Abstract

Neste artigo analisamos a inserção do agronegócio na educação, especificamente, a campanha De Olho no Material Escolar e seus atravessamentos com os procedimentos do Escola sem Partido. Para tanto, realizamos pesquisa bibliográfica e documental acerca das temáticas em questão. Como resultado, observamos indícios de que o De Olho no Material Escolar compõe uma frente de expansão político-ideológica do agronegócio que busca, por meio da combinação entre força e consenso, interferir nos rumos da educação brasileira, reproduzindo de forma mais sofisticada os métodos desenvolvidos pelo Escola sem Partido. 3. Identification of Cucumisin (Cuc m 1), a subtilisin‐like endopeptidase, as the major allergen of melon fruit

Abstract

Background: Allergenic components in melon extracts have not been described in spite of the fact that melon (Cucumis melo) is a frequent allergy-eliciting fruit. The aim of this study was to evaluate allergenic components in melon extract and to report the identification of cucumisin as a major melon allergen. Materials and methods: Sera from 35 patients allergic to melon were selected on the basis of clinical symptoms, skin prick tests and oral challenge test. Allergenic components were detected by sodium dodecyl sulphate polyacrylamide gel electrophoresis and immunoblotting. Molecular characterization of IgE-binding bands was performed by N-terminal amino acid sequencing. Results: More than 10 IgE-binding bands, between 10 and 80 kDa, were identified in melon extract. Out of them, four IgE-binding bands were major allergens: 14 kDa, 36 kDa, 54 kDa and 67 kDa. These major allergens, except 14 kDa band, showed the same N-terminal sequence: T-T-R-S-W-D-F-L. Research conducted with protein databases identified this N-terminal sequence as cucumisin, an alkaline serine protease, which shares structural homology with microbial subtilisin. The molecular mass of the identified bands corresponds with different molecular forms of cucumisin produced during the processing or degradation of the enzyme: 67 kDa native cucumisin, 54 kDa mature cucumisin and 36 kDa NH2-terminal cucumisin fragment. Conclusion: Cucumisin (Cuc m 1) and several N-terminal cucumisin fragments are the major allergens of melon. Abstract

Spectrophotometric distances in the K band have been reported by different authors for a number of obscured Galactic HII regions. Almost 50% of them show large discrepancies compared to the classical method using radial velocities measured in the radio spectral region. In order to provide a crucial test of both methods, we selected a target which does not present particular difficulty for any method and which has been measured by as many techniques as possible. The W3 star forming complex, located in the Perseus arm, offers a splendid opportunity for such a task. We used the NIFS spectrograph on the Frederick C. Gillett Gemini North telescope to classify candidate "naked photosphere" OB stars based on 2MASS photometry. Two of the targets are revealed to be mid O-type main sequence stars leading to a distance of d = 2.20 kpc. This is in excellent agreement with the spectrophotometric distance derived in the optical band (d = 2.18 kpc, Humphreys 1978) and with a measurement of the W3 trigonometric parallax (d = 1.95 kpc, Xu et al. 2006). Such results confirm that the spectrophotometric distances in the K band are reliable. The radio derived kinematic distance, on the contrary, gives a distance twice as large (d = 4.2 kpc, Russeil 2003). This indicates that this region of Perseus arm does not follow the Galactic rotation curve, and this may be the case also for other HII regions for which discrepancies have been found.

