Arbovirosis; Vigilància epidemiològica; Espanya Arbovirosis; Vigilancia epidemiológica; España Arbovirosis; Epidemiologic surveillance; Spain Objetivo Valorar los resultados obtenidos por una red de vigilancia epidemiológica y asistencial de arbovirosis compuesta por médicos y profesionales de enfermería de hospital y atención primaria (AP) formados en su identificación, confirmación diagnóstica y manejo clínico. Emplazamiento Zona Sanitaria Metropolitana Norte de Barcelona (1.400.000 habitantes; Cataluña, España) durante un año natural. Participantes Diecisiete médicos (7 de AP y 10 hospitalarios) más 4 enfermeros/as de AP. Tipo de estudio Estudio observacional prospectivo. Mediciones principales Se definieron variables demográficas, epidemiológicas (caso autóctono/importado, sospechoso/probable/confirmado) y asistenciales (síntomas, perfil serológico, periodo virémico). Resultados De los 34 pacientes identificados cumplían criterios de estudio 26 (76,5%) casos; de ellos, se confirmó alguna arbovirosis en 14 (53,8%): 13 fiebres dengues más 1 chikungunya. No se registraron casos de fiebre de zika. Existían antecedentes de viaje a zonas endémicas (23; 88,4%), pero no en 3 casos (11,6%), en los que se consideró la posibilidad de una transmisión autóctona; de ellos, se confirmó un caso de dengue. La incidencia estimada de arbovirosis fue de 0,4 (IC 95%: 0,33-0,51) casos × 10.000 hab./año, que, comparada con la incidencia estimada en la misma área geográfica durante el periodo 2009-2013 (0,19 casos × 10.000 hab./año; IC 95%: 0,07-0,31), mostró un incremento significativo (p = 0,044). Los pacientes en periodo de viremia al momento de la primera visita médica fueron 11 (42,3%). Conclusiones Un programa de vigilancia epidemiológica intensificada definido a nivel de AP y hospitalario es capaz de detectar significativamente más casos de arbovirosis importadas y transmitidas autóctonamente. Posiblemente asistimos a un aumento en la incidencia de arbovirosis importadas, por lo que las medidas encaminadas a su identificación y confirmación deben reforzarse. Objective To evaluate the results obtained by a surveillance network on arbovirosis composed by doctors and nurses located at hospitals and Primary Care trained in their identification, diagnostic confirmation and clinical management. Location North Metropolitan Area of Barcelona (1,400,000 inhabitants; Catalonia; Spain) during a calendar year. Participants Seven Primary Care and 10 hospital physicians plus 4 Primary Care nurses. Type of study A prospective observational study. Main measurements Demographic, epidemiological (autochthonous/imported, suspect/probable/confirmed case) and healthcare variables (symptoms, serological profile, viral period) were defined. Results Of the 34 patients identified, 26 (76.5%) met study criteria. Among them, any arbovirosis was confirmed in 14 (53.8%): 13 dengue plus 1 chikungunya fever. There were no cases of Zika fever. There was a history of travel to endemic areas 23 (88.4%), but not in 3 cases (11.6%) in which the possibility of an indigenous transmission was considered; of them, a case of dengue was confirmed. The estimated incidence of arbovirosis was 0.4 (95% CI: 0.33-0.51) cases × 10,000 hab/year which, when compared to the estimated incidence in the same geographical area during the period 2009-2013 (0.19 cases × 10,000 hab/year; 95% CI: 0.07-0.31), a significant increase was found ( P = .044). Patients within viremia period at the time of their first medical visit were 11 (42.3%). Conclusions An intensified epidemiological surveillance program defined at Primary Care and hospital levels is able to detect significantly more cases of imported and autochthonous arbovirosis. Possibly we are witnessing an increase in the incidence of imported arbovirosis and, thus, measures aimed at their identification and confirmation should be reinforced. BACKGROUND: Convalescent plasma has been proposed as an early treatment to interrupt the progression of early COVID-19 to severe disease, but there is little definitive evidence. We aimed to assess whether early treatment with convalescent plasma reduces the risk of hospitalisation and reduces SARS-CoV-2 viral load among outpatients with COVID-19. METHODS: We did a multicentre, double-blind, randomised, placebo-controlled trial in four health-care centres in Catalonia, Spain. Adult outpatients aged 50 years or older with the onset of mild COVID-19 symptoms 7 days or less before randomisation were eligible for enrolment. Participants were randomly assigned (1:1) to receive one intravenous infusion of either 250-300 mL of ABO-compatible high anti-SARS-CoV-2 IgG titres (EUROIMMUN ratio =6) methylene blue-treated convalescent plasma (experimental group) or 250 mL of sterile 0·9% saline solution (control). Randomisation was done with the use of a central web-based system with concealment of the trial group assignment and no stratification. To preserve masking, we used opaque tubular bags that covered the investigational product and the infusion catheter. The coprimary endpoints were the incidence of hospitalisation within 28 days from baseline and the mean change in viral load (in log(10) copies per mL) in nasopharyngeal swabs from baseline to day 7. The trial was stopped early following a data safety monitoring board recommendation because more than 85% of the target population had received a COVID-19 vaccine. Primary efficacy analyses were done in the intention-to-treat population, safety was assessed in all patients who received the investigational product. This study is registered with ClinicalTrials.gov, NCT04621123. FINDINGS: Between Nov 10, 2020, and July 28, 2021, we assessed 909 patients with confirmed COVID-19 for inclusion in the trial, 376 of whom were eligible and were randomly assigned to treatment (convalescent plasma n=188 [serum antibody-negative n=160]; placebo n=188 [serum antibody-negative n=166]). Median age was 56 years (IQR 52-62) and the mean symptom duration was 4·4 days (SD 1·4) before random assignment. In the intention-to-treat population, hospitalisation within 28 days from baseline occurred in 22 (12%) participants who received convalescent plasma versus 21 (11%) who received placebo (relative risk 1·05 [95% CI 0·78 to 1·41]). The mean change in viral load from baseline to day 7 was -2·41 log(10) copies per mL (SD 1·32) with convalescent plasma and -2·32 log(10) copies per mL (1·43) with placebo (crude difference -0·10 log(10) copies per mL [95% CI -0·35 to 0·15]). One participant with mild COVID-19 developed a thromboembolic event 7 days after convalescent plasma infusion, which was reported as a serious adverse event possibly related to COVID-19 or to the experimental intervention. INTERPRETATION: Methylene blue-treated convalescent plasma did not prevent progression from mild to severe illness and did not reduce viral load in outpatients with COVID-19. Therefore, formal recommendations to support the use of convalescent plasma in outpatients with COVID-19 cannot be concluded. FUNDING: Grifols, Crowdfunding campaign YoMeCorono.

