1. Building Timely Consensus Among Diverse Stakeholders: An Adapted Nominal Group Technique

Abstract

INTRODUCTION The participatory research approach is an important tool for family medicine and primary health care research. (1) By recognizing stakeholders' expertise and lived experiences, (2) this approach increases the [...], PURPOSE Building timely consensus among diverse stakeholders is important in primary health care research. Consensus can be obtained using the nominal group technique which includes 5 steps: (1) introduction and explanation; (2) silent generation of ideas; (3) sharing ideas; (4) discussion; and (5) voting and ranking. The main challenges in using this technique are a lack of representation of different stakeholder opinions and the amount of time taken to reach consensus. In this paper, we demonstrate how to effectively achieve consensus using an adapted nominal group technique that mitigates the challenges. METHODS This project aimed to reach consensus on the priority care domains for individuals aged 65 or older, using an adapted nominal group technique with 4 strategies: (1) recruit 4 stakeholders groups (older people, clinicians, managers, decision makers) by using maximum variation and snowballing sampling approaches; (2) use remote tools to ensure high participation; (3) add an individual pre-elicitation activity to increase effectiveness; and (4) adapt discussions to the stakeholders' preferences for meaningful engagement. RESULTS In total, 28 diverse stakeholders participated. After the pre-elicitation activity and 1 round of group discussion, we reached consensus on a priority domain called symptoms, functioning, and quality of care. Adaptive group discussions and remote tools were the most effective strategies. All participants strongly agreed that they were able to express their views freely. Some perceived a need for emphasizing the alignment between the research objectives and anticipated practice and policy implications. CONCLUSIONS This adapted nominal group technique is an effective and enriching method when timely consensus is needed among diverse stakeholders. Abstract

Background: Previous studies on the impact of the coronavirus disease 2019 (COVID-19) pandemic on persons living with dementia (PLWD) were mostly conducted in a single jurisdiction or focused on a limited number of outcomes. Our study estimates the impact of the first two pandemic waves on emergency department (ED) visits (all-cause/ambulatory care sensitive conditions), hospitalizations (all-cause/30-day readmissions), and all-cause mortality in four Canadian jurisdictions. Methods: Using administrative databases from Alberta, Ontario, Saskatchewan, and Quebec, we assembled two closed retrospective cohorts (2019/pre-pandemic control and 2020/pandemic) of PLWD aged 65+. Within community and nursing home settings, the rates of the above-mentioned outcomes in three pandemic periods (first wave, interim period, second wave) were compared to the corresponding pre-pandemic periods. We performed random effects meta-analyses on the provincial incident rate ratios. Results: Pre-pandemic and pandemic cohorts included 167,095 vs. 173,240 (community) and 93,374 vs. 92,434 (nursing home) individuals, respectively. During the first wave, community and nursing home populations experienced significant declines in the rates of all-cause ED visits (36% vs. 40%) and hospitalizations (25% vs. 22%), which persisted in the following periods in the community. These declines were greater for the rates of ambulatory care sensitive condition ED visits and 30-day readmissions. Mortality was 36% higher in nursing homes (first wave) and 13% higher in the community (second wave). Conclusions: It is key to prepare for future health crises and ensure that PLWD receive necessary care and services and do not have such a high mortality rate. Abstract

