PURPOSE: Magrolimab is a first-in-class humanized monoclonal antibody against cluster of differentiation 47, an antiphagocytic signal used by cancer cells to evade phagocytosis. Azacitidine upregulates prophagocytic signals on AML cells, further increasing phagocytosis when combined with magrolimab. We report final phase Ib data for magrolimab with azacitidine in patients with untreated AML ineligible for intensive chemotherapy (ClinicalTrials.gov identifier: NCT03248479). PATIENTS AND METHODS: Patients with previously untreated AML, including TP53-mutant AML, received magrolimab intravenously as an initial dose (1 mg/kg, days 1 and 4), followed by 15 mg/kg once on day 8 and 30 mg/kg once weekly or every 2 weeks as maintenance. Azacitidine 75 mg/m2 was administered intravenously/subcutaneously once daily on days 1-7 of each 28-day cycle. Primary end points were safety/tolerability and proportion with complete remission (CR). RESULTS: Eighty-seven patients were enrolled and treated; 72 (82.8%) had TP53 mutations with a median variant allele frequency of 61% (range, 9.8-98.7). Fifty-seven (79.2%) of TP53-mutant patients had European LeukemiaNet 2017 adverse-risk cytogenetics. Patients received a median of 4 (range, 1-39) cycles of treatment. The most common treatment-emergent adverse events included constipation (49.4%), nausea (49.4%), and diarrhea (48.3%). Thirty (34.5%) experienced anemia, and the median hemoglobin change from baseline to first postdose assessment was -0.9 g/dL (range, -3.6 to 2.5 g/dL). Twenty-eight (32.2%) patients achieved CR, including 23 (31.9%) patients with TP53 mutations. The median overall survival in TP53-mutant and wild-type patients were 9.8 months and 18.9 months, respectively. CONCLUSION: Magrolimab with azacitidine was relatively well tolerated with promising efficacy in patients with AML ineligible for intensive induction chemotherapy, including those with TP53 mutations, warranting further evaluation of magrolimab with azacitidine in AML. The phase III randomized ENHANCE-2 (ClinicalTrials.gov identifier: NCT04778397) and ENHANCE-3 (ClinicalTrials.gov identifier: NCT05079230) studies are recruiting frontline patients with AML.

Elevated air movement produced by fans can offset air-conditioning energy requirements by allowing temperature setpoints to be raised without compromising thermal comfort. These advantages are even greater in hot and humid climates that inherently have large and sustained indoor cooling requirements. Few studies have assessed the in-situ benefits of fans in actual buildings. We installed ceiling and desk fans into a Zero Energy office building (675 m2) in Singapore. Across an 11-week period, 35 occupants alternated between two conditions (no fan vs. fan): 24 °C setpoint with fans off, and 26.5 °C setpoint with fans on. When the temperature setpoint was raised and elevated air movement was provided, a 32% energy reduction was obtained. The energy savings accrued without any negative impacts occurring on thermal satisfaction. Overcooling caused by thermal preference to slightly warmer and warmer conditions was substantially reduced from 33 to 9%. No changes in perceived air-staleness or self-reported alertness and ability to concentrate occurred either, indicating parity across the no fan and fan conditions. Although occupants primarily relied on ceiling fans at the 26.5 °C setpoint, they were by default on at the beginning of each day, giving less incentive to use the desk fans. Our study took place in a high-performance Zero Energy building, whereby thermal dissatisfaction was already low (7%). Therefore, notable changes did not occur, but significant improvements to thermal comfort could still occur in buildings that are unable to maintain high levels of thermal satisfaction.