Abstract

We present a near infrared study of the stellar content of 35 H\,{\sc{ii}} regions in the Galactic plane. In this work, we have used the near infrared domain $J-$, $H-$ and $K_{s}-$ band color images to visually inspect the sample. Also, color-color and color-magnitude diagrams were used to indicate ionizing star candidates, as well as, the presence of young stellar objects such as classical TTauri Stars (CTTS) and massive young stellar objects (MYSOs). We have obtained {\it Spitzer} IRAC images for each region to help further characterize them. {\it Spitzer} and near infrared morphology to place each cluster in an evolutionary phase of development. {\it Spitzer} photometry was also used to classify the MYSOs. Comparison of the main sequence in color-magnitude diagrams to each observed cluster was used to infer whether or not the cluster kinematic distance is consistent with brightnesses of the stellar sources. We find qualitative agreement for a dozen of the regions, but about half the regions have near infrared photometry that suggests they may be closer than the kinematic distance. A significant fraction of these already have spectrophotometric parallaxes which support smaller distances. These discrepancies between kinematic and spectrophotometric distances are not due to the spectrophotometric methodologies, since independent non-kinematic measurements are in agreement with the spectrophotometric results. For instance, trigonometric parallaxes of star-forming regions were collected from the literature and show the same effect of smaller distances when compared to the kinematic results. In our sample of H\,{\sc{ii}} regions, most of the clusters are evident in the near infrared images. Abstract

We discuss the capability of AKARI in recovering diffuse far-infrared emission, and examine the achieved reliability. Critical issues in making images of diffuse emission are the transient response and long-term stability of the far-infrared detectors. Quantitative evaluation of these characteristics are the key to achieving sensitivity comparable to or better than that for point sources (< 20 -- 95 MJy sr-1). We present $K$-band spectra of the near infrared counterparts to IRS2E and IRS2W which is associated with the ultracompact HII region W51d, both of them embedded sources in the Galactic compact HII region W51 IRS2. The high spatial resolution observations were obtained with the laser guide star facility and Near infrared Integral Field Spectrograph (NIFS) mounted at the Gemini North observatory. The spectrum of the ionizing source of W51d shows the photospheric features NIII (21155 \AA) in emission and HeII (21897 \AA) in absorption which lead us to classify it as an young O3 type star. We detected CO overtone in emission at 23000 \AA in the spectrum of IRS2E, suggesting that it is a massive young object still surrounded by an accretion disc, probably transitioning from the hot core phase to an ultracompact HII region., Comment: Accepted by ApJ Letters

Abstract

We present K-band spectra of newly born OB stars in the obscured Galactic giant H II region W51A and ~ 0.8'' angular resolution images in the J, H and K_S-bands. Four objects have been spectroscopically classified as O-type stars. The mean spectroscopic parallax of the four stars gives a distance of 2.0 \pm 0.3 kpc (error in the mean), significantly smaller than the radio recombination line kinematic value of 5.5 kpc or the values derived from maser propermotion observations (6--8 kpc). The number of Lyman continuum photons from the contribution of all massive stars (NLyc ~ 1.5 x 10^{50} s^{-1}) is in good agreement with that inferred from radio recombination lines (NLyc = 1.3 x 10^{50} s^{-1}) after accounting for the smaller distance derived here. We present analysis of archival high angular resolution images (NAOS CONICA at VLT and T-ReCS at Gemini) of the compact region W51 IRS2. The K_S--band images resolve the infrared source IRS~2 indicating that it is a very young compact HII region. Sources IRS2E was resolved into compact cluster (within 660 AU of projected distance) of 3 objects, but one of them is just bright extended emission. W51d1 and W51d2 were identified with compact clusters of 3 objects (maybe 4 in the case of W51d1) each one. Although IRS~2E is the brightest source in the K-band and at 12.6 \micron, it is not clearly associated with a radio continuum source. Abstract

AKARI, the first Japanese satellite dedicated to infrared astronomy, was launched on 2006 February 21, and started observations in May of the same year. AKARI has a 68.5 cm cooled telescope, together with two focal-plane instruments, which survey the sky in six wavelength bands from the mid- to far-infrared. The instruments also have the capability for imaging and spectroscopy in the wavelength range 2 - 180 micron in the pointed observation mode, occasionally inserted into the continuous survey operation. The in-orbit cryogen lifetime is expected to be one and a half years. The All-Sky Survey will cover more than 90 percent of the whole sky with higher spatial resolution and wider wavelength coverage than that of the previous IRAS all-sky survey. Point source catalogues of the All-Sky Survey will be released to the astronomical community. The pointed observations will be used for deep surveys of selected sky areas and systematic observations of important astronomical targets. Abstract

