Abstract

The Parton Branching (PB) method describes the evolution of transverse momentum dependent (TMD) parton distributions, covering all kinematic regions from small to large transverse momenta kT. The small kT-region is very sensitive both to the contribution of the intrinsic motion of partons (intrinsic kT) and to the resummation of soft gluons taken into account by the PB TMD evolution equations. We study the role of soft-gluon emissions in TMD as well as integrated parton distributions. We perform a detailed investigation of the PB TMD methodology at next-to-leading order (NLO) in Drell-Yan (DY) production for low transverse momenta. We present the extraction of the nonperturbative "intrinsic-kT" distribution from recent measurements of DY transverse momentum distributions at the LHC across a wide range in DY masses, including a detailed treatment of statistical, correlated and uncorrelated uncertainties. Abstract

The role of soft (non-perturbative) gluons in collinear parton densities and parton shower event generators is investigated with the Parton Branching method as a solution of the DGLAP evolution equations. It is found that soft gluons play a significant role. Within the Parton Branching frame, the Sudakov form factor can be split into a perturbative and non-perturbative part. The non-perturbative part can be calculated analytically under certain conditions. It is shown that the inclusion of soft (non-perturbative) gluons in the parton density evolution is essential for the proper cancellation of divergent terms. It is argued that the non-perturbative part of the Sudakov form factor has its correspondence in Transverse Momentum Dependent parton distributions. Within the Parton Branching approach, this non-perturbative Sudakov form factor is constrained by fits of inclusive, collinear parton densities. We show that the non-perturbative Sudakov form factor and soft gluon emissions are essential for inclusive distributions (collinear parton densities and Drell-Yan transverse momentum spectra). We also show by using Parton Branching TMD parton shower, that the effect of soft gluons plays essentially no role in final state hadron spectra and jets., Comment: Text significantly updated

Abstract

Azimuthal correlations in Z+jet production at large transverse momenta are computed by matching Parton - Branching (PB) TMD parton distributions and showers with NLO calculations via MCatNLO. The predictions are compared with those for dijet production in the same kinematic range. The azimuthal correlations $\Delta\phi$ between the Z boson and the leading jet are steeper compared to those in dijet production at transverse momenta ${\cal O}(100)$ GeV, while they become similar for very high transverse momenta ${\cal O}(1000)$ GeV. The different patterns of Z+jet and dijet azimuthal correlations can be used to search for potential {\it factorization - breaking} effects in the back-to-back region, which depend on the different color and spin structure of the final states and their interferences with the initial states. In order to investigate these effects experimentally, we propose to measure the ratio of the distributions in $\Delta\phi$ for Z+jet - and multijet production at low and at high transverse momenta, and compare the results to predictions obtained assuming factorization. We examine the role of theoretical uncertainties by performing variations of the factorization scale, renormalization scale and matching scale. Abstract

The azimuthal correlation, $\Delta \phi_{12}$, of high transverse momentum jets in pp collisions at $\sqrt{s}=13$ TeV is studied by applying PB-TMD distributions to NLO calculations via MCatNLO together with the PB-TMD parton shower. A very good description of the cross section as a function of $\Delta \phi_{12}$ is observed. In the back-to-back region of $\Delta \phi_{12} \to \pi$, a very good agreement is observed with the PB-TMD Set 2 distributions while significant deviations are obtained with the PB-TMD Set 1 distributions. Set 1 uses the evolution scale while Set 2 uses transverse momentum as an argument in $\alpha_s$, and the above observation therefore confirms the importance of an appropriate soft-gluon coupling in angular ordered parton evolution. The total uncertainties of the predictions are dominated by the scale uncertainties of the matrix element, while the uncertainties coming from the PB-TMDs and the corresponding PB-TMD shower are very small. The $\Delta \phi_{12}$ measurements are also compared with predictions using MCatNLO together PYTHIA8, illustrating the importance of details of the parton shower evolution., Comment: 15 pages, 8 Figures

Abstract

The CASCADE3 Monte Carlo event generator based on Transverse Momentum Dependent (TMD) parton densities is described. Hard processes which are generated in collinear factorization with LO multileg or NLO parton level generators are extended by adding transverse momenta to the initial partons according to TMD densities and applying dedicated TMD parton showers and hadronization. Processes with off-shell kinematics within $k_t$-factorization, either internally implemented or from external packages via LHE files, can be processed for parton showering and hadronization. Abstract

