The fifth edition of Clinical Periodontology and Implant Dentistry brings to its readers another iteration of the unrivalled, unparalleled work on the specialty of periodontics. The editors have brought together contributions from experts all over the world to provide the reader with a comprehensive, cohesive text that fuses scholarship and science with clinical instruction and pragmatism. Clinical Periodontology runs the gamut of sub-disciplines and topics within periodontics and implant dentistry, supporting an intellectually and internationally inclusive approach. The fifth edition includes a wealth of new and revised chapters that reflect developments in the field in the years since the fourth edition was published. The sections on anatomy of the periodontium and the basic sciences underlying tooth and implant attachment have been expanded, as has the section on pathology and pathogenesis of periodontitis. Increased interest in the interdisciplinary aspects of periodontology is reflected by new chapters on orthodontic anchorage. Other important topics with increased prominence include: risk assessment and host-modulation therapies, regenerative therapies, and a full discussion of surgical and non-surgical therapies. With an approximate increase in length of 25% and over 15 new chapters, the 5th edition of Clinical Periodontology and Implant Dentistry has grown beyond the confines of one volume. Two volumes divide the topics into basic sciences and applied clinical concepts, at the same time maintaining a seamless integration between the two. This book is the cornerstone of periodontology, and represents a unique distillation of the specialty by an expert group of authors under the tutelage of a legendary editorial team.

Contains fulltext : 53228.pdf (publisher's version ) (Closed access), OBJECTIVES: To evaluate whether an oily calcium hydroxide suspension (OCHS) promotes bone healing when used as an adjunct to guided bone regeneration (GBR). MATERIAL AND METHODS: Rigid, hemispherical, teflon capsules were placed with their open part facing the lateral surface of the ramus on both sides of the mandible in 10 adult Wistar rats. In each animal, one capsule was filled out with an OCHS (test) before placement, while the capsule on the other side was left empty (control). After 4 months of healing, the animals were sacrificed and histological sections containing the capsules and the neighboring soft and hard tissues were prepared. On three to four sections taken by uniformly random sampling from each specimen, the relative volumes of (1) the newly formed bone (mineralized bone and marrow), (2) the soft connective tissue, (3) the residual OCHS, and (4) the acellular (empty) space inside the capsule were estimated by a point-counting technique, and expressed as percentage of the space originally created by the capsule. RESULTS: There was no new bone formation inside the capsules in all but one test specimen, where only a minimal amount of newly formed bone could be observed in continuation with the lateral surface of the ramus. OCHS had a homogenous appearance and occupied the major portion (79.4%) of the space created by the capsule. No signs of active resorption of the material could be observed. On the contrary, 31.5% of the space provided by the capsule was filled out with newly formed bone in the control group. The new bone had a trabecular appearance with large marrow spaces filled with hematopoietic and fatty marrow. The rest of the capsule space in the controls appeared empty. CONCLUSION: OCHS may hamper bone healing when used as an adjunct to GBR.

Contains fulltext : 47811.pdf (publisher's version ) (Closed access), The aim of the present study was to compare newly formed cementum following different types of regenerative therapy in humans. Eighteen patients, each displaying one advanced intrabony defect around teeth scheduled for extraction, were included in this study. The defects were treated with either guided tissue regeneration (GTR), enamel matrix protein derivative (EMD), EMD plus bioactive glass, bovine-derived xenograft (BDX), BDX plus GTR, or BDX plus EMD. After healing, the teeth were removed together with their surrounding soft and hard tissues. Cellular content, presence of artifactual splits between the new cementum and the old one or the dentin surface, and thickness of the new cementum were evaluated. Irrespective of treatment, the new cementum was of a reparative, cellular, extrinsic and intrinsic fiber type. There were no differences in cementum thickness among treatments. These findings indicate that in humans, (a) the new cementum formed after different types of regenerative therapy was, irrespective of the treatment, of a reparative, cellular, extrinsic and intrinsic fiber type, and (b) the regenerative modality does not seem to influence the type of newly formed cementum.

Udgivelsesdato: 1993-Jan, The aim of this study was to determine if a periodontal ligament can form around self-tapping, screw type titanium dental implants. Implants were inserted in contact with the periodontal ligament of root tips retained in the mandibular jaws of 7 monkeys. In each side of the mandible, 1 premolar and 2 molars were removed in such a manner that in approximately half the cases, the root tips were retained. Following healing, the experimental areas were examined on radiographs, and sites were selected for the insertion of the implants, so that every second implant would have a close contact to the retained root tips. Three months after placement of the implants, the animals were sacrificed, and ground sections were produced for histological analysis. The results demonstrated that newly-formed cementum had become deposited on the implant surface in the contact area between the implant and the retained roots, whereas osseointegration was consistently found on the remaining part of the implant surface as well as on the entire surface of the implants placed without contact to the retained root tips. The study has shown that, when placed in contact with periodontal ligament tissue of retained roots, a periodontal ligament can form on self-tapping, screw type titanium dental implants in areas where a void is present between the surrounding bone and the implant at the time of insertion.

Udgivelsesdato: null-null, The purpose of the present study was to examine the effect of treating bony craters around titanium dental implant with polytetrafluoroethylene membranes (PTFE), with and without grafting of hydroxyapatite (HA), and with HA alone. 4 standardized bone defects were prepared in the alveolar ridge of edentulous areas in each of 7 monkeys. A titanium implant was then placed centrally in each defect, and in each monkey, the defects were treated with 1 of the following 4 treatment modalities: (1) coverage with PTFE membrane; (2) grafting of HA and covering with a PTFE membrane; (3) HA grafting; (4) no treatment. Following a healing period of 12 weeks, all animals were sacrificed and mesio-distal ground sections of the treated areas were prepared. The histological analysis showed that all bone defects around the implants treated with PTFE membranes and 5 of the defects treated with HA and PTFE membranes were completely filled with new bone. The defects treated with HA alone and thecontrol defects with no treatment only demonstrated new bone formation in the bottom of the defects. The results suggest that bone defects around titanium implants can be treated wuccessfully with PTFE membranes.

Udgivelsesdato: null-null, The aim of the present study was to explore the possibility of achieving osseointegration of dental titanium implants, inserted into alveoli immediately after tooth extraction, by covering the recipient site with a teflon membrane. In each of 7 monkeys, mucoperiosteal flaps were raised on the buccal and palatal aspects of the maxillary molars in both sides of the jaw. The second molars were then extracted and self tapping titanium implants of the screw type (Astra Dental Implants) were inserted into the sockets of the largest roots. In the coronal portion of the sockets, a void was always present between the implant and the socket walls. In one side of the jaw, a membrane (Gore-Tex Augmentation Material) was adjusted to cover the implant. The implant in the other side of the jaw served as control and was only covered by the tissue flaps. Microscopic analysis after 3 months of healing revealed that soft tissue was facing the coronal portion of the implants to a varying degree in the control side, while osseointegration was consistently observed to the top of the membrane-covered implants which remained submerged throughout the experimental period. The results suggest that the membrane technique can secure complete osseointegration of implants inserted immediately into extraction sockets.