In 2000, Lamb and Reichart predicted that gamma-ray bursts (GRBs) and their afterglows occur in sufficient numbers and at sufficient brightnesses at very high redshifts (z > 5) to eventually replace quasars as the preferred probe of element formation and reionization in the early universe and to be used to characterize the star-formation history of the early universe, perhaps back to when the first stars formed. Here we report the discovery of the afterglow of GRB 050904 and the identification of GRB 050904 as the first very high redshift GRB. We measure its redshift to be 6.39(+0.11,-0.12), which is consistent with the reported spectroscopic redshift (6.29 +/- 0.01). Furthermore, just redward of Ly-alpha the flux is suppressed by a factor of three on the first night, but returns to expected levels by the fourth night. We propose that this is due to absorption by molecular hydrogen that was excited to rovibrational states by the GRB's prompt emission, but was then overtaken by the jet. Now that very high redshift GRBs have been shown to exist, and at least in this case the afterglow was very bright, observing programs that are designed to capitalize on this science will likely drive a new era of study of the early universe, using GRBs as probes., Comment: Submitted to Nature, 11 pages, 3 figures, LaTeX

11. The Stellar Content of Obscured Galactic Giant HII Regions V: G333.1--0.4

Abstract

We present high angular resolution near--infrared images of the obscured Galactic Giant HII (GHII) region G333.1--0.4 in which we detect an OB star cluster. For G333.1--0.4, we find OB stars and other massive objects in very early evolutionary stages, possibly still accreting. We obtained $K$--band spectra of three stars; two show O type photospheric features, while the third has no photospheric features but does show CO 2.3 $\mu$m band--head emission. This object is at least as hot as an early B type star based on its intrinsic luminosity and is surrounded by a circumstellar disc/envelope which produces near infrared excess emission. A number of other relatively bright cluster members also display excess emission in the $K$--band, indicative of disks/envelopes around young massive stars. Based upon the O star photometry and spectroscopy, the distance to the cluster is 2.6 $\pm$ 0.4 kpc, similar to a recently derived kinematic (near side) value. The slope of the $K$--band luminosity function is similar to those found in other young clusters. The mass function slope is more uncertain, and we find $-1.3 \pm 0.2 < \Gamma < -1.1 \pm 0.2$- for stars with M $> 5$ M$_\odot$ where the upper an lower limits are calculated independently for different assumptions regarding the excess emission of the individual massive stars. The number of Lyman continuum photons derived from the contribution of all massive stars in the cluster is 0.2 $\times$ $10^{50}$ $s^{-1}$ $< NLyc < 1.9$ $\times$ $10^{50}$ $s^{-1}$. The integrated cluster mass is 1.0 $\times$ $10^{3}$ $M_\odot < M_{cluster} < 1.3$ $\times$ $10^{3}$ $M_\odot$., Comment: 31 pages, including 12 figures and 3 tables. 12. The Stellar Content of Obscured Galactic Giant H II Regions IV.: NGC3576

Abstract

We present deep, high angular resolution near-infrared images of the obscured Galactic Giant H II region NGC3576. Our images reach objects to ~3M_sun. We collected high signal-to-noise K-band spectra of eight of the brightest objects, some of which are affected by excess emission and some which follow a normal interstellar reddening law. None of them displayed photospheric features typical of massive OB type stars. This indicates that they are still enshrouded in their natal cocoons. The K-band brightest source (NGC3576 #48) shows CO 2.3 micron bandhead emission, and three others have the same CO feature in absorption. Three sources display spatially unresolved H_2 emission, suggesting dense shocked regions close to the stars. We conclude that the remarkable object NGC3576 #48 is an early-B/late-O star surrounded by a thick circumstellar disk. A number of other relatively bright cluster members also display excess emission in the K-band, indicative of reprocessing disks around massive stars (YSOs). Such emission appears common in other Galactic Giant H II regions we have surveyed. The IMF slope of the cluster, Gamma = -1.51, is consistent with Salpeter's distribution and similar to what has been observed in the Magellanic Cloud clusters and in the periphery of our Galaxy., Comment: 14 pages, 11 figures, accepted for publication in AJ