It has been observed in the literature that measurements of low-mass Drell-Yan (DY) transverse momentum spectra at low center-of-mass energies $\sqrt{s}$ are not well described by perturbative QCD calculations in collinear factorization in the region where transverse momenta are comparable with the DY mass. We examine this issue from the standpoint of the Parton Branching (PB) method, combining next-to-leading-order (NLO) calculations of the hard process with the evolution of transverse momentum dependent (TMD) parton distributions. We compare our predictions with experimental measurements at low DY mass, and find very good agreement.In addition we use the low mass DY measurements at low $\sqrt{s}$ to determine the width $q_s$ of the intrinsic Gauss distribution of the PB-TMDs at low evolution scales. We find values close to what has earlier been used in applications of PB -TMDs to high-energy processes at the Large Hadron Collider (LHC) and HERA. We find that at low DY mass and low $\sqrt{s}$ even in the region of $p_t/m_{DY} \sim 1$ the contribution of multiple soft gluon emissions (included in the PB-TMDs) is essential to describe themeasurements, while at larger masses ($m_{DY} \sim m_{Z}$) and LHC energies the contribution from soft gluons in the region of $p_t/m_{DY}\sim 1$ is small., Comment: v2: references, clarifications and discussion added; results unchanged Version to appear in EPJC

By analyzing research conducted within the European Union during the 2020-2021 period, the post-crisis situation can be understood. The findings indicate an increase in digital activity among this population. Abstract

Background and Aims: A few case reports of autoimmune hepatitis–like liver injury have been reported after severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) vaccination. We evaluated clinical features, treatment response and outcomes of liver injury following SARS-CoV-2 vaccination in a large case series. Approach and Results: We collected data from cases in 18 countries. The type of liver injury was assessed with the R-value. The study population was categorized according to features of immune-mediated hepatitis (positive autoantibodies and elevated immunoglobulin G levels) and corticosteroid therapy for the liver injury. We identified 87 patients (63%, female), median age 48 (range: 18–79) years at presentation. Liver injury was diagnosed a median 15 (range: 3–65) days after vaccination. Fifty-one cases (59%) were attributed to the Pfizer-BioNTech (BNT162b2) vaccine, 20 (23%) cases to the Oxford-AstraZeneca (ChAdOX1 nCoV-19) vaccine and 16 (18%) cases to the Moderna (mRNA-1273) vaccine. The liver injury was predominantly hepatocellular (84%) and 57% of patients showed features of immune-mediated hepatitis. Corticosteroids were given to 46 (53%) patients, more often for grade 3–4 liver injury than for grade 1–2 liver injury (88.9% vs. 43.5%, p = 0.001) and more often for patients with than without immune-mediated hepatitis (71.1% vs. 38.2%, p = 0.003). All patients showed resolution of liver injury except for one man (1.1%) who developed liver failure and underwent liver transplantation. Steroid therapy was withdrawn during the observation period in 12 (26%) patients after complete biochemical resolution. None had a relapse during follow-up. Conclusions: SARS-CoV-2 vaccination can be associated with liver injury. Corticosteroid therapy may be beneficial in those with immune-mediated features or severe hepatitis. Outcome was generally favorable, but vaccine-associated liver injury led to fulminant liver failure in one patient.

Abstract

Background: Data regarding outcome of Coronavirus disease 2019 (COVID-19) in vaccinated patients with autoimmune hepatitis (AIH) are lacking. We evaluated the outcome of COVID-19 in AIH patients who received at least one dose of Pfizer- BioNTech (BNT162b2), Moderna (mRNA-1273) or AstraZeneca (ChAdOx1-S) vaccine. Patients and methods: We performed a retrospective study on AIH patients with COVID-19. The outcomes of AIH patients who had acute respiratory syndrome coronavirus 2 (SARS-CoV-2) breakthrough infection after at least one dose of COVID-19 vaccine were compared to unvaccinated patients with AIH. COVID-19 outcome was classified according to clinical state during the disease course as: (i) no hospitalization, (ii) hospitalization without oxygen supplementation, (iii) hospitalization with oxygen supplementation by nasal cannula or mask, (iv) intensive care unit (ICU) admission with non-invasive mechanical ventilation, (v) ICU admission with invasive mechanical ventilation or (vi) death, and data was analyzed using ordinal logistic regression. Results: We included 413 (258 unvaccinated and 155 vaccinated) patients (81%, female) with a median age of 52 (range: 17–85) years at COVID-19 diagnosis. The rates of hospitalization were (36.4% vs. 14.2%), need for any supplemental oxygen (29.5% vs. 9%) and mortality (7% vs. 0.6%) in unvaccinated and vaccinated AIH patients with COVID-19. Having received at least one dose of SARS-CoV-2 vaccine was associated with a significantly lower risk of worse COVID-19 severity, after adjusting for age, sex, comorbidities and presence of cirrhosis (adjusted odds ratio [aOR] 0.18, 95% confidence interval [CI], 0.10–0.31). Overall, vaccination against SARS-CoV-2 was associated with a significantly lower risk of mortality from COVID-19 (aOR 0.20, 95% CI 0.11–0.35). Conclusions: SARS-CoV-2 vaccination significantly reduced the risk of COVID-19 severity and mortality in patients with AIH.