Cluster of differentiation 47 blockade by magrolimab promotes macrophage-mediated phagocytosis of tumor cells and is synergistic with azacitidine, which increases expression of eat-me signals. We report final phase Ib data in patients with untreated higher-risk myelodysplastic syndromes (MDS) treated with magrolimab and azacitidine (ClinicalTrials.gov identifier: NCT03248479). PATIENTS AND METHODS: Patients with previously untreated Revised International Prognostic Scoring System intermediate-/high-/very high-risk MDS received magrolimab intravenously as a priming dose (1 mg/kg) followed by ramp-up to a 30 mg/kg once-weekly or once-every-2-week maintenance dose. Azacitidine 75 mg/m2 was administered intravenously/subcutaneously once daily on days 1-7 of each 28-day cycle. Primary end points were safety/tolerability and complete remission (CR) rate. RESULTS: Ninety-five patients were treated. Revised International Prognostic Scoring System risk was intermediate/high/very high in 27%, 52%, and 21%, respectively. Fifty-nine (62%) had poor-risk cytogenetics and 25 (26%) had TP53 mutation. The most common treatment-emergent adverse effects included constipation (68%), thrombocytopenia (55%), and anemia (52%). Median hemoglobin change from baseline to first postdose assessment was -0.7 g/dL (range, -3.1 to +2.4). CR rate and overall response rate were 33% and 75%, respectively. Median time to response, duration of CR, duration of overall response, and progression-free survival were 1.9, 11.1, 9.8, and 11.6 months, respectively. Median overall survival (OS) was not reached with 17.1-month follow-up. In TP53-mutant patients, 40% achieved CR with median OS of 16.3 months. Thirty-four patients (36%) had allogeneic stem-cell transplant with 77% 2-year OS. CONCLUSION: Magrolimab + azacitidine was well tolerated with promising efficacy in patients with untreated higher-risk MDS, including those with TP53 mutations. A phase III trial of magrolimab/placebo + azacitidine is ongoing (ClinicalTrials.gov identifier: NCT04313881 [ENHANCE]).

BackgroundPediatric tuberculous meningitis (TBM) commonly causes death or disability. In adults, high-dose rifampicin may reduce mortality. The role of fluoroquinolones remains unclear. There have been no antimicrobial treatment trials for pediatric TBM.MethodsTBM-KIDS was a phase 2 open-label randomized trial among children with TBM in India and Malawi. Participants received isoniazid and pyrazinamide plus: (i) high-dose rifampicin (30 mg/kg) and ethambutol (R30HZE, arm 1); (ii) high-dose rifampicin and levofloxacin (R30HZL, arm 2); or (iii) standard-dose rifampicin and ethambutol (R15HZE, arm 3) for 8 weeks, followed by 10 months of standard treatment. Functional and neurocognitive outcomes were measured longitudinally using Modified Rankin Scale (MRS) and Mullen Scales of Early Learning (MSEL).ResultsOf 2487 children prescreened, 79 were screened and 37 enrolled. Median age was 72 months; 49%, 43%, and 8% had stage I, II, and III disease, respectively. Grade 3 or higher adverse events occurred in 58%, 55%, and 36% of children in arms 1, 2, and 3, with 1 death (arm 1) and 6 early treatment discontinuations (4 in arm 1, 1 each in arms 2 and 3). Higher education has been positively linked with increased opportunity for women, including enhanced employability, increased migration, enriched cultural capital, and improved language skills. With the number of international students rising, understanding postdegree intentions is increasingly important for institutions, policy makers, and administrators. This qualitative study explored the postdegree intentions of female international undergraduate students at the Malaysian campus of an Australian university. In-depth interviews were conducted with students from a range of degree programs and data was studied using thematic analysis. Findings revealed that postdegree intentions were substantially influenced by other people and policies; a common aspiration was to balance career and family; postdegree intentions were not solely career-focused; and the students anticipated discrimination and inequality but were determined to successfully navigate these.

MultiPaxos and Raft are the two most popular and widely deployed state machine replication protocols. There is a more sophisticated family of generalized multi-leader state machine replication protocols like EPaxos, Caesar, and Atlas that have better performance, but they are extremely complicated and hard to understand. Due to their complexity, they have seen little to no industry adoption, and academically there has been a lack of clarity in analyzing, comparing, and extending the protocols. This paper is a tutorial on generalized multi-leader protocols. We explain why the protocols work the way they do, what they have in common, where they differ, which parts of the protocols are straightforward, which are more subtle than they appear, and so on. In doing so, we present four new generalized multi-leader protocols, identify key insights into existing protocols, and taxonomize the space.

State machine replication protocols, like MultiPaxos and Raft, are at the heart of numerous distributed systems. To tol- erate machine failures, these protocols must replace failed machines with new machines, a process known as reconfigu- ration. Reconfiguration has become increasingly important over time as the need for frequent reconfiguration has grown. Despite this, reconfiguration has largely been neglected in the literature. In this paper, we present Matchmaker Paxos and Matchmaker MultiPaxos, a reconfigurable consensus and state machine replication protocol respectively. Our protocols can perform a reconfiguration with little to no impact on the latency or throughput of command processing; they can per- form a reconfiguration in a few milliseconds; and they present a framework that can be generalized to other replication pro- tocols in a way that previous reconfiguration techniques can not. We provide proofs of correctness for the protocols and optimizations, and present empirical results from an open source implementation showing that throughput and latency do not change significantly during a reconfiguration.

