10. Accuracy Rate of Shear Wave Elastography in Detecting the Liver Fibrosis in Overweight and Obese Children with Hepatosteatosis.

Objective: The aim of this study was to compare the accuracy rate of liver stiffness calculated by shear wave elastography with liver biopsy results in obese and overweight children. Materials and Methods: Obese and overweight children between 3 and 18 years of age, who had hepatic steatosis and a healthy control group were included in this study. A blood sample was obtained for laboratory tests and shear wave elastography was performed for all subjects. Liver biopsies were performed only in patients with hepatosteatosis, providing permission for biopsy, and for whom the biopsy procedure was not contraindicated. Results: A cohort of 142 children (78 overweight/obese and 64 healthy) was included in this study. Shear wave elastography values were significantly higher in the patient group as compared to the control group (34.0 vs. 8.2 kPa; P < .001). Obese children had higher elastography values compared to non-obese children (50.2 vs. 23.7 kPa, P < .001). No correlation was detected between fibrosis score and elastography values. Elastography increased with increasing weight (correlation coefficient: 0.334, P = .003) and body mass index (correlation coefficient: 0.364, P = .001). Conclusion: In obese and overweight patients, elastography values are higher than in healthy subjects as well as patients with liver fibrosis. Disease-specific cut-off, mean, and normal reference range values should be defined with large-scale studies to improve interpretation of elastography values. Our results are contradictory in the determination of liver fibrosis with shear wave elastography in obese and overweight patients, thus further research with a larger patient population is recommended. 11. Adenoid Cystic Carcinoma of the Breast: Multimodality Imaging Findings and Review of the Literature.

- Abstract
Rationale and Objectives: Adenoid cystic carcinoma (ACC) of the breast is a rare type of breast cancer with favorable prognosis. There is limited data on the radiological findings of this rare tumor in literature. The aim of this study is to determine the most common imaging features and review the literature.Materials and Methods: Pathological databases of seven institutions from 2009 to 2021 were retrospectively reviewed, and patients with a diagnosis of ACC of the breast were determined. Thirteen patients whose imaging studies could be recalled from the picture archiving systems (PACS) were included in the study. Clinical and pathological findings as well as follow-up data were recorded. Radiological findings were analyzed and categorized based on BI-RADS 5th edition.Results: There were 16 mass lesions in 13 patients (two multifocal cases, one case with recurrence). Mammography demonstrated 14 masses, while ultrasound (US) demonstrated all. MRI was available in only seven cases, with eight masses. The most common findings were round or oval shape on all modalities (78.57%-93.75%). Other frequent findings were parallel orientation (81.25%), isoechoic or hyperechoic echogenicity (62.5%), high T2 signal (87.5%), restricted diffusion (71.43%), and homogeneous enhancement (62.5%). Mammography, US and MRI showed circumscribed margins resembling a benign lesion in 35.71%, 37.5% and 50% of the lesions respectively. Three patients had a cyst-like echogenicty on US. Half of the lesions were avascular on Doppler US (6/12) and half were soft (2/4) on strain elastography. Although there were benign features on all imaging modalities seperately, all lesions could be categorized as BI-RADS 4 or 5 when the findings were combined. However 9/16 masses were BI-RADS 4A, emphasizing the subtlety of the malignant features.Conclusion: ACC of the breast can present with findings resembling a benign lesion on different imaging modalities. Although combination of all imaging findings correctly indicated the suspicious nature of the lesions in all cases, final classification was BI-RADS 4A in most of them. Radiologists should be aware of the more frequent findings of ACC of the breast for early diagnosis. US findings of isoechoic or hyperechoic appearance, and cyst-like echogenicity have not been reported previously in literature. [ABSTRACT FROM AUTHOR]- Published