Abstract

La profunda imbricación de las pantallas y de las redes sociales en la familia afecta a la labor educativa que se realiza en su seno. Este artículo explora los riesgos antropológicos que puede conllevar para la relación materno-paterno-filial la realización acrítica de distintas modas online. Para ello, se realiza un estudio de casos sobre once retos de la red social TikTok, que involucran a padres e hijos a través de una investigación de corte cualitativo retrospectivo, que combina la etnografía virtual con el análisis de contenido temático. Los distintos desafíos fueron codificados en cinco categorías. Los hallazgos se analizan desde un enfoque estadístico y, a continuación, se realiza un análisis interpretativo. Los resultados ponen de manifiesto la facilidad con la que los dispositivos digitales pueden fomentar la pérdida de la diferencia simbólica entre generaciones afectando profundamente a elementos educativos propios de la institución familiar como la autoridad moral, los límites o la acogida incondicional; elementos que en conclusión resultan imprescindibles para que los hijos hallen en el seno familiar un lugar seguro desde el que comenzar a construir su identidad personal.

Abstract

Background and Aims: Data regarding outcome of COVID-19 in patients with autoimmune hepatitis (AIH) are lacking. Approach and Results: We performed a retrospective study on patients with AIH and COVID-19 from 34 centers in Europe and the Americas. We analyzed factors associated with severe COVID-19 outcomes, defined as the need for mechanical ventilation, intensive care admission, and/or death. The outcomes of patients with AIH were compared to a propensity score–matched cohort of patients without AIH but with chronic liver diseases (CLD) and COVID-19. The frequency and clinical significance of new-onset liver injury (alanine aminotransferase > 2 × the upper limit of normal) during COVID-19 was also evaluated. We included 110 patients with AIH (80% female) with a median age of 49 (range, 18-85) years at COVID-19 diagnosis. New-onset liver injury was observed in 37.1% (33/89) of the patients. Use of antivirals was associated with liver injury (P = 0.041; OR, 3.36; 95% CI, 1.05-10.78), while continued immunosuppression during COVID-19 was associated with a lower rate of liver injury (P = 0.009; OR, 0.26; 95% CI, 0.09-0.71). The rates of severe COVID-19 (15.5% versus 20.2%, P = 0.231) and all-cause mortality (10% versus 11.5%, P = 0.852) were not different between AIH and non-AIH CLD. Cirrhosis was an independent predictor of severe COVID-19 in patients with AIH (P < 0.001; OR, 17.46; 95% CI, 4.22-72.13). Continuation of immunosuppression or presence of liver injury during COVID-19 was not associated with severe COVID-19. Conclusions: This international, multicenter study reveals that patients with AIH were not at risk for worse outcomes with COVID-19 than other causes of CLD. Cirrhosis was the strongest predictor for severe COVID-19 in patients with AIH. At the onset of COVID-19, 234 patients (92.1%) were on treatment with glucocorticoids (n = 156), thiopurines (n = 151), mycophenolate mofetil (n = 22) or tacrolimus (n = 16), alone or in combinations. Overall, 94 (37%) patients were hospitalized and 18 (7.1%) patients died. Use of systemic glucocorticoids (adjusted odds ratio [aOR] 4.73, 95% CI 1.12-25.89) and thiopurines (aOR 4.78, 95% CI 1.33-23.50) for AIH was associated with worse COVID-19 severity, after adjusting for age-sex, comorbidities and presence of cirrhosis. Baseline treatment with mycophenolate mofetil (aOR 3.56, 95% CI 0.76-20.56) and tacrolimus (aOR 4.09, 95% CI 0.69-27.00) were also associated with more severe COVID-19 courses in a smaller subset of treated patients. CONCLUSION: Baseline treatment with systemic glucocorticoids or thiopurines prior to the onset of COVID-19 was significantly associated with COVID-19 severity in patients with AIH.