The limited availability of human heart tissue and its complex cell composition are major limiting factors for the reliable testing of drug efficacy and toxicity. Recently, we developed functional human and pig heart slice biomimetic culture systems that preserve the viability and functionality of 300 μm heart slices for up to 6 days. Here, we tested the reliability of this culture system for testing the cardiotoxicity of anti-cancer drugs. We tested three anti-cancer drugs (doxorubicin, trastuzumab, and sunitinib) with known different mechanisms of cardiotoxicity at three concentrations and assessed the effect of these drugs on heart slice viability, structure, function and gene expression. Slices incubated with any of these drugs for 48 h showed diminished in viability as well as loss of cardiomyocyte structure and function. Mechanistically, RNA sequencing of doxorubicin-treated tissues demonstrated a significant downregulation of cardiac genes and upregulation of oxidative stress responses. Trastuzumab treatment downregulated cardiac muscle contraction-related genes consistent with its clinically known effect on cardiomyocytes. Interestingly, sunitinib treatment resulted in significant downregulation of angiogenesis-related genes, in line with its mechanism of action. Similar to hiPS-derived-cardiomyocytes, heart slices recapitulated the expected toxicity of doxorubicin and trastuzumab, however, slices were superior in detecting sunitinib cardiotoxicity and mechanism in the clinically relevant concentration range of 0.1-1 μM. These results indicate that heart slice culture models have the potential to become a reliable platform for testing and elucidating mechanisms of drug cardiotoxicity.

Increasingly, universities in developed countries are engaging in transnational education. Responsibilities and opportunities to exercise management and leadership in the provision of transnational education depend on the organisational model adopted and whether the academics involved are on home or international campuses. Models range from neocolonial control to transnational partnerships. In the Australian Office for Learning and Teaching study that informs this paper, good practice in allocation and exercise of management and leadership responsibilities was identified and recommendations developed. A balance was struck between the home institution's quality assurance obligations, which imply a high level of home-based control, and the value of a degree of local control to the commitment of local academics involved, to their career opportunities, and to the educational experiences of their students.

In constructivist teaching, it is expected that students are able to apply skills and knowledge acquired from their course of study to the various situations that they encounter over the course of their professional lives. Constructivist classrooms engage learners actively in the learning process. Learners actively take knowledge, connect it to previously assimilated knowledge and make it theirs by constructing their own interpretation (Cheek, 1992). Formulating one's teaching philosophy requires the reflecting on personal beliefs and pedagogies about teaching and learning, and reviewing and evaluating teaching approaches, and visions for teaching. It necessitates aligning personal values with students' learning needs and setting out clear objectives for teaching. In the constructivist model, students are urged to be actively involved in their own process of learning, on the assumption that individuals construct knowledge instead of receiving it from others. The way in which knowledge is conceived and acquired, the types of knowledge, skills, and activities emphasized, the role of the learner and the teacher, how goals are established: All of these factors are expressed differently from the constructivist perspective (Christie & Stone, 1999). This paper proposes a framework for applying constructivist practices in a social peer learning environment while analyzing its strengths and limitations. (Contains 1 table and 1 figure.)

There is growing evidence that the lack of competence of university ESL (English as a second language) students in academic writing affects their overall academic performance. Olivas and Li (2006) connected low second-language proficiency levels in English to poor academic performance of international students studying at both university and college levels in the United States. Although, many ESL students at university have a general understanding of grammar rules, not many are able to write academically at levels expected of them. This is further exacerbated by a lack of awareness of students' own ability in academic writing. The paper reports on a case study conducted to identify critical gaps in academic writing standards among ESL students in a foundation studies programme. The study employed a pragmatic case study approach, drawing on qualitative methods as deemed appropriate. In this project, four essential criteria for developing good academic writing skills were investigated: attitudes towards academic writing tasks, planning, writing paragraphs and essays, and evaluating one's own writing. The study examined the challenges faced by students in academic writing and identified common grammatical, structural and syntactic errors made in writing tasks. Data from the study showed that most students enjoyed writing tasks, drafting essays and working with peers to brainstorm ideas and opinions for their drafts. A majority of respondents agreed that they were well aware of referencing systems and the need to substantiate their ideas with supportive evidence. However, many respondents were unable to evaluate their own work and admitted that their evaluation often did not match that of their instructors. The project aims to propose interventions and techniques to support student academic writing practices in the foundation year. (Contains 7 tables and 5 figures.)
