Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is a 'highly transmissible respiratory pathogen, leading to severe multi-organ damage. However, knowledge regarding SARS-CoV-2-induced cellular alterations is limited. In this study, we report that SARS-CoV-2 aberrantly elevates mitochondrial bioenergetics and activates the EGFR-mediated cell survival signal cascade during the early stage of viral infection. SARS-CoV-2 causes an increase in mitochondrial transmembrane potential via the SARS-CoV-2 RNA-nucleocapsid cluster, thereby abnormally promoting mitochondrial elongation and the OXPHOS process, followed by enhancing ATP production. Furthermore, SARS-CoV-2 activates the EGFR signal cascade and subsequently induces mitochondrial EGFR trafficking, contributing to abnormal OXPHOS process and viral propagation. Approved EGFR inhibitors remarkably reduce SARS-CoV-2 propagation, among which vandetanib exhibits the highest antiviral efficacy. Treatment of SARS-CoV-2-infected cells with vandetanib decreases SARS-CoV-2-induced EGFR trafficking to the mitochondria and restores SARS-CoV-2-induced aberrant elevation in OXPHOS process and ATP generation, thereby resulting in the reduction of SARS-CoV-2 propagation. Furthermore, oral administration of vandetanib to SARS-CoV-2-infected hACE2 transgenic mice reduces SARS-CoV-2 propagation in lung tissue and mitigates SARS-CoV-2-induced lung inflammation. Vandetanib also exhibits potent antiviral activity against various SARS-CoV-2 variants of concern, including alpha, beta, delta and omicron, in in vitro cell culture experiments. Taken together, our findings provide novel insight into SARS-CoV-2-induced alterations in mitochondrial dynamics and EGFR trafficking during the early stage of viral infection and their roles in robust SARS-CoV-2 propagation, suggesting that EGFR is an attractive host target for combating COVID-19.

Abstract It would be clinically valuable if the efficacy of antiarrhythmic drugs could be simulated in advance. We developed a digital twin to predict amiodarone efficacy in high-risk atrial fibrillation (AF) patients post-ablation. Virtual left atrium models were created from computed tomography and electroanatomical maps to simulate AF and evaluate its response to varying amiodarone concentrations. As the amiodarone concentration increased in the virtual setting, action potential duration lengthened, peak upstroke velocities decreased, and virtual AF termination became more frequent. Patients were classified into effective (those with virtually terminated AF at therapeutic doses) and ineffective groups. The one-year clinical outcomes after AF ablation showed significantly better results in the effective group compared to the ineffective group, with AF recurrence rates of 20.8% vs. 45.1% (log-rank p = 0.031, adjusted hazard ratio, 0.37 [0.14-0.98]; p = 0.046). This study highlights the potential of a digital twin-guided approach in predicting amiodarone’s effectiveness and improving personalized AF management. Abstract Background The impact of delaying atrial fibrillation catheter ablation (AFCA) for antiarrhythmic drug (AAD) management on the disease course remains unclear. This study investigated AFCA rhythm outcomes based on the diagnosis‐to‐ablation time (DAT) and AAD responsiveness in participants with persistent AF (PeAF). Methods We included data from 1038 AAD‐resistant PeAF participants, all of whom had a clear time point for AF diagnosis, especially PeAF at diagnosis time, and had undergone an AFCA for the first time. Participants who experienced recurrences of paroxysmal type on AAD therapy were analyzed as a cohort of AAD‐partial responders; those maintaining PeAF on AAD were AAD‐non‐responders. We determined the DAT cutoff for best discriminating long‐term rhythm outcomes using a maximum log‐likelihood estimation method based on the Cox proportional hazard regression model. Results Of the participants (79.8% male; median age 61), 806 (77.6%) were AAD‐non‐responders. AAD‐non‐responders had a higher body mass index and a larger left atrial diameter than AAD‐partial‐responders. The pandemic has changed daily urban life, including leisure activities and travel patterns. Although several studies have tracked changes in leisure and travel behavior due to the pandemic, few have analyzed the effects of COVID-19 preventive behavior by considering the interaction between activities and walking for leisure. This study aims to investigate the mutual relationship between outdoor activities and walking for leisure during the pandemic among 537 residents in the Seoul metropolitan area using a three-level least squares (3SLS) approach. The results demonstrate a bidirectional association between outdoor activities and walking during the pandemic. Individuals who avoided long-distance leisure travel increased their frequency of short-distance walking, and postponing or canceling social interactions led to more outdoor leisure activities. Notably, those who refrained from going out for leisure exhibited reduced outdoor activities but an increase in walking for leisure. These findings highlight the significance of considering the synergy between leisure activities and walking amid the pandemic.

