Background: Lead-related infections that might develop after pacemaker implantation associated with high mortality and morbidity rates are challenging to manage and pose high-cost. Patients with lead-related infections usually present with fever, chills and fatigue and the treatment can be challenging unless the implant system is extracted. Case presentation: A 66-year old male patient who underwent dual chamber pacemaker and implantable cardioverter defibrillator was admitted to the emergency service with a six-week history of complaints of hiccups and fever. After a detailed investigation, lead-related infective endocarditis was the diagnosis. The patient was initiated on antibiotic therapy and lead extraction was performed. Conclusions: Patients with signs of infection who underwent pacemaker implantation may present with atypical symptoms such as hiccup. In these cases, imaging, particularly echocardiography, should be performed as soon as possible and the localization of the pacemaker leads and signs of infective endocarditis should be investigated.

Amaç: Trombolitik tedavi bir revaskülarizasyon stratejisidir ve ST yükselmeli miyokard enfarktında önemli tedavilerden biridir. ST seg- ment elevasyonunun başarılı rezolüzyonu trombolitik uygulandıktan sonra >%50 oranında rezole olması olarak tanımlanır ve tromboli- tik tedavi başarısını yansıtır. Bizim amacımız trombolitik tedavi uygulanan ST yükselmeli miyokard enfarktı hastalarında başvurudaki he- mogram parametreleri ile ST segment rezolüsyonu arasındaki ilişkiyi incelemektir. Gereç ve Yöntem: ST yükselmeli miyokard enfarktı ile başvuran ve trombolitik tedavi uygulanan hastalardan dahil edilme kriterlerini kar- şılayan 93 hasta çalışmaya dahil edildi. ST segment yükselmesinin toplamı trombolitik ajan alımından sonraki 60-90 dk sonra ölçüldü. ST rezolüsyonuna göre hastalar iki gruba bölündü (STR >%50 ve STR ?%50). STR ile takip eden hemogram parametreleri arasındaki iliş- ki incelendi; kırmızı kan hücresi, hemoglobin, hematokrit, ortalama korpusküler hemoglobin, ortalama korpüsküler hacim, ortalama kor- püsküler hemoglobin konsantrasyonu, kırmızı hücre dağılım genişliği, platelet, ortalama platelet hacmi, platelet dağılım genişliği, pla- teletkrit, beyaz kan hücresi, nötrofil, lenfosit ve nötrofil/ lenfosit oranı. Bulgular: Bu çalışmada hastaların %69,8’i (n=65) STR >%50 grubunda, %30,2’si (n=28) STR ?%50 olan grupta idi. ST rezolüsyonu ile yaş, cinsiyet, hipertansiyon ve diabetes mellitus varlığı, başvurudaki kalp hızı ve sistolik kan basıncı arasında istatistiksel olarak anlamlı bir fark saptanmadı. Tedaviye başlama süreleri iki grupta benzerdi (4,2±2,6, 3,7±1,8, p=0,270). Trombolitik ajan tipi ve enfarkt ilişkili arter iki grupta da benzerdi (p=0,978, p=0,765, sırayla). Trombolitik tedavi uygulanan ST yükselmeli miyokard enfarktı hastalarında başvuru- daki hemogram parametreleri ile ST segment rezolüsyonu arasında herhangi bir ilişki saptamadık. Sonuç: Çalışmamızda ST yükselmeli miyokard enfarktüslü hastalarda hemogram parametreleri ile trombolitik tedavi başarısı arasında bir ilişki saptayamadık., Objective: Thrombolytic treatment is a revascularization strategy and the major treatment in patients with ST-elevation myocardial in- farction. Successful resolution of ST segment was defined as ST segment >50% after thrombolisis and reflects thrombolytic treatment success. We aim to investigate the relationship between admission hemogram parameters and ST-segment resolution