Background: Virtual care (VC) for dementia in primary care settings is an important aspect of healthcare delivery in Canada. However, the evidence informing optimal and sustainable provision of VC for persons living with dementia (PLWD) and their care partners is scarce. The objectives of this study were to (1) describe the frequency of VC use, (2) identify characteristics of PLWD, care partners, and family physicians (FPs) that are associated with the use of VC, and (3) explore FPs' perceptions of barriers and facilitators to provide VC for PLWD and their care partners. Methods: The Alzheimer Society of Canada and College of Family Physicians of Canada conducted three nationwide cross-sectional surveys between October 2020 and April 2021: (1) One with PLWD, (2) one with care partners of PLWD, and (3) one with FPs. Virtual care was defined as two-way synchronous communication by telephone and/or a web camera. The prevalence of VC use among FPs, PLWD, and care partners was described. Logistic regression models were used to determine characteristics of participants (sociodemographic, urbanicity, frequency and availability of support for connecting with FPs, and FPs' practice characteristics) associated with any VC use (phone and/or video). Inductive thematic analysis of open-ended questions explored FPs' perceptions. Results: 131 PLWD, 341 care partners, and 125 FPs participated. 61.2% of PLWD, 59.5% of care partners, and 77.4% of FPs reported using VC. The models for PLWD (included age and ethnicity) and care partners (included gender/sex, urbanicity, and receiving support from a family member/friend to connect with FP) were inconclusive. FPs with > 20 years in practice were less likely to provide VC (OR = 0.23, 95%CI: 0.08–0.62, p < 0.01). FPs perceived that preferences regarding virtual vs. in-person care, office/family support, technology and family presence, and remuneration for FPs influenced VC use. Conclusions: Virtual primary dementia care uptake in Canada is substantial and mainly performed via telephone. According to FPs, physician-patient-caregiver partnerships and infrastructure for VC play key roles in using VC. Virtual care could facilitate access to primary care and minimize potential disruptions to in-person care for PLWD. Outcomes of virtual primary care for dementia need further investigation. 13. METHODS FOR ESTIMATING MULTIREGIONAL INPUT-OUTPUT-TABLES

Abstract

Purpose – This report aims to address the methods used to obtain multi-regional input-output tables (MRIO-tables). Method – The research focuses on three gravity model of trade methods: simple gravity model estimation, doubly constrained gravity model estimation, and gravity model estimation with calibrated error function minimization. These methods are used for estimating and modelling multiregional trade flows, specifically in the context of MRIO-tables. These methods will be denoted as method 1, method 2 and method 3. Through a comparative analysis, the study focuses on the strengths and limitations of these methods and provides valuable insights for policymakers and researchers in the field. The findings contribute to a better understanding of the differences between the methods and their effectiveness in accurately representing MRIO-tables. Findings – This study evaluates three methods (mentioned above) for estimating multiregional trade flows, highlighting their performance. Method 1 and 2, exhibited similarities in their approach to estimating trade flows, both surpassing the performance of method 3 across various evaluation metrics. According to the results, method 1 and 2 are better than method 3 in accurately estimating multiregional trade flows. 14. Measuring Potentially Avoidable Acute Care Transfers From Long-Term Care Homes in Quebec: a Cross Sectional Study.

Abstract

Background Potentially avoidable emergency department transfers (PAEDTs) and hospitalizations (PAHs) from long-term care (LTC) homes are two key quality improvement metrics. We aimed to: 1) Measure proportions of PAEDTs and PAHs in a Quebec sample; and 2) Compare them with those reported for the rest of Canada. Methods We conducted a repeated cross-sectional study of residents who were received at one tertiary hospital between April 2017 and March 2019 from seven LTC homes in Quebec, Canada. The MedUrge emergency department database was used to extract transfers and resident characteristics. Using published definitions, PAEDTs and PAHs were identified from principal emergency department and hospitalization diagnoses, respectively. PAEDT and PAH proportions were compared to those reported by the Canadian Institute for Health Information. Results A total of 1,233 transfers by 692 residents were recorded, among which 36.3% were classified as being potentially avoidable: 22.8% 'PAEDT only', 11.6% 'both PAEDT & PAH', and 1.9% 'PAH only'. Shortness of breath was the most common reason for transfer. Pneumonia was the most common diagnosis from the 'both PAEDT & PAH' category. PAEDTs and PAHs accounted for 95% and 37% of potentially avoidable transfers, respectively. Among 533 hospitalizations, 31.3% were PAHs. These proportions were comparable to the rest of Canada, with some differences in proportions of transfers due to congestive heart failure, urinary tract infection, and implanted device management. Conclusions PAEDTs far outweigh PAHs in terms of frequency, and their monitoring is important for quality assurance as they may inform LTC-level interventions aimed at their reduction. Abstract