13. Comparing the Efficiency of Imaging Modalities in Detection of Recurrent Breast Cancer.

Objective: To investigate the effectiveness of the different imaging modalities in detecting recurrence in breast cancer follow-up. Materials and Methods: Sixty-four women with recurrent breast cancer were examined between January 2020 and July 2022. Recurrency was divided into four categories: local; regional; second primary; and distant metastasis. The detectability of recurrent lesions with mammography (MG), ultrasound (US) and magnetic resonance imaging (MRI), was evaluated. In addition, recurrences that firstly appeared by positron emission tomography (PET) scan were recorded. Results: Twenty-seven (42.2%) recurrences were local, 10 (15.6%) were regional and 27 (42.2%) were second primary. Forty-six (71.9%) of them were determined to have invasive carcinoma, 8 (12.5%) were ductal carcinoma in situ, and 10 (15.6%) were axillary metastases. Eight (12.5%) of them were first diagnosed by PET-computed tomography/MRI. Among the available images performed, 78.7% could be detected pathologically by MG, 95.2% by US, and 100% by MRI. Distant metastasis associated with other types of recurrence was detected in 6 (9.4%) cases. Conclusion: MRI is the most powerful imaging modality in detecting recurrent breast cancer. With the addition of US to routine MG follow-up, a higher rate and early detection of recurrent cancers can be achieved. [ABSTRACT FROM AUTHOR]

Amaç: Türkiye’nin doğusunda yaşayan hastaların radyolojik görüntüleme yöntemleri hakkındaki bilgi düzeyini belirlemek.Gereç ve Yöntem: Yerel hastane etik kurul onayının alınmasının ardından Mart-Haziran 2018 tarihleri arasında Radyoloji Bölümüne ultrasonografi (USG), bilgisayarlı tomografi (BT) ve manyetik rezonans görüntüleme (MRG) tetkiki için yönlendirilen 18-72 yaş aralığındaki 797 hastaya, radyolojik görüntüleme ile ilgili düzenlenen anket formları uygulandı. Elde edilen veriler SPSS 22.0 programında analiz edildi.Bulgular: Tanısal görüntüleme amacıyla radyoloji birimine başvuran ve anket formu uygulanan hasta sayıları USG, BT ve MRG için sırasıyla 332, 215 ve 250 idi. Hastalar arasında USG hakkındaki farkındalık, BT ve MRG’ye oranla daha fazla idi. Doğru ya da yanlış cevaplamaya etki eden en önemli prediktif faktörün eğitim düzeyi ve erkek cinsiyet olduğu görüldü (sırasıyla p değerleri p:0.001, p:0.024). BT çekilen hastaların %50’den fazlası incelemenin radyasyon içerdiğinin ve radyasyonun vücuda zararlı olduğunun bilincindeydi. BT ve MRG çekilen hastalarda en çok yanlış cevap verilen madde, incelemeyi yapacak kişinin bilinmemesi oldu.Sonuç: Çalışmamızda elde edilen sonuçlar ışığında insan sağlığı açısından az da olsa tehlike arz edebilecek herhangi bir radyolojik görüntüleme ya da tıbbi uygulamadan önce hastaların işlem hakkında bilgilendirilerek hasta düzeyinde görsel ya da işitsel eğitimlerle toplumsal düzeyde farkındalığın arttırılması gerektiğini düşünmekteyiz., Objective: To determine the level of knowledge about radiological imaging of patients in Eastern Turkey.Materials and Methods: After obtaining the local hospital ethics committee approval, between March and June 2018, 797 patients between the ages of 18-72 who were referred to the radiology department for ultrasonography (USG), computed tomography (CT) and magnetic resonance imaging (MRI) were included to the study. A questionnaire forms related to radiological imaging and personel information were applied. The obtained data were analyzed in SPSS 10.0 program.Results: The number of patients who applied to the radiology unit for diagnostic imaging was 332, 215 and 250 for USG, CT and MRI, respectively. The awareness to USG was higher among the patients compared to CT and MRI. It was observed that the most important predictive factor affecting correct or incorrect answers was education level and male gender (p: 0.001, p: 0.024, respectively). More than 50% of patients who underwent CT were aware that the examination included radiation and that radiation was harmful to the body. The most wrong answer in patients who underwent CT and MRI was not knowing the person who would perform the examination.Conclusion: In the light of the results obtained in our study, we think that patients should be informed about the procedure before any radiological imaging or medical application that may pose a danger to human health, and awareness should be increased at the patient level with continuous visual or auditory training.

