Microgravity is an environment incompatible with human physiology that induces unique physiological changes to the body. Exposure to microgravity environment can cause some possible physiological hazards, such as impairment of immune function, bone mass and skeletal muscle mass, in addition to possible negative psycological alterations (depression, anxiety, apathy, and personality changes). These pysico-physiological changes negatively impact health status and quality of life and result in operational difficulties on a space mission for the astronauts. Countermeasures designed to lessen the effect of microgravity on the human body include daily physical exercise and rehabilitation, psychotherapy, specific diets, pharmaceutical treatments, and the use of technologies. Virtual reality (VR) added to physical exercise programs, in the training and preparation of the astronauts for the space missions, could have more exuberant effects. Therefore, the aim of this perspective article is to present the application of VR and physical exercise as countermeasures for training and preparation before, during and after a space missions.

The recent incidental discovery of an asymptomatic venous thrombosis (VT) in the internal jugular vein of an astronaut on the International Space Station prompted a necessary, immediate response from the space medicine community. The European Space Agency formed a topical team to review the pathophysiology, risk and clinical presentation of venous thrombosis and the evaluation of its prevention, diagnosis, mitigation, and management strategies in spaceflight. In this article, we discuss the findings of the ESA VT Topical Team over its 2-year term, report the key gaps as we see them in the above areas which are hindering understanding VT in space. We provide research recommendations in a stepwise manner that build upon existing resources, and highlight the initial steps required to enable further evaluation of this newly identified pertinent medical risk.

Gamification i.e., the incorporation of game design elements into non-game contexts, represents a transformative tool in health education. In this sense, the increasingly intertwined fields of technology, education, and health present opportunities for informed decision-making and the training of educators, health professionals, and policymakers. This need to improve teaching and learning methods is particularly evident in the areas of first aid and basic life support (BLS), with the aim of improving clinical results and health outcomes. Our article carried out a narrative review, focusing on the intersection of gamification, technology and first aid/BLS training with the aim of investigating the application of gamification and serious games in emergency medicine with the aim of improving motivation and theoretical and practical skills. of health professionals. In addition, this review emphasizes the role of simulation-based education in first aid skills training, highlighting its contribution to preparing first aiders for real-world scenarios. Ultimately, this review underscores the transformative potential of serious games in medical education, showcasing their effectiveness across diverse contexts. In summary, it enhances comprehension of gamification's role in healthcare and medical education, particularly in first aid training, by underscoring the adaptability and efficacy of serious games in revolutionizing medical and emergency education and healthcare. Nevertheless, it underscores that this remains a relatively underexplored area of knowledge.