in the patient with ST-elevation myocardial infarction who administered thrombolytic agent. Material and Method: We included 93 patients suffering from ST-elevation myocardial infarction who were administered thrombolytic treatment and met the inclusion criteria. The sum of ST-segment elevation was measured at presentation and 60-90 minutes after ad- ministration of the thrombolytic agent. According to the sum of STR, patients were divided into two groups (patients with STR> 50% vs those with STR? 50%). The relationship between STR and following hemogram parameters has been studied; red blood cells, hemoglo- bin, hematocrit, mean corpuscular hemoglobin, mean corpuscular volume, mean corpuscular hemoglobin concentration, red cell distri- bution width, platelet, mean platelet volume, platelet distribution width, plateletcrit, white blood cell, neutrophil, lymphocyte and neu- trophil/ lymphocyte ratio. Results: In this study, 69.8% (n = 65) and 30.2% (n = 28) of patients were found to have STR> 50% and STR? 50%, respectively. There was no statistically significant association between STR and age, sex, the presence of hypertension and diabetes mellitus, admission heart rate, systolic blood pressure. Time to treatment was similar in two groups (4.2 ± 2.6, 3.7 ± 1.8, p= 0.270). The type of thrombolytic agent and the infarct-related artery was similar in two groups (p=0.978, p=0.765, respectively). We did not find any association between STR and admission hemogram parameters in the patient with ST-elevation myocardial infarction who administered thrombolytic agent. Conclusion: Our study failed to demonstrate a relationship between hemogram parameters and thrombolytic treatment success in the patient with ST-elevation myocardial infarction.

Amaç: Bu çalışmanın amacı non-dipper hipertansiyon ile bazı enflamatuvar belirteçlerin ilişkisini değerlendirmektir; nötrofil-lenfosit ora- nı, platelet-lenfosit oranı, kırmızı hücre dağılım genişliği ve monosit yüksek yoğunluklu lipoprotein oranı. Gereç ve Yöntem: Çalışmaya 170 hipertansif hasta dahil edildi. 24 saat ambulatuvar kan basıncı monitörizasyon cihazları ile belirlenen kan basıncı paternine göre hastalar dipper ve non-dipper olarak iki gruba ayrıldı. Kan örnekleri alındı ve tam kan sayımı, lipid paneli, kreatinin ve açlık glukozuna bakıldı. Enflamatuvar belirteçler bu kan örneklerinden hesaplandı. Bulgular: Seksenüç hasta dipper hipertansiyon grubunda, 87 hasta non-dipper hipertansiyon grubundaydı. Nötrofil-lenfosit oranı dipper hipertansiyon grubunda 2,61 [0,09-6,31], non-dipper hipertansiyon grubunda 2,04 [0,81-12,45] (p=0,824) olup benzerdi. Platelet-lenfo- sit oranı non-dipper hipertansiyon grubunda daha yüksekti fakat bu istatistiksel olarak anlamlı bir fark değildi (111 [18-265], 115 [44- 364], p=0,518). Monosit HDL oranı da iki grup arasında benzer bulundu; dipper hipertansiyon grubunda 0,11 [0-0,03], non-dipper hi- pertansiyon grubunda 0,11 [0-0,70] (p= 0,753). Kırmızı hücre dağılım genişliği non-dipper grubunda anlamlı olarak yüksek saptandı (13,5 [11,9-19,8], 14 [12,3-21], p= 0,008). Sonuç: Nondipper ve dipper hipertansiyon hastalarında nötrofil-lenfosit oranı, platelet-lenfosit oranı ve MHR bakımından fark yoktur. Ancak kırmızı hücre dağılım genişliği değeri non-dipper hipertansiyon ilişkili enflamasyonu göstermede kullanılabilir., Objective: This study aimed to evaluate the associations between non-dipper hypertension and some inflammatory markers, including; neutrophil-lymphocyte