Background Increased left atrial pressure (LAP) contributes to dyspnea and heart failure with preserved ejection fraction in patients with atrial fibrillation (AF). The purpose of this study was to investigate the differences in baseline LAP and LAP response to rapid pacing between paroxysmal and persistent AF. Abstract Although pulmonary vein isolation (PVI) gaps and extrapulmonary vein triggers contribute to recurrence after atrial fibrillation (AF) ablation, their precise mechanisms remain unproven. Our study assessed the impact of PVI gaps on rhythm outcomes using a human AF digital twin. We included 50 patients (76.0% with persistent AF) who underwent catheter ablation with a realistic AF digital twin by integrating computed tomography and electroanatomical mapping. We evaluated the final rhythm status, including AF and atrial tachycardia (AT), across 600 AF episodes, considering factors including PVI level, PVI gap number, and pacing locations. Abstract Background Homeostasis model assessment for insulin resistance (HOMA-IR) is a biomarker for type 2 diabetes mellitus (T2DM). However, the role of HOMA-IR in the non-diabetic is unclear. This study aimed to determine whether IR measured HOMA-IR value is associated with new onset diabetes as well as vascular disease and can be used as an early predictor for diabetes and vascular diseases in non-diabetic participants. Methods From a prospective community-based cohort of 10,030 individuals, 4314 individuals younger than 65 years and without diabetes were enrolled and divided into three groups by baseline HOMA-IR tertiles: low (n = 1454), moderate (n = 1414), and high (n = 1446). The primary outcome was new onset T2DM. Secondary outcomes were chronic kidney disease (CKD) and a composite of coronary artery disease, myocardial infarction, and stroke as macrovascular events. Results The mean age was 51 years. The prevalence of hypertension and cholesterol and HbA1c were higher in the high HOMA-IR group. New onset T2DM (5.8%) and CKD (12.2%) incidence in the high HOMA-IR group was higher than that in the others. The prevalence of macrovascular events did not differ among groups. High-HOMA-IR was an independent risk factor for new onset T2DM (odds ratio 1.86 [1.17–2.96]; p = 0.01) and CKD (1.49 [1.12–1.98]; p = 0.01). Conclusions High HOMA-IR was an early predictor of new onset T2DM and CKD, regardless of HbA1c in non-diabetic individuals. Further research on the specific cut off value will be needed.

Background Although successful atrial fibrillation (AF) ablation can maintain sinus rhythm and reduce the left atrial (LA) dimension, blunted LA reverse remodeling can be observed in patients with atrial myopathy. We explored the potential mechanisms and long‐term outcomes in patients with blunted LA reverse remodeling after successful AF catheter ablation. Methods and Results We included 1685 patients who underwent baseline and 1‐year follow‐up echocardiograms, had a baseline LA dimension ≥40 mm, and did not have a recurrence of AF within a year. The patients were divided into tertile groups according to the delta value of the change in LA dimension on the preprocedure and 1‐year postprocedure echocardiography. After propensity score matching for age, sex, AF type, and LA dimension, 1272 patients were finally included in the analyses (424 in each group; the least/blunted, moderate, and the most reverse remodeling group). The patients in the T1 group (blunted LA reverse remodeling) were independently associated with higher left ventricular mass index (odds ratio [OR], 1.014 [95% CI, 1.005–1.022], P=0.001), change in ΔH2FPEF score (heavy, hypertensive, atrial fibrillation, pulmonary hypertension, elder, filling pressure) score (OR, 1.445 [95% CI, 1.121–1.861], P=0.004), ventricular epicardial adipose tissue volume (OR, 1.010 [95% CI, 1.003–1.017], P=0.003), thinner LA wall thickness (OR, 0.461 [95% CI, 0.271–0.785], P=0.004), lower LA voltage (OR, 0.670 [95% CI, 0.499–0.899], P=0.008), and showed higher long‐term AF recurrence (log‐rank P

