The evidence for any benefit of systemic antibiotic therapy on pulp survival following extrusive luxation and avulsion is inconclusive. The aim of this study was to evaluate the effect of systemic antibiotic therapy with amoxycillin and tetracycline for 7 days on different aspects of dental pulp repair in a murine model.The right maxillary incisor of 180 4 to 8-week-old male Wistar rats underwent extrusive luxation or avulsion. The animals were then treated with saline solution (control), tetracycline, or amoxicillin administered intra-gastrically for 7 days, and euthanized at 7, 15, and 30 days post-operatively. The layer of odontoblast cells, reparative dentin deposition, Hertwig's epithelial root sheath, pulp necrosis, and periapical inflammatory infiltrate were histomorphologically analyzed and scored. Mann-Whitney, Kruskal-Wallis, and Dunn tests were used to compare mean scores for the different procedures, treatments, and times (p ≤ .05).Significant differences were observed between luxated and avulsed teeth (p .05). In luxated teeth, no differences were observed among treatments and times, except for more tertiary dentin deposition in the coronal third with the use of tetracycline compared with amoxicillin at 15 days (p .05). In avulsed teeth, higher scores for the layer of odontoblast cells and lower scores for periapical inflammatory infiltrate at 7 days and pulp necrosis at 7 and 15 days were observed in the amoxicillin group compared to the saline and tetracycline groups (p .05). At 30 days, however, no differences were observed among treatments.The use of systemic antibiotic therapy with tetracycline or amoxicillin for 7 days did not contribute to pulp repair following extrusive luxation or avulsion in rats. However, the results warrant further research into the use of amoxicillin for a more extended period of time in cases of avulsion.

Background Tooth avulsion is a common cause of tooth loss. However, there is a significant lack of knowledge about dental trauma and the appropriate emergency procedures to handle these cases. Aim The aim of this study was to evaluate the educational effect of notebook covers illustrated with figures and informative texts about tooth avulsion and replantation in sixth-grade primary school pupils. Material and Methods Sixth-grade pupils of two public school participated in this study. An experimental group consisting of 134 pupils received notebook covers illustrated with figures and informative texts about tooth avulsion and replantation, while a control group of 108 pupils received notebooks with neutral covers. An assessment of the educational effects on pupils was performed after 9 months, with a standardized questionnaire normally used to evaluate dental trauma. Statistical analysis was conducted with SPSS 21.0 (α = .05) for group description and comparison using chi-square and Student t tests. Results Compared with the control group, the experimental group showed statistically significant improvements in correct meaning of dental trauma (38% vs. 58.2%); procedures to follow in the case of dental avulsion (2.8% vs. 70.9%); storing the avulsed tooth in milk (18.5% vs. 76.9%); general knowledge about dental anatomy (61.1% vs. 95.5%); and the ideal time for the tooth to remain outside the mouth prior to replantation (20.4% vs. 59.7%). Conclusion The use of notebook covers illustrated with figures and informative texts about dental trauma led to a significant increase in the knowledge of schoolchildren about tooth avulsion, first aid, and replantation.

Dental avulsion is the most severe type of dentoalveolar trauma, and the treatment of choice for this type of injury should be the replantation of the exarticulated tooth. After conducting a review of the literature and holding discussions with experienced researchers and clinical specialists from different areas, the International Association of Dental Traumatology (IADT) proposed a protocol for tooth replantation, including a recommendation for systemic antibiotic therapy (SAT) to reduce the risk of root resorption. Several antibiotics have been tested, but tetracycline and amoxicillin are, by far, the most commonly used drugs for avulsion injuries. This paper reviews the literature on the prescription of SAT in cases of immediate and delayed replantation of accidentally avulsed permanent teeth, based on a critical analysis of full-length papers retrieved from the PubMed/Medline, Bireme and SciELO electronic databases. Original research reports and literature reviews published in English-language journals between 1966 and 2016, and directly or indirectly discussing the relation between antibiotic therapy and the replantation of permanent teeth, were searched using “tooth avulsion,” “systemic antibiotic therapy” and “tooth replantation” as key words. The prescription of antibiotics for tooth replantation was found to be a controversial subject, and randomized clinical studies are needed to determine whether antibiotics for tooth replantation are actual indicated. Until the results of these studies become available, the recommendation is to use SAT for the management of permanent teeth replanted after an accidental avulsion. The best prescription option is tetracycline for 7 days, and the second best prescription option is amoxicillin for 4 to 7 days.

Factors associated with the diagnosis, aetiology, and treatment of mandibular fractures occurring during the postoperative period following the removal of a lower third molar are discussed. The following databases were searched using specific key words: PubMed/MEDLINE, LILACS, Embase, and Scopus. The search yielded 124 cases. Sex, age, side, tooth position and angulation, bone impaction, relationship between the tooth and the inferior alveolar nerve, local pathological conditions, aetiology of the fracture, symptomatology, and time between surgery and fracture, as well as any displacement of the fracture and the treatment of the fracture, were evaluated. Material and Methods: Were examined clinical and radiographic records of 1,309 patients who underwent treatment of dentoalveolar trauma in the discipline of Integrated Clinic of the School of Dentistry of Araçatuba, UNESP, of which we selected 137 whose patients had concussion and subluxation injuries, with average age of 23.3 (SD - 10.96). The variables collected were: gender, age, history of previous and actual trauma, treatments performed, the presence of necrotic pulp, and time elapsed until the same trauma. The concussion and subluxation groups were subjected to statistical analyses using the SPSS 16.0 version software (α=0.05), Chi-square, and t-tests. Results: Of the 301 teeth involved, 49 (16.3%) suffered concussion and 252 (83.7%), subluxation, being the upper anterior teeth the most affected (75.1%) for both conditions. Subluxation and concussion traumas were more prevalent in men aged 10 to 20 years, most caused by cycling accidents (36.2%). There was a concomitant presence of crown fracture in 21% of cases of concussion and 34.7% of subluxation. Pulp necrosis was detected in 16.3% (concussion) and 27.1% (subluxation) (p=0.12), and most occurred within 6 months after the trauma (p=0.29). The pulp necrosis shows a positive correlation with motorcycle accidents (p=0.01), direct impact (p≤0.0001), crown fracture with pulp exposure (p≤0.0001), darkening of the crown (p=0.004) and spontaneous pain (p≤0.0001); and negative correlation with indirect impact (p≤0.0001). Conclusions: Although concussion and subluxation traumas are considered of minor degrees, they must be monitored, since the possibility of pulp necrosis exists, and its early treatment favors a good prognosis. A total of 28 incisors of 3 male dogs were extracted and replanted after 15 minutes, and randomly divided into 3 groups: Group I (n = 8) - endodontic treatment was performed before the extraction and replantation; Group II (n = 10) - endodontic treatment was performed 30 days after replantation and the root canal was filled with CH dressing; Group III (n = 10) - endodontic treatment was performed 30 days after replantation and root canals received temporary medication of paramonochlorophenol-Furacin followed by CH dressing. The animals were euthanized 90 days after replantation. The histomorphological events analyzed at the epithelial reattachment site were the intensity and extent of acute and chronic inflammatory processes, periodontal ligament (PDL) organization, the intensity and extent of acute and chronic inflammatory processes in the PDL space, root resorption, bone tissue, and ankylosis. Data were submitted to the Wilcoxon Signed Ranks Test for group comparison (α = 5%). In Groups I, II and III the periodontal ligament was regenerated and most of the resorption areas were repaired by newly formed cementum. The depth and extent of root resorption were significantly higher in Group II than in Group III. The use of paramonochlorophenol-furacin followed by CH dressing was more effective in controlling inflammatory root resorption after immediate tooth replantation. Introdução: a mucosa bucal é susceptível a lesões de origem externa ou interna, causados por diferentes agentes físicos, químicos e biológicos. Vários são os medicamentos testados para o tratamento das lesões da mucosa bucal. Objetivo: o propósito deste trabalho foi avaliar, microscopicamente em ratos, a reação do tecido conjuntivo subcutâneo ao implante de Hipoglós®. Material e Método: foram realizadas, no dorso de 15 animais incisões medindo 1 cm cada, com distância entre elas de 3 cm. A partir daí, dois grupos experimentais foram constituídos: Grupo I – Controle (tubo de polietileno vazio); Grupo II - Hipoglós® (tubo de polietileno contendo Hipoglós®), considerando os períodos de 7, 15 e 21 dias, com cinco animais em cada um. Resultados: na análise histológica, o Grupo II (Hipoglós®) mostrou ainda células inflamatórias aos 21 dias, apesar de estatisticamente esse grupo ter apresentado melhores resultados quando comparado ao grupo I (Controle). Conclusão: em razão dos seus componentes e da semelhança de resposta inflamatória com o grupo controle, a pomada Hipoglós® mostrou-se biocompatível em tecido conjuntivo subcutâneo de ratos e abre a perspectiva para a realização de outras pesquisas.Descritores: Vitamina A; Vitamina D; Óxido de Zinco; Tecido Conjuntivo.

