Our aim was to develop and validate a new model to predict exacerbations in patients with COPD. Patients and methods: The derivation cohort consisted of patients aged 65 years or over, with a COPD diagnosis, who were followed up over 24 months. The external validation cohort consisted of another cohort of COPD patients, aged 50 years or over. Exacerbations of COPD were defined as symptomatic deterioration requiring pulsed oral steroid use or hospitalization. Logistic regression analysis including backward selection and shrinkage were used to develop the final model and to adjust for overfitting. The adjusted regression coefficients were applied in the validation cohort to assess calibration of the predictions and calculate changes in discrimination applying C-statistics. Results: The derivation and validation cohort consisted of 240 and 793 patients with COPD, of whom 29% and 28%, respectively, experienced an exacerbation during follow-up. The final model included four easily assessable variables: exacerbations in the previous year, pack years of smoking, level of obstruction, and history of vascular disease, with a C-statistic of 0.75 (95% confidence interval [CI]: 0.69–0.82). Predictions were well calibrated in the validation cohort, with a small loss in discrimination potential (C-statistic 0.66 [95% CI 0.61–0.71]). Conclusion: Our newly developed prediction model can help clinicians to predict the risk of future exacerbations in individual patients with COPD, including those with mild disease. Abstract

Randomized trials typically estimate average relative treatment effects, but decisions on the benefit of a treatment are possibly better informed by more individualized predictions of the absolute treatment effect. In case of a binary outcome, these predictions of absolute individualized treatment effect require knowledge of the individual's risk without treatment and incorporation of a possibly differential treatment effect (i.e. varying with patient characteristics). In this paper we lay out the causal structure of individualized treatment effect in terms of potential outcomes and describe the required assumptions that underlie a causal interpretation of its prediction. Subsequently, we describe regression models and model estimation techniques that can be used to move from average to more individualized treatment effect predictions. We focus mainly on logistic regression-based methods that are both well-known and naturally provide the required probabilistic estimates. We incorporate key components from both causal inference and prediction research to arrive at individualized treatment effect predictions. While the separate components are well known, their successful amalgamation is very much an ongoing field of research. We cut the problem down to its essentials in the setting of a randomized trial, discuss the importance of a clear definition of the estimand of interest, provide insight into the required assumptions, and give guidance with respect to modeling and estimation options. Simulated data illustrates the potential of different modeling options across scenarios that vary both average treatment effect and treatment effect heterogeneity. 3. Age- and gender-specific risk of death after first hospitalization for heart failure

Abstract

Abstract Background Hospitalization for heart failure (HF) is associated with high-in-hospital and short- and long-term post discharge mortality. Age and gender are important predictors of mortality in hospitalized HF patients. However, studies assessing short- and long-term risk of death stratified by age and gender are scarce. Methods A nationwide cohort was identified (ICD-9 codes 402, 428) and followed through linkage of national registries. The crude 28-day, 1-year and 5-year mortality was computed by age and gender. Cox regression models were used for each period to study sex differences adjusting for potential confounders (age and comorbidities). Results 14,529 men, mean age 74 ± 11 years and 14,524 women, mean age 78 ± 11 years were identified. Mortality risk after admission for HF increased with age and the risk of death was higher among men than women. Hazard ratio's (men versus women and adjusted for age and co-morbidity) were 1.21 (95%CI 1.14 to 1.28), 1.26 (95% CI 1.21 to 1.31), and 1.28 (95%CI 1.24 to 1.31) for 28 days, 1 year and 5 years mortality, respectively. Conclusions This study clearly shows age- and gender differences in short- and long-term risk of death after first hospitalization for HF with men having higher short- and long-term risk of death than women. As our study population includes both men and women from all ages, the estimates we provide maybe a good reflection of 'daily practice' risk of death and therefore be valuable for clinicians and policymakers.