ratio, platelet-lymphocyte ratio, monocyte HDL ratio and red cell distribution width. Material and Method: A total of 170 hypertensive patients enrolled in the study. Study patients were divided into two groups accord- ing to blood pressure pattern measured by 24-hour ambulatory blood pressure monitoring devices; dipper and non-dipper. Blood sam- ples were taken to determine complete blood counts, lipid panel, creatinine, fasting glucose. Inflammatory markers were calculated from these blood samples Results: Eightythree patients were in dipper hypertension group, and 87 patients were in non-dipper hypertension group. Neutrophil- lymphocyte ratio was similar between two groups; 2.61 [0.09-6.31] in dipper hypertension group, 2.04 [0.81-12.45] in non-dipper hy- pertensive group (p=0.824). Platelet-lymphocyte ratio was higher in non-dipper hypertension group but it was not statistically signifi- cant (111 [18-265] , 115 [44-364], p=0.518). Also monocyte HDL ratio was similar between two groups; 0.11 [0-0.03] dipper hyperten- sion group, 0.11 [0-0.70] in non-dipper hypertension group (p= 0.753). Red cell distribution width was significantly higher in non-dipper hypertension group (13.5 [11.9-19.8], 14 [12.3-21], p= 0.008) Conclusion: There was no difference among neutrophil-lymphocyte ratio, platelet-lymphocyte ratio and monocyte HDL ratio in patients with non-dipper hypertension and dipper hypertension. However red cell distribution width value may be used to identify inflammation related non-dipper hypertension.

Pulmoner tromboemboli akut dönemde yüksek mortaliteye neden olan bir hastalıktır. Yüksek riskli pulmoner tromboemboli varlığında kontrendikasyon yoksa trombolitik tedavi hayat kurtarıcıdır. Fakat orta riskli pulmoner tromboemboli hastalarında antikoagülan tedavi-ye trombolitik ajan eklenmesi net değildir. Son zamanlarda yapılan çalışmalarda kanama risk profili düşük olan hastalarda antikoagü-lan tedaviye düşük doz trombolitik tedavinin güvenle eklenebileceği belirtilmektedir. Pulmoner tromboemboliye yatkınlık oluşturan bir çok klinik durum vardır. İmmobilizasyon, obezite, operasyon sonrası dönem, kanser varlığı, trombofili, oral kontraseptif kullanımı gibi du-rumlar bunlardan bazılarıdır. Otoimmün hastalıkların bir kısmı venöz tromboembolizm riskini arttırabilir. Romatoid artritin de venöz tromboembolizm riskini arttırdığı bildirilmiştir. Biz romatoid artritli bir hastada gelişen masif pulmoner emboli vakasını sunduk., Pulmonary thromboembolism is a disorder that causes high mortality rate in the acute phase. Thrombolytic therapy is a life saving treat-ment in patients with high risk pulmonary thromboembolism without contraindications. However addition thrombolytic agent to anti-coagulant therapy in patients with moderate risk pulmonary embolism is not clear. In recent studies, addition thrombolytic agent to an-ticoagulant therapy in patients with moderate risk pulmonary thromboembolism had found safely in patients with low bleeding risk pro-file. There are many situations that predispose to pulmonary embolism. Immobilization, obesity, post-operative period, the presence of cancer, thrombophilia and use of oral contraceptive are some of them. Some autoimmune diseases may increase the risk of venous thromboembolism. Also rheumatoid arthritis have been reported to increase the risk of venous thromboembolism. We reported a case that massive pulmonary thromboembolism in a patient with rheumatoid arthritis.