Introduction: There is a high prevalence of dental trauma in patients in need of orthodontic treatment, so it is important the knowledge about the orthodontic approach in cases of traumatized teeth. Thus, this study conducted a systematic review about orthodontic approach in dental trauma. Materials and Methods: Two investigators performed a systematic review with meta-analysis. MEDLINE/PubMed and Cochrane Library supplied relevant data from studies published between February 2004 and September 2015 on the relationship between dental trauma and orthodontic treatment. Results: Susceptibility to pulp necrosis was the dichotomous outcomes measure evaluated by risk ratio (RR) and the corresponding 95% confidence intervals (CI). The quality was evaluated using the Oxford Centre for Evidence-Based Medicine (EbM) guidelines. A search strategy and application of eligibility criteria enabled selection and evaluation of four studies on orthodontic treatment in traumatized teeth. In total, 1.696 patients (average age 11.6 years) with orthodontic treatment over 24.2 months were studied, and 484 had already suffered dental trauma and undergone orthodontic treatment. The primary observed sequelae after orthodontic treatment indicated greater susceptibility to pulp necrosis in traumatized teeth compared to no previous trauma. The patients with orthodontically treated traumatized maxillary incisors were more susceptible to pulp necrosis (RR: 7.6; 95% CI: 3.64 to 15.87; p= 0.00001) compared to patients with previous dental trauma to the maxillary incisors and no subsequent orthodontic treatment. Conclusion: There is no well-established protocol for these cases; such treatment must be careful and respect the healing period post-trauma, when these teeth are more susceptible to pulp necrosis and pulp obliteration. The prognosis of tooth replantation is usually related to the need of endodontic treatment, which has a direct relationship with the occurrence of root resorptions. Several studies have been undertaken in an attempt to prevent, delay, or treat these complications, which are the main causes of loss of replanted teeth. This literature review examines research evidence on intracanal dressings and root canal filling materials used in cases of tooth replantation. A comprehensive search was performed in the Medline/Pubmed, Bireme and Scielo full-text electronic journal databases to retrieve English-language articles referring to these topics that had been published between 1964 and 2010. Calcium hydroxide (CH) remains the usually recommended choice as an intracanal medicament in replanted teeth; however, there is evidence to support the initial use of a corticosteroid-antibiotic combination such as Ledermix paste to control potential early resorption, prior to the introduction of CH where the beneficial effect in the treatment of progressive root resorption has been well proven. Regarding root filling materials, CH-containing sealers are a good option because of their biological properties. Accurate diagnosis and adequate treatment plan may constitute very complex tasks, particularly in tooth avulsion because several variables are involved. In addition to the technical knowledge and clinical experience directed toward the quality of treatment, patient education may favorably influence the survival of replanted teeth.

Se isso não ocorrer, o coágulo periapical se torna organizado e dificulta a reposição. Neste artigo, o paciente encaminhado para a clínica de traumatismo dentário apresentava luxação extrusiva do dente 11. O paciente relatou que havia sofrido um acidente de bicicleta 12 dias antes e teve lesões dentárias. Tentou-se fazer o reposicionamento sem sucesso e uma forma alternativa de tratamento foi necessária para resolver o caso. Reimplante intencional, que é a extração do dente e seu reimplante de forma proposital, foi indicado. Esta técnica permite a remoção do coágulo e o reposicionamento correto do dente extruído. Cuidado deve ser observado quanto ao tratamento endodôntico, para a prevenção de uma reabsorção relacionada a uma infecção. Curativo de demora com hidróxido de cálcio foi realizado por 30 dias antes da obturação final. Contenção, antibióticos sistêmicos e evitar maiores danos à superfície da raiz também são importantes. O controle de 38 meses mostrou características clínicas e radiográficas de normalidade e demonstra a disponibilidade desta técnica para a adversidade no trauma. In the treatment of extrusive luxation cases, it is important that the repositioning of extruded tooth in the socket is done as soon as possible. If this does not occur, periapical clot becomes organized and makes replantation difficult reposition. In this article the patient referred to the Clinics for dental trauma sustaining extrusive luxation of the maxillary right central incisor. The patient reported having suffered a bicycle accident 12 days before, which caused traumatic tooth injuries. The repositioning was attempted without success and an alternative form of treatment was necessary to solve the case. Intentional tooth replantation, which is the deliberate extraction of the tooth and its replantation, was indicated. This technique allows clot removal and correct repositioning of the extruded tooth. Care should be taken as endodontic treatment is required for the prevention of subsequent infection-related resorption. Intracanal dressing with calcium hydroxide was used for 30 days before final root filling. Splint, systemic antibiotics and avoidance of further damage to the root surface is also important. After 49 months, showed clinical and radiographic characteristics of normality and demonstrates the availability of this technique to adversity in trauma. No. of bitstreams: 0 Previous issue date: 2016-05-01 Objectives The aim of the study was to analyze the effects of ovariectomy on periodontal tissues following immediate tooth replantation by histomorphometric, immunohistochemistry, and μCT analysis. Materials and methods Eighty wistar rats (Rattus norvegicus albinos) with normal estrous cycles were randomly divided into two groups: ovariectomized (OVX) and Sham. Two months after surgery, the rats' upper right incisor was extracted followed by immediate reimplantation. The animals were sacrificed after 28, 45, and 60 days healing time. Histomorphometric and immunohistochemical analysis were performed by evaluation of PCNA and TRAP straining. Results The periodontal ligament was reinserted into the bone and cementum in the both groups. The immunohistochemical analysis revealed PCNA positive cells on the periodontal ligament in both groups at 28 days. Root resorption was noted at 45 days with immunoreactive cells for TRAP present in bone and tooth surface however no statistical differences between the groups were noticed. Histomorphometric analysis showed significant difference between groups in the periodontal ligament and root resorption parameters for the sub-items: intensity of chronic inflammatory infiltrate at 60 days (p < 0.01), the organization of the periodontal ligament at 28 days (p < 0.05), depth of root resorption at 45 days (p < 0.05) and at 60 days (p < 0.001). The μCT analysis showed multiple areas of bone resorption in association with OVX at 28 and 60 days with no significant differences between times in vivo. Conclusion The ovariectomy did not have significant influence in periodontal tissue parameters following tooth reimplantation. One of the primary objectives of endodontic treatment of teeth with pulp necrosis is the elimination of microorganisms from the root canal system, as effectively as possible, especially in cases with chronic periapical lesions. AIM: The purpose of this study was to analyze the response of the periapical tissue of dogs' teeth with chronic periapical lesions to