Abstract

Flight feather moult is an energetically expensive stage of the annual cycle of birds. Its timing is adjusted to other important time- and energy-demanding activities, including migration. In the vast majority of migratory birds, primary moult occurs before or after migration or moulting is suspended during migration. The Black-headed Gull is an exception: it starts moulting flight feathers at the onset of the autumn migration. The course of primary moult in this species is still poorly understood due to the difficulty in obtaining sufficient data from the entire period of flight feather replacement. In this paper, we employed digital photos taken in the field to estimate the start, variation in the start date and duration of primary moult in adult and immature Black-headed Gulls, by using the Underhill–Zucchini likelihood moult model. On average, immatures started their moult on 7 June, 25 days earlier than adults, but their moult lasted 5 days longer. Consequently, there was a 20-day difference in the end of moult – that is, on average it occurred on 16 September and 6 October in immature and adult gulls, respectively. Adults showed greater variability in the mean primary moult start date than immatures, as the beginning of flight feather replacement may depend on breeding success (earlier after breeding failure) and on breeding phenology. The overlap of primary moult and migration produces a trade-off in resource allocation between those two processes, which leads to a low number of simultaneously growing primaries and a decrease in the growth rate of the two external, heaviest flight feathers. Existing reports of a later completion of primary moult in the first half of November in this species are based on the faulty assumption that the outermost primary is always the longest, which is not the case in 34% of individuals. Participants developed and implemented Quality Improvement (QI) interventions addressing perioperative pain management.Methods: PAIN OUT, an international QI and research network, provided tools for web-based auditing and feedback of pain management and patient-reported outcomes (PROs) in the clinical routine. Ward- and patient-level factors were evaluated with multi-level models. Change in proportion of patients reporting worst pain ≥ 6/10 between project phases was the primary outcome.Results: Participants created locally adapted resources for teaching and pain management, available to providers in the form of a website and a special issue of a national anesthesia journal. They offered teaching to anesthesiologists, surgeons, including residents, and nurses. Information was offered to patients and families. A total of 2658 patients were audited in 9 hospitals, between July 2016 and December 2018. Participants reported that the project made them aware of the importance of: training in pain management; auditing one’s own patients to learn about PROs and that QI requires collaboration between multi-disciplinary teams. Participants reported being unaware that their patients experienced severe pain and lacked information about pain treatment options. Worst pain decreased significantly between the two project phases, as did PROs related to pain interfering with movement, taking a deep breath/coughing or sleep. The opportunity of patients receiving information about their pain treatment options increased from 44% to 77%.Conclusions: Patients benefited from improved care and pain-related PROs. 8. Increasing body condition of autumn migrating Eurasian blackcaps Sylvia atricapilla over four decades.

Abstract

The effects of recent climate change on the phenology of avian breeding and migration, geographical range shifts, and decreasing migration distances are well documented. Only a few studies have focused on the long-term influence of these changes on the body condition of migrants, which is important as it is likely to affect the fitness of the whole population. Eurasian blackcaps have adapted their migratory behaviour to changing ecological factors. These changes seem to favour the species and its migratory populations have increased over the last decades in Europe. In the present study, we analysed the body condition index of juvenile blackcaps at their stop-over site in the southern Baltic region during autumn migration from 1967 to 2006. This index was distinctly higher since the 1990s than in the middle of the 1960s, when the number of migrating blackcaps was at least twofold lower. Moreover, there was a reduction in recapture rates of blackcaps from 14% to 6% during these periods which may imply that warblers were staying for a shorter time at the stop-over site as their body condition was higher. We hypothesized that higher berry crop availability resulted in the higher condition of migrating blackcaps. Body condition index also reflecting fuel load in migrants was higher in long-distance blackcaps compared to the short-distance ones. As the former are time-minimizing migrants, their higher body condition/fuel load may allow them to migrate faster to wintering grounds in sub-Saharan Africa. [ABSTRACT FROM AUTHOR]