Fractional flow reserve (FFR) is a highly reproducible, accurate and lesion-specific index to indicate inducible ischemia for a particular coronary artery lesion. Invasively measured aortic pulsatility (AP) is an indicator of aortic stiffness. In this study we aimed to evaluate the possible impact of AP in terms of aortic stiffness on FFR measurement.In this study, we reviewed the FFR evaluation of 90 patients who had intermediate lesions (40-70% stenosis measured with quantitative coronary analysis) at the left anterior descending artery (LAD). AP was calculated as the ratio of aortic pulse pressure (systolic-diastolic pressure) to mean pressure.Aortic systolic pressure, aortic diastolic pressure, aortic pulse pressure and also aortic pulsatility did not differ significantly between patients with FFR ≤ 0.80 and FFR0.80 (p = 0.44, p = 0.28, p = 0.93 and p = 0.41, respectively). In subgroups arranged according to the degree of luminal narrowing (40-50%, 51-60%, and 61-70%), we did not observe significant correlation between AP and FFR value in subgroups with 40-50% and 51-60% lesions (r = 0.03, p = 0.95 and r = 0.07, p = 0.69, respectively). However, a statistically significant negative correlation between FFR value and AP in the subgroup of patients with 61-70% lesions was detected (r = -0.54, p = 0.04).These findings suggested that aortic stiffness might have a possible impact on FFR measurement in coronary lesions of 61-70% stenosis evaluated quantitatively.Aortic pulsatility; Fractional flow reserve; Stiffness.

Giriş: Yapılan metaanalizlerde NSTEMI hastalarının %25-39 kadarında anjiografi sonucunda total tıkanıklık saptanmıştır. Yeni yapılan metaanalizlerde total tıkanıklık saptanan NSTEMI hastalarında kalp yetmezliği, ölüm ve major kardiyak istenmeyen olaylar daha sık görülmüştür. Monosit; makrofajların öncüsü olması sebebiyle inflamasyonda önemli bir yere sahiptir. HDL ise ateroskleroz gelişimini önleyen bir lipoproteindir. Ayrıca HDL'nin antiinflamatuvar, antioksidan ve antitrombotik etkileri bilinmektedir. Monosit/HDL oranı nisbeten yeni bir belirteçtir ve kardiyovasküler hastalıklar ile ilişkili olduğunu gösteren çalışmalar son zamanlarda yayınlanmıştır. Çalışmamızda total tıkanıklık saptanan NSTEMI hastalarında monosit/HDL oranı ile bazı biyokimyasal parametreler arasındaki ilişki araştırıldı.Metod: Kesitsel ve retrospektif olan çalışmamıza Ocak 2014- Aralık 2015 tarihleri arasında NSTEMI tanısıyla Bülent Ecevit Üniversite Hastanesine başvuran ve koroner anjiyografi yapılan hastalar dahil edildi. Çalışmaya tüm dahil edilme kriterlerini taşıyan 135 NSTEMI hasta alındı. Hastalar koroner anjiyografide total tıkanıklık saptanan (oklüde) ve total tıkanıklık saptanmayan (non-oklüde) olarak ikiye ayrıldı. Çalışma hastalarının yaş, cinsiyet, hipertansiyon, diyabet ve hiperlipidemi varlığı kaydedildi. Biyokimyasal parametre olarak açlık kan şekeri, üre, kreatinin, ürik asit, total kolestrol, düşük dansiteli lipoprotein (LDL), yüksek dansiteli lipoprotein (HDL) ve trigliresit (TG) değerleri alındı. Hemogram parametreleri olarak beyaz kan hücresi (WBC), hemoglobin, platelet, plateletkrit (PCT), kırmızı kan hücreleri dağılım genişliği (RDW), ortalama platelet hacmi (MPV), lenfosit, monosit, nötrofil, platelet/lenfosit oranı ve monosit/HDL oranı kaydedildi. Bu çalışma parametrelerini iki grup arasında karşılaştırdık.Bulgular: Çalışmaya alınan hastaların 102'si non-oklüde NSTEMI grubunda, 33'ü oklüde NSTEMI grubundaydı. Hastaların yaş ortalaması 60,7±12 saptandı. Yaş, cinsiyet, hipertansiyon, diyabet ve hiperlipidemi varlığı açısından iki grup benzerdi. Biyokimyasal parametrelerin ve hemogram parametrelerinin karşılaştırmalı analizinde iki grup