Abstract

The cause of degenerative changes to the intervertebral disc that can produce back pain are still indefinable. One newer avenue of investigation includes the analysis of vertebral endplates, which lie between vertebral bodies and their adjacent discs. This thesis investigates the structure and function of the vertebral endplate. Multiphoton microscopy was employed as the main imaging method as it has previously shown its ability to differentiate between tissue in osteochondral regions, and its ability to image label free, thick samples reduces sample processing. Animal endplate models were utilised as a healthy model, alongside degenerate human samples from surgery. This thesis aimed to investigate the structure of the vertebral endplate as well as its function. The structure of the endplate was confirmed to contain three distinct regions; the bony endplate, mineralised cartilage and non-mineralised cartilage. The thickness of the cartilage endplate was shown to be thicker in the cranial endplate (p = 0.008), and the mineralised region thicker than the non-mineralised (p = 0.005). Additionally, this study confirmed how anchorage of the intervertebral disc is achieved. Microstructurally, annular bundles penetrate through both the mineralised and non-mineralised regions of the cartilage endplate. Within these layers, the bundles sub-divide in a branched structure as an integration mechanism. Energy disperse spectroscopy showed a higher calcium to phosphate ratio in young adult ovine spines (p<0.05) compared with mature ovine spines, and age differences in the structure of the bony endplate were also noted. Additionally, Raman microscopy on bovine sections showed higher mineralisation in the endplate under the nucleus than annulus using the mineral to matrix ratio (peak ratios of 3.15 and 2.35 respectively). This work highlights the specificity of each of the tissues in the endplate, and how they change based on their location (nucleus/annulus, caudal/cranial) and with age. Degenerative human samples showed higher two photon fluorescence (TPF) signal than the bovine model, and also showed a highly aligned collagen region, parallel to the tidemark which agrees with previous literature. An important function of the endplate is its role as a nutritional pathway for the diffusion of solutes through the endplate and the convective transport of water. Vessels within the endplate were imaged using multiphoton microscopy. They were measured to be as small as 5 ¹m in diameter and within close proximity to the disc (< 70 μm), though never crossing the tide mark. Additionally, the use of a fluorescent tracer confirmed the diffusion from the disc into vessels in the endplate. Through the use of real-time imaging of a fluorescent tracer diffusing from the disc through the endplate, this study showed the tidemark as a barrier, initially holding the tracer up. The study additionally confirmed the presence of small pores in the mineralised cartilage post tidemark. Further relating to the function of the endplate, a series of compressive loading experiments were carried out in order to provide novel insight into the micromechanics of the regions of the endplate. The disc displayed the highest amount of micro-strain with measurements reaching as high as - 40 %, though initial strain appears primarily due to fibre realignment. This work highlights the importance of the cartilage components of the endplate in the micromechanics of the disc. Previously, the vertebral endplate has been underappreciated within the literature and clinical understanding of its role. This works highlights the intimacy between the intervertebral disc and vertebral endplate reinforcing the necessity for further investigation of this unique region.