endodontic treatment performed with utilization of metronidazole, calcium hydroxide, and an association of both as root canal dressings. METHODOLOGY: Forty root canals were submitted to pulpectomy and the root canals were kept exposed to the oral environment for 6 months. Then, they were submitted to biomechanical preparation and divided into 4 study groups with 10 specimens: group I - no root canal dressing; group II - calcium hydroxide; group III - metronidazole; group IV - calcium hydroxide associated to metronidazole. After 15 days, the root canals were filled with Fill Canal sealer. After 90 days, the animals were killed and the especimens processed for histological analysis. RESULTS: Calcium hydroxide dressing provided a significantly better outcome compared to other experimental groups (alpha = 0.01). Also, the results of the association of metronidazole and calcium hydroxide were similar to those observed for the metronidazole group. The worst results were obtained by the no root canal dressing group. CONCLUSION: The use of metronidazole alone or associated with Calcium hydroxide, did not improve periapical healing when compared to Calcium hydroxide dressing. Um dos principais objetivos do tratamento endodôntico de dentes com polpa necrosada é a eliminação máxima possível dos microrganismos presentes no sistema de canal radicular, principalmente nos casos que apresentam lesões periapicais crônicas. OBJETIVO: O presente trabalho teve como objetivo analisar a resposta dos tecidos periapicais de dentes de cães com lesão periapical crônica ao tratamento endodôntico utilizando como curativo de demora o metronidazol, o hidróxido de cálcio e a associação das duas substâncias. METODOLOGIA: Foram empregados 44 canais radiculares de 2 cães adultos, portadores de lesão periapical crônica induzida experimentalmente. Após o preparo biomecânico os dentes foram divididos em quatro grupos experimentais de acordo com o curativo de demora empregado: Grupo I - controle - sem curativo de demora; Grupo II - hidróxido de cálcio; Grupo III - associação de hidróxido de cálcio e metronidazol e Grupo IV - metronidazol. Após 15 dias todos os canais foram obturados com cimento Fill Canal e passados 90 dias os animais foram sacrificados. RESULTADOS: Os dados obtidos evidenciaram que o hidróxido de cálcio apresentou resultado superior aos demais tratamentos, com diferença estatísticamente significante (alfa = 0.01) e o metronidazol resultado semelhante à associação do hidróxido de cálcio com o metronidazol. Os piores resultados foram obtidos pelo grupo sem curativo de demora. CONCLUSÃO: O uso de metronidazol ou da sua associação com hidróxido de cálcio, não proporcionou melhoras no reparo quando comparado ao curativo de hidróxido de cálcio.

Its mechanism of action has some similarities to that of Ca(OH())2. The purpose of this study was to evaluate the repair process taking place in the delayed replantation of monkey teeth using calcium hydroxide and MTA as root canal filling materials. Five monkeys had their lateral incisors extracted and bench-dried for 60 minutes. After root canal preparation, the teeth were assigned to two groups according to root canal filling material: I, calcium hydroxide; and II, MTA. The same treatment sequence was followed for both groups: coronal seal, periodontal ligament removal, immersion of the tooth in 2% acidulated-phosphate sodium fluoride, irrigation of the socket with saline and replantation. Both groups exhibited replacement resorption, areas of ankylosis and absence of inflammatory root resorption. Statistically similar results (p > 0.05) were observed for both groups regarding replacement root resorption, but the groups differed significantly (p < 0.05) regarding the occurrence of ankylosis. MTA may be a viable clinical option for filling teeth submitted to delayed replantation, and is an acceptable option for treating replanted permanent teeth in order to prevent tooth resorption, particularly when dressing changes are not possible. Dental avulsion is the most severe type of traumatic tooth injuries because it causes damage to several structures and results in the complete displacement of the tooth from its socket in the alveolar bone. The ideal situation is to replant an exarticulated tooth immediately after avulsion because the extraoral time is a determinant factor for treatment success and for a good prognosis. However, it is not always possible. The success of replantation depends on a number of factors that may contribute to accelerate or minimize the occurrence of root resorption or ankylosis, among which is the type and characteristics of the medium used for temporary storage during the time elapsed between avulsion and replantation. Maintaining the tooth in an adequate wet medium that can preserve, as longer as possible, the vitality of the periodontal ligament cells that remain on root surface is the key to success of replantation. Recent research has led to the development of storage media that produce conditions that closely resemble the original socket environment, with adequate osmolality (cell pressure), pH, nutritional metabolites and glucose, and thus create the best possible conditions for storage. Although these storage media can now be purchased in the form of retail products, the most common scenario is that such a product will not be readily available at the moment of the accident This paper reviews the literature on the different storage media that have been investigated for avulsed teeth based on full-length papers retrieved from PubMed/Medline, Lilacs, BBO and SciELO electronic databases using the key words ‘storage medium’, ‘transportation medium’, ‘avulsion’, ‘tooth avulsion’, ‘replantation’, ‘tooth replantation’, ‘milk’ and ‘propolis’. After application of inclusion and exclusion criteria, 39 papers were selected and critically reviewed with respect to the characteristics, efficacy and ease of access of the storage medium. The review of the literature showed that a wide array of types of wet storage media have been evaluated in laboratory studies and clinical reports, including cell and tissue culture solutions like Hank's Balanced Salt Solution (HBSS); medical/hospital products developed specifically for organ storage purposes, such as Viaspan® and Euro-Collins®; culture media, like Minimum Essential Medium (MEM); saline; natural products like water, saliva, bovine milk and its variations, propolis, green tea, Morus rubra (red mulberry), egg white and coconut water; rehydrating solutions, like Gatorade® and Ricetral, and even contact lens solutions. Based on the literature, it could be stated that, so far, apart from Based on the literature, it could be stated that, so far, apart from solutions designed specifically for storage and culture purposes, regular pasteurized whole milk is the most frequently recommended and with the best prognosis among other solutions that are likely to be available at the scene of an accident, such as water, saline or saliva. Its advantages include its high availability, ready accessibility, physiologically compatible pH and osmolality (fluid pressure) with the root-surface adhered PDL cells, presence of nutrients and growth factors. However, there is not yet a single solution that fulfills all requirements to be considered as the ideal medium for temporary storage of avulsed teeth, and research on this field should carry on. Dentre os traumatismos dento-alveolares, a avulsão dentária é a mais severa por causar danos em muitas estruturas e por consistir num deslocamento total do dente de seu alvéolo ósseo. O tratamento ideal é o reimplante dentário imediatamente após avulsão, pois o tempo extra-bucal é fator determinante para um bom prognóstico e o sucesso do tratamento. Infelizmente, isto nem sempre é possível. O sucesso