Background: Periorbital fractional CO2 laser resurfacing has been employed for facial rejuvenation purposes. However, to the best of our knowledge, no study has objectively assessed periorbital neoformation and remodeling of local cutaneous collagen, in a split-face model, from skin samples obtained during upper blepharoplasty.Objectives: The authors sought to objectively evaluate neoformation and remodeling of local cutaneous collagen after periorbital skin fractional CO2 laser resurfacing.Methods: Sixteen female patients presenting with dermatochalasis and periorbital rhytids were evaluated in a prospective and comparative study. All patients underwent unilateral periorbital fractional CO2 laser resurfacing 30 days before upper blepharoplasty. Quantification of types I and III collagen from laser-treated and untreated eyelid skin samples obtained during upper blepharoplasty was assessed with histochemical analysis (Picrosirius Red staining). Laser resurfacing treatment was applied to the untreated side immediately after the upper blepharoplasty. Two blinded, independent physicians evaluated clinical improvement in pretreatment and 1- and 6-month posttreatment digital images.Results: Histochemical analysis showed significantly higher intensity in collagen types I (treated: 158.7 ± 5.3, untreated: 139.2 ± 5.0; P < 0.0001) and III (treated: 105.1 ± 7.7, untreated: 104.1 ± 7.1; P < 0.0001) in the fractional CO2 laser treatment samples; a greater difference was detected in collagen type I. A significant improvement in periorbital rhytidosis was observed 1 month after laser resurfacing (23%); a greater improvement in the periorbital region was observed 6 months after laser resurfacing and upper blepharoplasty (43.67%).Conclusions: Periorbital fractional CO2 laser resurfacing was an effective method to improve palpebral skin, with histochemical evidence of increase in collagen types I and III.Level Of Evidence:4: [ABSTRACT FROM AUTHOR]- Published

Ectropion is characterized by the eversion of the eyelid margin and the consequent exposure of the conjunctiva and cornea. The shortening of the anterior lamella of the lid causes cicatricial ectropion. We described a case of skin pathology causing cicatricial ectropion. The case is about a 68-year-old woman with a 2-year history of psoriatic arthritis. She complained of eyelid tearing and redness for two years. Due to the psoriasis, she presented a very dry skin, also in the periocular region, resulting in cicatricial ectropion. A skin graft was indicated to correct the eyelid malposition. Careful investigation should be performed in patients who have a skin disease that can lead to cicatricial ectropion.

OBJETIVO: Avaliar a influência da toxina botulínica na função lacrimal de pacientes com distonias faciais. MÉTODOS: Pacientes com diagnóstico de espasmo hemifacial ou blefaroespasmo essencial em atividade foram clinicamente avaliados e responderam questionário de desconforto ocular (OSDI) e foram submetidos aos testes de Schirmer I e basal, tempo de ruptura do filme lacrimal e clearance da lágrima para avaliação da função lacrimal. No dia seguinte, os pacientes receberam tratamento padronizado com toxina botulínica. O questionário e todos os exames iniciais foram repetidos 30 dias após o tratamento pelo mesmo examinador. RESULTADOS: Foram incluídos 26 pacientes no estudo, sendo 15 (57,7%) com espasmo hemifacial e 11(42,3%) com blefaroespasmo essencial. Nos pacientes com espasmo hemifacial a média de idade foi 70,9 ± 13,3 anos e a relação masculino/feminino foi de 1:1,5. Nos pacientes com blefaroespasmo essencial a média de idade foi 68,9 ± 8,4 anos com predomínio do sexo feminino (90,9%). Após o tratamento o escore do OSDI e os valores dos testes de Schirmer I e basal diminuíram significantemente nos dois grupos. O valor médio da ruptura do filme lacrimal aumentou significantemente nos dois grupos. No teste do clearance da lágrima houve um aumento no número de olhos que apresentaram drenagem completa da lágrima após o tratamento nos dois grupos. CONCLUSÃO: O tratamento com toxina botulínica melhorou os sintomas de olho seco em pacientes com distonias faciais. Apesar da porção aquosa da lágrima ter diminuído, as alterações no piscar aumentaram a estabilidade e melhoraram a drenagem da lágrima. PURPOSE: To analyze the influence of botulinum toxin on the lacrimal function of patients with facial dystonias. METHODS: Patients with the diagnosis of hemifacial spasm or benign essential blepharospasm were evaluated and invited to answer the Ocular Surface Index Disease (OSID) questionnaire. All patients underwent Schirmer I and basal tests; break-up time (BUT) test and lacrimal clearance evaluation. On the following day, the patients were treated with botulinum toxin. The Ocular Surface Index Disease questionnaire and all the initial tests were reapplied 30 days after the treatment by the same examiner. RESULTS: Twenty-six patients were enrolled in this study, 15 (57.7%) with hemifacial spasm and 11 (42.3%) with benign essential blepharospasm. The mean age of patients with hemifacial spasm was 70.9 ± 13.3 years and the male:female ratio was 1:1.5. In the group of patients with benign essential blepharospasm, the mean age was 68.9 ± 8.4 years with a female preponderance (90.0%). After the treatment, the Ocular Surface Index Disease score, Schirmer I and basal tests score decreased in both groups. The mean of Break-up time test increased significantly in both groups. The lacrimal clearance evaluation showed a greater number of eyes that achieved a complete drainage of the tears after the treatment in both groups. CONCLUSION: The treatment with botulinum toxin improved dry eye symptoms in patients with facial dystonia. Despite of the aqueous portion of tear have decreased, blink modifications improved the tear stability and drainage.