3. Living with rheumatoid arthritis during the coronavirus pandemic: a longitudinal interview study

Abstract

Background/Aims The COVID-19 pandemic placed patients with rheumatoid arthritis (RA) at increased risk of poor outcomes as a result of their condition, compounded by use of immunosuppressant medication, and higher prevalence of comorbidities. As a consequence, some patients were instructed within the UK to follow strict guidelines to ‘‘shield’’, severely restricting routine social interactions. This study explored patients’ longitudinal experiences of living with RA during the COVID-19 pandemic. Methods Patients with rheumatoid arthritis, from a community hospital-based rheumatology service, participated in two semi-structured telephone interviews at baseline in autumn 2020 and 2-4 months later. Interviews were recorded and transcribed verbatim. Interpretative phenomenological analysis was undertaken by two members of the research team with input from two patient partners (KR and MB). Results 15 participants (9 females, 10 retired, age range 45-79 years) were interviewed twice. Five themes were identified: i) fear, ii) social wellbeing, iii) physical health, iv) pre-existing self-management of RA as a coping mechanism, and v) vulnerability. The overriding emotion was one of fear of contracting COVID-19, which remained high throughout both interviews. Fear was influenced by patients’ existing knowledge of their RA and medications and the presence of other significant co-morbidities. Further influences on fear included mainstream media reports (increasing reporting of deaths and new variants) and personal knowledge (family and friends who had contracted COVID-19). The impact on social wellbeing became more pronounced as remote communications could not replicate the benefits of physical interaction. Participants reported no impact on their physical health, with increased rest resulting from restricted social interaction perceived to be beneficial. Many participants utilised the resilience they had learned as a result of having RA to cope, including stress management, pacing, and exercise. Being categorised as ‘‘clinically extremely vulnerable’’ led to a reassessment of self-identity, with participants not wanting to be perceived as being weak or helpless. Finally, many participants used lockdown to reflect on and reassess their personal priorities. Conclusion This longitudinal interview study with 15 people with RA highlights that the main impact of the pandemic appeared to be on emotional wellbeing brought about by fear of COVID-19, later compounded by lack of social interaction. In this small study, participants’ physical health was reported to be stable and participants were able to use self-management skills to cope. The realisation of the seriousness of contracting COVID-19 led to feelings of vulnerability and a reassessment of self-identity. The study raises important issues for those providing healthcare to people with RA, including effective communication with awareness of its likely impact, using pre-existing selfmanagement strategies to enhance wellbeing, and recognition of the potential for social isolation and the implications thereof.

Abstract

PURPOSE: To examine nursing students' perceptions of Experts by Experience impact on theoretical and practical learning. DESIGN AND METHODS: Qualitative exploratory study involving focus groups with undergraduate nursing students from five European countries and Australia. Data were analyzed thematically. FINDINGS: Participants described positive impacts as: bridging the theory and practice gap through first-hand experience, including sub-themes: bringing theory to life; can't be taught any other way, and innovative teaching methods fueling curiosity. RELEVANCE TO CLINICAL PRACTICE: Integrating theory and practice is key for quality mental health nursing practice. Experts by experience can potentially contribute to reducing this enduring gap.

Abstract

Reform to nursing education is essential to ensure future generations of nurses are strongly positioned to value, know, and deliver strength-based, recovery-oriented mental health practice. A promising pathway to effectively drive reform is the coproduction of curricula by nursing academics and people with lived experience of recovery from mental distress referred to as Experts by Experience. The Co-production in Mental Health Nursing Education (COMMUNE) project is an international collaboration for development and implementation of consumer coproduced curricula. This study evaluated the inclusion of Expert by Experience-led mental health nursing education on nursing students' attitudes to people labelled with mental illness, mental health nursing, and consumer participation. A repeated self-report measures design was implemented in Australia, Ireland, and Finland to ascertain level of generalizability of consumer involvement within undergraduate nursing programmes. Data were collected from nursing students (n = 194) immediately before and after the education module, using three self-report instruments on attitudes (Mental Health Nurse Education Survey, Consumer Participation Questionnaire, and Opening Minds Scale). Data were analysed using descriptive and inferential statistics. Eighty-nine per cent of the 27 points of change reflected more favourable and accepting attitudinal change. Of these, 41% were significant at Bonferroni adjusted alpha of 0.0025. There was a statistically significant increase in preparedness for practice in the mental health field in each of the three countries. The most pronounced change is related to the social and systemic inclusion of people with a diagnostic label and recovery-oriented care more broadly.

Abstract

Consumer participation is a clear expectation of contemporary mental health policy. Most activity has concentrated in direct service delivery, and academic roles for mental health consumers have been slow to establish. An international project was undertaken to implement and evaluate meaningful consumer involvement in mental health nursing education. A learning module was co-produced between 'Experts by Experience' (drawing on experience of mental distress and service use) and Mental Health Nurse Academics. This qualitative exploratory study aimed to capture how Experts by Experience perceive their contribution. Interviews were undertaken with Experts by Experience who delivered the learning module. Data were analysed thematically and subsequently interpreted with Critical Social Theory. Two main themes emerged from the findings: 'there wasn't a barrier' described how personal narratives enhanced relationships between Experts by Experience and students; and 'made the human being visible', described their experiences of allowing students to see the person behind a diagnosis. These findings suggest Experts by Experience teaching is valuable and potentially a tool in redressing stigma. Addressing poor public perceptions could attract higher numbers of quality practitioners to mental health and meet identified workforce shortages. The findings presented here strengthen the evidence base for Expert by Experience roles in mental health professional education. These findings can be considered in international curricula reviews and aid progress towards a more socio-political, humanistic focus in mental health nursing, congruent with rights-based reform agendas.