Background Dermatophytoses in children are common pathologies worldwide caused mainly by Trichophyton rubrum. However, due to the globalization and the atypical pets that people nowadays own, some zoonotic species are also involved in these lesions. Case report We present two cases of tinea faciei caused by the zoonotic mould Trichophyton erinacei in two children that owned a guinea pig and a hedgehog, respectively. Mycological diagnosis was performed inoculating skin scales on Sabouraud-glucose agar plates supplemented with chloramphenicol, with and without gentamicin, and on Sabouraud-glucose agar tubes, with and without cycloheximide. Microscopical examination in both cases and ITS region sequencing to confirm the identification (performed in one of them) were compatible with T. erinacei. Multiple treatments like corticosteroids and antibiotics were prescribed prior to the accurate diagnosis. Finally, both patients received topical and oral terbinafine, respectively, the lesions being resolved entirely. Conclusions Zoonotic fungi must be considered in the diagnosis of skin lesions. An accurate medical record, with a guided anamnesis about possible risk factors and an ongoing and open dialogue between health professionals, are essential to improve both the management of these exotic and zoophilic dermatophytoses.

Background: Actinotignum schaalii and the genus Aerococcus are considered emerging pathogens, since their isolation has been rising the last years thanks to the improvement of diagnosis techniques, such as the implementation of MALDI-TOF in microbiology laboratories for routine. Their patogenicity is nowadays well described in urinary tract infections affecting susceptible individuals, although both have been isolated from other biological samples. The aim of our study is to evaluate the impact of using mass spectrometry technology on the frequency of isolation of Aerococcus spp and A. schaalii in our hospital. Methods: From January 2014 and December 2015 44.654 urines were collected in our laboratory from patients that were expected to have an UTI. Samples were processed using a flow cytometer and cultured if applicable. After 48 h, microbial growth was assessed. Due to the suspicion of an Aerococcus spp or A. schaalii infection identification test was performed using Vitek2 until 2014 and MALDI-TOF from 2015. Results: Between the period of study, a total of 35 Aerococcus spp/A. schaalii isolates were collected from 34 patients. Six isolates were identified by Vitek2 and the other 29 were identified by MALDI-TOF. Out of 34 patients, 33 had at least one risk factor including age >65 years, immunosuppression or cancer, abnormality of the genitourinary tract, recurrent UTI, diabetes or a catheterization. Conclusions: Since the implementation of the MALDI-TOF in the laboratory the isolation of Aerococcus spp/A. schaalii has increased almost five times. The most frequent patient corresponds to an elderly patient with recurrent UTI and cancer. Resumen: Introducción: Actinotignum schaalii y el género Aerococcus son considerados patógenos emergentes debido al aumento en los últimos años de su aislamiento gracias a la mejora de las técnicas diagnósticas, como la implementación del MALDI-TOF en la rutina de los laboratorios de microbiología. Su patogenicidad está bien descrita en las infecciones del tracto urinario (ITU) en individuos susceptibles, aunque los dos géneros se han aislado también en otras muestras biológicas. El objetivo de nuestro estudio es evaluar el impacto del uso de la espectrometría de masas en la frecuencia de aislamiento de Aerococcus spp/A. schaalii en nuestro hospital. Métodos: Desde enero 2014 a diciembre 2015 se recibieron 44.654 orinas en nuestro hospital procedentes de pacientes con sospecha de ITU. Las muestras fueron procesadas por