2. Patients with Clinically Suspected Gallstone Disease: A More Selective Ultrasound May Improve Treatment Related Outcomes

This study aimed to quantify the confirmation of gallstones on ultrasound (US) in patients with suspicion of gallstone disease. To aid general practitioners (GPs) in diagnostic workup, a model to predict gallstones was developed. A prospective cohort study was conducted in two Dutch general hospitals. Patients (≥18 years) were eligible for inclusion when referred by GPs for US with suspicion of gallstones. The primary outcome was the confirmation of gallstones on US. A multivariable regression model was developed to predict the presence of gallstones. In total, 177 patients were referred with a clinical suspicion of gallstones. Gallstones were found in 64 of 177 patients (36.2%). Patients with gallstones reported higher pain scores (VAS 8.0 vs. 6.0, p < 0.001), less frequent pain (21.9% vs. 54.9%, p < 0.001), and more often met criteria for biliary colic (62.5% vs. 44.2%, p = 0.023). Predictors for the presence of gallstones were a higher pain score, frequency of pain less than weekly, biliary colic, and an absence of heartburn. The model showed good discrimination between patients with and without gallstones (C-statistic 0.73, range: 0.68–0.76). Clinical diagnosis of symptomatic gallstone disease is challenging. The model developed in this study may aid in the selection of patients for referral and improve treatment related outcomes.

Objectives To investigate sources of variation in a multicenter rectal cancer MRI dataset focusing on hardware and image acquisition, segmentation methodology, and radiomics feature extraction software. Methods T2W and DWI/ADC MRIs from 649 rectal cancer patients were retrospectively acquired in 9 centers. Fifty-two imaging features (14 first-order/6 shape/32 higher-order) were extracted from each scan using whole-volume (expert/non-expert) and single-slice segmentations using two different software packages (PyRadiomics/CapTk). Influence of hardware, acquisition, and patient-intrinsic factors (age/gender/cTN-stage) on ADC was assessed using linear regression. Feature reproducibility was assessed between segmentation methods and software packages using the intraclass correlation coefficient. Results Image features differed significantly (p Conclusions Significant variations are present in multicenter MRI data, particularly related to differences in hardware and acquisition, which will likely negatively influence subsequent analysis if not corrected for. Segmentation variations had a minor impact when using whole volume segmentations. Between software packages, higher-order features were less reproducible and caution is warranted when implementing these in prediction models. Key Points • Features derived from T2W-MRI and in particular ADC differ significantly between centers when performing multicenter data analysis. • Variations in ADC are mainly (> 60%) caused by hardware and image acquisition differences and less so ( • Features derived using different image segmentations (expert/non-expert) were reproducible, provided that whole-volume segmentations were used. Purpose To analyze how the MRI reporting of rectal cancer has evolved (following guideline updates) in The Netherlands. Methods Retrospective analysis of 712 patients (2011–2018) from 8 teaching hospitals in The Netherlands with available original radiological staging reports that were re-evaluated by a dedicated MR expert using updated guideline criteria. Original reports were classified as “free-text,” “semi-structured,” or “template” and completeness of reporting was documented. Patients were categorized as low versus high risk, first based on the original reports (high risk = cT3-4, cN+, and/or cMRF+) and then based on the expert re-evaluations (high risk = cT3cd-4, cN+, MRF+, and/or EMVI+). Evolutions over time were studied by splitting the inclusion period in 3 equal time periods. Results A significant increase in template reporting was observed (from 1.6 to 17.6–29.6%; p Conclusion Updated concepts of risk stratification have increasingly been adopted, accompanied by an increase in template reporting and improved completeness of reporting. Use of updated guideline criteria resulted in considerable downstaging (of mainly high-risk cT-stage and nodal stage). Graphic abstract

The comparative effectiveness of partial nephrectomy versus radical nephrectomy to preserve kidney function has not been well established. We determined the risk of clinically significant (stage 4 and higher) CKD after radical or partial nephrectomy among veterans treated for kidney cancer in the Veterans Health Administration (2001–2013). Among patients with preoperative eGFR≥30 ml/min per 1.73 m2, the incidence of CKD stage 4 or higher after radical (n=9759) or partial nephrectomy (n=4370) was 7.9% overall. The median time to stage 4 or higher CKD after surgery was 5 months, after which few patients progressed. In propensity score–matched cohorts, partial nephrectomy associated with a significantly lower relative risk of incident CKD stage 4 or higher (hazard ratio, 0.34; 95% confidence interval [95% CI], 0.26 to 0.43, versus radical nephrectomy). In a parallel analysis of patients with normal or near-normal preoperative kidney function (eGFR≥60 ml/min per 1.73 m2), partial nephrectomy was also associated with a significantly lower relative risk of incident CKD stage 3b or higher (hazard ratio, 0.15; 95% CI, 0.11 to 0.19, versus radical nephrectomy) in propensity score–matched cohorts. Competing risk regression models produced consistent results. Finally, patients treated with a partial nephrectomy had reduced risk of mortality (hazard ratio, 0.55; 95% CI, 0.49 to 0.62). In conclusion, compared with radical nephrectomy, partial nephrectomy was associated with a marked reduction in the incidence of clinically significant CKD and with enhanced survival. Postoperative decline in kidney function occurred mainly in the first year after surgery and appeared stable over time.