Wastewater treatment plants (WWTPs) generate useful data, but effectively utilizing these data remains a challenge. This study developed novel ensemble tree-based models to enhance real-time predictions of chemical oxygen demand (COD) and total nitrogen (TN) concentrations, which are difficult to monitor directly. The effectiveness of these models, particularly the Voting Regressor, was demonstrated by achieving excellent predictive performance even with the small, volatile, and interconnected datasets typical of WWTP scenarios. By utilizing real-time sensor data from the anaerobic–anoxic–oxic (A2O) process, the model successfully predicted COD concentrations with an R2 of 0.7722 and TN concentrations with an R2 of 0.9282. In addition, a novel approach was proposed to assess A2O process performance by analyzing the correlation between the predicted C/N ratio and the removal efficiencies of COD and TN. During a one and a half year monitoring period, the predicted C/N ratio accurately reflected changes in COD and TN removal efficiencies across the different A2O bioreactors. The results provide real-time COD and TN predictions and a method for assessing A2O process performance based on the C/N ratio, which can significantly aid in the operation and maintenance of biological wastewater treatment processes.

With growing environmental pollution caused in part by increase in particulate matter (PM), indoor air quality is becoming an important issue in modern society particularly as health and environment consciousness rises. In addition, as the need for a mask has emerged to prevent the spread of viruses such as the SARS-CoV-2 virus that causes the dreaded COVID-19, filter technology has become the focus of considerable research attention. In this study, we introduce a functional fiber that can improve the energy-efficiency for PM adhesion while lowering the pressure drop in an existing commercial high-efficiency particulate absorbing (HEPA) filter. Nanofibers made of polymer and metal nanoparticles were produced by electrospinning; the triboelectricity generated by friction between fibers and air was used to capture the PM. The PM adhesion performance and pressure loss of the developed filter (PVDF/Au) and the commercial HEPA filter were compared, and the charging effect of the filters was analyzed by comparing the power generation performance of the triboelectric nanogenerator. Compared to HEPA filter, PVDF/Au filter reduced pressure drop by about 25%, but, the PM adhesion-energy efficiency of PVDF/Au was superior to that of HEPA by 28.6% based on 90% PM2.5 removal. As a filter that increases the PM adhesion effect while lowering the energy consumption and the pressure drop of the filter, PVDF/Au is expected to be effectively utilized in the existing filtration type filter system.

