The paper reports findings from a research study carried out with adult education professionals working in Adult Education Centres (AECs) in Cyprus. It aims to explore how they experience their professional status in the programme as well as identify barriers that hinder their professionalisation and particular barriers caused in the course of the COVID-19 pandemic. The study harnesses qualitative methodology and adopts a bottom-up approach as it gives voice to adult educators and makes meaning out of their working experiences. It makes suggestions for the improvement of their professional status based on the idea of humanisation, a multifaceted process in which both the state and adult educators themselves should become communions.

3. Fast Intra-kernel Isolation and Security with IskiOS

The kernels of operating systems such as Windows, Linux, and MacOS are vulnerable to control-flow hijacking. Defenses exist, but many require efficient intra-address-space isolation. Execute-only memory, for example, requires read protection on code segments, and shadow stacks require protection from buffer overwrites. Intel's Protection Keys for Userspace (PKU) could, in principle, provide the intra-kernel isolation needed by such defenses, but, when used as designed, it applies only to user-mode application code. This paper presents an unconventional approach to memory protection, allowing PKU to be used within the operating system kernel on existing Intel hardware, replacing the traditional user/supervisor isolation mechanism and, simultaneously, enabling efficient intra-kernel isolation. We call the resulting mechanism Protection Keys for Kernelspace (PKK). To demonstrate its utility and efficiency, we present a system we call IskiOS: a Linux variant featuring execute-only memory (XOM) and the first-ever race-free shadow stacks for x86-64. Experiments with the LMBench kernel microbenchmarks display a geometric mean overhead of about 11% for PKK and no additional overhead for XOM. IskiOS's shadow stacks bring the total to 22%. For full applications, experiments with the system benchmarks of the Phoronix test suite display negligible overhead for PKK and XOM, and less than 5% geometric mean overhead for shadow stacks.

The soil pool is the primary sink for carbon in mangrove wetlands and plays a major role in mitigating climate change. However, aquaculture pond expansions go further to disrupt carbon storage in mangroves. The aim of this study is to estimate the stock of soil organic carbon (SOC) in the mangrove area of South Sulawesi, Indonesia. The mangroves of Sinjai District in South Sulawesi are a disturbed region with no previous study on SOC stock. We implemented a line transect method at five study sites, collected 15 soil cores at a depth of 0–15 cm, 15–30 cm, and 30–50 cm, and performed soil analysis using the Loss on Ignition method. We find that the mean value of SOC stock is 413.10±12.37 Mg C ha–1. More attention to the conservation and restoration of lost mangrove areas is a high priority. It may also increase the stock of SOC to mitigate climate change. This study will help to preserve the remaining mangroves.

6. Pustular Eruption following COVID-19 Vaccination: A Narrative Case-Based Review

From the beginning of public vaccinations until the relaxation of COVID-19 measures, many case reports, case series and case–control studies have been published indicating cutaneous side effects of COVID-19 vaccination. Post-vaccination pustular eruption was reported as well, with a challenging differential diagnosis between pustular psoriasis, AGEP (acute generalized exanthematous pustulosis) and neutrophil pustular eruptions. We report a case of 56-year-old woman presented with acute generalized pustular flare up culminated 5 days after the second dose of BNT162b2(Pfizer) vaccination. She was diagnosed with pustular psoriasis flare and due to the regulating role of IL-1 in pustular psoriasis and in the cytokine storm observed in cases of COVID-19 postvaccination inflammation; we decided to treat the patient with an IL-1 antagonist, subcutaneous anakinra (100 mg daily) along with acitretin. One week later, after anakinra withdrawal, she presented a pustular psoriasis flare and a 7-day anakinra re-administration led to a satisfactory improvement in the skin lesions. We also reviewed the medical literature and found 28 case reports with pustular eruption after the COVID-19 vaccination. We compared the patients reported, regarding sex, age, number of doses, post-vaccination period and vaccine brand, and compared those results with our patient. Finally, as indicated by our case and other cases with similarly treated pustular eruptions. targeted therapy to this cytokine imbalance such as anakinra (IL-1) antagonist can improve the clinical course of the patient.