Background: Although there have been no reported cardiac arrests in space to date, the risk of severe medical events occurring during long-duration spaceflights is a major concern. These critical events can endanger both the crew as well as the mission and include cardiac arrest, which would require cardiopulmonary resuscitation (CPR). Thus far, five methods to perform CPR in microgravity have been proposed. However, each method seems insufficient to some extent and not applicable at all locations in a spacecraft. The aim of the present study is to describe and gather data for two new CPR methods in microgravity. Materials and Methods: A randomized, controlled trial (RCT) compared two new methods for CPR in a free-floating underwater setting. Paramedics performed chest compressions on a manikin (Ambu Man, Ambu, Germany) using two new methods for a free-floating position in a parallel-group design. The first method (Schmitz-Hinkelbein method) is similar to conventional CPR on earth, with the patient in a supine position lying on the operator's knees for stabilization. The second method (Cologne method) is similar to the first, but chest compressions are conducted with one elbow while the other hand stabilizes the head. The main outcome parameters included the total number of chest compressions (n) during 1 min of CPR (compression rate), the rate of correct chest compressions (%), and no-flow time (s). The study was registered on clinicaltrials.gov (NCT04354883). Results: Fifteen volunteers (age 31.0 +/- 8.8 years, height 180.3 +/- 7.5 cm, and weight 84.1 +/- 13.2 kg) participated in this study. Compared to the Cologne method, the Schmitz-Hinkelbein method showed superiority in compression rates (100.5 +/- 14.4 compressions/min), correct compression depth (65 +/- 23%), and overall high rates of correct thoracic release after compression (66% high, 20% moderate, and 13% low). Spaceflight is still complex and has potential risk for incidents and accidents. The aim of this study was to analyze how safe it is for humans to travel in space. Objectives: This paper, therefore, summarizes incidents and accidents covering the six decades of manned spaceflight (1961-2020). Material and methods: Extensive PubMed, Cochrane, and Google Scholar searches were made with search strings of incidents, accident, spaceflight, and orbit, and including all vehicles so far. Search terms were combined by AND or OR in search strings. Of the results obtained, studies which evaluated manned spaceflight were included in the study. Data from the National Aeronautics Space Agency (NASA), the Russian Space Agency (ROSCOSMOS), the European Space Agency (ESA), and the Chinese Space Agency (CNSA), as well as from the Virgin Galactic and the SpaceX databases, were searched to complete data and to identify all the accomplished manned spaceflights, as well as all incidents and accidents that have occurred in the specific period. Search results were compared to findings on Wikipedia, Encyclopedia Astronautica, and other public webpages. Reference lists of included articles/homepages were also included for further potential data. Results: From 1961-2020, our data revealed an increasing number of manned space flights, n = 327. The number of times an astronaut has been sent to space, n = 1294, resulted in an accumulated n = 19,414 days spent in space. The number of days spent in orbit has constantly increased from 1961 until today. The number of incidents (altogether n = 36) and accidents (altogether n = 5) has constantly decreased. (1) Background: Cardiopulmonary resuscitation (CPR), as a form of basic life support, is critical for maintaining cardiac and cerebral perfusion during cardiac arrest, a medical condition with high expected mortality. Current guidelines emphasize the importance of rapid recognition and prompt initiation of high-quality CPR, including appropriate cardiac compression depth and rate. As space agencies plan missions to the Moon or even to explore Mars, the duration of missions will increase and with it the chance of life-threatening conditions requiring CPR. The objective of this review was to examine the effectiveness and feasibility of chest compressions as part of CPR following current terrestrial guidelines under hypogravity conditions such as those encountered on planetary or lunar surfaces; (2) Methods: A systematic literature search was conducted by two independent reviewers (PubMed, Cochrane Register of Controlled Trials, ResearchGate, National Aeronautics and Space Administration (NASA)). Only controlled trials conducting CPR following guidelines from 2010 and after with advised compression depths of 50 mm and above were included; (3) Results: Four different publications were identified. All studies examined CPR feasibility in 0.38 G simulating the gravitational force on Mars. Two studies also simulated hypogravity on the Moon with a force of 0.17 G/0,16 G. All CPR protocols consisted of chest compressions only without ventilation. A compression rate above 100/s could be maintained in all studies and hypogravity conditions. Two studies showed a significant reduction of compression depth in 0.38 G (-7.2 mm/-8.71 mm) and 0.17 G (-12.6 mm/-9.85 mm), respectively, with nearly similar heart rates, compared to 1 G conditions. Travelling to the stars is a dream nearly as old as mankind itself. Nowadays, spaceflight is in many ways common business, becoming even accessible to space tourists. However, many problems remain to be solved before humanity can venture into deep space with an acceptable level of risk, and medical preparedness is one of them. The management of any severe medical emergency will be extremely challenging in this extreme environment, with limited resources in crew and equipment. Here, we discuss the case of a cardiac arrest occurring during spaceflight, and present challenges around the recognition, immediate management including delivering cardio-pulmonary resuscitation and secondary measures such as organ support or evacuation. Given the current treatment capabilities in space, the survivability of cardiac arrest is expected to be lower than on the ground. (c) 2018 Elsevier Ltd. All rights reserved.

AIMS: To evaluate the efficacy of defibrillation in response to induced ventricular fibrillation and to describe the macroscopic changes in porcine hearts after truncated exponential capacitive discharge shock applied by a prototype defibrillator produced in Brazil and a reference, international market leader defibrillator. METHODS: Sixteen pigs were divided into four groups of four animals each, to verify the effectiveness of the defibrillator in response to induced ventricular fibrillation by applying loads of 50 J to 100 J, and assess the safety of the defibrillator by macroscopic analysis of the skin and heart. Erythema, edema, blistering, fluid in the pericardial cavity, burns, and post-shock charred tissue were evaluated. RESULTS: All pigs, after one minute of inducing ventricular fibrillation, developed tachycardia and progressed to ventricular fibrillation. When used low energy, the prototype equipment obtained 25% of success, with one reversed pig, while the reference equipment has obtained a success rate of 50%, with two reversals. However, with energy 100 J, the prototype equipment reestablished sinus rhythm in three pigs (75%) and the reference product only made effective defibrillation in one (25%). The application of 100 J energy was associated with skin burns Grade II in only one pig with the prototype defibrillator and four pigs with the reference equipment. The heart tissue injuries occurred in equal numbers with both defibrillators. CONCLUSIONS: The prototype defibrillator, when applied with 100 J energy, was more effective in reversing ventricular fibrillation and showed less injury than the reference defibrillator with the same energy. This study may help to raise the technological standard and to increase the supply of qualified health equipment produced in Brazil, replacing and/or decreasing imports and providing a secure next-generation device.