Abstract

Abstract Background As the population ages, peripheral arterial disease (PAD) in the lower extremities will become a larger public health problem. Awareness in patients as well clinicians of the high risk of morbidity and mortality is important but seems currently low. Insights in absolute mortality risks following admission for PAD in the lower extremities can be useful to improve awareness as they are easy to interpret. Methods A nationwide cohort of 4,158 patients with an initial admission for PAD in the lower extremities was identified through linkage of the national hospital and population register in 1997 and 2000. Results Over 60% of 4,158 patients were men. 28 days, 1 year and 5 year mortality risk were 2.4%, 10.3% and 31.0% for men and 3.5%, 10.4% and 27.4% for women. Coronary heart disease and stroke were frequent cause of death. Five years mortality risk was higher for men compared to women (HR 1.36, 95% CI 1.21–1.53). Conclusion In conclusion, our findings demonstrate that, 5 year mortality risk is high, especially in men and comparable to that of patients admitted for acute myocardial infarction or ischemic stroke. Though, in general population the awareness of the severity of PAD in the lower extremities is significantly lower than that for any other cardiovascular disease and it seems that cardiovascular risk factor management for prevention in PAD patients is very modest.

6. Cardiac imaging to detect coronary artery disease in athletes aged 35 years and older. A scoping review

Abstract

Sudden cardiac death (SCD) is a devastating event in athletes. Screening efforts that were first directed at athletes younger than 35 years, are now focusing on the rapidly growing group of older sportspersons. Athletes aged ≥35 years have a 10-fold increased risk of exercise-related cardiac arrest, mostly due to coronary artery disease (CAD). Although cardiac imaging is pivotal in identifying CAD, the role of imaging modalities in screening asymptomatic older sportspersons remains unclear. We performed a scoping review to identify the role of cardiac imaging to detect CAD in older sportspersons and to identify gaps in the existing literature. We searched Medline, Embase and the Cochrane library for studies reporting data on cardiac imaging of CAD in sportspersons ≥35 years. The systematic search yielded 1737 articles and 14 were included in this scoping review. Imaging modalities included 2 echocardiography, 1 unenhanced Computed Tomography (CT) for coronary artery calcium scoring (CACS), 3 CACS and contrast-enhanced CT angiography (CCTA), 2 CACS and Cardiac Magnetic Resonance (CMR), 1 CCTA with CMR and echocardiography, 2 CCTA, 2 CMR, and 1 myocardial perfusion imaging article. The low number of relevant articles and the selection bias introduced by studying specific groups, like veteran marathon runners, indicate the need for future research. Cardiac CT (CACS and CCTA) probably has the highest potential for pre-participation screening, with high diagnostic value to detect CAD and low radiation dose. However, currently there is insufficient evidence for incorporating routine cardiac imaging in the pre-participation screening of asymptomatic sportspersons over 35 years.

Abstract

Many abstracts of diagnostic accuracy studies are currently insufficiently informative. We extended the STARD (Standards for Reporting Diagnostic Accuracy) statement by developing a list of essential items that authors should consider when reporting diagnostic accuracy studies in journal or conference abstracts. After a literature review of published guidance for reporting biomedical studies, we identified 39 items potentially relevant to report in an abstract. We then selected essential items through a two round web based survey among the 85 members of the STARD Group, followed by discussions within an executive committee. Seventy three STARD Group members responded (86%), with 100% completion rate. STARD for Abstracts is a list of 11 quintessential items, to be reported in every abstract of a diagnostic accuracy study. We provide examples of complete reporting, and developed template text for writing informative abstracts.

