Olanzapine, an atypical antipsychotic drug, was administered to a patient with Huntington's disease (HD) with marked choreiform movements. Brain SPECT with 99mTc-HMPAO was performed before and after treatment. Brain SPECT imaging has been performed in patients with HD in order to determine the status of basal ganglia perfusion. The use of brain SPECT with 99mTc-HMPAO before and after treatment in patients with HD has not been yet reported. The marked hypoperfusion of the basal ganglia on brain SPECT performed before therapy with olanzapine improved significantly after treatment.

Pilonidal cysts and sinuses are described as dermoid cysts which contain follicles of hairs and sebaceous glands. They clinically present as a classic case of inflammation which comes with pain, local infection and redness. The origin of pilonidal disease remains controverse. There are many hypothesis as lack of hygiene on the affected area and a penetration and growth of a hair in the subcutaneus tissue caused by constant friction or direct trauma on the damaged area. The option for clinical treatment is very frequent. However, taking into consideration the incidence and the possibility of recidive, surgical treatment is presently recommended. Complications include cellulitis and abscess formation. Pilonidal cysts are mostly found on the sacral region. In the literature is found description of pilonidal cysts on the penis, interdigital region on the hands as well as on the cervical region. We present a case of pilonidal cyst located on the vault biparietal region, without malignant degeneration.

5. Analyzing Cooperative Monitoring and Dissemination of Critical Mobile Events by VANETs

The treatment of mobile and simultaneous critical urban events requires effective actions by the appropriate authorities. Additionally it implies communication challenges in the speed and accuracy of their occurrence by the entities, as well as dealing with the dynamics and speed in these environments. Cooperative solutions with shared resources that address these challenges become a real option in helping to handle these events. This paper presents an evaluation of dynamic monitoring and collaborative dissemination supported by vehicular groups. It aims to analyze the impact of multiple mobile and fixed events in an urban environment on information propagation, considering barriers imposed by the events and the environment. Differently from other studies in the literature, this work takes into account both fixed and mobile events, as well as simultaneous events. NS3 results show that the evaluated system monitored at least 87% and 51.5% of the time for mobile and fixed events respectively, and delivered information over 77% and 50% of the time for those events, with average delay remains close to 0.3s in most scenarios. The results also reveal that a more continuous monitoring of the mobile events is highly dependent on the orientation of the vehicles. The main contribution of this work consists of the performance analysis of both fixed and mobile simultaneous events to support studies on how moving events impact on the dissemination and delivery of real-time data, and thus encouraging the development of new data dissemination protocols for VANETs., Comment: This work has been submitted to the Wireless Networks (WINE) journal for possible publication

Cloud Network Slicing is a new research area that brings together cloud computing and network slicing in an end-to-end environment. In this context, understanding the existing scientific contributions and gaps is crucial to driving new research in this field. This article presents a complete quantitative analysis of scientific publications on the Cloud Network Slicing, based on a systematic mapping study. The results indicate the situation of the last ten years in the research area, presenting data such as industry involvement, most cited articles, most active researchers, publications over the years, main places of publication, as well as well-developed areas and gaps. Future guidelines for scientific research are also discussed.

Background: Enterotoxigenic Escherichia coli (ETEC) strains cause infectious diarrhea and colonize host intestine epithelia via surface-expressed colonization factors. Colonization factor antigen I (CFA/I), a prevalent ETEC colonization factor, is a vaccine target since antibodies directed to this fimbria can block ETEC adherence and prevent diarrhea. Methods: Two recombinant antigens derived from CFA/I were investigated with a vaccine adjuvant system that displays soluble antigens on the surface of immunogenic liposomes. The first antigen, CfaEB, is a chimeric fusion protein comprising the minor (CfaE) and major (CfaB) subunits of CFA/I. The second, CfaEad, is the adhesin domain of CfaE. Results: Owing to their His-tag, recombinant CfaEB and CfaEad, spontaneously bound upon admixture with nanoliposomes containing cobalt-porphyrin phospholipid (CoPoP), as well as a synthetic monophosphoryl lipid A (PHAD) adjuvant. Intramuscular immunization of mice with sub-microgram doses CfaEB or CfaEad admixed with CoPoP/PHAD liposomes elicited serum IgG and intestinal IgA antibodies. The smaller CfaEad antigen benefitted more from liposome display. Serum and intestine antibodies from mice immunized with liposome-displayed CfaEB or CfaEad recognized native CFA/I fimbria as evidenced by immunofluorescence and hemagglutination inhibition assays using the CFA/I-expressing H10407 ETEC strain. Conclusion: These data show that colonization factor-derived recombinant ETEC antigens exhibit immunogenicity when delivered in immunogenic particle-based formulations.

