Abstract: Background: A growing body of evidence suggests that non-viral hepatocellular carcinoma (HCC) might benefit less from immunotherapy. Materials and methods: We carried out a retrospective analysis of prospectively collected data from consecutive patients with non-viral advanced HCC, treated with atezolizumab plus bevacizumab, lenvatinib, or sorafenib, in 36 centers in 4 countries (Italy, Japan, Republic of Korea, and UK). The primary endpoint was overall survival (OS) with atezolizumab plus bevacizumab versus lenvatinib. Secondary endpoints were progression-free survival (PFS) with atezolizumab plus bevacizumab versus lenvatinib, and OS and PFS with atezolizumab plus bevacizumab versus sorafenib. For the primary and secondary endpoints, we carried out the analysis on the whole population first, and then we divided the cohort into two groups: non-alcoholic fatty liver disease (NAFLD)/non-alcoholic steatohepatitis (NASH) population and non-NAFLD/NASH population. Results: One hundred and ninety patients received atezolizumab plus bevacizumab, 569 patients received lenvatinib, and 210 patients received sorafenib. In the whole population, multivariate analysis showed that treatment with lenvatinib was associated with a longer OS [hazard ratio (HR) 0.65; 95% confidence interval (CI) 0.44-0.95; P = 0.0268] and PFS (HR 0.67; 95% CI 0.51-0.86; P = 0.002) compared to atezolizumab plus bevacizumab. In the NAFLD/NASH population, multivariate analysis confirmed that lenvatinib treatment was associated with a longer OS (HR 0.46; 95% CI 0.26-0.84; P = 0.0110) and PFS (HR 0.55; 95% CI 0.38-0.82; P = 0.031) compared to atezolizumab plus bevacizumab. In the subgroup of non-NAFLD/NASH patients, no difference in OS or PFS was observed between patients treated with lenvatinib and those treated with atezolizumab plus bevacizumab. All these results were confirmed following propensity score matching analysis. By comparing patients receiving atezolizumab plus bevacizumab versus sorafenib, no statistically significant difference in survival was observed. Conclusions: The present analysis conducted on a large number of advanced non-viral HCC patients showed for the first time that treatment with lenvatinib is associated with a significant survival benefit compared to atezolizumab plus bevacizumab, in particular in patients with NAFLD/NASH-related HCC.

One of the first analysis items is the study of the f radiative decays, which allows us to investigate the nature of light pseudoscalar and scalar mesons. An integrated luminosity of 100 pb−1 is expected for next KLOE run, resulting in 3 × 108 f collected. A detailed simulation and full event reconstruction of both signal and background events indicates a sensitivity reach of 1 ÷ 2% on the branching ratios. Some preliminary studies on real data are also presented, showing a good agreement with MonteCarlo distributions

Abstract: We have measured the ratio R = Γ ( K L → γγ )/ Γ ( K L → π 0 π 0 π 0 ) using the KLOE detector. From a sample of ∼10 9 φ -mesons produced at DAΦNE, the Frascati φ -factory, we select ∼1.6×10 8 K L -mesons tagged by observing K S → π + π − following the reaction e + e − → φ → K L K S . From this sample we select 27,375 K L → γγ events and obtain R =(2.79±0.02 stat ±0.02 syst )×10 −3 . Using the world average value for BR( K L → π 0 π 0 π 0 ), we obtain BR( K L → γγ )=(5.89±0.07±0.08)×10 −4 where the second error is due to the uncertainty on the π 0 π 0 π 0 branching fraction.

Abstract: We have measured the branching ratio BR( φ → π 0 π 0 γ ) with the KLOE detector using a sample of ∼5×10 7 φ decays. φ mesons are produced at DA Φ NE, the Frascati φ -factory. We find BR( φ → π 0 π 0 γ )=(1.09±0.03 stat ±0.05 syst )×10 −4 . We fit the two-pion mass spectrum to models to disentangle contributions from various sources.

