Abstract

Despite the importance of both members of the adult patient-care partner dyad, a majority of research on illness management is focused on the patient or the care partner. The basic principle of the Theory of Dyadic Illness Management is that illness management is a dyadic phenomenon; the theory focuses extensively on the dyad as an interdependent team. The way dyads appraise illness as a unit influences the ways in which they engage in behaviors to manage illness together in a recursive fashion that influences dyadic health. Optimizing the health of both members of the dyad is a goal of the theory. In turn, the health of the dyad can feedback to influence how they appraise and manage illness together. Finally, dyadic illness management is an inherently variable process that is influenced by several contextual factors. Supportive evidence and implications for practice and future research are presented.

11. Feasibility and Acceptability of a Remotely Delivered, Web-Based Behavioral Intervention for Men With Prostate Cancer: Four-Arm Randomized Controlled Pilot Trial

Abstract

BackgroundDiet and exercise may be associated with quality of life and survival in men with prostate cancer. ObjectiveThis study aimed to determine the feasibility and acceptability of a remotely delivered web-based behavioral intervention among men with prostate cancer. MethodsWe conducted a multi-site 4-arm pilot randomized controlled trial of a 3-month intervention (TrueNTH Community of Wellness). Eligibility included self-reported prostate cancer diagnosis, having a personal device that connected to the internet, age ≥18 years, and ability to read English and receive text messages and emails. Men receiving chemotherapy or radiation, or those who reported contraindications to exercise, could participate with physician clearance. Participants were randomized (1:1:1:1) to additive intervention levels: website; website and personalized diet and exercise prescription; website, personalized prescription, Fitbit, and text messages; and website, personalized prescription, Fitbit, text messages, and 2 30-minute phone calls—one with an exercise trainer and one with a registered dietician. Primary outcomes were feasibility (accrual and attrition) and acceptability (survey data and website use). We described self-reported diet and exercise behavior at the time of enrollment, 3 months, and 6 months as secondary outcomes. ResultsIn total, 202 men consented and were randomized between August 2017 and September 2018 (level 1: 49, level 2: 51, level 3: 50, level 4: 52). A total of 160 men completed the onboarding process and were exposed to their randomly assigned intervention (38, 38, 42, and 42 in levels 1, 2, 3, and 4, respectively). The follow-up rate was 82.7% (167/202) at 3 months and 77.2% (156/202) at 6 months. Participants had a median age of 70 years and were primarily White and college educated. Website visit frequency over the 3-month intervention period increased across levels (median: 2, 9, 11, and 16 visits for levels 1, 2, 3, and 4, respectively). Most were satisfied or very satisfied with the intervention (20/39, 51%; 27/42, 64%; 23/44, 52%; and 27/42, 64% for levels 1, 2, 3, and 4, respectively). The percentage of men who reported being very satisfied was highest among level 4 participants (10/42, 24% vs 4/39, 10%; 5/42, 12%; and 5/44, 11% for levels 1, 2, and 3, respectively). Dissatisfaction was highest in level 1 (5/39, 13% vs 1/42, 2%; 3/44, 7%; and 2/42, 5% for levels 2, 3, and 4, respectively). We observed small improvements in diet and physical activity at 3 months among men in level 4 versus those in level 1. ConclusionsA web-based, remotely delivered, tailored behavioral intervention for men with prostate cancer is feasible. Future studies are warranted to increase the effect of the intervention on patient behavior while maintaining sustainability and scalability as well as to design and implement interventions for more diverse populations. 22. Longitudinal Dyad Models in Family Research

Abstract

Multilevel modeling allows for the simultaneous analysis of data gathered at more than 1 unit of analysis (e.g., children nested in schools). It is often used to examine the effects of various contexts on individual differences in change. This paper promotes the application of multilevel models to longitudinal dyadic data in family research. By focusing on the dyad as context, researchers can examine within-dyad change and begin to understand the interactive processes that constitute the relationship between partners. They can then frame questions about interdyad differences in within-dyad change. We present several longitudinal models that researchers can use to examine the pattern of change within dyads, assess heterogeneity in change across dyads, and investigate cross-partner effects on change. We comment on the implications of these models for family research.

35. Shared decision- making in dementia: A review of patient and family carer involvement

Abstract

This paper reviews empirical findings concerning the decision-making process of persons with dementia and their family carers, with a particular focus on the extent and determinants of involvement of persons with dementia in the decision-making process. To be included in this review, studies needed to be published in peer-reviewed journals between 1999 and 2014, report empirical data from participants with dementia and/or their family carers, and pertain to the involvement of persons with dementia and their family carers in decisions about everyday care, medical care and treatment, or long-term care. A total of 36 studies were included. Results indicated that not all persons with dementia are excluded from participating in the decision-making process, but there is a broad spectrum of what constitutes shared decision-making in dementia. Studies concerning the determinants of shared decision-making mostly focused on non-modifiable factors. Future research is needed to better promote shared decision-making among persons with dementia and their family carers.

Abstract

The enactment of the Affordable Care Act in 2010 prompted the Institute of Medicine to release The Future of Nursing report to help guide the necessary changes in nursing to accommodate millions of newly insured individuals. A key point of the campaign was for advanced practice registered nurses (APRNs) to practice at the fullest extent of their licensure to meet the needs of the newly insured population. Certified Registered Nurse Anesthetists (CRNAs) experience the most restricted scope of practice among APRNs. An integrative review was conducted to examine CRNA practice and policy since The Future of Nursing was released. Ten research articles were included in the final review. Key findings related to CRNA practice and policy that emerged from the review were (1) patient complications and mortality rates are no different under the care of CRNAs compared with anesthesiologists; (2) CRNAs are more accessible to vulnerable populations and rural areas; and (3) state legislators are being influenced by factors other than evidence, such as strong professional group influence, to make policy decisions for CRNAs. Future interdisciplinary research investigating outcomes from the patients' perspectives could help remove bias and strengthen the evidence of the quality of care given by CRNAs., Competing Interests: Name: Caitlin M. Abstract

Purpose: The accumulation of shed plasma membrane vesicles in the peripheral circulation is unique to cancer. Because these membrane fragments (MFs) express biologically active components, such as Fas ligand (FasL), the objective of this study was to define the link between the presence of shed membrane vesicles, apoptosis, and suppression of T-cell receptor/CD3-zeta expression in T lymphocytes of patients with ovarian cancer., Experimental Design: MF shedding was measured chromatographically in sera from women with ovarian cancer (n = 11) and, as controls, non-cancer-bearing females (n = 9) and women with benign ovarian disease (n = 4). FasL associated with these shed fragments was assayed by Western immunoblots, whereas HLA class I expression was defined by slot-blotting. The effect of shed MFs on CD3-zeta expression was evaluated using a T-cell bioassay, and apoptosis of circulating T cells was measured by a cell-death ELISA and electrophoretic analysis of caspase-3., Results: MFs were undetectable in control sera, and their levels were significantly elevated in sera from women with ovarian cancer. These tumor-derived MFs expressed 41-kDa FasL and HLA class I antigens. In co-incubation experiments, dose-dependent suppression of T-cell receptor/CD3-zeta expression by MFs was observed. Decreases in zeta expression correlated with the level of FasL in MFs but not with the level of HLA. The suppression of CD3-zeta by MFs appeared to be linked to the induction of apoptosis and caspase-3 within T cells., Conclusion: Our results suggest that FasL associated with tumor-derived MFs is responsible for apoptosis of T lymphocytes and a concomitant loss of zeta-chain expression in patients with ovarian carcinoma.