do reimplante depende de muitos fatores que podem acelerar ou retardar a reabsorção radicular ou mesmo a anquilose e entre eles estão o tipo e as características do meio de armazenamento no qual esse dente foi mantido entre a avulsão e o reimplante. Manter o dente num meio úmido adequado que possa preservar pelo maior tempo possível a vitalidade das células do ligamento periodontal na superfície radicular é o elemento-chave do reimplante bem sucedido. Pesquisas recentes levaram ao desenvolvimento de meios de armazenamento que tem condições muito próximas às do alvéolo dental, possuindo osmolalidade (pressão celular) adequada, pH, metabólitos nutrientes e glicose. Embora estes meios de armazenamento possam ser adquiridos comercialmente, a situação mais comum é que o produto não esteja facilmente acessível no momento do acidente. Frente a isto, o objetivo deste artigo é apresentar uma revisão da literatura sobre diversos meios de armazenamento para dentes avulsionados, considerando suas características, efetividade e acessibilidade. Para isto, foi realizada uma busca de artigos nas bases de dados PubMed/Medline, Lilacs, BBO e Scielo por meio das palavras-chave: ‘storage media’, ‘tooth replantation’, ‘tooth avulsion’, ‘milk’ e ‘propolis’. Após a avaliação dentro dos critérios de inclusão e exclusão, 39 artigos foram selecionados e os meios de armazenamento neles descritos foram criteriosamente estudados quanto às suas características, efetividade e acessibilidade. A revisão da literatura revelou uma grande variedade de meios úmidos de estocagem avaliada em estudos laboratoriais e casos clínicos, incluindo soluções para cultura de células e tecidos, como a Hank's Balanced Salt Solution (HBSS); produtos médico-hospitalares desenvolvidos especificamente para armazenamento de órgãos, como Viaspan® e Euro-Collins®; meios de cultura como o Meio Mínimo Essencial (MEM); solução fisiológica; produtos naturais, como água, saliva, leite bovino em suas diferentes apresentações, própolis, chá verde, Morus rubra, clara de ovo e água de coco; produtos reidratantes como Gatorade® e Ricetral, até mesmo soluções para lentes de contato. Com base na literatura, pode-se afirmar que até agora, com exceção das soluções específicas para armazenamento e culturas, o leite pasteurizado integral é o mais indicado e o que tem o melhor prognóstico dentre as substâncias que estejam mais provavelmente disponíveis no local do acidente, como água, solução fisiológica ou saliva. Suas vantagens são a grande disponibilidade, fácil acesso, pH fisiologicamente compatível e osmolalidade (pressão do líquido) com as células do ligamento periodontal aderidas à superfície radicular, presença de nutrientes e fatores de crescimento. Apesar disso, o fato é que não se tem um produto que preencha todos os requisitos para ser considerado ideal para o armazenamento temporário de dentes avulsionados e as pesquisas para tanto devem prosseguir.

O relato de caso teve por finalidade demonstrar a importância da abordagem multidisciplinar para o tratamento de fratura corono-radicular pós-traumatismo. Paciente de 18 anos de idade, gênero masculino, sofreu uma queda de bicicleta resultando em traumatismos dentários. O 16 sofreu fratura corono-radicular complicada e o 35 fratura corono-radicular não-complicada. Foi realizado tratamento endodôntico, extrusão dentária controlada, cirurgia periodontal para recuperação do espaço biológico, coroa de porcelana e restauração tipo onlay. O resultado estético e funcional foi alcançado. O paciente foi acompanhado por 2 anos onde foi observado um manchamento na interface dente/onlay do 16 e fratura da onlay do 35. Foi realizado um reparo na interface do 16 e confeccionada uma coroa de porcelana para o 35. Foi possível concluir que a multidisciplinaridade do plano de tratamento é de extrema importância para uma adequada resolução dos casos de traumatismos dentoalveolares. The development of an accurate diagnosis and appropriate treatment plan can be a complex task, especially in cases of dentoalveolar trauma. The authors present a case report of crown-root fracture caused by trauma and highlight the importance of a multidisciplinary approach for the treatment. An eighteen year-old boy had a bicycle accident resulting in dental trauma. The upper right first molar showed a complicated crownroot fracture and the lower left second pre-molar showed an uncomplicated crown-root fracture. Endodontic treatment, controlled tooth extrusion, periodontal surgery for recovery of biological width, and porcelain crown and onlay restorations were performed. Esthetic and functional results were achieved. At the two-year follow-up it was observed that the tooth/onlay interface of the upper right first molar was stained and the onlay of the left lower second pre-molar was fractured. Therefore, the interface stained was repaired and a porcelain crown was made for the lower second premolar. The clinical case presented herein leads to the conclusion that a multidisciplinary treatment plan is extremely important for a proper resolution in cases of dentoalveolar trauma. Material e Método: Quarenta ratos Wistar após a extração do incisivo superior direito foram divididos em 4 grupos (n=10). No Grupo I (controle), os dentes foram reimplantados após 5 min extralveolar. Nos outros grupos o reimplante foi realizado após 60 min. No Grupo II, os canais foram obturados com pasta hidróxido de cálcio e os dentes foram reimplantados sem tratamento da superfície radicular. No Grupo III, os dentes foram imersos em ácido cítrico (pH1) por 3 min, os canais foram obturados com pasta de hidróxido de cálcio e reimplantados. No Grupo IV, a superfície radicular foi friccionada com gaze embebida em ácido cítrico e os dentes reimplandos. Os aminais sofreram eutanásia 60 dias após o reimplante. Resultados: Quanto à reabsorção por substituição houve diferença estatisticamente controle e do Grupo IV quanto a anquilose. O grupo controle apresentou menor reabsorção por substituição em comparação com os outros grupos (p < 0,05). A estrutura radicular foi mais comprometida pela reabsorção por substituição e anquilose no Grupo IV quando comparado aos grupos tratamento do ligamento periodontal cementário com ácido citrico reimplante dentário tardio em rato. Objective: to evaluated the effect of treating root-adhered necrotic periodontal ligament (PDL) with citric acid on the healing process in delayed rat tooth replantation. Material and Methods: Forty Wistar rats, assigned to 4 groups (n=10), had their upper right incisor extracted and kept dry on a workbench. For Group I (control), the teeth were replanted after a 5 min extra-alveolar time. For the other groups, replantation was done after 60 min. In Group II, the root canals were filled with a calcium hydroxide-based paste and the teeth were replanted with no root surface treatment. In Group III, the teeth were immersed in citric acid (pH 1) for 3 min, the canals were filled with calcium hydroxide and the teeth were replanted. In Group IV, instead of the immersion, the roots were scrubbed with gauze soaked in citric acid and the teeth were replanted. The animals were sacrificed 60 days postoperatively. Results: Regarding replacement resorption, there was statistically significant difference (p < 0.05) between the control group and the other three groups. No statistically significant difference (p > 0.05) was found among the groups regarding the areas of inflammatory resorption. There was also a statistically significant difference (p < 0.05) between the control group and Group IV regarding ankylosis. The control group showed the least replacement resorption percent means compared to the other groups (p < 0.05). The root structure was more affected by replacement resorption and ankylosis in Group IV compared to the Groups II