[EN] Introduction and objectives The cardiac sequelae of SARS-CoV-2 infection are still poorly documented. We conducted a cross-sectional study in healthcare workers to report evidence of pericardial and myocardial involvement after SARS-CoV-2 infection. Methods We studied 139 healthcare workers with confirmed past SARS-CoV-2 infection. Participants underwent clinical assessment, electrocardiography, and laboratory tests, including immune cell profiling and cardiac magnetic resonance (CMR). Clinically suspected pericarditis was diagnosed when classic criteria were present and clinically suspected myocarditis was based on the combination of at least 2 CMR criteria. Results Median age was 52 (41-57) years, 71.9% were women, and 16.5% were previously hospitalized for COVID-19 pneumonia. On examination (10.4 [9.3-11.0] weeks after infection-like symptoms), participants showed hemodynamic stability. Chest pain, dyspnea or palpitations were present in 41.7% participants, electrocardiographic abnormalities in 49.6%, NT-proBNP elevation in 7.9%, troponin in 0.7%, and CMR abnormalities in 60.4%. A total of 30.9% participants met criteria for either pericarditis and/or myocarditis: isolated pericarditis was diagnosed in 5.8%, myopericarditis in 7.9%, and isolated myocarditis in 17.3%. Most participants (73.2%) showed altered immune cell counts in blood, particularly decreased eosinophil (27.3%; P < .001) and increased cytotoxic T cell numbers (17.3%; P < .001). Clinically suspected pericarditis was associated (P < .005) with particularly elevated cytotoxic T cells and decreased eosinophil counts, while participants diagnosed with clinically suspected myopericarditis or myocarditis had lower (P < .05) neutrophil counts, natural killer-cells, and plasma cells. Se realizó un estudio transversal en trabajadores sanitarios para estudiar la prevalencia de afección pericárdica y miocárdica tras la infección por SARS-CoV-2. Métodos Se estudió a 139 trabajadores sanitarios con infección previa confirmada por SARS-CoV-2. Los participantes se sometieron a evaluación clínica, electrocardiograma, laboratorio, incluido el perfil de células inmunitarias, y resonancia magnética cardiaca (RMC). El diagnóstico clínico de pericarditis se realizó ante la presencia de los criterios clásicos y el diagnóstico clínico de miocarditis ante la presencia de al menos 2 criterios de RMC. La sospecha clínica de pericarditis se asoció (p < 0,005) particularmente con un elevado número de células T citotóxicas y recuento de eosinófilos disminuidos; mientras que los participantes con sospecha clínica de miopericarditis o miocarditis tenían recuentos de neutrófilos, células natural killer y células plasmáticas más bajos (p < 0,05). Introducción y objetivos Los datos respecto a la utilización del oxigenador extracorpóreo de membrana venoarterial (ECMO-VA) en adultos con shock cardiogénico (SC) son escasos. El objetivo del presente estudio fue describir los datos de un registro multicéntrico español, y analizar qué factores tuvieron un impacto en la mortalidad intrahospitalaria. Métodos Registro retrospectivo multicéntrico español de pacientes en situación de SC profundo que recibieron soporte circulatorio con ECMO-VA. Resultados Desde enero de 2012 hasta octubre de 2018, se asistieron 233 pacientes con ECMO-VA. La edad media fue 56,3 ± 12,18 años y 50 eran mujeres (21,5%). Recibieron el alta hospitalaria 109 pacientes (46,8%). Las complicaciones más frecuentes observadas fueron: insuficiencia renal aguda en 131 (59,0%), hemorragia en 70 (31,7%), hipoxemia en 86 (36,9%) e infección en 99 (43,4%). Los predictores independientes de mortalidad intrahospitalaria fueron lactato al implante (OR = 1,05; IC95%, 1,05-1,19), edad (OR = 2,19; IC95%, 1,05-4,58) y canulación intraparada cardiaca (OR = 1,04; IC95%, 1,01-1,06). Un total de 45 pacientes tenían edad > 60 años y lactato al implante > 8 mmol/l, su mortalidad intrahospitalaria fue de 77,8% y llegó al 91,7% en 12 de ellos en los que la canulación fue intraparada cardiaca. Conclusiones La terapia con ECMO-VA es factible en pacientes con SC profundo. La selección adecuada parece fundamental para evitar la futilidad del implante en pacientes con escasas posibilidades de éxito. Introduction and objectives The data regarding the use of venoarterial extracorporeal membrane oxygenation (VA-ECMO) in adults in with cardiogenic shock (CS) are scarce. The aim of this study was to describe the data of a Spanish multicenter registry, and to analyze which factors had an impact on