Abstract

The species-specific and age-related autumn migration strategies of the sedge, reed and great reed warbler were investigated at stopover sites between central Europe and north-eastern Africa during autumn. Adult reed warblers accumulated larger fat reserves than juveniles and consequently were able to cover longer distances from most stopover sites. Juvenile sedge warblers, on the other hand, left Europe with significantly larger fat reserves than adults as opposed to the populations migrating along the western route. Both juvenile and adult great reed warblers were potentially able to make long flights without refuelling from the northern part of the Mediterranean region onwards. It was the proximity of large ecological barriers that had the most prominent influence on the potential flight distances of adult and juvenile Acrocephalus warblers during their southward migration. In reed and sedge warblers, the differences in mean flight distances covered by adults and juveniles were the largest (ca. 600 km) just before the crossing of the Mediterranean Sea, but smaller (ca. 200 and 400 km, respectively) in front of the Sahara Desert. Juvenile and adult great reed warblers were potentially able to cover 1660 km from Turkey, which was also supported by very low numbers of individuals caught at the Egyptian ringing sites. The results of this study carried out in the key regions of the eastern European–African flyway documented that migration strategies differed between adults and juveniles of three long-distance migrant species, yet this variation depended on the migration stage and the proximity of natural ecological barriers. Reduction in postoperative pain increases patient satisfaction and enables better mobilization. Thus, pain management needs to be continuously improved. Problems are often caused not only by medical issues but also by organization and hospital structure. The present study shows how the quality of pain management can be increased by implementing a standardized pain concept and simple, consistent, benchmarking.Methods: All patients included in the study had undergone total hip arthroplasty (THA). Outcome parameters were analyzed 24 hours after surgery by means of the questionnaires from the German-wide project “Quality Improvement in Postoperative Pain Management” (QUIPS). A pain nurse interviewed patients and continuously assessed outcome quality parameters. A multidisciplinary team of anesthetists, orthopedic surgeons, and nurses implemented a regular procedure of data analysis and internal benchmarking. The health care team was informed of any results, and suggested improvements. Every staff member involved in pain management participated in educational lessons, and a special pain nurse was trained in each ward.Results: From 2014 to 2015, 367 patients were included. The mean maximal pain score 24 hours after surgery was 4.0 (±3.0) on an 11-point numeric rating scale, and patient satisfaction was 9.0 (±1.2). Abstract