and III, but this difference was not significant statistically (p > 0.05). Conclusion: the treatment of root surface-adhered necrotic periodontal ligament with citric acid was not able to prevent the occurrence of ankylosis, root resorption and inflammatory resorption in delayed rat tooth replantation. Euro-Collins solution was developed for the preservation of organs for transplantation, whose characteristics have raised interest for its use as a storage medium for avulsed teeth before replantation. This study evaluated histologically and morphometrically the healing process of dog teeth replanted after storage in Euro-Collins solution or bovine milk.Eighty roots of 4 young adult mongrel dogs were randomly assigned to 4 groups (n = 20) and the root canals were instrumented and obturated with gutta-percha and a calcium hydroxide-based sealer. After 2 weeks, the teeth were extracted and subjected to the following protocols: GI (negative control), replantation immediately after extraction; GII (positive control), bench-drying for 2 hours before replantation; GIII and GIV, immersion in 10 mL of whole bovine milk and Euro-Collins solution at 4 degrees C, respectively, for 8 hours before replantation. The animals were sacrificed 90 days postoperatively. The pieces containing the replanted teeth were subjected to routine processing for histologic and histometric analyses under light microscopy and polarized light microscopy.Root resorption was observed in all groups. GII exhibited the greatest loss of dental structure (P.01), and inflammatory resorption was predominant in this group. Storage in milk showed poorer results than immediate replantation and storage in Euro-Collins solution (P.01). The teeth stored in Euro-Collins solution presented similar extension of root resorption and periodontal ligament reorganization to those of immediately replanted teeth.The findings of this study suggest that the Euro-Collins solution is an adequate storage medium for keeping avulsed teeth for up to 8 hours before replantation.

No. of bitstreams: 0 Previous issue date: 2009-12-01 Objective: To analyze the reasons for replacing amalgam and composite resin restorations of patients treated at the Integrated Clinic Discipline of the Dental School of Araçatuba (UNESP), SP, Brazil. Method: After examining the patients for data collection to obtain a diagnosis, the treatment plan was outlined using a clinical form containing the restorations to be replaced, the reasons for replacing, the restorative material of choice and the number of restored surfaces. Next, all restorations indicated for replacement were examined as to their real need of replacement by 5 calibrated examiners taking into account the theorical concepts on the proposed criteria for the need of replacing or not. The analyzed period comprised 5 years (2001 to 2005). The criteria adopted for replacement were attached to the data collecting form in order to standardize the analysis of the restorations. Results: 856 patients were treated within the studied period and 753 needed restoration replacement. The main reasons for replacing amalgam restorations were defective marginal adaptation (40.9%), recurent caries (24.1%) and deficient anatomical form (15.4%). The main reasons for replacing resin restorations were esthetics (31.4%), defective marginal adaptation (29.2%) and recurent caries (20.7%). Conclusion: In spite of the little clinical experience of the studied population (undergraduate students), the reasons for replacing restorations were in accordance with the literature, having estehtics as the main reason for the replacement of composite resin restorations and defective marginal adaptation for amalgam restorations. No. of bitstreams: 0 Previous issue date: 2008-12-01 The purpose of this retrospective study was to analyze the cases of traumatic dental injuries involving root fracture and/or periodontal ligament injury (except avulsion) treated at the Discipline of Integrated Clinic, School of Dentistry of Araçatuba, São Paulo State University (UNESP), Brazil, from January 1992 to December 2002. Clinical and radiographic records from 161 patients with 287 traumatized teeth that had sustained root fracture and/or injuries to the periodontal ligament were examined. The results of this survey revealed that subluxation (25.09%) was the most common type of periodontal ligament injury, followed by extrusive luxation (19.86%). There was a predominance of young male patients and most of them did not present systemic alterations. Among the etiologic factors, the most frequent causes were falls and bicycle accidents. Injuries on extraoral soft tissues were mostly laceration and abrasion, while gingival and lip mucosa lacerations prevailed on intraoral soft tissues injuries. Radiographically, the most common finding was an increase of the periodontal ligament space. The most commonly performed treatment was root canal therapy. Within the limits of this study, it can be concluded that traumatic dental injuries occur more frequently in young male individuals, due to falls and bicycle accidents. Subluxation was the most common type of periodontal ligament injury. Root canal therapy was the type of treatment most commonly planned and performed. No. of bitstreams: 0 Previous issue date: 2008-06-01 Clinical practice has shown that most avulsed teeth are replanted after an extra-alveolar time that compromises the prognosis of replantation. In cases of delayed replantation, the use of adequate media for storage and transportation of the avulsed teeth may improve this prognosis considerably. Difficulties inherent to accidental dental avulsion include the lack of immediate access to ideal storage media, which accentuates the importance of saliva as a viable and readily available option. The authors report the case of an accidentally avulsed permanent maxillary central incisor that was kept into the patient's oral cavity from the moment of trauma until its replantation, 90 min later. Three years of follow-up revealed absence of root resorption, ankylosis or abnormal mobility, which demonstrates the feasibility of keeping avulsed teeth in saliva, at least when more indicated storage media are not available immediately. © 2008 The Authors. No. of bitstreams: 0 Previous issue date: 2006-09-01 One of the primary objectives of endodontic treatment of teeth with pulp necrosis is the elimination of microorganisms from the root canal system, as effectively as possible, especially in cases with chronic periapical lesions. AIM: The purpose of this study was to analyze the response of the periapical tissue of dogs' teeth with chronic periapical lesions to endodontic treatment performed with utilization of metronidazole, calcium hydroxide, and an association of both as root canal dressings. METHODOLOGY: Forty root canals were submitted to pulpectomy and the root canals were kept exposed to the oral environment for 6 months. Then, they were submitted to biomechanical preparation and divided into 4 study groups with 10 specimens: group I - no root canal dressing; group II - calcium hydroxide; group III - metronidazole; group IV - calcium hydroxide associated to metronidazole. After 15 days, the root canals were filled with Fill Canal sealer. After 90 days, the animals were killed and the especimens processed for histological analysis. RESULTS: Calcium hydroxide dressing provided a significantly better outcome compared to other experimental groups (α = 0.01). Also, the results of the association of metronidazole and calcium hydroxide were similar to those observed for the metronidazole group. The worst results were obtained by the no root canal dressing group. CONCLUSION: The use of metronidazole alone or associated with Calcium hydroxide, did not improve periapical healing when compared to