OBJETIVO: Avaliar a qualidade de vida dos portadores de blefaroptose e o impacto da correção cirúrgica sobre os mesmos. MÉTODOS: Foram aplicados questionários adaptados do "Visual Function Questionnaire" (VFQ) antes e após tratamento cirúrgico em 21 pacientes com blefaroptose congênita ou adquirida. RESULTADOS: A pontuação média dos pacientes, no pré-operatório, foi de 74 e, no pós-operatório, 81 e diferença pré e pós-cirurgia estatisticamente significante de 7 (p=0,004). Na avaliação das questões (alfa=0,002) e subdomínios (alfa=0,004) houve diferença estatisticamente significante apenas para o subdomínio saúde mental (p=0,001). CONCLUSÃO: A qualidade de vida dos pacientes com blefaroptose apresentou melhora significante após a intervenção cirúrgica na avaliação com o VFQ. Os resultados reforçam a necessidade do uso de questionários elaborados especificamente para esta doença. PURPOSE: To assess the health-related quality of life in blepharoptosis patients and the impact after surgical management on their quality of life. METHODS: Twenty-one patients with congenital or acquired blepharoptosis completed a questionnaire which was adapted from the Visual Function Questionnaire (VFQ), before and after surgical management. RESULTS: Mean preoperative score was 74 and mean postoperative score was 81 and there was a statistically significant difference between mean pre- and postoperative scores of 7 (p=0.004). There was a statistically significant difference only for the mental health subdomain (p=0.001) in the evaluation of the questions (alpha=0.002) and subdomains (alpha=0.004). CONCLUSION: The quality of life in blepharoptosis patients was significantly better after surgical management in the assessment using the VFQ. The outcomes emphasize the need for of using questionnaires designed specifically for this disease.

OBJETIVOS: Avaliar a importância dos exames objetivos, da história ocular, além da anatomia orbitária e periorbitária, na identificação dos pacientes com risco de desenvolver olho seco no pós-operatório da blefaroplastia. MÉTODOS: Foi realizado estudo prospectivo em 29 pacientes com dermatocálase, que foram avaliados antes e três meses após a blefaroplastia. Mudanças nos sintomas oculares, no exame físico e nos testes objetivos (teste de Schirmer, tempo de quebra do filme lacrimal e coloração com rosa bengala), foram avaliadas. RESULTADOS: Não foram encontradas alterações estatisticamente significativas nos resultados dos testes objetivos, à exceção do teste de Schirmer II, entre o pré e o pós-operatório. Em relação às modificações na anatomia palpebral, foram observadas alterações na fenda palpebral e na excursão do músculo levantador da pálpebra superior, que foram estatisticamente significativas no pós-operatório. CONCLUSÕES: Podemos afirmar que as modificações da fenda palpebral acontecem com elevada freqüência nos pacientes submetidos à blefaroplastia estética. Pode-se concluir ainda que, no presente estudo, os exames objetivos para a avaliação do olho seco não demonstraram alterações significativas no pós-operatório, à exceção do teste de Schirmer II. PURPOSES: To evaluate the importance of objective examinations in conjunction with ocular history and orbital and periorbital anatomy, to identify patients at risk of developing a post-blepharoplasty dry eye complication. METHODS: A prospective study was peformed on 29 patients with dermatochalasis who were studied before and three months after blepharoplasty. Changes in ocular symptoms, physical examination and in objective tests, like Schirmer's test, tear film break-up time and rose Bengal coloration were evaluated. RESULTS: No statistically significant alteration in results of objective tests, except Schirmer II test, was found between the pre- and postoperative period. Regarding eyelid anatomy, alterations in palpebral fissure and excursion of the upper eyelid were observed that were statistically significant postoperatively. CONCLUSIONS: It is possible to state that the palpebral fissure alterations happen frequently in patients submitted to esthetic blepharoplasty. It was also possible to conclude, by this study, that the objective tests for dry eye evaluation, did not show significant alterations postoperatively, except for Schirmer II test.