Deficits in the quality of pain management in hospitals have been described for years. The aim of this study was to assess structures and processes of pain management in departments for internal medicine in German hospitals.Data were collected using a standardized telephone interview (non-university hospital departments) on a randomized sample of hospitals (circa 1/3 of hospitals); all German departments of university hospitals were invited to participate and questioned separately using an online questionnaire (SurveyMonkey®).Data from 139 non-university departments (response rate: 21%) and 33 university hospital departments (only 21 questionnaires were fully answered, response rate 17 and 11%, respectively) were collected. Of 619 non-university hospital departments contacted, 441 explicitly refused to participate in the survey, most often on the grounds that there was no interest in pain management. Pain was regularly recorded as an independent parameter during the medical visit in 89% of the non-university hospital and 96% of the university hospital departments; written standardized treatment protocols for pain therapy were available in 57% of the non-university hospital departments (54% university hospital departments). In 76% of the non-university hospitals departments (100% university hospital departments), an acute pain service was also available for patients of internal medicine departments for co-treatment. Written cooperation agreements were less common (35% non-university hospital departments, 18% university hospital departments).The described implementation of pain management was satisfying in the participating departments. However, in consideration of the low participation and high rejection of participation due to explicit disinterest in the topic, the results should be critically assessed and presumably characterized by a significant positive bias.HINTERGRUND UND ZIEL DER ARBEIT: Seit Jahren werden Defizite der Qualität der Schmerztherapie im Krankenhaus beschrieben. Ziel der vorliegenden Untersuchung war es, Strukturen und Prozesse des Schmerzmanagements in internistischen Abteilungen darzustellen.Die Datenerfassung erfolgte mittels eines standardisierten Telefoninterviews (nichtuniversitäre Abteilungen); bei universitären Abteilungen separat mittels eines Onlinefragebogens (SurveyMonkey®).Daten von 139 nichtuniversitären Abteilungen (Rücklauf: 21 %) und 33 universitären Abteilungen (davon 21 vollständig beantwortete Fragebögen, Rücklauf 17 % bzw. 11 %) wurden erhoben. 441 von 619 kontaktierten nichtuniversitären Abteilungen lehnten die Teilnahme ausdrücklich ab, am häufigsten mit der Begründung, es bestünde kein Interesse am Thema Schmerzmanagement. In den 172 teilnehmenden Einrichtungen wurde Schmerz als eigenständiger Parameter in 89 % der nichtuniversitären Abteilungen (96 % universitär) regelmäßig während der Visite erfasst; schriftliche Behandlungsstandards zur Schmerztherapie lagen in 57 % der nichtuniversitären Abteilungen vor (54 % universitär). In 76 % der nichtuniversitären Krankenhäuser (100 % universitär) stand auch für die internistischen Patienten ein Akutschmerzdienst zur Mitbehandlung zur Verfügung, schriftliche Vereinbarungen zur Zusammenarbeit lagen bei 35 % vor (18 % universitär).Die berichtete Umsetzung des Schmerzmanagements war in den teilnehmenden Abteilungen gut. Gleichwohl sind die Ergebnisse bei niedriger Teilnahme und häufiger Ablehnung mit der Begründung einer fehlenden Relevanz nur eingeschränkt aussagekräftig. Die erfassten Daten reflektieren daher vermutlich eine erhebliche positive Selektion und sind nicht als repräsentativ für das Schmerzmanagement in der inneren Medizin zu werten.

Abstract

In 2016 the German Society of Anesthesiology and Intensive Care Medicine (DGAI) and the Association of German Anesthetists (BDA) published 10 quality indicators (QI) to compare and improve the quality of anesthesia care in Germany. So far, there is no evidence for the feasibility of implementation of these QI in hospitals.This study tested the hypothesis that the implementation of the 10 QI is feasible in German hospitals.This prospective three-phase national multicenter quality improvement study was conducted in 15 German hospitals and 1 outpatient anesthesia center from March 2017 to February 2018. The trial consisted of an initial evaluation of pre-existing structures and processes by the heads of the participating anesthesia departments, followed by a 6-month implementation phase of the QI as well as a final re-evaluation phase. The implementation procedure was supported by web-based implementation aids ( www.qi-an.org ) and internal quality management programs. The primary endpoint was the difference in the number of implemented QI per center before and after implementation. Secondary endpoints were the number of newly implemented QI per center, the overall number of successful implementations of each QI, the identification of problems during the implementation as well as the kind of impediments preventing the QI implementation.The average number of implemented QI increased from 5.8 to 6.8 (mean of the differences 1.1 ± 1.3; P 0.01). Most frequently the QI perioperative morbidity and mortality report (5 centers) and the QI temperature management (4 centers) could be implemented. After the implementation phase, the QI incidence management and patient blood management were implemented in all 16 centers. Implementation of other quality indicators failed mainly due to a lack of time and lack of structural resources.In this study the implementation of QI was proven to be mostly feasible in the participating German hospitals. Although several QI could be implemented with minor effort, more time, financial and structural resources would be required for some QI, such as the QI postoperative visit.HINTERGRUND: Im Jahr 2016 hat die Deutsche Gesellschaft für Anästhesiologie und Intensivmedizin (DGAI) und der Bund Deutscher Anästhesisten (BDA) 10 Qualitätsindikatoren (QI) veröffentlicht, um die Qualität anästhesiologischer Behandlungen vergleichen und verbessern zu können. Bis heute gibt es keine Evidenz darüber, inwieweit sich die Maßnahmen zur Implementierung der QI in den Kliniken umsetzen lassen.Im Rahmen dieser Studie wurde die Hypothese getestet, inwieweit die vorliegenden 10 QI in deutschen Krankenhäusern implementierbar sind.Diese prospektive, dreiphasige, nationale, Multizenterstudie wurde in 15 deutschen Krankenhäusern und einem ambulanten Anästhesie-Zentrum von März 2017 bis Februar 2018 durchgeführt. Sekundäre Endpunkte waren die Anzahl neu implementierter QI pro Zentrum, die Gesamtzahl erfolgreicher Implementierungen pro einzelnem QI, die Identifizierung von Problemen während der Implementierung sowie die Gründe, die die Implementierung eines QI verhindert haben.Die durchschnittliche Anzahl implementierter QI pro Zentrum stieg von 5,8 auf 6,8 (Mittelwert der Differenzen 1,1 ± 1,3; p 0,01). Am häufigsten konnten die QI Perioperativer Morbiditäts- und Mortalitätsbericht (5 Zentren) und QI Temperatur-Management (4 Zentren) implementiert werden. Nach der Implementierungsphase waren die QI Fehlermanagement und Patient-Blood-Management in allen Zentren implementiert. Viele der QI konnten mit relativ geringem Aufwand implementiert werden. Abstract