Calcium hydroxide dressing. Foram utilizados 30 ratos Rattus norvegicus albinos da linhagem Wistar que tiveram seus incisivos centrais extraídos e deixados sobre a bancada por 6 h. As polpas foram extirpadas e os canais irrigados com solução de hipoclorito de sódio a 1%. Após o preparo endodôntico, a superfície radicular de cada dente foi tratada com solução de hipoclorito de sódio a 1% por 10 min (trocada a cada 5 min) seguida de soro fisiológico por 10 min, e os dentes foram divididos em três grupos com 10 espécimes em cada um. Nos Grupos I, II e III, respectivamente, a superfície radicular foi tratada com fluoreto de sódio fosfato acidulado a 2%, vitamina C e vitamina C efervescente (2 g, Redoxon®). Após obturação com pasta de hidróxido de cálcio os dentes foram reimplantados e os animais foram sacrificados aos 10 e 60 dias. O Grupo I apresentou maiores áreas de reabsorção por substituição e anquilose. Comparando as formas de vitamina C utilizadas, a efervescente (Grupo III) foi a que apresentou resultados mais favoráveis com mais áreas de anquilose e reabsorção por substituição que áreas de reabsorção inflamatória. This study evaluated microscopically the effects of root surface treatment with three different solutions in delayed rat teeth replantation. Central incisors from 30 rats (Rattus norvegicus, albinus Wistar) were extracted and left on a bench for 6 h. The pulps were extirpated and root canals were irrigated with 1% sodium hypochlorite. After endodontic treatment, the root surfaces of all teeth were submitted to a 10-min treatment with 1% sodium hypochlorite, changed every 5 min. The teeth were then rinsed with saline for 10 min and assigned to 3 groups with ten specimens each. Groups I, II and III were treated, respectively, with 2% acidulated-phosphate sodium fluoride, vitamin C solution and effervescent vitamin C (2 g, Redoxon®). After root surface treatment, the teeth were filled with calcium hydroxide and replanted. The animals were sacrificed after 10 and 60 days. Group I (fluoride) presented the largest areas of replacement resorption and ankylosis. Comparing both vitamin C groups, Group III (effervescent vitamin C) yielded better results, showing more areas of ankylosis and replacement resorption than areas of inflammatory resorption. Considerando o estudo do traumatismo dento-alveolar em indivíduos com essa condição, o objetivo do trabalho foi analisar o efeito da ovariectomia sobre os tecidos periodontais após o reimplante dentário imediato por meio da análise histomorfométrica, imunoistoquímica e micro-ct em ratas. Oitenta ratas Wistar (Rattus norvegicus, albinus) com ciclos estrais normais foram divididas aleatoriamente em dois grupos: Grupo ovariectomizadas (OVX), e Grupo Sham (grupo controle). Após 2 meses da ovariectomia, foi realizado extração dos incisivos superiores seguido de reimplante dentário imediato e os animais sofreram eutanásia aos 28, 45 e 60 dias após o reimplante. Foi realizada a análise histomorfométrica e imunoistoquímica com avaliação das proteínas PCNA, OPG, CASPASE-3, RUNX-2 e TRAP. Em todos os grupos houve reinserção do ligamento periodontal no tecido ósseo e cemento e a imunorreatividade à PCNA foi evidente em células periodontais independente do grupo Sham ou OVX. A reabsorção radicular externa esteve mais acentuada a partir dos 45 dias com células imunorreativas à TRAP presentes na superfície óssea e dentária. Todavia, uma considerável parcela também estava dispersa no ligamento periodontal em todos os grupos. Não houve diferença estatisticamente significante entre os grupos para as proteínas PCNA e TRAP. Recent researchs suggest estrogen deficiency leads to changes in periodontal tissues. In order to study dental traumatology under this condition, the aim of the study was analyze the effects of ovariectomy on periodontal tissues following immediate tooth replantation through histomorphometric, immunohistochemistry and ?-ct analysis in rats. Eighty Wistar rats (Rattus norvegicus albinos) with normal estrous cycles were randomly divided into two groups (n = 20 per group): 1) Group ovariectomized (OVX), and 2) Sham group (control group). After two months, the rats were followed by extraction of upper right incisor and immediate replantation. The rats were sacrificed immediately 28, 45 and 60 days after tooth reimplantation. Histomorphometric and immunohistochemical analysis was performed by evaluation of PCNA, OPG, CASPASE-3, RUNX-2 and TRAP proteins. Periodontal ligament was reinseted in the bone and cementum and the immunohistochemical analysis revealed PCNA expression in the periodontal ligament independent of Sham or OVX group. At 45 days, the external root resorption was greater with expression of TRAP proteins in the bone and tooth surface. However, in both groups, were also dispersed in the periodontal ligament. Tem-se recomendado a antibioticoterapia sistêmica, porém faltam estudos que demonstrem o seu real papel nesse processo. Portanto, o objetivo do trabalho foi avaliar o efeito da antibioticoterapia sistêmica com amoxicilina e tetraciclina após reimplante dentário imediato em ratos. Noventa ratos (Rattus norvegicus albinus, variedade Wistar) tiveram os incisivos extraídos e armazenados em 20 ml de solução de soro fisiológico por 5 minutos. Em seguida, os dentes foram reimplantados e os animais divididos em 3 grupos de acordo com o antibiótico administrado por via intragástrica (Técnica de Gavagem): Grupo I - soro fisiológico, Grupo II - Amoxicilina e Grupo III - Tetraciclina. It has been recommended the systemic antibiotic therapy, although there are just a little studies that demonstrate its real effect in the process. Therefore, the aim of the study was to evaluate the effect of systemic antibiotic therapy with amoxicillin and tetracycline after immediate tooth replantation in rats. The incisors of 90 rats (Rattus norvegicus albinus variety Wistar) were extracted and stored in 20 ml of saline solution for 5 minutes. Then, the teeth were replanted and the animals were divided into 3 groups according to the antibiotic administered by gavage: Group I – saline, Group II - amoxicillin and Group III - tetracycline. The rats were euthanized at 7, 15 and 30 days after reimplantation. Regardless of the analyzed period, the connective tissue underlying the epithelial reattachment and periodontal ligament showed a more intense infiltrate inflammatory acute in the control group followed by the Tetracycline group. Systemic antibiotic therapy has been recommended in such cases, but its value is questionable. Therefore, the objective of this study was to evaluate the effect of systemic antibiotic therapy with Amoxicillin and Tetracycline after delayed tooth replantation in rats. Ninety rats were extracted incisors and held 60 minutes in the environment. After this time the pulp and periodontal ligament were removed, and the teeth were immersed in a solution of sodium fluoride and 2% acidulated phosphate. Then the root canal was filled with calcium hydroxide paste and an MTA apical plug, and the teeth were replanted. The animals were divided into three groups according to the types of the antibiotics. In Group I - control, Group II - Amoxicillin and Group III - Tetracycline. Evaluations were made at 7, 15 and 30 days after replantation. Regardless of the analyzed period the acute inflammatory infiltrate was lower in the Group II, with Amoxicillin. As for root resorption, it was presented with a lesser extent and depth also in the group of Amoxicillin, which indicates that systemic antibiotic therapy has a positive influence on the delayed tooth replantation repairing process, and Amoxicillin was more effective in controlling the inflammatory process