Abstract

Resumo Introdução O suicídio ocupa um lugar de destaque entre os agravos que atingem as populações devido ao grande impacto familiar, psicossocial e econômico. A atenção básica à saúde é o primeiro local onde são atendidos indivíduos que apresentam comportamento suicida, e as atitudes dos profissionais podem ser determinantes para a evolução dos casos. Objetivo Verificar as atitudes de profissionais da atenção básica diante do comportamento suicida. Método Estudo transversal, com aplicação do Questionário de Atitudes em Relação ao Comportamento Suicida aos profissionais da atenção básica do município de Barbacena-MG. Resultados A maioria da amostra foi representada por agentes de saúde, do sexo feminino, com idade entre 30-39 anos e com instrução média. Foram observadas mais atitudes positivas entre os médicos e os enfermeiros quando comparados aos outros profissionais. Não foram verificadas diferenças significativas das atitudes em relação ao sexo e à idade dos participantes. Conclusão Os técnicos de enfermagem e os agentes de saúde apresentaram atitudes mais negativas em relação aos demais. Esse é um dado que deve ser levado em conta nos programas de capacitação. O treinamento dos profissionais deve contemplar habilidades cognitivas e mudanças nas atitudes que possam favorecer a empatia e a compreensão do comportamento suicida, priorizando a capacitação dos agentes de saúde.

Abstract

O empreendedorismo feminino tem se tornado cada vez mais visível nas organizações e nos negócios em geral. As experiências bem sucedidas de empresas criadas por mulheres, bem como a sobrevivência do número significativo de novos empreendimentos geridos por estas, tem sido um fator de grande relevância para a realização de estudos e pesquisas. O objetivo desse estudo é analisar o perfil gerencial das mulheres empreendedoras na cidade Bacabal-MA, bem como verificar o comportamento estratégico, identificar sua capacidade criativa e verificar a formalização de seus empreendimentos. O presente artigo foi desenvolvido através de uma pesquisa descritiva junto a 50 empreendedoras em diferentes setores de atividades, no qual os dados foram levantados por meio da aplicação de questionários correspondendo a uma pesquisa quanti-qualitativa. Através da análise de dados constatou-se que as empreendedoras tem iniciado sua atividade em sua maioria pela vontade de ter o próprio negócio, uma vez que suas características mais marcantes foram capacidade de organização, relação interpessoal, praticidade, e a valorização da criatividade, posto pelas empreendedoras como diferencial competitivo, fazendo uso de uma gestão mais flexível e uma visão mais estratégica. Pode-se perceber ainda, que as empreendedoras pesquisadas são comprometidas e responsáveis com seus negócios, posto que, quase em sua totalidade já iniciaram sua atividade na formalidade, estão a mais de 10 anos no mercado e já tinham um conhecimento prévio de sua área de atuação. Os resultados permitiram concluir, que o perfil gerencial adotado por elas, tem contribuído para o sucesso e a sobrevivência significativa dos seus empreendimentos.