Abstract

The surprising announcement of an unknown pandemic caused by a coronavirus in the early 2020s confronted the world's education systems with a major challenge. Solutions, mainly technology-based, were quickly sought to continue formal education, but much non-formal education was stalled because its ultimate purpose was not official certification. This was the case for many University Programmes for Older Adults (UWP), which despite being well organised, systematic and presenting educational objectives, were severely affected by limitations. The aim of this article is to highlight the relevance of virtual training in this type of context through the specific example of the University of Burgos and to highlight the much-needed role of the University in the development of lifelong learning for all citizens regardless of their age or place of residence. Abstract

open access article This paper proposes pathways and triggers of change for city representatives and decision makers to consider for enabling transition to sustainable, resilient and adaptive cities. It investigates a range of triggers of change, including regulatory, structural/operational, behavioural, awareness, and resources. A conceptual framework for identifying the triggers of change is presented that was developed under a participatory process and tested during stakeholder dialogues with representatives from 15 cities from 12 European countries. The framework comprises of the following three steps: 1. Indicator-based vulnerability assessment, conducted to analyse city vulnerability and problem identification; 2. Constructed visions of the underpinning factors; and 3. Backcasting exercise, to detect the triggers of change. Following a prioritisation exercise across our European sample, regional differences and the prominence of the following patterns in supporting triggers of change have been noted. In Mediterranean region main triggers were public decision and political leadership, regulatory framework (including building codes, accountability, pricing, taxation, penalties and incentives) and learning from disasters triggers of change. Whereas in the Southern-Central region: adaptive multi-level governance, horizontal and vertical improved relationships governance were the main triggers of change. These patterns and framework are applicable to other cities, and indeed to other topics (e.g. mitigation, sustainability, etc.) that support implementation on the ground to achieve truly sustainable, resilient and adaptive cities. We acknowledge the challenges in deriving universally applicable triggers of change, however the study identifies eight overarching triggers of change that can facilitate the transformation of cities.

Actualmente, el estatus de las cuatro especies es diferente, por ello se recomienda la aplicación de distintos criterios de gestión para cada una.

Abstract

During the DESY summer student program 2021, young scientists from more than 13 different countries worked together, connecting from remote, to provide computer codes within the Rivet framework for 19 HERA measurements. Most of these measurements were originally available within the HZTool package, but no longer accessible for modern analysis packages such as Rivet. The temporary RivetHZTool interface was used to validate most of the new Rivet plugins.

Abstract

Background: data regarding outcome of Coronavirus disease 2019 (COVID-19) in patients with autoimmune hepatitis (AIH) are lacking. Patients and methods: we performed a retrospective study on AIH patients with COVID-19 from 34 centres in Europe and the Americas. We analyzed factors associated with severe COVID-19 outcomes defined as the need for mechanical ventilation, intensive care admission, and/or death. The outcomes of patients with AIH were compared to a propensity-score matched cohort of non-AIH patients with chronic liver diseases (CLD) and COVID-19. The frequency and clinical significance of new-onset liver injury (alanine aminotransferase>2xupper limit of normal) during COVID-19 was also evaluated. Results: we included 110 AIH patients (80%,female) with a median age of 49 (range:18-85) years at COVID-19 diagnosis. New-onset liver injury was observed in 37.1% (33/89) of the patients. Use of antivirals was associated with liver injury (p=0.041; odds ratio (OR) 3.36[1.05-10.78]) while continued immunosuppression during COVID-19 was associated with a lower rate of liver injury (p=0.009; OR 0.26[0.09-0.71]). The rates of severe COVID-19 (15.5% vs 20.2% p=0.231) and all-cause mortality (10% vs 11.5%; p=0.852) were not different between AIH and non-AIH CLD. Cirrhosis was an independent predictor of severe COVID-19 in patients with AIH (p<0.001; OR 17.46[4.22-72.13]). Continuation of immunosuppression or presence of liver injury during COVID-19 was not associated with severe COVID-19. Conclusions: this international, multi-center study reveals that patients with AIH were not at risk for worse outcomes with COVID-19 than other causes of CLD. Cirrhosis was the strongest predictor for severe COVID-19 in AIH patients. Maintenance of immunosuppression during COVID-19 was not associated with increased risk for severe COVID-19, but did lower the risk for new-onset liver injury during COVID-19.

