PurposePosterior fossa ependymoma comprises two distinct molecular variants termed EPN_PFA and EPN_PFB that have a distinct biology and natural history. The therapeutic value of cytoreductive surgery and radiation therapy for posterior fossa ependymoma after accounting for molecular subgroup is not known.MethodsFour independent nonoverlapping retrospective cohorts of posterior fossa ependymomas (n = 820) were profiled using genome-wide methylation arrays. Risk stratification models were designed based on known clinical and newly described molecular biomarkers identified by multivariable Cox proportional hazards analyses.ResultsMolecular subgroup is a powerful independent predictor of outcome even when accounting for age or treatment regimen. Incompletely resected EPN_PFA ependymomas have a dismal prognosis, with a 5-year progression-free survival ranging from 26.1% to 56.8% across all four cohorts. Although first-line (adjuvant) radiation is clearly beneficial for completely resected EPN_PFA, a substantial proportion of patients with EPN_PFB can be cured with surgery alone, and patients with relapsed EPN_PFB can often be treated successfully with delayed external-beam irradiation.ConclusionThe most impactful biomarker for posterior fossa ependymoma is molecular subgroup affiliation, independent of other demographic or treatment variables. However, both EPN_PFA and EPN_PFB still benefit from increased extent of resection, with the survival rates being particularly poor for subtotally resected EPN_PFA, even with adjuvant radiation therapy. Patients with EPN_PFB who undergo gross total resection are at lower risk for relapse and should be considered for inclusion in a randomized clinical trial of observation alone with radiation reserved for those who experience recurrence.

BackgroundPatients with incomplete surgical resection of medulloblastoma are controversially regarded as having a marker of high-risk disease, which leads to patients undergoing aggressive surgical resections, so-called second-look surgeries, and intensified chemoradiotherapy. All previous studies assessing the clinical importance of extent of resection have not accounted for molecular subgroup. We analysed the prognostic value of extent of resection in a subgroup-specific manner.MethodsWe retrospectively identified patients who had a histological diagnosis of medulloblastoma and complete data about extent of resection and survival from centres participating in the Medulloblastoma Advanced Genomics International Consortium. We collected from resections done between April, 1997, and February, 2013, at 35 international institutions. We established medulloblastoma subgroup affiliation by gene expression profiling on frozen or formalin-fixed paraffin-embedded tissues. We classified extent of resection on the basis of postoperative imaging as gross total resection (no residual tumour), near-total resection (30 Gy vs no craniospinal irradiation). The primary analysis outcome was the effect of extent of resection by molecular subgroup and the effects of other clinical variables on overall and progression-free survival.FindingsWe included 787 patients with medulloblastoma (86 with WNT tumours, 242 with SHH tumours, 163 with group 3 tumours, and 296 with group 4 tumours) in our multivariable Cox models of progression-free and overall survival. We found that the prognostic benefit of increased extent of resection for patients with medulloblastoma is attenuated after molecular subgroup affiliation is taken into account. We identified a progression-free survival benefit for gross total resection over sub-total resection (hazard ratio [HR] 1·45, 95% CI 1·07-1·96, p=0·16) but no overall survival benefit (HR 1·23, 0·87-1·72, p=0·24). We saw no progression-free survival or overall survival benefit for gross total resection compared with near-total resection (HR 1·05, 0·71-1·53, p=0·8158 for progression-free survival and HR 1·14, 0·75-1·72, p=0·55 for overall survival). No significant survival benefit existed for greater extent of resection for patients with WNT, SHH, or group 3 tumours (HR 1·03, 0·67-1·58, p=0·89 for sub-total resection vs gross total resection). For patients with group 4 tumours, gross total resection conferred a benefit to progression-free survival compared with sub-total resection (HR 1·97, 1·22-3·17, p=0·0056), especially for those with metastatic disease (HR 2·22, 1·00-4·93, p=0·050). However, gross total resection had no effect on overall survival compared with sub-total resection in patients with group 4 tumours (HR 1·67, 0·93-2·99, p=0·084).InterpretationThe prognostic benefit of increased extent of resection for patients with medulloblastoma is attenuated after molecular subgroup affiliation is taken into account. Although maximum safe surgical resection should remain the standard of care, surgical removal of small residual portions of medulloblastoma is not recommended when the likelihood of neurological morbidity is high because there is no definitive benefit to gross total resection compared with near-total resection.FundingCanadian Cancer Society Research Institute, Terry Fox Research Institute, Canadian Institutes of Health Research, National Institutes of Health, Pediatric Brain Tumor Foundation, and the Garron Family Chair in Childhood Cancer Research.