Aim: Recommendations on hygiene measures, personal protective equipment (PPE), isolation, and antibiotic prophylaxis were developed during the coronavirus 2019 disease (COVID-19) pandemic and have been revised several times to date. Some of the underlying literature indicates a large evidence gap. We suspect that this leads to a large variance of measures on German intensive care units (ICU).Methods: A mixed methods online survey among intensive-care specialists in Germany caring for COVID-19 patients was conducted in December 2020.Results: We received responses from 205 German ICUs that had treated COVID-19 patients to date. There was wide variation in the use of PPE. Polymerase Chain reaction (PCR) testing for severe acute respiratory syndrome coronavirus type 2 (SARS-CoV-2) was used by 94.8% of the units, with an average waiting time of 12 hours for the result. 18.7% of the respondents prescribed antibiotic prophylaxis in COVID-19 patients.Conclusion: We found a high variance in essential care strategies for COVID-19 patients on German intensive care units. This included differences in infection prophylaxis, personal protective equipment, and the indication of prophylactic antibiotic therapy. Based on our results, we recommend further studies to quantify and improve guideline adherence. Zielsetzung: Empfehlungen zu Hygienemaßnahmen, persönlicher Schutzausrüstung (PSA), Isolierung und Antibiotikaprophylaxe wurden während der COVID-19 Pandemie entwickelt und bis heute mehrfach überarbeitet. Ein Teil der zugrundeliegenden Literatur weist eine große Evidenzlücke auf. Wir vermuten, dass dies zu einer großen Varianz der Maßnahmen auf deutschen Intensivstationen führt.Methoden: Mixed-methods-Onlineumfrage unter leitenden Intensivmedizinern in Deutschland, die COVID-19-Patienten betreuen, durchgeführt im Dezember 2020.Ergebnisse: Wir erhielten Antworten von 205 deutschen Intensivstationen, die bis zu diesem Zeitpunkt COVID-19-Patienten behandelt hatten. Only blood pressure changes upon standing are included in the second consensus criteria but other autonomic function tests (AFT) are also useful to diagnose widespread and progressive autonomic failure typical of MSA. Additional diagnostic tools are of interest to improve accuracy of MSA diagnosis. Objectives: To assess the utility of diagnostic tools beyond brain imaging and AFT in enhancing a laboratory-supported diagnosis of MSA to support the upcoming revision of the consensus criteria. Methods: The International Parkinson and Movement Disorders Society MSA Study Group (MoDiMSA) performed a systematic review of original papers on biomarkers, sleep studies, genetic, neuroendocrine, neurophysiological, neuropsychological and other tests including olfactory testing and acute levodopa challenge test published before August 2019. Results: Evaluation of history of levodopa responsiveness and olfaction is useful in patients in whom MSA-parkinsonian subtype is suspected. Neuropsychological testing is useful to exclude dementia at time of diagnosis. Applicability of sphincter EMG is limited. When MSA-cerebellar subtype is suspected, a screening for the common causes of adult-onset progressive ataxia is useful, including spinocerebellar ataxias in selected patients. Diagnosing stridor and REM sleep behavior disorder is useful in both MSA subtypes. However, none of these tools are validated in large longitudinal cohorts of postmortem confirmed MSA cases. Conclusions: Despite limited evidence, additional laboratory work-up of patients with possible MSA beyond imaging and AFT should be considered to optimize the clinical diagnostic accuracy.