8. Synthesis of nanomaterials via anodic aluminium oxide templates

This thesis is concerned with the synthesis of ID nanomaterials via a template-assisted route. Porous anodic aluminium oxide templates prepared electrochemically have been utilised with two intrinsically different deposition techniques, sol-gel and high power pulsed magnetron sputtering (HPPMS), to obtain ID metal and metal oxide nano wires and nanotubes. The resultant morphologies and crystal structures were examined via SEM, XPS, XRD, TEM and EELS. A number of porous template alumina structures have been grown via the anodisation of pure and sputtered aluminium. The effects of surface pre-treatments, etching treatments and anodisation conditions on the resultant morphologies were investigated. It has been found that pore growth is largely dependent on the surface roughness of the substrate as well as the anodisation conditions. The anodisation duration is critical in promoting and allowing self-ordering. Obtained templates, varied in thickness from a few hundred nm to several tens of pm, with an average pore diameter of 70 nm, interpore distance of 100 nm and pore density of 4 x 1010 cm2 . The implementation of HPPMS led to the successful deposition of Ti inside the alumina template to depths of around 45-50 nm. It was found that templates with highly parallel pores on a rigid substrate such as Si, are more suited if this deposition method is to be used. Control of the pressure and substrate biasing is critical in avoiding ‘pinch-off and ‘bridging’ and leading to complete pore filling. The results have shown that HPPMS is a promising plasma technology for the synthesis of nanomaterials such as nanodots, nanopillars or nanowires, when used with porous alumina templates under appropriate conditions. The use of sol-gel deposition has led to the growth of a number of interesting materials and structures. Nanocrystalline CeO2 and Ce1-xZrxO2 and Ce1-xSmxO2 thin films and powders have been successfully obtained exhibiting novel micro- and nano-structures, likely to find useful applications in catalysis and gas sensing due to their redox properties and large surface to volume ratio. Furthermore, the treatment of porous alumina templates via a sol-gel/hydrothermal method led to the formation of Ce-doped γ-AI2O3 nanowires. Hence, a simple, direct and cost effective method for producing large scale AI2O3 (and doped AI2O3) nanowires is reported. Moreover, by annealing at temperatures above 600°C, nanowires of different crystallographic forms such as d-, 0- and a-AI2O3 can also be readily obtained. As the dopant Ce was successfully introduced through this method a wide range of doped-Al2O3 nanowires (by other rare earths such as Y, La, Gd, Sm), at various concentrations (e.g. 1, 3, 5 at. %) can be readily obtained.

BackgroundB-cell non-Hodgkin’s lymphoma (B-NHL) is one of the major complications of primary Sjögren’s syndrome (SS). Chronic inflammation and macrophages in SS minor salivary glands have been previously suggested as significant predictors for lymphoma development among SS patients. Lipoprotein-associated phospholipase A2 (Lp-PLA2)—a product mainly of tissue macrophages—is found in the circulation associated with lipoproteins and has been previously involved in cardiovascular, autoimmune, and malignant diseases, including lymphoma.ObjectiveThe purpose of the current study was to investigate the contributory role of Lp-PLA2 in B-NHL development in the setting of primary SS.MethodsLp-PLA2 activity in serum samples collected from 50 primary SS patients with no lymphoma (SS-nL), 9 primary SS patients with lymphoma (SS-L), and 42 healthy controls (HC) was determined by detection of [3H]PAF degradation products by liquid scintillation counter. Moreover, additional sera from 50 SS-nL, 28 SS-L, and 32 HC were tested for Lp-PLA2 activity using a commercially available ELISA kit. Lp-PLA2 mRNA, and protein expression in minor salivary gland (MSG) tissue samples derived from SS-nL, SS-L patients, and sicca controls (SC) were analyzed by real-time PCR, Western blot, and immunohistochemistry.ResultsSerum Lp-PLA2 activity was significantly increased in SS-L compared to both SS-nL and HC by two independent methods implemented [mean ± SD (nmol/min/ml): 62.0 ± 13.4 vs 47.6 ± 14.4 vs 50.7 ± 16.6, p-values: 0.003 and 0.04, respectively, and 19.4 ± 4.5 vs 15.2 ± 3.3 vs 14.5 ± 3.0, p-values:

Introduction Mesothelioma remains a lethal cancer. To date, systemic therapy with pemetrexed and a platinum drug remains the only licensed standard of care. As the median survival for patients with mesothelioma is 12.1 months, surgery is an important consideration to improve survival and/or quality of life. Currently, only two surgical trials have been performed which found that neither extensive (extra-pleural pneumonectomy) or limited (partial pleurectomy) surgery improved survival (although there was some evidence of improved quality of life). Therefore, clinicians are now looking to evaluate pleurectomy decortication, the only radical treatment option left.Methods and analysis The MARS 2 study is a UK multicentre open parallel group randomised controlled trial comparing the effectiveness and cost-effectiveness of surgery—(extended) pleurectomy decortication—versus no surgery for the treatment of pleural mesothelioma. The study will test the hypothesis that surgery and chemotherapy is superior to chemotherapy alone with respect to overall survival. Secondary outcomes include health-related quality of life, progression-free survival, measures of safety (adverse events) and resource use to 2 years. The QuinteT Recruitment Intervention is integrated into the trial to optimise recruitment.Ethics and dissemination Research ethics approval was granted by London – Camberwell St. Giles Research Ethics Committee (reference 13/LO/1481) on 7 November 2013. The paper explores the experiences and perceptions of older adults and their young educators regarding Intergenerational Learning (IGL) in the course of a non-formal adult education programme in Cyprus. It aims to identify the forms that IGL takes in the programme as well as any possible components of the programme that foster IGL. The research was located in the Adult Education Centres (AECs), a non-formal adult education programme, offered in the Republic of Cyprus. It is based on a qualitative research design and adopts a bottom-up approach, as it gives voice to older adults and their educators and makes meaning out of their IGL experiences. The results of the study present the different forms that IGL take in the programme under three different axes: the cognitive, the social and the psychological. They view IGL as a multilevel mutual beneficial form of learning for both the older adults and their younger educators. They also highlight the necessity of a learner-centered and cooperative learning approach for fostering IGL.

Background: Support has been gradually acknowledged as a significant factor that exerts a positive influence on hemodialysis patients' lives as well as on the outcome of the disease. The purpose was to explore factors associated with social support of hemodialysis patients. Methods: The sample studied consisted of 258 patients undergoing hemodialysis in public hospitals in Athens, Greece during 2015. Data collection was performed by the method of the interview using a specially designed questionnaire, which included the Multidimensional Scale of Perceived Social Support for the assessment of social support from significant others, family and friends as well as patients’ socio-demographic, clinical and other characteristics. Results: Of the 258 participants, 53.9% were male and 65% were over 60 yr. Regard to social support, patients felt highly supported by their significant others and their family (median 6 for both subscales) and less by their friends (median 4.5 neutral support levels). In terms of patients' characteristics, statistically significant association of support from significant others and family was observed with marital status (PPPPP=0.002 and P=0.007 respectively). Furthermore, statistically significant association of support from friends was observed with age (P P=0.002), and place of residence (P=0.017).Conclusion: Socio-demographic, clinical characteristics and patients' perceptions are associated with perceived Social Support. Provision of holistic individualized care to hemodialysis patients demands assessment of social support in the daily clinical practice.

Objectives RA and primary SS carry increased atherosclerotic risk, while B-cell activating factor holds a vital role in disease pathogenesis and atherosclerosis. We aimed to compare subclinical atherosclerosis profiles between the two clinical entities and define whether BAFF genetic variants alter atherosclerotic risk. Methods DNA from 166 RA, 148 primary SS patients and 200 healthy controls of similar age and sex distribution was subjected to PCR-based assay for the detection of five single nucleotide polymorphisms of the BAFF gene (rs1224141, rs12583006, rs9514828, rs1041569 and rs9514827). Genotype and haplotype frequencies were determined by SNPStats software and statistical analysis was performed by SPSS and Graphpad Software. Subclinical atherosclerosis was defined by the presence of carotid/femoral plaque formation and arterial wall thickening. Results Atherosclerotic plaque formation was more frequently detected in the RA vs primary SS group (80.7% vs 62.2%, P-value Conclusions Increased inflammatory burden, higher steroid doses and distinct BAFF gene variations imply chronic inflammation and B-cell hyperactivity as key contributors for the augmented atherosclerotic risk among autoimmune patients.

This article discusses the findings of qualitative case-study research that looks at pedagogical contexts of adult education programs for migrants in Cyprus, Scotland, Malta, and Estonia. The goal of this research is to understand how Learner-Centered Education (LCE) is promoted within a human-rights framework and implemented as an approach to emancipatory social change. The findings show significant discrepancies in the implementation of LCE. Differences emerged between educational interventions that influence the language-learning and assimilation-integration processes of adults with migrant backgrounds. Informed by these findings, this article builds a case for Emancipatory Learner-Centered Education (ELCE)—a pedagogical approach that goes beyond course adaptations, to engage migrants in consciousness-raising and confidence-building, while fostering communal action between migrants and adult educators.

