Background: Idiopathic intracranial hypertension (IIH) is a neurological disorder characterized by increased intracranial pressure. Whilst lumbar puncture (LP) is necessary for the diagnosis of IIH, its therapeutic effect remains unclear. Our aim was to evaluate the therapeutic effect of a single LP in people with IIH (pwIIH). Methods: In this prospective observational study, we analysed short-term neurological and ophthalmological outcomes in pwIIH before, one (D1) and seven days (D7) after the LP. The primary outcome was the change in papilledema degree from baseline. Secondary outcomes included visual outcomes, morphological changes in optical coherence tomography (peripapillary retinal nerve fibre layer [pRNFL] thickness and ganglion cell layer [GCL] volume) and transbulbar sonography (arachnoid optic nerve sheath diameter [AONSD]), and headache outcomes (peak and median headache severity and burden related to headache). Results: We included 30 pwIIH (mean age 32.8 years [SD 8.4], 93.3% female, median cerebrospinal fluid [CSF] opening pressure 33.0 cmCSF [IQR 26.9–35.3], median body mass index (BMI) 34.8 kg/m2 [IQR 30.9–40.9]). Median pRNFL thickness had decreased significantly at D7 (-9 μm [-62.5, -1.3], p = 0.035), with pRNFL thickness at baseline being associated with the pRNFL change (F(1,11) = 18.79, p = 0.001). Mean AONSD had decreased significantly at both D1 (-0.74 mm [0.14], p < 0.001) and D7 (-0.65 mm [0.17], p = 0.01), with AONSD at baseline being associated with the change in AONSD at both time points (D1: β= -0.89, 95% CI -1.37, -0.42, p = 0.002; D7: β= -0.85, 95% CI -1.42, -0.28, p = 0.007). Peak headache severity was slightly lower at D7 (-1/10 [-3, 0], p = 0.026), whereas median headache severity and headache burden remained unchanged. Conclusions: This short-term follow-up study in pwIIH undergoing a single LP suggests a moderate effect on ophthalmological but not headache outcomes. The usefulness of LP as a therapeutic measure in IIH remains controversial and should likely be reserved for patients with limited treatment options, e.g., in pregnancy or intolerability to medication. Introduction. Ocular trauma is a common cause of permanent vision loss in adults. The combination of an accurate clinical examination and imaging offers the best prognostic indicators for patients and helps to navigate treatment modalities. This is a retrospective chart review of examination and imaging findings for ocular trauma and how they correlate with treatment course and visual acuity (VA) outcomes. Methods. Adult patients with ocular trauma presenting to a single institution between January 2013 and December 2020 were evaluated. Initial examination and imaging findings were compared for associations with each other and with VA outcomes. Results. 136 ocular traumas on 134 patients were included. The median presenting logMAR VA was 2.7 (interquartile range (IQR) 1.2–3.7) with 62% open globe injuries. The most commonly reported finding on initial CT scan was globe deformity (30%), on B‐scan was choroidal detachment (20%), and on ultrasound biomicroscopy was intraocular foreign body, ciliochoroidal effusions, or angle recession (21% each). Worse vision was observed for patients positive for retinal detachment on initial B‐scan compared to those negative for this finding at 6‐month (median logMAR 2.7 vs. 0.5; P < 0.0001) and at final post‐injury evaluation (median logMAR 3.7 vs. 0.4; P < 0.0001). Similarly, worse VA was observed for patients with choroidal detachment on initial B‐scan compared to those without this finding at 6‐month (median logMAR 1.4 vs. 0.5; P = 0.002) and at final post‐injury evaluation (median logMAR 2.0 vs. 0.4; P < 0.0001). If positive conjunctiva/sclera examination findings were identified, 66% had positive findings on B‐scan, whereas if the conjunctiva/sclera examination findings were absent, 41% had positive findings on B‐scan (P = 0.005). If anterior chamber (AC) examination findings were positive, 59% had positive findings on B‐scan, whereas if the AC examination findings were absent, 37% had positive findings on B‐scan (P = 0.03). Discussion. The predictive value of examination findings in this study may offer insight as to long‐term visual prognosis. Objective: To evaluate the peripapillary retinal nerve fiber layer (pRNFL) and ganglion cell--inner plexiform layer thickness (GC--IPL) in patients with multiple sclerosis (MS) by sector and quadrant. Materials and Methods: Seventy-eight eyes of 39 patients with MS and 82 eyes of 41 healthy participants were analyzed using spectral domain optical coherence tomography. Our study defined MS eyes with optic neuritis (ON) as MS ON eyes and those without ON as MS non-ON eyes. Comparisons of the GC-- IPL and pRNFL thicknesses were assessed and the measurements compared with healthy controls (HCs). Results: The comparison of the average and three quadrants (superior, inferior, and temporal) measurements of the pRNFL thickness and the average and six quadrant measurements of the GC--IPL thickness between the MS ON eyes and the MS non-ON eyes revealed statistically significant differences (P < 0.05). The average and four quadrants thickness of pRNFL and the average and six quadrants thickness of GC--IPL were significantly reduced in a comparison of MS ON vs. HC with MS non-ON vs. HC eyes (P < 0.05). Conclusion: The evaluation of pRNFL and GC--IPL thicknesses in MS ON and MS non-ON eyes may be beneficial in determining the central nervous system axonal integrity. [ABSTRACT FROM AUTHOR]

Objectives: To evaluate the effectiveness of optic nerve sheath fenestration (ONSF) on visual functions in patients with increased intracranial pressure (ICP). Materials and Methods: The medical records of 24 eyes of 17 patients who had ICP due to idiopathic intracranial hypertension, cerebral venous sinus thrombosis, or intracranial cyst and underwent ONSF surgery to prevent visual loss were evaluated. Pre- and postoperative visual acuity, optic disc images, and visual field findings were reviewed. Results: The mean age of the patients was 30.4±8.5 years, and 88.2% were female. The patients' mean body mass index was 28.67±6.1 kg/m². The mean follow-up time was 24±12.1 months (range: 3-44). At postoperative 3 months, the mean best-corrected distance visual acuity had improved in 20 eyes (83.3%) and stabilized in 4 eyes (16.7%) compared to preoperative values. In visual field mean deviation, an improvement was observed in 10 eyes (90.9%), while 1 eye (9.1%) eye remained stable. Optic disc edema decreased in all patients. Conclusion: This study indicates that ONSF has beneficial effects on visual function in patients with rapidly progressive visual loss caused by increased intracranial pressure. Objectives: To evaluate the demographic characteristics, clinical findings, and treatment approach of patients with sympathetic ophthalmia (SO). Materials and Methods: The records of 14 patients with SO between 2000 and 2020 were retrospectively reviewed. The patients' Snellen best corrected visual acuity (BCVA), detailed ophthalmological examination, optical coherence tomography (OCT), enhanced depth imaging-OCT (EDI-OCT), fundus fluorescein angiography findings, and treatment approaches were recorded. Results: The study included the 14 sympathizing eyes of 14 patients with SO (7 female, 7 male). The mean age was 48.5±15.4 years (range: 28-75), and the mean follow-up duration was 55.1±48.7 months (range: 6-204). Ten patients (71%) had a history of ocular trauma and 4 (29%) had a history of ocular surgery. The time to symptom onset in the sympathizing eye after trauma or ocular surgery ranged from 15 days to 60 years. The most common posterior segment findings were optic disc edema (36%) and exudative retinal detachment (36%). In the acute period, the mean choroidal thickness value on EDI-OCT was 716.5±63.6 p.m (range: 635-772) and decreased to 296±81.6 p.m (range: 240-415) after treatment. Treatment with high-dose systemic corticosteroid was given to 8 patients (57%), azathioprine (AZA) to 7 (50%), AZA and cyclosporine-A combination to 7 (50%), and tumor necrosis factor-alpha inhibitors to 3 patients (21%). Recurrence was observed in 4 patients (29%) during follow-up. At last follow-up, BCVA values were better than 20/50 in 11 (79%) of the sympathizing eyes. Remission was achieved in 13 patients (93%), but 1 patient (7%) lost her vision due to acute retinal necrosis. Conclusion: SO is a bilateral inflammatory disease that presents with granulomatous panuveitis after ocular trauma or surgery. Purpose. To investigate the baseline parameters of peripapillary regions in both eyes of patients with unilateral branch retinal vein occlusion (BRVO) using optical coherence tomography angiography (OCTA) and their association with best-corrected visual acuity (BCVA). Methods. Forty-eight unilateral BRVO patients were enrolled. The 4.5 × 4.5 mm disc angiogram was acquired in the BRVO eyes and fellow eyes using the OCTA. Radial peripapillary capillary (RPC), whole vessel density, and retinal nerve fiber layer (RNFL) thickness in different regions and optic nerve head (ONH) analysis were automatically calculated. The partition includes the whole image, peripapillary, superior hemifield, inferior hemifield, eight equally divided sectors, and nine evenly divided square areas. Results. All vessel density and capillary vessel density in the whole and peripapillary regions of BRVO eyes were significantly lower than those in fellow eyes. The RNFL peripapillary thickness in BRVO eyes was significantly higher than those in fellow eyes. In eyes with supertemporal vein occlusion, all vessel density both in the superior hemifield and in the G12 region was significantly reduced compared with that in the fellow eyes. The capillary vessel density was significantly lower in the superior hemifield, superior temporal (ST), superior nasal (SN), and temporal superior (TS) areas than in the fellow eyes. The RNFL thickness in the NI, IN, TI, and TS sectors was significantly higher than in fellow eyes (all P < 0.05). Conclusions. OCTA provided quantitative information on peripapillary vascular density and RNFL thickness changes in BRVO. Branch retinal vein occlusion not only affects the blood vessel density in the macular area but also decreases the radial peripapillary capillaries. Purpose. This study aimed to determine the expression levels of vascular endothelial growth factor (VEGF), interleukin-6 (IL-6), intercellular adhesion molecule-1 (ICAM-1), and vascular cell adhesion molecule-1 (VCAM-1) in the aqueous humor of patients with macular edema (ME) caused by branch retinal vein occlusion (BRVO), as well as to investigate the relationship between the cytokines as mentioned earlier and best-corrected visual acuity (BCVA), ME, and the degree of ME from the molecular level. Methods. In a prospective observational study, fluorescein fundus angiography (FFA) and optical coherence tomography (OCT) were used to classify 58 patients with non-ischemic BRVO-ME into three groups according to the degree of ME: 14-mild, 17-moderate, and 27-severe. The specific concentration of IL-6, VEGF, ICAM-1, and VCAM-1 in the aqueous humor was detected using the BD CSCanto™ II Flow Cytometer (US). Spearman or Pearson correlation analysis was used to test the correlation between the levels of BCVA and severity of ME and the expression levels of IL-6, VEGF, ICAM-1, and VCAM-1 in the aqueous humor. Results. According to the obtained data, BCVA did not correlate with the severity of ME, and these four cytokines expression levels in patients' aqueous humor (P > 0.05). Moreover, BCVA did not correlate with mild, moderate, or severe ME as well (P > 0.05). However, the levels of these four cytokines were correlated with the severity of the ME. These underlined cytokines were linked to the mild, moderate, and severe degrees of ME. VEGF was also significantly correlated (r > 0.8, P < 0.0001) with the severity of ME. Conclusions. This study suggests that the severity of ME in BRVO-ME patients is significantly correlated with the expression levels of IL-6, VEGF, ICAM-1, and VCAM-1 in the aqueous humor. Lowering the level of disease-associated cytokines may potentially reduce the degree of ME. Therefore, an in-depth study of the levels and the relationship may provide some evidence for the pathogenesis, treatment, and prevention of BRVO-ME. [ABSTRACT FROM AUTHOR]

Automatic and accurate segmentation of optic disc (OD) and optic cup (OC) in fundus images is a fundamental task in computer-aided ocular pathologies diagnosis. The complex structures, such as blood vessels and macular region, and the existence of lesions in fundus images bring great challenges to the segmentation task. Recently, the convolutional neural network-based methods have exhibited its potential in fundus image analysis. In this paper, we propose a cascaded two-stage network architecture for robust and accurate OD and OC segmentation in fundus images. In the first stage, the U-Net like framework with an improved attention mechanism and focal loss is proposed to detect accurate and reliable OD location from the full-scale resolution fundus images. Based on the outputs of the first stage, a refined segmentation network in the second stage that integrates multi-task framework and adversarial learning is further designed for OD and OC segmentation separately. The multi-task framework is conducted to predict the OD and OC masks by simultaneously estimating contours and distance maps as auxiliary tasks, which can guarantee the smoothness and shape of object in segmentation predictions. The adversarial learning technique is introduced to encourage the segmentation network to produce an output that is consistent with the true labels in space and shape distribution. We evaluate the performance of our method using two public retinal fundus image datasets (RIM-ONE-r3 and REFUGE). Extensive ablation studies and comparison experiments with existing methods demonstrate that our approach can produce competitive performance compared with state-of-the-art methods. Complications of peribulbar anaesthesia include retrobulbar haemorrhage, globe perforation and brainstem anaesthesia. Therefore, this study took measurements relating the proximity of medial canthus to the optic nerve and also the safe angle between orbit and globe using 200 multiplanar reconstructed computed tomography (CT) images of the orbit. The principal results show that in 1.5% of the sample, the optic nerve is within 20 mm of the medial canthus, with a minimum distance of 15 mm. One% have a safe angle of 10 degrees or less between bone and globe. None of the demographic data, nor axial length were predictive of these results. We have shown that there are a minority of patients with unusual orbital anatomy. This places them at a theoretical higher risk of complications. Introduction retinal age derived from fundus images using deep learning has been verified as a novel biomarker of ageing. We aim to investigate the association between retinal age gap (retinal age–chronological age) and incident Parkinson's disease (PD). Methods a deep learning (DL) model trained on 19,200 fundus images of 11,052 chronic disease-free participants was used to predict retinal age. Retinal age gap was generated by the trained DL model for the remaining 35,834 participants free of PD at the baseline assessment. Cox proportional hazards regression models were utilised to investigate the association between retinal age gap and incident PD. Multivariable logistic model was applied for prediction of 5-year PD risk and area under the receiver operator characteristic curves (AUC) was used to estimate the predictive value. Results a total of 35,834 participants (56.7 ± 8.04 years, 55.7% female) free of PD at baseline were included in the present analysis. After adjustment of confounding factors, 1-year increase in retinal age gap was associated with a 10% increase in risk of PD (hazard ratio [HR] = 1.10, 95% confidence interval [CI]: 1.01–1.20, P  = 0.023). Compared with the lowest quartile of the retinal age gap, the risk of PD was significantly increased in the third and fourth quartiles (HR = 2.66, 95% CI: 1.13–6.22, P  = 0.024; HR = 4.86, 95% CI: 1.59–14.8, P  = 0.005, respectively). The predictive value of retinal age and established risk factors for 5-year PD risk were comparable (AUC = 0.708 and 0.717, P  = 0.821). Classic serpiginous choroiditis (CSC) is a rare bilateral, multifocal, recurrent inflammatory disease of the choroid and retina observed mainly in young and middle-aged male patients. We present three cases of otherwise healthy men (ages 30, 52, and 54 years) with the diagnosis of CSC based on clinical and multimodal imaging findings. During the observation period, slow progression of the fundus changes was noted in each patient despite various individualized immunosuppressive treatments (corticosteroids, cyclosporin, cyclophosphamide, azathioprine) used. Progressive macular involvement was associated with a significant irreversible bilateral reduction in visual acuity. Ocular issues are a common reason to present to the emergency department (ED). This case discusses a patient who presented to the ED with unilateral atraumatic partial vision loss. The patient underwent a point of care ultrasound that was concerning for a vitreous hemorrhage. Although vitreous hemorrhages require urgent, rather than emergent evaluation, it is important to differentiate this diagnosis from vitreous and retinal detachment. The aim of the present study was to report two cases of refractory dry eye syndrome (DES) after