This article analyzes the insertion of agribusiness into education, specifically, the campaign De Olho no Material Escolar and its crossings with the procedures of Escola sem Partido movement. To this end, we conducted bibliographical and documentary research on the topics in question. 17. Alternativa restauradora por meio de endocrown: uma revisão da literatura

Abstract

Introdução: A coroa endocrown é uma restauração indireta confeccionada em cerâmica adesiva em monobloco e cimentada com técnicas adesivas. A coroa possui como retenção a câmara pulpar do dente tratado endodonticamente sem que se utilizem os condutos. Dentes tratados endodonticamente possuem redução da rigidez e resistência devido à perda da integridade estrutural. Este fato pode acarretar em longo prazo falhas biomecânicas. Objetivo: Realizar uma revisão de literatura do tipo narrativa acerca das principais características das endocrowns. Metodologia: A pesquisa foi realizada no período de 20 de Janeiro a 02 de Abril de 2019. Foram utilizados artigos científicos das seguintes bases de dados: SCIELO (Scientific Eletronic Library Online), BVS (Biblioteca Virtual em Saúde), e monografias que atendiam aos requisitos no período entre 2008 a 2019, com exceção de artigos clássicos, em língua nacional, português, bem como em inglês e espanhol. Foram utilizados como descritores isolados, ou combinados, para a busca: "Endocrown”, “Endocrown Prótese”, “Endocrown Prosthesis”, “Endocrown Prótesis”, com um resultado de 31 artigos utilizados. Conclusão: As endocrowns são indicadas para dentes posteriores tratados endodonticamente, confeccionada na maioria das vezes por dissilicato de lítio e apresenta estudos que comprovem que o risco de fratura é inferior às reabilitações sob uso de retentores intrarradiculares. Sob boa indicação e avaliação, é possível obter previsibilidade de sucesso, porém, ainda são necessários mais pesquisas para comprovar cada vez mais a eficácia dessa alternativa.Descritores: Prótese Dentária; Prótese Parcial Fixa; Odontologia.ReferênciasRocca GT, Rizcalla N, Krejci I. Trabalhamos em diálogo com ométodo deleuzeguatarriano de cartografia e produzimos umdeslocamento do conceito de literatura menor para a noção de históriamenor. A "história menor" é aqui entendida como um dispositivoinstituinte que nos ajuda a pensar as trajetórias escolares das pessoassurdas como micropolíticas de resistência às formas que majoritariamentevêm sendo inventadas e, em decorrência destas invenções, produzindoefeitos de exclusão escolar.

Abstract

La hipertensión arterial es un factor de riesgo para el desarrollo de enfermedades cardiovasculares, principales causas de mortalidad en todo el mundo. El objetivo de este trabajo fue determinar la frecuencia de hipertensión arterial y su asociación con otros factores de riesgo cardiovascular en pacientes de primera consulta en el consultorio de la Primera Cátedra de Clínica Médica del Hospital de Clínicas, FCM, UNA en 2012. Se realizó unestudio observacional, descriptivo, retrospectivo, con componente analítico en el que se estudió a pacientes entre 18 y 90 años, seleccionados por muestreo aleatorio simple. En el periodo de estudio se registraron 4.046 historias clínicas en primera consulta, y 3.854 fueron consideradas para la aleatorización. Se estudiaron las variables demográficas, antropométricas, bioquímicas, presencia de hipertensión arterial, diabetes mellitus tipo 2, consumo de alcohol y tabaco. Se analizaron las historias clínicas de 457 pacientes con una edad media de 44,13±17 años; 48,23±18 años en hombres y 46,61±11 años en mujeres,300 (66%) fueron mujeres. La frecuencia de la hipertensión arterial± IC95%fue de30,41±4,2%, sin diferencia significativa entre mujeres y hombres (32,33±4,3% versus 28±4,1%). La obesidad fue el factor de riesgo cardiovascular más frecuente. Alrededor del 60% de los pacientes presentaba uno o más factores de riesgo cardiovascular.La hipertensión arterial se asoció conla obesidad (OR:4,427; IC95%: 2,345-4,876; p=0,032), la diabetes (OR: 2,141; IC 95%: 1,261-2,347; p=0,005) y niveles de c-HDL