The development of targeted anti-cancer therapies through the study of cancer genomes is intended to increase survival rates and decrease treatment-related toxicity. We treated a transposon-driven, functional genomic mouse model of medulloblastoma with 'humanized' in vivo therapy (microneurosurgical tumour resection followed by multi-fractionated, image-guided radiotherapy). Genetic events in recurrent murine medulloblastoma exhibit a very poor overlap with those in matched murine diagnostic samples (

Higiene e segurança no trabalho ,Apicultura - Abstract
This study's goal was to produce empirical evidence on the relationship between the dimensions of positive psychological capital (PsyCap) and the dimensions of perceived performance, as well as what moderating effect the scouting experience - and its duration - may have on this relationship. Based on existing literature, the following hypotheses were extracted: The PsyCap dimensions significantly, and positively influence the performance dimensions (1), the scouting experience moderates the relationship between the PsyCap dimensions and the performance dimensions in a manner that magnifies the positive relationship between them (2) the scouting experience, and duration moderate the relationship between the PsyCap dimensions and the performance dimensions in a manner that magnifies the positive relationship between them (3). For this study, a sample of 570 Portuguese workers, of which 411 participants were, or are, participants of the CNE (Corpo Nacional de Escutas), were inquired by means of an online questionnaire, which included the measuring scales of the variables of interest to this study - namely the scales of PsyCap and perceived performance. The results showed that PsyCap dimensions are partially related to perceived performance dimensions, as well as that the scouting experience doesn't moderate this relationship. However, the results also showed that the duration of the scouting experience partially moderates the relationship between PsyCap and perceived performance dimensions. Therefore, this investigation contributes towards research in the field of PsyCap and its relationship with perceived performance, specifically the organizational practice of the processes of attraction, selection, integration and individual performance of collaborators.

Objectives: To identify and characterise the paediatric cases of Auditory Neuropathy Spectrum Disorder (ANSD) and to analyse its rehabilitation. Study Design: Descriptive observational study. Material e Methods: Analysis of clinical processes from 671 patients evaluated in a first auditory rehabilitation appointment between 2012 and 2019. From 467 cases with sensorineural hearing loss, those with absent or profoundly altered brainstem auditory evoked responses and present otoacoustic emissions and/or pure tone thresholds disproportional to electrophysiological results were included. Twelve cases of ANSD were obtained. Results: The prevalence of ANSD was 2.6%. Most cases had risk factors for hearing loss. Audiometric results were heterogeneous and fluctuating. Four children were rehabilitated with hearing aids and three with cochlear implants. Conclusion: ANSD is an infrequent condition. It is associated with several risk factors and its diagnosis and management are challenging. Customised and guided by the child’s functional development auditory rehabilitation is mandatory. Desenho do Estudo: Estudo observacional descritivo.

