Non-neoplastic cysts localized in salivary glands are quite rare and represent approximately 2-5% of all salivary gland lesions. Salivary duct cyst, also known as sialocyst is a true cyst with the epithelium lining the inner side of its walls. It is generally observed in minor salivary glands and it rarely involves the parotid. Patients are mostly affected between the ages of 30 and 40 and the lesion is rarely seen in children. The current case is a rare report, as the patient was a child. The diagnosis has been difficult to ascertain due to the presence of pain, symptom characteristic for inflammatory lesions. We present the case of a 64-year old female, with a 5 year history of symptomatic multiple cerebral cavernomas, who developed paresthesia in the right leg and arm, followed by vertigo, slurred speech and forward gaze diplopia. Brain investigation revealed two lesions with supra- and infratentorial localization with signs of recent hemorrhage of different ages. The basal cell adenoma (BCA) is a rare tumor entity of the salivary glands. It represents an adenoma monomorphic form, first mentioned by Kleinasser and Klein in 1967. (13) The most frequent location is at the level of the parotid gland. It presents itself usually as a tumorous firm mass, mobile, with slow increase. Microscopically, the tumour contains monomorphic basal cells with the absence of myoepithelial cells. Histologically, it is characterised by the presence of hilar stroma presence, soft, and with the absence of the myxoid or chondroid stroma. It presents similarities with the mixed tumour, but also with the adenocarcinoma with basal cells and with the cystic adenoid carcinoma. Therefore, questions may arise regarding the diagnostic and treatment. It tends to be multifocal and the rate of relapse after the surgical excision is high. Taking into consideration all these aspects, in this paper we would want to show a case of adenoma with basal cells with parotid localization which presented a series of clinical-morphological particularities in parallels with the data existing in the specialized literature. Aim: In this work, we compared the histological features of the gingival lesions clinically diagnosed as fibrotic overgrowths due to various etiologic factors as well as an immunohistochemical assessment of fibroblasts phenotypic heterogeneity using the specific labeling for vimentin, α-smooth muscle actin (α-SMA) and fibroblast specific protein-1 (FSP1)., Materials and Methods: Tissue samples were obtained from 12 patients clinically diagnosed with fibrotic gingival overgrowth, divided in four groups. Fragments of gingiva were processed for paraffin embedding. Serial sections were used for routine staining Hematoxylin-Eosin, trichromic Masson and Goldner-Szekely, and for immunohistochemical reactions to label vimentin, α-SMA and FSP1 using for signal amplification several techniques (EnVision, LSAB, ABC)., Results: Storage of collagen fibers, increase of fibroblast number and frequent presence of inflammatory infiltrate are histological issues of all fibrotic gingival overgrowth. The incidence of granulation tissue varies but the frequency of its presence point the attention to the involvement in collagen metabolism imbalance. Immunostaining for vimentin showed a difference between its expression in samples from different groups. Except the cases of fibrosis induced by orthodontic devices, cells positive for α-SMA were rare. FSP1-positive fibroblasts were the most frequent in all cases from all the groups selected for this study., Conclusions: The phenotype of fibroblasts is different in gingival fibrosis in relation to the risk factor, at present the most common being vimentin-positive and FSP1-positive fibroblasts. Myofibroblasts are rare in gingival fibrosis, the most numerous being in local lesions caused by wearing orthodontic devices and in syndromic fibromatosis. Further studies are required to elucidate the manner in which the active fibroblasts are recruited in relation to the etiologic factor of gingival overgrowth.