citometría de flujo y sembradas según criterios establecidos. Pasadas 48 h, se evaluó el crecimiento. Ante la sospecha de infección por Aerococcus spp/A. schaalii, se realizó un test de identificación con Vitek2® hasta 2014 y con MALDI-TOF desde 2015. Resultados: Durante el periodo de estudio se registraron 35 aislamientos de Aerococcus spp/A. schaalii correspondientes a 34 pacientes. Seis aislados se identificaron por Vitek2® y 29 por MALDI-TOF. De los 34 pacientes, 33 tenían como mínimo un factor de riesgo (>65 años, inmunosupresión o cáncer, anormalidades del tracto urinario, ITU recurrente, diabetes o cateterismo). Conclusiones: Desde la implementación del MALDI-TOF en el laboratorio, el aislamiento de Aerococcus spp/A. schaalii ha aumentado 5 veces. El perfil más afectado es el de un individuo de edad avanzada con ITU recurrentes y cáncer. Despite the low prevalence of parasitic infections in Europe, the diagnosis of intestinal parasites is difficult and laborious for microbiology laboratories. Currently, the antigens detection kits and the molecular biology have allowed an easier diagnosis. But these techniques have also limitations due to the fact that they do not detect all possible parasites presents in the samples. The objective of the study was to evaluate the accuracy and the usefulness of SediMAX2® (77 Elektronika, Budapest, Hungary) automated microscopy in the detection of parasitic structures from feces. A total of 197 formol-fixed stool samples were processed in parallel with wet mount examination and by SediMAX2®. Sensitivities, specificities and predictive values were analyzed, reaching sensitivity of 89.51% and specificity of 98.15%. Predictive values were also calculated with a very good positive predictive value (99.22%). SediMAX2® is a good tool for a reliable diagnosis of intestinal parasitic infections. The easiness of use, processing and the flexibility in the images analyse allows its incorporation in the day to day laboratory work as an extra step for the parasitologists workload.

Background The rapid spread of Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) around the world has caused a global pandemic, infecting millions of individuals, with an unprecedented impact in health care systems worldwide. Healthcare workers are one of the risk groups that need to be well protected, due to their strategic role in patient management, presently and in prevention of healthcare needs for future outbreaks. Here, we present the results of the first SARS-CoV-2 seroprevalence study in the Northern Metropolitan Area of Barcelona, Spain. Methods IgG SARS-CoV-2 antibodies were analyzed in serum samples from 7563 healthcare workers of the Northern Metropolitan Area of Barcelona. Samples were collected after the first pandemic wave (from May 4th to May 22nd, 2020) and were analyzed by automated chemiluminescence assays. All samples were tested for IgG anti-S1/S2. Participant samples with negative or equivocal results but with analytical signals above the limit of detection and/or previously confirmed COVID-19 diagnosis were also tested for IgG anti-Nucleocapsid. Results A total of 779 of 7563 (10.3%) healthcare workers were positive for anti-SARS-CoV-2 IgG (specific for either S1/S2 or N antigens). No significant differences were observed between those working at primary care or at the reference hospital. Interestingly, among 341 participants with a confirmed COVID-19 diagnosis, 36 (10.55%) tested negative for SARS-CoV-2 IgG (both S1/S2 and recombinant N antigen). Conclusion Seroprevalence of anti-SARS-CoV-2 IgG in the healthcare workers of the North Metropolitan Area of Barcelona was higher than in the general population in the same geographical area. Safety measures have to be stressed in order to protect these essential workers from future pandemic waves.