transconjunctival excision of the palpebral lobe of the lacrimal gland. A 25-year-old female patient with a chief complaint of a palpable mass in both upper eyelids visited our medical center. Preoperative orbital computer tomography showed high-attenuation lesions in both lacrimal glands. Incisional biopsy of the lacrimal gland palpebral lobe via transconjunctival incision was performed in January 2019. At 1 month after the biopsy, a lack of tears and persistent corneal erosions were found in both eyes. Artificial tears, punctal occlusion, autologous serum eye drops, and therapeutic contact lenses were applied in an attempt to control the dry eye symptoms. The patient continues to suffer from intractable DES at 2.5 years after the procedure. The second case involved a 52-year-old female patient who visited our medical center with a chief complaint of a palpable mass in both upper eyelids. Bilateral orbital tumors were diagnosed with preoperative magnetic resonance imaging. An incisional biopsy of the lacrimal gland was performed. Immunoglobulin G4-related dacryoadenitis was confirmed through lacrimal palpebral lobe incisional biopsy. Intractable DES and corneal erosion of her left eye persisted thereafter. A transconjunctival incision is an effective approach for minimizing postoperative scars and is suitable for the biopsy of tumors that are visible through the conjunctiva. After a biopsy of the palpebral lobe of the main lacrimal glands, the secretion of reflex tears decreases due to damage to the secreting ducts of the main lacrimal glands. However, total tear secretion can be maintained by basal tear secretion from the accessory lacrimal glands. In this report, we describe two cases of refractory DES due to decreased total tear secretion, although only the palpebral lobes of the main lacrimal glands were biopsied. [ABSTRACT FROM AUTHOR]

Purpose. To study the variation of iris thicknesses in different regions and explore the possible correlations with age and gender. Methods. Healthy Chinese adults were recruited; the anterior segment of their eyes was imaged by swept-source optical coherence tomography (SS-OCT). The horizontal scan of the right eye was selected, and the thicknesses of both the nasal and temporal irises were measured at 199 evenly spaced points. Results. A total of 233 subjects with an average age of 36.79 ± 10.04 years (range 19 to 62) were included in the final analysis. The mean iris thicknesses of the temporal and nasal sides were 364.79 ± 47.58 μm and 372.44 ± 43.75 μm, respectively. The mean nasal iris thickness was positively correlated with age (β = 0.9 μm/year; P = 0.002), but the temporal one was not (β = 0.077 μm/year; P = 0.806). At 139 points of the nasal iris and 146 points of the temporal iris, the iris thickness was significantly correlated with age (P < 0.05). The thicknesses of the peripheral and pupillary parts were positively correlated with age, while the middle part was negatively correlated with age. No significant difference was observed in the mean iris thickness between genders (temporal: t = 1.597, P = 0.112; nasal: t = 1.870, P = 0.063), but females had a thicker iris than males at 50 points in the temporal side and 49 points in the nasal side (P < 0.05); no point in males was observed to have thicker iris compared to females. Conclusion. Using SS-OCT and the novel method, thicknesses of the iris at different regions were measured. Objective. We aimed to evaluate the effect of inferior rectus muscle thickening on intraocular pressure (IOP) in patients with thyroid-associated ophthalmopathy (TAO). Materials and Methods. We analyzed 33 patients with TAO (50 eyes) who presented with hypotropia in the primary position. There was significant eyeball movement restriction and inferior rectus muscle thickening was confirmed on computed tomography or magnetic resonance imaging. We measured IOP changes in patients with TAO and normal participants in the primary position and upgaze using Icare tonometer. Moreover, we measured the preoperative and postoperative IOP in 13 patients with hypotropia who underwent inferior rectus recession. Results. The average inferior rectus thickness in the TAO and control groups was 0.71 ± 0.13 mm and 0.36 ± 0.12 mm, respectively. In the TAO group, the mean IOP was 16.8 ± 2.1 mm Hg in primary position (hypotropia), which increased by 8.9 ± 2.7 mm Hg to 25.7 ± 3.1 mm Hg in upgaze (horizontal vision). In the control group, the mean IOP in the primary position (horizontal vision) was 15.1 ± 1.9 mm Hg, which increased by 2.5 ± 1.4 mm Hg to 17.6 ± 2.1 mm Hg in upgaze. Compared with normal participants, patients with TAO who presented inferior rectus muscle thickening had a significantly greater increase in the IOP P < 0.0001 . In the patients with TAO who underwent inferior rectus muscle recession, there was a postoperative reduction in the IOP in the horizontal vision by 9.4 ± 5.2 mm Hg. Conclusion. Inferior rectus muscle thickening in patients with TAO causes a significant increase in the IOP upon upgaze compared to that in normal individuals. Given the ease of misdiagnosis as glaucoma, IOP measurement in patients with TAO should be performed in the primary position. [ABSTRACT FROM AUTHOR]

Aim. This is a prospective interventional clinical trial to assess the depth of the demarcation line in transepithelial versus epithelium-off accelerated corneal cross-linking (AXL) in keratoconus patients. Methods. This prospective clinical trial was conducted on 40 eyes of 20 patients. Each patient had transepithelial AXL in one eye and epithelium-off AXL in the contralateral eye applying UVA light with an irradiance of 45 mW/cm2 for 2.4 minutes and 30 mW/cm2 for 4 minutes. The depth of the demarcation line was measured using anterior segment OCT (Topcon 3D OCT-2000) one month postoperative for both eyes. Results. The demarcation line was patchy in 50% of the transepithelial AXL eyes, the other half showing a demarcation line at a mean depth of 183 ± 41.6 μm. In the epithelium-off AXL technique, the demarcation line was well defined in all cases with a mean depth of 219 ± 27.3 μm. There was a statistically significant difference in corneal demarcation line depth between transepithelial and epithelium-off techniques (P = 0.008 and P < 0.05). The shallower demarcation line in the transepithelial group suggests that it is less effective. Conclusion. Based on the depth of the demarcation line, the cross-linking effect of epi-off AXL seems more efficacious than after transepithelial AXL. Purpose. To present a new method of measuring ocular axial dimensions, termed OCT biometry (B-OCT). Design. Observational cross-sectional study and evaluation of new diagnostic technology. Methods. B-OCT was implemented in the spectral domain OCT device for posterior and anterior segment imaging (REVO NX, Optopol Technology). A total of 349 eyes (214 of healthy subjects, 115 of patients with cataract, and 20 with severe macular diseases) were enrolled in the study. The results of B-OCT were compared to swept source OCT-based IOLMaster 700 (Carl Zeiss Meditec). Differences in measurement values between the two biometers were determined using the paired t-test. Agreement was assessed through intraclass correlation coefficients (ICCs) and Bland–Altman plots. Results. B-OCT obtained with REVO NX provides excellent interobserver reproducibility (ICC for: axial length (AXL) = 1.000; central corneal thickness (CCT) = 0.933; anterior chamber depth (ACD) = 0.933; lens thickness (LT) = 0.985) and intraobserver repeatability (ICC for: AXL = 1.000; CCT ≥ 0.994; ACD = 0.998; LT ≥ 0.993). The correlation between measurements made using both devices was outstanding (ICC for: AXL, healthy = 1.000; AXL, cataractous = 1.000; ACD, healthy = 0.998; ACD, cataractous = 0.997; LT, healthy = 0.998; LT, cataractous = 0.997; CCT, healthy = 0.989; CCT, cataractous = 0.979). The mean AXL measurement difference in healthy eyes was −0.001 ± 0.016 mm (the 95% LoA ranged from −0.034 to 0.031); mean ACD difference was 0.000 ± 0.024 mm (95% LoA, −0.047 to 0.047); mean LT difference was −0.002 ± 0.024 mm (95% LoA, −0.050 to 0.046); and mean CCT difference  was −0.8 ± 5.1 μm (95% LoA, −10.81 to 9.26). The mean AXL measurement difference in cataractous eyes was −0.003 ± 0.022 mm (95% LoA, −0.046 to 0.039); mean ACD difference was 0.003 ± 0.029 mm (95% LoA, −0.054 to 0.059); mean LT difference was −0.002 ± 0.025 (95% LoA, −0.051 to 0.048); and mean CCT difference was 2.7 ± 6.4 μm (95% LoA, −9.80 to 15.7). Conclusion. The study shows small, nonsignificant differences between the biometric measurements obtained with REVO NX B-OCT and IOLMaster 700, which is of high significance for IOL power selection. As B-OCT utilizes a conventional OCT device, the measurements of the ocular axial dimensions are