16. Evaluation of the Appearance Characteristics of Suspicious Microcalcifications Detected by Ultrasonography

Objective: To investigate the imaging properties of mammographically suspicious microcalcifications (MC) that can be detected by ultrasonography (USG). Methods: Cases with suspected MC in categories 4 and 5 according to the Breast Imaging-Reporting and Data System (BI-RADS) in screening mammography between June 2019 and January 2021 were included in the study. The patients were scanned with USG. USG guided core needle biopsy was performed for those whose MC area could be detected by USG and evaluated histopathologically. Mammographic features of MCs [morphological type (fine pleomorphic, amorphic, coarse heterogeneous, fine linear pleomorphic branching)], distribution (clustered, regional, segmental, diffuse, linear) and BI-RADS (4A, 4B, 4C, 5) category was determined. Other findings accompanying MC areas on mammography and USG were recorded. Results: The mean age of 43 patients included in the study was 45.27 +/- 8.58 (30-84). 48.8% (n=21) of the calcifications were fine pleomorphic, 30% (n=13) amorphous, 16% (n=7) coarse heterogeneous, 4.6% (n=2) fine pleomorphic branching type. While 70% (n=30) of the patients had asymmetrical density accompanying MCs on mammography, parenchymal distortion was found in 12% (n=5); on USG, hypoechoic area was found in 58% (n=25) of the patients, irregular ductal ectasia in 19% (n=8), parenchymal distortion in 9% (n=4), and 10% (n=4) microcyst cluster were accompanying pathologies. As a result of the histopathological evaluation, 42% of the patients had benign histopathology (n=18), 16% had ductal carcinoma in situ (n=7), and 42% (n=18) had invasive ductal cancer. Conclusion: In our study, the distinctive features of BI-RADS 4 and 5 MCs that can be followed by USG were evaluated. Although MCs that can be detected on USG are more likely to be malignant than benign ones, fine pleomorphic MC areas can be observed most frequently. Knowing the features of mammographic suspected MCs on USG will be helpful in daily radiology practice and biopsy planning.

18. Second Look Ultrasonography-Guided Breast Biopsy with Magnetic Resonance Imaging Confirmation by Intralesional Contrast Injection

Objective: This study aimed to introduce an alternative pre-biopsy confirmation technique that combines sonography-guided intra-lesional contrast injections and single non-enhanced magnetic resonance imaging (MRI) pulse sequence in order to identify sonographic correlates of incidentally detected breast MRI lesions which were occult on primary ultrasonography (USG) and mammography examination. Materials and Methods: From May 2014 through May 2015, a total of 37 incidental breast lesions of 37 patients, which were detected by breast MRI, were evaluated with targeted second look ultrasound (SLUS). The suspected lesion on USG was marked with a gadolinium-based contrast agent under USG guidance. After a single non-enhanced T1 weighted control MR sequence, positively correlated lesions with initial MRI were sampled by USG guided core biopsy. Results: Of the 37 lesions evaluated, 32 (86%) lesions showed a correlation between MRI and SLUS findings. On SLUS core biopsy, there were eight (25%) malignant and 11 (34.4%) high-risk lesions among these 32 cases with correlated MRI findings; while the remaining 13 (40.6%) cases had benign histopathology. Eleven (34.4%) of the SLUS-discovered lesions were focus, 11 (34.4%) were non-mass enhancements, and the remaining 10 (31.2%) were mass lesions. Of the five lesions (13.5%) that showed no correlations on MRI and SLUS examinations, four were non-mass enhancements and one was focus. Conclusion: SLUS represents a method for identifying MRI-detected lesions and provides a bridge to ultrasound-guided biopsy for histopathological diagnosis. There is a need for confirmation of biopsies to avoid false negative results. We describe a cheap, safe, and easy-to-apply USG-guided pre-biopsy lesions marking method in order to ensure definite correlation.

