Abstract

Compressed sensing of magnetic resonance imaging refers to the reconstruction of magnetic resonance images from partially sampled k-space data. The k-space data reduces reconstruction processing time but at the cost of increasing artifacts - especially with the higher reduction factor of the raw data. This work proposes a segmented region-based reconstruction technique to reduce image artifacts with enhanced quality and high temporal resolution. The proposed method segments partially sampled k-space data in two segments according to their frequencies. Lower frequency components at the central region are selected and predicted using nuclear norm minimization. This part and the peripheral part of the k-space components at higher frequencies are merged. The recovery technique iterates to reconstruct more accurate images in terms of conventional compressed sensing techniques. The performance of the proposed method is evaluated and compared with compressed sensing, two-stage compressed sensing, and modified total variation technique. Better results in term of normalized mean square error NMSE, reconstruction time and structural similarity index measure SSIM show the effectiveness of the proposed method with a high reduction factor of data.

Abstract

In this paper, two well-known topologies of proposed Double Sided Field Excited Linear Flux Switching Machine with Segmented Secondary (FELFSMSS) i.e. Dual Stator and Dual Mover are quantitatively compared with a valid decision of selecting Dual Stator FELFSMSS (DSFELFSMSS) based on low numerical values of detent force, thrust force ripples, and normal force, and high values of open circuit flux linkage, average thrust force, and thrust force density. In order to uplift overall thrust force profile of selected DSFELFSMSS at the rate of minimum thrust force ripple ratio, geometry based deterministic optimization and appropriate selection of AC and DC current density approach is adopted. Finally, a novel technique of auxiliary end tooth DC winding is applied to limit thrust force ripple ratio less than 10 percent.

Abstract

Image processing systems based on neighborhood operations i.e. Neighborhood Processing Systems (NPSs) are computationally expensive and memory intensive. Field Programmable Gate Array (FPGA) based parallel processing architectures accelerate calculations of NPS provided if they have fast external-memory data access by using on-chip data buffers. The conventional data buffers namely full Row Buffers (RBs) implemented with FPGA embedded memory resources i.e. Block RAMs (BRAMs) are resource inefficient. It makes overall NPS implementation on FPGA expensive and infeasible especially for resource-constraint environment. This paper presents compact and efficient image buffering architecture with an additional feature of pre-fetching. Proposed design fits in minimal BRAMs by using small yet efficient Main Control Unit (MCU). Its optimal multi-rated BRAM data accessing technique reduces BRAM cost to provide multiple pixels of pre-fetched data/clock to NPS in a fixed pattern. It controls and synchronizes BRAMs operations to attain throughput of 1 clock/pixel. Thus our buffer architecture with 66% reduction in BRAM requirement as compared to conventional RBs is capable to support buffering for real time systems with high resolution (1080x1920@62fps). Therefore proposed buffer architecture can suitably replace conventional RB in any real time NPS application.

The etiology of MALS and its existence have been debated and questioned. We aimed to identify plasma biomarkers indicating mesenteric ischemia in patients with CMI and MALS.Methods: Plasma α-glutathione S-transferase (α-GST), intestinal fatty acid-binding protein (I-FABP), citrulline, and ischemia modified albumin (IMA) were analyzed in fifty-eight patients with CMI (Group A, n=44) and MALS (Group B, n=14) before and after revascularization. The plasma levels of these potential biomarkers were compared with those of healthy individuals (Group C, n=16). Group comparison was performed with the Mann–Whitney U-test. Cross-tabulation and its derivatives were obtained. Receiver operating characteristic (ROC) curves and area under the curve (AUC) were calculated.Results: Plasma levels of α-GST were significantly raised in the patients with CMI (7.8 ng/mL, p< 0.001) and MALS (8.4 ng/mL, p< 0.001), as compared with the control Group C (3.3 ng/mL). The threshold for normal median plasma α-GST levels of 4 ng/mL yielded a sensitivity of 93% and 86%, specificity of 86% and 88%, respectively, for the diagnosis of CMI due to atherosclerosis and MALS. AUC of ROC curves was 0.96 (p< 0.0001) for CMI and 0.85 (p< 0.002) for MALS. The patient groups did not differ from the healthy controls in any other biomarkers.Conclusion: Plasma α-GST levels are elevated in CMI and MALS patients. We aimed to follow up the results of the laparoscopic decompression of the patients with MALS.Patients and Methods: In a single-center, 52 consecutive MALS patients were followed-up, prospectively, after transperitoneal laparoscopic decompression. MALS was diagnosed with a computed tomography angiography (CTA) verified stenosis, ≥ 50% of the celiac artery (CA), and with duplex ultrasound, a peak systolic velocity (PSV) ≥ 2.0 m/s. Postoperative, CTA, and duplex ultrasound were performed, and the patients were followed-up at 3, 6, 12 months, and yearly after that.Results: Mean age of the patients was 47 ± 21 years, and 65% were females. The patients had a mean weight loss of 8.4 ± 7.2 kg. Fifty-one patients had the laparoscopic operation with a mean operation time of 102 ± 28 minutes. Forty-seven patients (90%) achieved relief from the symptoms either completely (67%) or partially (23%) at 3– 6 months of follow-up. Significant improvement in postoperative PSV was found compared to the preoperative values, p< 0.001. Five patients (10%) with no immediate effect of the operation, but two of them became free from symptoms during the mean study follow-up of 2.4 ± 2 years. Five patients (10%) had operative complications, including one trocar injury to the liver, one pneumothorax, and three cases of bleeding from the branches of CA. Two patients died of cancer disease during the study period. Only two patients (4%) had symptoms relapse, both later treated successfully.Conclusion: Laparoscopic transperitoneal decompression provides most of the patients a persistent relief from MALS symptoms.Keywords: median arcuate ligament syndrome, postprandial pain, laparoscopy, mesenteric ischemia