Study Question: Do children born after vitrified-thawed embryo transfers (ETs) using donated oocytes have worse perinatal outcomes when compared with fresh ET?Summary Answer: No significant difference in birthweight and prematurity rates between fresh or frozen embryo transfers (FETs) in newborns after oocyte donation was found. What Is Known Already: Autologous singletons born after fresh ET have been previously associated with higher rates of preterm birth and low birthweight, while FETs seem to confer a higher risk of hypertensive disorders during pregnancy and macrosomia. However, studies comparing these outcomes using autologous oocytes are unable to adequately disentangle the putative detrimental consequences of embryo vitrification from the possible effects that ovarian stimulation and endometrial preparation may have on endometrial receptivity prior to ET. The oocyte donation model is, for this reason, a more appropriate setting to study these hypotheses; however so far, the information available regarding neonatal outcomes in this patient population is limited to either small and/or heterogeneous studies.Study Design, Size, Duration: We performed a multicentre retrospective cohort study including 5848 singletons born between 2009 and February 2020 following oocyte donation and single blastocyst transfer, subdivided according to whether a fresh ET or FET was performed. We also performed two additional sensitivity analyses, subgrouping the sample according to the type of endometrial preparation (natural versus artificial) and whether the donated oocytes had previously been vitrified or not.Participants/materials, Setting, Methods: Patients with a first singleton livebirth after single blastocyst transfer were compared using multivariable regression analysis to account for potential confounding factors. The primary outcome was birthweight. Secondary outcomes were birthweight z-scores and percentiles, small/large for gestational age, gestational age at delivery, gender, prematurity (<37 weeks and <32 weeks), neonatal morbidity (Apgar scores and need for neonatal intensive care) and maternal morbidity (gestational hypertensive disorders, gestational diabetes and caesarean delivery).Main Results and the Role Of Chance: There was no significant difference between the fresh ET and FET groups in terms of mean birthweight (3215 g versus 3200 g) and birthweight z-scores (0.03 versus 0.1), in both the unadjusted and confounder-adjusted models. However, artificial endometrial preparation was associated with a higher birthweight (3220 g versus 3105 g) and birthweight z-scores (0.06 versus -0.13) when compared with a transfer in a natural cycle. Although a 1-day statistically significant difference in gestational age at birth (275 versus 274 days) was detected, premature birth rates (<37 weeks) did not vary significantly between groups (9.9% and 11.2% for fresh ET and FET, respectively). No other statistically significant differences were found in the remaining neonatal and maternal outcomes studies between the fresh ET and FET groups.Limitations, Reasons For Caution: This study is limited by its retrospective design and lack of information regarding congenital malformations. Moreover, the sample selection criteria that were used may limit the generalizability of our results.Wider Implications Of the Findings: Perinatal outcomes did not seem to be affected significantly by the embryo vitrification process in an oocyte donation model. Hence, other factors may contribute to the hindered perinatal outcomes described in ART, particularly the potential effect that ovarian stimulation and endometrial preparation may have on endometrial receptivity.Study Funding/competing Interest(s): No specific funding was obtained for this study. Metabolic remodeling is a critical skill of malignant cells, allowing their survival and spread. The metabolic dynamics and adaptation capacity of cancer cells allow them to escape from damaging stimuli, including breakage or cross-links in DNA strands and increased reactive oxygen species (ROS) levels, promoting resistance to currently available therapies, such as alkylating or oxidative agents. Therefore, it is essential to understand how metabolic pathways and the corresponding enzymatic systems can impact on tumor behavior. Cysteine aminotransferase (CAT) per se, as well as a component of the CAT: 3-mercaptopyruvate sulfurtransferase (MST) axis, is pivotal for this metabolic rewiring, constituting a central mechanism in amino acid metabolism and fulfilling the metabolic needs of cancer cells, thereby supplying other different pathways. In this review, we explore the current state-of-art on CAT function and its role on cancer cell metabolic rewiring as MST partner, and its relevance in cancer cells' fitness. publishersversion published

Summary Introduction: 'Elective' is an opportunity to deepen clinical competences and/or work in a different assistance reality and promotes personal curriculum. Subjects and methods: A descriptive analysis of the elective internships carried from 2012 to 2017 at the last year of course and compare international and national ones. The students complete an online survey regarding the activities which include demographic, development and outcomes variables. Results: 253 clinical internships carried out by 154 medical students with a mean age of 28 years and 55% were women. The majority (59%) chosen to carry out international internship, particularly United Kingdom (8.3%) and Brazil (7.5%), and only 11% stayed in Algarve region. Half of the internships were carried out in tertiary hospitals being observational-practical (66%). Until half of students (47%) chose medical specialties not included in official curriculum being preference areas: international health (15%) and anesthesiology (8.3%). The majority of internships (66%) were observational-practical. The students are responsible for all the necessary arrangements, being external financial support practically no existent). The final internship rank student was 4.46, on a scale of 0-5, and 99% would recommend the experience to their colleagues. Conclusions: The 'elective' aims to promote students ability to adapt a new professional and social realities, test their competences and sometimes confront themselves with their potential future professional choice.