combined with high-resolution macular scans for the simultaneous assessment of central retina as a part of screening for macular pathology. The presented method is the first spectral domain OCT-based biometry technique and the only one integrated into a standard OCT device. Thus, it brings novel functionality to OCT technology. Purpose: The aim of this study was to present an experimental optical coherence tomography (OCT)-guided anterior segment (AS) imaging chick embryo model. Through this model, we aimed to reveal similarities and differences between human cornea, AS tissues, and chick embryo tissues by quantitative image analysis.Methods: Ex vivo, the chick embryos' globes were determined by detailed AS camera of spectral-domain (SD)-OCT in 10 fertilized specific pathogen-free eggs on the 20th day. Quantitative image analysis of anterior chamber tissues was performed with SD-OCT in detail. After imaging, cross sections of the chick embryo globes containing cornea with anterior chamber were histologically examined and compared with human tissues. The similarities of our model with data in the human cornea and AS studies in the literature were compared.Results: SD-OCT imaging was able to successfully delineate the AS tissues of chick embryos such as the cornea, iris, lens, pupil, conjunctiva, ciliary body, anterior chamber, and lens. Quantitative semi-automated measurements showed the following: mean central corneal thickness: 213.4 ± 7.05 μm (197-223 μm), mean anterior chamber depth: 878.9 ± 41.74 (804-919 μm), mean anterior chamber area: 2.43 ± 0.16 mm2 (2.17-2.73 mm2), mean corneoscleral junction (limbal) thickness: 322.8 ± 20.05 μm (289-360 μm), and mean iris thickness: 230.4 ± 13.27 μm (203-245 μm). In addition, detailed histological comparisons of the AS tissues with human tissues were evaluated to be very similar.Conclusion: In conclusion, this chick embryo model mimics human tissues and it can be considered as a platform for the study of teratogen-induced malformations and AS dysgenesis during gestation of AS tissues. The article describes the case of pregnancy after rituximab for orbital eosinophilic angiocentric fibrosis (EAF) in a 37-year-old female. Topics discussed include the medical history of the patient such as left squint surgery at 18 months of age, fibroid uterus and endometriosis which causes infertility, improvement in ocular redness symptom of the patient following the commencement of high-dose oral glucocorticoids, and improvement in the inflammatory component of her endometriosis.

Purpose. To observe the recovery process of postexercise Schlemm’s canal (SC) and intraocular pressure (IOP) in healthy individuals. Methods. Twenty healthy individuals were recruited. SC and IOP were evaluated before exercise, immediately after exercise, and 15, 30, and 60 minutes after exercise. Superior, inferior, nasal, and temporal SC quadrants were evaluated using swept-source optical coherence tomography (SS-OCT). Results. Average SC area (3726.81 ± 1167.06 vs. 4660.57 ± 1284.82 µm2) and perimeter (324.11 ± 58.95 vs. 367.19 ± 73.34 µm) increased, and IOP (14.02 ± 2.33 vs. 11.65 ± 1.90 mmHg) decreased significantly during exercise (all p<0.001). After exercise, both SC and IOP recovered to preexercise values, and the recovery time for postexercise SC dimensions (15 minutes) was shorter than that for postexercise IOP (60 minutes). After adjusting for age, gender, axial length, central corneal thickness, and spherical equivalent, postexercise changes in SC dimensions were not significantly associated with postexercise changes in IOP compared with preexercise values (all p>0.05). There were no significant differences in the observable SC proportion before and after exercise (all p>0.05). Conclusions. The exercise-induced SC expansion and IOP reduction could recover to preexercise values after exercise, and SC recovered to preexercise values ahead of IOP. Background: We have shown that the foveal contour was asymmetrical after idiopathic macular hole (MH) closure by pars plana vitrectomy (PPV) with internal limiting membrane (ILM) peeling. The purpose of this study was to determine whether these morphological changes differ in eyes after PPV without ILM peeling. Methods: Ten eyes of 10 patients that underwent PPV without ILM peeling and 12 eyes of 11 patients with ILM peeling were studied. The MH in all eyes was <400 µm in diameter. Six months after the PPV, the macular thickness and foveal slope around the closed MH were determined by spectral-domain optical coherence tomography. The thickness of the ganglion cell complex was measured by another spectral-domain optical coherence tomography instrument >6 months after the surgery. Results: The mean parafoveal retinal thickness in the non-peeled group was 367.1 µm in the nasal (N), 353.0 µm in the temporal (T), 366.9 µm in the superior (S), and 357.3 µm in the inferior (I) sectors. The T, S, and I sectors were significantly thicker than the corresponding sectors in the ILM peeled group (p=0.0008, 0.003, and 0.03, respectively). The mean ganglion cell complex was thicker not only in the N sector but also in the T sector in the non-peeled group. The mean retinal slopes in the non-peeled group (N, 40.2°; T, 37.6°; S, 41.2°; I, 39.5°) were flatter than those in the peeled group (N, 52.3°; T, 43.6°; S, 50.8°; I, 51.9°; p=0.009, 0.09, 0.008, and 0.017, respectively). Imaging corneal biomechanical changes or abnormalities is important for better clinical diagnosis and treatment of corneal diseases. We propose a novel ultrasound-based method, called ocular pulse elastography (OPE), to image corneal deformation during the naturally occurring ocular pulse. Experiments on animal and human donor eyes, as well as synthetic radiofrequency (RF) data, were used to evaluate the efficacy of the OPE method. Using very high-frequency ultrasound (center frequency = 55 MHz), correlation-based speckle tracking yielded an accuracy of less than 10% error for axial tissue displacements of 0.5~\mu \textm or above. Satisfactory speckle tracking was achieved for out-of-plane displacements up to 32~\mu \textm . Using synthetic RF data with or without a pre-defined uniform strain, the OPE method detected strains down to 0.0001 axially and 0.00025 laterally with an error less than 10%. Experiments in human donor eyes showed excellent repeatability with an intraclass correlation of 0.98. The measurement outcome from OPE was also shown to be highly correlated with that of standard inflation. Efficient enhancement of noisy optical coherence tomography (OCT) images is a key task for interpreting them correctly. In this paper, to better enhance details and layered structures of a human retina image, we propose a collaborative shock filtering for OCT image denoising and enhancement. Noisy OCT image is first denoised by a collaborative filtering method with new similarity measure, and then the denoised image is sharpened by a shock-type filtering for edge and detail enhancement. For dim OCT images, in order to improve image contrast for the detection of tiny lesions, a gamma transformation is first used to enhance the images within proper gray levels. Despite the growth in research and development in the area of virtual reality over the past few years, virtual worlds do not yet convey a feeling of presence that matches reality. This is particularly due to the difference in visual perception of flat images as compared to actual 3D. We studied the impact of two parameters of the stereoscopic configuration, namely, the inter-camera distance (ICD) and the presence of a depth of field blur (DOF blur). We conducted an experiment involving 18 participants in order to evaluate this impact, based on both subjective and objective criteria. We examined six configurations which differed in the presence or absence of DOF blur and the value of the ICD: fixed and equal to the anatomical interpupillary distance, fixed and chosen by the participant, or variable, depending on the depth of the viewer’s point of regard (POR). The DOF blur and the variable ICD require the use of an eye tracking system in order to be adjusted with respect to the POR. To our knowledge, no previously published research has tested a gaze-contingent variable ICD along with dynamic DOF blur in a Cave Automatic Virtual Environment. Our results show that the anatomical and variable ICD performed similarly regarding each criterion of the experiment, both being more efficient than the fixed ICD. Besides, as with earlier similar attempts, the configurations with DOF blur obtained lower subjective evaluations. Although mainly not significant, the results obtained by the variable ICD and DOF blur are likely due to a noticeable delay in the parameters update. We also designed a new methodology to objectively compare the geometry