in-hospital mortality. Methods Retrospective Spanish multicenter registry of patients with deep CS treated with VA-ECMO circulatory support. Results From January 2012 to October 2018, 233 patients were treated with VA-ECMO. Mean age was 56.3 ± 12.18 years and 50 were female (21.5%). A total of 109 patients were successfully discharged (46.8%). The most frequent complications were acute kidney injury in 131 (59.0%), hypoxemia in 86 (36.9%) major bleeding in 70 (31.7%), and infection in 99 (43.4%). The independent in-hospital mortality risk factors were lactate before implant (OR, 1.05; 95%CI, 1.05-1.19), age (OR, 2.19; 95%CI, 1.05-4.58), and cannulation during cardiac arrest (OR, 1.04; 95%CI, 1.01-1.06). A total of 45 patients > 60 years with lactate before implant > 8 mmol/l had an in-hospital mortality rate of 77.8%, in-hospital mortality and reached 91.7% in 12 of them with cannulation during cardiac arrest. Conclusions VA-ECMO therapy for patients with deep cardiogenic shock is feasibly. Appropriate selection seems critical in order to avoid the futility of using ECMO in those patients with low survival chances. Resumen Introduccion y objetivos Se han descrito previamente variantes geneticas del gen HCN4 con el fenotipo combinado de sindrome del seno enfermo (SSE) y miocardiopatia no compactada del ventriculo izquierdo (MCNC). Actualmente hay pocos casos en los que se haya probado esta relacion y ningun caso previo se ha relacionado con dilatacion de la auricula izquierda (DAi). El objetivo es estudiar un trastorno familiar caracterizado por SSE, DAi e hipertrabeculacion/fenotipo de MCNC para identificar las bases geneticas y electrofisiologicas subyacentes. Metodos Se realizo una evaluacion clinica, genetica y electrofisiologica de una familia con SSE y MCNC. Se practicaron electrocardiograma, Holter, ecocardiografia y ergometria, asi como resonancia magnetica en los casos patologicos. Se realizaron pruebas geneticas con tecnicas next-generation-sequencing (NGS) y un estudio funcional de la variante genetica candidata en celulas de ovario de hamster chino. Resultados Se observo bradicardia sinusal en 12 familiares, asociada con criterios diagnosticos de MCNC en 4, hipertrabeculacion en 6 y DAi en 9. Se detecto la variante genetica HCN4 c.1123C > T (p.Arg375Cys) en heterocigosis en todos los pacientes afectados y ausente en individuos normales. Los analisis electrofisiologicos mostraron que la amplitud y las densidades de las corrientes de HCN4 (IHCN4) generadas por canales HCN4 con la variante genetica p.R375C eran significativamente menores que las generadas por canales no mutados. Conclusiones El fenotipo combinado de SSE, DAi y fenotipo de MCNC esta asociado con la variante genetica de HCN4 c.1123C > T (p.Arg375Cys). El gen HCN4 deberia incluirse en el diagnostico genetico de la MCNC y en formas familiares de SSE, pero tambien en individuos con bradicardia sinusal y DAi.

Cardiogenic shock (CS) is a condition associated with high morbidity and mortality. Our study aimed to perform a risk score for in-hospital mortality that allows for stratifying the risk of death in patients with CS.This is a retrospective analysis, which included 135 patients from a Spanish university hospital between 2011 and 2020. The Santiago Shock Score (S3) was created using clinical, analytical, and echocardiographic variables obtained at the time of admission.The in-hospital mortality rate was 41.5%, and acute coronary syndrome (ACS) was the responsible cause of shock in 60.7% of patients. Mitral regurgitation grade III-IV, age, ACS etiology, NT-proBNP, blood hemoglobin, and lactate at admission were included in the score. The S3 had good accuracy for predicting in-hospital mortality area under the receiver operating characteristic curve (AUC) 0.85 (95% confidence interval (CI) 0.78-0.90), higher than the AUC of the CardShock score, which was 0.74 (95% CI 0.66-0.83). Predictive power in a cohort of 131 patients with profound CS was similar to that of CardShock with an AUC of 0.601 (95% CI 0.496-0.706) versus an AUC of 0.558 (95% CI 0.453-0.664). Three risk categories were created according to the S3: low (scores 0-6), intermediate (scores 7-10), and high (scores 11-16) risks, with an observed mortality of 12.9%, 49.1%, and 87.5% respectively (P0.001).The S3 score had excellent predictive power for in-hospital mortality in patients with nonprofound CS. It could aid the initial risk stratification of patients and thus, guide treatment and clinical decision making in patients with CS.

