IL-6 levels were assessed using ELIZA. Results: Patients with TIDM showed a significant decrease in the expression of miRNA-146, with a cut-off value ≤ 3.3, 48 % specificity, and 92.1 % sensitivity, whereas miRNA-34 had the highest sensitivity (95.5 %) and specificity (97.2 %) for differentiating diabetic patients from controls. Furthermore, other diagnostic proinflammatory markers showed lower sensitivity and specificity. Conclusion: Serum levels of miRNA-34a, miRNA-146, IL-6, and TNF-α provide new insights into T1DM pathogenesis and could be used for screening and diagnosis purposes. They can be also a potential therapeutic target, as well as allowing for more strategies to improve T1DM disease outcomes.

AL amyloidosis is a rare systemic disease characterized by the deposition of amyloid protein in various organs, including the kidneys, heart, peripheral nervous system, digestive tract, skin, and muscles. Peritoneal involvement in AL amyloidosis is exceptionally rare. We present a unique case of AL amyloidosis with concurrent cardiac, cutaneous, and peritoneal manifestations. The patient initially presented with ascites and respiratory symptoms. An etiological workup revealed multiple myeloma as the underlying cause. This case highlights the importance of considering AL amyloidosis in the differential diagnosis of peritoneal ascites, providing valuable insights for radiologists in recognizing atypical presentations of this disease.

Congenital hernia of the diaphragmatic dome is defined as a congenital embryopathy where all or part of the diaphragmatic dome fails to form during embryonic development. The classification is based on the location of the defect in the diaphragm.In this report, we present a case of Morgani hernia in a 52-year-old man whose sole initial symptom was dyspnea, occurring within the context of a SARS-CoV-2 (COVID-19) infection.Morgagni hernia is a congenital diaphragmatic hernia. It is rare and comprises only about 2% of all diaphragmatic hernias. Most cases of congenital diaphragmatic hernia (CDH) manifest with respiratory distress during infancy. CDH is a rare condition that can occur in adults and can have various symptoms that are often misdiagnosed or not fully investigated. Morgagni's hernia is currently associated with other malformations or anomalies such as Trisomy 21. CT scan of the chest and abdomen remains the best imaging method to confirm the diagnosis. Surgical correction is the only treatment for Morgagni hernia.

Summary: Human genetic studies of critical COVID-19 pneumonia have revealed the essential role of type I interferon-dependent innate immunity to SARS-CoV-2 infection. Conversely, an association between the HLA-B∗15:01 allele and asymptomatic SARS-CoV-2 infection in unvaccinated individuals was recently reported, suggesting a contribution of pre-existing T cell-dependent adaptive immunity. We report a lack of association of classical HLA alleles, including HLA-B∗15:01, with pre-omicron asymptomatic SARS-CoV-2 infection in unvaccinated participants in a prospective population-based study in the United States (191 asymptomatic vs. 945 symptomatic COVID-19 cases). Moreover, we found no such association in the international COVID Human Genetic Effort cohort (206 asymptomatic vs. 574 mild or moderate COVID-19 cases and 1,625 severe or critical COVID-19 cases). Finally, in the Human Challenge Characterisation study, the three HLA-B∗15:01 individuals infected with SARS-CoV-2 developed symptoms. As with other acute primary infections studied, no classical HLA alleles favoring an asymptomatic course of SARS-CoV-2 infection were identified.

The Pregnancy-Associated Glycoproteins (PAGs) belong to aspartic protease family and are synthesized by trophoblast cells of ruminant placenta and secreted into maternal blood and milk. These biomarkers very closely associated to pregnancy have demonstrated their efficiency and reliability in detection of vital embryos or pregnancy loss. In order to measure the concentration of PAGs in bovine serum, a direct radioimmunoassay (RIA) system was developed and validated. The radioiodinated bPAG tracer was prepared by chloramine-T direct method and the solid phase was used in RIA/PAGs as a separation system. The optimization tests allowed to set the assays procedure as working buffers, time and temperature of the assays incubation. Assay sensitivity was 0.63 ng/mL, the intra and inter-assay CVs were 7.7% and 5.3% respectively and the accuracy of the assays was expressed by a good parallelism and recovery tests. The serums bPAG values determinated by this developed system were consistent with the reproduction status and pregnancy stages of cows. The developed RIA/PAG system can provide the laboratory with an efficient method for the accurate detection of pregnancy, which could offer veterinarians and farmers an important tool for the reproductive management in dairy herds. Moreover, this diagnostic tool can also be developed for other livestock species.