O objetivo do trabalho foi avaliar o MTA e o Ca(OH)2, no interior dos tecidos, após a reabsorção radicular externa em reimplante tardio de dente de rato. Foram utilizados 20 ratos (Rattus norvegicus albinus) machos que tiveram o incisivo direito extraído e mantido em meio seco por 60 minutos. Em seguida, a papila dentária, o órgão do esmalte, a polpa dentária e o ligamento periodontal foram removidos. Os dentes foram imersos em solução de fluoreto de sódio fosfato acidulado a 2% pH 5,5, por 10 minutos. Em seguida os canais foram secos e divididos em 2 grupos: Grupo I - canais radiculares foram preenchidos com pasta de Ca(OH)2 e soro fisiológico e Grupo II - com MTA. Os alvéolos foram irrigados com soro fisiológico e os dentes reimplantados. Após 80 dias foi possível observar extensas áreas de reabsorção por substituição e algumas inflamatória nos dois grupos experimentais. Houve uma reação inflamatória mais intensa em contato com o Ca(OH)2 quando comparado ao MTA. È possível concluir que o MTA pode ser uma opção clínica viável para a obturação de dentes tardiamente reimplantados sujeitos à reabsorção radicular externa devido a sua biocompatibilidade com os tecidos Clinical practice has shown that most of reimplanted teeth are late, this condition produce necrosis of periodontal ligament cells and the pathological resorption may occur and produce dental loss. The aim of this study was to assess the biological behavior of MTA and Ca(OH)2, within the tissues after external root resorption in delayed late reimplanted rat teeth. Twenty male rats (Rattus norvegicus albinus) were used. The upper right incisor was extracted and was kept in a dry environment for 60 minutes. Then, the dental papilla, the enamel organ, the dental pulp and the periodontal ligament were removed. The teeth were immersed in 2% acidulous sodium fluoride solution pH 5.5 for 10 minutes. After that, the canals were dried and divided into 2 groups: Group I - the canals were filled with Ca(OH)2 and saline and Group II - with MTA. Sockets were irrigated with saline solution and the teeth were reimplanted. After 80 days was possible to observe large areas of replacement resorption and some inflammatory root in both groups There was more intense inflammatory reaction in contact with Ca(OH)2 when compared to MTA. At the bottom of the alveolar socket, bone neoformation was greater in the group of MTA, but there was no statistically significant difference between the groups. A proposta deste trabalho é avaliar o processo de reparo no reimplante imediato de dentes de macaco após o emprego de curativo de demora de hidróxido de cálcio [Ca(OH)2] por 30 e 180 dias seguido da obturação definitiva com cimento à base de Ca(OH)2 (Sealapex®) por meio de análise histomorfométrica. Os incisivos laterais superiores e inferiores de quatro macacas Cebus Apella foram extraídos, mantidos em soro fisiológico por 15 minutos, reimplantados e contidos com resina composta e fio de aço por 10 dias. No Grupo I, 10 dias após o reimplante, iniciou-se a terapia endodôntica, colocação do curativo de hidróxido de cálcio que foi mantido por 30 dias e a obturação do canal radicular foi finalizada aos 40 dias após o reimplante dentário. No Grupo II, o curativo de Ca(OH)2 permaneceu por 180 dias com uma troca aos 90 dias antes da obturação definitiva dos canais radiculares. A eutanásia dos animais foi realizada após nove meses do reimplante dentário. Em todos os grupos, a reinserção epitelial localizou-se na junção cemento-esmalte ou muito próxima a ela, o ligamento periodontal se apresentou organizado e as reabsorções radiculares de superfície eram de pequena extensão e profundidade reparadas por cemento neoformado. Não houve diferença estatisticamente significativa quanto à reabsorção radicular entre os grupos. No reimplante imediato a obturação definitiva do canal radicular pode ser realizada aos 30 dias não necessitando de longo período de curativo de Ca(OH)2. The endodontic treatment is important for treatment of tooth reimplantation but the period for definitive root canal filling is not still established. The aim of this study is to evaluate the healing process of immediate reimplantation of monkey teeth after calcium hydroxide curative [Ca(OH)2] for 30 and 180 days followed by definitive obturation with Ca(OH)2 cement (Sealapex®) through histomorphometric analysis. The superior and inferior lateral incisors of four female monkeys Cebus Apella were extracted, stored in saline for 15 minutes, reimplanted, and splinted with composite resin and steel wire during 10 days. In the Group I, the endodontic treatment began 10 days after tooth reimplantation with calcium hydroxide curative for 30 days and root canal filling at 40 days after reimplantation. In the Group II, the Ca(OH)2 curative remained during 180 days with a change to the 90 days before root canal filling. The euthanasia of the animals was carried out after 9 months of tooth reimplantation. All groups exhibited epithelial reinsertion at the cementoenamel junction or near, organized periodontal ligament and root surface resorptions with minor extension and depth that were repaired by neoformed cement. There was no statistically significant difference for root resorption among the groups. The root canal filling can be performed earlier without a long period of Ca(OH)2 curative for immediate reimplantation.

O objetivo do trabalho é avaliar o processo de reparo no reimplante tardio de dentes de rato, utilizando o hidróxido de cálcio (Ca(OH)2) associado ao paramonoclorofenol canforado (PMCC) ou à clorexidina (CHX) como curativo de demora em relação ao Ca(OH)2 associado ao soro fisiológico na prevenção e/ou controle da reabsorção radicular inflamatória. Trinta ratos (Rattus norvegicus albinus) machos, divididos em 3 grupos de 10 animais, tiveram o incisivo direito extraído e mantido em meio seco por 60 minutos. Em seguida, a papila dentária, o órgão do esmalte, a polpa dentária e o ligamento periodontal foram removidos. The prognosis of delayed tooth replantation is linked to the need of endodontic treatment, once that necrotic pulp has a direct relationship with the rise of inflammatory external root resorption. Much research has been conducted with the objective of preventing or treating this complication and calcium hydroxide is the most commonly used substance because of its antimicrobial and tissue repairing properties. The aim of this study is to evaluate the repair process of the delayed teeth replantation in rat, treated with calcium hydroxide (Ca(OH)2) associated with camphorated p-monochlorophenol (CMCP) or chlorhexidine 2 % (CHX), and Ca(OH)2 in saline solution as temporization dressing to prevent and/or control inflammatory resorption. Thirty rats (Rattus norvegicus albinos) had extracted the right incisor and that was kept in a dry state for 60 minutes. Subsequently, the dental papilla, the enamel organ, the dental pulp and periodontal ligament were removed. The teeth were immersed in a solution of sodium fluoride acidulated 2% pH 5.5 for 10 minutes. After this procedure, the canals were dried with absorbent paper cone and divided into 3 groups, of 10 animals of which, according to the dressing: Group I: root canals filled with Ca(OH)2 and saline solution, Group II: with Ca(OH)2 and CMCP and Group III: with Ca(OH)2 and CHX 2%. The teeth sockets were irrigated with saline solution and the teeth were replanted. While histological analysis revealed the presence of inflammatory resorption, ankylosis and replacement in all three groups, statistical analysis showed a significant difference between group III and other groups. Departing from these results, it was concluded that the association between the use of Ca(OH)2 with PMCC or CHX did not show an advantage over the use of Ca(OH) 2 paste and saline solution to prevent and/or control the inflammatory root reabsorption in delayed replantation of rat teeth.