Background: The quantitative hepatitis B surface antigen (qHBsAg) threshold of 1,000 IU/ml has been proposed to distinguish HBeAg-negative chronic infections from HBeAg-negative chronic hepatitis, to assess risk of liver disease progression, and to predict HBsAg clearance. There is evidence that qHBsAg vary significantly among genotypes, however, there is scarce data on genotype F, the most prevalent in Latin America. Aims: To analyze the impact of HBV genotype F on qHBsAg inpatients with HBeAg-negative chronic infection and to describe clinical and virological outcomes. Methods: HBV-DNA and qHBsAg serum levels of 141 patients with HBeAg-negative chronic infection were correlated with HBV genotype, who were followed for 10.6±7.4 years. Results: The overall genotype distribution was as follows: F 46.8%, D 26.1%, A 25.2%, and B 0.7% and C 0.7%. While no impact of the HBV genotype on HBV DNA levels was observed, qHBsAg differed significantly among genotypes (p3.0 Log10IU/ml, no cases of advanced liver disease or hepatocellular carcinoma were observed at the end of follow-up. Conclusions: This study provides new insights into the impact of HBV genotypes on serum HBsAg levels, emphasizing the need to implement genotype-specific cut-off to achieve diagnostic certainty in the identification of HBeAg-negative chronic infection and the risk of liver disease progression, particularly on infections with genotypes A and F. Moreover, HBsAg serum levels can became a reliable biomarker to predict HBsAg clearance.

Abstract

The scientific community agrees that climate change is one of the greatest challenges that our society will have to face this century. Indeed, the fourth report from the Intergovernmental Panel on Climate Change (IPPC) confirms the certitude of this phenomenon and its impacts, which can range from droughts to floods, health problems and food shortages. The principal objective of this paper is to assess the impact of climate change on flood events and its consequences on human. This point is necessary to define and evaluate different adaptation options. Even taking into account the medium greenhouse emission scenario, according to the results, it is expected that extreme precipitation will increase during the twenty century, although there is an uncertainty in the percentage due to the climatic models. It is expected an increase in peak discharge between 14±9 and 15±8 for 3 models and for the period 2001-2040 with a return period exceeding 40 years. For the period 2041-2080 there is more uncertainty, it is expected an increase between 12±8 and 19±10 for 2 models. According to the results, changes in flood event are expected with its associated uncertainty and new flood zones are detected with greater hazard to people. [ABSTRACT FROM AUTHOR]

Abstract

Background: data regarding outcome of Coronavirus disease 2019 (COVID-19) in patients with autoimmune hepatitis (AIH) are lacking. Patients and methods: we performed a retrospective study on AIH patients with COVID-19 from 34 centres in Europe and the Americas. We analyzed factors associated with severe COVID-19 outcomes defined as the need for mechanical ventilation, intensive care admission, and/or death. The outcomes of patients with AIH were compared to a propensity-score matched cohort of non-AIH patients with chronic liver diseases (CLD) and COVID-19. The frequency and clinical significance of new-onset liver injury (alanine aminotransferase>2xupper limit of normal) during COVID-19 was also evaluated. Results: we included 110 AIH patients (80%,female) with a median age of 49 (range:18-85) years at COVID-19 diagnosis. New-onset liver injury was observed in 37.1% (33/89) of the patients. Use of antivirals was associated with liver injury (p=0.041; odds ratio (OR) 3.36[1.05-10.78]) while continued immunosuppression during COVID-19 was associated with a lower rate of liver injury (p=0.009; OR 0.26[0.09-0.71]). The rates of severe COVID-19 (15.5% vs 20.2% p=0.231) and all-cause mortality (10% vs 11.5%; p=0.852) were not different between AIH and non-AIH CLD. Abstract