Background. Enterotoxigenic E. coli (ETEC) is a principal cause of diarrhea in travelers, deployed military personnel, and children living in low to middle-income countries. ETEC expresses a variety of virulence factors including colonization factors (CF) that facilitate adherence to the intestinal mucosa. We assessed the protective efficacy of a tip-localized subunit of CF antigen I (CFA/I), CfaE, delivered intradermally with the mutant E. coli heat-labile enterotoxin, LTR192G, in a controlled human infection model (CHIM). Methods. Three cohorts of healthy adult subjects were enrolled and given three doses of 25 μg CfaE + 100 ng LTR192G vaccine intradermally at 3-week intervals. Approximately 28 days after the last vaccination, vaccinated and unvaccinated subjects were admitted as inpatients and challenged with approximately 2 × 10
7 cfu of CFA/I+ ETEC strain H10407 following an overnight fast. Subjects were assessed for moderate-to-severe diarrhea for 5 days post-challenge. Results. A total of 52 volunteers received all three vaccinations; 41 vaccinated and 43 unvaccinated subjects were challenged and assessed for moderate-to-severe diarrhea. Naïve attack rates varied from 45.5% to 64.7% across the cohorts yielding an overall efficacy estimate of 27.8% (95% confidence intervals: −7.5–51.6%). In addition to reducing moderate–severe diarrhea rates, the vaccine significantly reduced loose stool output and overall ETEC disease severity. Conclusions. This is the first study to demonstrate protection against ETEC challenge after intradermal vaccination with an ETEC adhesin. Further examination of the challenge methodology is necessary to address the variability in naïve attack rate observed among the three cohorts in the present study. [ABSTRACT FROM AUTHOR]- Published

Many IoT scenarios demand continuous capture of information from multifunctional sensors and smart units, as well as sending those data to cloud centers. However, allocating tasks to these sensors is not straightforward due to the urgency and priority that each type of data collection requires depending on the needs of the urban environment. This paper presents the POSITRON scheme for managing the sensing allocation in a multifunctional IoT network from previously defined policies. The policies take into account the characteristics of the applications running on the network and the different specifications of the available devices. We implemented POSITRON in a network simulator aiming to analyze its efficiency in allocating network resources. The results point out that considering the requirements demanded by applications and the distinct characteristics of multifunctional IoT devices brings benefits to resource allocation. CD161 expression on CD4+ T cells is associated with a Th17 functional phenotype, as well as with an innate capacity to respond to interleukin (IL)-12 and IL-18 without T cell receptor (TCR) stimulation. Chronic HIV-1 infection is associated with loss of the CD161+ CD4 T cell population, and non-human primate studies suggest that their depletion is associated with disease progression. However, the dynamics of the CD161+ CD4+ T cell population during acute HIV-1 infection remains unknown. In this study, we characterize peripheral blood CD161+ CD4+ T cells in detail, and examine how they are affected during the earliest stages of HIV-1 infection. Unbiased surface proteome screening and principal component analysis indicated that CD161+ CD4+ T cells are relatively phenotypically homogeneous between donors, and are intermediates between conventional CD4 T cells and innate-like T cells. In acute untreated HIV-1 infection, the circulating CD161+ CD4+ T cell population decreased in frequency, as did absolute cell counts starting from peak viral load, with elevated levels of activation and exhaustion markers expressed throughout acute HIV-1 infection. The capacity of these cells to respond to stimulation with IL-12 and IL-18 was also reduced. Early initiation of anti-retroviral treatment (ART) during acute HIV-1 infection restored the functionality of peripheral blood CD161+ CD4+ T cells, but not their frequency. In contrast, early ART initiation prevented the decline of colonic CD161+ CD4+ T cells that otherwise started during acute infection. Furthermore, loss of peripheral and colonic CD161+ CD4+ T cells in untreated infection was associated with levels of viral load. These results suggest that acute HIV-1 infection has profound effects on the CD161+ CD4+ T cell population that could not be completely prevented by the initiation of ART.