19. An Unusual Presentation of T-Cell Lymphoblastic Lymphoma with Isolated Renal Involvement

The clinical presentation of Non-Hodgkin lymphoma (NHL) is frequently associated with the involvement of the abdomen and mediastinal lymphadenopathies, but rarely the kidney, ovaries, and testicles. Here, we report a rare case of T-cell lymphoblastic lymphoma (T-LBL) presenting with bilateral nephromegaly without acute renal failure (ARF) as the first manifestation. A 30-month-old boy was admitted to the department of pediatric nephrology exhibiting abdominal distension. Physical examination revealed bilateral renal palpation up to the inguinal region. Elevated lactate dehydrogenase (LDH) levels were detected in his blood. Bilateral diffuse enlarged kidneys with increased hypoechogenicity were found on abdominal ultrasonography. In the next step, contrast-enhanced computed tomography showed diffusely enlarged kidneys, which were compressing the intestinal bowels and midline structures. Renal biopsy demonstrated precursor T-LBL. We wish to report our patient with renal T-LBL presenting with diffuse renal enlargement, which has rarely been reported in the literature.

Amaç: Shear Wave Elastografi (SWE), sonografinin yanında doku elastisitesi ile ilişkili kantitatif bilgi sağlayabilen bir modalitedir. Lizil oksidaz (LOX) enzimi de ekstrasellüler matrikste kollajen çapraz bağlarını düzenleyerek doku elastisitesini etkileyen bir enzimdir. Çalışmamızda meme kanseri açısından BIRADS kriterlerine göre yüksek şüpheli lezyonu bulunan hastalarda lezyonların LOX, LOXL1 ve LOXL2 ekspresyon değeri ile SWE değerleri arasındaki korelasyonun araştırılması amaçlanmıştır.Materyal- Metod: Çalışmaya BIRADS 5 lezyonu bulunan yaşları 36 ile 70 arasında değişmekte olan (ortalama 49,57±9,59) 30 kişilik hasta grubu ve redüksiyon mamoplasti uygulanan, 17 kişilik kontrol grubu dahil edildi. Lezyonlar SWE ile değerlendirilerek Emean kPa, Emax kPa, SWE-SD ve SWE- ratio değerleri elde edildi. Ardından lezyonun perifer ve santral kesiminden genetik ve histopatolojik değerlendirme için ultrasonografi eşliğinde tru-cut biyopsi işlemi uygulandı. Lezyonların enzim ekspresyonu düzeyi ile SWE parametreleri arasındaki korelasyon istatistiksel açıdan değerlendirildi.Bulgular: Meme kanseri olguları ile kontrol grubu bireyler arasında LOX, LOXL2 değerleri açısından anlamlı fark saptanmadı. Meme kanseri olgularında LOXL1 santral değerleri kontrol grubundan anlamlı düzeyde yüksek olduğu saptandı (p:0,022). SWE değerleri ile LOX, LOXL1 ve LOXL2 değerleri arasında istatistiksel olarak anlamlı ilişki saptanmadı. Aksiller metastaz olan olguların SWE-ratio değerlerinin aksiller metastaz olmayan olguların değerlerinden (p:0,048), uzak metastaz olan olguların Emean değerlerinin de uzak metastaz olmayan olguların değerlerinden anlamlı düzeyde yüksek olduğu saptandı (p:0,022). Sonuç: Çalışmamızda lezyonların SWE değerleri ile tüm LOX ekspresyon düzeyleri arasında anlamlı korelasyon saptanamamıştır. Gelecekte daha fazla olgunun dahil edildiği prospektif çalışmalara ihtiyaç vardır. Çalışmamızda SWE ile aksiller ve uzak metastaz arasında korelasyon saptanmış olup SWE'nin prognostik özellikleri vurgulanmıştır. Lysyl oxidase (LOX) is an enzyme which is playing role in tissue elasticity by organising cross links between collogen fibers in extracellular matrix. The aim of our study is to investigate the correlation between SWE and LOX expression levels of highly suspicious breast lesions. Material and Metods: 30 patients who had BIRADS 5 breast lesions, ages ranged from 36 to 70 years and 17 control group who had reduction mammoplasty operation, were included to our study. Lesions're evaluated by SWE, Emean, Emax kPa, SWE-SD and SWE-ratio values were obtained. Afterwards, for genetic and histopathological examination ultrasound guided tru-cut biopsy was performed from the periferic and central parts of the lesions. Findings were evaluated for statistical correlations.Results: There was no statistically significant difference between LOX, LOXL2 gene expression levels in breast cancer and control group. LOXL1 central values were found to be significantly higher from control group (p = 0.022). There was no significant relationship between LOX, LOXL1, LOXL2 values and SWE parameters. SWE-ratios of patients with axillary metastasis is statistically significant higher from the cases without metastasis (p = 0.048). Distant metastasis was assosiated with statistically significantly higher Emean values (p = 0.022). Conclusion: No correlations found between LOX expression and SWE parameters in breast cancer patients. Although further prospective studies're needed. In this study, axillary and distant metastasis were found to be correlated with SWE features and this study emphasized the prognostic features of SWE. 90