Zika virus belongs to the Flaviridae, an extended phylogenetic family containing dengue or yellow fever, viruses whose shared main vector are Aedes aegypti mosquitoes. The virus originally came from Central African simian reservoirs and, from there, expanded rapidly across the Pacific to South America. The disease is an example of exantematic fever usually mild. Mortality is very low and mainly limited to secondary Guillain–Barré or foetal microcephaly cases. Diagnostic confirmation requires a RT-PCR in blood up to the 5th day from the onset or in urine up to the 10–14th day. Specific IgM are identifiable from the 5th symptomatic day. Clinically, a suspected case should comply with: (a) a journey to epidemic areas; (b) a clinically compatible appearance with fever and skin rash, and (c) a generally normal blood count/basic biochemistry. There is some evidence that causally relates Zika virus infection with foetal microcephaly. While waiting for definitive data, all pregnant women coming from Central or South America should be tested for Zika virus.

Resumen El virus Zika es un Flavivirus filogeneticamente cercano al de la fiebre amarilla o del dengue, cuyo vector principal es el mosquito Aedes aegypti. El virus procede de un reservorio simiano africano y ha protagonizado una expansion fulminante a traves del Pacifico hasta Sudamerica. Provoca una enfermedad leve caracterizada por fiebre con exantema. La mortalidad se circunscribe a casos de Guillain-Barre y de malformacion encefalica fetal con microcefalia. Un caso sospechoso sera aquel con: a) antecedente epidemiologico de desplazamiento a zona endemica; b) cuadro pseudogripal con exantema, y c) hemograma/bioquimica levemente alteradas o normales. La confirmacion diagnostica requiere identificar al virus por RT-PCR en sangre (hasta el quinto dia sintomatico), orina (hasta el dia 10-14) o IgM especificas a partir del quinto dia. Existe alguna evidencia que da soporte a la relacion causa-efecto con la microcefalia fetal. A la espera de datos definitivos, las mujeres embarazadas procedentes de Centro y Sudamerica deben ser testadas para descartar la infeccion.

Early diagnosis of urinary tract infections (UTIs) is essential to avoid inadequate or unnecessary empirical antibiotic therapy. Microbiological confirmation takes 24 to 48 h. The use of screening methods, such as cytometry and automated microscopic analysis of urine sediment, allows the rapid prediction of negative samples. In addition, matrix-assisted laser desorption ionization–time of flight mass spectrometry (MALDI-TOF MS) is a widely established technique in clinical microbiology laboratories used to identify microorganisms. We evaluated the ability of MALDI-TOF MS to identify microorganisms from direct urine samples and the predictive value of automated analyzers for the identification of microorganisms in urine by MALDI-TOF MS. A total of 451 urine samples from patients with suspected UTIs were first analyzed using the Sysmex UF-1000 i flow cytometer, an automatic sediment analyzer with microscopy (SediMax), culture, and then processed by MALDI-TOF MS with a simple triple-centrifuged procedure to obtain a pellet that was washed and centrifuged and finally applied directly to the MALDI-TOF MS plate. The organisms in 336 samples were correctly identified, mainly those with Gram-negative bacteria (86.10%). No microorganisms were misidentified, and no Candida spp. were correctly identified. Regarding the data from autoanalyzers, the best bacteriuria cutoffs were 1,000 and 200 U/μl for UF-1000 i and SediMax, respectively. It was concluded that the combination of a urine screening method and MALDI-TOF MS provided a reliable identification from urine samples, especially in those containing Gram-negative bacteria.