Abstract

Although modern clinical management strategies have improved the outcome of paediatric patients with severe autoimmune and inflammatory diseases over recent decades, a proportion will experience ongoing or recurrent/relapsing disease activity despite multiple therapies often leading to irreversible organ damage, and compromised quality of life, growth/development and long-term survival. Autologous and allogeneic haematopoietic stem cell transplantation (HSCT) have been used successfully to induce disease control and often apparent cure of severe treatment-refractory autoimmune diseases (ADs) in children. However, transplant-related outcomes are disease-dependent and long-term outcome data are limited in respect to efficacy and safety. Moreover, balancing risks of HSCT against AD prognosis with continually evolving non-transplant options is challenging. This review appraises published literature on HSCT strategies and outcomes in individual paediatric ADs. We also provide a summary of the European Society for Blood and Marrow Transplantation (EBMT) Registry, where 343 HSCT procedures (176 autologous and 167 allogeneic) have been reported in 326 children (<18 years) for a range of AD indications. HSCT is a promising treatment modality, with potential long-term disease control or cure, but therapy-related morbidity and mortality need to be reduced. Further research is warranted to establish the position of HSCT in paediatric ADs via registries and prospective clinical studies to support evidence-based interspeciality guidelines and recommendations.

Abstract

The Unani system of medicine (USM) is one of the popular traditional systems of medicine. World widely, this system of medicine becomes an important health care system. Since many years back Unani medicines were used by the Unani physicians for treating the diseases ailments. In USM most of the drugs are of plant origin and their therapeutic effects are due to the presence of active phytochemicals. The proper standardization methods must be necessary for preparation of formulations otherwise secondary metabolites or active constituents might not be up to the quality. The standard operating procedures (SOP's) are adopted for the preparation of Unani formulation. This study evaluated the quality of polyherbal unani formulation Habb-i-Shahtara, which is used in the treatment of skin diseases. The standardization parameters which were used for the development of quality standard of the study formulation were pharmacognostical studies, physicochemical and phytochemical parameters, HPTLC analysis, microbial load, aflatoxins, heavy metals and pesticide residues determination to evaluate the pharmacopoeial standard. The polyherbal Unani formulation Habb-i-Shahtara have been successfully standardized and the data and procedures which were used may serve the guidelines and standard reference in future for the preparation of formulation and for further study. Abstract

Although modern clinical management strategies have improved the outcome of paediatric patients with severe autoimmune and inflammatory diseases over recent decades, a proportion will experience ongoing or recurrent/relapsing disease activity despite multiple therapies often leading to irreversible organ damage, and compromised quality of life, growth/development and long-term survival. Autologous and allogeneic haematopoietic stem cell transplantation (HSCT) have been used successfully to induce disease control and often apparent cure of severe treatment-refractory autoimmune diseases (ADs) in children. However, transplant-related outcomes are disease-dependent and long-term outcome data are limited in respect to efficacy and safety. Moreover, balancing risks of HSCT against AD prognosis with continually evolving non-transplant options is challenging. This review appraises published literature on HSCT strategies and outcomes in individual paediatric ADs. We also provide a summary of the European Society for Blood and Marrow Transplantation (EBMT) Registry, where 343 HSCT procedures (176 autologous and 167 allogeneic) have been reported in 326 children (<18 years) for a range of AD indications. HSCT is a promising treatment modality, with potential long-term disease control or cure, but therapy-related morbidity and mortality need to be reduced. Further research is warranted to establish the position of HSCT in paediatric ADs via registries and prospective clinical studies to support evidence-based interspeciality guidelines and recommendations.