Abstract: We present a measurement of the branching ratio BR(KS --> pi e nu) performed using the KLOE detector. KS mesons are produced in the reaction e+ e- --> phi --> KS KL at the DAFNE collider. In a sample of about 5 million KS-tagged events we find 624 +- 30 semileptonic KS decays. Normalizing to the KS --> pi+ pi- count in the same data sample, we obtain BR(KS --> pi e nu) = (6.91 +- 0.37) 10^-4, in agreement with the Standard Model expectation., Comment: 9 pages, 5 Encapsulated Postscript figures. Abstract: The Data Acquisition system for the KLOE experiment, presently running at the Laboratori Nazionali di Frascati DAΦNE collider, has been designed to sustain an acquisition throughput of 50 Mbyte / s for an event rate of 10 kHz . Its two major components are the front end data readout, based on custom buses, and a complex network of computers and storage devices hosting a set of distributed processes. The end result is a seamless data transport from the readout system to the storage library, accompanied by concurrent on line calibrations and data quality control.

Abstract: We have measured the ratio R=Gamma(K_L -> gamma gamma)/ \Gamma(K_L -> 3 pi^0) using the KLOE detector. From a sample of ~ 10^9 phi-mesons produced at DAFNE, the Frascati phi-factory, we select ~ 1.6 10^8 K_L-mesons tagged by observing K_S -> pi^+ pi^- following the reaction e^+ e^- -> phi -> K_L K_S. From this sample we select 27,375 K_L -> gamma gamma events and obtain R = (2.79 \pm 0.02_{stat} \pm 0.02_{syst}) \times 10^{-3}. Using the world average value for BR(K_{L} -> 3 pi^0), we obtain BR(K_{L} -> gamma gamma) = (5.89 \pm 0.07 \pm 0.08) \times 10^{-4} where the second error is due to the uncertainty on the 3 pi^0 branching fraction.

Abstract: We present a study of the reaction e+e- --> pi+pi-pi0 at the phi peak W=M(phi)=1019.4 MeV observed with the KLOE detector at Dafne. The reaction is dominated by phi production and decay, e+e- --> phi --> pi+pi-pi0. From a fit to the Dalitz plot density distribution we obtain the rho-meson parameters for its three charge states. We also find the relative amplitudes for phi --> rhopi and phi -> pi+pi-pi0 and the cross-section for e+e- --> omega pi0 with omega --> pi+pi-.

Abstract: We report the results on the radiative decays phi --> eta'gamma, phi --> pi(0)pi(0)gamma and phi --> etapi(0)gamma obtained with the KLOE detector at the DAPhiNE phi-factory, using a sample of similar to 16 pb(-1). The value of the branching ratio for the phi --> eta'gamma decay is related to the s (s) over bar and gluon content of the eta'. A fit to the two-pseudoscalar invariant mass for the pi(0)pi(0)gamma and etapi(0)gamma final states gives the contribution of the direct terms phi --> f(0)gamma, phi --> sigmagamma and phi --> a(0)gamma, allowing us to investigate the nature of these light scalar mesons. Abstract: In the fixed energy environment of the $e^{+}e^{-}$ collider DA$\Phi$NE, KLOE can measure the cross section of the process $e^{+}e^{-} \to$ hadrons as a function of the hadronic system energy using the radiative return. At energies below 1 GeV, $e^{+}e^{-} \to \rho \to \pi^{+}\pi^{-}$ is the dominating hadronic process. Abstract: We have measured the radiative decays phi --> etagamma, phi --> eta'gamma selecting pi(+)pi(-)gammagammagamma final state in a sample of similar to 5 x 10(7) phi-mesons produced at the Frascati phi-factory DAPhiNE. We obtain Gamma(phi --> eta'gamma)/Gamma(phi --> etagamma) = (4.70 +/- 0.47 +/- 0.31) x 10(-3). From this result we derive new accurate values for the branching ratio BR(phi --> eta'gamma) = (6.10 +/- 0.61 +/- 0.43) x 10(-5) and the mixing angle of pseudoscalar mesons in the flavour basis phi(P) = (41.8(-1.6)(+1.9))degrees.

Details of the design and construction of this detector, together with preliminary results from the test of its full scale prototype, constructed and operated under a test beam at CERN, are presented and discussed.