Cardiovascular disease (CVD) involves the second cause of death in low-risk myelodysplastic syndrome (MDS) population. Prospective study to characterise the CVD and to identify predictors for the combined event (CE) cardiovascular event and/or all-cause mortality in transfusion dependent low-risk MDS patients. Thirty-one patients underwent a cardiac assessment including biomarkers and cardiac magnetic resonance (cMR) with parametric sequences (T1, T2 and T2* mapping) and myocardial deformation by feature tracking (FT) and were analysed for clonal hematopoiesis of indeterminate potential mutations. Cardiac assessment revealed high prevalence of unknown structural heart disease (51% cMR pathological findings). After 2·2 [0·44] years follow-up, 35·5% of patients suffered the CE: 16% death, 29% cardiovascular event. At multivariate analysis elevated NT-proBNP ≥ 486pg/ml (HR 96·7; 95%-CI 1·135–8243; P = 0·044), reduced native T1 time < 983ms (HR 44·8; 95%-CI 1·235–1623; P = 0·038) and higher left ventricular global longitudinal strain (LV-GLS) (HR 0·4; 95%-CI 0·196–0·973; P = 0·043) showed an independent prognostic value. These variables, together with the myocardial T2* time < 20ms, showed an additive prognostic value (Log Rank: 12·4; P = 0·001). In conclusion, low-risk MDS patients frequently suffer CVD. Background ICD patients with episodes of nonsustained ventricular tachycardias (NSVT) are at risk of appropriate therapies. However, the relationship between the cycle length (CL) of such NSVTs and the subsequent incidence of appropriate interventions is unknown. Methods 416 ICD patients with LVEF Results We analyzed 2201 NSVTs (mean CL = 323 ms) that occurred in 250 patients; 111 of such episodes were fast (CL ≤ 300 ms). Secondary prevention (HR = 1.7; p p p = 0.04) were independent predictors of appropriate interventions; however, the mean CL of NSVTs was not (p = 0.6). There was a correlation between the mean CL of NSVTs and the CL of the first monomorphic VT: r = 0.88; p 5 NSVTs (r = 0.97; p p Conclusions Unlike the burden, the CL of NSVTs is not a predictor of subsequent appropriate interventions. However, there is a close relationship between the CL of NSVTs and that of arrhythmias that will later lead to appropriate therapies.

Background: The integrated approach to electrical cardioversion (EC) in atrial fibrillation (AF) is complex; candidates can resolve spontaneously while waiting for EC, and post-cardioversion recurrence is high. Thus, it is especially interesting to avoid the programming of EC in patients who would restore sinus rhythm (SR) spontaneously or present early recurrence. We have analyzed the whole elective EC of the AF process using machine-learning (ML) in order to enable a more realistic and detailed simulation of the patient flow for decision making purposes. Methods: The dataset consisted of electronic health records (EHRs) from 429 consecutive AF patients referred for EC. For analysis of the patient outcome, we considered five pathways according to restoring and maintaining SR: (i) spontaneous SR restoration, (ii) pharmacologic-cardioversion, (iii) direct-current cardioversion, (iv) 6-month AF recurrence, and (v) 6-month rhythm control. We applied ML classifiers for predicting outcomes at each pathway and compared them with the CHA2DS2-VASc and HATCH scores. Results: With the exception of pathway (iii), all ML models achieved improvements in comparison with CHA2DS2-VASc or HATCH scores (p < 0.01). Compared to the most competitive score, the area under the ROC curve (AUC-ROC) was: 0.80 vs. 0.66 for predicting (i); 0.71 vs. 0.55 for (ii); 0.64 vs. 0.52 for (iv); and 0.66 vs. 0.51 for (v). For a threshold considered optimal, the empirical net reclassification index was: +7.8%, +47.2%, +28.2%, and +34.3% in favor of our ML models for predicting outcomes for pathways (i), (ii), (iv), and (v), respectively. As an example tool of generalizability of ML models, we deployed our algorithms in an open-source calculator, where the model would personalize predictions. Conclusions: An ML model improves the accuracy of restoring and maintaining SR predictions over current discriminators. The proposed approach enables a detailed simulation of the patient flow through personalized predictions.