Introduction: Enterotoxigenic E. coli (ETEC) is a leading cause of diarrhea in travelers as well as for children living in low- to middle-income countries. ETEC adhere to intestinal epithelium via colonization factors (CFs). CFA/I, a common CF, is composed of a polymeric stalk and a tip-localized minor adhesive subunit, CfaE. Vaccine delivery by the transcutaneous immunization of dscCfaE was safe but was poorly immunogenic in a phase 1 trial when administered to volunteers with LTR(192G) and mLT. To potentially enhance the immunogenicity of CfaE while still delivering via a cutaneous route, we evaluated the safety and immunogenicity of two CfaE constructs administered intradermally (ID) with or without mLT. Methods: CfaE was evaluated as a donor strand-complemented construct (dscCfaE) and as a chimeric construct (Chimera) in which dscCfaE replaces the A1 domain of the cholera toxin A subunit and assembles non-covalently with the pentamer of heat-labile toxin B (LTB). Subjects received three ID vaccinations three weeks apart with either dscCfaE (1, 5, and 25 µg) or Chimera (2.6 and 12.9 µg) with and without 0.1 µg of mLT. Subjects were monitored for local and systemic adverse events. Immunogenicity was evaluated by serum and antibody-secreting cell (ASC) responses. Results. The vaccine was well-tolerated with predominantly mild and moderate local vaccine site reactions characterized by erythema, induration and post-inflammatory hyperpigmentation. High rates of serologic and ASC responses were seen across study groups with the most robust responses observed in subjects receiving 25 µg of dscCfaE with 0.1 mcg of LT(R192G). Conclusion: Both ETEC adhesin vaccine prototypes were safe and immunogenic when co-administered with mLT by the ID route. The observed immune responses induced with the high dose of dscCfaE and mLT warrant further assessment in a controlled human infection model. [ABSTRACT FROM AUTHOR]

O gerenciamento manual de ativos de rede está suscetível a erros de configuração, falta de padronização, grande quantidade de trabalho repetitivo e pouca ou nenhuma rastreabilidade de alterações ao longo do tempo. A abordagem Infrastructure as Code (IaC) permite automatizar o processo de configuração de recursos como sistemas operacionais, serviços de rede, contêineres e aplicações, tratando-os como software e possibilitando a padronização e reversão da configuração. Este artigo propõe uma arquitetura integrada a partir de ferramentas de softwares diferentes e que usam a abordagem IaC, a fim de automatizar a configuração de ativos de rede, considerando diferentes modelos e fabricantes. Uma avaliação quantitativa mostra a eficiência da arquitetura em tempo de resposta e escalabilidade. A arquitetura obteve um ganho proporcional de 83% no tempo médio de resposta para gerenciar 128 ativos, sem aumento significativo no uso de processamento e memória.

O panóptico não era apenas uma construção física, mas correspondia à transposição para a realidade do ideário político-social de uma época. Retrata uma relação de poderes em que o sujeito se vê controlado por um outro, cuja invisibilidade é catalisadora da percepção de vigilância e opressão. A partir do supereu, o representante da cultura no sujeito, busca-se articular os dois conceitos e, do encontro, abordar as mudanças oriundas da mudança do lugar do pai na contemporaneidade e suas implicações na subjetivação.

