1. Economic evaluation of empowering older patients transitioning from hospital to home: Trial-based analysis of the 'Your Care Needs You' intervention

Abstract

Background: Transitioning from hospital to home presents significant challenges, particularly for patients aged 75 and above. The 'Your Care Needs You' (YCNY) intervention, evaluated through a cluster randomised controlled trial (cRCT), aims to empower these patients by providing essential information and skills to prepare them for being at home. This study evaluates the cost-effectiveness of the YCNY intervention compared to standard care within the UK National Health Service (NHS) over the trial's duration (90 days post-discharge). Methods: The economic evaluation utilised individual patient-level data from the YCNY cRCT. The analysis was conducted from the perspective of the NHS and Personal Social Services. The analysis calculated the adjusted mean differences in costs and Quality-Adjusted Life Years (QALYs) between the YCNY intervention and control groups. In our base case, Multilevel Mixed-Effects Generalised Linear Models (MME-GLM) were employed to account for the hierarchical data structure. Additional alternative scenarios and a probabilistic sensitivity analysis were conducted to assess the robustness of the findings. Results: The base case results indicated the YCNY intervention reduced costs ({\pounds}269) and improved outcomes (QALY gain of 0.0057). The net health benefit (NHB) of the YCNY intervention using a cost-effectiveness threshold of {\pounds}15.000/QALY was 0.0246. These findings suggest the YCNY intervention dominates usual care, with a high probability (89%) of the YCNY intervention being cost-effective at a {\pounds}15,000 per QALY threshold. The results remained robust across several alternative scenario analyses. Discussion: The YCNY intervention could represent a promising and potentially cost-effective approach for enhancing the safety and experience of older adults transitioning from hospital to home in the UK.

Abstract

Despite being the most cost-effective tobacco control policy, tobacco taxation is the least implemented component of the World Health Organization MPOWER package to reduce smoking worldwide. In Mexico, both smoking prevalence and taxation have remained stable for more than a decade. This study aims to provide evidence about the potential effects of taxation to reduce the burden of tobacco-related diseases and the main attributable social costs in Mexico, including informal (unpaid) care costs, which are frequently ignored. We employ a first-order Monte Carlo microsimulation model that follows hypothetical population cohorts considering the risks of an adverse health event and death. First, we estimate tobacco-attributable morbidity and mortality, direct medical costs and indirect costs, such as labour productivity losses and informal care costs. Then, we assess the potential effects of a 50% cigarette price increase through taxation and two alternative scenarios of 25% and 75%. The inputs come from several sources, including national surveys and vital statistics. Each year, 63 000 premature deaths and 427 000 disease events are attributable to tobacco in Mexico, while social costs amount to MX$194.6 billion (US$8.5)—MX$116.2 (US$5.1) direct medical costs and MX$78.5 (US$3.4) indirect costs—representing 0.8% of gross domestic product. Current tobacco tax revenue barely covers 23.3% of these costs. Increasing cigarette prices through taxation by 50% could reduce premature deaths by 49 000 over the next decade, while direct and indirect costs averted would amount to MX$87.9 billion (US$3.8) and MX$67.6 billion (US$2.9), respectively. The benefits would far outweigh any potential loss even in a pessimistic scenario of increased illicit trade. Tobacco use imposes high social costs on the Mexican population, but tobacco taxation is a win-win policy for both gaining population health and reducing tobacco societal costs. Abstract

This article presents the teaching and research experience of a group of teachers who jointly teach an interdisciplinary subject in the bachelor's degree of Early Childhood Education. This subject uses performance art as the unifying element of three disciplines, which are art, music and bodily expression. Although performance art can be found in the realm of education and in academics, this work aims to specifically highlight its ability to allow Early Childhood Education students to connect with the concept of play and to promote a more active and creative teacher profile. Data from student surveys are included, in addition to reflections about the subject by the authors. The collected data cover various topics such as art, pedagogy and organization. Even though the management of such a complex subject entails some challenges and has called for, and still calls for, many changes, the overall result is very positive. Moreover, performance is revealed as a very powerful learning tool for future Early Childhood Education teachers., En este artículo se presenta la experiencia docente e investigadora de un grupo de profesores del Grado en Magisterio de Educación Infantil en una asignatura interdisciplinar que incluye la educación artística, la musical y la expresión corporal a través de la performance como elemento unificador de los tres lenguajes. Si bien la performance está presente en algunos espacios educativos y en el ámbito académico, en este trabajo se quiere poner especialmente en valor su capacidad para servir de conexión con el concepto de juego en el alumnado de Educación Infantil y para actualizar el rol del profesor hacia un perfil más activo y creativo. Además de las reflexiones del equipo docente de la asignatura, se incluyen datos de encuestas realizadas al alumnado, de los que se extraen resultados que abarcan temas artísticos, pedagógicos, organizativos, etc. A pesar de que la gestión de una asignatura tan compleja acarrea algunos problemas y haya implicado y siga implicando muchos cambios, el balance general es muy positivo. La performance, por otra parte, se revela como una herramienta muy poderosa para el aprendizaje del futuro profesorado de Infantil.