Abstract Aim To evaluate the effects of montelukast in smoke-induced lung injury. Methods 28 Wistar-Albino rats were enrolled into 4 groups with 7 rats per group. The healthy control group was exposed to fresh air while all rats in the 3 experimental groups were exposed to cigarette smoke for 20 weeks for 2 hours per day. After histopathological verification of smoke induced lung injury, montelukast (0.1 mg/kg) dissolved in Na2CO3 was given in one group (MON), Na2CO3 only was given in another group (MON control) and placebo was injected in the third group (COPD control) intraperitoneally for 21 days. At the end of this period blood samples were obtained for serum TNF-α assessment and light and electron microscopy analyses were performed on the lung tissues of sacrificed rats. Results Serum TNF-α levels in the MON group were significantly lower than in the MON control and COPD control groups (38.84 ± 4.9 pg/ml, 77.5 ± 5.8 pg/ml and 79.2 ± 6.9 pg/ml respectively, p < 0.05). Furthermore there was no statistically significant difference between the MON group and healthy controls with respect to serum TNF-α levels (38.84 ± 4.9 pg/ml vs. 29.5 ± 3.6 pg/ml, p > 0.05). Light and electron microscopic evaluation of the lungs demonstrated that the total histopathological damage score of the lung samples was significantly lower in the MON group than in MON controls and COPD controls (5.14 ± 0.5, 8.4 ± 0.6 and 8.7 ± 0.4 respectively, p < 0.05), while there was no significant difference between the MON group and healthy controls (5.1 ± 0.6 vs 2.3 ± 0.2, p > 0.05). Conclusion These findings suggest that montelukast might have a protective effect on smoke-induced lung injury in rats both from a histopathological and inflammatory point of view. more...

Abstract

The COVID-19 is a global pandemic declared as a result of the spread of a novel coronavirus having emerged in Wuhan, China in December 2019. While the restrictive and protective measures have helped minimize the adverse effects of the pandemic threatening the physical health of societies, they have brought destructive impacts on mental health. Undoubtedly, every segment of the society has been affected by the pandemic, and the elderly, who are in the high-risk group, are the most vulnerable group against the virus due to the epidemiology of the virus and the characteristics of their life-period. Understanding their loneliness, anxiety of being sick, and fear of death, introduced by social isolation measures, and producing policies strengthening their mental health are as important and vital as protecting their physical health. This study aimed to examine the impact of the COVID-19 pandemic on older adult mental health in the light of the relevant literature and current data and offer viable, community-based social work intervention recommendations by evaluating these effects from a social work perspective., Covid 19 pandemisi, 2019 yılının Aralık ayında Çin’in Wuhan kentinde ortaya çıkan yeni bir tür koronavirüsün tüm dünyaya yayılması neticesinde ilan edilmiş bir küresel salgındır. Fiziksel sağlığı tehdit eden pandeminin olumsuz etkilerini minimize etmek amacıyla alınan kısıtlayıcı ve koruyucu tedbirler toplumun fiziksel sağlığını korumaya yardımcı olurken ruh sağlığı anlamında yıkıcı etkileri beraberinde getirmiştir. Pandemi sürecinden toplumun şüphesiz her kesimi etkilenmiş olup yüksek riskli grupta yer alan yaşlılar, virüsün epidemiyolojisi gereği ve içinde bulundukları yaşam dönemi özellikleri gereği virüse karşı en savunmasız grubu oluşturmaktadır. Yaşlı bireylerin bu süreçte deneyimledikleri sosyal izolasyon ile beraber gelen yalnızlaşmayı, hasta olma kaygısını, ölüm korkusu anlamak ve yaşlıların ruh sağlığını güçlendirici politikalar üretmek, en az yaşlı bireylerin fiziksel sağlığını korumak kadar önemli ve yaşamsaldır. Abstract