Araştırmamıza Konya İl Milli Eğitim Müdürlüğü'ne bağlı 15 İlköğretim Okulundan seçilen öğrenciler, Konya 1.2.3. ve 4. Aile Mahkemelerine intikal eden, 7-14 yaş arası velayeti anneye verilmiş çocuklar ve Dosteli Derneğinin yardım kapsamına giren ailelerin çocukları katılmıştır. 300 çocuğa ulaşma hedeflendiği halde veri toplamada karşılaşılan sıkıntılar ve çocukların bu konuda görüş bildirmekte isteksizlikleri nedeniyle 252 çocukla çalışma yürütülmüştür. Araştırmada veri toplama aracı olarak Kişisel Bilgi Formu, Ebeveyn Kabul Red Ölçeği/ Çocuk Kısa Formu, Coopersmith Özsaygı Envanteri kullanılmıştır. Araştırmanın sonucunda, çocuğun anne tarafından kabul olduğu algısı arttıkça özsaygı düzeyi de artmaktadır. Çocuğun red algısı arttıkça benlik saygısı düzeyi azalmaktadır. Çocukların Özsaygı Ölçeği puanları ile annenin çocuğu Kabul-Red Ölçeği alt boyutlarından sıcaklık ve sevgi alt boyutu puanları ile özsaygı puanları arasında pozitif yönde anlamlı bir ilişki olduğu, saldırganlık- kin, ilgisizlik- ihmal ve ayrıştırılmamış reddetme alt boyutu puanları ile özsaygı puanı arasında negatif yönde anlamlı ilişki olduğu görülmektedir. Çocuğun anne Kabul-red algısı çocuğun benlik saygısı düzeyini %23 düzeyinde açıklamaktadır., To examine the detection of the maternal acceptance and rejection of the children of the broken families and it's effect on their self-esteem The purpose of this study, was to examine the level of acceptance or refusal in broken families, of mothers and children living with their mothers' and to investigate the relation of the development of their self-esteem. 15 primary schools were selected under the Konya Provincial Directorate of Education. Children between 7-14 years of age participated that were transferred from the first second, third and fourth Family court and who were granted custody to their mother and were children of families covered by the assistance of the Dosteli Association. The researcher aimed to reach 300 children however, due to the difficulties encountered in data collection, the study was conducted with 252 children largely for the reason that those children were reluctant to express their opinions. As a data collection tool, a Personal Information Form, the Parental Acceptance Rejection Scale / Children Short Form, and the Coopersmith Self-Esteem Inventory were adopted. The findings express that there was a positive relationship between children's Self-Esteem Scale scores and acceptance-rejection scale of mother's, under the subscale of warmth and love, whereas there was a significantly negative correlation between self-esteem and aggression- hatred, apathy-negligence and unsorted refuse subscale scores. Higher levels of child's perceived acceptance by their mother expressed higher levels of self-esteem. However, higher levels of perceived rejection expressed lower Self-esteem levels of the child. Children's perceptions of maternal acceptance-rejection describes the child's self-esteem levels of 23%.