Purpose: Weight loss before bariatric surgery with a low-calorie diet (LCD) has several advantages, including reduction of liver volume and an improved access to the lesser sac. Disadvantages include performing surgery in a state of undernutrition, side effects, costs and patient compliance. Omega-3 fatty acids may serve as an alternative to reduce liver steatosis. Materials and Methods: A randomised controlled open-label trial was done to compare the effects of a LCD with Modifast (800 kcal/day) during 2 weeks with 2 g of omega-3 fatty acids a day and a normal diet (2000 kcal/day) during 4 weeks. Total liver volume (TLV) and volume of the left liver lobe (LLL), visceral fat area (VFA) and muscle area (SMA) at the L3–L4 level were measured with MRI before and after preoperative treatment. Results: Sixty-two morbidly obese women undergoing laparoscopic Roux-en-Y gastric bypass surgery (LRYGB) were recruited. In both groups, there was a significant decrease in LLL, TLV and VFA. For LLL and TLV reduction, the LCD had a significantly larger effect (p < 0.05). Only in the LCD group was there a significant decrease in SMA with significantly more side effects and worse compliance. Conclusion: Both the LCD and omega-3 diet reduced LLL, TLV and VFA. The LCD outperformed the omega-3 diet in LLL and TLV reduction, but induced significant loss of SMA and had worse compliance due to more side effects. Omega-3 fatty acids may provide a safe and more patient-friendly alternative for a LCD and further research is indicated. Introduction Men who are considering vasectomy as a means of contraception may have significant anxiety about their future sexual potency. As a result, couples may choose other forms of contraception with lower efficacy. Aim We sought to determine the relationship between vasectomy and the frequency of sexual intercourse. Methods We analyzed data from cycles 6 (2002) to 7 (2006–2008) of the National Survey of Family Growth to compare the frequency of sexual intercourse of men who had undergone vasectomy with men who had not. Analysis was performed using data from male and female responders, and excluded men who had never had sex and those below age 25. We constructed a multivariate logistic regression model to adjust for demographic, socioeconomic, reproductive, and health factors. Main Outcome Measure The main outcome measure was the sexual frequency in the last 4 weeks. Results Among male responders, a total of 5838 men met criteria for our study; 353 had undergone vasectomy. For vasectomized men, the average frequency of sexual intercourse was 5.9 times per month compared with 4.9 times for nonvasectomized men. After adjusting for age, marital status, race, education, health, body mass index, children, and income, vasectomized men had an 81% higher odds (95% confidence interval [CI] 6–201%) of having intercourse at least once a week compared with nonvasectomized men. A total number of 5211 female respondents reported 670 of their partners had undergone vasectomy. For partners of vasectomized men, the average frequency of intercourse was 6.3 times per month, compared with 6.0 times for partners of nonvasectomized men. After adjustment, women with vasectomized partners had a 46% higher odds (95% CI 5–103%) of having sexual intercourse at least once a week compared with women with nonvasectomized partners (P = 0.024). Conclusion Vasectomy is not associated with decreased sexual frequency. This finding may be helpful to couples as they consider contraceptive options.

Objective To assess whether patient factors, such as age and preoperative kidney function, were associated with receipt of partial nephrectomy in a national integrated healthcare system. Materials and Methods We identified patients treated with a radical or partial nephrectomy from 2002 to 2014 in the Veterans Health Administration. We examined associations among patient age, sex, race or ethnicity, multimorbidity, baseline kidney function, tumor characteristics, and receipt of partial nephrectomy. We estimated the odds of receiving a partial nephrectomy and assessed interactions between covariates and the year of surgery to explore whether patient factors associated with partial nephrectomy changed over time. Results In our cohort of 14,186 patients, 4508 (31.2%) received a partial nephrectomy. Use of partial nephrectomy increased from 17% in 2002 to 32% in 2008 and to 38% in 2014. Patient race or ethnicity, age, tumor stage, and year of surgery were independently associated with receipt of partial nephrectomy. Black veterans had significantly increased odds of receipt of partial nephrectomy, whereas older patients had significantly reduced odds. Partial nephrectomy utilization increased for all groups over time, but older patients and patients with worse baseline kidney function showed the least increase in odds of partial nephrectomy. Conclusion Although the utilization of partial nephrectomy increased for all groups, the greatest increase occurred in the youngest patients and those with the highest baseline kidney function. These trends warrant further investigation to ensure that patients at the highest risk of impaired kidney function are considered for partial nephrectomy whenever possible.