and depth rendering, based on the reproduction of the same scene in the real and virtual setups, and then on the study of resulting ocular convergence and angular deviation from a target. In recent years, the iris recognition system has been gaining increasing acceptance for applications such as access control and smartphone security. When the images of the iris are obtained under unconstrained conditions, an issue of undermined quality is caused by optical and motion blur, off-angle view (the user's eyes looking somewhere else, not into the front of the camera), specular reflection (SR) and other factors. Such noisy iris images increase intra-individual variations and, as a result, reduce the accuracy of iris recognition. A typical iris recognition system requires a near-infrared (NIR) illuminator along with an NIR camera, which are larger and more expensive than fingerprint recognition equipment. Hence, many studies have proposed methods of using iris images captured by a visible light camera without the need for an additional illuminator. In this research, we propose a new recognition method for noisy iris and ocular images by using one iris and two periocular regions, based on three convolutional neural networks (CNNs). Experiments were conducted by using the noisy iris challenge evaluation-part II (NICE.II) training dataset (selected from the university of Beira iris (UBIRIS).v2 database), mobile iris challenge evaluation (MICHE) database, and institute of automation of Chinese academy of sciences (CASIA)-Iris-Distance database. Purpose: To identify sectors of the optical coherence tomography (OCT) macular map that could be used to effectively assess structural progression in patients with normal-tension glaucoma (NTG). Methods: This study examined 117 eyes of 117 NTG patients to establish axonal tract-dependent macular sectors, and also examined a separate group of 122 eyes of 81 NTG patients to evaluate the ability of these sectors to reveal glaucoma progression. Longitudinal data, including macular maps from at least 5 OCT examinations performed over at least 2 years, was available for all patients in this group. Circumpapillary retinal nerve fiber layer thickness (cpRNFLT), temporal clockwise sector scans (from 7 to 11 o’clock), macular retinal nerve fiber layer thickness (mRNFLT), and macular ganglion cell layer plus inner plexiform layer thickness (mGCIPLT) were measured with spectral-domain OCT (3D OCT-2000, TOPCON). The axonal tract-dependent macular sectors were identified by calculating Spearman’s rank correlation coefficient for each point on a grid overlaid on the macular map and cpRNFLT in each clockwise scan sector. Trend and event analyses for the slope of progression in each sector and macular map were performed. Visual field progression in the macula was defined by the presence of more than 2 progressive test points in the 16 central test points of the Humphrey field analyzer SITA standard 24–2 program, evaluated with Progressor software. Results: The slope of progression in the entire macular area was -0.22 ± 0.58 μm/year for mRNFLT and -0.35 ± 0.52 μm/year for mGCIPLT. The fastest-progressing mRNFLT sector (-1.00 ± 0.84 μm/year, p < 0.001) and mGCIPLT sector (-1.16 ± 0.63 μm/year, p < 0.001) progressed significantly faster than the overall macula. Classifying patients according to visual field progression showed that baseline mRNFLT in the inferior hemifield, 7 and 8 o’clock sectors, as well as baseline mGCIPLT in the overall macular map, inferior hemifield, and 8 o’clock sector, were significantly lower in progressors (22 eyes) than non-progressors (100 eyes). There were significant differences in mRNFLT slope in 8 o’clock sector and in the fastest progressing sector in progressors and non-progressors, but mGCIPLT did not differ, even in the fastest-progressing sector. Event analysis showed that progression occurred most frequently in inferior mRNFLT and superior mGCIPLT in this study. Conclusion: Axonal tract-dependent OCT macular sectors could effectively reveal structural change in patients with NTG. Furthermore, mRNFLT slope was consistent with visual field progression. Purpose: To investigate the sub-classification of myopic glaucomatous eyes by optic disc and peripapillary features. Methods: Optic disc tilt and torsion were determined from retinal nerve fiber layer photographs. Based on the location of the Bruch’s membrane (BM) opening within the β-zone of the peripapillary atrophy (PPA) area, the widths of β-zone PPA (PPA1W), PPA+BM (PPA2W), and PPA-BM (PPA3W) were measured with enhanced depth imaging spectral-domain optical coherence tomography. Cluster analysis that employed partitioning around medoids was performed with these parameters, the presence of inward rotation of BM ending axial length (AXL), and central corneal thickness. Results: A total of 115 eyes (AXL≥24 mm) were included. Two clusters produced maximum overall silhouette widths (average = 0.43). Visual field (VF) mean deviation was not different between cluster 1 (52 eyes; -4.02±3.01 dB) and cluster 2 (63 eyes; -5.21±5.62 dB; p = 0.174). In cluster 1 compared to cluster 2, optic disc tilt was significantly greater, PPA1W and PPA3W were longer, and AXL was longer (all p<0.001). The presence of an inward rotation of BM ending was more frequent in cluster 2 (p = 0.043). Forty-one eyes (78.8%) in cluster 1 had superior VF defects while 10 eyes (19.2%) had inferior defects, and only one eye (2%) had defects in both hemifields. Eyes in cluster 2 were more evenly distributed: 55.6% had superior defects, 34.9% had inferior defects, and 9.5% had defects in both hemifields (p = 0.023). Conclusions: Myopic glaucomatous eyes characterized by optic disc and peripapillary configurations can be classified as two distinct types, and the most distinct difference between the two was degree of optic disc tilt and width of PPA. This study investigated the topographic relationships among the eyeball and four orbital margins with the aim of identifying the correlation between orbital geometry and eyeball protrusion in Koreans. Three-dimensional (3D) volume rendering of the face was performed using serial computed-tomography images of 141 Koreans, and several landmarks on the bony orbit and the cornea were directly marked on the 3D volumes. The anterior-posterior distances from the apex of the cornea to each orbital margin and between the orbital margins were measured in both eyes. The distances from the apex of the cornea to the superior, medial, inferior, and lateral orbital margins were 5.8, 5.8, 12.0, and 17.9 mm, respectively. Differences between sides were observed in all of the orbital margins, and the distances from the apex of the cornea to the superior and inferior orbital margins were significantly greater in females than in males. The anterior-posterior distance between the superior and inferior orbital margins did not differ significantly between males (6.3 mm) and females (6.2 mm). The data obtained in this study will be useful when developing practical guidelines applicable to forensic facial reconstruction and orbitofacial surgeries. [ABSTRACT FROM AUTHOR]

Perceptual linearity of grayscale images based on a contrast sensitivity model is a widely recognized and used standard for medical imaging visualization. This approach ensures consistency across devices and provides perception of luminance variations in direct relationship to changes in image values. We analyze the effect of aging of the human eye on the precept of linearity and demonstrate that not only the number of just-noticeable differences diminishes for older subjects but also linearity across the range of luminance values is significantly affected. Purpose: To report a case of inadvertent bleb formation presenting 18 months after pars plana M3 Molteno implant tube obstruction in a patient with mixed mechanism glaucoma. Materials and Methods: An 84-year-old Caucasian male with mixed mechanism glaucoma underwent slit-lamp examination, gonioscopy, colour anterior segment photography and anterior segment optical coherence tomography (AS-OCT). Results: An inadvertent bleb developed 18 months after pars plana implant tube re-positioning with a 6/0 Vicryl tie ligature. The bleb was located in the area anterior to the implant plate; it was characterised by a thin, transparent, avascular and multi-cystic wall, with a visible stoma at the posterior edge of the bleb. The bleb was functioning as demonstrated by an intraocular pressure of 6 mm Hg at presentation and a punctate fluorescein uptake pattern of the bleb wall. The bleb over the plate of the Molteno implant was non-functioning, likely secondary to tube obstruction by vitreous in the early postoperative period. AS-OCT showed a tract from the anterior chamber commencing at an entry wound through