19. Geospatial analysis of the Varybobi wildfire (2021) effects on hydrology, soil and the environment

Wildfires are frequently observed in watersheds with a Mediterranean climate and seriously affect the formation and degradation of the forest ecosystem (soil, climate, vegetation, hydrology, and ecosystems) as they cause abrupt changes in land use/cover.This paper presents the analysis of the impact of the catastrophic wildfire that occurred in August 2021 in the wider area of Varybobi, Athens, Greece. First, an analysis over the extent and severity of the fire and its effect on the vegetation is conducted. The history of fires in the specific area is then analyzed and regrowth models are developed based on the historical data using satellite imagery. In particular, the restoration of the area is systematically monitored and spatially mapped using both remote sensing methods and on-site observations. Subsequently, the effects on the soils of the area are analyzed and mapped with systematic on-site measurements, laboratory measurements and remote sensing methods. Based on this information, the effect on the hydrological behavior of the affected area is systematically analyzed. Specifically, the effect of the fire on the increase of direct runoff volume and the flood hazard, as well as on the hydrological balance of the respective drainage basins in assessed. The analysis is conducted in a spatially distributed form to delineate the critical areas in which immediate interventions are required for the rapid restoration of the hydrological behavior of the basin. As a last step, the period required for the restoration of the hydrological functioning of the watershed is assessed based on the vegetation regrowth model., The 28th IUGG General Assembly (IUGG2023) (Berlin 2023)

20. Simplified approach to consider watersheds heterogeneity in direct runoff estimation using SCS-CN model

The most established method for direct runoff estimation is the Soil Conservation Service-Curve Number method (SCS-CN). Despite the wide use of this method, there is no established and commonly accepted procedure for estimating the curve number (CN) from rainfall and runoff measurements. Previous studies have shown that the major problem is the large variation observed in the estimated CN values for different rainfall events with different total precipitation depths. A key study showed that this can be attributed to the spatial heterogeneity of watersheds’ physical characteristics and proposed a methodology to estimate the spatial distribution of CN in the watershed based on rainfall - runoff measurements.In this paper, the effect of impervious surfaces on direct runoff estimation with SCS-CN method is investigated and a simplified method considering watersheds heterogeneity in SCS-CN model is proposed. This method considers the spatial distribution of impervious surfaces in the watershed in the process of estimating CN from rainfall and runoff measurements at the watershed outlet. The proposed method has improved performance compared to the original SCS-CN method and similar to that of SCS-CN method in entirely spatially distributed form. Its application is much easier since it does not prerequisite the detailed spatial distribution of soil - land cover characteristics of the watershed and avoids the problems of over-parameterization. Introduction: The preoperative factors that have been studied for their relationship with postoperative pain are the demographic, general and clinical characteristics of the patients, data related to the surgery but also mental health factors.Purpose: The purpose of this literature review was to explore the relationship between psychological factors of anxiety and depression, with postoperative pain, the first postoperative day.Methodology: Research articles about the psychological factors of anxiety and depression were researched as preoperative factors influencing postoperative pain and then a secondary search was performed about the effect of these factors in the pain of the first postoperative day (early postoperative pain). The search was performed in the international databases PubMed, Scopus and Google Scholar, using keywords that served the purpose of the literature review.Results: Several studies were found that correlated anxiety and depression with postoperative pain but there were few studies that correlated these factors with pain on the first postoperative day. Most of the correlations were positive, with the researchers agreeing about the need for preoperative assessment and treatment of patients' mental health prior to surgery that would contribute to more effective postoperative analgesia and hospitalization of surgery patients.Conclusions: Anxiety and depression have a significant effect on the pain experience of the first postoperative day, but additional research is needed to draw conclusions and compare the results, using the same methodology for pain recording times and mental health data collection tools and also postoperative pain questionnaires.

COVID-19 causes significant thrombosis and coagulopathy, with elevated D-dimer a predictor of adverse outcome. The precise mechanism of this coagulopathy remains unclear, one hypothesis is that loss of Angiotensin Converting Enzyme 2 activity during viral endocytosis leads to pro-inflammatory angiotensin II accumulation, loss of angiotensin-1-7 and subsequent vascular endothelial activation. We undertook a double blind randomised, placebo controlled experimental medicine study to assess the effect of TRV027, a synthetic angiotensin-1-7 analogue on D-dimer in 30 patients admitted to hospital with COVID-19 (REC ref. 20/HRA/3414), Clinical Trial No. NCT04419610. The study showed a similar rate of adverse events in TRV027 and control groups. There was a numerical decrease in D-dimer in the TRV027 group and increase in D-dimer in the placebo group, however, this did not reach statistical significance (p=0.15). A Bayesian analysis demonstrated there was a 92% probability that this change represented a true drug effect.