28. Multivariate analysis of agro-morphological and quality traits of a durum wheat collection under rainfed Algerian conditions

This study assesses the variability, heritability, and correlation of some agro-morphological and quality traits in 125 durum wheat germplasm and groups them into clusters. These germplasms include landraces and cultivars from a variety of countries. The field experiment was conducted at the Algiers experimental station during the 2020-2021 season using an augmented randomized complete block design, with data collected for 11 quantitative traits. The analysis of variance revealed significant differences between the genotypes for the majority of the traits examined. Plant height, flag leaf area, thousand-kernel-weight, number of kernels/spikes, sedimentation volume, yellow index, and wet and dry gluten all showed high heritability coupled with high to moderate genetic advance as a percentage of the mean. The principal components analysis explained 46.67% of the total variation in the first two axes, allowing the identification of traits that significantly contributed to this variation. Pearson correlation coefficients revealed strong associations between plant height and days to heading, which affects seed yield improvement, as well as protein content, gluten content, and sedimentation volume, which influence durum wheat nutritional quality. Cluster analysis divided the 125 durum wheat germplasm samples into five clusters based on trait similarities. Genotypes in clusters II, IV, and V exhibit superior agronomic and quality traits, making them valuable resources for breeding and crossing programs aimed at improving durum wheat. The findings also highlight the value of Algerian landrace germplasm and its potential for use in the genetic improvement of other external genetic materials, particularly in terms of quality trait parameters.

29. Hepatitis A virus infection associated with bilateral pleural effusion, ascites, and acalculous cholecystitis in childhood: a case report

Abstract Background Acute hepatitis A infection is common among children in developing nations. The clinical presentation in children is usually asymptomatic and anicteric, and it is a self-limiting infection. Rarely, it can be associated with extrahepatic complications such as pleural effusion, acalculous cholecystitis, and ascites. Case presentation An 8-year-old middle eastern child presented with abdominal pain, jaundice in the sclera, yellowish color of urine, and poor appetite. In the last two days, abdominal distension developed. After conducting diagnostic investigations, the child was diagnosed with HAV hepatitis associated with bilateral pleural effusion, acalculous cholecystitis, and ascites. He was managed conservatively with vitamin K supplementation and supportive parenteral fluids. After 4 days, clinical improvement was observed. Conclusion Hepatitis A infections presented with extrahepatic manifestations like pleural effusion, acalculous cholecystitis, and ascites are very rare, especially in children. There have been some reports of these manifestations occurring in isolation, but for them to co-exist to our knowledge, this has only been reported in two cases in the literature, and this is the third case with all these three rare complications being presented simultaneously in a single child. Although HAV infection is an asymptomatic and self-limiting viral disease in childhood, it can manifest with rare extrahepatic complications, so pediatricians should be aware of this rare association to avoid unnecessary investigations.

Abstract Background Evaluation publications typically summarize the results of studies to demonstrate the effectiveness of an intervention, but little is shared concerning any changes implemented during the study. We present a process evaluation protocol of a home-based gait, balance, and resistance exercise intervention to ameliorate persistent taxane-induced neuropathy study according to 7 key elements of process evaluation. Methods The process evaluation is conducted parallel to the longitudinal, randomized control clinical trial examining the effects of the home-based gait, balance, and resistance exercise program for women with persistent peripheral neuropathy following treatment with taxanes for breast cancer (IRB approval: Pro00040035). The flowcharts clarify how the intervention should be implemented in comparable settings, fidelity procedures help to ensure the participants are comfortable and identify their individual needs, and the process evaluation allows for the individual attention tailoring and focus of the research to avoid protocol deviation. Conclusions The publication of the evaluation protocol plan adds transparency to the findings of clinical trials and favors process replication in future studies. The process evaluation enables the team to systematically register information and procedures applied during recruitment and factors that impact the implementation of the intervention, thereby allowing proactive approaches to prevent deviations from the protocol. When tracking an intervention continuously, positive or negative intervention effects are revealed early on in the study, giving valuable insight into inconsistent results. Furthermore, a process evaluation adds a participant-centered element to the research protocols, which allows a patient-centered approach to be applied to data collection. 31. Dietary algal-sourced zinc nanoparticles promote growth performance, intestinal integrity, and immune response of Nile tilapia (Oreochromis niloticus)