Abstract: The KLOE detector was designed primarily For the study of CP violation in neutral kaon decays at DAPhiNE, the Frascati Phi-factory. The detector consists of a tracker and an electromagnetic calorimeter. A lead-scintillating-fiber sampling calorimeter satisfies best the requirements of the experiment. providing adequate energy resolution and superior timing accuracy. We describe in the following the construction of the calorimeter, its calibration and how the calorimeter information is used to obtain energy, point of entry and time of the arrival of photons, electrons and charged particles. With e(+)e(-) collision data at DAPhiNE for an integrated luminosity of some 2 pb(-1) we find for electromagnetic showers, an energy resolution of 5.7%/rootE(GeV) and a time resolution of 54/rootE(GeV) ps. Abstract: The design and construction of the large Drift Chamber for the KLOE experiment at the Frascati φ-factory, DAΦNE, are described. The relevant aspects of the various elements of the detector are reviewed together with a description of the track reconstruction program and of the calibration procedures. The performance of the detector based on measurements with cosmic rays and with e+e− colliding beams during DAΦNE commissioning is presented.

Abstract: The quadrupole tile calorimeters of KLOE (QCAL) are two compact detectors placed close to the interaction point and surrounding the focusing quadrupoles. Their purpose is to increase the hermeticity of KLOE calorimetry. Each QCAL consists of a sampling structure of lead plates and scintillator tiles with wavelength shifter (WLS) fibers and mesh photomultiplier readout arranged in 16 azimuthal sectors. The arrangement of WLS fibers allows the measurement of the longitudinal position of the showers from time of flight. In this paper we describe the QCAL design and assembly and present preliminary results obtained with both cosmic rays and photons from KL decays. The time and energy calibration procedures are also discussed in detail.

Abstract: We have measured the branching ratio BR(phi-->pi(0)pi(0)gamma) with the KLOE detector using a sample of similar to5x10(7) phi decays. phi mesons are produced at DAPhiNE, the Frascati phi-factory. We find BR(phi-->pi(0)pi(0)gamma) = (1.09+/-0.03(stat)+/-0.05(syst)) x 10(-4). We fit the two-pion mass spectrum to models to disentangle contributions from various sources. Abstract: We have measured the ratio R-S(pi) = Gamma(K-S --> pi(+)pi(-)(gamma))/Gamma(K-S --> pi(0)pi(0)) with the KLOE detector at the DAPhiNE e(+)e(-) collider. This measurement is fully inclusive with respect to the pi(+)pi(-)gamma final state. The sample of over 106 two-pion decays of tagged K-S mesons allows a statistical error as low as similar to 0.1% to be obtained. The accuracy is limited by systematic uncertainties, which are estimated primarily from data. We find R-S(pi) = 2.236 +/- 0.003(stat) +/- 0.015(syst).

Abstract: A double-level trigger system has been developed for the KLOE experiment. Custom electronics asserts a trigger in a 2 mus decision time. The decision is based on the combined information of the electromagnetic calorimeter and the drift chamber. The entire trigger system is continuously monitored, and data flowing from the trigger system hal e allowed both an efficient online monitoring of the detector and an online luminosity measurement. Abstract: The main aim of the KLOE experiment at DA Phi NE, the Frascati phi -factory, is to study CP violation in the K-0-(K) over bar (0) system. Requirements on shower detection are very stringent. An hermetic, lead-scintillating fiber sampling calorimeter has been chosen and built. A review of the methods used to calibrate and reconstruct energy and timing is reported in this paper. Emphasis is given to the calibration procedures developed using the 2.4 pb(-1) collected in 1999. An energy resolution of 5.7% rootE/GeV is achieved together with a linearity in energy response better than 1% above 50 MeV. A time resolution of similar to 54 ps/ rootE/GeV is also measured on samples of radiative Bhabha and Phi decays. (C) 2001 Elsevier Science B.V. Ail rights reserved.

Abstract: KLOE is acquiring and analyzing hundreds of terabytes of data, stored as tens of millions of files. In order to simplify the access to these files, a URI-based mechanism has been put in place. The KID package is an implementation of that mechanism and is presented in this paper.