Cellulose acetate replaces celluloid in image and sound records at the beginning of the 20th century, having been presented as a non-flammable substitute. However, this material is inherently unstable, thus putting at risk a significant part of our cultural heritage from the end of the 19th century and 20th century, where it supports the layer(s) where the image and/or sound are located. Despite this risk, the fundamental studies carried out in the second half of the 20th century, do not yet allow a global understanding of its degradation mechanisms. In order to overcome this gap and to propose innovative and sustainable packaging methods for cellulose derivatives, the European project NEMOSINE (https://nemosineproject.eu) was born. This dissertation is part of this project, proposing an in-depth analysis of the material and its degradation mechanisms. Thus, taking into account that the degradation mechanism is based on the hydrolysis of the side (ester) and main (ether) chains, it is shown that it is possible to follow its scission through infrared spectroscopy, by reducing the bands associated with OH and COC distensions. It was also proved that through its reason it is possible to calculate the degree of substitution of cellulose acetate (DS), which is related to the degree of degradation. Thus, a tool for monitoring degradation is proposed, in reference and historical samples, supported by a calibration curve (DS = 2.03r2 - 4.90r + 2.93; R2 = 0.973); which was inspired by the strategy developed by Fei et al.. In the future, this tool will be fundamental in the diagnosis and monitoring of several case studies in museums or archives. This tool was tested as part of a pioneering study, where samples of cellulose acetate films (DS = 2.4 and 3.0) and historical films from the Phonogrammarchiv of the Vienna Academy of Sciences were irradiated (irr ≥ 280 nm, 60ºC). This irradiation simulates, in an accelerated way, the degradation process. The results show that the weight loss in the films is related to the scission of the main and lateral chains, with the first presenting a faster kinetics. These results are in accordance with the mechanism proposed in the literature. It was also possible to correlate the scission of the side chain with physical properties that determine the performance of these films. Thus, a decrease in hardness can be related with the degree of substitution. In addition, it is concluded that historical films have degraded more rapidly than reference films. These data point to the need to deepen: i) the effect of additives such as plasticizers and, ii) the influence of the complex stratigraphy of historical films, which contemplates the possibility of migration of compounds between the different layers. The diagnosis of the conservation status of the cellulose acetate used in three sculptures created in the 1960s by José Escada (Relief orange, La fête and Dans la plage) was also made.

HIV-1 subtypes associate with differences in transmission and disease progression. Thus, the existence of geographic hotspots of subtype diversity deepens the complexity of HIV-1/AIDS control. The already high subtype diversity in Portugal seems to be increasing due to infections with sub-subtype A1 virus. We performed phylogenetic analysis of 65 A1 sequences newly obtained from 14 Portuguese hospitals and 425 closely related database sequences. 80% of the A1 Portuguese isolates gathered in a main phylogenetic clade (MA1). Six transmission clusters were identified in MA1, encompassing isolates from Portugal, Spain, France, and United Kingdom. The most common transmission route identified was men who have sex with men. The origin of the MA1 was linked to Greece, with the first introduction to Portugal dating back to 1996 (95% HPD: 1993.6-1999.2). Individuals infected with MA1 virus revealed lower viral loads and higher CD4+ T-cell counts in comparison with those infected by subtype B. The expanding A1 clusters in Portugal are connected to other European countries and share a recent common ancestor with the Greek A1 outbreak. The recent expansion of this HIV-1 subtype might be related to a slower disease progression leading to a population level delay in its diagnostic., Supported by FEDER, COMPETE, and FCT by the projects NORTE-01-0145-FEDER-000013, POCI-01-0145-FEDER-007038 and IF/00474/2014; FCT PhD scholarship PDE/BDE/113599/2015; FCT contract FCT IF/00474/2014; European Funds through grant BEST HOPE (project funded through HIVERA, grant 249697) and by FCT PTDC/DTP-EPI/7066/2014. The elderly population faces challenges related to ageism and the media have an important role in the construction, perpetuation and deconstruction of the stereotypes related to this phase of life. Children are especially sensitive to images transmitted by the media and animation films constitute a privileged content by this public and can have a strong influence on the way they understand the world. Identifying and analyzing the representation of older people in animated films can be an important tool to understand how animated films represent the aging process and the elderly, which represent a strong influence on the child and adolescent public. In order to carry out the analysis of these representations, we present the proposal of an analysis grid, which results from the adaptation of the analysis grid developed and used in previous studies. published