Resumen Introduccion y objetivos Describir por primera vez valores de referencia del indice vascular corazon-tobillo (ICT), la velocidad de la onda de pulso brazo-tobillo (VOP-BT), la velocidad de la onda de pulso carotida-femoral (VOP-CF) y el indice de aumento central y establecer relacion con factores de riesgo cardiovascular en poblacion adulta espanola de 35 a 75 anos de edad sin enfermedad cardiovascular. Metodos Estudio descriptivo transversal. Mediante muestreo aleatorio estratificado por edad y sexo, se incluyo a 501 sujetos sin enfermedad cardiovascular, con una media de edad de 55,9 anos; el 50,3% eran mujeres. Mediante los dispositivos SphigmoCor y Vasera VS-1500 se realizaron las mediciones. Resultados Todas las medidas, excepto el indice de aumento central, mostraron valores mayores en varones, y aumentaron con la edad y la presion arterial. Los valores medios fueron: ICT, 8,01 ± 1,44; VOP-BT, 12,93 ± 2,68 m/s; VOP-CF, 6,53 ± 2,03 m/s e indice de aumento central, 26,84 ± 12,79. En el analisis de regresion multiple, la presion arterial media se asocio con las 4 medidas, la glucohemoglobina, con todas excepto el indice de aumento central, y el indice de masa corporal mostro asociacion inversa con el ICT. Por otro lado, la capacidad explicativa de la edad, el sexo y la presion arterial media es para la VOP-BT un 62%; la VOP-CF, un 49%; el ICT, un 54% y el indice de aumento central, un 38%. En la regresion logistica, la hipertension se asocio con el ICT (OR = 3,45), la VOP-BT (OR = 3,44), la VOP-CF (OR = 3,38) y el indice de aumento central (OR = 3,73). Conclusiones Todas las medidas de rigidez aumentan con la edad; el ICT y la VOP-CF presentan valores mayores en los varones y el indice de aumento central, en las mujeres, sin diferencias en la VOP-BT. Background: Right ventricular (RV) to pulmonary circulation (PC) coupling can stratify prognosis in heart failure (HF). In this study, we assessed the prognostic role of the mean velocity of the pulmonary artery (mvPA) determined by cardiac magnetic resonance (CMR) in HF with preserved ejection fraction (HFpEF). Methods: Inclusion of 58 HFpEF outpatients that underwent CMR with measurement of RV–PC coupling parameters including mvPA between 2016 and 2019. The primary combined endpoint was a composite of HF readmissions and all-cause mortality. Results: Optimal cut-off value of mvPA calculated by receiver operating curve for the prediction of the primary endpoint was 9 cm/s. Over a median follow-up of 23 months (interquartile range: 24), 21 patients met the primary endpoint. The primary endpoint was more frequent in patients with mvPA ≤ 9 cm/s, as indicated by Kaplan–Meier survival curves; Log-Rank: 9.193, p = 0.02, regardless of RV dysfunction. On Cox multivariate analysis, mvPA ≤ 9 cm/s emerged as an independent prognostic predictor of the primary endpoint (HR: 4.11, 95% CI: 1.28–13.19, p = 0.017), together with left atrial area by CMR (HR: 1.08, 95% CI: 1.01–1.24, p = 0.034). Conclusions: In our HFpEF cohort, mvPA was associated with a higher rate of the primary endpoint, regardless of RV function, thus enabling identification of patients at higher risk of cardiovascular events before structural damage onset.