Abstract: The KLOE experiment's major physics goal is to measure the R( ϵ′ ϵ ) , with a precision of 10−4, via both the traditional double ratio method and the kaons' quantum interferometry. The experiment has started taking data at DAfNE, the f-factory at the Frascati National Laboratory (LNF) of INFN in Italy, beginning in April 1999. Integrated luminosity exceeding 100 pb−1 (108 f's) are expected to be delivered for end 1999/early 2000. A description of the detector's design and of its performances is presented. At the moment of writing the experiment has collected its first collisions for an integrated luminosity of 220 nb−1; the peak luminosity reached has been of 2 × 1030 cm−2sec−1.

Abstract: KLOE, a new state of the art detector recently commissioned for physics operation at DAFNE, has many innovative interesting features, especially in the DAQ and Trigger areas. Custom electronics assert a trigger in a 2 microseconds decision time and distributes it to the FEE with a 50 ps time resolution. Data are read out using 10 front-end data acquisition chains and sent to a farm of online servers through a FDDI Gigaswitch. This design of the KLOE DAQ system allows us to manage an input data rate as high as 50 Mbyte/s, and is completely scalable by extending the number of computers connected to the switch

30. Predicting survival in patients with hepatocellular carcinoma treated by transarterial chemoembolisation

Abstract: Transarterial chemoembolisation (TACE) is first-line treatment in unresectable hepatocellular carcinoma (HCC) and rescue treatment after failure of radical treatments in early stage HCC. Prognostic tools for HCC using time-fixed Cox models may be unreliable in patients treated with TACE because time-varying predictors interact.To explore time-dependent variables as survival predictors in patients with HCC receiving TACE as first-line or second-line treatment.Eighty four consecutive patients with HCC (mean age 68; male gender 62%; Child-Pugh class: A n=73, B n=11; Barcelona Clinic Liver Cancer class: A n=44, B n=24, C n=16) treated with TACE were enrolled. Clinical, laboratory and radiological follow-up data were collected from the time of first treatment. Time-fixed and time-dependent Cox analyses were done.Overall survival rates were 89.6% (95% CI 82.5-97.2) at 12months, 58.8% (95% CI 46.2-74.9) at 24, 35.4% (95% CI 22.3-56.1) at 36 and 17.2% (95% CI 7.0-41.7) at 48months. Performance status (P0.001), number of nodules (P0.016) and prior therapy (P=0.017) were the only variables strongly linked to survival by time-fixed Cox model. Performance status (P0.001), prior therapy (P=0.005), number of treatments (P=0.013), complete response after TACE (P=0.005) and bilirubin level (P0.001) were associated with survival using a time-dependent Cox model.Survival after TACE is influenced most by performance status, complete response and bilirubin. Compared with the time-fixed models, a time-dependent Cox model has the potential to estimate a more precise prognosis in HCC patients treated with TACE.

Abstract: The KLOE detector, mainly designed to measure the ℛ(ε′/ε) parameter of CP violation in K L decay to an absolute precision of ≈ 10 -4, is completed and will start data taking by spring 1999 at the e + e - φ-factory DAΦNE in Frascati. The primary K-meson physics and the experimental requirements at DAΦNE are summarized. The design strategies, the salient features and the main physics performances of all components of the experimental apparatus are reviewed.

Abstract: Hepatocellular carcinoma (HCC) is a challenging malignancy of global importance, it is associated with a high rate of mortality and its prevalence in the United States and in Western Europe is increasing. Cirrhosis is the strongest and the most common known risk factor for hepatocellular carcinoma, particularly cirrhosis related to hepatitis C virus (HCV) and hepatitis B virus (HBV) infections. The stage of cancer dictates the therapeutic choice, making early detection a primary objective. Early diagnosis of hepatocellular carcinoma is feasible because HCC develops in the background of well-known, readily identifiable and potentially avoidable environmental risk factors. Many observational studies have reported that HCC is diagnosed at an earlier stage in patients who received surveillance. Current guidelines advocate the use of abdominal ultrasound (US) at 6-12 months frequency to screen for HCC in high-risk patients. The use of AFP alone is strongly discouraged, and its use in addition to US is controversial. Patients with abnormal screening tests require additional investigation. Although the optimal methods of screening and the cost-effectiveness of surveillance for HCC remain to be established, systematic screening still offers the best hope for early diagnosis, treatment eligibility, and improved survival.