Abstract This study introduces localized surface plasmon resonance (L-SPR) mediated heating filter membrane (HFM) for inactivating universal viral particles by using the photothermal effect of plasmonic metal nanoparticles (NPs). Plasmonic metal NPs were coated onto filter membrane via a conventional spray-coating method. The surface temperature of the HFM could be controlled to approximately 40–60 °C at room temperature, owing to the photothermal effect of the gold (Au) NPs coated on them, under irradiation by visible light-emitting diodes. The management and disposal of excess sludge are emerging issues owing to the high costs associated with treatment. In this study, the viability of a modified bentonite was investigated as a conditioning agent for the stabilisation of heavy metals (i.e., Cu, Zn, Cr, Pb, and Cd) and the retention of nutrient species (i.e., total nitrogen (TN), total phosphorus (TP), available nitrogen (available N), and Olsen-phosphorus (Olsen-P)) in sewage sludge for agricultural use. Five grams of modified bentonite resulted in the highest stabilisation rate of heavy metals and strongly contributed to the stabilisation of heavy metals. However, increased amounts of modified bentonite might increase the TN, available N, and TP losses in the conditioned sewage sludge. Through the analytic hierarchy process modelling, optimal concentrations of nutrient species and heavy metals remaining in the conditioned sewage sludge were achieved when the ratio of bentonite to sewage sludge was 1:12.5 (4 g bentonite : 50 g sludge). Moreover, the optimal mixing ratio of the conditioned sewage sludge to the soil (1:2) was suggested for agricultural use. Based on these observations, modified bentonite allowed the sewage sludge to be used as a fertiliser in agriculture by stabilising heavy metals and retaining nutrient species. Ion exchange membranes, especially cation exchange membranes (CEMs), are an important component in membrane-based energy generation and storage because of their ability to transport cations via the electrochemical potential gradient while preventing electron transport. However, developing a CEM with low areal resistance, high permselectivity, and stability remains difficult. In this study, electrospun graphene oxide/polyethersulfone (GO/PES) composite nanofibers were prepared with varying concentrations of GO. To fabricate a CEM, the pores of the electrospun GO/PES nanofiber substrates were filled with a Nafion ionomer. The pore-filled PES nanofiber loaded with 1% GO revealed a noticeable improvement in hydrophilicity, structural morphology, and mechanical properties. The 1% GO/PES pore-filled CEM was compared to a Nafion membrane of a varying thickness and without a nanofiber substrate. The CEM with a nanofiber substrate showed permselectivity of 85.75%, toughness of 111 J/m3, and areal resistance of 3.7 Ω cm2, which were 12.8%, 4.3 times, and 4.0 times better, respectively, than those of the Nafion membrane at the same thickness. The development of a reinforced concrete-like GO/PES nanofiber structure containing stretchable ionomer-enhanced membrane surfaces exhibited suitable areal resistance and reduced the thickness of the composite membrane without compromising the mechanical strength, suggesting its potential application as a cation exchange membrane in electrochemical membrane-based systems.

The reuse of wastewater has been identified as an important initiative for the sustainable development of the environment; thus, the removal of secondary effluent organic matter (EfOM) to ensure the safety of reused wastewater is the key step and a subject of extensive research. In this study, Al2(SO4)3 and anionic polyacrylamide were selected as coagulant and flocculant, respectively, for the treatment of secondary effluent from a food-processing industry wastewater treatment plant to meet the standard regulatory specifications for water reuse. In this process, the removal efficiencies of chemical oxygen demand (COD), components with UV254, and specific ultraviolet absorbance (SUVA) were 44.61%, 25.13%, and 9.13%, respectively, with an associated reduction in chroma and turbidity. The fluorescence intensities (Fmax) of two humic-like components were reduced during coagulation, and microbial humic-like components of EfOM had a better removal efficiency because of a higher Log Km value of 4.12. Fourier transform infrared spectroscopy showed that Al2(SO4)3 could remove the protein fraction of the soluble microbial products (SMP) of EfOM by forming a loose SMP protein complex with enhanced hydrophobicity. Furthermore, flocculation reduced the aromaticity of secondary effluent. The cost of the proposed secondary effluent treatment was 0.034 CNY t−1 %COD−1. These results demonstrate that the process is efficient and economically viable for EfOM removal to realize food-processing wastewater reuse.

Parkinson’s disease (PD) is a neurodegenerative disorder that causes gait abnormalities. Early and accurate recognition of PD gait is crucial for effective treatment. Recently, deep learning techniques have shown promising results in PD gait analysis. However, most existing methods focus on severity estimation and frozen gait detection, while the recognition of Parkinsonian gait and normal gait from the forward video has not been reported. In this paper, we propose a novel spatiotemporal modeling method for PD gait recognition, named WM–STGCN, which utilizes a Weighted adjacency matrix with virtual connection and Multi-scale temporal convolution in a Spatiotemporal Graph Convolution Network. The weighted matrix enables different intensities to be assigned to different spatial features, including virtual connections, while the multi-scale temporal convolution helps to effectively capture the temporal features at different scales. Moreover, we employ various approaches to augment skeleton data. Experimental results show that our proposed method achieved the best accuracy of 87.1% and an F1 score of 92.85%, outperforming Long short-term memory (LSTM), K-nearest neighbors (KNN), Decision tree, AdaBoost, and ST–GCN models. Our proposed WM–STGCN provides an effective spatiotemporal modeling method for PD gait recognition that outperforms existing methods. It has the potential for clinical application in PD diagnosis and treatment.