- Abstract
Amaç: Süper mikrovasküler görüntüleme (SMG), renkli Doppler US (CDUS) ile karşılaştırıldığında düşük hızlı kan akım sinyallerini hareket artefaktlarından ayırarak küçük damarların daha hassas bir şekilde değerlendirilmesini sağlayan yeni bir tekniktir. Yüzeyel tromboflebit (STM), Behçet sendromlu (BS) hastalarda yaygın bir bulgudur ve prokoagülan bir durumdan ziyade damar duvarının iltihabı ile ilişkili olduğu düşünülmektedir. BS'li hastaların STM lezyonlarını kontrollerle birlikte SMG kullanarak değerlendirmeyi amaçladık. Yöntem: Fizik muayenede nodüler lezyonları olan 45 BS (16 K/29 E, ortalama yaş: 40,0±12,2) hastasını ve BS olmayan 10 (6 K/4 E ortalama yaş: 45,0±10,4) hastayı inceledik. B-mod US, CDUS ve SMG aynı radyolog tarafından yapıldı ve kaydedildi. Görüntüler daha sonra iki radyolog tarafından ayrı ayrı lezyon alanın ven içerip içermemesi, ven mevcutsa lümen içi trombüs varlığı, ven duvar kalınlığı ve ven duvarındaki renk kodlamalarının varlığı değerlendirildi. Her iki radyolog da teşhislere ve birbirlerinin değerlendirmelerine karşı kördü. Bulgular: On altı BS ve 3 BS olmayan hastanın nodüler lezyonlarına STM tanısı konuldu. BS'li 4 hastaya, BS olmayan 1 hastaya tromboz olmadan flebit ve kalan 20 BS ve 6 BS olmayan hastaya eritema nodozum tanısı kondu. On üç/16 (%81) STM'li BS hastasında, flebitli 1 (%25) BS hastasında görüntüleme yöntemlerinden sadece SMG ile ven duvarını çepeçevre saran halo şeklinde sinyal tespit edildi (Şekil 1). Eritema nodozumlu BS hastalarında bu bulgu gözlenmedi. BS olmayan STM'li 3 hastadan sadece 1'inde (%33) venöz halo işareti vardı. BS olmayan diğer hastaların hiçbirinde venöz halo işareti yoktu. Gözlemciler arası güvenilirlik iyiydi (κ=0,96, p<0,001). Sonuç: STM'li BS hastalarının çoğunda SMG ile damar duvarında enflamasyonla ilişkili olduğunu düşündüğümüz venöz halo bulgusu saptandı. Tanımladığımız bu bulgunun BS'ye özgüllüğünü ve önemini anlamak için daha fazla sayıda BS hastasının farklı vasküler lezyonlarının uygun kontrollerle birlikte değerlendirilmesi gerekmektedir. 28. Diagnostic Characteristics and Clinical Relevance of Incidental Hypermetabolic Breast Lesions Detected on 18 F-FDG PET-CT: A Retrospective Evaluation.

- Abstract
Rationale and Objectives: The study aimed to evaluate demographic and radiological characteristics of breast incidentalomas found on 18-fluorodeoxyglucose Positron Emission Tomography-Computed Tomography (
18 F-FDG PET-CT) performed for extramammary indications., Materials and Methods: A total of 1263318 F-FDG PET-CT scans performed between January 1, 2018 and January 1, 2024, were retrospectively reviewed. Breast incidentalomas that had undergone breast imaging, tissue diagnosis, or at least 2-year radiological follow-up were included. Demographic data and lesion size were recorded. Maximum and average standardized uptake values (SUVmax -SUVavg ) and SUV corrected for lean body mass (SUL) were calculated using region of interest (ROI)., Results: The inclusion criteria were met in 101 lesions (81 benign and 20 malignant). The most common benign lesion was fibroadenoma (n = 21), followed by stable lesions during follow-up (n = 18) and benign breast parenchyma (n = 11). The most common malignant lesion was invasive ductal carcinoma (n = 11). The diagnostic characteristics of SUVmax ≥ 3, SULmax ≥ 2, SUVavg ≥ 0.735, SULavg ≥ 0.48, and BI-RADS≥ 4 were 75%, 70%, 75%, 70% and 100% for sensitivity, 69%, 69%, 62%, 62% and 67% for specificity, and 69.3%, 68.3%, 62.4%, 61.4% and 73.3% for accuracy, respectively. The highest negative predictive values (NPV) were obtained with BI-RADS and SUVmax (100% and 92%, respectively). No significant difference in malignancy rate was observed for the lesion size and age of the patients (p > 0.05)., Conclusion: There is a risk of detecting malignancy in incidental lesions showing18 F-FDG uptake. 29. A Rare Cause of Breast Swelling in Lactating Women: Aquafilling ® Gel Injection.