Background Determining the mechanism of supraventricular tachycardias with prolonged ventriculoatrial (VA) intervals is sometimes a challenge, because the usual diagnostic manoeuvers based on pacing during the ongoing tachycardia cannot be performed or do not offer diagnostic accuracy. Purpose To analyse the determinants, time course and diagnostic accuracy (atypical atrioventricular nodal reentrant tachycardias [AVNRT] versus orthodromic reentrant tachycardias through an accessory pathway [ORT]) of spontaneous VA intervals variation in patients with narrow QRS tachycardias and prolonged VA. Methods A total of 156 induced tachycardias were studied (44 with atypical AVNRT and 112 with ORT). Two sets of 10 measurements were performed for each patient: after tachycardia induction and one minute later. VA and VV intervals were determined. Results Among patients with AVNRT, there was a marked variability in the VA intervals after induction (Mn-VA: mean beat-to-beat variation of VA; MX-VA: maximum of the beat-to-beat VA variation). Figure 1. No significant differences were found in the variability of the VA interval between slow-slow versus fast-slow atypical AVNRTs (p>0.5 in all comparisons). And, even though the variability of the VA interval at one minute continued to be considerable, it tended to decrease after induction. However, as shown in Figure 1, in subjects with ORT, no relevant beat-to-beat changes in the duration of the VA intervals were seen, neither after induction nor at one minute later. The values of VA interval variability in ORT with septal versus free-wall accessory pathways were similar. Additionally, in 9 (20%) patients with atypical AVNRT and in 11 (10%) with ORT the Dif-VA increased from induction to one minute, with the maximum increment being of 29 ms and 6 ms, respectively. The difference between the longest and the shortest VA interval (Dif-VA) correlates significantly with the diagnosis of atypical AVNRT (C coefficient=0.95 and 0.85 after induction and at one minute, respectively; p Conclusions The analysis of VA interval variability after induction and at one minute later correctly discriminates atypical AVNRT from ORT in almost all cases. Such variability of VA intervals is related to the fluctuations of VV intervals in atypical AVRNTs but not in ORTs.

Resumen Existen pocos temas de actualidad equiparables a la posibilidad de la tecnologia actual para desarrollar las mismas capacidades que el ser humano, incluso en medicina. Esta capacidad de simular los procesos de inteligencia humana por parte de maquinas o sistemas informaticos es lo que conocemos hoy en dia como inteligencia artificial (IA). Este articulo pretende aclarar diferentes terminos que todavia nos resultan lejanos como IA, machine learning (aprendizaje automatico, AA), deep learning (aprendizaje profundo, AP), data science o big data; describir en profundidad el concepto de IA y sus tipos, las tecnicas de aprendizaje y la metodologia que se utiliza en el AA, el analisis en imagen cardiaca con AP, la aportacion de esta revolucion tecnologica a la estadistica clasica, sus limitaciones actuales, sus aspectos legales y, fundamentalmente, sus aplicaciones iniciales en cardiologia. En este sentido se ha realizado una busqueda detallada en PubMed de la evolucion en el ultimo lustro de las contribuciones de la IA a las diferentes areas de aplicacion en cardiologia, y se ha identificado un total de 673 articulos originales. Se describen en detalle 19 ejemplos de diferentes areas de la cardiologia que utilizando IA han mostrado mejoras diagnosticas y terapeuticas, y que facilitaran la comprension de la metodologia AA y AP.

Introduction The weaning from veno-arterial extracorporeal membrane oxygenation (VA-ECMO) decision involves a meticulous evaluation of different factors, among which echocardiography is a decisive factor. Purpose Evaluate echocardiographic changes from the beginning of the support to the weaning test. Methods Analysis of VA-ECMO cases in a referral hospital which completed a weaning test. We evaluated the changes of echocardiographic parameters during the VA-ECMO support (initial and during weaning test) and successful weaning (survival >24h after explant and no mortality from cardiogenic shock/heart failure or cardiac arrest during admission). Results 85 out of 134 VA-ECMO patients were included from 2013 to Oct-2020 (table 1.1). Successful weaning was achieved in 61.2% and final survival at discharge 49.4%. We observed a significant increase in left ventricular ejection fraction (LVEF), left ventricular outflow tract time–velocity integral (LVOT VTI), tricuspid annular plane systolic excursion (TAPSE) and the percentage of patients with normal qualitative right ventricular function and 1:1 aortic valve opening from the initial to the weaning echocardiography (table 1.2), without significant changes in ventricular diameters. Among these changes, the increase in LVOT VTI was the only parameter that was related to weaning success (p=0.037, table 1.2). A higher LVEF increase from the initial value showed a trend to greater successful VA-ECMO disconnection (p=0.053 table 1.2). Conclusion Left and right ventricular function parameters improved significantly from baseline to weaning test echocardiography in VA-ECMO supported patients. Of these