Abstract

Despite its crucial role in initiation of cytotoxic immune responses, the molecular pathways underlying antigen cross-presentation remain incompletely understood. The mechanism of antigen exit from endocytic compartments into the cytosol is a long-standing matter of controversy, confronting two main models: transfer through specific channels/transporters or rupture of endocytic membranes and leakage of luminal content. By monitoring the occurrence of intracellular damage in conventional dendritic cells (cDCs), we show that cross-presenting cDC1s display more frequent endomembrane injuries and increased recruitment of endosomal sorting complex required for transport (ESCRT)-III, the main repair system for intracellular membranes, relative to cDC2s. Silencing of CHMP2a or CHMP4b, two effector subunits of ESCRT-III, enhances cytosolic antigen export and cross-presentation. This phenotype is partially reversed by chemical inhibition of RIPK3, suggesting that endocytic damage is related to basal activation of the necroptosis pathway. Membrane repair therefore proves crucial in containing antigen export to the cytosol and cross-presentation in cDCs.

Abstract

Le calcul de la consommation d'énergie par des véhicules en circulation demande un équipement de capteurs embarqués; cette étude a pour objet de permettre une évaluation assez précise de la consommation sans appareillage. Elle s'appuie pour cela sur une approche nouvelle de l'analyse de la cinématique des déplacements, lesquels sont découpés en constituants élémentaires successifs ou séquences. A partir d'un important fichier de données cinématiques enregistrées sur des véhicules dans leurs conditions usuelles d'utilisation, il a été identifié 4 classes de séquences et 3 règles de successions de séquences au sein des déplacements. Les séquences types apparaissent comme des cycles de conduite représentatifs des conditions de circulation. Connaissant les consommations séquentielles (par mesure embarquée ou sur banc d'essai) il devient facile de reconstituer les déplacements et leur consommation avec un simple cumul. La validité de ces résultats a fait l'objet d'une étude approfondie. Calculating power consumption by operating vehicles requires on-board sensors. The aim of this survey is to make a fairly accurate assessment of consumption without equipment. This is based on a new approach to the analysis of trip kinematics by breaking the trips down into successive elementary constituents or sequences. From a large file of kinematic data recorded for the vehicles under actual operating conditions, four classes of sequences were identified along with three rules for sequence succession during trips. Typical sequences appear as driving cycles that are representative of operating conditions. When the sequental consumptions are known (by on-board measurement or on a test bench), it becomes easy to reconstitute the trips and their consumption by a simple cumulation. The validity of these findings has been analyzed in depth.

Abstract

We describe the first case of thrombotic thrombocytopenic purpura (TTP) in a patient with Legionnaires’ disease. TTP was demonstrated by the low level of adams TS13 activity that resolved after plasmapheresis and remained normal in the long-term follow up.

Abstract

The genus Bartonella now includes four species which may infect humans : B. bacilliformis, B. quintana, B. henselae, and B. elizabethae. B. bacilliformis, the agent of Carrion's disease, was the only species of the genus since 1993 when Rochalimaea species were removed from the genus Rochalimaea and included in the genus Bartonella, within the family Bartonellaceae. B. quintana is the etiologic agent of trench fever, bacillary angiomatosis, septicemia, endocarditis, and chronic lymphadenopathy. B. henselae is responsible for bacillary angiomatosis, peliosis of the liver or the spleen, septicemia, endocarditis, and cat scratch disease. There is a single isolate of B. elizabethae, which was recovered from the blood of a patient involved with endocarditis. The spectrum of clinical manifestations related to Bartonella species has extended since 1990, partly because of newly available molecular biological techniques. However, some aspects of Bartonella-related diseases remain unsettled, including epidemiology, physiopathology, and optimum therapy to be administered.

Abstract

Eastern France was not known as a region endemic for tularemia until year 2006. We report on 2 cases of typhoidal tularemia in Eastern France, a 43-year-old hospitalized woman and her husband. Diagnosis was established after fever clearance on serodiagnosis criteria. The source of infection is unclear. As persons in the same family may likely be exposed to a common zoonotic source of infection, tularemia should be considered in the etiologies of familial fever epidemics.

Abstract

International audience; CONTEXT: Tularemia is a zoonosis affecting humans and hares in France. We describe the results of surveillance in both species, in 2007 and 2008. METHODS: Human tularemia cases are mandatorily notifiable in France since 2003. In hares, surveillance relies on volunteer hunter associations in all districts of the country. Data from mandatory reports and volunteer surveillance in 2007/2008 were analyzed and compared with previous results. RESULTS: In 2007/2008, 144 cases were reported in humans and 117 cases in hares. This was a 100% increase compared to previous years. Human cases differed from those of previous years only by the frequency of contact with breeding animals. Human cases without any documented risk exposure were also more frequent. CONCLUSION: An increase of tularemia cases occurred in 2007/2008 in both species. Complementary studies are needed to identify the species reservoir in France to understand the causes of this peak of cases.

Abstract

OBJECTIVE: We estimated the adequacy of antibiotic therapy to guidelines for nosocomial and community-acquired urinary tract infections in hospital. DESIGN: For 4 weeks, all adult patients hospitalized with positive bacteriuria were included in our retrospective study. Data was collected from urine culture results and from patient medical files. Adequacy to guidelines was analyzed by two infectious disease specialists, focusing on the indication, antibiotic choice, dosage, route of administration, and duration of treatment. RESULTS: Overall 202 patients were enrolled in the study (63.9% women). The decision of initiating or not antibiotic therapy was appropriate in 66.8% of cases. Antibiotherapy indication and antibiotic choice were adequate in 94 cases in empiric prescription (50.8%) and in 123 cases (60.9%) after receiving culture antibiogram results. Route of administration was adequate in 94.4% and dosage in 70.8% of prescriptions. This poor compliance with guidelines was mainly due to unnecessary prescriptions in asymptomatic bacteriuria, unnecessary biotherapies and spectrum errors. CONCLUSIONS: It seems important to remind prescribers of recommendations for urinary tract infections.

Abstract

Human brucellosis is now a rare disease in countries where eradication programs (especially vaccination) against brucellosis in cattle, sheep, and goats have been successfully implemented. In France, fewer than 50 brucellosis cases are annually notified to the National Institute for Infection Surveillance. Human brucellosis, however, remains endemic in the Mediterranean basin, Middle East, Western Asia, Africa, and South America. Shortcomings of standard diagnostic methods for brucellosis (variable sensitivity of culture, frequent serological cross reactions) have been only partially resolved by modern molecular biology techniques. There are now 3 new challenges to be faced by the medical and veterinarian community: the expanding wildlife reservoir of brucellosis, with a possible impact on domestic animals; the emergence of Brucella. melitensis infections in cattle, for which prophylactic efficacy of available vaccines has not been established; and recent recognition of a huge animal reservoir of Brucella species in marine mammals, for which the potential virulence in humans remains unknown.