Conclusions: Both DAT and AAD responsiveness influenced AFCA rhythm outcomes. Although self-adhesive resin cements are convenient and less technique-sensitive materials for dental clinicians, they exhibit a lower degree of conversion due to acidic components in their composition. Supplementation of the initiator, accelerator, and activator in self-adhesive resin cements has been suggested to compensate for the lower degree of conversion. This study aimed to evaluate the effects of different combinations of self-curing initiators, self-curing activators, and accelerators on the degree of conversion (DC) of self-adhesive resin cements. A dual-cured self-adhesive resin was prepared using six combinations of initiators, activators, and accelerators. The change in the DC over time was evaluated with and without light curing. The film thickness, flow properties, and cytotoxicity of each formulation were assessed. The results showed that all supplemental components had an effect on increasing the DC, but a greater increase in the DC was observed in the following order: activator, accelerator, and initiator. The cytotoxicity of the resin cements was related to the DC values, as resin cements with lower DC values exhibited higher cytotoxicity. The film thickness met the ISO standards for all groups. The results suggest that utilizing an activator is the most effective approach to enhance the DC in self-adhesive resin cement and that cytotoxicity tended to increase with lower DC values, whereas film thickness and flow properties demonstrated no correlation with DC values. Among the graft options for anterior cruciate ligament reconstruction (ACLR), hamstring autografts are widely regarded as the preferred choice for primary ACLR among orthopedic surgeons worldwide. However, concerns persist regarding postoperative knee flexor weakness. We aimed to compare knee extensor and flexor strengths between hamstring autograft and tibialis anterior allograft groups in ACLR patients, who were propensity score-matched based on baseline characteristics. A retrospective analysis included 58 matched pairs who underwent isokinetic strength tests at 6 and 12 months post operation. Isokinetic muscle strength tests found no significant difference in knee extensor and flexor strength at 6 months post operation between the hamstring autograft and tibial anterior allograft groups. At 12 months, the hamstring autograft group exhibited significantly greater knee flexor deficit (total work and average power) compared to the allograft group, despite no differences in extensor strength or patient-reported outcomes. Tsutsugamushi disease was first described in China by Hong Ge in 313. In his book Zhouhou Beiji Fang, three escharassociated febrile diseases were described: Shashidu, Zhongxidu, and Shegongdu. Shashidu was identified as being identical to tsutsugamushi disease in Japan: it occurred in riverside areas, exhibited an eschar, and was transmitted by tiny red “sand lice”. The nature of Zhongxidu remains unknown, but we propose that it is another type of Orientia tsutsugamushi infection: it occurred in mountainous areas, an eschar was observed, and the causative vector was not identified. Moreover, Zhongxidu would have predated Shashidu by five centuries; thus, the first documentation of tsutsugamushi disease would date back 2.2 millennia. O. tsutsugamushi infection without eschar has not been identified in ancient Chinese literature and may be included in Shanghan. Several ancient Chinese books describe that Shegongdu occurs following a Shegong bite. Shegong is described as a bug resembling a cockroach or cicada with a crossbow-like structure, possibly the hypostome and unfolded palps of tick, in its mouth. Thus, Shegong refers to an engorged tick and Shegongdu is a tick-borne rickettsiosis. Abstract In cloud computing, there is a trade-off between SLAV (Service Level Agreement Violation) and system operating cost. Violation rates can be decreased when using more hosts, which increases system operating costs. Therefore, to manage the resources of those hosts efficiently, finding an optimal balancing point between SLAV and system operating cost is critical. In addition, a cost-effective load balancing approach based on the proper migration of VMs (Virtual Machines) in the hosts is needed. For this purpose, some indicators are also necessary to identify the abnormal hosts that violate SLA. One of the primary indicators, CPU usage, is closely related to energy consumption and can be used to reduce SLAV and energy consumption effectively. Our approach focuses on the special environment such as the cloud environment for the scientific data. Here, most of the jobs are data-intensive and a large amount of disk operations is required. Owing to disk operations are likely to affect the performance degradation of the host, disk bandwidth usage as well as CPU usage should be also considered. In this study, we propose the Min-Max Exclusive VM Placement (MMEVMP) strategy to minimize both SLAV and energy consumption. The current working system called KIAF (KISTI Analysis Facility), the CERN ALICE experimental cloud environment for scientific data analysis, is used to analyze the characteristics of data-intensive jobs within it. In this experiment, a lightweight version of the CloudSim simulator was developed and the results were compared to the other methods of different policies. Our evaluation showed that our proposed strategy can reduce SLA violation reasonably as well as the system operating cost-effectively.