OBJECTIVE: To evaluate the presence of menstrual headaches prior to pregnancy according to the International Headache Society (IHS) classification criteria, 2004, and also study the outcome (frequency and intensity) of these pre-existing headaches during the gestational trimesters. METHOD: This study involved 1,101 pregnant women (12 to 45 years old). A semi-structured questionnaire was used to interview the women during the first, second and third gestational trimesters as well as after delivery. All the interviews were conducted by one of the researchers by applying the IHS Classification (IHSC-2004). RESULTS: A 1,029 women out of the 1,101 women interviewed presented headaches prior to gestation, which made it possible to study headaches in 993 women during the gestational trimesters. Menstrually related headaches were presented by 360 of the 993 women. Migraine was reported by 332/360 women (92.22%) with menstrual headaches and 516/633 women (81.51%) without menstrual headaches, respectively, prior to gestation. The majority of the women with menstrual migraine presented a headache improvement or disappearance during gestation (62.22% during the first trimester; 74.17% during the second trimester; 77.78% during the third trimester). CONCLUSION: Most of the pregnant women with menstrual or non-menstrual headaches prior to gestation presented migraine, which either improved or disappeared during pregnancy. Women who suffered from non-menstrual headaches improved during pregnancy but not as much as women with menstrual headaches.OBJETIVO: Avaliar a presença de cefaléia relacionada ao ciclo menstrual, antes da gestação, classificá-las, segundo os critérios da Sociedade Internacional de Cefaleia (SIC) de 2004, e estudar o comportamento (freqüência e intensidade) dessas cefaléias pré-existentes à gestação durante os trimestres gestacionais. MÉTODO: Foram estudados 1101 mulheres grávidas (12 a 45 anos) entrevistadas através de questionário semi-estruturado durante o primeiro, o segundo e o terceiro trimestres gestacionais e imediatamente após o parto. Todas as entrevistas foram conduzidas por um dos autores, usando a classificação da SIC, 2004. RESULTADOS: De 1101 mulheres, 1029 apresentavam cefaléia antes da gestação, sendo possível estudar o comportamento das cefaléias durante os trimestres gestacionais em 993. Apresentaram cefaléia relacionada à menstruação 360/993 mulheres. Encontramos migrânea em, 332/360 (92,22%) mulheres com cefaléia menstrual e em 516/633 (81,51%) mulheres com cefaléia não menstrual antes da gestação. A maioria das mulheres com migrânea menstrual apresentou melhora da cefaléia durante a gestação (62,22% no primeiro trimestre; 74,17% no segundo trimestre; 77.78% no terceiro trimestre). CONCLUSÃO: A maioria das mulheres grávidas, com cefaléia tanto menstrual quanto não menstrual, antes da gestação, apresentou migrânea, sendo que a mesma melhora ou desaparece durante a gestação. Mulheres com cefaléia não menstrual antes da gestação melhoram em proporção menor do que aquelas com cefaléia menstrual.

INTRODUCTION: Patients with severe brain lesions (SBL) and brain-dead patients (BD) frequently present with vasopressin (AVP) secretion disorders. OBJECTIVE: To evaluate AVP serum levels in SBL and BD patients. DESIGN: Prospective, open label, observational trial. SETTING: A general teaching hospital. METHOD: Three groups of adult subjects (age> 18y) of both sexes were included in this study: control group: 29 healthy volunteers; SBL group: 17 patients with Glasgow Coma Scale (GCS)< 8; and BD group: 11 brain-dead patients. Samples of venous blood were collected in the morning at rest from healthy volunteers and at 8 hourly intervals over a period of 24h from SBL and BD patients for AVP determinations. Concomitantly, some clinical and laboratorial variables were also recorded. RESULTS: AVP serum levels (pg/ml) were [mean (SD); median]: control [2.2(1.1); 2.0]; SBL [5.7(6.3); 2.9]; and BD [2.6(1.0); 2.8]. AVP serum levels varied greatly in SBL patients, but without statistically significant difference in relation to the other groups (p=0.06). Hypotension (p=0.02), hypernatremia (p=0.0001), serum hyperosmolarity (p=0.0001) and urinary hypoosmolarity (p=0.003) were outstanding in BD patients when compared with SBL. CONCLUSIONS: The AVP serum levels did not demonstrate significant statistical difference between the groups, only showing a greater variability in SBL patients (manifested as serum spike levels). Hypernatremia and hyperosmolarity were present in BD patients, indicating a failure of the hypothalamic-pituitary system in AVP production and release.INTRODUÇÃO: Pacientes com lesão cerebral grave (LCG) ou com morte encefálica (ME) freqüentemente apresentam alterações na secreção de vasopressina (AVP). We report a case of neurocryptococcosis which is unique in the literature because the patient had a pseudocystic form of the disease during pregnancy and without any evidence of AIDS. The clinical picture was that of intracranial hypertension and the epidemiological background was highly suggestive of cysticercosis. CT showed multiple round hypodense lesions in the basal ganglia and cerebellum, without contrast enhancement. Since a scolex was not visible, the diagnosis of neurocysticercosis was considered probable. CSF examination was not performed in view of its high risk. The patient had progressive downhill course. Autopsy disclosed multiple gelatinous pseudocysts in the cerebral and cerebellar gray matter, containing abundant Cryptococcus neoformans. Meningeal involvement was minimal. The child was delivered by caesarean section and was free of infection, but died later of hyaline membrane disease. The neuroimaging appearances of this rare instance of the pseudocystic form of neurocryptococcosis mimicked closely neurocysticercosis and only postmortem examination allowed correct diagnosis. The pseudocystic form has so far only been reported in AIDS.Relatamos o primeiro caso da forma pseudocística da neurocriptococose na gravidez, em paciente sem evidência de AIDS. O quadro clínico era de hipertensão intracraniana que se manifestou no segundo trimestre da gravidez. A tomografia computadorizada do crânio evidenciava múltiplas lesões hipodensas, císticas, não impregnadas por contraste, localizadas na substância cinzenta cerebral e cerebelar. Dados epidemiológicos, clínicos e tomográficos apontavam para o diagnóstico provável de neurocisticercose. A autópsia, porém, revelou múltiplos pseudocistos de material gelatinoso com Cryptococcus neoformans em abundância. Havia mínimo comprometimento meníngeo. A revisão da literatura mostrou que a forma pseudocística da neurocriptococose só foi até o momento relatada em pacientes com AIDS. Os casos de neurocriptococose na gravidez até hoje publicados foram todos da forma meningítica.