24. Exploring Adult Digital Literacy Using Learners' and Educators' Perceptions and Experiences: The Case of the Second Chance Schools in Greece

The research reported in this paper aspires to shed light into adult digital literacy using learners' and educators' experiences and perceptions at Second Chance Schools, a project in Greece aiming at combating social exclusion through education. In exploring the above, this investigation uses a case-study approach within a qualitative paradigm and draws upon a heuristic that brings together a set of ideas on adult program development to guide research techniques and analysis procedures. The latter focuses on five key elements of program development for adults: needs identification, planning, design, climate, and evaluation. The findings identify the importance placed by the participants on digital literacy and indicate some tentative points and practices that, when suitably adapted, could pave the way for effective preparation and delivery of digital literacy courses for adults. Introduction: The World Health Organization (WHO) describes the prevalence of obesity as a "global epidemic" affecting the population of both developed and developing countries, all socio-economic and age groups.Purpose: To explore characteristics of obese patients who choose the surgical treatment of obesity.Material and method: We studied 100 obese patients who came for scheduled bariatric surgery at the Endoscopic Unit of Bariatric Surgery, of the General Hospital of Athens "Hippokration". On the day of admission, patients’ characteristics were recorded in a specially designed questionnaire that served the purpose of the research. The statistical program SPSS 22.0 was used for data analysis.Results: Of the 100 patients, 61 were women and 39 men, with a mean body mass index (BMI) of 47.58 Kg/m² and a mean age of 40.45 years. The 58 patients underwent Laparoscopic Sleeve Gastrectomy and 42 underwent Laparoscopic One Anastomosis Gastric bypass. The 58% of patients were of "secondary" educational level (22 men, 36 women), 51% were "married" (23 men, 28 women), 51% had a "physical illness" (16 men, 35 women) and 65% had undergone other surgeries (22 men, 43 women). In addition, medication was reported in 44%, chronic pain in 33%, and smoking in 35% of patients.Conclusions: The obese patients who underwent bariatric surgery were about 40 years old, mostly women, married, of secondary educational level and suffering from morbid obesity with concomitant diseases and a history of previous surgeries.

Monomodal systemic glucocorticoids remain the mainstay of treatment for bullous pemphigoid (BP). In this retrospective, single-arm study, we evaluated the feasibility (efficacy and tolerability) of the combination of methylprednisolone and low-dose (up to 12.5 mg/week) methotrexate (MP + MTX) for BP. At week 12, 53/55 (96.4%) patients initiated on MP + MTX during a five-year period (potential follow up time: ≥4 years) remained on treatment. At this time-point, BP remission was achieved in all compliant patients (including n = 24 cases of dipeptidyl peptidase-4 inhibitors-associated BP; 12-week remission rate: 100% [95% CI: 91.9–100.0%]; mean time to remission: 29.5 days, SEM: 2.3 days) at a mean cumulative MP dose to disease control of 678.4 mg (SEM = 49.4 mg). Eight patients relapsed during follow up (10.81 [95% CI: 5.16–21.72] relapses/100 person years, py), and seven manifested a severe adverse event (6.80 [95% CI: 3.00–14.28] severe adverse events/100 py); however, 73.4% (±7.9%) had suffered neither a relapse nor a SAE at the three-years follow up. Continuing low dose MP intake (≤8 mg/day) beyond week 12 in combination with MTX minimized the risk of a feasibility limiting event (p = 0.013). Conclusively, the combination of methylprednisolone with methotrexate is a promising, safe, and efficient modality for BP patients, which enables rapid glucocorticoid tapering.