© 2022 American Association for the Study of Liver Diseases.Background and Aims: A few case reports of autoimmune hepatitis–like liver injury have been reported after severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) vaccination. We evaluated clinical features, treatment response and outcomes of liver injury following SARS-CoV-2 vaccination in a large case series. Approach and Results: We collected data from cases in 18 countries. The type of liver injury was assessed with the R-value. The study population was categorized according to features of immune-mediated hepatitis (positive autoantibodies and elevated immunoglobulin G levels) and corticosteroid therapy for the liver injury. We identified 87 patients (63%, female), median age 48 (range: 18–79) years at presentation. Liver injury was diagnosed a median 15 (range: 3–65) days after vaccination. Fifty-one cases (59%) were attributed to the Pfizer-BioNTech (BNT162b2) vaccine, 20 (23%) cases to the Oxford-AstraZeneca (ChAdOX1 nCoV-19) vaccine and 16 (18%) cases to the Moderna (mRNA-1273) vaccine. The liver injury was predominantly hepatocellular (84%) and 57% of patients showed features of immune-mediated hepatitis. Corticosteroids were given to 46 (53%) patients, more often for grade 3–4 liver injury than for grade 1–2 liver injury (88.9% vs. 43.5%, p = 0.001) and more often for patients with than without immune-mediated hepatitis (71.1% vs. 38.2%, p = 0.003). All patients showed resolution of liver injury except for one man (1.1%) who developed liver failure and underwent liver transplantation. Steroid therapy was withdrawn during the observation period in 12 (26%) patients after complete biochemical resolution. None had a relapse during follow-up. Conclusions: SARS-CoV-2 vaccination can be associated with liver injury. Corticosteroid therapy may be beneficial in those with immune-mediated features or severe hepatitis. Outcome was generally favorable, but vaccine-associated liver injury led to fulminant liver failure in one patient.

Abstract

Background We investigated associations between baseline use of immunosuppressive drugs and severity of Coronavirus Disease 2019 (COVID-19) in autoimmune hepatitis (AIH). Patients and methods Data of AIH patients with laboratory confirmed COVID-19 were retrospectively collected from 15 countries. The outcomes of AIH patients who were on immunosuppression at the time of COVID-19 were compared to patients who were not on AIH medication. The clinical courses of COVID-19 were classified as (i)-no hospitalization, (ii)-hospitalization without oxygen supplementation, (iii)-hospitalization with oxygen supplementation by nasal cannula or mask, (iv)-intensive care unit (ICU) admission with non-invasive mechanical ventilation, (v)-ICU admission with invasive mechanical ventilation or (vi)-death and analysed using ordinal logistic regression. Results We included 254 AIH patients (79.5%, female) with a median age of 50 (range, 17-85) years. At the onset of COVID-19, 234 patients (92.1%) were on treatment with glucocorticoids (n = 156), thiopurines (n = 151), mycophenolate mofetil (n = 22) or tacrolimus (n = 16), alone or in combinations. Overall, 94 (37%) patients were hospitalized and 18 (7.1%) patients died. Use of systemic glucocorticoids (adjusted odds ratio [aOR] 4.73, 95% CI 1.12-25.89) and thiopurines (aOR 4.78, 95% CI 1.33-23.50) for AIH was associated with worse COVID-19 severity, after adjusting for age-sex, comorbidities and presence of cirrhosis. Baseline treatment with mycophenolate mofetil (aOR 3.56, 95% CI 0.76-20.56) and tacrolimus (aOR 4.09, 95% CI 0.69-27.00) were also associated with more severe COVID-19 courses in a smaller subset of treated patients. Abstract