Abstract

Holistic and person-centred nursing care is commonly regarded as fundamental to nursing practice. These approaches are complementary to recovery which is rapidly becoming the preferred mode of practice within mental health. The willingness and ability of nurses to adopt recovery-oriented practice is essential to services realizing recovery goals. Involving consumers (referred herein as Experts by Experience) in mental health nursing education has demonstrated positive impact on the skills and attitudes of nursing students. A qualitative exploratory research project was undertaken to examine the perspectives of undergraduate nursing students to Expert by Experience-led teaching as part of a co-produced learning module developed through an international study. Focus groups were held with students at each site. Data were analysed thematically. Understanding the person behind the diagnosis was a major theme, including subthemes: person-centred care/seeing the whole person; getting to know the person, understanding, listening; and challenging the medical model, embracing recovery. Participants described recognizing consumers as far more than their psychiatric diagnoses, and the importance of person-centred care and recovery-oriented practice. Understanding the individuality of consumers, their needs and goals, is crucial in mental health and all areas of nursing practice. These findings suggest that recovery, taught by Experts by Experience, is effective and impactful on students' approach to practice. Further research addressing the impact of Experts by Experience is crucial to enhance our understanding of ways to facilitate the development of recovery-oriented practice in mental health and holistic and person-centred practice in all areas of health care.

24. 'It could probably help someone else but not me': a feasibility study of a snack programme offered to meals on wheels clients

Abstract

Objectives Community-based services, such as Meals on Wheels (MOW), allow older adults to remain in their homes for as long as possible. Many MOW recipients experience decreased appetite that limits intake at mealtimes. This pilot study aimed to determine the feasibility of providing high protein high energy snacks to improve nutrient intakes of MOW clients in a regional centre of New South Wales, Australia. Participants A convenience sample of 12 MOW clients. Intervention Participants received snacks five times a week, in addition to their usual MOW order, for four weeks. Measurements Nutritional status was assessed using the Mini Nutritional Assessment tool. Pre-post changes in dietary intake were assessed using a diet history and food frequency questionnaire. Qualitative interviews conducted in clients’ homes were digitally recorded, transcribed verbatim, and themes identified. Results Post-intervention, there was a trend for an increased energy (mean = +415kJ (SD=1477)/day) and protein (+7.2 (±14.06) g/day) intake. MNA scores significantly increased (P= 0.036) and proportion of respondents categorised as ‘malnourished’ or ‘at risk of malnutrition’ decreased from 17% to 8%, and 67% to 25%, respectively (P Conclusion Provision of an additional daily mid-meal snack may be a useful addition to existing MOW services, for improved energy and protein intakes. However, not all MOW clients at risk of malnutrition perceived the snacks to be beneficial to them.

Abstract

Two charts are proposed for calculating the flow coefficient and the area correction factor used in the equation for the flow rate through a sharp-edged orifice. The proposed charts account for variations in the discharge coefficient of sharp-edged orifices and can be used with any pressure ratio for both subcritical and supercritical flow conditions. They can also be used for any gas by using the appropriate gas constant and ratio of specific heats. The application of the charts is illustrated by examples.