Os autores relatam o caso de uma lactente de 50 dias de vida que foi trazida ao Pronto Socorro Pediátrico do Hospital de Clínicas da UNICAMP com crises convulsivas. Ao exame apresentava-se sem sinais externos de trauma, hipoativa e com crises convulsivas generalizadas. A fundoscopia evidenciou hemorragia retiniana difusa bilateral. Foi submetida a tomografia de crânio que mostrou hemorragia inter-hemisférica e "swelling" cerebral. A criança foi internada na UTI pediátrica, mantida sob ventilação assistida e recebendo hidantal porém, apesar do tratamento, evoluiu para óbito três dias depois. Desde o início havia suspeita de maus-tratos, o que foi confirmado um dia após a internação quando o pai relatou que "chacoalhou" a criança porque ela chorava bastante. O presente estudo tem como objetivo ressaltar aspectos sociais, epidemiológicos e a dificuldade diagnostica desta síndrome que algumas vezes pode ser fatal, como neste caso.The authors report the case of a 50-day-old baby who was brought to the Pediatric Emergency Room of the "Hospital de Clinicas da UNICAMP" with seizures. At the examination she presented without external signs of trauma, hypoactive and with generalized seizures. Numerous hemorrhages were found in the ocular fundi. CT scan showed interhemisferic hemorrhage and brain swelling. The child was admitted to the Pediatric Intensive Care Unit, kept under controlled ventilation and hydantal. Although these measures, she died three days later. Since the beginning there was suspicion of child abuse. One day after the admission the father told that he had shaken the baby because she was crying too much. The present paper discuss social and epidemiological aspects, and about the difficulties in the diagnosis of this syndrome that sometimes may be fatal, as in this case.

We report the case of a 73-year-old female who presented facial numbness and pain in the first division of the trigeminal nerve, ptosis, diplopia and visual loss on the right side for the previous four months. The neurological, radiological and histological examination demonstrated a rare case of invasive fungal aspergillosis of the central nervous system, causing orbital apex syndrome, later transformed in temporal brain abscess. She died ten months later due to respiratory and renal failure in spite of specific antimycotic therapy. Relatamos o caso de mulher de 73 anos de idade que apresentou nos últimos 4 meses amortecimento facial, dor no território do primeiro ramo do nervo trigêmeo, ptose palpebral, diplopia e perda da acuidade visual, à direita. A investigação neurológica, radiológica e histológica demonstrou se tratar de uma lesão no ápice da órbita de origem fúngica, aspergilose com transformação tardia em abscesso cerebral. A paciente faleceu 10 meses após o tratamento inicial embora tenha sido tratada com medicação antifúngica específica.