Abstract

Coronavirus disease-19 (COVID-19), caused by Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2), represents one of the biggest challenges of 21st century, threatening public health around the globe. Increasing age and presence of co-morbidities are reported risk factors for severe disease and mortality, along with autoimmune diseases (ADs) and immunosuppressive treatments such as haematopoietic stem cell transplantation (HSCT), which are also associated with adverse outcomes. We review the impact of the pandemic on specific groups of patients with neurological, rheumatological, and gastroenterological indications, along with the challenges delivering HSCT in adult and pediatric populations. Moving forward, we developed consensus-based guidelines and recommendations for best practice and quality of patient care in order to support clinicians, scientists, and their multidisciplinary teams, as well as patients and their carers. These guidelines aim to support national and international organizations related to autoimmune diseases and local clinical teams delivering HSCT. Areas of unmet need and future research questions are also highlighted. The waves of the COVID-19 pandemic are predicted to be followed by an “endemic” phase and therefore an ongoing risk within a “new normality”. These recommendations reflect currently available evidence, coupled with expert opinion, and will be revised according to necessary modifications in practice.

Abstract

Excessive fuel demand thrusts the Pakistani government to import large volumes of fuel from foreign sources, creating adverse effects on the country's economy. Therefore, exploring an alternative to fossil fuels is unavoidable. The option of environmentally friendly fuel like biodiesel produced from indigenous waste is an additional bonus for the populous developing country like Pakistan where likelihood of waste generation is huge. There exists a potential option for sustainable biodiesel production utilizing excessive waste cooking oil available in the country which otherwise is an ecological burden. The present work is focused to sturdily vindicate the appropriateness of waste cooking oil-based biodiesel generation and utilization in Pakistan through SWOT-AHP, TOWS and PESTLE analysis. The prioritization of SWOT through AHP in view of experts’ perception displayed the strengths and opportunities in highest group priority values (Strengths: 0.51, Opportunities: 0.29). Furthermore, TOWS analysis suggests promising strategies for the sustainable implementation of commercial aspect of waste oil-based biodiesel in Pakistan. Political, Economic, Social, Technological, Legal and Environmental (PESTLE) analysis favors the strengths and opportunities factors of SWOT and TOWS strategies for the application of waste cooking oil based biodiesel in country. At the end, regional recommendations have been provided for the implementation of biodiesel production scenario in country.

Abstract

These updated EBMT guidelines review the clinical evidence, registry activity and mechanisms of action of haematopoietic stem cell transplantation (HSCT) in multiple sclerosis (MS) and other immune-mediated neurological diseases and provide recommendations for patient selection, transplant technique, follow-up and future development. The major focus is on autologous HSCT (aHSCT), used in MS for over two decades and currently the fastest growing indication for this treatment in Europe, with increasing evidence to support its use in highly active relapsing remitting MS failing to respond to disease modifying therapies. aHSCT may have a potential role in the treatment of the progressive forms of MS with a significant inflammatory component and other immune-mediated neurological diseases, including chronic inflammatory demyelinating polyneuropathy, neuromyelitis optica, myasthenia gravis and stiff person syndrome. Allogeneic HSCT should only be considered where potential risks are justified. Compared with other immunomodulatory treatments, HSCT is associated with greater short-term risks and requires close interspeciality collaboration between transplant physicians and neurologists with a special interest in these neurological conditions before, during and after treatment in accredited HSCT centres. Other experimental cell therapies are developmental for these diseases and patients should only be treated on clinical trials.

Abstract

International audience; Over the last 20 years, haematopoietic stem cell transplantation (HSCT) has been used to treat patients with severe autoimmune and inflammatory diseases whose response to standard treatment options has been limited, resulting in a poor long-term prognosis in terms of survival or disability. The vast majority of patients have received autologous HSCT where an increasing evidence-base supports its use in a wide range of autoimmune diseases, particularly relapsing remitting MS, systemic sclerosis and Crohn’s disease. Compared with standard treatments for autoimmune diseases, HSCT is associated with greater short-term risks, including a risk of treatment-related mortality and long-term complications. There is a need for a careful appraisal of potential benefits and risks by disease and transplant specialists working closely together with patients and carers to determine individual suitability for HSCT. HSCT should be conducted in accredited transplant centres with robust arrangements for long-term follow-up with both disease and transplant specialists. The aim of this open-access position statement is to provide plainly worded guidance for patients and non-specialist clinicians considering HSCT for an autoimmune disease, especially when treatment abroad is being considered. Recent technical publications in the field have been referenced to support the statement and provide more detail for clinicians advising patients.