Abstract

Protected area networks facilitate community changes in responses to climate warming. However, the contribution of the site environmental and conservation-oriented characteristics to these responses to climate warming are not well understood. Here, we investigate how composition of non-breeding waterbird communities within the European Union Natura 2000 (N2K) network changes in response to increases in temperature. We measured the community reshuffling of 97 waterbird species in 3,018 N2K sites over 25 years in 26 European countries. We find that N2K sites explicitly designated for protection of waterbirds and with a management plan had faster climate-driven community changes. In contrast, the designation period of the N2K sites was not associated with community adjustment, and sites funded under EU-LIFE had lower climate-driven community changes. Our findings imply that efficient conservation policy that may help waterbird community responses to climate warming is to manage sites that are specifically designated for waterbirds.

Abstract

Background: Progressive supranuclear palsy is a neurodegenerative disorder associated with tau protein aggregation. Tilavonemab (ABBV-8E12) is a monoclonal antibody that binds to the N-terminus of human tau. We assessed the safety and efficacy of tilavonemab for the treatment of progressive supranuclear palsy. Methods: We did a phase 2, multicentre, randomised, placebo-controlled, double-blind study at 66 hospitals and clinics in Australia, Canada, France, Germany, Italy, Japan, Spain, and the USA. Participants (aged ≥40 years) diagnosed with possible or probable progressive supranuclear palsy who were symptomatic for less than 5 years, had a reliable study partner, and were able to walk five steps with minimal assistance, were randomly assigned (1:1:1) by interactive response technology to tilavonemab 2000 mg, tilavonemab 4000 mg, or matching placebo administered intravenously on days 1, 15, and 29, then every 28 days through to the end of the 52-week treatment period. Randomisation was done by the randomisation specialist of the study sponsor, who did not otherwise participate in the study. The sponsor, investigators, and participants were unaware of treatment allocations. The primary endpoint was the change from baseline to week 52 in the Progressive Supranuclear Palsy Rating Scale (PSPRS) total score in the intention-to-treat population. Adverse events were monitored in participants who received at least one dose of study drug. Prespecified interim futility criteria were based on a model-based effect size of 0 or lower when 60 participants had completed the 52-week treatment period and 0·12 or lower when 120 participants had completed the 52-week treatment period. This study is registered at ClinicalTrials.gov, number NCT02985879. Findings: Between Dec 12, 2016, and Dec 31, 2018, 466 participants were screened, 378 were randomised. Abstract

