Contains fulltext : 291411.pdf (Publisher’s version ) (Open Access) Functional neurological disorder (FND) is a common and disabling disorder, often misunderstood by clinicians. Although viewed sceptically by some, FND is a diagnosis that can be made accurately, based on positive clinical signs, with clinical features that have remained stable for over 100 years. Despite some progress in the last decade, people with FND continue to suffer subtle and overt forms of discrimination by clinicians, researchers and the public. There is abundant evidence that disorders perceived as primarily affecting women are neglected in healthcare and medical research, and the course of FND mirrors this neglect. We outline the reasons why FND is a feminist issue, incorporating historical and contemporary clinical, research and social perspectives. Background Studies have reported elevated rates of dissociative symptoms and comorbid dissociative disorders in functional neurological disorder (FND); however, a comprehensive review is lacking. Aims To systematically review the severity of dissociative symptoms and prevalence of comorbid dissociative disorders in FND and summarise their biological and clinical associations. Method We searched Embase, PsycInfo and MEDLINE up to June 2021, combining terms for FND and dissociation. Studies were eligible if reporting dissociative symptom scores or rates of comorbid dissociative disorder in FND samples. Risk of bias was appraised using modified Newcastle-Ottawa criteria. The findings were synthesised qualitatively and dissociative symptom scores were included in a meta-analysis (PROSPERO CRD42020173263). Results Seventy-five studies were eligible (FND n = 3940; control n = 3073), most commonly prospective case-control studies (k = 54). Dissociative disorders were frequently comorbid in FND. Psychoform dissociation was elevated in FND compared with healthy (g = 0.90, 95% CI 0.66-1.14, I 2 = 70%) and neurological controls (g = 0.56, 95% CI 0.19-0.92, I 2 = 67%). Greater psychoform dissociation was observed in FND samples with seizure symptoms versus healthy controls (g = 0.94, 95% CI 0.65-1.22, I 2 = 42%) and FND samples with motor symptoms (g = 0.40, 95% CI −0.18 to 1.00, I 2 = 54%). Somatoform dissociation was elevated in FND versus healthy controls (g = 1.80, 95% CI 1.25-2.34, I 2 = 75%). Dissociation in FND was associated with more severe functional symptoms, worse quality of life and brain alterations. Conclusions Our findings highlight the potential clinical utility of assessing patients with FND for dissociative symptomatology. However, fewer studies investigated FND samples with motor symptoms and heterogeneity between studies and risk of bias were high. Rigorous investigation of the prevalence, features and mechanistic relevance of dissociation in FND is needed.

BackgroundWe examined demographic, clinical, and psychological characteristics of a large cohort (n = 368) of adults with dissociative seizures (DS) recruited to the CODES randomised controlled trial (RCT) and explored differences associated with age at onset of DS, gender, and DS semiology.MethodsPrior to randomisation within the CODES RCT, we collected demographic and clinical data on 368 participants. We assessed psychiatric comorbidity using the Mini-International Neuropsychiatric Interview (M.I.N.I.) and a screening measure of personality disorder and measured anxiety, depression, psychological distress, somatic symptom burden, emotional expression, functional impact of DS, avoidance behaviour, and quality of life. We undertook comparisons based on reported age at DS onset (v. ⩾40), gender (male v. female), and DS semiology (predominantly hyperkinetic v. hypokinetic).ResultsOur cohort was predominantly female (72%) and characterised by high levels of socio-economic deprivation. Two-thirds had predominantly hyperkinetic DS. Of the total, 69% had ⩾1 comorbid M.I.N.I. diagnosis (median number = 2), with agoraphobia being the most common concurrent diagnosis. Clinical levels of distress were reported by 86% and characteristics associated with maladaptive personality traits by 60%. Moderate-to-severe functional impairment, high levels of somatic symptoms, and impaired quality of life were also reported. Women had a younger age at DS onset than men.ConclusionsOur study highlights the burden of psychopathology and socio-economic deprivation in a large, heterogeneous cohort of patients with DS. The lack of clear differences based on gender, DS semiology and age at onset suggests these factors do not add substantially to the heterogeneity of the cohort.

A apropriação do método e das ferramentas do design estratégico que auxiliam o designer ativista na sua atuação em redes de projeto para inovação social. A exploração por meio de prática do Ecossistema inspira uma revisão teórica dos temas: design para inovação social, práticas de co-design, ecossistemas e comunidades criativas, cenários orientadores de design e, por fim o Social Innovation Journey - modelo de atuação de design no campo da inovação social, elaborado por Fassi, Meroni e Simeone (2013). Tal jornada se apresenta como uma proposta a ser apropriada e redesenhada para uso do Ecossistema da Moda Sustentável, a fim de suprir suas necessidades específicas e ajudar a superar os desafios que o grupo encontra nos seus processos e na potencialização das suas ações. A experimentação prática e revisão de bibliografia apontam o problema de pesquisa, como prolongar a existência de projetos de design no campo da inovação social? O trabalho foca na permanência do designer, em seu papel de ativista social, como fator chave no prolongamento desses projetos de inovação social. O modelo do Social Innovation Journey é criticado e redesenhado para atender às necessidades específicas do Ecossistema da Moda Sustentável. Através da produção de cenários orientadores de design a partir do modelo, conseguem facilitar o diálogo e a cocriação entre designer e artesão, os quais resultam não só no prolongamento dessa rede de projeto, como também em real inovação social na região e na vida dos participantes em situação de vulnerabilidade social, que usam o artesanato como forma de renda. The present work presents a project journey within the Ecossistema da Moda Sustentável project, developed by a diverse group of supporters, whose purpose is to disseminate knowledge about sustainable fashion in Rio Grande do Sul. The appropriation of the method and tools of strategic design that help the activist designer in his role in project networks for social innovation. Exploration through the practice of the Ecosystem inspires a theoretical review of the themes: design for social innovation, co-design practices, ecosystems and creative communities, design guiding scenarios and, finally, the Social Innovation Journey - model of design performance in the field of social innovation, prepared by Fassi, Meroni and Simeone (2013). Such a journey presents itself as a proposal to be appropriate and redesigned for the use of the Ecossistema da Moda Sustentável, in order to meet its specific needs and help to overcome the challenges that the group encounters in its processes and in the enhancement of its actions. Practical experimentation and bibliography review point out the research problem, how to prolong the existence of design projects in the field of social innovation? The work focuses on the permanence of the designer, in his role as a social activist, as a key factor in the continuation of these social innovation projects. The Social Innovation Journey model is criticized and redesigned to meet the specific needs of the Ecossistema da Moda Sustentável. Through the production of design-oriented scenarios based on the model, they are able to facilitate dialogue and co-creation between designer and craftsman, which result not only in the extension of this project network, but also in real social innovation in the region and in the lives of the participants in situations of social vulnerability, who use handicrafts as a form of income.