Abstract Background Trace elements play a crucial role in fish nutrition, with zinc (Zn) being one of the most important elements. BIO-sourced zinc nanoparticles were synthesized using the green microalga Pediastrum boryanum (BIO-ZnNPs, 29.35 nm). 30 or 60 mg/ kg dry feed of the BIO-ZnNPs (BIO-ZnNPs30 and BIO-ZnNPs60) were mixed with the Nile tilapia (Oreochromis niloticus) basal diet and fed to the fish for 8 weeks to evaluate their impact on fish growth, digestion, intestinal integrity, antioxidative status, and immunity. Results A significant enhancement was observed in all investigated parameters, except for the serum protein profile. BIO-ZnNPs at 60 mg/kg feed elevated the activities of reduced glutathione (GSH) and catalase (CAT), enzymatic antioxidants, but did not induce oxidative stress as reflected by no change in MDA level. Fish intestinal immunity was improved in a dose-dependent manner, in terms of improved morphometry and a higher count of acid mucin-producing goblet cells. Interleukin-8 (IL-8) was upregulated in BIO-ZnNPs30 compared to BIO-ZnNPs60 and control fish groups, while no significant expressions were noted in tumor necrosis factor-alpha (TNFα), nuclear factor kappa B (NF k B), and Caspase3 genes. Conclusion Overall, BIO-ZnNPs inclusion at 60 mg/kg feed showed the most advantage in different scenarios, compared to BIO-ZnNPs at 30 mg/kg feed. The positive effects on growth and intestinal health suggest that BIO-ZnNPs supplementation of aquafeeds has many benefits for farmed fish.

The rapid development of coronavirus disease nineteen (COVID-19) into a pandemic within months has posed significant risks, particularly for the elderly and individuals with underlying medical conditions. The primary driver of COVID-19 severity and mortality is the cytokine inflammatory storm. Elevated levels of ferritin, resulting from this storm and secondary hemophagocyticlymphohistiocytosis, have been observed in severe COVID-19 patients. The cytokine storm represents an uncontrolled and dysfunctional immune response within the pathogenic mechanism of COVID-19, leading to acute respiratory distress syndrome (ARDS) and systemic organ failure. Ferritin, an iron storage protein crucial for regulating cellular oxygen metabolism, shows a significant correlation with disease severity in COVID-19 patients.This study, conducted across three Libyan hospitals and two private laboratories from 2018 to 2022, targeted 500 cases to investigate ferritin's role in COVID‐19. Polymerase Chain Reaction (PCR) tests confirmed COVID-19 positivity. Ferritin levels were measured using a fully automatic device, revealing high levels in almost all positive cases, with some experiencing levels as high as 4532 ng/ml. Analysis of 209 negative COVID-19 cases before and after the pandemic showed consistently low or normal ferritin levels. Among the positive cases, 250 were admitted to the ICU, with a significant proportion experiencing elevated ferritin levels, and 82 ICU patients succumbed to the disease. Gender and age did not seem to influence ferritin elevation in COVID-19 cases.In conclusion, increased ferritin levels may indicate worsening COVID-19 cases and could be associated with disease severity and mortality.

Abstract This paper presents an innovative control scheme designed to significantly enhance the power factor of AC/DC boost rectifiers by integrating an adaptive neuro-fuzzy inference system (ANFIS) with predictive current control. The innovative control strategy addresses key challenges in power quality and energy efficiency, demonstrating exceptional performance under diverse operating conditions. Through rigorous simulation, the proposed system achieves precise input current shaping, resulting in a remarkably low total harmonic distortion (THD) of 3.5%, which is well below the IEEE-519 standard threshold of 5%. Moreover, the power factor reaches an outstanding 0.990, indicating highly efficient energy utilization and near-unity power factor operation. To validate the theoretical findings, a 500 W laboratory prototype was implemented using the dSPACE ds1104 digital controller. Steady-state analysis reveals sinusoidal input currents with minimal THD and a power factor approaching unity, thereby enhancing grid stability and energy efficiency. Transient response tests further demonstrate the system’s robustness against load and voltage fluctuations, maintaining output voltage stability within an 18 V overshoot and a 20 V undershoot during load changes, and achieving rapid response times as low as 0.2 s. Comparative evaluations against conventional methods underscore the superiority of the proposed control strategy in terms of both performance and implementation simplicity. By harnessing the strengths of ANFIS-based voltage regulation and predictive current control, this scheme offers a robust solution to power quality issues in AC/DC boost rectifiers, promising substantial energy savings and improved grid stability. The results affirm the potential of the proposed system to set new benchmarks in power factor correction technology.