9. Methods for Evaluating Medical Tests and Biomarkers

Abstract

Background and objectives: Prognostic models are, among other things, used to provide risk predictions for individuals who are not receiving a certain treatment, in order to assess the natural course of a disease and in turn guide treatment decisions. As treatment availability and use changes over time, researchers may be faced with assessing the validity of existing models in partially treated populations, and may account for treatment use in different ways. We aimed to investigate how treatment use contributes to the poor performance commonly observed in external validation studies, and to explore methods to address the issue. Methods: The effect of treatment use on the observed performance of a model was evaluated analytically. Development data sets representing untreated individuals were simulated using a logistic model and “optimal” models were developed using those sets. Validation sets drawn from the same theoretical population were simulated to receive an effective (binary) treatment. The prevalence and effectiveness of treatment were varied, with and without being dependent on true risk. Model performance in the validation sets was expressed in terms of calibration slope, observed:expected ratio (O:E) and C statistic. We examined the results of i) ignoring treatment, ii) restricting validation to untreated patients, and iii) adjusting the observed event rates to account for treatment effects. This was expressed through the difference (Δ) between each performance measure after applying a method and the value observed in the untreated set. Results: Validation of a model derived in untreated individuals in a treated validation set resulted in poorer model performance than that observed in the same population, if left untreated. Treatment of 50% of patients with a highly effective treatment (higher risk patients had a higher probability of receiving treatment; treatment effect odds ratio: 0.5), resulted in a decrease in the O:E from 1.0 to 0.7, and a decrease in the C statistic from 0.67 to 0.62 when compared to the observed statistics in an untreated set. This trend was observed across settings with different mechanisms for treatment allocation and different population risk distributions. As treatment prevalence and effectiveness increased, the observed model performance almost invariably decreased. Restricting the validation to only untreated individuals resulted in performance measures closer to those observed in the full untreated validation set, at the cost of precision (Δ O:E = 0.0; C statistic = 0.03). When treatment allocation was completely based on risk (I.e. according to a risk threshold), the restriction approach was less effective (ΔO:E = 0.0; ΔC statistic = 0.07). Increasing the observed event rates to account for treatment effects improved the observed model calibration, but this was highly sensitive to incorrect assumptions about treatment use and effectiveness. Conclusions: Validating a model designed to make predictions of the “natural course” of an individual’s health in a validation data set containing treated individuals may result in an underestimation of the performance of the model in untreated individuals. Current methods are not sufficient to account for the effects of treatment, and findings from such studies should be interpreted with caution.

Abstract

Background: Informative titles and abstracts are important for the identification of potentially relevant studies and communication of research results. Many readers and reviewers base their decision to read the full text of a publication on clarity and detail presented in the title and abstract. Clear and informative reporting in title and abstract is therefore essential. The TRIPOD Statement, published in 2015, is a guideline for Transparent Reporting of a multivariable prediction model for Individual Prognosis Or Diagnosis. TRIPOD provides general recommendations for the reporting of title and abstracts, however, more detailed guidance is desirable. Objectives: To develop specific guidance for informative reporting of diagnostic or prognostic prediction model studies in both journal and conference abstracts. Methods: We conducted a literature review on the reporting of prediction model studies and established a list of potentially relevant items to report in abstracts. This list served as the basis for a modified Delphi procedure. In the first round a panel of 110 experts in the field of prediction modeling studies were asked to rate to what extent each candidate item is essential. A maximum of two Delphi rounds will be carried out to reach consensus whether to include an item and to provide insight into potential wording. Results: Preliminary analyses from our literature review showed that objectives, setting, participants, sample size, outcome and conclusions were reported in over 75% of 134 abstracts. Candidate predictors, internal validation technique and results for calibration were addressed in fewer than 25% of abstracts. The modified Delphi procedure is currently being carried out. We will present the results of this procedure and the guidance resulting from it. Conclusions: We present the development of a specific checklist and corresponding guidance for the reporting of diagnostic or prognostic prediction model studies in both journal and conference abstracts: TRIPOD for Abstracts. The guidance will be applicable to abstracts of publications that describe development or external validation of a prediction model.