a corneal tunnel to the posterior stoma at the base of the inadvertent bleb. Conclusions: We hypothesise that the pathophysiologic factors resulting in an inadvertent bleb are a result of a combination of apoptosis, late-onset wound dehiscence and internal gaping of a centrally placed corneal wound. Background: Previous studies of neuromyelitis optica spectrum disorder (NMOSD) using spectral domain optical coherence tomography (SD-OCT) showed that the outer nuclear layer (ONL) in eyes without a history of optic neuritis (ON) was thinner than that of healthy controls. It remains unclear whether the ONL thinning is caused by a direct attack on the retina by an autoantibody or a retrograde degeneration. Objective: To determine the mechanisms involved in the retinal damage in eyes with NMOSD without ON. Methods: SD-OCT was used to determine the thicknesses of the different retinal layers of 21 eyes of 12 NMOSD patients without prior ON and 19 eyes of 10 healthy controls. Eyes with peripapillary retinal nerve fiber layer (RNFL) thinning were excluded to eliminate the confounding effects of retrograde degeneration. Microperimetry was used to determine the central retinal sensitivity. The data of the two groups were compared using generalized estimated equation models to account for inter-eye dependencies. Results: The ganglion cell plus inner plexiform layer and the inner nuclear layer plus outer plexiform layer thicknesses of the NMOSD eyes were not significantly different from that of the control eyes (P = 0.28, P = 0.78). However, the ONL and average macular thickness (AMT) in the NMOSD eyes were significantly thinner than that of the control eyes (P = 0.022, P = 0.036). The retinal sensitivity in the central 10°, 10° to 2°, and 2° sectors were significantly lower in the NMOSD eyes than in the control eyes (P = 0.013, P = 0.022, P = 0.002). Conclusions: The ONL thinning, AMT thinning, and reduced retinal sensitivity in eyes with NMOSD without significant peripapillary RNFL thinning are most likely due to direct retinal pathology. Purpose: To report an asymmetry analysis of macular inner retinal layers using swept-source optical coherence tomography (OCT) and to evaluate the utility for glaucoma diagnosis. Design: Observational, cross-sectional study. Participants: Seventy normal healthy subjects and 62 glaucoma patients. Methods: Three-dimensional scans were acquired from 70 normal subjects and 62 open angle glaucoma patients by swept-source OCT. The thickness of the retinal nerve fiber layer, ganglion cell-inner plexiform layer (GCIPL), ganglion cell complex, and total retina were calculated within a 6.2×6.2 mm macular area divided into a 31×31 grid of 200×200 μm superpixels. For each of the corresponding superpixels, the thickness differences between the subject eyes and contra-lateral eyes and between the upper and lower macula halves of the subject eyes were determined. The negative differences were displayed on a gray-scale asymmetry map. Black superpixels were defined as thickness decreases over the cut-off values. Results: The negative inter-ocular and inter-hemisphere differences in GCIPL thickness (mean ± standard deviation) were -2.78 ± 0.97 μm and -3.43 ± 0.71 μm in the normal group and -4.26 ± 2.23 μm and -4.88 ± 1.46 μm in the glaucoma group. The overall extent of the four layers’ thickness decrease was larger in the glaucoma group than in the normal group (all Ps<0.05). The numbers of black superpixels on all of the asymmetry maps were larger in the glaucoma group than in the normal group (all Ps<0.05). The area under receiver operating characteristic curves of average negative thickness differences in macular inner layers for glaucoma diagnosis ranged from 0.748 to 0.894. Conclusions: The asymmetry analysis of macular inner retinal layers showed significant differences between the normal and glaucoma groups. The diagnostic performance of the asymmetry analysis was comparable to that of previous methods. These findings suggest that the asymmetry analysis can be a potential ancillary diagnostic tool. Purpose: To compare the choroidal structure of the subfoveal area in the eyes of children with anisohypermetropic amblyopia to that of the fellow eyes and to age-matched controls using a binarization method of the images obtained by enhanced depth imaging optical coherence tomography (EDI-OCT). Methods: This study was performed at Nara Medical University Hospital, Tokushima University Hospital, and Kagoshima University Hospital, Japan. Forty amblyopic eyes with anisohypermetropic amblyopia and their fellow eyes (5.9 ± 2.1 years, mean ± standard deviation), and 103 age-matched controls (6.7 ± 2.4 years) were studied. The control eyes were divided into myopic, emmetropic, and hyperopic eyes. The total choroidal area, luminal area and stromal area of the subfoveal choroid were measured by the binarization method. The luminal/stromal ratio and the axial length of the amblyopic eyes were compared to that of the control eyes. Results: The total choroidal area in the amblyopic eyes was significantly larger than that of the fellow eyes (P = 0.005). The luminal/stromal ratio was significantly larger in the amblyopic eyes than that of the fellow eyes (P<0.001) and the control hyperopic eyes (P<0.001). There was a significant negative correlation between the luminal/stromal ratio and the axial length in the control eyes (r = -0.30, P = 0.001), but no significant correlation was found in the amblyopic eyes. Conclusions: The choroidal structure of the amblyopic eyes was different from that of the fellow and the control hyperopic eyes. The choroidal changes are related to amblyopia. The aim of this study was to determine whether objective ocular torsion in intermittent exotropia (IXT) changes after recession surgery, and to evaluate the relationship between change in ocular torsion and clinical parameters in IXT. Sixty patients between 3 and 14 years of age underwent lateral rectus (LR) recession for IXT. Digital fundus photographs were obtained from both eyes of each subject and the disc-foveal angle (ocular torsion) was calculated using image software. We compared the preoperative and postoperative amount of ocular torsion, and analyzed the correlation between the difference in ocular torsion (DOC) and clinical parameters including age, duration of strabismus, stereoacuity, amount of preoperative exodeviation, and mean dose response. We categorized the patients according to DOC value: positive DOC value as group 1, and negative DOC value as group 2. A correlation between ocular torsion dominance and fixation preference was also investigated using the Kappa test. The mean ocular torsion was 15.8 ± 4.6 degrees preoperatively and 13.7 ± 5.1 degrees postoperatively. Compared with preoperative values, the mean ocular torsion showed a significant decrease after LR recession (p<0.001), and a greater preoperative ocular torsion was significantly associated with the amount of DOC (r = 0.37, p<0.001). Degree of stereopsis, mean dose-response, and postoperative exodeviation were significantly different between group 1 (positive DOC) and group 2 (negative DOC) (p<0.001, 0.030, and 0.001 respectively). The Kappa test showed that there was a significant correlation between the dominance of ocular torsion and fixation preference (p = 0.020). Therefore, change in ocular torsion after LR recession can be a useful supplementary indicator for evaluating the degree of fusional control and for predicting postoperative surgical response in IXT. Retinal and intra-retinal layer thicknesses are routinely generated from optical coherence tomography (OCT) images, but on-board software capabilities and image scaling assumptions are not consistent across devices. This study evaluates the device-independent Iowa Reference Algorithms (Iowa Institute for Biomedical Imaging) for automated intra-retinal layer segmentation and image scaling for three OCT systems. Healthy participants (n = 25) underwent macular volume scans using a Cirrus HD-OCT (Zeiss), 3D-OCT 1000 (Topcon), and a non-commercial long-wavelength (1040nm) OCT on two occasions. Mean thickness of 10 intra-retinal layers was measured in three ETDRS subfields (fovea, inner ring and outer ring) using the Iowa Reference Algorithms. Where available, total retinal thicknesses were measured using on-board software. Measured axial eye length (AEL)-dependent scaling was used throughout, with a comparison made to the system-specific fixed-AEL scaling. Inter-session repeatability and agreement between OCT systems and segmentation methods was assessed. Inter-session coefficient of repeatability (CoR) for the foveal subfield total retinal thickness was 3.43μm, 4.76μm, and 5.98μm for the Zeiss, Topcon, and long-wavelength images respectively. For the commercial software, CoR was 