Background General Practitioners (GPs) are the main providers of primary palliative care (PPC). At the same time they are the main initiators of specialised palliative homecare (SPHC). In Germany, little is known about factors which influence GPs in their involvement of SPHC. Aim of our study is to identify factors that drive GPs to give value to and involve SPHC. Methods A cross-sectional survey was performed. In 2018, questionnaires were mailed to 6000 randomly selected GPs from eight German federal states, focusing on the extent of GPs’ palliative care activities and their involvement of SPHC. Results With a response rate of 19.4% and exclusion of GPs working in SPHC-teams, n = 1026 questionnaires were appropriate for analysis. GPs valued SPHC support as the most “important/very important” for both “technical/invasive treatment measures” (95%) and availability outside practice opening hours (92%). The most relevant factor influencing perceived SPHC-importance was GPs’ self-reported extent of engagement in palliative care (β = − 0.283; CI 95% = − 0.384;−0.182), followed by the perceived quality of utilised SPHC (β = 0.119; CI 95% = 0.048;0.190), involvement in treatment of palliative patients after SPHC initiation (β = 0.088; CI 95% = 0.042;0.134), and conviction that palliative care should be a central part of GPs’ work (β = − 0.062; CI 95% = − 0.116;−0.008). Perceived SPHC-importance is also associated with SPHC-referrals (β =0.138; p Conclusions GPs with low reported activity in palliative care are more likely to initialise SPHC for palliative care activities they do not deliver themselves for various reasons, which might mean that the involvement of SPHC is substitutive instead of complementary to primary palliative care. This finding and its interpretation should be given more attention in the future policy framework for (specialised) palliative homecare. Abstract

Background: Cognitive impairment is common in patients with PD. Core markers of Alzheimer’s dementia have been related also to PD dementia, but no disease-specific signature to predict PD dementia exists to date. Objectives: The aim of this study was to investigate CSF markers associated with cognition in early PD. Methods: A high-throughput suspension bead array examined 216 proteins in CSF of 74 PD patients in the AETIONOMY project. Cognitive function was assessed with Repeatable Battery for the Assessment of the Neuropsychological Status, Montreal Cognitive Assessment, and Mini-Mental State Examination. Results: Of 69 patients with complete data, 34 had high (≥90) and 35 had low Repeatable Battery for the Assessment of the Neuropsychological Status total score (<90). Of 14 proteins in CSF that differed in levels between groups, increased kininogen-1, validated with several antibodies, was independently associated with lower Repeatable Battery for the Assessment of the Neuropsychological Status and Montreal Cognitive Assessment scores after adjustment for confounders. Although neuropsychiatric symptoms (eg, psychosis, depression, anxiety, behavioral problems) are frequently present, their prevalence and clinical predictors remain unknown. OBJECTIVE: To determine the prevalence and predictors of neuropsychiatric symptoms in late-stage PD. METHODS: We conducted a multinational study of patients with PD with ≥7 years disease duration and either a Hoehn and Yahr stage ≥4 or a Schwab and England score ≤ 50% in the on stage. Neuropsychiatric symptoms were assessed through interviews with carers using the Neuropsychiatric Inventory, with a frequency × severity score ≥ 4, indicating clinically relevant symptoms. The determinants analyzed were demographic characteristics, medication, and motor and nonmotor symptoms. Univariate and multivariate logistic analyses were performed on predictors of clinically relevant neuropsychiatric symptoms. RESULTS: A total of 625 patients were recruited in whom the Neuropsychiatric Inventory could be completed. In 92.2% (576/625) of the patients, at least 1 neuropsychiatric symptom was present, and 75.5% (472/625) had ≥1 clinically relevant symptom. The most common clinically relevant symptoms were apathy (n = 242; 38.9%), depression (n = 213; 34.5%), and anxiety (n = 148; 23.8%). The multivariate analysis revealed unique sets of predictors for each symptom, particularly the presence of other neuropsychiatric features, cognitive impairment, daytime sleepiness. CONCLUSION: Neuropsychiatric symptoms are common in late-stage PD. The strongest predictors are the presence of other neuropsychiatric symptoms. Clinicians involved in the care for patients with late-stage PD should be aware of these symptoms in this specific disease group and proactively explore other psychiatric comorbidities once a neuropsychiatric symptom is recognized.