Abstract Background The spleen contains immune cells and exhibits a pattern of infarction different from other organs; as such, splenic infarction (SI) may provide important clues to infection. However, the nature of the relationship between SI and infectious disease(s) is not well understood. Accordingly, this retrospective study investigated the relationship between SI and infection. Methods Hospital records of patients with SI, who visited Inha University Hospital (Incheon, Republic of Korea) between January 2008 and December 2018, were reviewed. Patient data regarding clinical presentation, causative pathogens, risk factors, and radiological findings were collected and analyzed. Results Of 353 patients with SI, 101 with infectious conditions were enrolled in this study, and their data were analyzed to identify associations between SI and infection. Ten patients were diagnosed with infective endocarditis (IE), and 26 exhibited bacteremia without IE. Twenty-seven patients experienced systemic infection due to miscellaneous causes (negative result on conventional automated blood culture), including the following intracellular organisms: parasites (malaria [n = 12], babesiosis [n = 1]); bacteria (scrub typhus [n = 5]); viruses (Epstein–Barr [n = 1], cytomegalovirus [n = 1]); and unidentified pathogen[s] (n = 7). Splenomegaly was more common among patients with miscellaneous systemic infection; infarction involving other organs was rare. Thirty-eight patients had localized infections (e.g., respiratory, intra-abdominal, or skin and soft tissue infection), and most (35 of 38) had other risk factors for SI. Conclusions In this study, various infectious conditions were found to be associated with SI, and intracellular organisms were the most common causative pathogens. Further studies are needed to examine other possible etiologies and the underlying pathophysiological mechanisms.

Abstract. Epstein–Barr virus (EBV) is frequently reactivated by coronavirus 2019 (COVID-19), and a high incidence of EBV viremia has been reported in patients with severe COVID-19. However, the impact of EBV viremia on progression to severe COVID-19 is unclear. Therefore, we conducted a study to evaluate the effect of EBV on COVID-19 progression. We investigated EBV viremia at the time of admission in COVID-19 patients hospitalized between February 1, 2020, and April 11, 2021. A cross-sectional study was performed to compare the severity of COVID-19 according to the presence or absence of EBV viremia. However, since it is difficult to analyze the influence of EBV viremia on COVID-19 progression with cross-sectional studies, a retrospective cohort study, limited to patients with mild COVID-19, was additionally conducted to observe progression to severe COVID-19 according to the presence or absence of EBV viremia. Two hundred sixty-nine COVID-19 patients were tested for EBV viremia. In a cross-sectional study that included patients with both mild and severe COVID-19, the EBV viremia group had more severe pneumonia than the EBV-negative group. However, in the cohort study limited to mild cases (N = 213), EBV viremia was not associated with COVID-19 progression. COVID-19 severity may affect EBV viremia; however, there was no evidence that EBV viremia was a factor in exacerbating pneumonia in patients with mild COVID-19.