4.63μm (Zeiss) and 7.63μm (Topcon). The Iowa Reference Algorithms demonstrated higher repeatability than the on-board software and, in addition, reliably segmented all 10 intra-retinal layers. With fixed-AEL scaling, the algorithm produced significantly different thickness values for the three OCT devices (P<0.05), with these discrepancies generally characterized by an overall offset (bias) and correlations with axial eye length for the foveal subfield and outer ring (P<0.05). This correlation was reduced to an insignificant level in all cases when AEL-dependent scaling was used. Overall, the Iowa Reference Algorithms are viable for clinical and research use in healthy eyes imaged with these devices, however ocular biometry is required for accurate quantification of OCT images. The visual system is able to recognize a scene based on a sketch made of very simple features. This ability is likely crucial for survival, when fast image recognition is necessary, and it is believed that a primal sketch is extracted very early in the visual processing. Such highly simplified representations can be sufficient for accurate object discrimination, but an open question is the role played by color in this process. Rich color information is available in natural scenes, yet artist's sketches are usually monochromatic; and, black-and-white movies provide compelling representations of real world scenes. Also, the contrast sensitivity of color is low at fine spatial scales. We approach the question from the perspective of optimal information processing by a system endowed with limited computational resources. We show that when such limitations are taken into account, the intrinsic statistical properties of natural scenes imply that the most effective strategy is to ignore fine-scale color features and devote most of the bandwidth to gray-scale information. We find confirmation of these information-based predictions from psychophysics measurements of fast-viewing discrimination of natural scenes. We conclude that the lack of colored features in our visual representation, and our overall low sensitivity to high-frequency color components, are a consequence of an adaptation process, optimizing the size and power consumption of our brain for the visual world we live in. Purpose: To determine the factors influencing the anterior lamina cribrosa (LC) surface depth (LCD) in patients with open-angle glaucoma (OAG), focusing on the association between LCD and the position of the central retinal vessel trunk (CRVT) at the anterior LC surface. Methods: Optic nerve heads of 205 OAG eyes were scanned using swept-source optical coherence tomography (SS-OCT). After processing the images using adaptive compensation, the LCD was determined from 11 horizontal B-scan images that divided the optic disc vertically into 12 equal parts. Eyes were divided into two groups (central or peripheral) according to where the CRVT exits from the anterior LC surface. The influence of CRVT position on LCD was evaluated, taking into account age, gender, untreated intraocular pressure (IOP), IOP at optic-disc scanning, retinal nerve fiber layer (RNFL) thickness, visual-field mean deviation, central corneal thickness, and axial length. Results: Patients in the peripheral CRVT group were younger and more myopic, and had a larger mean LCD and thinner global RNFL than those in the central CRVT group (all P≤0.023). On multivariate analysis, the peripheral CRVT location was significantly associated with a larger LCD (P = 0.002), together with the significant association of younger age (P<0.001), higher untreated IOP (P = 0.010), and thinner RNFL (P = 0.003) on the larger LCD. Conclusion: In OAG, CRVT location was an independent factor influencing the LCD, together with age, untreated IOP, and global RNFL thickness. The data indicate that the CRVT may contribute to the resistance of the LC against deformation. A longitudinal prospective observation is required to clarify this relationship. Objective: To report visual outcomes, complication rate, and safety of retropupillary iris-claw intraocular lens (ICIOL) in ectopia lentis in Marfan syndrome (MFS). Design: Retrospective study. Methods: Six eyes of three MFS patients with ectopia lentis underwent surgery for subluxation lens and retropupillary ICIOL implantation from October 2014 to October 2015 at the Department of Ophthalmology, Santa Maria Hospital in Lisbon, Portugal. Demographics, preoperative and postoperative best-corrected visual acuity (BCVA), and intraocular pressure were evaluated. Endothelium cell count was assessed using specular microscopy; anterior chamber depth was measured using Pentacam postoperatively; and intraocular lens position was viewed by ultrasound biomicroscopy. All patients were female; mean age was 20±14.264 years (range: 7-38 years). Results: The average follow-up period was 6.66 months (range: 4-16 months). Preoperative BCVA was 0.568±0.149 logMAR units, and postoperative BCVA was 0.066±0.121 logMAR units. The mean BCVA gain was -0.502±0.221 on the logMAR scale. Postoperative average astigmatism and intraocular pressure were 1.292±0.697 mmHg (range: 0.5-2.25 mmHg) and 16 mmHg (range: 12-18 mmHg), respectively. The average endothelial cell density decreased from 3,121±178 cells/mm2 before surgery to 2,835±533 cells/mm2 after surgery (measured at last follow-up visit) and in the last follow-up, representing an average endothelial cell loss of 9.16%. Mean anterior chamber depth was 4.01 mm (±0.77 mm), as measured by Pentacam. No complications were found intra- or postoperatively in any of the six studied eyes. Conclusion: Retropupillary ICIOL implantation is a safe and effective procedure in the treatment of aphakia in MFS eyes, without capsular support after surgery for ectopia lens. Purpose: To apply M-CHARTS for quantitative measurements of metamorphopsia in eyes with acute branch retinal vein occlusion (BRVO) and to elucidate the pathomorphology that causes metamorphopsia. Methods: This prospective study consisted of 42 consecutive patients (42 eyes) with acute BRVO. Both at baseline and one month after treatment with ranibizumab, metamorphopsia was measured with M-CHARTS, and the retinal morphological changes were examined with optical coherence tomography. Results: At baseline, metamorphopsia was detected in the vertical and/or horizontal directions in 29 (69.0%) eyes; the mean vertical and horizontal scores were 0.59 ± 0.57 and 0.52 ± 0.67, respectively. The maximum inner retinal thickness showed no association with the M-CHARTS score, but the M-CHARTS score was correlated with the total foveal thickness (r = 0.43, p = 0.004), the height of serous retinal detachment (r = 0.31, p = 0.047), and the maximum outer retinal thickness (r = 0.36, p = 0.020). One month after treatment, both the inner and outer retinal thickness substantially decreased. However, metamorphopsia persisted in 26 (89.7%) of 29 eyes. The posttreatment M-CHARTS score was not correlated with any posttreatment morphological parameters. However, the posttreatment M-CHARTS score was weakly correlated with the baseline total foveal thickness (r = 0.35. p = 0.024) and closely correlated with the baseline M-CHARTS score (r = 0.78, p < 0.001). Conclusions: Metamorphopsia associated with acute BRVO was quantified using M-CHARTS. Initial microstructural changes in the outer retina from acute BRVO may primarily account for the metamorphopsia. Purpose. To describe the morphological characteristics and efficacy of OCTA in detecting CNV in nAMD. We retrospectively reviewed 53 patients (86 eyes) with suspected CNV secondary to wet AMD. All the patients underwent a multimodal assessment for CNV. Two independent readers calculated the sensitivity and specificity of OCTA in detecting CNV compared with FA. A qualitative analysis of OCTA was also performed to describe the morphological appearance of CNV. Among 86 eyes of 53 patients, 52 eyes were diagnosed as having CNV based on the FA imaging analysis. According to FA, CNV was classified as classic in 28 eyes, predominantly classic in 6 eyes, minimally classic in 9 eyes, and occult in 9 eyes. In 56 eyes, CNV was visualized on OCTA and corresponding OCT B-scans. In total, 46.4% (26/56) had well-circumscribed vessels, and 53.6% (30/56) showed poorly circumscribed vessels. There were 11 false positives and 7 false negatives using OCTA. The specificity of OCTA for the detection of CNV was 67.6%, with sensitivity of 86.5%. Context: Sunglasses should follow minimum requirements to sufficiently protect eyes. It is not known whether all items obtainable from the market are appropriately designed. Aims: To compare ultraviolet (UV)-protective properties of commercially available sunglasses obtained from authorized and unauthorized Iranian sellers. Settings and Study Design: An