15. Implementation of the Calypso system: a commissioning experience.

Abstract

Background: The aim of this study was to describe the clinical implementation of the Calypso system with its potential impact on the treatment delivery. Materials and methods: The influence of the electromagnetic array was investigated on the kilovoltage cone beam computed tomography (kV-CBCT) image quality using the CATPHAN 504 CBCT images. Then, the QFix kVue Calypso couch top and the array attenuation, and their dosimetric influence on the Volumetric modulated arc therapy (VMAT) treatments of prostate was evaluated. Results: Regarding the image quality, a significant increase of noise (p < 0.01) was detected with the array in place, resulting in a significant decrease in signal noise ratio (SNR) (p < 0.01). No difference in absolute contrast was observed. Finally, there was a significant decrease in contrast noise ratio (CNR) (p < 0.01) even if the deviation was only of 2.5%. For the dosimetric evaluation, the maximum attenuation of the couch was 12.02% and 13.19% for X6 and X6 flattening filter free (FFF), respectively (configuration of rails out). Besides, the mean attenuation of the array was 1.15% and 1.67% for X6 and X6 FFF, respectively. For the VMAT treatment plans, the mean dose was reduced by 0.61% for X6 and by 0.31% for X6 FFF beams when using the electromagnetic array. Conclusions: The Calypso system does not affect significantly the kV-CBCT image quality and the VMAT plan dose distribution. [ABSTRACT FROM AUTHOR]

Abstract

Abstract Background To compare volumetric-modulated arc therapy plans with conventional radiation therapy (3D-CRT) plans in pancreatic and bile duct cancers, especially for bilateral kidney preservation. Methods A dosimetric analysis was performed in 21 patients who had undergone radiotherapy for pancreatic or bile duct carcinoma at our institution. We compared 4-field 3D-CRT and 2 arcs RapidArc (RA) plans. The treatment plan was designed to deliver a dose of 50.4 Gy to the planning target volume (PTV) based on the gross disease in a 1.8 Gy daily fraction, 5 days a week. Planning objectives were 95% of the PTV receiving 95% of the prescribed dose and no more than 2% of the PTV receiving more than 107%. Dose-volume histograms (DVH) for the target volume and the organs at risk (right and left kidneys, bowel tract, liver and healthy tissue) were compared. Monitor units and delivery treatment time were also reported. Results All plans achieved objectives, with 95% of the PTV receiving ≥ 95% of the dose (D95% for 3D-CRT = 48.9 Gy and for RA = 48.6 Gy). RapidArc was shown to be superior to 3D-CRT in terms of organ at risk sparing except for contralateral kidney: for bowel tract, the mean dose was reduced by RA compared to 3D-CRT (16.7 vs 20.8 Gy, p = 0.0001). Similar result was observed for homolateral kidney (mean dose of 4.7 Gy for RA vs 12.6 Gy for 3D-CRT, p < 0.0001), but 3D-CRT significantly reduced controlateral kidney dose with a mean dose of 1.8 Gy vs 3.9 Gy, p < 0.0007. Compared to 3D-CRT, mean MUs for each fraction was significantly increased with RapidArc: 207 vs 589, (p < 0.0001) but the treatment time was not significantly different (2 and 2.66 minutes, p = ns). Conclusion RapidArc allows significant dose reduction, in particular for homolateral kidney and bowel, while maintaining target coverage. This would have a promising impact on reducing toxicities.

Abstract Background To present the results of quality assurance (QA) in IMRT of film dosimetry and ionization chambers measurements with an eight year follow-up. Methods All treatment plans were validated under the linear accelerator by absolute and relative measures obtained with ionization chambers (IC) and with XomatV and EDR2 films (Kodak). Results The average difference between IC measured and computed dose at isocenter with the gantry angle of 0° was 0.07 ± 1.22% (average ± 1 SD) for 2316 prostate, 1.33 ± 3.22% for 808 head and neck (h&n), and 0.37 ± 0.62% for 108 measurements of prostate bed fields. Pelvic treatment showed differences of 0.49 ± 1.86% in 26 fields for prostate cases and 2.07 ± 2.83% in 109 fields of anal canal. Composite measurement at isocenter for each patient showed an average difference with computed dose of 0.05 ± 0.87% for 386 prostate, 1.49 ± 1.86% for 158 h&n, 0.37 ± 0.34% for 23 prostate bed, 0.80 ± 0.28% for 4 pelvis, and 2.31 ± 0.56% for 17 anal canal cases. On the first 250 h&n analyzed by film in absolute dose, the average of the points crossing a gamma index 3% and 3 mm was 93%. This value reached 99% for the prostate fields. Conclusion More than 3500 beams were found to be within the limits defined as validated for treatment between 2001 and 2008.