Abstract Cutaneous T‐cell lymphomas (CTCLs) are unique non‐Hodgkin lymphomas due to the proliferation of skin‐homing T cells. Sezary syndrome (SS), a subtype of CTCL, typically presents as erythroderma with specific blood involvement and seldomly evolves from tumor‐stage mycosis fungoides (MF). The article examines the case of a 62‐year‐old female patient who developed SS while under atezolizumab treatment for tumor‐stage transformed MF. Postdiagnosis, she underwent treatments including topical corticosteroids, phototherapy, and other drug regimens, experiencing both complete responses and relapses over the years. Three years postdiagnosis, her condition deteriorated with facial tumor lesions, confirmed as MF. Despite multiple treatments, including oral drugs, radiation therapy, and a range of other therapeutic agents, her condition did not show sustained improvement. The patient was then administered atezolizumab, an antibody targeting programmed death ligand 1 (PD‐L1). Although initial results saw a disappearance of her skin tumors, the subsequent cycles led to symptoms consistent with stage IVA1 SS, coupled with an evident shift in malignant T‐cell clone dominance. Atezolizumab, a humanized monoclonal antibody, impedes the interaction of PD‐L1 with its receptor PD1, which is notably overexpressed in malignant T‐cells of SS. This case illustrates the emergence of SS during the course of atezolizumab treatment for a transformed MF condition, underlining the potential risks and complexities in using PD‐L1 inhibitors for treating CTCL.

Purslane was extensively described as a potent hypoglycemic and hypolipidemic agent owing to its rich content of antioxidants. Therefore, this report aimed to assess the renoprotective potentials of methanol (MO) and methylene chloride (MC) fixed oil extracts of purslane seeds in a diabetic nephropathy (DN) model. Methods Purslane seeds were extracted using absolute methanol and methylene chloride, and type-1 diabetes was induced with a single 55 mg/kg dose of Streptozotocin (STZ) dissolved in 100 mmol/L citrate buffer (pH 4.5), and then diabetic animals were received MO, MC, for 42 consecutive days to compare their antidiabetic effect relative to the reference drug “Losartan”. Turnover is a common occurrence where employees choose to leave an organization. Despite previous research on turnover intentions and job satisfaction, the impact of work experience and perceived alternative job opportunities, especially during economic crises, has not been adequately studied. This study aims to explore the relationship between turnover intentions and job satisfaction, considering factors of work experience and perceptions of other job opportunities. This study was conducted in Lebanon, and 156 educators were surveyed via electronic questionnaires. The data analysis included descriptive statistics, Pearson correlation, and hierarchical regression modeling. The findings reveal a significant negative correlation between turnover intention and job satisfaction (r = –0.315*, p < 0.05). Perceived alternative job opportunities do not show a significant correlation with job satisfaction (ß = –0.028, p > 0.05), thus having no moderating role. However, work experience significantly moderates the relationship between job satisfaction and turnover intentions (ß = –0.052*, p < 0.05). Despite the negative impact on job satisfaction, Lebanon’s economic crisis affects teachers’ decision to stay due to limited job opportunities. Abstract Context Recently, prioritize has been given to using natural phytogenic or nano compounds as growth promoters and immunostimulants in fish diets as an alternative to antibiotics. Aims The main propose of this trial was to determine the impact of supplementing diets with spirulina or curcumin nanoparticles on the performance and health indicators of Nile tilapia fingerlings. Methods In a 56-day feeding trial, 180 tilapia fingerlings were assigned into three main groups, as follows: 1st, control group, 2nd, Spirulina platensis (SP; 5 g kg-1 diet) and 3rd, curcumin nanoparticles (CUR-NPs; 30 mg kg-1 diet). Key results Incorporating tilapia diets with SP or CUR-NPs significantly improved performance, body chemical analysis, blood biochemical and hematological indices, digestive enzyme activities, and antioxidant and immunostimulant features compared to the control. Conclusion Fortified tilapia diets with CUR-NPs or SP efficiently boost the productivity and health of Nile tilapia fingerlings. Implications The research introduces new practical solutions for applying safe feed additives as alternatives to antibiotics in tilapia farming.