These strategies have been configured considering functional and competitive levels for open agricultural production systems, and usually based on low-cost technologies such that Wireless Sensor Networks (WSN), Internet of Things (IoT), Unmanned Aerial Vehicles (UAV), Cloud Computing, and Computational Algorithms. This approach allows the configuration, planning, and implementation of technological strategies for the agricultural sector, impacting in a positive way, generating higher production levels and intensive production cycles to strengthen the smallholder farmers.

Abstract

El cambio climático en el planeta ha afectado la producción mundial de trigo y los pronósticos aseguran que estos cambios y efectos serán mayores conforme pase el tiempo. Considerando la importancia de este cereal como alimento de la dieta básica de los seres humanos, se propone una nueva forma de evaluación del cultivo que permita pronosticar el daño probable por etapa fenológica por medio de una red neuronal artificial entrenada con datos históricos del comportamiento climático global e impacto fenológico. Un sistema de resultados interpreta la salida y constituye la interfaz de usuario. La propuesta fue validada y mostró su capacidad de generalización alcanzando alta precisión de 84%, por lo que se considera adecuada., Climate Change on the planet has affected the world wheat production and forecasts ensure that these changes and effects will be greater over time. Considering the importance of this cereal as a basic diet of human beings, this work proposes a new form of crop evaluation that allows to forecast probable damage by phenological stage, through an Artificial Neural Network trained with historical data of global climatic behavior and phenological impact. A results system interprets the output and constitutes the user interface. The proposal was validated, showed its generalization capacity and reached high precision of 84%, so it is considered adequate.

Resumen: INTRODUCCIÓN : El consumo de alcohol es uno de losprincipales factores de riesgo. Para relevar las condiciones previasal establecimiento de políticas públicas orientadas a disminuirel consumo de alcohol en Argentina, se fijaron tres objetivos: a)caracterizar la demanda y oferta de bebidas alcohólicas; b) evaluarla situación normativa respecto de políticas de control de publicidad,promoción y patrocinio, y las pautas de publicidad televisiva; c) definirqué modelos de carga de enfermedad atribuible y costo-efectividadde las intervenciones podrían ser aplicables. MÉTODOS : Se analizarondatos de la Encuesta Nacional de Gasto de los Hogares 2004/5 y2012/3 y de la base Euromonitor Internacional. Se examinaron elmarco normativo y las acciones televisivas de publicidad, promocióny patrocinio. Se revisó la literatura sobre modelos de carga deenfermedad y costo-efectividad de intervenciones. RESULTADOS :El consumo de bebidas alcohólicas no se modificó entre 2004/5 y2012/3, y la industria está muy concentrada. El marco de regulaciónde la publicidad es laxo; más de la mitad de los avisos de TV seemiten en horario diurno, por lo que se expone a menores y seincumplen los acuerdos internacionales subscritos. Hay tres enfoquesprincipales de modelamiento epidemiológico y económico que podrían ser aplicables. CONCLUSIONES : Se caracterizaron aspectoseconómicos, jurídico-legales y epidemiológicos útiles para impulsaruna agenda pública orientada a disminuir el consumo de alcohol. To assess the preconditions for the establishmentof public policies aimed at reducing alcohol consumption inArgentina, three aims were set: a) to characterize the demandand supply of alcoholic beverages; b) to evaluate the regulatoryframework regarding advertising, promotion and sponsorshipcontrol policies, and television advertising; c) to define which modelsof attributable disease burden and cost-effectiveness of interventions could be applicable. METHODS : Data from the National HouseholdExpenditure Survey 2004/5 and 2012/3 and from the EuromonitorInternational database were analyzed. The regulatory framework,and television advertisements were examined. The literature onmodels of disease burden and cost-effectiveness of interventions was reviewed. RESULTS : The consumption of alcoholic beveragesdid not change significantly between 2004/5 and 2012/3, and theindustry is highly concentrated. The regulation framework foradvertising is lax; more than half of the TV ads are broadcastedduring daytime, exposing minors and thus, violating internationalagreements.