- Abstract
Aquafilling® gel has been used in recent years as an alternative method to breast augmentation surgery. In this case report, we aimed to discuss radiological imaging findings and complications of Aquafilling® gel injection. Case: A 34-year-old lactating female patient presented with swelling and pain in the left breast. Ultrasonography showed massive septate fluid collections with dense content in both breasts and between the pectoral muscle fibers. On mammography, both breasts appeared dense with large mass opacities. On magnetic resonance imaging, extensive fluid-signal cystic areas were observed. Peripheral enhancement around the fluid in the left breast was present in the contrast-enhanced series. We learned from her anamnesis that Aquafilling® was applied to both breasts 5 years ago. Ultrasonography-guided sampling was performed from the cystic areas in the left breast and cytological examination revealed basophilic Aquafilling® material surrounded by diffuse inflammatory infiltrates. Breast augmentation history should be questioned in suspicious cases because Aquafilling® gel injection and its complications can present with a wide variety of symptoms and may mimic various other conditions on radiological imaging, such as cancer, abscess, granulomatous mastitis, and parasite infections., Competing Interests: None declared., (© Copyright 2023 by The Medical Bulletin of Sisli Etfal Hospital.)

Background: Idiopathic granulomatous mastitis that has not had a clear consensus about its treatment since the day it was identified as a rare, benign inflammatory breast disease that mimics malignancy due to its appearance features., Aims: In our research, we intended to compare the efficiency of intralesional and systemic steroids administration in the treatment of idiopathic granulomatous mastitis., Study Design: Prospective randomized controlled study., Methods: A total of 36 female patients who had been histopathologically diagnosed with idiopathic granulomatous mastitis and whose other factors had been microbiologically excluded were included in the study. The patients were randomized into two sub-groups that would be treated with systemic and intralesional steroids. All patients were evaluated through physical examination one week after the completion of the treatment. Subsequently, the follow-up of the patients was performed thorough physical examination and ultrasonography and/or magnetic resonance imaging at the 1st, 3rd, and 6th months., Results: All patients adapted to treatment. Complete clinical regression occurred in 32 patients, while 30 of 36 patients responded to treatment both radiologically and clinically. A total of 4 patients had minor side effects. It was determined that there was no statistically significant difference between local and systemic steroid groups in terms of complete clinical regression, responded to treatment side effects, and recurrence rates., Conclusion: Intralesional steroid administration was also considered just as a successful treatment method as the systemic steroid administration., Key Words: Idiopathic granulomatous mastitis, Intralesional steroid, Systemic steroid.

Purpose: We aimed to evaluate the utility of shear wave elastography (SWE) for assessing the placenta in preeclampsia disease., Methods: A total of 50 pregnant women in the second or third trimester (23 preeclampsia patients and 27 healthy control subjects) were enrolled in the study. Obstetrical grayscale and Doppler ultrasonography, SWE findings of placenta, and prenatal/postnatal clinical data were analyzed and the best SWE cutoff value which represents the diagnosis of preeclampsia was determined. Sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and diagnostic accuracy of preeclampsia were calculated based on SWE measurements., Results: Mean stiffness values were much higher in preeclamptic placentas in all regions and layers than in normal controls. The most significant difference was observed in the central placental area facing the fetus where the umbilical cord inserts, with a median of 21 kPa (range, 3-71 kPa) for preeclampsia and 4 kPa (range, 1.5-14 kPa) for the control group (P < 0.01). The SWE data showed a moderate correlation with the uterine artery resistivity and pulsatility indices. The cutoff value maximizing the accuracy of diagnosis was 7.35 kPa (area under curve, 0.895; 95% confidence interval, 0.791-0.998); sensitivity, specificity, PPV, NPV, and accuracy were 90%, 86%, 82%, 92%, and 88%, respectively., Conclusion: Stiffness of the placenta is significantly higher in patients with preeclampsia. SWE appears to be an assistive diagnostic technique for placenta evaluation in preeclampsia.