Assim, foi objetivo deste estudo, avaliar por meio de análise histomorfométrica e imunoistoquímica, a ação do laser em baixa intensidade (LLLT) sobre o processo de reparo após reimplante dentário em ratos. Sessenta ratos (Rattus norvegicus albinus, Wistar) tiveram seus incisivos superiores direitos extraídos e foram aleatoriamente divididos em seis grupos: C0, C30 e C45 considerados controles, onde o reimplante dentário foi realizado imediatamente, trinta e quarenta e cinco minutos após a exodontia respectivamente, sem nenhum tratamento do ligamento periodontal remanescente e L0, L30 e L45, onde o reimplante foi realizado nos mesmos tempos extra-alveolares dos controles, porém a superfície radicular e alvéolo, com remanescentes do ligamento periodontal, foram tratados com LLLT (Arseneto de Gálio e Alumínio) antes do reimplante. Os animais foram eutanasiados após 60 dias. Todavia, período extrabucal prolongado ocasiona danos ao ligamento periodontal, que desencadeiam processos de reabsorção radicular externa. O objetivo desse estudo foi avaliar histomorfometricamente, a viabilidade da solução de própole e a de flúor no controle da reabsorção radicular externa no processo de reparo do reimplante tardio. Trinta ratos (Rattus norvegicus albinus, variedade Wistar) tiveram seu incisivo superior direito extraído. Os dentes foram mantidos em meio seco por 60 minutos. Após esse período a papila dentária, o órgão do esmalte e o ligamento periodontal removidos com lâmina de bisturi e a polpa foi extirpada. Os dentes foram divididos em três grupos experimentais: grupo I - dentes imersos em 20 ml de solução de soro fisiológico; grupo II - dentes imersos em 20 ml de solução de fluoreto de sódio fosfato acidulado a 2%; grupo III - imersão dos dentes em 20 ml de solução de própole a 15%. Após 10 minutos de imersão nas soluções, os canais radiculares foram secos e preenchidos com pasta de hidróxido de cálcio e os dentes reimplantados. A eutanásia dos animais ocorreu 60 dias após o ato operatório. Os resultados mostraram presença de reabsorção inflamatória, por substituição e anquilose nos três grupos experimentais, não apresentando diferença significativa quando submetidos à análise comparativa, o que levou a conclusão de que o tratamento da superfície radicular com própole se mostrou semelhante ao do flúor com relação à reabsorção radicular externa. Apesar de não apresentarem diferença estatisticamente significante, houve maior quantidade de reabsorção inflamatória do soro fisiológico quando comparado ao flúor e à própole. Tooth replantation is the best option for treatment of an avulsed tooth. Nevertheless, an extended extraoral period damages the periodontal ligament, and results in external root resorption. The purpose of this study was to assess by histologic and histometric analysis, the influence of propolis and the fluoride solution used as root surface treatment on the healing process after delayed tooth replantation. Thirty Wistar (Rattus norvegicus albinus) were submitted to extraction of their upper right incisor. The teeth were maintained in a dry environment for 60 minutes. After this time the pulp was extirpated and the papilla, enamel organ and periodontal ligament were removed with scalpel. The teeth were divided in to three experimental groups: group I - teeth immersed in 20 ml of physiologic saline; group II - teeth immersed in 20 ml of 2% acidulated phosphate sodium fluoride; group III - teeth immersed in 20 ml of 15% propolis. After 10 minutes of immersion in the solutions, the root canals were dried and filled with calcium hydroxide paste and the teeth were replanted. The animals were euthanized 60 days after replantation. The results showed the similar external root resorption was seen in the propolis and fluoride groups. Teeth treated with physiologic saline tended to have more inflammatory root resorption compared to those treated with fluoride or propolis. However, the comparative analysis did not reveal any statistically signifant differences between the treatment modalities when used for delayed tooth replantation.

Esse tratamento inclui a remoção do ligamento periodontal cementário necrótico que pode favorecer o aparecimento das reabsorções radiculares externas, maior causa da perda dos dentes reimplantados nessas condições. O objetivo desse estudo foi comparar a efetividade de duas técnicas de remoção mecânica do ligamento periodontal cementário necrosado e a preservação da camada de cemento. Foram utilizadas 45 raízes de pré-molares hígidos extraídos com finalidade ortodôntica. Após a extração os dentes permaneceram por um período de uma hora em meio ambiente e passado esse tempo foram imersos em soro fisiológico por mais dez minutos. A partir desse passo foram divididos em 3 grupos experimentais. Grupo 1: controle, o ligamento periodontal cementário foi mantido. Grupo 2: remoção do ligamento periodontal cementário por meio de raspagem com lâmina de bisturi (RLB). Grupo 3: remoção do ligamento periodontal cementário por meio de escova de Robinson montada em baixa rotação e pasta de pedra pomes e água (RER). Após processamento das peças foram feitos cortes transversais e longitudinais nessas raízes para análise em microscopia óptica e eletrônica de varredura (MEV). Pela análise qualitativa e quantitativa dos resultados concluiu-se que a técnica RER foi mais eficiente que a RLB quanto à remoção do ligamento periodontal cementário, porém ambas preservaram a camada de cemento. One of the important features related to the success of delayed tooth replantation seems to depend on the type of root surface treatment. This treatment encloses the removal of necrotic cemental periodontal ligament, which may favors the occurrence of external root resorptions that are the major cause of loss of teeth replanted in such conditions. The aim of this study was to compare the effectiveness of two techniques for mechanical removal of necrotic cemental periodontal ligament and the preservation of the cementum layer. Forty-five roots of sound premolars extracted for orthodontic purposes were selected. After extraction, the teeth were maintained at room ambient conditions for 1 hour and then immersed in saline for additional 10 minutes. Afterwards, the roots were assigned to three experimental groups. Group 1: control, the cemental periodontal ligament was maintained. Group 2: removal of the cemental periodontal ligament by scaling with a scalpel blade (SBR). Group 3: removal of the cemental periodontal ligament using a Robinson bristle brush, at low-speed, with pumice/water slurry (RBR). After processing of the pieces, the roots were sectioned transversally and longitudinally and the specimens obtained were examined under optical and scanning electron microscopy (SEM). The quantitative and qualitative analyses of the results showed that the RBR technique was more effective than the SBR technique, as regards the removal of the cemental periodontal ligament. Both techniques preserved the cementum layer.