This retrospective analysis of the phase III GOYA study investigated the prognostic value of baseline metabolic tumor volume parameters and maximum standardized uptake values for overall and progression-free survival in treatment-naive diffuse large B-cell lymphoma. Baseline total metabolic tumor volume (determined for tumors >1 mL using a threshold of 1.5 times the mean liver standardized uptake value +2 standard deviations), total lesion glycolysis, and maximum standardized uptake value positron emission tomography data were dichotomized based on receiver operating characteristic analysis and divided into quartiles by baseline population distribution. Of 1,418 enrolled patients, 1,305 had a baseline positron emission tomography scan with detectable lesions. Optimal cut-offs were 366 cm3 for total metabolic tumor volume and 3,004g for total lesion glycolysis. High total metabolic tumor volume and total lesion glycolysis predicted poorer progression-free survival, with associations retained after adjustment for baseline and disease characteristics (high total metabolic tumor volume hazard ratio: 1.71 [95% CI, 1.35-2.18]; total lesion glycolysis hazard ratio: 1.46 [95% CI, 1.15-1.86]). Total metabolic tumor volume was prognostic for progression-free survival in subgroups with International Prognostic Index scores 0-2 and 3-5, and those with different cell-of-origin subtypes. Maximum standardized uptake value had no prognostic value in this setting. High total metabolic tumor volume associated with high International Prognostic Index or non-germinal center B-cell classification identified the highest-risk cohort for unfavorable prognosis. In conclusion, baseline total metabolic tumor volume and total lesion glycolysis are independent predictors of progression-free survival in patients with diffuse large B-cell lymphoma after first-line immunochemotherapy. more...

OBJECTIVE: This study was carried out for the purpose of determining the factors that affect the usage of healthcare services by elderly individuals.MATERIAL AND METHODS: 266 elderly patients who visited a family health center between the dates of 01.02.2019 – 01.06.2019 were randomly selected, and the questionnaires were applied to those who agreed to participate in the study with the method of face-to-face interviews. Sociodemographic characteristics as age, marital status, educational level, employment status, gender, scope of health insurance, household income and chronic disease status were given as frequencies and percentages. Fisher’s exact test was used to analyze the relationships among usage of healthcare services and marital status and household income. Simple binary logistic regression analysis was used to determine which factor was a significant predictor of healthcare usage.RESULTS: It was determined that gender, perceived health status and having a chronic disease were significantly related to usage of healthcare services, while usage of healthcare services among women was 2.23 times more than that in men, and it was 3.27 times more among those with at least one chronic disease than those without.CONCLUSIONS: The data in our study may be used by healthcare policymakers and program administrators for the purpose of increasing the access to healthcare services among the elderly and especially for the sake of promotion and improvement of health campaigns. This way, it may be achieved to increase the probability of reaching healthcare counseling after being diagnosed with a chronic disease, and thus more severe complications and injuries that may occur may be prevented., AMAÇ: Bu çalışma, yaşlı bireylerin sağlık hizmeti kullanımını etkileyen faktörleri incelemek amacıyla yapılmıştır.GEREÇ VE YÖNTEM: Bir aile sağlığı merkezine 01.02.2019 – 01.06.2019 tarihleri arasında başvuran 266 yaşlı hasta rastgele seçilerek, araştırmaya katılmayı kabul eden yaşlılara yüz-yüze görüşme tekniği ile anketler uygulandı. Yaş, medeni durum, eğitim düzeyi, iş durumu, cinsiyet, sağlık sigortası kapsamı, hane halkı geliri, kronik hastalık durumu gibi sosyodemografik özellikler sayı ve yüzde olarak verildi. Sağlık hizmeti kullanımı ile medeni durum ve aylık hanehalkı geliri arasındaki ilişkilerin analizinde, Fisher exact testi kullanıldı. Hangi faktörün sağlık hizmeti kullanımının önemli bir yordayıcısı olduğunu belirlemek için basit ikili lojistik regresyon analizi yapıldı.BULGULAR: Cinsiyet, algılanan sağlık durumu ve kronik bir hastalığa sahip olmanın sağlık hizmeti kullanımı ile istatistiksel olarak anlamlı şekilde ilişkili olduğu, kadınların sağlık hizmetlerini kullanım sıklığının erkeklere oranla 2,23 kat, en az bir kronik hastalığa sahip olan yaşlıların ise olmayanlara göre 3,27 kat fazla olduğu saptandı.SONUÇ: Çalışmamızdaki veriler sağlık politikaları yapanlar ve program yöneticileri tarafından yaşlılar için sağlık hizmetlerine erişimin arttırılması amacıyla, özellikle sağlığın teşviki ve geliştirilmesi kampanyalarında kullanılabilir. Abstract