Objectives RA and primary SS carry increased atherosclerotic risk, while B-cell activating factor holds a vital role in disease pathogenesis and atherosclerosis. We aimed to compare subclinical atherosclerosis profiles between the two clinical entities and define whether BAFF genetic variants alter atherosclerotic risk. Methods DNA from 166 RA, 148 primary SS patients and 200 healthy controls of similar age and sex distribution was subjected to PCR-based assay for the detection of five single nucleotide polymorphisms of the BAFF gene (rs1224141, rs12583006, rs9514828, rs1041569 and rs9514827). Genotype and haplotype frequencies were determined by SNPStats software and statistical analysis was performed by SPSS and Graphpad Software. Subclinical atherosclerosis was defined by the presence of carotid/femoral plaque formation and arterial wall thickening. Results Atherosclerotic plaque formation was more frequently detected in the RA vs primary SS group (80.7% vs 62.2%, P -value <0.001), along with higher rates of family CVD history, current steroid dose and serum inflammatory markers. The TT genotype of the rs1224141 variant was more prevalent in RA but not primary SS patients with plaque and arterial wall thickening vs their counterparts without. Regarding the rs1014569 variant, among RA patients the TT genotype increased the risk for plaque formation while in primary SS patients the AT genotype conferred increased risk. Haplotype GTTTT was protective in the RA cohort, while TATTT and TTCTT haplotypes increased susceptibility for arterial wall thickening in the primary SS cohort. Conclusions Increased inflammatory burden, higher steroid doses and distinct BAFF gene variations imply chronic inflammation and B-cell hyperactivity as key contributors for the augmented atherosclerotic risk among autoimmune patients. Το Πομφολυγώδες Πεμφιγοειδές (ΠΠ) αποτελεί τη συχνότερη επίκτητη αυτοάνοση πομφολυγώδη δερματοπάθεια και πλήττει κυρίως την 3η ηλικία. Η παθογένεσή του διέπεται από αυτοάνοσους μηχανισμούς που στοχοποιούν επιτόπους επί δομών των ημιδεσμοσωματίων της βασικής μεμβράνης και καταλήγουν σε φλεγμονώδη αντίδραση με αποτέλεσμα την αποκόλληση της επιδερμίδας από το χόριο και τον σχηματισμό υποεπιδερμιδικής πομφόλυγας. Η εκδήλωσή του δύναται να σχετίζεται με άλλα υποκείμενα νοσήματα (π.χ. αυτοάνοσα, νεοπλασίες) ή πυροδοτικούς παράγοντες όπως τραύματα ή φάρμακα, μεταξύ των οποίων τα τελευταία χρόνια κυρίαρχο ρόλο φαίνεται να διαδραματίζουν οι αναστολείς του ενζύμου DPP4 (DPP4i) ή αλλιώς γλιπτίνες - υπογλυκαιμικοί παράγοντες χορηγούμενοι σε ασθενείς με Σακχαρώδη Διαβήτη τύπου 2. Οι τελευταίοι αποτελούν μια ιδιαίτερα ευάλωτη ομάδα ασθενών, δεδομένης και της δυνητικής απορρύθμισης του σακχάρου τους λόγω της συνακόλουθης διακοπής της γλιπτίνης καθώς και της πιθανής περαιτέρω επιβάρυνσής τους από τη χορήγηση συστηματικών κορτικοειδών που αποτελούν τον ακρογωνιαίο λίθο της θεραπείας του σοβαρού-εκτεταμένου ΠΠ σύμφωνα με τις υπάρχουσες κατευθυντήριες οδηγίες.Στην παρούσα μελέτη αναδείχθηκε η καταλυτική συμβολή της διαχρονικά αυξανόμενης χρήσης των γλιπτινών στην παρατηρηθείσα αύξηση του επιπολασμού του ΣΔ2 μεταξύ νεοδιαγνωσθέντων περιστατικών ΠΠ κατά τη χρονική περίοδο 2010-2016, συντελώντας στην επίσης παρατηρηθείσα ανοδική τάση του συνόλου των πρωτοδιαγνωσθέντων περιστατικών με ΠΠ στη Δερματολογική κλινική του Πανεπιστημιακού Νοσοκομείου Ιωαννίνων κατά το ίδιο χρονικό διάστημα.Επιπλέον συγκρίθηκαν τα επιδημιολογικά, κλινικά και εργαστηριακά ευρήματα μεταξύ ασθενών με και χωρίς προηγηθείσα λήψη γλιπτίνης. Από τις συγκρίσεις δεν προέκυψαν στατιστικά σημαντικές διαφορές σε δημογραφικές παραμέτρους (ηλικία,φύλο), σε αποτελέσματα εργαστηριακών εξετάσεων που τεκμηριώνουν τη διάγνωση (βιοψία βλαβης, άμεσος/ έμμεσος ανοσοφθορισμός, ELISA), στον φαινότυπο, την κλινική πορεία και τις θεραπευτικές απαιτήσεις για την επίτευξη ελέγχου και διατήρηση της ύφεσης του νοσήματος στο πρώτο τρίμηνο κατά το οποίο είχαμε και τα περισσότερα δεδομένα παρακολούθησης (follow up). Σε ό,τι αφορά τη σύγκριση των συνοδών νοσημάτων δεν παρατηρήθηκε σημαντική συσχέτιση ανάμεσα σε κάποια συννοσηρότητα και στο ιστορικό λήψης γλιπτίνης με μοναδική εξαίρεση τις στενά συσχετιζόμενες με Σακχαρώδη Διαβήτη μεταβολικές παθήσεις (δυσλιπιδαιμία, υπερουριχαιμία, χ2 =10,546, p=0,001).