Abstract

Incomplete reporting has been identified as a major source of avoidable waste in biomedical research. Essential information is often not provided in study reports, impeding the identification, critical appraisal, and replication of studies. To improve the quality of reporting of diagnostic accuracy studies, the Standards for Reporting of Diagnostic Accuracy Studies (STARD) statement was developed. Here we present STARD 2015, an updated list of 30 essential items that should be included in every report of a diagnostic accuracy study. This update incorporates recent evidence about sources of bias and variability in diagnostic accuracy and is intended to facilitate the use of STARD. As such, STARD 2015 may help to improve completeness and transparency in reporting of diagnostic accuracy studies.

27. Are novel non-invasive imaging techniques needed in patients with suspected prosthetic heart valve endocarditis? A systematic review and meta-analysis

Abstract

Multimodal non-invasive imaging plays a key role in establishing a diagnosis of PHV endocarditis. The objective of this study was to provide a systematic review of the literature and meta-analysis of the diagnostic accuracy of TTE, TEE, and MDCT in patients with (suspected) PHV endocarditis. Studies published between 1985 and 2013 were identified via search and cross-reference of PubMed/Embase databases. Studies were included if (1) they reported on the non-invasive index tests TTE, TEE, or MDCT; (2) data was provided on PHV endocarditis as the condition of interest; and (3) imaging results were verified against either surgical inspection/autopsy or clinical follow-up reference standards, thereby enabling the extraction of 2-by-2 tables. Twenty articles (including 496 patients) met the inclusion criteria for PHV endocarditis. TTE, TEE, and MDCT + TEE had a pooled sensitivity/specificity for vegetations of 29/100 %; 82/95 %, and 88/94 %, respectively. The pooled sensitivity/specificity of TTE, TEE, and MDCT + TEE for periannular complications was 36/93 %, 86/98 %, and 100/94 %, respectively. TEE showed good sensitivity and specificity for establishing a diagnosis of PHV endocarditis. Although MDCT data are limited, this review showed that MDCT in addition to TEE may improve sensitivity in detecting life-threatening periannular complications. aEuro cent Multimodal imaging is an important ingredient of diagnostic workup for PHV endocarditis. aEuro cent Transthoracic and transesophageal echography may miss life-threatening periannular complications. aEuro cent MDCT can improve sensitivity for the detection of life-threatening periannular complications.

49. Ignoring dependency between linking variables and its impact on the outcome of probabilistic record linkage studies.

Abstract

OBJECTIVES This study sought to examine the differences between ignoring (naïve) and incorporating dependency (nonnaïve) among linkage variables on the outcome of a probabilistic record linkage study. DESIGN AND MEASUREMENTS We used the outcomes of a previously developed probabilistic linkage procedure for different registries in perinatal care assuming independence among linkage variables. We estimated the impact of ignoring dependency by re-estimating the linkage weights after constructing a variable that combines the outcomes of the comparison of 2 correlated linking variables. The results of the original naïve and the new nonnaïve strategy were systematically compared for 3 scenarios: the empirical dataset using 9 variables, the empirical dataset using 5 variables, and a simulated dataset using 5 variables. RESULTS The linking weight for agreement on 2 correlated variables among nonmatches was estimated considerably higher in the naïve strategy than in the nonnaïve strategy (16.87 vs. 13.55). Therefore, ignoring dependency overestimates the amount of identifying information if both correlated variables agree. The impact on the number of pairs that was classified differently with both approaches was modest in the situation in which there were many different linking variables but grew substantially with fewer variables. The simulation study confirmed the results of the empirical study and suggests that the number of misclassifications can increase substantially by ignoring dependency under less favorable linking conditions. CONCLUSION Dependency often exists between linking variables and has the potential to bias the outcome of a linkage study. The nonnaïve approach is a straightforward method for creating linking weights that accommodate dependency. The impact on the number of misclassifications depends on the quality and number of linking variables relative to the number of correlated linking variables. [ABSTRACT FROM AUTHOR]