The ASTIC trial of AHSCT failed to meet its stringent composite primary endpoint, but 1-year follow-up data demonstrate sustained efficacy. Jamun (Syzygium cumini Linn) is one of the medicinal plants used for therapeutic purposes in Dhayābīus ārr (diabetes mellitus), Ishāl-e- Damwī (haemorrhagic diarrhoea), Is'haal-i-Safrāwī (bilious diarrhoea), ?u'f al-Ishtihā'(loss of appetite), Za?īr (dysentery) and Qulā (oral ulcer). In recent past its fruits, seeds, leaves, stem bark and its secondary metabolites have shown medicinal properties in various experimental and clinical studies. In this review we have tried to explore its ethnobotanical uses and pharmacological actions described in classical literature and scientific publications based on experimental studies. It is found that this plant plays an important role in prevention and management of non-communicable diseases such as Dhayābī?us ?ārr (diabetes mellitus), Sar?ān (cancer), Niqris (gout), ischaemic heart disease etc. Several preclinical studies have revealed that it has Mu?allil-i- Awrām (anti-inflammatory), Mu?āfi?-i- Qalb (cardio protective), Dafi'-i-?ummā (anti-pyretic) and Muqawwī-i-Jigar (hepatotonic) properties. Abstract

Nar Farsi (Eczema) is a very common skin inflammatory disorder. Its prevalence varies according to age groups and country to country. The Unani system of medicine offers its successful management. In this study, efficacy and safety of a topical Unani formulation was assessed and compared with a standard drug. A prospective, non inferiority, randomized, active controlled and parallel group study was designed to study 60 participants of eczema (30 participants in each group). The Unani formulation Marham Safeda Kafoori was studied as a test drug for 6 weeks versus allopathic medicine Clobetasol Propionate in the study. The study findings suggested that the Unani formulation was non inferior to Clobetasol Propionate in terms of percentage of reduction in itching, oozing, exudation, erythema, lichenification and cracking at 6 weeks of treatment. It was observed that the difference of the mean±SD of SASSAD Score was measured as 12.83±4.76 and 14.13±5.27 in test and control groups respectively. The result was analysed as per protocol and it was found that the efficacy of the Unani formulation was statistically significant (p<0.001). Abstract

Diabetes mellitus type 2 (Zayābīṭus Sukkari Qism Sāni) is a major health concern in 21st century. Despite tremendous advances in modern sciences, there is a lack of relatively safe and effective drug for its management. The primary objective of this study was to evaluate the efficacy and safety of a polyherbal Unani formulation containing Gurmar booti (Gymnema sylvestre), Gilo (Tinospora cordifolia) and Jamun (Syzygium cumini) in the management of Diabetes Mellitus Type 2 (DMT2). It was a randomised controlled clinical study conducted on 60 participants of DMT2 inadequately controlled by diet and exercise. The test drug was given to group-A participants (n=30) 6 g twice daily orally for 12 weeks and the standard drug metformin (500 mg) was given twice daily orally to group-B participants (n=30). It was observed that the difference between the Mean (± SD) value of fasting blood glucose (FBG), postprandial blood glucose (PPBG) and glycosylated haemoglobin (HbA1c) in Test and Control groups at the end of the study in comparison to baseline was significant (p<0.05). Abstract

Over the past 15 years, SCT has emerged as a promising treatment option for patients with severe autoimmune diseases (ADs). Mechanistic studies recently provided the proof-of-concept that restoration of immunological tolerance can be achieved by haematopoietic SCT in chronic autoimmunity through eradication of the pathologic, immunologic memory and profound reconfiguration of the immune system, that is, immune ‘resetting’. Nevertheless, a number of areas remain unresolved and warrant further investigation to refine our understanding of the underlying mechanisms of action and to optimize clinical SCT protocols. Due to the low number of patients transplanted in each centre, it is essential to adequately collect and analyse biological samples in a larger cohort of patients under standardized conditions. The European society for blood and marrow transplantation Autoimmune Diseases and Immunobiology Working Parties have, therefore, undertaken a joint initiative to develop and implement guidelines for ‘good laboratory practice’ in relation to procurement, processing, storage and analysis of biological specimens for immune reconstitution studies in AD patients before, during and after SCT. The aim of this document is to provide practical recommendations for biobanking of samples and laboratory immune monitoring in patients with ADs undergoing SCT, both for routine supportive care purposes and investigational studies.