Abstract

Background: Suicide and suicide attempts occupy a prominent place among diseases affecting the population due to their great impact on family, psychosocial and economy. Health professionals' attitudes are important since their knowledge and view about suicide can significantly influence their behavior toward patients. Objective: To verify the attitudes of health care professionals towards the suicidal patients. Method: This is a cross-sectional study in which we applied the Suicide Behavior Attitude Questionnaire to health care professionals in the city of Barbacena, Minas Gerais. Results: The majority of the sample was represented by health agents, female, age between 30 and 39 years, with average level of education. Doctors and nurses had the most positive attitudes when compared to other professionals. There were no significant differences in attitudes regarding gender and age of participants. Conclusion: The results indicated that nursing technicians and health agents presented more negative attitudes when compared to doctors and nurses. This is a relevant fact and should be taken into account by the training programs. The training of professionals should contemplate cognitive skills and changes in attitudes that may favor the empathy and understanding of these behaviors. [ABSTRACT FROM AUTHOR]

Con el presente trabajo se llevo a cabo la obtención caracterización química y evaluación biológica de un extracto (BM21) de una planta marina con lo que se demostraron sus propiedades beneficiosas para el cuidado de la piel. The natural products, in special, the marine products, have in the actuality, a great use in the cosmetological industry and good acceptation for the people in general. In the present paper was exposure the system to obtain, the chemical characterization and the biological evaluation of the BM21 extract of the marine vegetable, it showed his properties to care of the skin as antioxidant and to prevent the photo aging. Abstract

In the absence of bioclimatic models, and given the scarcity of information about parameters and other input data, the panel data methodology was applied to establish the magnitude and direction of expected consequences in milk production, which is the main economic activity of Jimaguayú municipality. This study presents a summary about the theory of panel data models. Several specifications are evaluated, obtaining expressions of long-term elasticity of rainfall and temperature and its effect on milk production. Applied empirical models indicate that milk production in Jimaguayú could decrease by approximately 200 thousand L per year, due to the combined effect of the decrease in rainfall and the increase in temperature, with economic losses in the order of constant 100 thousand dollars informed in 2014. Abstract

Background: Cystic echinococcosis (CE) is a chronic disease considered a neglected one. Cystic echinococcosis is endemic in Uruguay and the region. Surgery, using various technical approaches, has the potential to safely remove the cyst(s) and lead to a complete cure in a high number of patients with simple forms of CE. However, surgery may be impractical in patients with multiple cysts in several organs, high surgical risk, or in patients with previous multiple surgeries. In these cases, the pharmacological treatment with the benzimidazolic drug Albendazole (ABZ) alone or combined with Praziquantel (PZQ), has been promising as the best choice to achieve improvement or cure., Methods: In this study, we analyze the results obtained on the anti-parasitic treatment of 43 patients diagnosed with CE between the years 2003 and 2020. Patients were treated before and/or after surgery with ABZ or the combination ABZ/PZQ. The standardize protocol of the anti-parasitic drug treatment before surgery was 7 days, 15 days or 1 month depending on the urgency and availability of the surgical procedure. All cases that involved confirmed locations on lungs underwent immediate surgery with minimal pre-treatment when possible. After surgery, the standardize protocol of anti-parasitic drug treatment consisted of six cycles of 30 days each and resting intervals of 15 days in between. ABZ was used in all cases, administered orally, twice daily, at a total dosage of 15 mg/kg/day, with food high in fat content for improved absorption. The follow up was carried out according to WHO-IWGE guidelines for 5 years., Results: Of the 43 patients fourteen were ≤ 15 years of age and had a differentiated pre-surgical treatment. From the ≥ 16 years of age, 36 completed the treatments and the 5 years follow up. Four patients changed geographical locations, without a forwarding contact, after the post-surgery treatment. No patient died during the study. Of the 36 patients that completed the study, 32 were treated only with ABZ; 93.75% achieved treatment success as determined by improvement or cure, and 6.25% treatment failure determined by no change or worsening. The last four patients received the ABZ/PZQ combination therapy and achieved 100% treatment success., Conclusion: The pharmacological treatment resulted in a good option not only as palliative but also as potentially curative. The main relevance of its use was in cases with previous multiple surgeries or surgeries with potential life-threatening complications due to the number and location of cysts and concurrent comorbidities. 38. First genetic characterization of human cystic echinococcosis in Uruguay.