Background & Aims: Latin America is a region of great interest for studying the clinical presentation of idiosyncratic drug-induced liver injury (DILI). A comprehensive analysis of patients enrolled into the LATINDILI Network over a decade is presented., Methods: Demographics, clinical presentation, histological findings and outcome of prospectively recruited DILI cases in the LATINDILI Network were analyzed. Suspected culprit drugs were classified according to the Anatomical Therapeutic Chemical classification. Causality was assessed using the Roussel Uclaf Causality Assessment Method (RUCAM) scale., Results: Overall, 468 idiosyncratic DILI cases were analyzed (62% women; mean age, 49 years). Hepatocellular injury predominated (62%); jaundice was present in 60% of patients, and 42% were hospitalized. Of the cases, 4.1% had a fatal outcome, and 24 patients (12%) developed chronic DILI. The most common drug classes were systemic anti-infectives (31%), musculoskeletal agents (12%), antineoplastic and immunomodulating agents (11%), and herbal and dietary supplements (9%). Notably, none of the patients with DILI due to antibacterials or immunosuppressants had a fatal outcome. In fact, Hy's law showed to have drug-specific predictive value, with anti-tuberculosis drugs, nimesulide, and herbal and dietary supplements associated with the worst outcome, whereas DILI caused by amoxicillin-clavulanate, nitrofurantoin, and diclofenac, which fulfilled Hy's law, did not have a fatal outcome., Conclusion: Features of DILI in Latin America are comparable to other prospective registries. However, the pattern of drugs responsible for DILI differs. An increasing incidence of herbal and dietary supplements, with high mortality rate, and likewise, nimesulide and nitrofurantoin, was noted. 48. Conservative Management of Acute Sports-Related Concussions: A Narrative Review.

Abstract

This review explores the application of the conservative management model for pain to sports-related concussions (SRCs), framing concussions as a distinct form of pain syndrome with a pathophysiological foundation in central sensitization. Drawing parallels with proven pain management models, we underscore the significance of a proactive approach to concussion management. Recognizing concussions as a pain syndrome allows for the tailoring of interventions in alignment with conservative principles. This review first covers the epidemiology and controversies surrounding prolonged concussion recovery and persistent post-concussion symptoms (PPCS). Next, the pathophysiology of concussions is presented within the central sensitization framework, emphasizing the need for early intervention to mitigate the neuroplastic changes that lead to heightened pain sensitivity. Five components of the central sensitization process specific to concussion injuries are highlighted as targets for conservative interventions in the acute period: peripheral sensitization, cerebral metabolic dysfunction, neuroinflammation, glymphatic system dysfunction, and pain catastrophizing. These proactive interventions are emphasized as pivotal in accelerating concussion recovery and reducing the risk of prolonged symptoms and PPCS, in line with the philosophy of conservative management.

Abstract

Background & Aims: Widespread use of direct-acting antivirals for hepatitis C virus infection has been paralleled with increased numbers of patients with hepatocellular carcinoma (HCC) after achieving sustained virologic response (post-SVR HCC) worldwide. Few data compare regional differences in the presentation and prognosis of patients with post-SVR HCC., Methods: We identified patients with advanced fibrosis (F3/F4) who developed incident post-SVR HCC between March 2015 and October 2021 from 30 sites in Europe, North America, South America, the Middle East, South Asia, East Asia, and Southeast Asia. We compared patient demographics, liver dysfunction, and tumor burden by region. We compared overall survival by region using Kaplan-Meier analysis and identified factors associated with survival using multivariable Cox regression analysis., Results: Among 8796 patients with advanced fibrosis or cirrhosis who achieved SVR, 583 (6.6%) developed incident HCC. There was marked regional variation in the proportion of patients detected by surveillance (range: 59.5%-100%), median maximum tumor diameter (range, 1.8-5.0 cm), and the proportion with multinodular HCC (range, 15.4%-60.8%). The prognosis of patients highly varied by region (hazard ratio range, 1.82-9.92), with the highest survival rates in East Asia, North America, and South America, and the lowest survival rates in the Middle East and South Asia. After adjusting for geographic region, HCC surveillance was associated with early stage detection (Barcelona Clinic Liver Cancer stage 0/A, 71.0% vs 21.3%; P < .0001) and lower mortality rates (adjusted hazard ratio, 0.29; 95% CI, 0.18-0.46)., Conclusions: Clinical characteristics, including early stage detection, and prognosis of post-SVR HCC differed significantly across geographic regions. Abstract

The Baveno VII consensus workshop has provided several novel recommendations regarding the management of patients with clinically significant portal hypertension (CSPH). The expert panel summarized the existing data into simple clinical rules to aid clinicians in their clinical practice. The use of non-invasive tests (NITs), especially liver stiffness measurement (LSM), have gain an important role in daily practice. The use of LSM alone or in combination with platelet count can be used to rule-in and rule-out compensated advanced chronic liver disease (cACLD) and CSPH. Further decompensation events were defined as a prognostic stage associated with an even higher mortality than that associated with first decompensation. Moreover, the term hepatic recompensation was introduced in Baveno VII consensus implying a partial or complete regression of the functional and structural changes of cirrhosis after the removal of the underlying etiology.