Olanzapine, an atypical antipsychotic drug, was administered to a patient with Huntington's disease (HD) with marked choreiform movements. Brain SPECT with 99mTc-HMPAO was performed before and after treatment. Brain SPECT imaging has been performed in patients with HD in order to determine the status of basal ganglia perfusion. The use of brain SPECT with 99mTc-HMPAO before and after treatment in patients with HD has not been yet reported. The marked hypoperfusion of the basal ganglia on brain SPECT performed before therapy with olanzapine improved significantly after treatment.

We studied the induction of antigen-specific lgA memory B cells (BM) in volunteers who received live attenuated Shigella flexneri 2a vaccines. Subjects ingested a single oral dose of 107 , 108 or 109 CFU of S. flexneri 2a with deletions in guaBA (CVD 1204) or in guaBA, set and sen (CVD 1208). Antigen-specific serum and stool antibody responses to LPS and lpa B were measured on days 0, 7, 14, 28 and 42. lgA BM cells specific to LPS, lpa Band totallgA were assessed on days 0 and 28. We show the induction of significant LPS-specific lgA BM cells in anti-LPS lgA seroresponders. Positive correlations were found between anti-LPS lgA BM cells and anti-LPS lgA in serum and stool; lgA BM cell responses to lpaB were also observed. Olanzapina, um antipsicótico atípico, foi administrada a uma paciente com doença de Huntington (DH) com acentuados movimentos coreicos. O SPECT cerebral com HMPAO-99mTc foi realizado antes da paciente iniciar qualquer tratamento e após tratamento com olanzapina. O SPECT cerebral tem sido realizado em pacientes com DH para avaliar a perfusão dos núcleos da base. O uso do SPECT cerebral com HMPAO-99mTc antes e após terapia em pacientes com DH ainda não foi relatado. A acentuada hipoperfusão observada nos núcleos da base na imagem de SPECT cerebral, obtida antes da terapia com olanzapina, melhorou de forma significativa após o tratamento.

Pilonidal cysts and sinuses are described as dermoid cysts which contain follicles of hairs and sebaceous glands. They clinically present as a classic case of inflammation which comes with pain, local infection and redness. The origin of pilonidal disease remains controverse. There are many hypothesis as lack of hygiene on the affected area and a penetration and growth of a hair in the subcutaneus tissue caused by constant friction or direct trauma on the damaged area. The option for clinical treatment is very frequent. However, taking into consideration the incidence and the possibility of recidive, surgical treatment is presently recommended. Complications include cellulitis and abscess formation. Pilonidal cysts are mostly found on the sacral region. In the literature is found description of pilonidal cysts on the penis, interdigital region on the hands as well as on the cervical region. We present a case of pilonidal cyst located on the vault biparietal region, without malignant degeneration. Cistos ou seios pilonidais são descritos como cistos dermóides que contêm folículos pilosos, pêlos e glândulas sebáceas. Clinicamente se manifestam com quadro inflamatório clássico traduzido por dor, tumefação local e vermelhidão. A etiopatogenia dos cistos pilonidais permanece controversa. Há várias hipóteses dentre as quais a falta de higiene local e a penetração e crescimento de pêlo no tecido subcutâneo geralmente causado por atrito constante ou trauma sobre o local. A opção pelo tratamento conservador é frequente. Contudo, levando-se em consideração a incidência e a possibilidade de recidivas, esta patologia tem hoje sido tratada cirurgicamente. As complicações comuns do cisto pilonidal incluem celulite, formação de abscesso e recorrência do cisto após o tratamento. Cistos pilonidais são quase na totalidade das vezes encontrados na região sacro-coccígea. Encontramos na literatura descrição de cistos pilonidais no pênis, na região interdigital das mãos e região cervical. Apresentamos um caso de cisto pilonidal de localização biparietal no crânio, sem degeneração maligna.

