Recent advances in sensor technology for air pollution monitoring open new possibilities in the field of environmental epidemiology. The low spatial resolution of fixed outdoor measurement stations and modelling uncertainties currently limit the understanding of personal exposure. In this context, air quality sensor systems (AQSSs) offer significant potential to enhance personal exposure assessment. A pilot study was conducted to investigate the feasibility of the NO2 sensor model B43F and the particulate matter (PM) sensor model OPC-R1, both from Alphasense (UK), for use in epidemiological studies. Seven patients with chronic obstructive pulmonary disease (COPD) or asthma had built-for-purpose sensor systems placed inside and outside of their homes at fixed locations for one month. Participants documented their indoor activities, presence in the house, window status, and symptom severity and performed a peak expiratory flow test. The potential inhaled doses of PM2.5 and NO2 were calculated using different data sources such as outdoor data from air quality monitoring stations, indoor data from AQSSs, and generic inhalation rates (IR) or activity-specific IR. Moreover, the relation between indoor and outdoor air quality obtained with AQSSs, an indoor source apportionment study, and an evaluation of the suitability of the AQSS data for studying the relationship between air quality and health were investigated. The results highlight the value of the sensor data and the importance of monitoring indoor air quality and activity patterns to avoid exposure misclassification. The use of AQSSs at fixed locations shows promise for larger-scale and/or long-term epidemiological studies.

Background:. Malposition of the femoral tunnel during medial patellofemoral ligament (MPFL) reconstruction may increase the risk of recurrence of patellar dislocation due to isometric changes during flexion and extension. Different methods have been described to identify the MPFL isometric point using fluoroscopy. However, femoral tunnel malposition was found to be the cause of 38.1% of revisions due to patellar redislocation. This high rate of malposition has raised the question of individual anatomical variability. Methods:. Magnetic resonance imaging (MRI) was performed on 80 native knees using the CLASS (MRI-generated Compressed Lateral and anteroposterior Anatomical Systematic Sequence) algorithm to identify the femoral MPFL insertion. The insertions were identified on the MRI views by 2 senior orthopaedic surgeons in order to assess the reliability and reproducibility of the method. The distribution of the MPFL insertion locations was then described in a 2-plane coordinate system and compared with MPFL insertion locations identified with other methods in previously published studies. Results:. The CLASS MPFL footprint was located 0.83 mm anterior to the posterior cortex (line 1) and 3.66 mm proximal to the Blumensaat line (line 2). Analysis demonstrated 0.90 and 0.89 reproducibility and 0.89 and 0.80 reliability of the CLASS method to identify the anatomical femoral MPFL insertion point. The distribution did not correlate with previously published data obtained with other methods. The definitions of the MPFL insertion point in the studies by Schöttle et al. and Fujino et al. most closely approximated the CLASS location in relation to the posterior femoral cortex, but there were significant differences between the CLASS method and all 4 previously published methods in relation to the proximal-distal location. When we averaged the distances from line 1 and line 2, the method that came closest to the CLASS method was that of Stephen et al., followed by the method of Schöttle et al. Conclusions:. The CLASS algorithm is a reliable and reproducible method to identify the MPFL femoral insertion from MRI views. Measurement using the CLASS algorithm shows substantial individual anatomical variation that may not be adequately captured with existing measurement methods. While further research must target translation of this method to clinical use, we believe that this method has the potential to create a safe template for sagittal fluoroscopic identification of the femoral tunnel during MPFL surgical reconstruction. The use of metal-on-metal bearing couples in total hip arthroplasty can lead to an increased release of metal ions, particularly cobalt and chromium over time. This can lead to local and systemic metallosis, which has cytotoxic, genotoxic, and immunotoxic effects and can cause a host of secondary disorders. We describe the case of a 37-year-old female patient that was diagnosed with warm-antibody autoimmune hemolytic anemia (WAIHA) one and a half years after bilateral large-diameter head metal-on-metal total hip arthroplasty. For 11 years, it was refractory to all therapy, including splenectomy and rituximab, requiring long-term oral prednisone for disease control. Ultimately, systemic metallosis and periprosthetic joint infection were diagnosed, requiring explantation of the prostheses. By the sixth week postoperatively, she experienced complete spontaneous remission of her WAIHA. In conclusion, WAIHA can be associated with systemic metallosis in patients with metal-on-metal prosthetic joint replacements. Both hematologists and orthopedic surgeons should be aware of this.