There is currently a lack of evidence regarding the self-reported concerns of SCT patients before and after SCT.Aim and design: This exploratory study performed a secondary analysis of self-reported, written concerns of SCT patients before and after transplant to determine patients' concerns.Methods: Content analysis of text box entries of SCT patients collected between 2005 and 2007 at the Seattle Cancer Care Alliance. Text box entries were collected as part of symptom assessment using the Electronic Self-Report Assessment – Cancer instrument. The assessment was presented to 137 patients undergoing SCT at two time points: prior to ambulatory visits before any therapy had begun (T1) and at the first visit after hospital discharge following SCT (T2).Results: Text box entries were made before (n = 52) and after (n = 87) the transplant, resulting in 139 text box entries made by 137 patients representing 133 concerns. Using content analysis, the entries were categorized and ranked according to frequency. After symptom concerns, patients ranked work and financial issues the most frequent concerns prior to SCT. After SCT, symptoms remained the most frequently entered area of concern, followed by survival.Conclusion: Oncology providers need to assess SCT patients for work and financial concerns before and after transplant. Appropriate and timely referrals may ease the burden of these concerns for patients. Thus, assessment of financial and work concerns by the oncology team should be an integral part of quality health care for patients undergoing SCT.Keywords: self-report, electronic self-report assessment – cancer, cancer, patient concerns

43. Trends and challenges in mango nursery production in Pakistan

Abstract

Pakistan is the fifth largest mango growing country in the world. Despite being second most important fruit to the national economy, mango nursery production remained an under-developed enterprise in the country. Districts of Multan, Rahim Yar Khan (Punjab) and Mir Pur Khas (Sindh) are the leading areas for mango production due to large sales volumes estimated collectively about 0.12 million plants per annum. Inarching was the most preferred method for clonal propagation until late seventies. However, introduction of polyethylene grafting tape promoted the more efficient and cost-effective method of T-grafting which was later replaced by side grafting. Despite advances in propagation, raising seedling rootstocks from seed continued under trees or around commercial mango orchards which encouraged the proliferation of diseases like malformation, anthracnose and mango sudden death syndrome early in the trees life. Recently, Australia-Pakistan Agriculture Sector Linkage Program (ASLP) funded by Australian Center for International Agriculture Research (ACIAR), highlighted the problem and began to establish and popularize the concept of modern disease free mango nurseries. Their efforts are duly supported by Punjab Fruit and Vegetable Development Project (F and VDP). Work has been undertaken on development of potting mixtures using the indigenous low-cost ingredients and "Mango Germplasm Units" for provision of sanitized and true-to-type bud wood are being established. These efforts will provide basis for the nursery certification program launched by Federal Seed Certification and Registration Department. A few commercial nursery operators have taken the cleaner nurseries initiatives that show the beginning of a new era of future mango nurseries in the country. © ISHS 2013.

01 mei 2012, Item does not contain fulltext, T-prolymphocytic leukemia (T-PLL) has a very poor prognosis with conventional immunochemotherapy. Incidental reports suggest that allogeneic hematopoietic stem cell transplantation (allo-HSCT) might have a role in this disease. Therefore, the purpose of the present study was to analyze the outcome of transplants for T-PLL registered with the European Group for Blood and Marrow Transplantation database and the Royal Marsden Consortium. Eligible were 41 patients with a median age of 51 (24-71) years; median time from diagnosis to treatment was 12 months, and in complete remission (CR) (11), partial remission (PR) (12), stable or progressive disease (13) and unknown in 5 patients. A total of 13 patients (31%) received reduced-intensity conditioning. Donors were HLA-identical siblings in 21 patients, matched unrelated donors in 20 patients. With a median follow-up of surviving patients of 36 months, 3-year relapse-free survival (RFS) and OS was 19% (95% CI, 6-31%) and 21% (95% CI, 7-34%), respectively. Multivariate analysis identified TBI and a short interval between diagnosis and HSCT as factors associated with favorable RFS. Three-year non relapse mortality and relapse incidence were each 41% with the majority of relapses occurring within the first year. These data indicate that allo-HSCT may provide effective disease control in selected patients with T-PLL.