In this study, we sought to examine the association between cardiovascular risk factors and people's perception on cardiovascular risk.Methods: This was an epidemiological, cross-sectional, descriptive, prospective study of Masonic men aged >40 years in Boa Vista, Brazil. Participants completed a health survey, which included three questions about perception of their stress level, overall health status, and risk of a heart attack. In addition, demographic and biological data were collected.Results: A total of 101 Masonic men took part in the study; their mean age (± standard deviation) was 55.35±9.17 years and mean body mass index was 28.77±4.51 kg/m(2). Answers to the lifestyle questionnaire suggested an overall healthy lifestyle, including good diet and moderate exercise, although despite this ~80% were classified as overweight or obese. The majority of participants felt that they had a low stress level (66.3%), good overall general health (63.4%), and were at low risk of having a heart attack (71.3%). Masons who were overweight were significantly more likely to perceive themselves to be at risk of a heart attack (P=0.025).Conclusion: Despite over half of participants having a moderate to high risk of cardiovascular disease according to traditional risk factors, less than a third perceived themselves to be at high risk. Public health campaigns need to better communicate the significance of traditional cardiovascular risk in order to improve awareness of risk among the general population. Attenuated yellow fever (YF) virus 17D/17DD vaccines are the only available protection from YF infection, which remains a significant source of morbidity and mortality in the tropical areas of the world. The attenuated YF virus vaccine, which is used worldwide, generates both long-lasting neutralizing antibodies and strong T-cell responses. However, on rare occasions, this vaccine has toxic side effects that can be fatal. This study presents the design of two non-viral DNA-based antigen formulations and the characterization of their expression and immunological properties. The two antigen formulations consist of DNA encoding the full-length envelope protein (p/YFE) or the full-length envelope protein fused to the lysosomal-associated membrane protein signal, LAMP-1 (pL/YFE), aimed at diverting antigen processing/presentation through the major histocompatibility complex II precursor compartments. The immune responses triggered by these formulations were evaluated in H2b and H2d backgrounds, corresponding to the C57Bl/6 and BALB/c mice strains, respectively. Both DNA constructs were able to induce very strong T-cell responses of similar magnitude against almost all epitopes that are also generated by the YF 17DD vaccine. The pL/YFE formulation performed best overall. In addition to the T-cell response, it was also able to stimulate high titers of anti-YF neutralizing antibodies comparable to the levels elicited by the 17DD vaccine. More importantly, the pL/YFE vaccine conferred 100% protection against the YF virus in intracerebrally challenged mice. These results indicate that pL/YFE DNA is an excellent vaccine candidate and should be considered for further developmental studies. Author Summary: DNA and other nucleic acid vaccine technologies are advancing quickly, and new potent delivery methods are demonstrating great potential in human clinical trials. In this manuscript, we report a highly protective DNA vaccine against the yellow fever virus. This vaccine was engineered with a molecular adjuvant technology to enhance the exposure of the vaccine antigens to the immune system, resulting in augmented CD4+ helper responses. We postulate that the robust CD4+ responses help the B cells and the CD8+ cells mature more efficiently and produce better antibodies and cytotoxic cells, respectively. Our results show that vaccination with this yellow fever DNA formulation elicited protective levels of neutralizing antibodies and very strong cellular responses at similar levels to the responses elicited by the live attenuated 17DD vaccine. In addition, these results also suggest a very important role for cellular responses in mediating protection against yellow fever virus. We have previously demonstrated that a DNA vaccine encoding HIV-p55gag in association with the lysosomal associated membrane protein-1 (LAMP-1) elicited a greater Gag-specific immune response, in comparison to a DNA encoding the native gag. In vitro studies have also demonstrated that LAMP/Gag was highly expressed and was present in MHCII containing compartments in transfected cells. In this study, the mechanisms involved in these processes and the relative contributions of the increased expression and altered traffic for the enhanced immune response were addressed. Cells transfected with plasmid DNA constructs containing p55gag attached to truncated sequences of LAMP-1 showed that the increased expression of gag mRNA required p55gag in frame with at least 741 bp of the LAMP-1 luminal domain. LAMP luminal domain also showed to be essential for Gag traffic through lysosomes and, in this case, the whole sequence was required. Further analysis of the trafficking pathway of the intact LAMP/Gag chimera demonstrated that it was secreted, at least in part, associated with exosome-like vesicles. Immunization of mice with LAMP/gag chimeric plasmids demonstrated that high expression level alone can induce a substantial transient antibody response, but targeting of the antigen to the endolysosomal/secretory pathways was required for establishment of cellular and memory response. The intact LAMP/gag construct induced polyfunctional CD4