changes, the increase in LVOT VTI was related to successful disconnection from VA-ECMO and the LVEF improvement showed a trend to weaning success. Resumen Introduccion y objetivos El hospital de dia del area del corazon (HDC) es una alternativa asistencial a la hospitalizacion convencional relacionada con procedimientos cardiologicos programados. Los objetivos de este estudio son analizar la actividad asistencial, la calidad de la asistencia y el coste-efectividad del HDC. Metodos Estudio observacional descriptivo de la actividad asistencial durante el primer ano de funcionamiento del HDC. La calidad asistencial se analizo mediante el indice de sustitucion (ambulatorizacion de los procedimientos programados), la tasa de cancelacion, complicaciones y una encuesta de satisfaccion. Para el coste-efectividad, se calculo el ahorro economico relacionado con las estancias hospitalarias evitadas. Resultados Se atendio a un total de 1.646 pacientes (media de edad, 69 ± 15 anos; el 60% varones). Se programaron 2.550 procedimientos con una tasa de cancelacion del 4%; la cardioversion electrica fue el procedimiento con mas suspensiones. La ambulatorizacion de los procedimientos invasivos programados fue del 66%. Unicamente fue necesario reingresar a 1 paciente por insuficiencia cardiaca. La mayoria de los pacientes encuestados consideraron buena o muy buena la atencion recibida en el HDC (95%). La ambulatorizacion parcial de los procedimientos invasivos supuso un ahorro economico en estancias hospitalarias de 219.199,55 euros, superior a los costes del primer ano de funcionamiento del HDC. Conclusiones El HDC del centro ha permitido la ambulatorizacion de mas de 2 tercios de los procedimientos invasivos manteniendo la calidad de la asistencia. En el primer ano de funcionamiento se ha amortizado el gasto derivado de su puesta en marcha, gracias a una importante reduccion de los ingresos hospitalarios.

Background: Paravalvular leak occurs in 5–17% of patients following surgical valve replacement, more often in mitral position. The prognosis without treatment is poor. Percutaneous device closure represents an alternative to repeat surgery. The objective of this work is to evaluate the medium and long-term results in the percutaneous closure of PVL in mitral prosthesis. Methods: This observational study is based on a retrospective registry including consecutive mitral PVL cases undergoing percutaneous closure at a single tertiary-care center from April 2010 to December 2020. The safety and efficacy results of the procedure, at 90 days and in the long term, were analyzed. Also, predictors of procedure failure and long-term events were identified. Results: A total of 128 consecutive mitral paravalvular leak closure procedures were included. Technical success was achieved in 115 (89.8%) procedures. The presence of multiple PVLs was the sole factor that independently predicted procedural failure. Median follow-up of our sample was 41.8 months (mean 47.7 ± 35.7 months). Underlying hemolytic anemia as the indication for PVL closure, a recent admission for decompensated HF, and lack of improvement in functional class emerged as consistent predictors of MACE and death during long-term follow-up, while lack of procedural success during the first PVL procedure and chronic kidney disease were also associated with MACE during follow-up. Conclusions: Percutaneous mitral PVL closure displayed high technical and procedural success rates, with an acceptable safety profile, in a high-risk population. Percutaneous mitral PVL closure achieved an improvement in short- and long-term functional class and a reduction of hemolysis in the vast majority of patients. In addition, long-term survival in our study was good, in particular for patients undergoing successful PVL closure procedures.

Aims There is a lack of evidence regarding the benefits of β-blocker treatment after invasively managed acute myocardial infarction (MI) without reduced left ventricular ejection fraction (LVEF). Methods and results The tREatment with Beta-blockers after myOcardial infarction withOut reduced ejection fracTion (REBOOT) trial is a pragmatic, controlled, prospective, randomized, open-label blinded endpoint (PROBE design) clinical trial testing the benefits of β-blocker maintenance therapy in patients discharged after MI with or without ST-segment elevation. Patients eligible for participation are those managed invasively during index hospitalization (coronary angiography), with LVEF >40%, and no history of heart failure (HF). At discharge, patients will be randomized 1:1 to β-blocker therapy (agent and dose according to treating physician) or no β-blocker therapy. The primary endpoint is a composite of all-cause death, non-fatal reinfarction, or HF hospitalization over a median follow-up period of 2.75 years (minimum 2 years, maximum 3 years). Key secondary endpoints include the incidence of the individual components of the primary composite endpoint, the incidence of cardiac death, and incidence of malignant ventricular arrhythmias or resuscitated cardiac arrest. The primary endpoint will be analysed according to the intention-to-treat principle. Conclusion The REBOOT trial will provide robust evidence to guide the prescription of β-blockers to patients discharged after MI without reduced LVEF.