Abstract: A major problem in assessing the likelihood of survival of patients with hepatocellular carcinoma (HCC) arises from a lack of models capable of predicting outcome accurately.To compare the ability of the Italian score (CLIP), the French classification (GRETCH) and the Barcelona (BCLC) staging system in predicting survival in patients with HCC.We included 406 consecutive patients with cirrhosis and HCC. Seventy-eight per cent of patients had hepatitis C. Independent predictors of survival were identified using the Cox model.One-hundred and seventy-eight patients were treated, while 228 were untreated. The observed mortality was 60.1% in treated patients and 84.9% in untreated patients. Among treated patients, albumin, bilirubin and performance status were the only independent variables significantly associated with survival. Mortality was independently predicted by bilirubin, alpha-fetoprotein and portal vein thrombosis in untreated patients. CLIP achieved the best discriminative capacity in the entire HCC cohort and in the advanced untreatable cases, while BCLC was the ablest in predicting survival in treated patients.Overall predictive ability of BCLC, CLIP and GRETCH staging systems was not satisfactory, and was not uniform for treated patients and untreated patients. None of the scoring systems provided confident prediction of survival in individual patients.

Abstract: We have measured the ratio R-S(pi) = Gamma(K-S --> pi(+)pi(-)(gamma))/Gamma(K-S --> pi(0)pi(0)) with the KLOE detector at the DAPhiNE e(+)e(-) collider. This measurement is fully inclusive with respect to the pi(+)pi(-)gamma final state. The sample of over 106 two-pion decays of tagged K-S mesons allows a statistical error as low as similar to 0.1% to be obtained. The accuracy is limited by systematic uncertainties, which are estimated primarily from data. We find R-S(pi) = 2.236 +/- 0.003(stat) +/- 0.015(syst). (C) 2002 Elsevier Science B.V. Abstract: We have measured the radiative decays φ→ηγ, φ→η′γ selecting π+π−γγγ final state in a sample of ∼5×107 φ-mesons produced at the Frascati φ-factory DAΦNE. We obtain Γ(φ→η′γ)/Γ(φ→ηγ)=(4.70±0.47±0.31)×10−3. From this result we derive new accurate values for the branching ratio BR(φ→η′γ)=(6.10±0.61±0.43)×10−5 and the mixing angle of pseudoscalar mesons in the flavour basis ϕP=(41.8+1.9−1.6)°.

Abstract: The construction of the KLOE lead-scintillating fibres electromagnetic calorimeter has been completed. The final results of the extensive quality checks of all the calorimeter modules are presented here. The measurements have been done in the Frascati Laboratories at a cosmic ray test facility using minimum ionizing particles. The results show that all the calorimeter modules are well fulfilling the design specifications concerning time and energy resolutions. Furthermore, by using the same cosmic ray facility, all the calorimeter elements have been also pre-calibrated.

Abstract: We present a study of the reaction e+e- --> pi+pi-pi0 at the phi peak W=M(phi)=1019.4 MeV observed with the KLOE detector at Dafne. The reaction is dominated by phi production and decay, e+e- --> phi --> pi+pi-pi0. From a fit to the Dalitz plot density distribution we obtain the rho-meson parameters for its three charge states. We also find the relative amplitudes for phi --> rhopi and phi -> pi+pi-pi0 and the cross-section for e+e- --> omega pi0 with omega --> pi+pi-.

Abstract: The main goal of the KLOE experiment at the Frascati DAΦNE φ − factory is the study CP violation in kaon decays. The tracking device of the experiment is a drift chamber whose dimensions, 4 m of diameter and 3.3 m length, provide a large acceptance volume for the decay products of low momentum K L ( λ L =3.4 m ). A complete stereo geometry with 12.582 cells arranged in 58 layers guarantees a high and uniform efficiency in the reconstruction of the charged K L decays. Very light materials have been chosen both for the drift medium, a helium-based gas mixture, and for the mechanical structure, made of carbon fiber, to minimize multiple scattering and conversion of low-energy photons. The design requirements, the adopted solutions together with the calibration procedure and the tracking performances of the drift chamber are discussed.