+ T cell response, which presence at the time of immunization was required for CD8+ T cell priming. LAMP-mediated targeting to endolysosomal/secretory pathway is an important new mechanistic element in LAMP-mediated enhanced immunity with applications to the development of novel anti-HIV vaccines and to general vaccinology field. Infants born to HIV-infected mothers are at high risk of becoming infected during gestation or the breastfeeding period. A search is thus warranted for vaccine formulations that will prevent mother-to-child HIV transmission. The LAMP/gag DNA chimeric vaccine encodes the HIV-1 p55gag fused to the lysosome-associated membrane protein-1 (LAMP-1) and has been shown to enhance anti-Gag antibody (Ab) and cellular immune responses in adult and neonatal mice; such a vaccine represents a new concept in antigen presentation. In this study, we evaluated the effect of LAMP/gag DNA immunization on neonates either before conception or during pregnancy. LAMP/gag immunization of BALB/c mice before conception by the intradermal route led to the transfer of anti-Gag IgG1 Ab through the placenta and via breastfeeding. Furthermore, there were an increased percentage of CD4+CD25+Foxp3+T cells in the spleens of neonates. When offspring were immunized with LAMP/gag DNA, the anti-Gag Ab response and the Gag-specific IFN-c-secreting cells were decreased. Inhibition of anti- Gag Ab production and cellular responses were not observed six months after immunization, indicating that maternal immunization did not interfere with the long-lasting memory response in offspring. Injection of purified IgG in conjunction with LAMP/gag DNA immunization decreased humoral and cytotoxic T-cell responses. LAMP/gag DNA immunization by intradermal injection prior to conception promoted the transfer of Ab, leading to a diminished response to Gag without interfering with the development of anti-Gag T- and B-cell memory. Finally, we assessed responses after one intravenous injection of LAMP/gag DNA during the last five days of pregnancy. The intravenous injection led to in utero immunization. In conclusion, DNA vaccine enconding LAMP-1 with Gag and other HIV-1 antigens should be considered in the development of a protective vaccine for the maternal/fetal and newborn periods. -Epitopes from all available full-length sequences of yellow fever virus (YFV) and dengue fever virus (DENV) restricted by Human Leukocyte Antigen class I (HLA-I) alleles covering 12 HLA-I supertypes were predicted using the NetCTL algorithm. A subset of 179 predicted YFV and 158 predicted DENV epitopes were selected using the EpiSelect algorithm to allow for optimal coverage of viral strains. The selected predicted epitopes were synthesized and approximately 75% were found to bind the predicted restricting HLA molecule with an affinity, K
D , stronger than 500 nM. The immunogenicity of 25 HLAA*02:01, 28 HLA-A*24:02 and 28 HLA-B*07:02 binding peptides was tested in three HLA-transgenic mice models and led to the identification of 17 HLA-A*02:01, 4 HLA-A*2402 and 4 HLA-B*07:02 immunogenic peptides. The immunogenic peptides bound HLA significantly stronger than the non-immunogenic peptides. All except one of the immunogenic peptides had KD below 100 nM and the peptides with KD below 5 nM were more likely to be immunogenic. In addition, all the immunogenic peptides that were identified as having a high functional avidity had KD below 20 nM. A*02:01 transgenic mice were also inoculated twice with the 17DD YFV vaccine strain. Three of the YFV A*02:01 restricted peptides activated T-cells from the infected mice in vitro. All three peptides that elicited responses had an HLA binding affinity of 2 nM or less. The results indicate the importance of the strength of HLA binding in shaping the immune response. Background: Steatocystoma simplex is an uncommon skin lesion with a histological pattern that is identical to that of steatocystoma multiplex. We are reporting this case of steatocystoma simplex for its uncommon location in the penile foreskin, and its occurrence in a Wapishana man.Case Presentation: A 56-year-old man of Wapishana ethnicity presented with complaints of referred penile discomfort and pain during sexual intercourse for 5 years. A physical examination revealed a mobile, compressible subcutaneous non-tender mass of 4 cm diameter located on the left-side of his penile foreskin. There were no signs of inflammation, no grip on the penile shaft, and no urethral discharge or enlargement of lymph nodes. We found no evidence of other cysts on cutaneous examination. We performed classical excision of the lesion under local anesthesia and confirmed the diagnosis of steatocystoma with the pathological report. As there were no complications, we discharged him the same day.Conclusion: Steatocystoma can be considered a differential diagnosis for cystic lesions on and around the penis.