Abstract

Cystic echinococcosis is endemic and hyperendemic in Uruguay. The objective of this study was to determine the species and genotype of Echinococcus granulosus sensu lato in symptomatic patients with cystic echinococcosis who underwent surgery, together with the location and stage of the cysts. The study included 13 patients aged between 6 and 57 years old. Samples of cysts from these cases were analyzed using DNA extraction, polymerase chain reaction amplification and sequencing. The results revealed the presence of E. granulosus sensu stricto in all cases, with 12 samples belonging to the G1 genotype and 1 to the G3 genotype, suggesting that disease persistence might be related to the dog-sheep cycle. However, other intermediate hosts, such as cattle, could also be involved. Cysts were most frequently found in the liver, followed by muscle and other sites (e.g. pulmonary, vertebral, pelvic and cardiac); and stage CE1 was most frequently found, followed by CE2 and CE3b. Three cases occurred in children or adolescents, suggesting an active parasite cycle in at least some areas of the country. Since there is considerable diversity of E. granulosus sensu lato species and genotypes in South America, it is important to continue the present study in order to draw stronger epidemiological conclusions.

39. Postoperative Bowel Perforation due to Heterotopic Ossification (Myositis Ossificans Traumatica): A Case Report and Review of the Literature.

Abstract

Heterotopic ossification (HO) is the ectopic development of normal bone within soft tissue that can occur after traumatic injury. It is uncommon and may be missed or misdiagnosed, which can lead to complications. We report the case of an 84-year-old male with a previous history of a laparotomy who underwent resection of an intra-abdominal tumor through a midline incision. On postoperative day six, the patient was taken to the operating room, as succus was draining from the incision. Upon re-exploration, sharp bone-like material was found in the wound directly adjacent to an enterotomy. Pathology confirmed mature lamellar bone and the diagnosis of HO. This is the first report of postoperative intestinal perforation secondary to HO in a midline wound. We report this case to encourage accurate reporting of HO and its morbidity and complications for the benefit of appropriate surgical planning and epidemiologic tracking of outcomes.

Abstract

In this multicenter, randomized study, we compared ease of insertion, postinsertion hemodynamic repercussion, quality of ventilation, and the capacity to achieve a "hands-free" anesthesia delivery between two new devices: the ProSeal laryngeal mask airway (PLMA) and the Laryngeal Tube (LT). The incidence of postoperative laryngopharyngeal discomfort was examined after short surgical interventions in spontaneously breathing patients. After induction with fentanyl and propofol, the respective airways were inserted into 70 adult ASA physical status I and II patients (35 patients in each group). First-attempt insertion success rates were more frequent for the PLMA (77% versus 51%; P < 0.05), but success rates were similar (100% versus 97%) after 3 attempts. The anesthesiologists considered that insertion of the PLMA was easier (P < 0.001). Expired tidal volume was larger with the PLMA (404.9 versus 328.4 mL; P < 0.005) and the ability to achieve hands-free ventilation was more frequent with the PLMA (32 versus 21 cases; P < 0.004). Positional maneuvers with the LT to correct ventilation deficiencies were not always completely effective (5 of 13). There were no differences in the incidence of intolerance, sore throat, dysphagia, and/or dysphonia between the two devices. We conclude that the PLMA showed greater ease of insertion and reliability than the LT for use in nonparalyzed anesthetized patients.