In 49 patients with benign prostatic hyperplasia, 24 metastatic prostatic carcinoma patients all under palliative hormonal treatment, 17 untreated prostatic carcinoma patients without metastases and 14 untreated prostatic carcinoma patients with metastases, plasma levels of thrombin-antithrombin III complex, D-dimer and plasmin-alpha2-antiplasmin were determined. The coagulation activation marker thrombin-antithrombin III complex did not show any significant difference between the different patient groups. Of the fibrinolysis markers, D-dimer levels were elevated in both metastatic groups compared to the non-metastatic group and the benign prostatic hyperplasia group. Surprisingly, the levels of the other fibrinolysis marker, plasmin-alpha2-antiplasmin, showed no significant difference. The nature of these findings is discussed and related to other relevant literature. The general conclusion is that fibrinolysis may not play such a prominent role in prostatic carcinoma as described and expected.

- Abstract
Tumor cell migration and invasion are critical steps in the complex process of metastasis formation. It has been demonstrated that interferons (IFNs) inhibit the motility of human fibroblasts. In the present investigation we tested the effect of human leukocyte IFN and murine fibroblast IFN on the chemotactic migration of transformed and tumor-derived cells towards fibroblast conditioned medium. We were able to show that IFNs preferentially inhibit the chemotaxis of transformed and tumor-derived cell lines when compared to control fibroblasts. Inhibition was dose-dependent and most tumor cell strains were sensitive to concentrations of IFNs 10- to 100-fold lower than fibroblast cultures. Furthermore, leukocyte interferon was able to inhibit the invasion of transformed 3T3 fibroblasts through a gel of reconstituted basement membrane (Matrigel). The comparative effectiveness of partial nephrectomy versus radical nephrectomy to preserve kidney function has not been well established. We determined the risk of clinically significant (stage 4 and higher) CKD after radical or partial nephrectomy among veterans treated for kidney cancer in the Veterans Health Administration (2001-2013). Among patients with preoperative eGFR≥30 ml/min per 1.73 m
2 , the incidence of CKD stage 4 or higher after radical ( n =9759) or partial nephrectomy ( n =4370) was 7.9% overall. The median time to stage 4 or higher CKD after surgery was 5 months, after which few patients progressed. In propensity score-matched cohorts, partial nephrectomy associated with a significantly lower relative risk of incident CKD stage 4 or higher (hazard ratio, 0.34; 95% confidence interval [95% CI], 0.26 to 0.43, versus radical nephrectomy). In a parallel analysis of patients with normal or near-normal preoperative kidney function (eGFR≥60 ml/min per 1.73 m2 ), partial nephrectomy was also associated with a significantly lower relative risk of incident CKD stage 3b or higher (hazard ratio, 0.15; 95% CI, 0.11 to 0.19, versus radical nephrectomy) in propensity score-matched cohorts. Competing risk regression models produced consistent results. Finally, patients treated with a partial nephrectomy had reduced risk of mortality (hazard ratio, 0.55; 95% CI, 0.49 to 0.62). In conclusion, compared with radical nephrectomy, partial nephrectomy was associated with a marked reduction in the incidence of clinically significant CKD and with enhanced survival. The oldest anatomically preserved and physiologically apparently independent gametophytes are described from the Lower Devonian of Scotland. These gametophytes have upright, leafless axes with terminally borne, bowl-shaped game-tangiophores. The antheridia are stalked and their walls are multicellular. The archegonia are clustered in groups on common bases.

Two patients with bullous pemphigoid were treated with radio cobalt or betatron irradiation. Thereafter bullous eruptions developed in the non-irradiated skin. It is known from the literature that ultraviolet light applied to the normal-appearing skin of patients with bullous pemphigoid produced fresh histologic and immunohistochemical changes in the irradiated skin. X-ray irradiation applied to human skin biopsy specimens using single doses of 7 000 and 8 000 R resulted in an increase of basement membrane zone antibody binding of 2-3 titre dilution steps. An increase of 3-4 titre dilution steps was achieved by irradiation with ultraviolet light. Infrared irradiation of skin biopsy specimens, on the contrary, caused the total loss of the basement membrane zone fluorescence. It merits further study, whether these contrary effects on the basement membrane zone antibody binding of human skin in vitro (activation of bound antigen after X-ray or ultraviolet irradiation and inactivation of antigen caused by infrared irradiation) are of clinical value concerning the bullous pemphigoid problem in vivo.