arasında PCT ve HDL dışında istatistiksel olarak anlamlı bir fark saptanmadı. MHR açısından da iki grup arasında anlamlı bir fark yoktu (p= 0.289). PCT oklüde NSTEMI grubunda % 0.239 (0.112-0.531), non-oklüde NSTEMI grubunda % 0.213 (0.093- 0.637) olarak bulundu (p=0.032). HDL ise oklüde NSTEMI grubunda 45 (28-101) mg/dl, non-oklüde NSTEMI grubunda 40 (18-72) mg/dl olarak bulundu (p=0.031). Sonuç: Çalışmamızda hastaların yaklaşık dörtte birinde total oklüde koroner arter saptandı. NSTEMI hastalarında oklüzyon ve MHR arasında istatiksel olarak anlamlı bir ilişki saptamadık. HDL ve PCT değerleri oklüde NSTEMI grubunda non-oklüde NSTEMI grubuna göre istatistiksel olarak anlamlı bir şekilde yüksek saptandı. Çalışmamızın sonuçlarının genellenebilmesi için daha büyük ölçekli, prospektif çalışmalarla desteklenmesi gerekir. Introduction: %25-39 total occlusion has been detected on the coroner angiography at NSTEMI patients according to metaanalyses. NSTEMI patients with total occlusion has been shown that its associated more heart failure, major cardiac undesirable event and death according to recent metaanalyses. Monocytes has a prominent role in inflamation because it is a precursor of macrophages. HDL is an antiaterogenik lypoprotein. Moreover HDL has antioxidan, antitrombotic and antiinflammatory effects. Monocyte/HDL rate is a relatively new marker and studies showing that it is associated with cardiovascular diseases has recently been published. In our study, we investigated the relationship between the ratio of monocyte/HDL and some biomechanical parameters in NSTEMI patients with total occlusion.Method: Coronary angiography of NSTEMI patients at a time period between january 2014 and december 2015 at Bülent Ecevit University has been included to our cross-sectional and retrospective study. 135 patients with all inclusion criteria were included in the study. The age and gender of patients and the presence of hypertension diabetes and hyperlipidemia were recorded in study patients. Blood glucose, urea, creatinin, uric asid, total cholesterol, low density lipoprotein (LDL), high density lipoprotein (HDL) and triglyceride (TG) values were taken as biochemical parameters. White blood cell (WBC), hemoglobin, platelet, plateletcrit, red blood cell distribution width (RDW), mean platelet volume (MPV), lymphocyte, monocyte, neutrophil, platelet/lymphocyte ratio, monocyte/high density lipoprotein ratio were recorded as hemogram parameters. We compared these study parameters between two groups.Results: 102 of the patients included in the study were non occluded NSTEMI group and 33 of them were in occluded NSTEMI group. The average age of patients were 60,7±12. Age, gender, hypertension, diyabetes and hyperlipidemia were similar in two groups. There was no statistically significant difference between two groups in terms of biochemical parameters and hemogram parameters except PCT and HDL. There was no significant difference between the two groups in terms of MHR (p = 0.289). PCT was 0.239% (0.112-0.531) in the occluded NSTEMI group and 0.213% (0.093 - 0.637) in the non-occluded NSTEMI group (p = 0.032). HDL was 45 (28-101) mg / dl in the occluded NSTEMI group and 40 (18-72) mg/dl in the non-occluded NSTEMI group (p=0.031). Conclusion: In our study, occluded coronary artery was detected in approximately one fourth of the study patients. We did not find a statistically significant relationship between occlusion and MHR in NSTEMI patients. HDL and PCT levels were significantly higher in the occluded NSTEMI group than the non-occluded NSTEMI group. In order to generalize the results of our study, it should be supported with larger scale and prospective studies. 80