Abstract: Test beam results of a full scale barrel prototype of the KLOE electromagnetic calorimeter satisfied the experimental requirements, so initiating the calorimeter mass production. At the Frascati National Laboratory of INFN a test stand was set up to use cosmic rays for quality control and first calibrations of the modules produced in mass construction. The results show that the fibers and photomultipllers performances exceed our expectations. Combining cosmic rays and test beam measurements a time resolution of 55 ps/√E(GeV) and an energy resolution of 4.7%/√E(GeV) are expected for electromagnetic showers. The end of modules assembling and calibration is foreseen for the early 1997.

Abstract: The design and the status of construction of the KLOE electromagnetic calorimeter are described in this report. 18 out of 24 barrel modules have been fully assembled and more than 50% of the end-cap, modules are built. All experimental specifications are fulfilled as shown by the test beam results of the final size prototype. Since the quality of fibers and photomultipliers have gone through improvements, the final calorimeter performances will exceed our expectations. The main parameters of each calorimeter module (light yield, attenuation length and time resolution) are fully surveyed using cosmic rays. Extrapolating the results to electromagnetic showers, a time resolution of 55 ps/root E(GeV) and a resolution of 0.9 cm/root E(GeV) an the coordinate along the fibers are obtained. An energy resolution of 4.7%/root E(GeV) can also be quoted.

Abstract: The Frascati e +e - collider, DAΨNE, operates at a center of mass energy of ∼1020MeV, the mass of the φ-meson. Three detectors have been realized, KLOE, DEAR, and FINUDA. The first two are presently collecting data in the two DAΨNE interaction regions (IR), while the third is being assembled in its pit inside the DAΨNE hall and will take data in the IR occupied by DEAR. The KLOE experiment, which is designed to measure CP and CPT violation parameters in the K SK L system, has collected so far ∼300 pb -1. Measurements obtained using the year 2000 data sample will be presented, which are focussed on K S physics and φ radiative decays. The DEAR experiment aims at studying the scattering lengths of the kaon-nucleon system, through a precise measurement of the K α line shifts in kaonic hydrogen and kaonic deuterium. With 2001 and 2002 data-taking periods the DEAR collaboration has demonstrated the capability of creating and detecting exotic atoms using the kaon flux from DAΨNE, by measuring the X-ray spectrum of kaonic nitrogen. The present status and perspectives of the DEAR experiment will be discussed. A brief report will be given at the end on the FINUDA physics program, aiming at the study of the A-hypernuclei levels and lifetimes.

Abstract: We present a measurement of the branching ratio BR(K-S --> pi(+/-)e(-/+)v(v)) performed using the KLOE detector. K-S-mesons are produced in the reaction e(+)e(-) --> phi --> KSKL at the DAPhiNE collider. In a sample of similar to 5 x 106 K-S-tagged events we find 624+/-30 semileptonic K-S decays. Normalizing to the K-S --> pi(+)pi(-) count in the same data sample, we obtain BR(K-S --> piev) = (6.91+/-0.37) x 10(-4), in agreement with the Standard Model expectation.

Abstract: In a sample of 5.3 x 10(7), phi-decays observed with the KLOE detector at the Frascati phi-factory DAPhiDNE we find 605 etapi(o)gamma events with eta --> gammagamma and 197 etapi(o)gamma events with eta --> pi(+)pi(-)pi(o). The decay etapi(o)gamma is dominated by the process phi --> a(o)gamma. From a fit to the etapi(o) mass spectrum we find BR(phi --> a(o)(980)gamma) = (7.4 +/- 0.7) x 10(-5).

Abstract: The main goal of the KLOE experiment at the Frascati DAPhiNE phi -factory is the study CP violation in kaon decays. The tracking device of the experiment is a drift chamber whose dimensions, 4 m of diameter and 3.3 m length, provide a large acceptance volume for the decay products of low momentum K-L (lambda(L) = 3.4m). A complete stereo geometry with 12.582 cells arranged in 58 layers guarantees a high and uniform efficiency in the reconstruction of the charged K-L decays. Very light materials have been chosen both for the drift medium, a helium-based gas mixture, and for the mechanical structure, made of carbon fiber, to minimize multiple scattering and conversion of low-energy photons. The design requirements, the adopted solutions together with the calibration procedure and the tracking performances of the drift chamber are discussed. (C) 2002 Published by Elsevier Science B.V.