37. Effects of side lying on lung function in older individuals.

Abstract

BACKGROUND AND PURPOSE: Body positioning exerts a strong effect on pulmonary function, but its effect on other components of the oxygen transport pathway are less well understood, especially the effects of side-lying positions. This study investigated the interrelationships between side-lying positions and indexes of lung function such as spirometry, alveolar diffusing capacity, and inhomogeneity of ventilation in older individuals. SUBJECTS AND METHODS: Nineteen nonsmoking subjects (mean age=62.8 years, SD=6.8, range=50-74) with no history of cardiac or pulmonary disease were tested over 2 sessions. The test positions were sitting and left side lying in one session and sitting and right side lying in the other session. In each of the positions, forced vital capacity (FVC), forced expiratory volume in 1 second (FEV1), single-breath pulmonary diffusing capacity (DLCO/VA), and the slope of phase III (DN2%/L) of the single-breath nitrogen washout test to determine inhomogeneity of ventilation were measured. RESULTS: Compared with measurements obtained in the sitting position, FVC and FEV1 were decreased equally in the side-lying positions, but no change was observed in DLCO/VA or DN2%/L. CONCLUSION AND DISCUSSION: Side-lying positions resulted in decreases in FVC and FEV1, which is consistent with the well-documented effects of the supine position. These findings further support the need for prescriptive rather than routine body positioning of patients with risks of cardiopulmonary compromise and the need to use upright positions in which lung volumes and capacities are maximized. Abstract

Circuit and electronic component design for Office of Civil Defense radiological instruments were studied. Three, transistorized, dc-to-dc converter power supplies were designed and test results are reported. Two electrometer amplifiers were designed to operate on these converters, and their theoretical operating time on one standard D cell is 500-3000 hr. Other activities reported are an investigation of integrated circuit techniques and their compatibility with circuit design, radiation effects on semiconductors, and an evaluation of D cell kits and a comparison of their capabilities with commercially manufactured cells. Abstract

Circuit and electronic component designs for the Office of Civil Defense radiological instrumentation were studied. Miniature gas-discharge ionization chamber instruments were studied, and two experimental modules were constructed and tested. Additional observations and data were recorded to determine radiation effects on metal-oxide field-effect transistors. A commercial operational amplifier was studied for application to electrometer type radiological instruments. A study of proportional counters for use in gamma survey meters was started. Abstract

Objective Rheumatoid arthritis (RA) is an autoimmune, inflammatory, systemic condition that requires specific drug treatment to suppress disease activity and prevent joint deformity. To manage the ongoing symptoms of joint pain and fatigue patients are encouraged to engage in self-management activities. People with RA have an increased incidence of serious illness and mortality, with the potential to impact on quality of life. This study explored patients’ experiences of living with RA on physical, psychological, and social wellbeing as well as their ability to employ self-management skills during the coronavirus pandemic. Design Qualitative, longitudinal (baseline, 16th September to 23rd November 2020 and after 2-4 months, 11th January to the 17th January 2021), semi-structured telephone interviews. Setting A rheumatology service based in a community hospital. Participants 15 adults with rheumatoid arthritis. Main Outcomes Data were analysed using interpretative phenomenological analysis. Results Five themes were identified which related to impact on i) fear: the dominant emotion, ii) social connections and work practices, iii) physical health, iv) identity and v) self-management as a coping mechanism. The overriding emotion was one of fear, which remained high throughout both interviews. The negative impact on social wellbeing increased as the pandemic progressed. Conversely, physical health was not affected at either time point, although participants reported difficulty in interpreting whether physical symptoms were attributable to their RA or COVID. Recognition of increased vulnerability led to a reassessment of self-identity, however respondents reported using previously learnt self-management techniques to cope in the context of the pandemic. Conclusions The main impact was on emotional and social wellbeing. Levels of fear and vulnerability which affected self-identity remained high throughout the pandemic and the impact on social wellbeing increased over time. Physical health remained largely unaffected. Self-management skills were used to maintain a sense of wellbeing.

Abstract

Involving research users in setting priorities for research is essential to ensure research outcomes are patient-centred and to maximise research value and impact. The Musculoskeletal (MSK) Disorders Research Advisory Group Versus Arthritis led a research priority setting exercise across MSK disorders. The Child Health and Nutrition Research Initiative (CHRNI) method of setting research priorities with a range of stakeholders was utilised, involving four stages and two surveys, to: 1) gather research uncertainties; 2) consolidate these; 3) score uncertainties against importance and impact; and 4) analyse scoring, for prioritisation. 213 and 285 people responded to the first and second surveys respectively, representing clinicians, researchers and people with MSK disorders. Key priorities included developing and testing new treatments, better treatment targeting , early diagnosis, prevention and better understanding and management of pain, with an emphasis on understanding underpinning mechanisms. We present a call to action to researchers and funders to target these priorities.