OBJETIVOS: Comparar a sensibilidade tátil corneal entre olhos normais e aqueles com phthisis bulbi e avaliar as alterações provocadas na sensibilidade tátil corneal pelo uso de lente escleral cosmética em phthisis bulbi. MÉTODOS: Estudo prospectivo de 23 pacientes com phthisis bulbi unilateral. Foram realizadas medidas da sensibilidade tátil corneal em ambos os olhos utilizando-se o estesiômetro de Cochet-Bonnet antes e após 3 meses do uso da lente escleral cosmética. RESULTADOS: Em todos os pacientes, a medida da sensibilidade tátil corneal no olho com phthisis bulbi foi menor do que no contralateral (controle). Em 96% houve diminuição da sensibilidade tátil corneal após a adaptação da lente escleral cosmética. CONCLUSÃO: Após a adaptação da lente escleral cosmética, ocorre diminuição da sensibilidade tátil corneal nos olhos. A fisiopatologia é semelhante à encontrada nos usuários de lentes de contato. PURPOSE: To compare corneal sensitivity between normal eyes and those whith phthisis bulbi and also to analyze the alterations of corneal sensitivity in phthisis bulbi induced by wearing ocular prosthesis. METHODS: Prospective study of 23 patients with unilateral phthisis bulbi. Bilateral cornea sensitivity was evaluated using the Cochet-Bonnet esthesiometer before and after 3 months of wearing ocular prosthesis. RESULTS: In all patients, corneal sensitivity of the eye with phthisis bulbi was lower than that of the normal eye (control). In 96% there was decrease of corneal sensitivity after adaptation of ocular prosthesis. CONCLUSION: After wearing ocular prosthesis, there is a reduction in corneal sensitivity in phthisis bulbi. The pathophysiology seems to be the same as that found in contact lens wearers.

14. Treatment of the Tear Trough Deformity With Hyaluronic Acid.

- Abstract
Background: Volume loss and muscular hyperactivity are two major components of the aging process that contribute to the formation of the folds and wrinkles. Tear trough deformity is one of the most difficult depressions to correct surgically. Objective: The authors evaluate the results of periorbital filling with hyaluronic acid (HA) in a small series of patients. Methods: Between June 2008 and December 2009, 25 patients were treated with HA to correct tear trough deformities. The HA was administered into the preperiosteal tissues with a serial puncture technique and approximately 0.1 mL was injected at each pass. Each patient's before and after photographs were reviewed by three surgeons; to objectively assess the outcomes, a quantitative scale was used to grade the pre- and postinjection results. The significance of subjective aesthetic evaluation of the photographs was evaluated with the Mann-Whitney U-test. Differences were regarded as significant if probabilities were less than 0.05. Results: The mean (SD) volume per side needed to achieve correction was 0.54 (0.27) mL on the right and 0.61 (0.30) mL on the left. Complications included some degree of bruising, erythema, and local swelling. Most patients (88%) had cosmetic improvement according to the independent evaluation. Conclusions: All patients were very satisfied with their results. During the course of the study, the authors determined that the ideal candidates for this treatment are young, with thick skin and a definite hollow. [ABSTRACT FROM AUTHOR]

Objective: To evaluate the results of fifty patients undergoing transcutaneous lower eyelid blepharoplasty., Methods: We conducted a prospective, randomised, controlled trial between April 2005 and May 2007. Patients were randomly divided into two groups: Group 1 consisted of 25 surgical patients undergoing traditional lower blepharoplasty and routine lateral canthopexy; Group 2 consisted of 25 patients undergoing lower blepharoplasty with fat bags transposition and routine lateral canthopexy. We used the Rosenberg Self-Esteem Scale UNIFESP / EPM to assess patients' self-esteem. We also requested the participation of three independent surgeons, who examined the pre and postoperative photographs and quantified the results with the aid of a topographic scale., Results: The mean age was 48.8 years, predominantly females (96%). The analysis of photographs showed a 96% significant improvement. Self-esteem scores improved from a preoperative average of 5.1 (SD = 4.1) to a mean of 3.6 (SD = 3.5) six months after the operation (p = 0.001)., Conclusion: Both procedures are safe and effective with low complication rates, causing improvement of self-esteem assessed six months after the operation.