Home healthcare is a service model forwhich there is a currently growing need, especially for elderly patients. Theexamination of the home healthcare requirement risks under certain headings isof great importance for institutions or units providing home healthcare and forindividuals applying for these services. To fill this gap, a 2-grade (yes/no)scale has been developed comprising 21 items to measure the home healthcarerequirement risks of elderly patients in Turkey. In the context of thedevelopment of the scale, following a pilot study, a trial form of the scalewas applied to 506 elderly individuals in the province of Burdur. Explanatoryand confirmatory factor analysis was applied to the data, using R program, SPSSvn 24 (IBM Statistical Package for Social Sciences) and Lisrel 8.7 software.The scale that emerged at the end of the analysis comprised two dimensions;“The General Health Status and Physical Autonomy of the Patient” and “TheSuitability of the Living Area of the Patient and the Economic Status”. The KMOvalue calculated for the scale was 0.603. the KR-20 internal consistencycoefficient calculated was 0.895 for the first dimension, 0.680 for the seconddimension and 0.840 for the whole scale., Evde bakım, günümüzde özellikle yaşlı hastaların sıklıkla ihtiyaç duyduğu bir hizmet modelidir. Evde bakım ve evde sağlık hizmeti veren kurum ve üniteler açısından bu hizmete başvuran bireylerin evde bakım ihtiyacı risklerinin belirli boyutlar altında incelenmesi büyük önem taşımaktadır. Bu boşluğun doldurulması amacıyla Türkiye’deki yaşlı hastaların evde bakım ihtiyacı riskini ölçmeye yönelik 21 maddeden oluşan 2 dereceli(evet/hayır) bir ölçek geliştirilmiştir. Ölçek geliştirme çalışması kapsamında pilot uygulamanın ardından, Burdur ilinden 506 yaşlı bireye ulaşılarak ölçeğin deneme formu uygulanmıştır. Veriler açımlayıcı ve doğrulayıcı faktör analizine tabi tutulmuştur. Veriler R programı, IBM Sosyal Bilimler için İstatistik Paket Programı (IBM Statistical Package for Social Sciences-SPSS 24) ve Lisrel 8.7 kullanılarak analiz edilmiştir. Analiz sonrasında ortaya çıkan ölçeğin, “Hastanın Genel Sağlık ve Fiziksel Özerklik Durumu” ve “Hastanın Yaşam Alanının Uygunluğu ve Ekonomik Durumu” olmak üzere iki boyuttan oluştuğu tespit edilmiştir. Abstract

Purpose: Natural killer (NK) cells are key effector cells for anti-CD20 monoclonal antibodies (mAb), such as obinutuzumab and rituximab. We assessed whether low pretreatment NK-cell count (NKCC) in peripheral blood or tumor tissue was associated with worse outcome in patients receiving antibodybased therapy. Patients and Methods: Baseline peripheral blood NKCC was assessed by flow cytometry (CD3(-) CD56(+) and/or CD16(+) cells) in 1,064 of 1,202 patients with follicular lymphoma treated with obinutuzumab or rituximab plus chemotherapy in the phase III GALLIUM trial (NCT01332968) and 1,287 of 1,418 patients with diffuse large B-cell lymphoma (DLBCL) treated with obinutuzumab or rituximab plus cyclophosphamide, doxorubicin, vincristine, and prednisone (G-CHOP or R-CHOP) in the phase III GOYA trial (NCT01287741). The prognostic value of tumor NK-cell gene expression, as assessed by whole-transcriptome gene expression using TruSeq RNA sequencing, was also analyzed. The association of baseline variables, such as treatment arm, was evaluated using multivariate Cox regression models using a stepwise approach. Results: In this exploratory analysis, low baseline peripheral blood NKCC was associated with shorter progression-free survival (PFS) in both follicular lymphoma [hazard ratio (HR), 1.48; 95% confidence interval (CI), 1.02-2.14; P = 0.04] and DLBCL (HR, 1.36; 95% CI, 1.01-1.83; P = 0.04), and overall survival in follicular lymphoma (HR, 2.20; 95% CI, 1.26-3.86; P = 0.0058). Low tumor NK-cell gene expression was associated with shorter PFS in G-CHOP-treated patients with DLBCL (HR, 1.95; 95% CI, 1.22-3.15; P < 0.01).