In continuing our studies on the characteristics of the antigenic structures of the basement membrane, procedures used in histochemistry for the identification and blocking of reactive groups have been adapted in this present work to the pretreatment of cryostat sections of guinea-pig tongue. These sections have then been studied by the indirect immunofluorescence method. By means or reagents, which produce a reversible extinction of the basement membrane fluorescence produced with pemphigoid sera, it proved possible to determine the existence of certain chemical groupings which bind the anti-basement membrane antibodies, namely: free carboxyl groups deriving from amino acids, hydrogen bridges in which the hydroxyl groups from the carbohydrates in the antigen are probably involved, bonds involving the free aldehydes of the antigen (either cross-links from aldehydes, or formation of Schiff's bases with amino groups of the antibodies), and lastly, to a lesser extent, disulphide bonds. The antigen/antibody reaction probably occurs in the neighbourhood of the alpha-glucosido-beta-galactosido-hydroxylysine. A reaction model has been developed from these findings. There were also some indications that the antigenic structures of the intercellular substance are chemically related to those of the basement membrane. The cell nucleus antigens, on the other hand, appear to possess different chemical properties.

31. Lower devonian gametophytes: relation to the phylogeny of land plants.

Three gametophytic plants now known from the Lower Devonian of Scotland and Germany show common features in their fertile parts. The morphological and anatomical structures indicate affinities to bryophytic gametophytes, although there is no evidence for a parasitic sporogonium-like sporophyte as in the Bryophyta. Several of the vascular plant sporophytes from the Rhynie Chert also have a few characteristics reminiscent of bryophytes. But these ancient gametophytes, if related to the sporophytes, indicate a closer relation of Bryophyta to Tracheophyta than would the study of sporophytes alone.

Cholesterol ester and triglyceride metabolism was examined in intact fibroblast monolayers from normal individuals and patients with Wolman's disease and cholesterol ester storage disease. Cholesterol esters were introduced into cells by incubation in medium containing [3H]cholesteryl linoleate (CL) bound to human low density lipoprotein. Triglycerides were introduced by incubation with glycerol tri[1-14C]oleate (triolein) bound to human very low-density lipoprotein. Both types of mutant cell lines accumulated the unhydrolyzed substrates to a greater extent than did normal cells with the greatest accumulation observed in Wolman's disease cells. Wolman's disease cells hydrolyzed CL at 10-22% and triolein at 11-19% the rate of normal cells; cholesterol ester storage disease cells hydrolyzed these substrates at 28-49 and 30-47% the normal rate, respectively. In contrast, assays of acid lipase activity in cell lysates revealed less than 1% of control activity in both disorders. The data suggest that the mutant acid lipase present in Wolman's disease and cholesterol ester storage disease is more active in the intact cell than assays of cell lysates would indicate. In addition, the differences observed between the two disorders provide a biochemical explanation for the different phenotypes associated with the two disorders.

Basement zone antibody titres were studied in two patients with bullous pemphigoid over a period of 1-1/2 and 3-1/2 years. Titres remained almost constantly even after a complete healing of all lesions, without any treatment. Direct immunofluorescence revealed basement zone antibodies in one of these two patients when examined after healing had occurred. Three patients with bullous dermatoses showed pemphigus antibodies at the inception of the disease. Basement zone antibodies were present simultaneously in one patient. In the subsequent course of observation in the two other patients, pemphigus antibodies were replaced by basement zone antibodies and in one case by nuclear antibodies too. These findings make the pathogenetic import of these "auto-antibodies" difficult to understand.

Hitherto in the literature no proof has been published of a complement activation on the basement membrane in cases of pemphigoid. In this paper, for the first time, it is demonstrated, even if only in vitro, that complement, whose presence in the region of the basement membrane has long been known, plays an active role in a reaction. Through the action of the haemolytic system (amboceptor and sheep erythrocytes) and of anti C3 on reaction specimens for indirect immunofluorescence in pemphigoid, it is shown that the indicator reaction of the fluorescein-labelled anti-IgG with the immune complex attached to the basement membrane is markedly dependent on complement. Free, dissolved IgG is precipitated by appropriate anti-serum, even without complement.

Following brief exposure of cryostat sections of human skin to the action of proteolytic enzymes (papain or trypsin), there was a pronounced increase in the antibody binding sites of the basement membrane zone, shown in indirect immunofluorescence by an increased intensity in fluorescence of the basement membrane zone as compared with preparations which had not previously undergone incubation, and by an increase of 5-6 (papain) or 4-5 (trypsin) titre dilution steps. This effect was practically absent when guinea-pig tongue was used as the antigenic substrate. In conjunction with findings published in the literature, our results can be interpreted as indicating that the activation of the basement membrane zone antigens by proteolytic enzymes is associated with an increase in antigenicity which results in the formation of "auto-antibodies" of the basement membrane zone antibody type.