Background Tuberculosis (TB) is a disease of poverty that contributes significantly to ill-health in developing countries. Drug resistant TB is a major challenge to disease control. Early diagnosis and rapid determination of drug sensitivity is of paramount importance in eradication of TB. Although automated liquid culture based methods are available for rapid detection of drug resistance, the high cost of these tests prevent them from being used routinely in low resource settings. This study compares two phenotypic methods, the manual Mycobacteria Growth Indicator Tube (MGIT) and the Nitrate Reductase Assay (NRA) in liquid medium, with the agar proportion method (APM), the gold standard for susceptibility testing of Mycobacterium tuberculosis. Methodology Fourteen day old M. tuberculosis strains (n=373) grown on solid media were used for drug susceptibility testing by APM, NRA and the manual MGIT method. Rifampicin free and rifampicin incorporated (final concentration, 1 μg/ml) media were inoculated with the recommended concentrations of mycobacterial suspensions and incubated at 37°C in 5% CO2. In the APM, the proportion of colonies in the drug containing medium was determined. In the NRA, the colour change in the medium was compared with a standard colour series after day 6 and day 12 of incubation. Growth in the MGIT was detected using the manual MGIT reader from day 2 onwards. The 2 methods were compared with the gold standard, APM to determine sensitivity and specificity and agreement between the methods was calculated using kappa statistics. Results Thirty one (31) rifampicin resistant isolates were identified. When compared with the APM, the sensitivity of detection of rifampicin resistance was 85% for the NRA and 93% for the manual MGIT and the specificity was 99% and 100% respectively. Both assays, NRA (κ=0.86) and manual MGIT method (κ= 0.94) were in excellent agreement with the APM. The mean turnaround time for manual MGIT method and NRA were 08 days and 10 days respectively. Conclusion The NRA in liquid medium and manual MGIT are useful alternatives to APM for drug susceptibility testing of M. tuberculosis in low resource settings.

The nematode Strongyloides stercoralis has a very particular autoinfection life-cycle which leads to chronic infections remaining undetected for decades. However, hyperinfection can occur in patients receiving immunotherapy resulting in high mortality rates. The main objective of this study was to assess the results of a 10-year multicenter surveillance program performed in an area with dense immigration in Barcelona, Spain.From January 2003 to December 2012, all individuals with Strongyloides stercoralis infection attending the four centers with diagnostic capability in the North Metropolitan area of Barcelona were recorded.The annual detection rate was 0.2 new diagnosed cases x10 000 inhabitants/year and 1 case x10 000 immigrants/year. Many patients were immigrants (63; 90.0%), asymptomatic (45; 64.3%) and with a high eosinophil count (63; 90.0%). Immunosuppression was present in 11 (15.7%) patients, among whom two (2.8%) cases of disseminated hyperinfection were recorded. Ivermectin was prescribed in 45 (76.3%) and albendazole in 14 (23.7%). Following treatment seven patients (11.9%) receiving albendazole presented relapse, that is, albendazole failed to clear the parasite in 50% of these drug-treated patients (p0.001).During the study period, 90% of the cases of Strongyloides stercoralis diagnosed could be considered as imported by immigrants, most being asymptomatic and with eosinophilia. The infection is probably largely underestimated and population-based studies are needed to determine its true prevalence. Meanwhile, diagnosis must be based on active investigation of the helminth (serology and feces culture), especially in immunocompromised patients. The implementation of pre-immunosuppression protocols with the aim of identifying Strongyloides stercoralis is encouraged with empirical treatment with ivermectin being recommended in sites without diagnostic facilities.

To describe the infectious diseases imported by immigrants visiting friends and relatives in their countries of origin.We performed a cross-sectional descriptive study of all imported infectious diseases among adult patients between 01/2001-12/2008. The study population was classified in 3 groups: a) immigrants visiting friends and relatives; b) tourists, and c) non-travelling immigrants. Diseases were classified as (a) cosmopolitan or (b) tropical endemic, and, according to their severity as (a) mild, (b) serious, and (c) potentially lethal.A total of 761 patients were analyzed. The mean age was 31.7 (SD: 12.4) years. Of these, there were 90 immigrants visiting friends and relatives, 269 tourists and 402 non-travelling immigrants. Immigrants visiting friends and relatives attended International Health Centres prior to travel significantly less than tourists (32.2% vs. 57.2%; p0.001) and more frequently travelled to sub-Saharan Africa (33.3% vs. 20.4%; p=0.01). Imported diseases considered as potentially lethal were more likely among immigrants visiting friends and relatives than tourists [OR=5.16 (95%CI: 2.08-12.8)] and non-travelling immigrants [OR=7.17 (95%CI: 2.82-18.2)].Immigrants visiting friends and relatives travelled more to sub-Saharan Africa and more frequently imported potentially lethal acute diseases.