Abstract Background Coronary heart disease (CHD) is often associated with mental disorders (MDs). Comorbid MDs reduce the quality of life and increase cardiac morbidity and mortality. Nevertheless, there is little and inconsistent research on the management of MDs in CHD patients. To bridge this gap, this study aims to gain insight into the long-term course of MD-related health care for patients with CHD, in order to identify opportunities for care improvement. Methods In this prospective cohort study, we investigated whether CHD patients with or without expert-rated MD at baseline (N = 364) received different MD-related health care from either their general practitioner (GP) or cardiologist at follow-up, M = 2.7 [2.0–4.0] years later. In the follow-up assessment, N = 131 CHD patients participated and received questionnaires capturing sociodemographic, mental health, and MD-related health care characteristics. Abstract Reinforcement of the Internet of Medical Things (IoMT) network security has become extremely significant as these networks enable both patients and healthcare providers to communicate with each other by exchanging medical signals, data, and vital reports in a safe way. To ensure the safe transmission of sensitive information, robust and secure access mechanisms are paramount. Vulnerabilities in these networks, particularly at the access points, could expose patients to significant risks. Among the possible security measures, biometric authentication is becoming a more feasible choice, with a focus on leveraging regularly-monitored biomedical signals like Electrocardiogram (ECG) signals due to their unique characteristics. A notable challenge within all biometric authentication systems is the risk of losing original biometric traits, if hackers successfully compromise the biometric template storage space. Current research endorses replacement of the original biometrics used in access control with cancellable templates. These are produced using encryption or non-invertible transformation, which improves security by enabling the biometric templates to be changed in case an unwanted access is detected. This study presents a comprehensive framework for ECG-based recognition with cancellable templates. This framework may be used for accessing IoMT networks. An innovative methodology is introduced through non-invertible modification of ECG signals using blind signal separation and lightweight encryption. The basic idea here depends on the assumption that if the ECG signal and an auxiliary audio signal for the same person are subjected to a separation algorithm, the algorithm will yield two uncorrelated components through the minimization of a correlation cost function. Hence, the obtained outputs from the separation algorithm will be distorted versions of the ECG as well as the audio signals. The distorted versions of the ECG signals can be treated with a lightweight encryption stage and used as cancellable templates. Security enhancement is achieved through the utilization of the lightweight encryption stage based on a user-specific pattern and XOR operation, thereby reducing the processing burden associated with conventional encryption methods. The proposed framework efficacy is demonstrated through its application on the ECG-ID and MIT-BIH datasets, yielding promising results. The experimental evaluation reveals an Equal Error Rate (EER) of 0.134 on the ECG-ID dataset and 0.4 on the MIT-BIH dataset, alongside an exceptionally large Area under the Receiver Operating Characteristic curve (AROC) of 99.96% for both datasets. These results underscore the framework potential in securing IoMT networks through cancellable biometrics, offering a hybrid security model that combines the strengths of non-invertible transformations and lightweight encryption.

Abstract Person re-identification (ReID) aims to find the person of interest across multiple non-overlapping cameras. It is considered an essential step for person tracking applications which is vital for surveillance. Person ReID could be investigated either using image-based or video-based. Video-based person ReID is considered more discriminating and realistic than image-based ReID due to the massive information extracted for each person. Different deep-learning techniques have been used for video-based ReID. In this survey, recently published articles are reviewed according to video-based ReID system pipeline: deep features learning, deep metric learning, and deep learning approaches. The deep feature learning approaches are categorized into spatial and temporal approaches, while deep metric learning is divided into metric and metric learning approaches. The deep learning approaches are differentiated into: supervised, unsupervised, weakly-supervised, and one-shot learning. A detailed analysis is held for the architectures of the state-of-the-art deep learning approaches. And their performance on four benchmark datasets is compared.