Η συγκριτική ανάλυση του ιστορικού συννοσηροτήτων και συγχορηγούμενων φαρμάκων μεταξύ ασθενών με θετικό ιστορικό λήψης γλιπτίνης, με και χωρίς νόσηση από ΠΠ, ανέδειξε στατιστικά σημαντική την παρατηρηθείσα πιο συχνή συγχορήγηση στατίνης σε ασθενείς που δεν εμφάνισαν το νόσημα παρά την προηγηθείσα λήψη γλιπτίνης. Η παρατήρηση αυτή σε συνδυασμό με δεδομένα από την βιβλιογραφική ανασκόπηση υποστηρίζει έναν σαφή ανοσορρυθμιστικό ρόλο των στατινών, δυνητικά προστατευτικό έναντι της εκδήλωσης του ΠΠ πιθανώς και μεταξύ ασθενών που έχουν κάνει χρήση DPP4i.Κατόπιν διαπίστωσης ότι το θετικό ιστορικό λήψης γλιπτίνης δεν επέφερε διαφοροποιήσεις στη θεραπευτική διαχείριση των ασθενών – πλην της διακοπής του σκευάσματος DPP4i, αξιολογώντας κύρια (ποσοστό επίτευξης ελέγχου νόσου, ποσοστό ασθενών με εκδήλωση υποτροπής) και δευτερεύοντα καταληκτικά σημεία (χρόνος μέχρι έλεγχο νόσου και μέχρι εκδήλωσης 1ης υποτροπής, αθροιστική δόση μεθυλπρεδνιζολόνης και ρυθμός αποκλιμάκωσης της ημερήσιας δόσης μεθυλπρεδνιζολόνης μέχρι τον έλεγχο της νόσου) με στατιστική επεξεργασία των δεδομένων, ανεδείχθη η εφαρμοσιμότητα και η υψηλή (100%) αποτελεσματικότητα της εξαρχής συστηματικής συνδυαστικής χορήγησης μεθυλπρεδνιζολόνης (ΜΡ) και μεθοτρεξάτης (ΜΤΧ) στο σύνολο των ασθενών με επαρκή συμμόρφωση στη θεραπεία. Παράλληλα, η παρακολούθηση θεραπευτικών παραμέτρων κατά το 1οτρίμηνο (ημερήσια δόση ΜΡ και εβδομαδιαία δόση ΜΤΧ στο τέλος του 1ου τριμήνου, αθροιστική δόση ΜΡ μετά από 3 μήνες θεραπείας) επέτρεψε την ανάδειξη της συμβολής της μεθοτρεξάτης ως παράγοντα εξ’οικονόμησης της συνολικής δόσης του χορηγούμενου γλυκοκορτικοστεροειδούς. Επιπρόσθετα, σε μια προσπάθεια μακροχρόνιας αξιολόγησης τηςαποτελεσματικότητας του θεραπευτικού πρωτοκόλλου, αξιοποιώντας τα δεδομένα από την ανάλυση των υποτροπών και στο βαθμό που επέτρεψε ο αναδρομικός χαρακτήρας της παρούσας μελέτης, άξιο επισήμανσης αποτελεί το εύρημα ότι με την έναρξη της παρούσης θεραπείας πάνω από το 50% των ασθενών (73,6±10,2%) παρέμειναν χωρίς υποτροπή για 4 χρόνια μετά τη διάγνωση (Μέθοδος Kaplan-Meier).Το προφίλ ασφαλείας της εφαρμοσθείσας αγωγής ήταν επίσης ικανοποιητικό, κατόπιν αξιολόγησης των εκδηλωθέντων ανεπιθύμητων ενεργειών οι οποίες θα μπορούσαν να αποδοθούν αποκλειστικά στα συστηματικά γλυκοκορτικοστεροειδή [2 υπερήλικες ασθενείς απεβίωσαν (3,77% επί του πληθυσμιακού δείγματος) και σε 4/53ασθενείς (7,55%) η αγωγή τροποποιήθηκε ουσιωδώς].Δεδομένου του αναδρομικού χαρακτήρα της μελέτης, βασικός περιορισμός ήταν η απουσία ομάδας ελέγχου ασθενών αντιμετωπισθέντων με μεθυλπρεδνιζολόνη ως μονοθεραπεία – 1ης γραμμής θεραπεία βάσει κατευθυντηρίων οδηγιών. Για τον λόγο αυτό η αξιολόγηση της αποτελεσματικότητας της εφαρμοσθείσας θεραπευτικής στρατηγικής έγινε αξιοποιώντας διαφορετικά θεωρητικά σενάρια συντεταγμένης ανταπόκρισης στη θεραπεία με ΜΡ στη βάση των κατευθυντηρίων οδηγιών (γρήγορου, ενδιάμεσου και αργού ρυθμού μείωσης ΜΡ) μέσω της σύγκρισης των υπολογισθέντων τιμών σε συγκεκριμένες παραμέτρους έκβασης της εφαρμοσθείσας θεραπείας - καταληκτικά σημεία (χρόνος ελέγχου νόσου, ημερήσια και αθροιστική δόση ΜΡ στο τέλος του 1ου τριμήνου) με τις αντίστοιχες εκτιμηθείσες για κάθε σενάριο. Τα αποτελέσματα των συγκρίσεων επιβεβαίωσαν την εφάμιλλη ή και ενίοτε υπερέχουσα αποτελεσματικότητα της συνδυαστικής αγωγής ως προς τις εξεταζόμενες παραμέτρους.Τέλος, από τη συγκριτική ανάλυση των παραμέτρων έκβασης της θεραπείας για τα διαφορετικά συνδυαστικά σχήματα έναρξης αγωγής που εφαρμόστηκαν παρέχονται ενδείξεις υπέρ της υπεροχής του αρχικού σχήματος 48mg MP+7,5mg MTX στο σύνολο των εξεταζόμενων παραμέτρων.Συμπερασματικά, η παρούσα μελέτη συμβάλλει στην τεκμηρίωση της κλινικής εικόνας των περιστατικών Πομφολυγώδους Πεμφιγοειδούς που σχετίζονται με τη λήψη γλιπτίνης καθώς και στη διερεύνηση μιας αποτελεσματικής αλλά και ασφαλέστερης θεραπευτικής στρατηγικής για τη βελτιστοποίηση της διαχείρισης του νοσήματος με τη μείωση της επιβάρυνσης με συστηματικά χορηγούμενα γλυκοκορτικοστεροειδή, ιδιαίτερα στην ευάλωτη ομάδα των ηλικιωμένων ασθενών με Σακχαρώδη Διαβήτη.