Aims: Anthracycline-induced cardiotoxicity (AIC) is a serious adverse effect among cancer patients. A central mechanism of AIC is irreversible mitochondrial damage. Despite major efforts, there are currently no effective therapies able to prevent AIC. Methods and results: Forty Large-White pigs were included. In Study 1, 20 pigs were randomized 1:1 to remote ischaemic preconditioning (RIPC, 3 cycles of 5 min leg ischaemia followed by 5 min reperfusion) or no pretreatment. RIPC was performed immediately before each intracoronary doxorubicin injections (0.45 mg/kg) given at Weeks 0, 2, 4, 6, and 8. A group of 10 pigs with no exposure to doxorubicin served as healthy controls. Pigs underwent serial cardiac magnetic resonance (CMR) exams at baseline and at Weeks 6, 8, 12, and 16, being sacrifice after that. In Study 2, 10 new pigs received 3 doxorubicin injections (with/out preceding RIPC) and were sacrificed at week 6. In Study 1, left ventricular ejection fraction (LVEF) depression was blunted animals receiving RIPC before doxorubicin (RIPC-Doxo), which had a significantly higher LVEF at Week 16 than doxorubicin treated pigs that received no pretreatment (Untreated-Doxo) (41.5 ± 9.1% vs. 32.5 ± 8.7%, P = 0.04). It was mainly due to conserved regional contractile function. In Study 2, transmission electron microscopy (TEM) at Week 6 showed fragmented mitochondria with severe morphological abnormalities in Untreated-Doxo pigs, together with upregulation of fission and autophagy proteins. At the end of the 16-week Study 1 protocol, TEM revealed overt mitochondrial fragmentation with structural fragmentation in Untreated-Doxo pigs, whereas interstitial fibrosis was less severe in RIPC+Doxo pigs. Conclusion: In a translatable large-animal model of AIC, RIPC applied immediately before each doxorubicin injection resulted in preserved cardiac contractility with significantly higher long-term LVEF and less cardiac fibrosis. RIPC prevented mitochondrial fragmentation and dysregulated autophagy from AIC early stages. RIPC is a promising intervention for testing in clinical trials in AIC. Fil: Galán Arriola, Carlos. BACKGROUND Myocardial deformation with echocardiography allows early detection of systolic dysfunction and is related to myocardial iron overload (MIO) determined by T2* in hereditary anemias under transfusion support. Our aim was to analyze the diagnostic and prognostic usefulness of magnetic resonance feature tracking (MR-FT) myocardial strain in low-risk myelodysplastic syndromes (LR-MDS) patients. METHODS Prospective study in transfusion-dependent LR-MDS patients and healthy controls who underwent a cardiac MR-FT. We analyzed the relationships between strain MR-FT and iron overload parameters and its prognostic impact in cardiovascular events and/or death. RESULTS Thirty-one patients and thirteen controls were included. MIO (T2* < 20 ms) was detected in 9.7% of patients. Left ventricular global longitudinal strain (LV-GLS) by MR-FT was pathological (> −19.3%) in 32.3% of patients. Less negative strain values correlated with lower T2* (R = −0.37, p = 0.033) and native myocardial T1 (R = −0.39, p = 0.031) times. LV-GLS by MR-FT was significantly associated with higher incidence of the combined cardiovascular events and/or all-cause death (p = 0.047), with a cut-off value of −17.7% for predicting them (63% sensitivity and 81% specificity, area under the curve = 0.69). After adjusting analysis including demographic, biomarkers and imaging variables, a higher LV-GLS value by MR-FT remained as predictor of combined event in transfusion-dependent LR-MDS patients (hazard ratio, 0.4; confidence interval, 0.15–0.98; p = 0.045). CONCLUSIONS Longitudinal myocardial strain by MR-FT in LR-MDS patients is associated to MIO and correlates with adverse events in the follow-up, what could serve as a prognostic tool.