Abstract Aim Investigate the influence of organizational intelligence on the development of soft skills and the overall thriving of nurses at Alexandria Main University Hospital in Egypt. Design A cross-sectional descriptive design following STROBE guidelines examined the relationship between organizational intelligence, soft skills, and nurses’ thriving. Methods and tools Data were collected from 740 nurses working across critical care units using structured questionnaires. The questionnaires assessed organizational intelligence, soft skills, and thriving at work. Sociodemographic characteristics, including age, gender, education, and nursing experience, were also collected. Statistical analyses were used to analyze the data, including ANOVA, t-tests, Pearson correlation, and stepwise regression. Results The findings revealed nurses’ positive perceptions of organizational culture and documentation skills. However, areas such as measures and rewards and staff relations and communication indicated opportunities for improvement. Nurses reported high vitality levels but needed more learning opportunities at work. Significant correlations were found between demographic variables, organizational intelligence, soft skills, and thriving. Organizational intelligence demonstrated a robust positive relationship with both soft skills and thriving. Age, gender, education, and experience significantly influenced nurses’ soft skills and thriving. Conclusion The study highlights the importance of organizational intelligence in enhancing nurses’ professional capabilities and well-being. Addressing demographic factors and fostering a supportive work environment is crucial for optimizing nursing practice and organizational effectiveness. Nursing implications Insights from this study can inform targeted interventions and policy decisions to enhance nursing practice, organizational development, and healthcare outcomes in Egypt. Fostering organizational intelligence and soft skills among nurses can improve patient care, increase job satisfaction, and overall organizational success.

The evolution of mega-ships and maritime alliances, in tandem with global changes, has intensified hub port competitiveness, increasing the need for innovation and adaptation, with a focus on sustainability. This study analyzed the competitiveness of the Mediterranean maritime hub port with Northern European ports. It provided a comprehensive analysis of the competitiveness of ten leading international hub ports in the Mediterranean and Northern Europe. Using the Hybrid F-AHP TOPSIS method, this research first identified and assigned weights to 10 criteria across five categories of port competitiveness from a shipping liner perspective in the Mediterranean and Northern Europe. It then ranked those ten leading ports based on the weighted criteria. The results showed that handling cost, operational efficiency, and infrastructure were the main factors for defining a competitive hub port, with growing emphasis on sustainability considerations. The results also highlight that Tangier Med, Algeciras, and Port Said were the leading ports in the region, excelling in technical efficiency and maritime costs compared to Northern European ports. The analysis further provided a holistic overview of competitiveness in the region, identifying the strengths and opportunities of each port based on 5 aspects with a special focus on the hub ports of the Southern Mediterranean. This research offers valuable insights for maritime shippers and port authorities in understanding hub port performance and competitiveness in a sustainable context.

The objective of this study is to examine the nature of the relationship between the internal work environment including its all-encompassing dimensions (organizational structure, participation in decision-making, incentives) and job alienation in Higher education institutions through focusing on faculty members at Jijel University in Algeria. That being the case, the study aims to fill a research gap by investigating the underexplored relationship between the Internal Work Environment and Job Alienation, hence while there is ample literature on work alienation, studies specifically focusing on work alienation within higher education institutions through the prism of internal work environment are notably scarce. The study utilized a descriptive approach, employing a survey sampling method to collect data from the target population with a specifically designed questionnaire for this purpose. The questionnaire consisting of 60 items was administered to a randomly selected sample of 167 faculty members at Jijel University. The collected data were analyzed through the Statistical Package for Social Sciences (SPSS), version 22. The study's findings illustrate that faculty members perceive their work environment as inadequate for carrying out their activities, coupled with a notably high level of job alienation. Additionally, the research underscores a significant correlation between the internal work environment, encompassing its various dimensions, and the prevalence of job alienation among faculty members at Jijel University. Background: The persistently rising cesarean section rate in Pakistan is largely driven by cultural preferences for larger families, primary cesarean sections, induction of labor, and the desire to avoid litigation. These practices have resulted in numerous complications, posing significant challenges for obstetricians worldwide. This study aimed to determine the maternal morbidity and mortality associated with multiple repeat cesarean sections. Methods: This cross-sectional study was carried out at the Department of Gynecology and Obstetrics, at a tertiary care hospital, Abbottabad. Patients (106), with four or more previous cesarean sections, (elective or emergency repeat CS), over 6 months (October 2022 to March 2023), were selected. The primary outcome including uterine scar dehiscence (USD), scar rupture, bladder injury, bowel injury, grade 2 adhesions, morbidly adherent placenta (MAP), obstetrical hysterectomy, intensive care admission, blood transfusion, prolonged hospital admission, and maternal mortality, were recorded through follow-up four weeks postpartum. Data was analyzed using SPSS vr20. Results: Cesareans performed were 798, and HOCS 106 (13.2%). Out of these, 95 (89.6%) had their fourth, 9(8.5%) fifth, and 2(1.8%) sixth CS. Abdominopelvic adhesions were seen in 53(50%), USD in 23(21.6%), MAP in 5.6% leading to obstetrical hysterectomy in 6(5.6 %), bladder & bowel injury occur in 2(1.9%). No maternal death was recorded. Conclusion: HOCS pose significant challenges. Reducing primary and elective repeat CS rates by promoting alternative delivery methods and trial of labor after one CS. Proper counseling and education on the risks of multiple cesarean sections and the use of modern contraceptive techniques are essential.