Migration as a political process and migrants as political subjects or as active participants in the educational process are largely absent from the adult education system in the Cypriot context. This article attempts to inquire on this absence by offering a critical analysis of key policy texts and proposals as well as prominent programmes and initiatives that have shaped the recent transformations of adult education in Cyprus. On the one hand, it is important to reflect on the gaps, mishaps and silences of Cypriot educational policies in relation to the challenges posed by migration in the era of globalised mobility. On the other, it is also critical to highlight the connections between adult education policies and the formulation of the state’s policy towards adults’ migration and its concrete initiatives towards fostering a politics of integration. The article offers a critique of the presence situation in Cyprus, arguing that it confines adult migrants to a position where they cannot play a significant role in co-shaping how adult learning is organised, structured and implemented in Cyprus.

With this review we firstly describe uncommon manifestations of paraprotein-associated hematological diseases in 2 patients presenting with hemoptysis. The differential diagnosis in both cases included qualitative and quantitative platelet abnormalities, infectious diseases, malignancies, vasculitis, collagen diseases and blood disorders. Among other interventions undertaken, bone marrow biopsy provided evidence of a primary hematological disease while serum protein electrophoresis and immunofixation indicated the presence of a paraprotein. As a “take-away” lesson from the literature review performed on the basis of these cases the bleeding diathesis that is occasionally associated with paraproteinemia should be pointed out, irrespectively of the primary hematological condition. Thus, while investigating a patient with bleeding diathesis, at least a serum protein electrophoresis should be among the requested laboratory tests. Moreover, clinical vigilance is required in order to recognize other unusual manifestations of not so unusual hematological abnormalities.

49. Anxiety and depression affect early postoperative pain dimensions after bariatric surgery

- Abstract
Uncontrolled postoperative pain and prolonged immobilization after bariatric surgery have been associated with increased postoperative complications and prolonged hospitalization. The aim of our study was to evaluate the postoperative pain that follows bariatric surgery and iden-tify any psychological factors that may affect the early postoperative perception of pain. The study included 100 patients with obesity (women, n = 61; age 37.4 ± 9.9 years, mean ± standard deviation; Body Mass Index (BMI)47.6 ± 6.5 kg/m²) who underwent bariatric surgery. Preoperative anxiety and depression were evaluated by the Hospital Anxiety and Depression Scale (HADS), and the quanti-tative and qualitative dimension of early postoperative pain were evaluated by the McGill Pain Questionnaire Short Form (MPQ-SF). Furthermore, the postoperative analgesia protocol was rec-orded for each patient. Pain declined gradually during the first 24 h postoperative. Although preoperative anxiety had no correlation with the overall pain of postoperative Day 0, patients with a higher level of preoperative anxiety had significantly more intense and more unpleasant pain at 1 h post operation. In addition, depression influences both the intensity and unpleasantness of pain at different time points (1 h, 4 h and 24 h postoperative). Preoperative pain correlated with educational level, but not with age, BMI, gender, marital status, smoking and surgery type. In conclusion, preoperative anxiety and depression influence the early postoperative pain after bariatric surgery, and their preoperative identification is of major importance to enhance the implementation of fast-track postoperative protocols to prevent complications and prolonged hospitalization. © 2020 by the authors.