analytic-descriptive study performed in a metropolitan area (Tehran). Materials and Methods: Using a UV-visible standard spectrophotometer, the percentage transmittance was scanned between 280 and 400 nm in 348 pairs of nonprescription sunglasses (price range: 20-80 US$) obtained anonymously and randomly from authorized (permitted by the Ministry of Health, 189 pairs) and unauthorized (159 pairs) sellers in the Iranian capital city, Tehran. The Australian/New Zealand Standard (AS/NZS) and the American National Standards Institute [ANSI] standards were followed. Statistical Analysis: Chi-square test, independent samples t-test or Mann-Whitney U-test. Results UV-protective properties of the sunglasses obtained from authorized sellers complied with AS/NZS and ANSI guidelines in 92.6% and 95.2% of items, respectively. The corresponding rates for sunglasses obtained from unauthorized sellers were 0% and 8.2%, respectively (P < 0.001 for both). The rate of defective polarizing capability of lenses was 27.4% in sunglasses obtained from authorized sellers versus 90.4% in sunglasses obtained from unauthorized sellers (P < 0.001). Neither brand nor price played significant contributions to UV protection/lens polarizing capability of sunglasses obtained from authorized sellers. Conclusions: Sunglasses provided by unauthorized sellers are alarmingly unreliable and could be potentially hazardous for the eye. We report ophthalmic and genetic findings in families with autosomal recessive rod-cone dystrophy (arRCD) and RP1 mutations. Detailed ophthalmic examination was performed in 242 sporadic and arRCD subjects. Genomic DNA was investigated using our customized next generation sequencing panel targeting up to 123 genes implicated in inherited retinal disorders. Stringent filtering coupled with Sanger sequencing and followed by cosegregation analysis was performed to confirm biallelism and the implication of the most likely disease causing variants. Sequencing identified 9 RP1 mutations in 7 index cases. Eight of the mutations were novel, and all cosegregated with severe arRCD phenotype, found associated with additional macular changes. Among the identified mutations, 4 belong to a region, previously associated with arRCD, and 5 others in a region previously associated with adRCD. Our prevalence studies showed that RP1 mutations account for up to 2.5% of arRCD. These results point out for the necessity of sequencing RP1 when genetically investigating sporadic and arRCD. It further highlights the interest of unbiased sequencing technique, which allows investigating the implication of the same gene in different modes of inheritance. Aim: The aim was to evaluate subjects with a moderate cup to disc ratio using optical coherence tomograph (OCT) and Heidelberg retina tomograph (HRT) 3. Settings and Design: We included 80 patients with early glaucoma and 80 nonglaucomatous subjects with moderate cup/disc ratio (range of 0.5-0.8) to this cross-sectional study. Subjects and Methods: We compared results of color-coded algorithms of HRT 3 (Moorfields regression analysis [MRA] and Glaucoma probability score [GPS]) and OCT. All outputs are classified into three categories: Within normal limits (WNLs), borderline and outside normal limits (ONLs). Diagnostic accuracies of algorithms were determined using the highest sensitivity criteria. Results: The sensitivities of global MRA, GPS and OCT were 0.75, 0.925 and 0.725, respectively, in average disc area group and 0.85, 1.0 and 0.425, respectively, in large disc area group. The specificities of global MRA, GPS and OCT were 0.55, 0.15 and 0.85, respectively, in average disc area group and 0.425, 0.025 and 0.80, respectively, in large disc area group. Area under receiver operating characteristic curve (AUROC) of global MRA, GPS and OCT were 0.667, 0.617 and 0.792, respectively, in average disc area group and 0.746, 0.576 and 0.627, respectively, in large disc area group. AUROC of global MRA and OCT combination in the average and large disc area groups were 0.828 and 0.825, respectively. Conclusions: In contrast to GPS and OCT algorithms, diagnostic performance of MRA algorithm increased in large disc area group. The article discusses the evolution of optical coherence tomography (OCT) into a spectral-domain (SD) that can provide an image of the retinal pigment epithelium (RPE) through the principle of Fourier transformation. A comparison between SD and time-domain (TD) technology is presented. Also discussed are quantitative and qualitative analysis of RPE in patients with early and intermediate dry age-related macular degeneration (AMD), visualizing RPE deposits, and imaging of pseudodrusen.

Spectral-domain optical coherence tomography (SD-OCT) is a three-dimensional imaging technique that allows direct visualization of retinal morphology and architecture. The various retinal layers may be affected differentially by various diseases. An automated graph search algorithm is developed to sequentially segment 11 retinal surfaces in SD-OCT volumes using a three-stage approach. In stage 1, the fourmost easily discernible and/or distinct surfaces are identified in four-times-downsampled images and are used as a priori information to limit the graph search for the other surfaces in stage 2. Eleven surfaces were then detected in two-times-downsampled images in stage 2, and refined in the original images in stage 3 using the graph search integrating the estimated morphological shape models. Twenty macular SD-OCT volume scans from 20 normal subjects are used in this initial study. The overall mean and absolute mean differences in border positions between the automated and manual segmentation for the 11 surfaces are -0.20 ± 0.53 voxels (-0.76 ± 2.06 µm) and 0.82 ±0.64 voxels (3.19 ± 2.46 µm), respectively. Intensity/reflectivity and thickness properties in various retinal layers are also investigated. Abstract: Imaging is a beneficial aid to the oculoplastic surgeon especially in orbital and lacrimal disorders when the pathology is not visible from outside. It is a powerful tool that may be benefited in not only diagnosis but also management and follow-up. The most common imaging modalities required are CT and MRI, with CT being more frequently ordered by oculoplastic surgeons. Improvements in technology enabled the acquisition times to shorten incredibly. Radiologists can now obtain images with superb resolution, and isolate the site and tissue of interest from other structures with special techniques. Better contrast agents and 3D imaging capabilities make complicated cases easier to identify. Color Doppler imaging is becoming more popular both for research and clinical purposes. Magnetic resonance angiography (MRA) added so much to the vascular system imaging recently. Although angiography is still the gold standard, new software and techniques rendered MRA as valuable as angiography in most circumstances. Stereotactic navigation, although in use for a long time, recently became the focus of interest for the oculoplastic surgeon especially in orbital decompressions. Improvements in radiology and nuclear medicine techniques of lacrimal drainage system imaging provided more detailed analysis of the system. [Copyright &y& Elsevier]

Purpose: To evaluate the etiological factors, diagnosis, follow-up and treatment procedures in cases of orbital cellulitis. Material and Method: A retrospective review was performed on medical records of patients with orbital cellulitis treated between 2009 and 2011 in our clinic. The patients were studied for age, ophthalmologic examination features, laboratory and radiology results, treatment modalities and the response to these treatments. Results: Eleven patients (7 male, 4 female) having an average age of 9.7 years (6 months-25 years) participated in the study. All patients had eyelid oedema, hyperemia and ocular pain; with chemosis in 3, gaze restriction in 6, relative afferent pupillary defect and proptosis in 4 cases. Orbital cellulitis was observed to occur secondary to paranasal sinusitis in 10 patients (90.9%) and 1 patient had only history of superior respiratory tract infection. Sinus infection was localized in ethmoid and maxillary sinuses in all except one case. Laboratory tests showed leukocytosis in 8 patients. Surgical drainage was performed in 7 of 8 patients with abscess formation observed with radiological imaging. The remainder of the patients were followed with sole medical treatment. All patients recovered without any vision loss or life-threatening complication. Discussion: Orbital cellulitis cases must be followed with radiologic imaging for any complication that may occur and patients with abscess should be evaluated for surgical drainage besides antimicrobial treatment.(Turk J Ophthalmol 2012; 42: 284-7) [ABSTRACT FROM AUTHOR]