Abstract Background This qualitative study explores the experiences of peer support workers (PSWs) and service users (or peers) during transition from in-person to virtual mental health services. During and following the COVID-19 pandemic, the need for accessible and community-based mental health support has become increasingly important. This research aims to understand how technological factors act as bridges and boundaries to mental health peer support services. In addition, the study explores whether and how a sense of community can be built or maintained among PSWs and peers in a virtual space when connections are mediated by technology. This research fills a gap in the literature by incorporating the perspectives of service users and underscores the potential of virtual peer support beyond pandemic conditions. Methods Data collection was conducted from a community organization that offers mental health peer support services. Semi-structured interviews were conducted with 13 employees and 27 service users. Thematic analysis was employed to identify key themes and synthesize a comprehensive understanding. Results The findings highlight the mental health peer support needs that were met through virtual services, the manifestation of technology-based boundaries and the steps taken to remove some of these boundaries, and the strategies employed by the organization and its members to establish and maintain a sense of community in a virtual environment marked by physical distancing and technology-mediated interrelations. The findings also reveal the importance of providing hybrid services consisting of a mixture of in person and virtual mental health support to reach a broad spectrum of service users. Conclusions The study contributes to the ongoing efforts to enhance community mental health services and support in the virtual realm. It shows the importance of virtual peer support in situations where in-person support is not accessible. A hybrid model combining virtual and in-person mental health support services is recommended for better accessibility to mental health support services. Moreover, the importance of organizational support and of equitable resource allocation to overcome service boundaries are discussed.

47. Microbiome and mitogenomics of the chigger mite Pentidionis agamae: potential role as an Orientia vector and associations with divergent clades of Wolbachia and Borrelia

Abstract Background Trombiculid mites are globally distributed, highly diverse arachnids that largely lack molecular resources such as whole mitogenomes for the elucidation of taxonomic relationships. Trombiculid larvae (chiggers) parasitise vertebrates and can transmit bacteria (Orientia spp.) responsible for scrub typhus, a zoonotic febrile illness. Orientia tsutsugamushi causes most cases of scrub typhus and is endemic to the Asia-Pacific Region, where it is transmitted by Leptotrombidium spp. chiggers. However, in Dubai, Candidatus Orientia chuto was isolated from a case of scrub typhus and is also known to circulate among rodents in Saudi Arabia and Kenya, although its vectors remain poorly defined. In addition to Orientia, chiggers are often infected with other potential pathogens or arthropod-specific endosymbionts, but their significance for trombiculid biology and public health is unclear. Results Ten chigger species were collected from rodents in southwestern Saudi Arabia. Chiggers were pooled according to species and screened for Orientia DNA by PCR. Two species (Microtrombicula muhaylensis and Pentidionis agamae) produced positive results for the htrA gene, although Ca. Orientia chuto DNA was confirmed by Sanger sequencing only in P. agamae. Metagenomic sequencing of three pools of P. agamae provided evidence for two other bacterial associates: a spirochaete and a Wolbachia symbiont. Phylogenetic analysis of 16S rRNA and multi-locus sequence typing genes placed the spirochaete in a clade of micromammal-associated Borrelia spp. that are widely-distributed globally with no known vector. For the Wolbachia symbiont, a genome assembly was obtained that allowed phylogenetic localisation in a novel, divergent clade. Cytochrome c oxidase I (COI) barcodes for Saudi Arabian chiggers enabled comparisons with global chigger diversity, revealing several cases of discordance with classical taxonomy. Complete mitogenome assemblies were obtained for the three P. agamae pools and almost 50 SNPs were identified, despite a common geographic origin. Conclusions P. agamae was identified as a potential vector of Ca. Orientia chuto on the Arabian Peninsula. The detection of an unusual Borrelia sp. and a divergent Wolbachia symbiont in P. agamae indicated links with chigger microbiomes in other parts of the world, while COI barcoding and mitogenomic analyses greatly extended our understanding of inter- and intraspecific relationships in trombiculid mites.