Abstract

La modélisation thermique et énergétique d'un capteur solaire hybride nécessite de connaître les coefficients optiques et thermiques des différents éléments du capteur. Le polycarbonate est proposé comme couverture de protection du capteur solaire hybride. Le facteur de transmission globale du polycarbonate dans la gamme du domaine visible de 400 nm à 900 nm a été mesuré. Les facteurs de transmission, de réflexion et d'absorption ont été mesurés pour une longueur d'onde allant de 0,4 μm à 25 μm. A partir des résultats, l'indice de réfraction et le coefficient d'extinction ont été calculés. L'émissivité du polycarbonate à basse température a été mesurée pour la gamme spectrale allant de 8 μm à 14 μm. Les valeurs obtenues seront utilisées ultérieurement dans la modélisation thermique du capteur solaire hybride avec concentrateur.

Abstract

International audience; Le verre joue un rôle majeur comme protection supérieure, substrat et piégeage de la lumière dans la plupart des systèmes PV. Cependant, des travaux ont montré qu'au cours de sa durée de vie, un panneau solaire photovoltaïque perd entre 10 et 15% de son rendement, essentiellement à cause de pertes optiques. Nous avons testé une méthode de contrôle non destructif et non intrusif permettant de diagnostiquer l'état de vieillissement du verre sodocalcique, à l'aide d'une caméra LWIR. La caractérisation du verre sodocalcique exposé à la chaleur humide durant 1000 h a mis en évidence la pertinence de l'utilisation de l'émissivité apparente comme indicateur de vieillissement. Deux cinétiques de dégradation sont alors apparues lors du vieillissement accéléré. Nous avons également montré la correspondance entre vieillissement naturel et vieillissement accéléré sous UV. Les modifications provoquées par le vieillissement sous UV sont qualitativement identiques pour les spectres de transmittance dans la gamme [300-2300 nm], au contraire du vieillissement sous chaleur humide.

Abstract

Les modules photovoltaïques en exposition extérieure longue subissent de nombreuses dégradations (jaunissement de l'encapsulant, délamination, bulles, corrosion, « hot spots »…) qui entraînent une diminution de leurs rendements. Dans la présente étude, la production électrique d'un module photovoltaïque avec des cellules en silicium est simulée pour différents instants pris sur sa durée de vie (25 ans). Le circuit équivalent à une diode est utilisé avec des paramètres électriques et optiques dépendant du temps. Les lois d'évolution pour ces paramètres sont établis à partir des résultats de tests accélérés en chaleur humide 85°C/85% rh obtenus par Hulkoff. En se basant sur l'équivalence entre les effets de ce test accéléré et ceux de l'exposition extérieure à Miami, la production électrique d'un module photovoltaïque avec des cellules en silicium monocristallin est simulée pour un usage dans ce lieu. L'impact du vieillissement sur la production électrique est apprécié pour une journée de beau temps et pour une journée grise.

Abstract

A thermographic approach is used to determine the temperature of an aluminium nitride hot plate as a glass substrate heater for depositing thin films by spray CVD (Chemical Vapour Deposition). The true temperature of the hot plate is determined by measuring its emissivity and the background temperature. The emissivity is found by means of a commercial infrared camera in the temperature range of [40°C, 540°C] with a better than 3% accuracy. The measured thermographic values are then corrected with a temperature precision of less than 4% for the highest temperatures considered. Moreover, the temperature profiles are plotted and good temperature homogeneity is observed.

Abstract

International audience; Toll-like receptors are ubiquitous and very well conserved throughout evolution, with important functions mediating innate and adaptative immunological mechanisms. The importance of these receptors and their agonists has been recently pointed out in immunology and cancerology, although the accurate underlying mechanisms are still under investigation. The association of agonists of these receptors with ionizing radiation has been studied in preclinical experiments with promising results. Part of these compounds is flagellin, which seems to be able to modulate the radiosensitivity of both tumors and healthy tissues.

Abstract

Ductal carcinoma in situ of the breast is associated with low mortality rates, but local relapse is a matter of concern in this disease. Risk factors for local relapse include young age, close or positive margins, and tumor necrosis. Whole breast irradiation following breast-conserving surgery for ductal carcinoma in situ significantly reduces the risk of local relapse as compared to breast-conserving surgery alone. Studies point to similar outcomes between breast-conserving surgery plus radiotherapy and mastectomy, in the absence of extensive disease. A complementary boost to the surgical bed improves outcomes for patients with invasive breast cancer. However, the effect of this strategy has never been prospectively reported for ductal carcinoma in situ. Two randomized controlled trials assessing this issue are ongoing. This paper represents an update on available literature about radiotherapy for DCIS with a special focus on the role of a radiotherapy boost to the tumor bed. [ABSTRACT FROM AUTHOR]
