The integration of realist evaluation principles within randomised controlled trials (‘realist RCTs’) enables evaluations of complex interventions to answer questions about what works, for whom and under what circumstances. This allows evaluators to better develop and refine mid-level programme theories. However, this is only one phase in the process of developing and evaluating complex interventions. We describe and exemplify how social scientists can integrate realist principles across all phases of the Medical Research Council framework. Intervention development, modelling, and feasibility and pilot studies need to theorise the contextual conditions necessary for intervention mechanisms to be activated. Where interventions are scaled up and translated into routine practice, realist principles also have much to offer in facilitating knowledge about longer-term sustainability, benefits and harms. Integrating a realist approach across all phases of complex intervention science is vital for considering the feasibility and likely effects of interventions for different localities and population subgroups.

PURPOSE: Health education in school classrooms can be effective in promoting sexual health and preventing violence and substance use but effects are patchy and often short term. Classroom education is also challenging because of schools' increasing focus on academic-performance metrics. Other school-based approaches are possible, such as healthy school policies, improving how schools respond to bullying, and parent outreach, which go beyond health education to address broader health determinants. Existing systematic reviews include such interventions but often alongside traditional health education. There is scope for a systematic review of reviews to assess and synthesize evidence across existing reviews to develop an overview of the potential of alternative school-based approaches. METHODS: We searched 12 databases to identify reviews published after 1980. Data were reviewed by two researchers. Quality was assessed using a modified Assessing the Methodological Quality of Systematic Reviews checklist and results were synthesized narratively. RESULTS: We screened 7,544 unique references and included 22 reviews. Our syntheses suggest that multicomponent school-based interventions, for example, including school policy changes, parent involvement, and work with local communities, are effective for promoting sexual health and preventing bullying and smoking. There is less evidence that such intervention can reduce alcohol and drug use. Economic incentives to keep girls in school can reduce teenage pregnancies. School clinics can promote smoking cessation. There is little evidence that, on their own, sexual-health clinics, antismoking policies, and various approaches targeting at-risk students are effective. CONCLUSIONS: There is good evidence that various whole-school health interventions are effective in preventing teenage pregnancy, smoking, and bullying.

BACKGROUND: Randomised controlled trials (RCTs) of social interventions are often criticised as failing to open the 'black box' whereby they only address questions about 'what works' without explaining the underlying processes of implementation and mechanisms of action, and how these vary by contextual characteristics of person and place. Realist RCTs are proposed as an approach to evaluation science that addresses these gaps while preserving the strengths of RCTs in providing evidence with strong internal validity in estimating effects. METHODS: In the context of growing interest in designing and conducting realist trials, there is an urgent need to offer a worked example to provide guidance on how such an approach might be practically taken forward. The aim of this paper is to outline a three-staged theoretical and methodological process of undertaking a realist RCT using the example of the evaluation of a whole-school restorative intervention aiming to reduce aggression and bullying in English secondary schools. DISCUSSION: First, informed by the findings of our initial pilot trial and sociological theory, we elaborate our theory of change and specific a priori hypotheses about how intervention mechanisms interact with context to produce outcomes. Second, we describe how we will use emerging findings from the integral process evaluation within the RCT to refine, and add to, these a priori hypotheses before the collection of quantitative, follow-up data. Third, we will test our hypotheses using a combination of process and outcome data via quantitative analyses of effect mediation (examining mechanisms) and moderation (examining contextual contingencies). The results are then used to refine and further develop the theory of change. CONCLUSION: The aim of the realist RCT approach is thus not merely to assess whether the intervention is effective or not, but to develop empirically informed mid-range theory through a three-stage process. There are important implications for those involved with reporting and reviewing RCTs, including the use of new, iterative protocols. Many orphans in Jordan, according to Islamic Family Law, lack the lawful lineage that forms the basis of rights such as the right to a family name or inheritance. In a society that is characterized by a tribal sense of belonging and patrilineal entitlement, these legal disadvantages, added to the hardships of orphanhood, manifest themselves in orphans’ everyday lives, resulting in social stigma and discrimination. Orphans and youth without lawful lineage recently began organizing to expose the discrimination toward them, demand their rights, and push for social change by breaking through fear and taboo, but change proved difficult. Using empirical data and interviews with orphans in Jordan, this article investigates how they experience the patriarchy of law, society, and the state. It situates and deconstructs contested notions of identity, citizenship, and nationality in the Jordanian context and considers how these constructions converge to affect the lives of these youth, in both gendered and generational ways. We argue that orphans’ direct action did not yield its expected results because of the social and legal constrictions of their citizenship, bolstered by an ethos of charity and a facade of democracy.

La transmission intergénérationnelle de la violence envers les enfants et les perceptions reliées aux expériences personnelles de violence dans l’enfance ont été peu étudiées auprès de la population générale et encore moins auprès des pères. L’objectif de la présente étude est de déterminer le lien entre, d’une part, la fréquence rapportée de la violence physique sévère vécue dans l’enfance et la légitimité perçue de cette violence et d’autre part, la violence physique mineure et psychologique envers les enfants au sein de la famille actuelle de 204 pères québécois. Les données sont issues de l’enquête sur la violence familiale dans la vie des enfants du Québec réalisée en 2004 par l’Institut de la Statistique du Québec. Il y a un lien significatif entre la fréquence rapportée de la violence physique sévère vécue dans l’enfance et sa légitimité perçue, ainsi qu’avec la violence envers les enfants dans la famille actuelle des pères. Les pères rapportant avoir souvent ou très souvent vécu de la violence physique sévère dans l’enfance ont actuellement des enfants qui vivent plus de violence psychologique que ceux qui en ont parfois vécu. Plus les pères considèrent la violence physique sévère vécue dans l’enfance comme méritée, plus leurs enfants vivent de la violence psychologique. Enfin, les enfants des pères qui considèrent la violence subie comme parfois méritée vivent moins de violence physique mineure que ceux des pères qui la considèrent comme souvent ou très souvent méritée. Les implications pour la recherche et la pratique sont discutées., Intergenerational patterns of violence against children and perceptions of childhood abuse have not been studied a lot among general population or fathers. The objective of the present study is to determine the link between frequencies of childhood severe physical violence and its perceived legitimacy (first), and minor physical and psychological violence against children in actual family (second) in a sample of 204 fathers from Quebec. Results indicate a significant link between frequencies of childhood severe physical violence and its perceived legitimacy, and violence against children in actual family. Fathers who have often or very often lived childhood severe physical violence have children who suffer more psychological violence than fathers who have lived it sometimes. The more fathers think that childhood violence was deserved, the more their children suffer psychological violence. Finally, children of fathers who think that childhood severe physical violence was sometimes deserved suffer less minor physical violence than those of fathers who think that it was often or very often deserved. Implications for research and practice are discussed.