Abstract Expression of serotonergic (5HT) receptors in patients with chronic rhinosinusitis with nasal polyposis and in normal nasal mucosa: a case-control study. Background Chronic rhinosinusitis with nasal polyposis is one of the challenging conditions regarding treatment and management. Many recent studies introduced new modalities for treatment like targeted immunomodulating drugs rather than antihistamines, local and systemic steroids, and endoscopic sinus surgery. Understanding the pathology behind the disease by studying its immunologic pathways and substances that take part in the inflammatory process can help in the introduction of new treatment options that can improve outcomes and decrease the costs of treatment. Objective The aim of our study is to demonstrate the presence of 5-HT receptors in nasal polyps and inferior turbinate mucosa in patients with chronic rhinosinusitis with nasal polyposis and then compare this expression of 5-HT receptors with inferior turbinate mucosa of the normal control group. Methods A case-control study was conducted on 80 patients. Specimens from nasal polypi and turbinate mucosa of patients with chronic rhinosinusitis (cases) and specimens from normal turbinate mucosa (controls) were stained with serotonin receptor Immunohistochemical staining by automated immunostainer in the pathology department laboratory in Ain Shams University Hospital with ratio 1:200–400 as recommended by the manufacturing company. Then, compared as regards stain distribution and intensity of mucosal lining, glands, and blood vessels. Results There was a significant difference between polyp cases and controls as regards lining epithelium and mucosal gland stain distribution and stain intensity. Also significant difference between the polyp cases and controls as regards blood vessel strain distribution. However, no significant difference was found as regards blood vessel stain intensity. There was a significant difference between turbinate cases and controls as regards lining epithelium stain distribution, and stain intensity. There was no significant difference between turbinate cases and controls as regards gland stain distribution. However, a significant difference was found between turbinate and controls as regards gland stain intensity. There was a significant difference between turbinate cases and controls as regards blood vessel stain distribution; however, no significant difference was present as regards stain intensity. There was no significant difference between polyp and turbinate cases as regards lining epithelium and blood vessel stain distribution and stain intensity. There was no significant difference between polyp and turbinate cases as regards gland stain distribution; however, a highly significant difference between polyp and turbinate cases as regards gland stain intensity. Conclusion Serotonin receptors are highly expressed in patients with chronic rhinosinusitis with nasal polyposis in both turbinate and polypoidal tissues which proves that serotonin has a strong role in the formation and growth of nasal polypi and allergic reactions. Hence serotonin modulating drugs can be studied as a new therapy for chronic rhinosinusitis with nasal polyposis.

MiR-762 can stimulate the proliferation and metastasis of various types of cancer. Hippo pathway is one of the pathways that regulate tissue development and carcinogenesis. Dysregulation of this pathway plays a vital role in the progression of cancer. This study aimed to evaluate the possible correlation between miR-762, the Hippo signaling pathway, TWIST1, and SMAD3 in patients with lung cancer, as well as patients with chronic inflammatory diseases. The relative expression of miR-762, MST1, LATS2, YAP, TWIST1, and SMAD3 was determined in 50 lung cancer patients, 30 patients with chronic inflammatory diseases, and 20 healthy volunteers by real-time PCR. The levels of YAP protein and neuron-specific enolase were estimated by ELISA and electrochemiluminescence immunoassay, respectively. Compared to the control group, miR-762, YAP, TWIST1, and SMAD3 expression were significantly upregulated in lung cancer patients and chronic inflammatory patients, except SMAD3 was significantly downregulated in chronic inflammatory patients. MST1, LATS2, and YAP protein were significantly downregulated in all patients. MiR-762 has a significant negative correlation with MST1, LATS2, and YAP protein in lung cancer patients and with MST1 and LATS2 in chronic inflammatory patients. MiR-762 may be involved in the induction of malignant behaviors in lung cancer through suppression of the Hippo pathway. MiR-762, MST1, LATS2, YAP mRNA and protein, TWIST1, and SMAD3 may be effective diagnostic biomarkers in both lung cancer patients and chronic inflammatory patients. High YAP, TWIST1, SMA3 expression, and NSE level are associated with a favorable prognosis for lung cancer.
